Den foreliggende oppfinnelsen vedrører visse umettede fettsyrederivater av terapeutisk aktive glukokortikostereoider og farmasøytiske formuleringer som inneholder disse. De nevnte derivatene henvises til som "Forbindelser med formel (I)" i den foreliggende beskrivelsen og kravene. Forbindelser med formel (I) kan anvendes ved behandling av kreft- og inflammasjonssykdommer. Behandling av både hematologiske og faststoffe cancere og steroidresistente cancere, astma så vel som steroidresistente inflammasjoner og KOLS er inkludert.
Glukokortikosteroider benyttes i stor utstrekning ved behandling av inflammasjonssykdommer, like astma, reumatoid artritt, inflammatorisk tarmsykdom og autoimmune sykdommer. Glukokortikosteroider er også grunnprinsippmedikamenter ved behandlingen av akutt lymfoblastisk leukemi (ALL) (1). Selv om deres anvendelse ved behandling av kroniske inflammasjonssykdommer og kreft er vanlig, er den optimale virkeevnen begrenset av iboende og ervervet motstand mot medikamentet.. Steroidresistens er også et stort problem ved håndteringen av pasienter med inflammatorisk tarmsykdom (2). Et lite antall astmatiske pasienter responderer ikke godt eller ikke i det hele tatt på kortikosteroidterapi (3). Selv om problemstillingen med glukokortikosteroidresistent kan være mindre utforsket f.eks. når det gjelder reumatoid artritt enn hematologisk cancer, deler de to sistnevnte vanlige resistensmekanismer, og strategier for å overvinne resistens etter sykdomsbehandling vil være av felles interesse (7).
De fordelaktige effektene av glukokortikosteroider antas å være formidlet av undertrykking av inflammatorisk genekspresjon. De virker ved å binde seg til en enkelt reseptor (glukokortikoidreseptor, GR) som er lokalisert i cellers cytosol. Ved aktivering transporteres GR til kjernen hvor den enten slår på (transaktivering) anti-inflammatoriske gener eller slår av (transrepresjon) inflammatoriske gener (4). Geninduksjon krever GR dimerisering og DNA binding til spesifikke GR responselementer (GRE) lokalisert i promoterregionen til mottakelige gener. Glukokortikosteroider kan øke transkripsjonen av gener som koder for anti-inflammatoriske proteiner, slik som lipokortin-1 og interleukin-10. Den mest slående effekten til glukokortikosteroider er å hemme ekspresjonen av multiple inflammatoriske gener (cytokiner, enzymer, reseptorer og adhesjonsmolekyler). Den proinflammatoriske cytokin granulocytt-makrofag koloni-stimulerende faktoren (GM-CSF) er et eksempel. Anvendelsen av glukokortikoidene er begrenset av alvorlige bivirkninger (5), på grunn av deres endokrin- og metabolistiske virkninger, antakelig formidlet av indusert transkripsjonen regulering av direkte GR regulerte målgener. De mulige ugunstige effektene som er rapportert inkluderer osteoporose og undertrykking av den hypotalamiske hypofyse-adrenokortikale akse, etter systemisk bruk, reduksjon av veksthastighet hos barn, benmineraltap, okulære symptomer og hudforandringer etter inhalering av kortikosteroid (6). Reguleringen av transaktiveringen og transrepresjonen, både på en vev- og passasjespesifikk måte kan gi forbedring av terapeutisk indeks og åpne for svært selektive nye kortikosteroider (6) med høyere terapeutiske effekt mot inflammasjon eller kreft og færre bivirkninger på grunn av lavere transkripsjonen regulering av de GR-regulerte genene som er viktige for bivirkningene.
Videre medfører det mekanistiske grunnlaget også for anti-cancer effektene til glukokortikosteroidene en interaksjon med GR og dets målgener som kontroller ekspresjon av celledød/apoptoseproteiner.
Cellelinjene U937 og THP1 er humane monocytiske/makrofagceller og representerer cellelinjemodellsystemer som benyttes for å teste produkter med hensyn på anti-inflammatorisk og anti-canceraktivitet, der cellelinjene også er relevante med hensyn på glukokortikoidresistens, da de også er motstandsdyktige mot vekstinhibisjon med deksametason og prednisolon (7).
Den felles overdratte publikasjonen W098/32718 beskriver adrenokortikosteroider som kan være derivatisert med lipofile grupper. Blant forbindelsene som er nevnt er betametason, deksametason og beklometason. Disse derivatiserte forbindelsene er benyttet for å behandle inflammasjon.
Fettsyrekonjugater av budesonid, mer spesifikt budesonidoleat, palmitat, linoleat, palmitoleat og arakidonat har blitt beskrevet. Disse konjugatene dannes ved behandlingen av inflammasjon, mer spesifikt astma, men fettsyrekonjugatene av steroidene som er nevnt, sies å være farmakologisk inaktive lipoidal konjugater (12).
EP 170642 A2 vedrører et farmasøytisk preparat som omfatter derivatiserte steroider i kombinasjon med liposomer for administrering primært til luftveiene. Den biologiske virkningen påstås å være mer selektiv for applikasjonsområdet i lungene, men ingen endret effekt er vist.
Den foreliggende oppfinnelsen har overraskende vist at visse umettede fettsyrederivater av glukokortikosteroider demonstrerer nye og overraskende terapeutiske aktiviteter. Et stort antall av de nevnte derivater utgjør nye kjemiske forbindelser som ikke tidligere er beskrevet. Derivatene ifølge den foreliggende oppfinnelsen har vist økt aktivitet og færre bivirkninger sammenlignet med tidligere beskrevne glukokortikoidderivater og de har vist aktivitet i steroidresistente systemer.
Det er kjent at glukokortikosteroider har en posisjon ved behandlingen av kreftsykdommer og inflammasjon. Iboende og ervervet resistens for behandling med glukokortikosteroider er velkjent. Det er rapportert at de human U937 og THP1 monocytt/makrofagcellene representerer et cellelinjemodellsystem kjennetegnet ved iboende resistens for glukokortikosteroidene dexametason og prednisolon (7). Vi har bekreftet disse funnene og i tillegg vist at cellene er kryssresistente for det svært potente steroidet flutikasonpropionat, der ingen aktivitet ses opptil lOOuM.
Som vist i eksemplene har vi overraskende funnet høy anti-proliferativ aktivitet for glukokortikoidfettsyrederivatene i disse resistente cellelinjene. Disse funnene muliggjør behandling av sykdommer som er resistente for behandling med kjente glukokortikoider.
Vi har overraskende funnet en økt anti-inflammatorisk aktivitet kombinert med en mindre uttrykt aktivering av gener som er relevante for bivirkninger med glukokortikoid fettsyrederivatene. En 22 ganger mer potent inhibisjon av cytokin GM-CSF frigivelsen har blitt observert for flutikasonelaidinsyrederivatet sammenlignet med flutikasonpropionat i aktiverte lungeceller. Samtidig er 7 ganger reduksjon i bivirkninger funnet. Bivirkningene representeres ved aktivering av GRE elementer. Genaktiveringen er 85 % mindre for steroidderivatene i dette systemet der aktivering er et mål for bivirkninger. Disse resultatene øker den relative terapeutiske indeksen med 154 ganger.

Forbindelsene kan også anvendes for behandling av inflammasjon. Mer spesifikt er forbindelsene vel egnet for behandling av steroidresistente inflammasjoner. Forbindelsene kan også anvendes ved behandling av KOLS (kroniske obstruktiv lungesykdom).
Forbindelsene ifølge den foreliggende oppfinnelsen forårsaker færre bivirkninger enn basisstereoidene og har en mer spesifikk anti-inflammatorisk aktivitet.

De nevnte eksipientene er valgt i henhold til den ønskede farmasøytiske formen og den ønskede administreringsmåten. Nevnte eksipienter sammen med en forbindelse med formel (I) ifølge oppfinnelsen vil typisk formuleres på en doseringsform som er tilpasset for administrering til en pasient ved den tiltenkte administreirngsruten. Doseringsformer inkluderer således former som er tilpasset for: (a) inhaleringsadministrering slik som aerosoler, løsninger og tørre pulvere; (b) topisk administrering slik som kremer, lotion, pastaer, salver, løsninger, spray og geler; (c) intravenøs administrering slik som sterile løsninger, suspensjoner og pulvere for rekonstruksjon; (d) oral administrering slik som tabletter, kapsler, piller, pulvere, siruper, eliksirer, suspensjoner, oppløsninger, emulsjoner og poser ("sachets"). Forbindelsen med formel (I) kan også administreres som øyedråper.
Egnede farmasøytisk akseptable eksipienter vil variere avhengig av den bestemte doseringsformen. Farmasøytisk akseptable eksipienter inkluderer blant annet de følgende: fortynningsmidler, smøremidler, fyllstoffer, sprengmidler, oppløsningsmidler, fuktemidler, suspenderingsmidler, emulgatorer, granuleringsmidler, beleggingsmidler, bindemidler, smaksstoffer, smaksmaskerende stoffer, søtningsmidler, myknere, viskositetsmidler, antioksidanter, stabilisatorer, overflateaktive stoffer og buffermidler. Visse eksipienter kan tjene mer en funksjon.
Egnede fortynningsmidler og fyllstoffer inkluderer laktose, sukrose, dekstrose, mannitol, sorbitol, potetstivelse, maisstivelse, pregelatinisert stivelse, cellulose og derivater derav. Egnede bindemidler inkluderer stivelse, gelatin, akaika, natriumalginat, alginsyre, tragakant, guargummi, povidon og cellulose og derivater derav. Egnede sprengmidler inkluderer natriumstivelseglykolat, alginsyre, krospovidon og natriumkarboksymetylcellulose.
Forbindelsene med formel (I) kan også bli koplet til bionedbrytbare polymerer for kontrollert frigivelse av et medikament.
De farmasøytiske preparatene ifølge oppfinnelsen kan fremstilles ved å benytte metoder og teknikker som er kjent for en fagmann på området.

11B- Elaidinsvreester av flutikasonpropionat
Til en løsning av flutikasonpropionat (1,02 g, 2,04 mmol) i diklormetan (35 ml) ble elaidinsyre (576 mg, 2,04 mmol) tilsatt etterfulgt av 4-(dimetylamino)pyridin (249 mg, 2,04 mmol) og 1,3-dicykloheksylkarbodiimid (421 mg, 2,04 mmol). Den oppnådde løsningen ble rørt i 24 timer og da avslørte TLC at en betydelig mengde av utgangsmaterialet fremdeles var til stede. Blandingen ble konsentrert under vakuum (~10 ml løsning) og rørt i ytterligere 140 timer. Blandingen ble videre konsentrert under vakuum og renset ved flashkromatografi på silikagel ved å eluere med heksan/EtOAc (3:1) til å gi 1.23 g (79 %) av den ønskede forbindelsen som en fargeløs olje.
6a, 9a- Dilfuoro- llB. 17a- dihvdroksv- 16a- metyl- 3- oksoandrosta- l, 4- dien- 173-karboksylsyre
Til en suspensjon av flumetason (1,19 g, 2,9 mmol) i tetrahydrofuran (11 ml) ble det tilsatt en løsning av periodesyre (2,25 g, 9,9 mmol) i vann (5.3 ml) tilsatt. Den oppnådde løsningen ble rørt ved romtemperatur i 1 time. Tetrahydrofuran ble fjernet under vakuum for å etterlate en vandig suspensjon som ble filtrert, og det faste stoffet ble vasket med vann og tørket til å gi 1,17 g (100 %) av tittelforbindelsen som en fargeløs olje.
6a, 9a- Dilfuoro- 17a- eIaidovloksv- llB- hydroksv- 16a- metvl- 3- oksoandrosta- l, 4-dien- 17B- karboksylsyre
En løsning av 6oc,9a-difluoro-l ip,17a-dihydroksy-16a-metyl-3-oksoandrosta-l,4-dien-17p-karboksylsyre (1,22 g, 3,1 mmol) og trietylamin (0,72 g, 7,1 mmol) i aceton (25 ml) ble tilsatt elaidoylklorid i diklormetan (10 ml). Elaidoylklorid var blitt fremstilt fra elaidinsyre (2,0 g, 7,1 mmol), oksalylklorid (2,6 ml, 30 mmol) og DMF (katalytisk mengde) i toluen (45 ml) ved å røre ved omgivelsestemperatur i 17 timer og deretter fordampe til tørrhet. Den oppnådde blandingen ble rørt i 2 timer ved romtemperatur, behandlet med dietylamin (0,97 ml, 9,2 mmol) og den oppnådde løsningen ble rørt i ytterligere 1.5 timer ved romtemperatur. 1 M HCl ble deretter tilsatt og blandingen ble ekstrahert med diklormetan (3 x 50 ml). De forenede organiske ekstraktene ble tørket (Na2S04), filtrert og fjernet under vakuum. Råproduktet ble oppløst i EtOAc, tilsatt heksan og satt i kjøleskapet over natten. Det off-white faste stoffet (2,07 g, som inneholdt noe iV^/V-dietylamid) som var oppnådde ble samlet opp ved filtrering, tørket og benyttet direkte i det neste trinnet. Noe av tittelforbindelsen (170 mg) ble renset ved flashkromatografi på silikagel ved å eluere med heksan/EtOAc (4:1) etterfulgt av heksan/EtOAc/AcOH (100:100:1) til å gi 123 mg (72 %) som et hvitt faststoff.
6a. 9a- Dilfuoro- 17f3-( ArJV- dimetvlkarbamovltio) karbovl- 17a- elaidoyloksv- liP-hvdroksv- 16a- metyl- 3- oksoandrosta- l, 4- dien
En løsning av 6a,9a-difluoro-17a-elaidoyloksy-l ip-hydroksy-16cc-metyl-3-oksoandrosta-l,4-dien-17P-karboksylsyre (1,9 g, 2,9 mmol) og A^V-dimetyltio-karbamoylklorid (0,71 g, 5,8 mmol) i aceton (40 ml) ved romtemperatur ble behandlet sekvensielt med trietylamin (0,58 g, 5,8 mmol), vannfri natriumiodid (0,43 g, 2,9 mmol) og vann (0,19 ml, 10 % vekt/vekt med steroidet). Løsning ble rørt i 18 timer, tilsatt vann (100 ml) og EtOAc (100 ml) og fasene ble separert. Den organiske fasen ble vasket med 1 M HC1, en 5 % vandig løsning med natrium hydrogen karbonat, vann, tørket (Na2SC>4), filtrert og fjernet under vakuum. Flashkromatografi på silikagel ved å eluere med heksan/EtOAc (3:2) ga 1,55 g (~70 %, to trinn) av tittelforbindelsen som et gult faststoff.
6a, 9a- Dilfuoro- 17a- elaidovloksv- llB- hvdroksv- 16a- metvl- 3- oksoandrosta- 1. 4-dien- 17B- karbotioinsyre
En løsning av 6a,9a-difluoro-17B-(Ayv"-dimetylkarbamoyltio)karboyl-17a-elaidoyloksy-HB-hydroksy-16a-metyl-3-oksoandrosta-l,4-dien (1,55 g, 2,1 mmol) i dietylamin (16 ml) ble kjørt med tilbakeløp i 3 timer. Løsningen ble avkjølt til omgivelsestemperatur, helt over i kald 3 M HC1 (150 ml) og ble ekstrahert med EtOAc (2 x 150 ml). De forenede organiske ekstraktene ble vasket med vann, og saltoppløsning før tørking (Na2S04), filtrering og fordampning under vakuum. Råproduktet (1,35 g, off-white faststoff) ble benyttet direkte i det neste trinnet.
S-Klormetvl 6a, 9a- difluoro- 17a- elaidovloksv- llB- hvdroksv- 16a- metyl- 3-oksoandrosta- 1. 4- dien- 17B- karbotioat
En rørt løsning av 6a,9a-difluoro-17a-elaidoyloksy-l ip-hydroksy-16oc-metyl-3-oksoandrosta-l,4-dien-17p-karbotioinsyre (1,35 g, 2 mmol) i diklormetan (40 ml) ble tilsatt trietylamin (0,28 ml, 2 mmol) etterfulgt av kloridodometan (0,56 ml, 8 mmol). Den oppnådde løsningen ble rørt ved omgivelsestemperatur i 70 timer, tilsatt en mettet vandig løsning med ammoniumklorid og fasene ble separert. Den vandige fasen ble ekstrahert med diklormetan (2 x), og de forenede organiske fasene ble tørket (Na2S04), filtrert og fordampet under vakuum. Flashkromatografi på silikagel ved å eluere med heksan/EtOAc (3:1) ga 1.04 g (72 %) av tittelforbindelsen som en fargeløs olje.
S- Iodometyl 6at9a- difluoro- 17a- elaidovloksv- llB- hvdroksv- 16a- metyl- 3-oksoandrosta- l, 4- dien- 17B- karbotioat
En løsning av S-klormetyl 6a,9a-difluoro-17ct-elaidoyloksy-l ip-hydroksy-16a-metyl-3-oksoandrosta-l,4-dien-173-karbotioat (1,04 g, 1,4 mmol) i aceton (100 ml) ble tilsatt natriumiodid (0,86 g, 5,7 mmol) og refluksert i 22 timer. Løsningsmidlet ble fordampet under vakuum og resten ble oppløst i EtOAc og vasket med vann, 10 % vandig løsning med natriumhydrogenkarbonat, vann, tørket (Na2S04), filtrert og fordampet under
vakuum. Råproduktet (1,1 g) ble benyttet direkte i det neste trinnet.
S- Fluorometvl 6a. 9a- difluoro- 17a- elaidovloksv- llB- hvdroksv- 16a- metvl- 3-oksoandrosta- l, 4- dien- 17B- karbotioat
En løsning av S-iodometyl 6a,9a-difluoro-16a-elaidoyloksy-lip-hydroksy-16a-metyl-3-oksoandrosta-l,4-dien-17p-karbotioat (1,1 g, 1,35 mmol) i acetonitril (65 ml) ble tilsatt sølvfluorid (1,7 g, 13,5 mmol) og rørt i mørket ved romtemperatur i 40 timer. Blandingen ble fortynnet med EtOAc og filtrert gjennom en kort celittplugg og silikagel. Filtratet ble vasket med vann, tørket (Na2S04), filtrert og fordampet under vakuum. 'H NMR av råproduktet avslørte at noe ureagert klorid var til stede, og de siste to trinnene ble gjentatt for fullstendig å omdanne kloridet til fluorid. Produktet ble deretter renset ved flashkromatografi på silikagel ved å eluere med heksan/EtOAc (3:1) til å gi 0,65 g (46 %, to trinn) av tittelforbindelsen som et fargeløst fast stoff.
Eksempler
Eksemplene nedenfor illustrerer den foreliggende oppfinnelsen. Disse eksemplene skal imidlertid ikke fortolkes som begrensninger av omfanget av den foreliggende oppfinnelsen.
Eksempel 1:
U937 og THP-1 cellelinjene ble sådd ut, 20 000 celler pr. brønn, i 96-brønnplater. 50 ul cellekulturmedium ble tilsatt til hver brønn. Samtidig ble testforbindelser tilsatt i 5 forskjellige konsentrasjoner og inkubert i 48 timer. CellTiter 96 ® ikke-radioaktivt celleproliferasjonsassay (Promega) ble benyttet for å studere cytotoksisiteten til testforbindelser i disse cellene. Dette assayet er en kolorimetrisk metode for bestemmelse av antallet levedyktige celler i proliferasjons- eller kjemosensitivitets-assays. Det er sammensatt av løsninger av en ny tetrazoliumforbindelse (3-(4,5-dimetyltiazol-2-yl)-5-(3-karboksymetoksyfenyl)-2-(4-sulfofenyl)-2H-tetrazolium, indre salt; MTS) og et elektronkoplingsreagens (fenazinmetosulfat; PMS). MTS bioreduseres av celler til et formazanprodukt som er løselig i vevskulturmedium. Absorbansen til formazan ved 490nm kan måles direkte fra 96 brønnassayplater uten ytterligere bearbeiding. Mengden av formazanprodukt som målt ved mengden av 490nm absorbans er direkte proporsjonal med antallet levende celler i kultur (IC50verdier). Testforbindelsene var flutikasonpropionat og flutikasonelaidinsyrederivat. I disse glukokortikoidresistente cellelinjene var flutikasonelaidinsyre svært potente med IC50verdier i mikromolarområdet, der U937 cellene var de mest følsomme.
Eksempel 2:
Apenyre COS-1 celler (ATCC CRL 1650) ble dyrket i Dulbecco's modifiserte Eagle's medium (Gibco BRL, Grand Island, NY) som beskrevet andre steder (9). Forbigående transfeksjoner av COS-1 celler ble utført som beskrevet andre steder (10). Cellene ble lagt ut i en tetthet på 2xl0<5>celler/brønn. Hver brønn fikk 5 u.g testplasmid, 5 u.g P-galaktosidase kontrollplasmid som en intern kontroll og enten 2 jag ekspresjon plasmid, pMT-hGR eller pGL3-basisk som bærer. Plasmiddetaljer: Muse PPARcc gen er beskrevet andre steder (11). Vektorer som uttrykket LUC reportergenet under kontrollen av PPARa 5'-side- og promoterregionen ble konstruert i pGL3-LUC vektoren (Promega). PPARa 5'-sidesekvensen mellom -2800bp og +100bp ble klonet til Nhel fordøyd pGL3-LUC for å generere PP AR (- 2800/+ 100) LUC plasmid. Hver transfeksjon ble utført i triplikat. Disse cellene ble transfektert 24 timer etter utbredning sammen med friskt medium som inneholder ligand. Etter 72 timer ble celler høstet, cytosolekstrakter ble fremstilt og LUC aktiviteter ble målt i henhold til Promega protokollen. Resultatene var normalisert mot P-galaktosidaseaktiviteter målt ved å inkubere 100 ul ekstrakt med 0,28 mg o-nitrofenyl-D-galaktosid (ONPG) i 50 mM fosfatbuffer pH 7,0,10 mM KC1,1 mM MgCl2i 30 min. ved 30°C og absorbans ble avlest ved 420 nm.
Testforbindelsene var flutikasonpropionat, flutikasonelaidinsyrederivat og 6a,9o> difluoro-17a-elaidoyloksy-113-hydroksy-16a-metyl-3-oksoandrosta-1,4-dien-17p-karboksylsyre. Steroidforbindelsen, flutikasonpropionat var i stand til å indusere reportergenaktivitet med 100%, mens flutikasonelaidinsyreesterderivat og 6a,9o> difluoro-17a-elaidoyloksy-l 1 p-hydroksy-16a-metyl-3-oksoandrosta-l ,4-dien-l 7P-karboksylsyre overraskende bare induserte reportergenaktivitet med hhv. 15% og 2%. Steroidanalogene var mye mindre potent ved genaktivering av gener som representerer bivirkninger.
Eksempel 3:
IL-lB-stimulering forårsaker en økning i granulocyt-makrofagkolonistimulerende faktor (GM-CSF) frigivelse etter 24 timer i A549 TRE (AP-1 regulering) celler. Inhibisjon av GM-CSF frigivelse måles etter 24 timers eksponering for en testforbindelse. Flutikasonelaidinsyreesterderivat frembrakte en konsentrasjonsavhengig inhibisjon av IL-lB-stimulert GM-CSF frigivelse i A549 TRE celler med en IC50= 1.4 x 10 "nM. Flutikasonpropionat frembrakte en konsentrasjonsavhengig inhibisjon av GM-CSF frigivelse i A549 TRE celler på IC503.1xl0"<10>M (Tabell 3). Flutikasonelaidinsyreester-derivatet var 22 ganger mer potent ved inhibisjon av GM-CSF frigivelsen, dvs. 22 ganger mer potent anti-inflammatorisk aktivitet ved dette assayet.
Induksjon av GRE-luciferaseaktivitet ble observert i stabile transfekterte A549 GRE-luciferaseceller.
