Description
Foreliggende oppfinnelse vedrører nye forbindelser, blandinger og fremgangsmåter for anvnedelse av forbindelsene som adrenerge midler og ved behandlingen av nasal kongestion.
Nasal kongestion er karakteristisk for flere sykdommer som omfatter vanlig forkjølelse, allergisk rhinities, sinusitis og høysnue. Nasal kongestion forårsakes av utvidelse av blodkarene i den nasale slimhinne, som fører til svelling av vevene som omgir nesehulen. Blodkarene i den nasale slimhinne er rik på adrenergne reseptorer. Blandt de vanlig anvendte atopisk nasale dekongestanter som virker ved adre-nerg stimuluasjon er imidazolinderivatene naphzoline, oxymetazoline, xylometazoline og tetrahydrozoline. Disse adrenergene stimulanter innsnevrer blodkarene i den nasale slimhinne og letter slik oppsvulmingen av de nasale vev.
En rekke sideeffekter er forbundet med bruken av disse midler. For eksempel forårsaker ofte oxymetazoline og napha-zoline irritasjon av den nasale slimhinne. Utbredt eller forlenget bruk av topiske nasale dekongestanter kan forårsake tilbakekommende dekongestion eller rhinitis medica-mentosa. Tilbakekommende kongestion eller rhinitis medica-mentosa er en tilstand hvorved kongestionen er redusert i begynnelsen, men som deretter følges av en tilbakekomst av en sterkere tetthet i nesen som ikke lettes når ytterigere doser av dekongestant tilføres. Videre kan bieffektene omfatte søvnløshet, hodepine, kvalme, irritabilitet, svimmel het, svettetokter, hypertensjon, tachycardia, hjertebank og hjertearytmi, hvis de topiske dekongestanter absorberes sysetmatisk.
Derfor er detønskelig med en forbindelse med god topisk dekongestant aktivitet og redusert potensial for nasal irritasjon og andre bi-effekter.
Forbindelser med foremel;
hvori og R- som kan være like eller forskjellige er Cl, Br, I eller F og deres farmasøytiske akseptable salter er alpha-1 adrenergisk reseptor stimulanter og alpha-2 adrenergisk reseptor innhibitorer og er også nasale dekongestanter. Disse forbindelser utviser en kombinasjon av en alpha-1 adrenergisk reseptor mot alpha-2 adrenergisk reseptor antagonist aktivitet, og har ingen alpha-2 adrenergisk reseptor virkning. Når administrert intranasalt som en spray eller en topisk oppløsning, observeres lettelse av den nasale kongestion, mens uønskede bi-effekter reduseres eller elimineres.
Farmasøytiske akseptable salter av forbindelsene i henhold til denne oppfinnelsen omfatter, men er ikke begrenset til, ikke toksiske syreaddisjonssalter med uorganiske syrer så som hydroklor, hydrobrom, svovel, bor eller fosforsysre, eller med organiske syrer så som eddik, oksal, palmitin, stearin, laurin, benzon, fumar, rav, malein, malin, vin eller sitronsyre, eller med organiske sulfonsyrer så som metansulfon- eller p-toluensulfonsyrer.
Forbindeslene i henhold til oppfinnelsen kan administreres i enhver effektiv farmasøtisk akseptabel form, for eks. i oral, parenteral eller infeserbar doseringsform, eller intranasal doseringsform. Egnede parenterale administrerings-former omfatter for eks. intr amuskulaer, intravenøs, intra-peritoneal eller substansial administrering av forbindelsen
I tillegg til den aktive forbindelse, kan blandinger i henhold ti; denne oppfinnelse for parenteral injeksjon inne holde farmasøytisk akseptable sterile vandige eller ikke-vandige oppløsninger, suspensjoner, aerosoler eller emulsjoner. Eksempler på egnede ikke vandige bærere fortynnere, oppløsningsmidler eller emner omfatter propylen glykol, polyetylen glykol, vegetabilske oljer så som olivenolje og injeserbare organiske estere så som etyloleat. Slike blandinger kan også inneholde tilsettningsstoffer så som pres-erverings, fuktig, emulgerings og dispergeringsmidler. De kan være sterilisert for eks. ved filtrering gjennom et bakterie tilbakeholdende filter, eller ved innlemmelse av steriliserende midler i blandingene. De kan også fremstilles i form av sterile faste sammensetninger som kan opp-løses i sterilt vann eller i andre sterile injeserbare medier, direkte for bruk.
Faste doserinsformer for oral administrering omfatter kapsler, tabletter, piller, pulvere og granuler. I slik fast doseringsform, kan den aktive forbindelse blandes sammen med minst en inert forbinder så som sakkarose, melkesukker eller stivelse. Slike doseringsformer kan også omfatte tilleggssubstanser andre enn inerte fortynningsmidler, hvilket er vanlilg praksis, og for eks. smøremidler så som magnesium stearat. I tilfelle tabletter, kapsler og piller, kan doseringsformene også omfatte buffrende midler. Tabletter og piller kan i tillegg fremstilles med tarmovertrekk.
Flytende doseringsformer for oraladministrering omfatter farmasøytiske akseptable emulsjoner, oppsløninger, suspensjoner, sirup, og ekiksirer som inneholder inerte fortynnere vanligvis anvendt i faget vanligvis som vann. For-uten slike inerte fortynnere kan blandingene også inneholde tilsettningsstoffer så som fuktemidler, emulgerigns og sus-penderingsmidler og søtstoffer, smaksstoffer og parfyme-stof f er..
Doseringsformer for intranasaladministrering omfatter opp-løsninger, suspensjoner eller emulsjoner av den aktive forbindelse i et flytende medium for administrering som dråper eller en spray, egnede flytende medier omfatter vann, propylen glykol og andre farmasøytisk akseptable alkoholer og sesam eller peanøttolje og andre farmasøytisk. Doseringsformer for intranasaladministrering omfatter også oppløs-ninger, suspensjoner eller emulsjoner av den aktive forbind -else i flytende drivmidler så som diklorodifluormetan eller klorotrifluormetan for administrering fra en trykk-beholder. Doserignsformer for intranasal dosering kan også omfatte salver eller geleer som inneholder den aktive forbindelse. Doseringsformene for intranasaladministrering kan også være steriliserte og/eller kan inneholde tilsettningsstoffer så som preservativer, stabiliseringsstoffer, emul-geringsstoffer, salter for å variere det osmotiske trykk eller buffere.
Doseringsnivåer av den aktive forbindelse i blandinger i henhold til oppfinnelsen kan varieres for å oppnå den ønskede terapeutiske respons for en spesiell blanding og admin-istreringsmåte. Generelt vil den aktive forbindelse administreres som en vandig oppløsningsspray på fra 0.0001 til 1.0 % konsentrasjon. Mer foretrukket vil konsentrasjonen være fra 0.025 til 0.10 %. Hvis ønsket kan den daglige dose deles opp i flere doser for administrering.
Det forutnevnte kan forstås bedre ved de følgende eksempler som fremlegges for å illustrere og som ikke er ment å begrense omfanget av oppfinnelsen.
EKSEMPEL 1
3, 5- Dikloro- 4- amino benzyl alkohol
Metanol (400 ml) ble tilsatt dråpevis i to timer til en blanding av metyl-3,5-dikloro-4-aminobenzoat (110 g), NaBH4(142,5 g) og t-butanol (1,1 1). Ved tilført tilsettning, ble kokt under tilbakeløp i 3 timer og avkjølt og deretter ble U^ O (1,2 1 ) tilsatt forsiktig. De resulterende lag ble adskilt og den organiske fase ble vasket med H20 (2 x 200 ml), adskilt, tørket (MgS04), filtrert og fordampet for å gi det ønskede produkt (89 g), smp 110-112°C.
EKSEMPEL 2
N-( 2, 6- diklori- 4- klorometylfenyl) sulfinyl imid.
3,5-Dikloro-4-amino benzylalkohol (88 g) ble oppløst i di-cloretan (900 ml) og tionyl klorid (141 g) i dikloretan (200 ml) ble raskt tilsatt under omrøring. En eksoterm raksjon oppstod mens blandingen tyknet. Efter ca. 30 min. var ikke lengere reaksjonsblandingen viskøs og ble ravgul. Tilbakeøpet ble fortsatt tilleggsvis i 1 time, fulgt av konsentrasjonen og reaksjonsoppløsningen. Dette gav 117 g av det ønskede produkt som en olje som var oppløsbar i fryser.
EKSEMPEL 3
3, 5. Dikloro- 4- amino benzylnitril
Produktet fra eksempel 2 (125 g) ble oppløst i eter (300 ml og tilsatt under omrøring til en iskald blanding av konsentrert HC1 (300 ml) og eter (200 ml). Ett faststoff oppstod som ble filtrert etter 15 min og tilsatt i en iskald sus-pensjon av pulverisert NaCN (86 g) i DMSO (430 ml). Blandingen ble oppvarmet til 60°C og omført i 45 min. Reak-sjonen ble avkjølt, is ble tilsatt og faststoffet filtrert. Det filtrerte faststoffet ble oppløsti CH2C12(400 ml),
og så vasket suksessivt med oppløst vandig NaOH og i mettet saltlake. De organiske lag ble separert og tørket (MgSO^) for å gi 67 g av det ønskede produkt, smp 117 - 119°C.
EKSEMPEL 4
2-[( 3, 3- Dikloro- 4- aminobenzyl)[ imidazolin hydroklorid.
Produktet fra eksempel 3 (10 g) ble oppløst i DME (80 ml) og MeOH (9,5 ml). Oppløsningen ble avkjølt i et isbad og HC1 (g) ble boblet opp i oppløsningen i 30 min. Reaksjons-kolben ble stoppet og tillatt og stå i romtemperatur over natten. Oppløsningen var konsentrert, eter (50 ml) ble tilsatt og det resulterende faststoff ble filtrert. Faststoffet ble tilsatt til EtHH (80 ml) og oppløsningen ble av-kjølt, fulgt en tilsettning avetylendiamin (17,8 ml). Etter omrøring i 15 min ble opplsningen kokt ved tilbakekløp i 15 min. Reaksjonsblandingen ble konsentrert og CHCl^ble tilsatt, fulgt av en tilsettning av U^ O og vanndig 45%
KOH (5 ml). Efter blanding ble de organiske lag separert og vasket med H^ O (2 x 50 ml). De organiske lag ble igjen separert og metanolisk HC1 ble tilsatt. Efter tørking over MgSo^, førte fordaming til et rått produkt. Dette ble opp løst i CH^CN (150 ml) og varmet over dampbad i ca. 3 min. Oppløsningen ble avkjølt og de resulterende krystaller ble filtrert, vasket med kald CH^CN (20 ml) og derpå eter (20 ml) for å gi detønskede produkt, Smp 261 - 264°C.
EKSEMPEL 5
2-[( 3, 5- Difluor- 4- amino) benzyl] imidazolin hydroklorid
Begynne med metyl-3,5-Difluor-4-amino benzonat og ved å bruke fortløpende fremgangsmåtene i eksemplene 1-4, for å gi det ønskede produkt.
EKSEMPEL 6
2-[( 3, 5- Diiodi- 4- amino) benzyl] imidazolin hydroklorid
Begynne med metyl-3,5-diiodo-4-amino benzoat og bruke fort-løpende fremgangsmåtene ji eksemplene 1-4, for å gi detønskede produkt.
EKSEMPEL 7
Alpha- l- Receptor vekselvirknig i kanin aorta
a) Fremgangsmåte Spiralformede strimler av torakal aorta (4 x 20 mm) fra en hunkanin ble neddykket i 10 ml vevbad
som inneholdt bikarbonat buffer av følgende sammensettning:
(mM): NaCl 119, KC1 4,7, CaCl22,5 MgS041,5,
KH2P041,2, NaHCO^25, dextrose 11, absorberende syre
0,3, og NaEDTA 0,03. Oppløsningen ble gjennomstrømmet av en gassblanding 95% 02+ 5% C02 ved 37°C, pH 7,40. Isometriske kontraksjoner av vevet, preladet til en spenning på 2 G ble målt med Grass FT03 deformasjonsmåler og nedtegnet på en Grass Nodel 7 polygraff. Efterfulgt av en egu-ilibrerings periode på 90 min, ble vevene tilbakejustert til 2 G spenning, og en kontroll kummulativ dose-respons kurve ble oppnådd for standard agonisten, norepinephrine. Efter utvasking av norepinephrine (60 -90 min) ble vevene igjen eguilibrert og en kumulativ dose-responskurve av for-søksagonisten ble oppnådd.
b) Resultater Produktet fra eksempel 4, når utprøvd i henhold til a var en full (alpha-1) agonist i kanin aorta da
den var 1,31 +/- 0,22 ganger sterkere enn norepinephrine.
EKSEMPEL 8
Alpha- 2 receptor vekselvirkning i fenoksybenzamin ( PBZ)-behandlede hundelegg årer
a) Fremgangsmåte Ringer (3-4 mm bredde) av sidelegg årer tatt ut fra beagle hunder av begge kjønn, ble neddykket i 10 ml vevbad som inneholdt bekarbonat buffer med følgende sammensettning: (mM) : NaCl 119, KC1 4,7, CaCl2 2,5, MgSC>41,5, KH2P041,2, NaHC0320, dextrose 11, askorbinsyre 0,3, NaEDTA 0,03, kokkain 0,03, hydrokortison hemisuccinat 0,04, og propranoll 0,004. Oppløsningen ble gjennomstrømmet med gass av følgende sammensettning 95% + 5% C02ved 37°C, pH7,40. Isometriske kontraksjoner av vevet, ble forladet med en spenning på 2 G målt med Grass FT03 form-asjonsmålere og nedtegnet på en Grass model 7 polygraf. Efter en equilibreringsperiode på 15 - 20 min og maksimal kontraksjon med norepinephrine (1E-4M) ble vevene vasket i 60 min hvoretter de ble utsatt for (PBZ, 1E-7M) i 30 min.
Efter PBZ behandlingen fulgte en grundig utvasking i 60 min Vevene ble deretter gjenjustert til 2 G spenning og en kontroll kummulativ dose-responskurve ble oppnådd for standard agonisten, norepinephrine. Etter utvasking av norepinephrine (45 - 60 min), ble vevene igjen equilibrert og en kummulativ dose-responskurve av den utprøvde agonist ble oppnådd. Når antagonisten utprøves i denne modell, administreres forskjellige konsentrasjoner av et slikt middel i vevbadet 30 min før den andre dose-respons kurve av norepinephrine. Dette eksperiment gjentas i flere vev for hver konsentrasjon av f orsøksf orbindelsen eller kontrollbasreren. Den konkurrerende antagonisme bestemmes og karakteriseres under anvendelse av pA2(1/affinitet) og dets stigning.
b) Resultater Produktet fra eksempel 4, når utprøvd i henhold til fremgangsmåte "a" var fri for alpha-2 agnostisk
aktivitet i dette vev til en konsentrasjon på 10 ^ (n=2). På den annen side utviste produktet fra eksempel 4 en konkurrerende alpha-2 antagonisme av norepinephrine kon-
traksjon i dette vev (pA2=6,51 + /- 0,23; Slope= 0,93
+/- 0,19.
EKSEMPEL 9
Alpha- 1/ Alpha- 2 reseptor interaskjon ved pre- postsynaptisk
reseptorer i kaninlunge arterier.
a) Fremgagnsmåte Spiralformede strimpler av hovedlunge-arterien (4 x 30 mm) fra hunkaniner ble festet vertikalt
mellom to platin elektroder til Grass FT03 deformasjonsmål-ere etter 60 min inkubering med H norepinephrine (0,375
x 10 ^M ved 11 Ci/mmol). Prepareringen ble deretter overhelt med en buffer av følgende sammensettning: (mM): NaCl 119, KC1 4,7, CaC12 2,5, MgSC>41,5, NACHC>325 KH2~
P041,2, glukose 11, askorbinsyre 0,3, NaEDTA 0,03, kokkain 0,03, hydrokortison hemisuccinat 0,04, og propranolol 0,004. Oppløsningen ble gjennomstrømmet med gass av sammensettning 95% 02+ 5% C02 ved 37°C, pH7,40, og helt over ved en konstant hastighet på 2 ml/min. Elektriske felt stimulasjoner (2 Hz; 9 V; 0,3 msek; 3 min) ble påført til vevene i 48 min. intervaller for å frembringe frigivelse av endogen norpinephrine, hvilket reflekteres av enøkning av tritium overstrømning i de fraksjonelle 3-min samlinger av væsken som helles (presynaptisk begivenhet) formidler de isometiske konraksjon av vevet (postsynaptisk begivenhet).
b) Resultater I forsøket med overhelt kaninlune arterie utviste produktet fra eksempel 4 en tidlig stimulasjon av
den postsynaptiske (alpha-1) reseptor ved konsentrasjoner
- 7
som var sa lave som 10 M.
I de samme forsøk på den annen side var produktet fra eksempel 4, fri for enhver hemmende effekt av den stimulerte overstrømning (ved konsentrasjoner opp til 10 ^M), som viser at den ikke er i besittelse av presynaptisk (alpha-2) agonistisk aktivitet. Faktisk forsterker produktet fra eksempel 4 den stimulerte titium overstrømning (ved konsen trasjoner 10 ^M), og viser slik igjen at denne forbindelse har alpha-2 antagonistisk aktivitet.
EKSEMPEL 10
Effekt på luftveismotstand i bedøvede hunder etter intranasal spray administrering
a) Fremgangsmåte Beaglehunder av begge kjønn, som veide 9
- 12 kg ble bedøvet med nembutal (30 mg/kg, intravenøst)
med tilleggsdoser tilført iløpet av forsøket som nødvendig. Hundene ble intubert med en chuffertendotrake'før og ble ventilert med romluft ved hjelp av en Harvard respirasjons-pumpe. Arteriellt blostrykk ble opptegnet for en lårarterie under anvendelse av en Statham P23Gb trykk transduser. En takygrafisk nedtegning av hjertehastigheten ble oppnådd fra blodtrykkssignalet.
En konstant strøm av luft (2 l/min), frembrakt av en anest-esimaskin (CAECO) ble administrert til nesehulen gjennom et 6 cm. langt plastikrør tilspisset til en diameter på 6 mm. for å passe inn i det høyre nesebor. Luften overstrømmet nesehulen og slapp ut igjennom munnen. Motstanden mot luft-strømmen utøvet av det store overflateareal til neseslim-hinnen ble målt som et nasaltrykk i cm. av vann, under anvendelse av en Model MP 45 Validyne transduser. For medikament administrering, tillot en "bypass" anordning i luft-rørsystemet en strøm gjennom en 690 Ultrasonic fukter som inneholdt det forstøvede medikament. Dette instrument for-støver medikamentoppløsningen til en damp som ble ført med luftstrømmen inn i neseveiene. Det ble kalibrert til å for-støve 1,6 ml av væske over et 5 min. tidsrom av overstrøm-ning. Da mengden av medikament administrert på denne måte er meget liten og ikke direkte målbar, ble dosene kalkulert fra konsentrasjonen av medikamentet i fukteren og volumet av væsken som ble brukt ved hver administering (i mg. per hund). Den nasale motstand, hjertehastigheten og blodtrykket ble kontrollert før og deretter 2 t. etter medikament- administrering. En nedgang i nasal motstand er indikerende for hva som er vasokonstriktuær aktivitet. Forandringene i hjertehastighwet og blodtrykk reflekterer en systematisk absorbsjons av av forsøksforbindelsen.
b) Resultater Når utprøvd i henhold til fremgangsmåte a, frembrakte produktet fra eksempel 4 administert i aerosol
en sterk og doseavhengig nedgang av motstand mot luftstrøm ved dens passaje gjennom nesehulen. Terskeldosen for produktet fra eksemple 4 som frembrakte denne effekt var 0,00165 mg. En begynnende forbigående økning av denne para-meter ble observert med alle doser av alle forbindelser utprøvd, hvilket kan tilbakeføres til manipulasjon av før-støvningsarbeidstrinnet. Ved den effektive nasale dekonge-stantdose, var produktet fra eksempel 4 fri for enhver systematisk hemodynamisk effekt som kun observeres med en alpha agonist. Kun den høyest utprøvde dose av produktet fra eksempel 4 (1,65 mg) frembrakte en svak forhøyelse av blodtrykket, omtrent 10% over grunnivået. Denne observasjon antydet at produktet fra eksempel 4 absorberes dårlig fra den nasale slimhinne og det er derfor usansynlig at det vil forårsake negative bieffekter.
EKSEMPEL 11
Effekt på nasal irritasjon i rotter
Produktet fra eksempel 4 ble oppløst i fosfat bufferet nor-mal saltoppløsning og justert til en Ph på omtrent 7. Oppløsningen ble fremstilt ved følgende konsentrasjoner: 0,05, 0,15, 0,5 og 1,5%. Grupper på 5 mannlige rotter (CD stamme) ble administrert hver dose. Rottene var omtrent 5 uker gamle ved begynnende behandling. Før dosering, ble
ketaminhydroklorid administrert intraperitonealt for å frem
-bringe en forbigående bedøvning for å lette administrering av forsøksforbindelsen. Rottene i hver gruppe ble gitt 0,04 ml/dag av den tilsvarende oppløsning i 14 dager. Dosene ble oppdelt likt mellom begge nesebor (dvs 0,02 ml/nesebor/dag)
Produktet fra eksempel 4 forårsaket ingen skade av den nasale slimhinne eller systemiske toksiske effekter selv når dens største konsentrasjoner ble utprøvd, dvs 1,5%.
Som kontrast forårsaket oksymetazolin, når testet en nasal slimhinneskade ved 0,5&% og 1,5% konsentrasjon. Systemiske sideeffekter og død oppstod i de fleste dyr og etter gjen-tatt administrering av disse doser, hvilket antyder systemiske og toksisitet forbundet med oksymetazolin. Oksymetazolin forårsaket ikke skader ved konsentrasjoner på 0,05% til 0,15%.
Summerende utviser forbindelsene i henhold til oppfinnelsen alpha-1 agonist aktivitet, alpha-2 antagonist aktivitet, effektivitet som nasale dekongestanter og ingen nasal slim-hinneirritasjon.
Det forutgående er kun illustrativt for oppfinnelsen og er ikke ment å begrense oppfinnelsen til de viste forbindelser fremgangsmåter og blandinger. For eks. kan forbindelsene i henhold til oppfinnelsen også være nyttige ved behandlingen av ophtalmi eller otisk kongesjon. De kan også anvendes som antidepressjonsmidler, antihypertensivemidler eller hemmere av blodplate aggresjon. Andre variasjoner og forandringer som er åpenlyse for fagfolk skal også være innen omfanget og naturen av oppfinnelsen sonm er definert i de vedlagte krav.
