Abstract
Description
The invention relates to medicine, namely to surgical gastroenterology.
Gastrectomy is one of the main methods of surgical treatment of patients with gastric cancer.
The most difficult moment of gastrectomy is the imposition of an esophageal-intestinal anastomosis. Anastomotic suture failure is the most frequent and severe complication of gastrectomy, which in most cases leads to death. The reasons for the failure of the seams are diverse. One of the main reasons is the absence of the serous membrane of the esophagus during the formation of the anastomosis, and therefore all surgeons strive to cover the suture line and deserted section of the esophagus with a loop of the intestine to prevent insolvency, which also does not guarantee high reliability of the sutures.
The purpose of the invention is the prevention of insolvency of the sutures of the esophageal-intestinal anastomosis during gastrectomy performed for cancer of the stomach without switching to the esophagus.
The purpose of the invention is achieved by preserving the serous membrane of the abdominal part of the esophagus and the formation of the esophageal-intestinal anastomosis according to a precision technique, using monofilament filaments of small sizes.
The method is as follows. Produce a laparotomy, a revision of the stomach to determine the prevalence of the process. In the future, gastrectomy is performed with removal of the small and large omentum, lymphadenectomy, the duodenal stump is sutured. The stomach is cut off from the esophagus without its mobilization. The latter after that, as a rule, is shortened and represents a short segment (up to 1.5 cm).
When applying the esophageal-intestinal anastomosis, precision technique and small-sized monofilament filaments were used. In this case, the sutures were made through the wall of the esophagus and intestines in the submucosal layer, with the nodes located outside. Additional peritonization of the joints was not performed.
According to this technique, 3 patients were operated on. There were no complications and mortality when applying this method.
The high reliability of the newly formed esophageal-intestinal anastomosis is ensured, in our opinion, by the preservation of the serous membrane of the esophagus and precision surgical technique, while using small reactive sutures.
