Abstract
Description
Изобретение относится к способам размещения не требующего фиксации к тканям имплантата при аллогерниопластике и может быть использовано в хирургии грыж.The invention relates to methods of placement requiring no fixation to implant tissues in allogeneernoplasty and can be used in hernia surgery.
Наиболее близким к рассматриваемому способу является, по мнению авторов, способ подапоневротического размещения имплантата при аллогерниопластике, который осуществляется в результате ручного помещения, разглаживания и постоянного удержания имплантата инструментами, швами или фиксаторами в подапоневротическом пространстве [1, 2]. Однако в отличие от заявленного способа данный - имеет другую оперативную технику, не обеспечивает отсутствие складчатости, смещения и надежного плоскостного расположения имплантата на всех этапах выполнения аллогерниопластики и вызывает ряд специфических осложнений.The closest to the method under consideration is, according to the authors, the method of subaponeurotic placement of the implant during allogeneioplasty, which is carried out as a result of manual placement, smoothing and constant holding of the implant with instruments, sutures or fixatives in the subaponeurotic space [1, 2]. Отличительные признакиFeatures
При выполнении аллогерниопластики не требующий фиксации имплантат прошивается по четырем углам и по середине, через противоположные края, с выведением без завязывания длинных лигатур (Фигура №1-2). Имплантат ориентируется крючками из полимолочной кислоты к апоневрозу. Угловые лигатуры последовательно, вначале лигатуры верхнего края, а затем лигатуры нижнего края имплантата, выводятся на кожу брюшной стенки путем выкола из соответствующего угла подапоневротического пространства (Фигура №3-6). Осуществляется одновременная тракция за верхние и нижние угловые лигатуры. Имплантат одномоментно и целиком размещается в подапоневротическом пространстве по всей площади (Фигура №7-8). Краевые лигатуры выводятся на кожу брюшной стенки путем выкола из соответствующего края подапоневротического пространства (Фигура №9). На выведенные лигатуры накладывается постоянная, пассивная тракция для стабильного плоскостного расположения имплантата при ушивание апоневроза (Фигура №10-11). Осуществляется ушивание апоневроза «край-в-край» и мягких тканей брюшной стенки (Фигура №12-14). Permanent, passive traction is imposed on the removed ligatures for a stable planar placement of the implant during suturing of the aponeurosis (Figure 10-11). Suturing of the edge-to-edge aponeurosis and soft tissues of the abdominal wall is carried out (Figure No. 12-14). The ligatures brought to the abdominal wall are removed (Figure No. 15).
