Description
Denne oppfinnelsen vedrører farmasøytiske blandinger for behandling av forøket intraokulært trykk omfattende bimatoprost.
Bimatoprost vist under er et prostamid kommersielt markedsført for behandling av glaukom og okulær hypertensjon.
Benzalkoniumklorid (BAK) er et konserveringsmiddel anvendt i mange kommersielle øyeprodukter for å forhindre mikrobiologisk kontaminasjon i multi-anvendelsesprodukter. De kommersielle øyedråper (Bimatoprost, Allergan, Inc., Irvine, CA) inneholder 0,03% bimatoprost og 0,005% BAK. Selv om ingen andre prostamiderfor tiden markedsføres for behandling av glaukom er mange prostaglandinanaloger kommersielt tilgjengelige som anvender BAK som et konserveringsmiddel. Disse inkluderer latanoprost (Xalatan), travoprost (Travatan), og unoprostonisopropyl (Rescula), som trenger vesentlig mer BAK fra 150-200 ppm, for å møte antimikrobielle effektivitetstester i USA og Europa.

I visse blandinger er konsentrasjonen av BAK fra 150 ppm til 200 ppm. I andre blandinger er konsentrasjonen av BAK fra 150 ppm til 200 ppm. I andre blandinger er konsentrasjonen av BAK fra 150 ppm til 250 ppm.
I øyeblandinger kan et geleringsmiddel anvendes for å forsterke konserveringsmidlets effektivitet. Egnede geleringsmidler er dem vel kjent i faget og, uten å være ment begrensende, er edetatsalter (EDTA) nyttige geleringsmidler.
I visse blandinger er konsentrasjonen av EDTA på mindre enn 0,001%. I andre blandinger er konsentrasjonen av EDTA på mindre enn 0,01%. I andre blandinger er konsentrasjonen av EDTA på 0,15% eller mindre. I andre blandinger er konsentrasjonen av EDTA 0,1% eller mindre. I andre blandinger er konsentrasjonen av EDTA 0,05% eller mindre.
Visse blandinger omfatter fra 150 til 250 ppm BAK og en effektiv mengde av
EDTA.
Som vel kjent i faget er buffere vanlig anvendt for å justere pH ved et ønsket område for anvendelse i øyet. Generelt er en pH på rundt 6-8 foretrukket, og i visse blandinger er en pH på 7,4 foretrukket. Mange buffere inkluderer salter av uorganiske syrer slik som fosfat, borat og sulfat kjent.
Andre vanlig anvendte hjelpestoffer i blandinger for øyet er en viskositetsforsterker eller et fortykningsmiddel. Fortykningsmidler er anvendt av flere grunner fra å forbedre formen av formuleringen for egnet administrering til forbedring av kontakten med øyet til å forbedre til biotilgjengeligheten. Viskositetsforsterkende midler kan omfatte en polymer omfattende hydrofile grupper slik som monosakkarider, polysakkarider, etylenoksydgrupper, hydroksylgrupper, karboksylsyrer eller andre ladete funksjonelle grupper. I øyeblandinger er ofte tonisitetsmidler anvendt for å justere sammensetningen av formuleringen til det ønskede isotonisk området. Tonisitetsmidler er vel kjent innenfor området og noen eksempler inkluderer glyserin, mannitol, sorbitol, natriumklorid, og andre elektrolytter.
En blanding har en pH på 7,4 og består vesentlig av 0,015% bimatoprost, 200 ppm benzalkoniumkloird, fra 0 til 0,03% EDTA, en fosfatbuffer, NaCI og vann.
Andre blandinger har en pH på 7,4 og omfatter 0,02% bimatoprost, 200 ppm benzalkoniumklorid, fra 0 til 0,03% EDTA, en fosfatbuffer, NaCI og vann.
Andre blandinger har en pH på 7,4 og består av 0,01% bimatoprost, 200 ppm benzalkoniumklorid, fra 0 til 0,03% EDTA, en fosfatbuffer, NaCI og vann.

Eksempel 1
Formuleringer inneholdende 0,268% natriumfosfat dibasisk heptahydrat, 0,014% sitronsyre, 0,83% natriumklorid, med pH justert til 7,3 i qs vann, og mengdene av bimatoprost, BAK og EDTA som oppført i tabell 1 under ble fremstilt ved konvensjonelle fremgangsmåter vel kjent innenfor faget.
Eksempel 2
Studier ble utført for å bestemme effekten av benzalkoniumklorid (BAK) og d-alfatokoferylpolyetylenglykol 1000 succinat (TPGS) på okulær absorpsjon av bimatoprost in vivo. For in vivo studien ble atten hunn-kaniner gitt en enkelt 28 ul øyedråpe bilateralt og vandige væskeprøver ble oppsamlet (n = 3 dyr med 6 øyne pr. formulering) ved 60 min. postdose. To kaniner (4 øyne) forble ubehandlet for å tjene som predosebioanalytiske kontroller. Bimatoprost og dens stamkarboksylsyre ekstrahert fra vandig væske og in vitro prøver ble analysert ved en væskekromatografi tandem massespektrometri (LC-MS/MS) fremgangsmåte med et kvantiteringsområde på 0,25-60 ng/ml.
Forårsaket av ekstensiv metabolisme av bimatoprost i kaninøyne ble stamsyren anvendt som et surrogat for å bestemme okulær absorpsjon av bimatoprost. Konsentrasjonen av syren i kanin vandig væske etterfulgt enkelt dose av 6 ulike bimatoprostformuleringer er oppsummert i figur 1 og tabell 2 under.
Testformuleringene inneholdende 0,015%, 0,2%, 0,4% og 1,0% TPGS resulterte i lavere vandig væske karboksylsyrekonsentrasjon sammenlignet med bimatoprost ved hhv. 52%, 59%, 62% og 72%. I motsetning resulterte 0,03% bimatoprost inneholdende 200 ppm BAK i 57% høyere vandig væske AGN 191522 konsentrasjon sammenlignet med bimatoprost (50 ppm BAK).
Sammenlignet med bimatoprostkontroll resulterte formuleringer inneholdende TPGS i avtagende biomatoprostpermeabilitet. I motsetning resulterte formuleringer med høyere BAK i høyere permeabilitet.
Eksempel 3
Formuleringer inneholdende 0,268% natriumfosfat dibasisk heptahydrat, 0,014% sitronsyre, 0,83% natriumklorid, med en pH justert til 7,3 i qs vann og mengder av bimatoprost, BAK og EDTA som oppført i tabell 3 under ble fremstilt ved konvensjonelle metoder vel kjent innen faget.
Eksempel 4
Effekten av benzalkoniumklorid (BAK) og etylendiamintetraeddiksyre (EDTA) på bimatoprostpermeabilitet over primærkultur av
kaninhornhinneepitelcellelag (RCECL). Hornhinneepitelceller ble høstet fra new
zealandske hvite kaniner og dyrket på Transwell™-filtere til konfluens (dag 5). For transporteksperiment ble celler først ekvilibrert i transportbuffer i 1 time ved 37°C. Doseringsløsning inneholdende 0,015% eller 0,03% bimatoprost med varierende konsentrasjoner av BAK og EDTA ble så applisert til det apikale kammer av Transwell™ (2 kulturer; n=3-4 pr. kultur) og celler ble inkubert ved 37°C. 200^l prøver ble tatt ved 30, 60, 90 og 120 minutter etter dose fra det basolaterale kammer for apikal til basolateral (AB) transport. Prøvene ble analysert ved væskekromatografi tandem massespektrometri (LC-MS/MS) fremgangsmåte med kvantiteringsområde på 1-600 ng/ml.
Resultatene er presentert i figur 2.
Eksempel 5
En dråpe av formulering J administreres én gang pr. dag topisk til øyet av en person som lider av glaukom. Etter noen få timer observeres intraokulære trykkdråper mer eller mindre hyperemi enn det ville bli observert for formulering A. Senkede intraokulære trykk vedvarte for så lenge som behandlingen pågikk.
Claims (11)
1. Blanding for behandling av forøket intraokulært trykk omfattende fra 0,005 vekt% til 0,02 vekt% bimatoprost og fra 100 ppm til 250 ppm benzalkoniumklorid, minst en buffer og med en pH på omtrent 7,3, hvor nevnte blanding er en vandig væske som er formulert for administrasjon til øyet.
2. Blanding ifølge krav 1, hvor blandingen er for behandling av forøket intraokulært trykk hos pasienter som lider av glaukom eller intraokulær hypertensjon.
3. Blanding ifølge krav 1, hvor konsentrasjonen av benzalkoniumklorid er 200 ppm.
4. Blanding ifølge krav 1, som består vesentlig av fra 0,01 vekt% til 0,02 vekt% bimatoprost og fra 150 ppm til 200 ppm benzalkoniumklorid, en fosfatbuffer, NaCI, sitronsyre monohydrat, saltsyre, natrium hydroksid og vann.
5 Blanding ifølge krav 1, hvor konsentrasjonen av bimatoprost er 0,01 vekt % og hvor blandingen administreres en gang daglig.
