Foreliggende oppfinnelse angår anvendelse av en kombinasjon av antibakterielle midler, mer spesielt gemifloksasin eller et salt derav, og et P-laktamantibiotikum, for fremstilling av et medikament til behandling av bakterielle infeksjoner.
Gemifloksasin [(R,S)-7-(3-aminometyl-4-metoksyiminopyrrolidin-l-yl)-l-syklopropyl-6-fluor-4-okso-l,4-dihydro-l,8-naftyridin-3-karboksylsyre] er et antibakterielt fluorkinolinmiddel som har forsterket antibakteriell aktivitet in vitro mot Gram-positive bakterier, mens det bibeholder utmerket aktivitet mot Gram-negative bakterier. Uten å være bundet eller begrenset til en teori er gemifloksasin ment å virke via inhibering av bakteriell topoisomerase II og IV. Gemifloksasin er meget selektiv for bakteriell heller enn human topoisomerase II.
EP 688772 beskriver nye naftyridinkarboksylsyrederivater, inkludert gemifloksasin. WO 98/42705 beskriver gemifloksasinmesylat og hydrater derav inkludert sesquihydratet.
Det er fremdeles behov for forbedrede fremgangsmåter for å behandle bakterielle infeksjoner. Særlig fortsetter infeksjoner forårsaket av Pseudomonas aeruginosa å representere et terapeutisk problem. I klinisk praksis har kombinasjonen av p-laktamantibiotika og antibakterielle aminoglykosidmidler blitt vist å ha en forbedret virkning ved behandling av infeksjoner forårsaket av P. aeruginosa. Øket resistens hos P. aeruginosa mot aminoglykosider koblet med deres potensielle nefrotoksisitet betyr imidlertid at det fremdeles er behov for alternative behandlinger.
Andre eksempler på infeksjoner hvor forbedrede fremgangsmåter for behandling trenges inkluderer de forårsaket av Enterococcus faecalis, Staphylococcus saprophyticus, og Escherichia coli.
Foreliggende oppfinnelse tilveiebringer som nevnt en anvendelse av en kombinasjon av antibakterielle midler, mer spesielt gemifloksasin eller et salt derav, og et p-laktamantibiotikum, for fremstilling av et medikament for behandling av bakteriell infeksjon hvori en effektiv mengde av gemifloksasin eller et salt derav og et P-laktamantibiotikum administreres, separat, samtidig, eller etterfølgende til en pasient som behøver det. Pasienten kan være et menneske eller dyr, og er i et foretrukket utførelseseksempel et menneske.
Gjeldende oppfinnere har funnet kombinasjoner av gemifloksasin eller salt derav og et P-laktamantibiotikum som tilveiebringer et antibakterielt regime som har et bredere spektrum av aktivitet enn hvert middel alene. Spesielt har slike kombinasjoner blitt funnet å vise synergistisk aktivitet mot flere kliniske isolater av Pseudomonas aeruginosa, i tillegg til refereanselinjer av Enterococcus faecalis, Staphylococcus saprophyticus eller Escherichia coli, i forhold til hvert middel alene.
Egnede salter av gemifloksasin inkluderer de beskrevet i WO 98/42705, EP 688772 og US patent nr. 5 776 944.1 foretrukne utførelseseksempler er saltet av gemifloksasin valgt fra mesylat og hydrater derav, spesielt sesquihydrat slik beskrevet i WO 98/42705.
Egnede (3-laktamantibiotikaer anvendelige i henhold til oppfinnelsen inkluderer penicillinene: f.eks. amoksycillin, ampicillin, apalcillin, aspokicillin, azidocillin, azlocillin, aztreonam, benzylpenicillin, bacampicillin, karbenicillin, kloksacillin, syklacillin, dikloksicillin, epicillin, flukloksacillin, lenampicillin, mecillinam, meticillin, mexlocillin, fenoksymetylpenicillin, piperacillin, pivampicillin, propocillin, sulbenicillin, talampicillin, og tikarcillin; og cefalosporinene: f.eks. sefakolor, sefadroksil, sefatrizin, sefsilidin, sefamandol, sefazolin, sefbuperazon, sefkaneldaloksat, sefdinir, sefepime, sefetametpivoksil, sefiksim, sefminoks, sefminoksim, sefmetazol, sefonisid, sefoperazon, cefotaksim, sefotetan sefotiam, sefotiamhekstil, sefoksitin, sefpimizol, sefpiramid, sefpiriom, sefpodoksimproksitil, sefprozil, ceftazidim, seftibuten, seftizoksim seftriakson, sefuroksimaksetil, sefuroksim, sefasetril, sefhaleksin, sefhaloridin, sefhalotin, sefamanodolnaftat, sefapirin, sefhoperazon, sefuslodin, sefuzonam, sefradin, loarakarbef, DQ 2556, ME1207, S-1006, SCE-2787 og moksalaktam.
I spesielt foretrukne utførelseseksempler er P-laktamantibiotikumet anvendt i henhold til oppfinnelsen valgt fra cefalosporinene, f.eks. cefotaksim og ceftazidim.
P-laktamantibiotikaene referert til i denne søknaden kan være i form av frie syrer eller farmasøytisk akseptable salter eller in vivo hydrolyserbare estere.
Gemifloksasin eller et salt derav og et P-laktamantibiotikum eller en kombinasjon som omfatter de samme, kan benyttes i henhold til foreliggende oppfinnelse for å modulere bakteriell metabolisme (f.eks. kliniske isolater, referansebakterier, patogene bakterier) og/eller til å behandle infeksjoner forårsaket av slike bakterier. Kliniske isolater eller referansebakterier inkluderer Streptococcus pneumoniae (f.eks. ATCC 49619), Haemophilus influenzae (f.eks. ATCC 49247), Moraxella catarrahalis (f.eks. 1502), Staphylococcus aureus (f.eks. ATCC 29213), Staphylococcus saprophyticus (f.eks. 662), Klebsielle pneumoniae (f.eks. E70), Proteus vulgaris (f.eks. ATCC 13315), Enterococcus faecalis (f.eks. ATCC 29212), Escherichia coli (f.eks. ATCC 25922) og Pseudomonas aeruginosa (f.eks. ATCC 27853, 6016, 6156, 6168, P003, 6140, PA018R, 6003, 6021, 6030, 6045, 6047, 6113, 6123, 6173, P032, P036, P039, 6003, 6021, 6034, 6102, 6104, 6113, 6170, 6173, 6178, P020, P022, P028, P033, P035, P039, P045, P049,.P050, P061, P070, P071).
Visse utførelseseksempler av foreliggende oppfinnelse inkluderer følgende behandlingsregimer:
a) gemifloksasin og ceftazidim for behandling av infeksjoner forårsaket av Pseudomonas aeruginosa; b) gemifloksasin og cefotaksim for behandling av infeksjoner forårsaket av Enterococcus faecalis; c) gemifloksasin og amoksicillin for behandling av infeksjoner forårsaket av Staphylococcus saprophyticus; og d) gemifloksasin og sefaklor for behandling av infeksjoner forårsaket av Escherichia coli.
Det er fordelaktig at gemifloksasin eller et salt derav og P-laktamantibiotikumet blir administrert i et forhold av fra omtrent 10:1 til omtrent 1:10, mer egnet omtrent 5:1 til 1:5, typisk omtrent 2:1 vekt/vekt, uttrykt som henholdsvis vekten av den frie syren og den frie basen.
Det er fordelaktig at gemifloksasin eller et salt derav og p-laktamantibiotium anvendt i henhold til foreliggende oppfinnelse administreres i all hovedsak samtidig. Dette kan enkelt oppnås ved samadministrering av separate farmasøytiske sammensetninger som omfatter et floksasin eller et salt derav og et P-laktamantibiotikum. Slike separate sammensetninger kan på nyttig vis fremskaffes som et sett som omfatter en gemifloksasin, eller et salt derav, sammensetning og en P-laktamantibiotikasammensetning. Settet inneholder fortrinnsvis tilstrekkelige doser av gemifloksasin, eller et salt derav, og P-laktamantibiotikumet for en enkel terapigjennomgang for den spesielle infeksjonen som skal behandles, sammen med admini streringsinstruksj oner.
I henhold til foreliggende oppfinnelse tilveiebringes også et sett av deler for anvendelse i behandling av bakterielle infeksjoner i pattedyr som omfatter gemifloksasin eller et salt derav og en P-laktamantibiotiumformulering. Settet i følge oppfinnelsen omfatter for eksempel en antibakterielt effektiv mengde av (a) en farmasøytisk sammensetning som omfatter gemofloksasin eller et salt derav, og en farmasøytisk akseptabel bærer, og (b) en farmasøytisk sammensetning som omfatter et P-laktamantibiotikum og en farmasøytisk akseptabel bærer.
Alternativt kan gemofloksasin, eller et salt derav, og et P-laktamantibiotikum bli formulert sammen og administrert i en enkel sammensetning.
Det beskrives videre heri en farmasøytisk sammensetning som omfatter gemifloksasin eller et salt derav, et P-laktamantibiotikum, og en farmasøytisk akseptabel bærer.
Det beskrives også heri fremgangsmåter for behandling av bakterielle infeksjoner som omfatter administrering til et pattedyr i behov av slik behandling, en terapeutisk effektiv mengde av gemifloksasin eller et salt derav og et P- laktamantibiotikum. I foretrukne utførelseseksempler omfatter behandlingen administrering av terapeutisk effektive mengder av gemifloksasin eller et salt derav og et p-laktamantibiotikum, hvori aktiviteten av antibiotikaene mot den bakterielle infeksjonen er synergistisk.
Det beskrives også heri en fremgangsmåte for modulering av metabolismen av bakterier, som omfatter å skape kontakt mellom bakteriene og en antibakterielt effektiv mengde av gemifloksasin og et salt derav, og et P-laktamantibiotikum (valgfritt i form av en antibakterielt effektiv sammensetning eller et sett deravnsetninger, slik beskrevet heri). Modulering av metabolisme vil egnet omfatte inhibering av vekst av bakteriene eller å drepe bakteriene. Kontaktskapingen med bakteriene kan fordelaktig omfatte trinnet å introdusere antibiotikumet eller sammensetningen eller settet av sammensetninger som omfatter samme inn i et pattedyr. I foretrukne utførelseseksempler, omfatter fremgangsmåten å skape kontakt mellom bakteriene og en antibiotisk effektiv mengde av gemifloksasin eller et salt derav og et P-laktamantibiotikum, eller sammensetning eller sett av sammensetninger som omfatter samme, hvori aktiviteten av antibiotikaene mot bakterien er synergistisk.
Egnede formuleringer som omfatter gemifloksasin inkluderer de beskrevet i WO 98/42705, EP 688772, og US patent nr. 5 776 944.
Egnede formuleringer som omfatter et P-laktamantibiotikum er vel kjent og er lett tilgjengelig kommersielt.
Gemifloksasin eller et salt derav kan formuleres med et p-laktamantibiotikum og standard farmasøytiske bærere eller fortynningsmidler i henhold til konvensjonelle prosedyrer som er vel kjent. Disse prosedyrene kan involvere blanding, granulering og komprimering eller løsning av ingrediensene slik som er egnet for ønsket fremstilling.
Det beskrives videre en fremgangsmåte for fremstilling av en farmasøytisk sammensetning som omfatter gemifloksasin eller et salt derav og en P-laktamantibiotikum hvor fremgangsmåten omfatter å blande sammen en kombinasjon av gemifloksasin eller et salt derav og en p-laktamantibiotikum og en farmasøytisk akseptabel bærer.
Det beskrives videre en formulering for behandling av bakterielle infeksjoner som omfatter gemifloksasin eller et salt derav og et P-laktamantibiotikum. I foretrukne utførelseseksempler omfatter formuleringen en mengde av gemifloksasin eller salt derav og et p-laktamantibiotikum hvori aktiviteten av antibiotikaene i behandling av bakterielle infeksjon er synergistisk.
Det beskrives videre bruk av en formulering som omfatter gemifloksasin eller et salt derav og et P-laktamantibiotikum i fremstilling av et medikament for behandling av bakteriell infeksjoner.
I følge anvendelsen og settet i henhold til foreliggende oppfinnelse er den bakterielle infeksjonen fortrinnsvis én forårsaket av P. aeruginosa, S. saprophyticus, E. faecalis eller E. coli.
Infeksjoner forårsaket av P. aeruginosa inkluderer sårfunksjoner, urinveisinfeksjoner og luftveisinfeksjoner, sammen med generelle infeksjoner i en immunsvekket pasient. Infeksjoner forårsaket av S. saprophyticus og E. faecalis inkluderer urinveisinfeksjoner og generelle infeksjoner i en immunsvekket pasient. Infeksjoner forårsaket av E. coli inkluderer urinveisinfeksjoner.
Det kontaktskapende trinnet og administreringstrinnet i en hvilken som helst av fremgangsmåtene som beskrives heri kan utføres på mange måter som vil være lett synlig for en gjennomsnitts fagperson. Det er imidlertid foretrukket at det kontaktskapende trinnet og administreringstrinnet er en tilveiebringelse av en sammensetning som omfatter gemifloksasin eller et salt derav og et p-laktamantibiotikum, eller i tilfellet av et sett i henhold til foreliggende oppfinnelse, en sammensetning som omfatter gemifloksasin eller et salt derav og en sammensetning som omfatter P-laktamantibiotika, til en human pasient som har behov for slik(e) sammensetning(er), eller direkte til bakterie i kulturmediet eller buffer.
Når det bringes i kontakt til en human pasient eller i kontakt med nevnte bakterie i en human pasient eller in vitro, kan sammensetningen(e) som omfatter gemifloksasin eller et salt derav og/eller et P-laktamantibiotikum, fortrinnsvis farmasøytiske sammensetning(er), administreres i en hvilken som helst effektiv, praktisk vis inkluderende måte, f.eks., administrering via lokale, orale, anale, vaginale, intravenøse, intraperitoneale, intramuskulære, subkutane, intranasale eller intradermale ruter blant andre.
Det er også foretrukket at disse sammensetningene blir benyttet i kombinasjon med en ikke-steril eller steril bærer eller bærere for anvendelse i celler, vev eller organismer, så som en farmasøytisk bærer som er egnet for administrering til et individ. Slike sammensetninger omfatter, f.eks., en terapeutisk effektiv mengde av gemifloksasin eller et salt derav, og/eller et P-laktamantibiotikum, en farmasøytisk akseptabel bærer eller bindemiddel, og valgfritt et medietilsetningsstoff. Slike bærere kan inkludere, men er ikke begrenset til, fysiologisk saltvann, bufret saltvann, dekstrose, vann, glyserol, etanol og kombinasjoner derav. Formulering(ene) bør passe til administreringsmåten.
I forhold til terapi eller som et forebyggende middel, administreres gemifloksasin eller et salt derav og P-laktamantibiotika fortrinnsvis til et individ som en injiserbar sammensetning (inkluderer sammensetninger som omfatter begge antibiotika, eller separate injeksjonssammensetninger som omfatter det ene eller det andre antibiotikum), f.eks. som en steril vandig dispersjon, fortrinnsvis en isotonisk en.
Alternativt kan gemifloksasin eller et salt derav og P-laktamantibiotika i i henhold til foreliggende anvendelse bli formulert for topisk påføring f.eks. i form av salver, kremer, hudkrem, øyesalve, øyedråper, øredråper, munnvask, fuktede bandasjer og suturer og aerosoler, og kan inneholde egnede konvensjonelle tilsetningsstoffer, inkludert, f.eks., konserveringsmidler, løsemidler for å assistere medikamentinntrenging og mykgj ørere i salver og kremer. Slike topiske formuleringer kan også inneholde egnede konvensjonelle bærere, f.eks. krem eller salvebaser, og etanol eller oleylalkohol for hudkremer. Slike bærere kan utgjøre fra omtrent 1 % til omtrent 98 % av vekten av formuleringen; vanligvis vil de utgjøre opptil omtrent 80 % av vekten av formuleringen.
For administrering til pattedyr, og spesielt mennesker, er det forventet at den antibakterielt effektive mengde er et daglig dosenivå av det aktive middel fra 0,001 mg/kg til 10 mg/kg, typisk rundt 0,1 mg/kg til 1 mg/kg, fortrinnsvis omtrent 1 mg/kg. En lege vil uansett bestemme den faktiske dosering som er best egnet for et gitt individ og vil variere i forhold til alder, vekt og respons til det spesielle individet. Ovennevnte doseringer er eksempler i forhold til gjennomsnittlige tilfeller. Det kan naturligvis være individuelle tilfeller hvor høyere eller lavere doseringsområder er på sin plass, og slike er innenfor rekkevidden av oppfinnelsen. Det er foretrukket at doseringen er valgt til å modulere metabolismen av bakterien på et slikt vis at den inhiberer eller stopper veksten av nevnte bakterier eller ved å drepe nevnte bakterier. Fagpersonen kan identifisere denne mengden slik tilveiebrakt heri så vel som ved å benytte andre fremgangsmåter kjent i faget, f.eks. ved bruk av MIC-tester.
Et ytterligere utførelseseksempel av oppfinnelsen legger grunnlaget for at kontakttrinnet eller administreringstrinnet i fremgangsmåten videre omfatter kontakt med en innsatt anordning i en pasient. Innsatt anordninger inkluderer, men er ikke begrenset til, kirurgiske implantater, prostetiske anordning og katetre, dvs. anordninger som er introdusert til kroppen av et individ og som blir i posisjon for en utstrakt tid. Slike anordninger inkluderer, f.eks., kunstige ledd, hjerteventiler, pacemakere, vaskulære innsatser (grafts), vaskulære katetre, serebrospinale væskeventiler, urinveiskatetre, og kontinuerlige ambulatoriske peritoneale dialyse (CAPD)-katetre.
Gemifloksasinet eller saltet derav og P-laktamantibiotikumet, eller sammensetningen eller settet derav i henhold til oppfinnelsen, kan administreres ved innsetting for å oppnå en systemisk effekt mot relevante bakterier, kort før
injisering av en innsatt anordning. Behandlingen kan fortsette etter kirurgi i løpet av tiden anordningen befinner seg i kroppen. I tillegg kan også sammensetningen eller settet av sammensetninger benyttes til å forbedre dekningen rundt selve operasjonen i forhold til en hvilken som helst kirurgisk teknikk for å forhindre bakterielle sårinfeksjoner.
I tillegg til terapien beskrevet over kan gemifloksasin eller et salt derav og et P-laktamantibiotikum, eller en sammensetning eller sett av samme sammensetninger benyttet i fremgangsmåtene som beskrives heri bli benyttet generelt som et sårbehandlingsmiddel for å forhindre fastklebing av bakterier til matriksproteiner, eksponert i sårvev og for forebyggende bruk i tannbehandling som et alternativ til eller i sammen med antibiotikaprofylakse.
Alternativt kan gemifloksasin eller et salt derav og P-laktamantibiotika, eller sammensetning eller sett av sammensetninger i henhold til oppfinnelsen benyttes for å bade en innsatt anordning umiddelbart før innsetting. Det aktive midlet vil fortrinnsvis være tilstede ved en konsentrasjon av 1 ug/ml til 10 mg/ml for vaksing av sår eller innsatte anordninger.
Oppfinnelsen vil herved bli beskrevet ved følgende eksempler som er illustrative og ikke er tenkt å begrense oppfinnelsen slik den er beskrevet over.
Eksempler
Potensialet for kombinasjonsterapien med gemifloksasin og et P-laktamantibiotikum ble undersøkt. 1) Den kombinerte virkning av gemifloksasin og cefotaksim ble undersøkt i E. faecalis ved bruk av en sjakkbrettitreringsmetode . Evaluering for synergi aktivitet ble utført ved beregning av fraksjonelt inhiberende konsentrasjoner (FICer) i henhold til fremgangsmåten til Eliopoulos et al. (1996, Antimicrobial Combinations, Antibiotics in Laboratory Medicine, Victor Lorian, 4. utgave, sider 337-338, Baltimore, MD, Williams and Wilkins).
E. faecalis ATCC 29212 ble innhentet fra SmithKline Beecham Anti-Infectives Research Culture Collection og lagret ved -80°C i 10 % glyserol. Før testing ble isolatet dyrket på agarplater (tryptikase soyaagar som inneholdt 5 % saueblod) fra frossent lager i to etterfølgende dager. Kation justert Mueller Hinton medie (BBL, Cockeysville, MD) ble brukt for isolatet.
Sjakkbrettmikrotitermedie-fortynningsplater ble klargjort ved å bruke Hamilton MicroLab AT Plus system (Reno, NV). Doble seriefortynninger (50 ul) av cefotaksim ble utført i kolonner 1-11 Då en mikrotiterolate. Doble seriefortvnnineer av gemifloksasin ble klargjort manuelt og MicroLab AT Plus ble benyttet for å utplassere 25 ul av gemifloksasin ved hver konsentrasjon i radene A til G av mikrotiterplaten. Konsentrasjonsområdet som ble testet varierte for hver medikament/organismekombinasjon for å passe til MIC-endepunktene av de individuelle komponentene. Kolonne 12 inneholdt doble seriefortynninger av kun gemifloksasin, og rad H inneholdt doble seriefortynninger av kun cefotaksim. Den siste brønnen (H) i kolonne 12 ble benyttet som en positiv vekstkontrollbrønn, og inneholdt kun mediet og testisolat.
Bakteriene ble fortynnet til en 0,5 McFarland-standard og deretter ytterligere fortynnet 1/50. Hver brønn ble inokulert med 25 ul av isolatet for å gi en avsluttende inokulumtetthet på tilnærmet 5x10<5>cfu/ml. MicroLab AT Plus 2 ble brukt for å tilsette inokulum til mikrotiterplatene. Etter inokulering ble platene dekket med et 96-brønners mikrotitcrplatelokk og inkubert ved 35°C i omgivelsesluft for 20-24 timer. En 10 ul porsjon av podestoffet ble platet på tryptikasesoyaagar som inneholder 5 % saueblod for å bestemme renheten til det avsluttende testpodestoffet.
Etter inkubering ble en mikrotiterspeilleser (Cooke Instruments Ltd., England) brukt til å assistere bestemmelsen av mikrofortynnings MIC-endepunkter. MIC ble bestemt som den laveste forbindelseskonsentrasjon som inhiberte synlig vekst av organismen.
De fraksjonerte inhiberende konsentrasjoner (FIC) ble beregnet ved å bruke følgende formel:

Gemifloksasin/cefotaksimkombinasjoner viste en synergistisk effekt mot E. faecalis ATCC 29212.
Andre gemifloksasin/cefotaksimkombinasjoner som ble testet viste en additiv eller likegyldig effekt mot E. faecalis ATCC 29212.1 brønner hvor det ikke ble observert vekst og hvor MIC til én av midlene hadde blitt oppnådd eller overtrådt, ble resultatet rapportert som inhibert. 2) S. saprophyticus 662 ble innhentet fra SmithKline Beecham Anti-Infectives Research Culture Collection og lagret ved -80°C i 10 % glyserol. Før testing ble isolatet dyrket på agarplater (tryptikase soyaagar som inneholdt 5 % saueblod) fra det frosne lageret i to etterfølgende dager. Kationjustert Mueller Hinton medie (BBL, Cockeysville, MD) ble benyttet for isolatet.
Den kombinerte effekten av gemifloksasin og amoksicillin ble undersøkt i & saprophyticus 662 ved å benytte sjakkbrettitreringsmetoden beskrevet i eksempel 1.
FIC-indeksene ble kalkulert og tolket på samme vis som i eksempel 1. En gemifloksasin/amoksisillinkombinasjon viste en synergistisk effekt mot S. saprophyticus 662.
Andre testede gemifloksasin/amoksicillin-kombinasjoner viste en additiv eller likegyldig effekt mot S. saprophyticus 662.1 brønner hvor ingen vekst ble observert og hvor MIC til ett av midlene hadde blitt oppnådd eller oversteget, ble rapportert som inhibert. 3) E. coli ATCC 25922 ble innhentet fra SmithKline Beecham Anti-Infectives Research Culture Collection og lagret ved -80°C i 10 % glyserol. Før testing ble isolatet dyrket på agarplater (tryptikase soyaagar som inneholdt 5 % saueblod) fra det frosne lageret i to etterfølgende dager. Kationjustert Mueller Hinton medie (BBL, Cockeysville, MD) ble benyttet for isolatet.
Den kombinerte effekten av gemifloksasin og sefaklor ble undersøkt i E. coli ATCC 25922 ved å benytte sjakkbrettitreringsmetoden beskrevet i eksempel 1. FIC-indekser ble beregnet og tolket som i eksempel 1.
En gemifloksasin/sefaklorkombinasjon viste en synergistisk effekt mot E. coli 25922.
Andre testede gemifloksasin/sefaklorkombinasjoner viste en additiv eller likegyldig effekt mot E. coli 25922.1 brønner hvor ingen vekst ble observert og hvor MIC til ett av midlene hadde blitt oppnådd eller oversteget, ble resultatet rapportert som inhibert. 4) Synergistisk aktivitet av gemifloksasin i kombinasjon med sefazidim ble undersøkt i 27 klinisk isolerte P. aeruginosa ved å bruke sjakkbrettitrerings-fremgangsmåten, og sammenlignet med den til en siprofloksasin/ceftazidim-kombinasjon.
Gemifloksasin/sefazidim-kombinasjonen viste en god, synergistisk effekt mot P. aeruginosa og hadde bedre synergistiske profiler enn siprofloksazin/ceftazidim-kombinasjonen. For gemifloksasin/ceftazidimkombinasjonen, ble det observert en synergistisk effekt (FIC < 0,5) i 13 av 27 stammer og en additiv eller likegyldig effekt (0,5 > FIC < 4) i 14 av 27 stammer. Med en siprofloksasin/ceftazidim-kombinasjon, ble en synergistisk effekt sett i 7 stammer og en additiv eller likegyldig effekt i 20 stammer. Ingen antagonisme (FIC > 4) ble sett i noen av kombinasjonene. I tillegg viste gemifloksasin/ceftazidim bedre MIC-profiler mot P. aeruginosa (MIC rekkevidde: 0,063-1 ug/ml) enn siprofloksasin/ceftazidim (MIC rekkevidde: 0,063-4 ug/ml), selv om gemifloksasin selv (MIC rekkevidde: 0,25-32 ug/ml) var mindre potent enn siprofloksasin (MIC rekkevidde: 0,13-16 ug/ml).
Testorganismene var kliniske isolater som var samlet inn i Korea. Organismene var frosset ved -70°C i forkant av testingen. Testingen ble utført ved sjakkbrettmetoden i 96-brønners plater ved hjelp av Mueller-Hinton medie. Gemifloksasin og siprofloksasin ble testet ved 13 konsentrasjoner (0,016-64 ug/ml), og ceftazidim ble testet ved 6 konsentrasjoner (1-32 ug/ml). Gemifloksasin og siprofloksasin-løsninger ble satt ut alene i den første raden og kombinert med ceftazidim i de gjenværende radene. Ceftazidim ble også satt ut alene i den første raden i den første kolonnen. Teststammer av P. aeruginosa ble dyrket i 18 timer i Mueller Hinton medium, og deretter ble disse overnattkulturene fortynnet med samme friske medie til en tetthet av omtrent IO<7>CFU/ml. De bakterielle konsentrasjonene ble bestemt ved måling av optisk tetthet eller turbiditet av suspensjonen og ble perfusert ved standard kolonitellinger på antibiotikafrie agarplater. En fortynnet bakteriell løsning ble påført til 96-brønners plater som inneholdt seriefortynnede antimikrobielle midler for å gi IO<4>CFU pr. punkt. Platene ble inkubert aerobt over natten.
Minimale inhiberende konsentrasjoner (MICer) for hvert separate medikament ble bestemt. For brønner langs vekst-ikke-vekst grenselinjen, ble det bestemt synergi ved å beregne fraksjonell inhiberende konsentrasjon (FIC) indeks. De fraksjnelt inhiberende konsentrasjons (FlC)-indeksene ble beregnet ved hjelp av følgende formel:
FIC indeks = FICA+ FICB= [A]/MICA+ [B]/MICB
hvori:
FICA(eiierB) er FIC til legemiddel A (eller B)
MICa (ellerb) er MIC for organismen til legemiddel A (eller B)
A (eller B) er konsentrasjonen av legemiddel A (eller B) som er den laveste inhiberende konsentrasjon.
FIC-indeksene ble fortolket ved å bruke følgende kriterier:
< 0,5 = synergi;
> 0,5-1,0 = additiv;
> 1,0-4,0 = indifferens;
> 4,0 = antagonisme
Følgende gemifloksasin/ceftazidimkombinasjoner viste synergi mot det benyttede P. aeruginosa stamme:
Andre testede gemifloksasin/ceftazidim-kombinasjoner viste en additiv eller likegyldig effekt mot andre P. aeruginosa- stammer.
5) Den synergistiske aktivitet av gemifloksasin eller siprofloksasin i kombinasjon med ceftazidim ble undersøkt i 6 klinisk isolerte P. aeruginosa stammer. Testorganismene var kliniske isolater innsamlet i Korea. Organismene var frosset ved -70°C i forkant av testing. Den kombinerte effekt av gemifloksasin eller siprofloksasin og ceftazidim ble undersøkt ved sjakkbrettfremgangsmåten i 96-brønners plater ved bruk av Mueller Hinton medie. Gemifloksasin og siprofloksasin ble testet ved 12 konsentrasjoner (0,016-32 ug/ml), og ceftazidim ble testet ved 8 konsentrasjoner (0,25-32 ug/ml). Gemifloksasin eller siprofloksasin løsning ble satt ut alene i første rad og kombinert med ceftazidim i de gjenværende rader. Ceftazidim ble også satt ut alene i den første raden i den første kolonnen. Teststammer av P. aeruginosa ble dyrket i 18 timer i Mueller Hinton medie, og deretter ble disse overnattkulturene fortynnet med samme friske medie til en tetthet på omtrent 10 CFU/ml. De bakterielle konsentrasjonene ble bestemt ved måling av optisk tetthet eller turbiditet av suspensjonen og ble verifisert ved standard kolonitellinger på antibiotikafrie agarplater. Fortynnet bakteriell løsning ble satt på 96-brønnersplater som inneholdt seriefortynnede antimikrobielle midler til en mengde av 5 x IO<5>CFU/ml. Platene ble inkubert ved 35°C i 18 timer.
MIC'er for hvert separate medikament ble bestemt, og for brønnene langs vekst-ikke-vekst grensesnittet, ble synergien bestemt ved beregning av FIC-indekser som i eksempel 4. FIC-indekser ble beregnet og tolket på samme vis som i eksempel 4.
Kombinasjoner av gemifloksasin/seftaszidim var synergistiske for P. aeruginosa 6168, P003 og 6140.
Kombinasjoner av testet gemifloksasin/ceftazidim viste en additiv effekt mot P. aeruginosa 6016, 6156 og PA018R.
Industriell anvendelse
I henhold til foreliggende oppfinnelse, tilveiebringer kombinasjoner av gemifloksasin eller et salt derav og et p-laktamantibiotikum et antibakterielt regime som har et bredere spektrum av aktivitet enn en hvilken som helst av midlene alene. Spesielt viser slike kombinasjoner synergistisk aktivitet mot flere kliniske isolater av Pseudomonas aeruginosa, og samtidig referansestammer av Enterococcus faecalis, Staphylococcus saprophyticus eller Escherichia coli, i forhold til et hvilket som helst av midlene alene.
Claims (10)
1. Anvendelse av gemifloksasin eller et salt derav og et P-laktamantibiotikum for fremstilling av et medikament behandling av bakterielle infeksjoner ved separat, samtidig eller etterfølgende administrering til en pasient med behov for samme, av en effektiv mengde av gemifloksasin eller et salt derav, og et P-laktamantibiotikum.
2. Anvendelse i henhold til krav 1, hvori gemifloksacinmesylat anvendes til fremstilling av et medikament for behandling av bakterielle infeksjoner.
3. Anvendelse i henhold til krav 1, hvori P-laktamantibiotikumet er et cefalosporin.
4. Anvendelse i henhold til krav 3, hvori cefalosporinet er cefotaksim.
5. Anvendelse i henhold til krav 3, hvori cefalosporinet er ceftazidim.
6. Anvendelse i henhold til et hvilket som helst av de foregående krav, hvori gemifloksasin eller et salt derav og et P-laktamantibiotikum administreres i et forhold på fra omtrent 10:1 til omtrent 1:10 (vekt/vekt).
7. Anvendelse i henhold til et hvilket som helst av de foregående krav, hvori den bakterielle infeksjonen er forårsaket av Pseudomonas aeruginosa.
8. Anvendelse i henhold til et hvilket som helst av de foregående krav, hvori en effektiv mengde av gemifloksasin eller et salt derav og et P-laktamantibiotium i hovedsak administreres samtidig.
9. Anvendelse i henhold til krav 8, oppnådd gjennom samadministrering av separate sammensetninger som omfatter gemifloksasin eller et salt derav og et P-laktamantibiotikum.
10. Sett omfattende gemifloksasin eller et salt derav og en P-laktamantibiotikaformulering.
