Общепризнанно, что основная цель комплексного консервативного лечения начальных стадий идиопатического сколиоза - создание мышечного "корсета", способствующего активному удержанию позвоночника в вертикальном положении. Одним из средств, решающих эту задачу, является электростимуляция мышц туловища, с помощью которой возможно избирательно укрепить те или иные мышечные группы. В настоящее время для коррекции сколиотической деформации позвоночника широко используется методика электростимуляционной тренировки мышечного аппарата, предложенная Я.М. Коцем с соавт. (И.И. Кон, Э.М. Демина, Я.М. Коц. Применение метода электростимуляции мышц у детей, больных диспластическим сколиозом. - Актуальные вопросы травматологии и ортопедии. 1978, Вып.13, М., с. 102-107. ). Методика заключается в пассивной электростимуляции мышц - разгибателей спины посредством аппарата "Стимул-1" при наложении электродов на проекции данных мышц на вершине основной сколиотической дуги со стороны ее выпуклости. Этот способ электростимуляции принят нами за прототип. Однако ему присущи следующие недостатки:
- не требует активного участия больного, вследствие чего протекает в условиях, весьма далеких от реального функционирования мышц;
- воздействие преимущественно на фазические, а не на постуральные мышцы (так как в статике и динамике двигательных актов участвуют разные мышечные группы);
- коррекция бокового компонента сколиотической деформации без влияния на патологическую ротацию позвонков (это связано с тем, что электроды устанавливаются на значительном расстоянии от позвоночного столба до 8 см);
- короткое время проведения процедуры (до 10 минут) в режиме посылка-пауза 10:50 с, что является недостаточной (щадящей) тренировочной нагрузкой для мышц - разгибателей спины.This method of electrical stimulation is accepted by us as a prototype. However, it has the following disadvantages:

- correction of the lateral component of scoliotic deformation without affecting the pathological rotation of the vertebrae (this is due to the fact that the electrodes are installed at a considerable distance from the spinal column to 8 cm);
- short time of the procedure (up to 10 minutes) in the package-pause mode 10:50 s, which is insufficient (sparing) training load for muscles - extensors of the back.
Таким образом, методика электрической стимуляции мышц по Я.М.Коцу с соавт. оказывает влияние только на часть патобиомеханических элементов идиопатического сколиоза, тем самым затягивает сроки лечения и не достаточно влияет на предотвращение дальнейшего прогрессирования заболевания. С целью избежать вышеперечисленные недостатки для коррекции сколиотической деформации позвоночника нами предложен новый способ электростимуляции - активная динамическая электростимуляция поперечено-остистых мышц с асимметричным расположением электродов. Данный способ основывается на следующих клинико-функциональных данных (А.И.Казьмин, И.И.Кон, В.Е.Беленький. Сколиоз. - М.: Медицина. 1981, С.113-116):
- важное место в лечении сколиотической болезни занимают активные динамические упражнения лечебной физкультуры асимметричного характера;
- основную роль в удержании тела на уровне искривления играют поперечно-остистые мышцы, выполняющие особую асимметричную антигравитарную функцию (в верхней и средней частях дуги искривления напряжены мышцы выпуклой стороны, а в нижней части - мышцы вогнутой стороны);
- при электростимуляции импульсными токами максимальная волна возбуждения возникает не только в поверхностных, но и в глубоко расположенных мышцах спины, к которым относятся многораздельные мышцы, поперечно-остистые мышцы и другие (Waters R.L., Morris I. Anat, 1972, 111, 2, pp.191-199).In order to avoid the above disadvantages for the correction of scoliotic spinal deformity, we proposed a new method of electrical stimulation - active dynamic electrical stimulation of transverse-spinous muscles with an asymmetric arrangement of electrodes. - an important place in the treatment of scoliotic disease is occupied by active dynamic exercises of therapeutic exercise of an asymmetric nature;
- the main role in maintaining the body at the level of curvature is played by the transverse-spinous muscles, which perform a special asymmetric antigravity function (the muscles of the convex side are strained in the upper and middle parts of the curvature arc, and the muscles of the concave side are stressed in the lower part);
- during electrical stimulation by pulsed currents, the maximum excitation wave occurs not only in the superficial, but also in the deep muscles of the back, which include the multifidium muscles, transversely-spinous muscles and others (Waters RL, Morris I. Electrical activity of the trunk during walking. - J. Anat, 1972, 111, 2, pp. 191-199).
Целью изобретения является улучшение результатов коррекции сколиотической деформации позвоночника путем асимметричного воздействия на поперечно-остистые мышцы при сочетании электрического импульса с активным мышечным напряжением. Цель достигается тем, что благодаря асимметричному расположению электродов на проекции поперечно-остистых мышц сколиотической дуги (в верхней и средней частях дуги искривления с выпуклой стороны, а в нижней части - с вогнутой стороны) при одновременном выполнении активных динамических упражнений асимметричного характера с процедурой электровоздействия происходит коррекция как бокового, так и торсионного компонентов сколиотической деформации позвоночника. The goal is achieved by the fact that due to the asymmetric arrangement of the electrodes on the projection of the transverse-spinous muscles of the scoliotic arc (in the upper and middle parts of the curvature arc from the convex side, and in the lower part from the concave side), while performing active dynamic exercises of an asymmetric nature with the electric exposure procedure, correction of both lateral and torsion components of scoliotic spinal deformity.
Асимметричное воздействие на постуральные мышцы в сочетании с активными динамическими упражнениями выгодно отличает данный способ электростимуляции от прототипа. Способ осуществляют следующим образом. The method is as follows.
Исходное положение пациента - лежа на животе, под таз положить валик (диаметром 20-30 см), на стопы положить мешки с песком (по 5 кг). В положении рук возможны вариации: руки на поясе, руки в замке за головой, руки в замке тянутся вперед и вверх, в руках гимнастическая палка, гантели или закрепленные манжеты (от 0,3 до 3 кг). После первых 2-3-х процедур конечности располагаются в асимметричном корригирующем положении (пример: при правостороннем грудном сколиозе левая нижняя конечность отведена в сторону на угол 30-40o, правая - вдоль туловища; левая верхняя конечность отведена в сторону на угол 90-100o, правая - на поясе). При прохождении электрического тока и напряжении мышц пациент втягивает в себя живот, соединяет лопатки и слегка приподнимает туловище над полом (не прогибаясь в пояснице), удерживая его до окончания стимуляции.The initial position of the patient is lying on his stomach, put a roller (20-30 cm in diameter) under the pelvis, put sandbags (5 kg each) on the feet. With the passage of electric current and muscle tension, the patient draws in a stomach, connects the shoulder blades and slightly raises the body above the floor (without bending in the lower back), holding it until the end of stimulation.

Для электростимуляции используем накожные эластичные электроды на основе углеродных волокнистых материалов прямоугольной формы, размером 3х6 см (ГОСТ Р 15.013-94). При электростимуляции поперечно-остистых мышц на основной дуге сколиотического искривления используем 4 электрода для двухполюсной методики одновременно на два поля. Первая пара устанавливается в верхней части искривления на стороне выпуклости деформации на расстоянии 1 см от задней срединной линии вдоль остистых отростков, активный электрод (+) вверху, индифферентный (-) внизу, с зазором между электродами 2-3 см. Вторая пара располагалась аналогично, но в нижней части дуги с вогнутой стороны искривления (фиг. 1). Для воздействия на поперечно-остистые мышцы мы применяем методику электростимуляции на аппарате "Стимул-1", являющегося источником переменного синусоидального тока с частотой 1-2,5 кГц, создающего амплитудную модуляцию импульсами частотой 50 Гц с глубиной модуляции 100%, используемого в импульсном режиме "посылка-пауза" - 5-10 с. Методика проведения электростимуляции. Процедура активной динамической электростимуляции поперечно-остистых мышц заключается в том, что при подаче стимула на два асимметрично расположенных поля пациент дополнительно напрягает мышцы спины и приподнимает туловище до горизонтального уровня. После 5 мин стимуляции в таком режиме на руках у пациента закрепляли груз (манжеты, гантели) и продолжали стимуляцию, подбирая интенсивность тока, переносимого больным и достаточного для выпрямления туловища. Процедуру заканчивали при появлении признаков утомления мышцы. В течение всего времени электростимуляции исполнитель процедуры контролировал правильное положение туловища пациента. Продолжительность стимуляции составляла по 20-30 мин 2 раза в день. В последующие процедуры груз закрепляли на руках с самого начала. С 3-5 процедуры его величину увеличивали. Курс электростимуляции составлял 15-30 процедур. The technique of electrical stimulation. In the following procedures, the load was secured in the hands from the very beginning. From 3-5 procedures, its value was increased. The course of electrical stimulation was 15-30 procedures.
В клинике проведено по одному курсу электростимуляции у 23 детей с идиопатическим сколиозом 1-2 степеней в возрасте 10-16 лет. Результаты лечения оценивали по данным рентгенологического обследования до и после курса электростимуляции, проводимого в положении больного стоя и лежа в прямой проекции с определением углов деформации по методу Cobb, величины ротации позвонков по методу Perdriolle и компенсации искривления по методу А.И.Казьмина. У всех больных была получена достоверная положительная динамика: коррекция общего угла искривления основной дуги исчислялась в среднем 5o, вторичной дуги - 8o, уменьшение угла ротации позвонков 4,3o, индекс стабильности возрастал с 0,68 до 0,79.The clinic conducted one course of electrical stimulation in 23 children with idiopathic scoliosis of 1-2 degrees at the age of 10-16 years.
