Abstract
Description
The invention relates to the field of surgery, traumatology and orthopedics, in particular to methods for surgical treatment of intraarticular comminuted fractures of metaepiphyses of tubular bones.
There is a method of fixing fragments and fragments during intraarticular fractures with tightening screws alone or in combination with a protective plate (Mueller M., Algover M., Schneider R., Willinger X. Guide to internal osteosynthesis. M .: 1996, - p.194), consisting in maintaining the articular surface under interfragmental compression after reposition of the fracture.
The disadvantage of this method is that fragments consisting of a large articular cartilage surface and a small subchondral bone mass are held indirectly by compression with a tightening screw, since a large degree of compression leads to deformation or displacement of the fragment. In addition, in the case of a large number of fragments, their retention and fixation in the correct position becomes impossible. This is especially true for fractures with compression of fragments, when, after their reposition, a defect in the bone structure is formed.
The closest in technical essence is the method of fixation using knitting needles (Revenko T.A., Guryev V.N., Shesternya N.A. Surgery for injuries of the musculoskeletal system. M: Medicine, 1987, p. 76), which fix the repaired fragment by introducing them through the cortical layer of bone tissue.
The disadvantage of the method chosen as a prototype is that in the case of a multi-fragmented nature of the fracture, small fragments, but having significant articular cartilaginous surfaces (which cannot be removed without causing significant damage to the joint), cannot be compared, retained and simultaneously fixed in the correct position. In addition, a needle inserted through the cortical layer and, accordingly, having a way out of the bone, undergoes migration during treatment, which leads to a secondary displacement of the fragments.
The objective of our invention is to ensure accurate reposition and fixation of fragments during the intra-articular compression multi-fragmented nature of the fracture, eliminating the migration of retainers and secondary displacement of fragments.
The problem is achieved in that in the method for the treatment of comminuted compression intraarticular fractures of metaepiphysis of tubular bones, including reposition of fragments and fixation with needles, the fragments are repaired and fixed with knitting needles, each subsequent fragment is placed on the protruding end of the knitting needle, and the knitting needles do not extend beyond the cuticle after that, the entire fracture is fixed with a bone plate with screws.
New in the proposed method is that the fragments sequentially after reposition are installed on the spokes without going beyond the cortical layer, which prevents their migration and the secondary displacement of the fragments.
According to the scientific, technical and patent literature in medicine, alternate reposition and fixation of fragments is not known due to intraosseous knitting needles that do not extend beyond the cortical layer.
The implementation of the method is illustrated in figures 1, 2, 3. Under anesthesia, the area of the intraarticular comminuted comminuted fracture of the metaepiphyseal bone is exposed. The fragments (1) are sequentially repaired and fixed with the needles (2) that extend 2 mm beyond each fragment. The fragments are alternately mounted (“mounted”) on the spoke. The spokes do not extend beyond the cortical layer of the bone (3). After this, the entire fracture is fixed, for example, with a osseous plate (4) with screws.
Thus, this method allows technically simple to perform osteosynthesis in severe intraarticular comminuted fractures of metaepiphyses of tubular bones. It allows you to effectively carry out the fixation in the multi-fragmented nature of the fracture. Eliminates the migration of spokes and the secondary displacement of fragments.
