The length of the incision may vary and depends on the degree and extent of the lesion of the GLP, all branches and lacunae of the GLP are opened, it is also freed from stones, calc-necrotic masses. После мобилизации ПЖ производят ее пересечение на уровне перешейка. Для этого участок нижнего края ПЖ над воротной веной дважды прошивают капроновой мононитью сечением 0,2 мм с расстоянием между швами 1 см и шагом 0,5 см для временного гемостаза нижних панкреатических сосудов. Лигатуры берут на зажимы, разводят в противоположные стороны и натягивают, приподнимая и тем самым отделяя ПЖ от верхней брыжеечной вены (ВБВ). Пересекают скальпелем ткань ПЖ между затянутых лигатур, защищая вену от случайного ранения тупфером. Дальнейшим подтягиванием кверху и разведением созданных держалок обнажают освобожденный участок передней поверхности ВБВ. Далее продолжают туннелизацию маленьким рабочим тупфером между задней поверхностью перешейка ПЖ и передней поверхностью верхней брыжеечной и воротной вен вдоль их оси. Освобожденные участки ПЖ порционно прошивают описанным выше способом, разводят в разные стороны, подтягивают кверху при помощи держалок и пересекают. Таким образом, поэтапно тупо и остро производят выделение из спаек и сращений ВБВ. Далее отделяют заднюю поверхность ПЖ от селезеночной и воротной вен, также освобождая последние из рубцовых сращений. Затем ПЖ пересекают по перешейку и разделяют на две части (головка и хвост). Сдвигая вену тупфером с головки медиально, обнажают крючковидный отросток и подтягивают его вверх и латерально. В головке производят ревизию ГПП с помощью пуговчатого зонда соответствующего диаметра. Для сохранения кровоснабжения производят рассечение паренхимы железы со стенкой протока по нижнему краю головки ПЖ в бессосудистой зоне между передней и задней нижними панкреатодуоденальными сосудами. При этом выполняют тщательный гемостаз обкалывающими швами вглубь кровоточащего сосуда (капроновая мононить 0,16-0,18 мм).After mobilization of the pancreas, it intersects at the level of the isthmus. At the same time, thorough hemostasis is performed with punctured seams deep into the bleeding vessel (nylon monofilament 0.16-0.18 mm).

Больной Е., 42 лет, история болезни №163, поступил в клинику РЦФХГ с жалобами на боли в эпигастрии, правом мезогастрии иррадиирующие в поясницу, поносы до 5-6 раз в сутки. После проведенного комплекса диагностических мероприятий (УЗИ, эндоскопическое УЗИ, ретроградная панкреатохолангиография, компьютерная томография) установлен клинический диагноз: «Хронический осложненный панкреатит, непрерывно рецидивирующее течение с умеренно выраженной внешнесекреторной недостаточностью. Вирсунголитиаз. Блок главного панкреатического протока». После соответствующей предоперационной подготовки больному выполнено оперативное вмешательство в объеме: «Медиальная резекция поджелудочной железы с прямой вирсунголитотомией и формированием билатерального концевопетлевого панкреатоэнтероанастамоза».Patient E., 42 years old, medical history No. 163, was admitted to the RCSCH clinic with complaints of epigastric pain, right mesogastric pain radiating to the lower back, and diarrhea up to 5-6 times a day. В головке ПЖ выполнили ревизию ГПП с помощью пуговчатого зонда соответствующего диаметра. Для сохранения кровоснабжения произвели рассечение паренхимы железы со стенкой протока по нижнему краю головки ПЖ в бессосудистой зоне между передней и задней нижними панкреатодуоденальными сосудами, выполнили тщательный гемостаз прецизионными обкалывающими швами капроновой мононитью диаметром 0,18-0,16 мм. В средней трети протока головки ПЖ определяется конкремент диаметром 0,9-1,0 см, произвели экстракцию конкремента, санацию ГПП, зонд беспрепятственно проникает в двенадцатиперстную кишку. Для восстановления целостности стенки протока головки ПЖ наложили первичный шов «на дренаже» (двухрядные одиночные узловые швы, проходящие через всю толщу паренхимы (рассасывающаяся мононить - PDS 5.0)). Перед ушиванием протока в него уложили хлорвиниловую трубку с боковыми отверстиями соответствующего диаметра для отведения сока из зоны соустья.In the head of the pancreas, the GLP was revised using a button probe of the appropriate diameter. Послеоперационный период протекал без осложнений. Больной выписан на 10-е сутки. На контрольном осмотре через 2 месяца больной жалоб не предъявляет, поправился на 3,5 кг.
