Description
Foreliggende oppfinnelse angår et preparat omfattende tramadol og et opioid.
US-PS 3 652 589 beskriver en klasse analgetiske cykloalkanol-substituerte fenolestere med en basisk aminogruppe i cykloalkylringen. Forbindelsen (1 RS, 2 RS) trans-2-[(di-metylamino)-metyl]-l-(3-metoksyfenyl)cykloheksanol, vanligvis kjent som tramadol, er spesielt beskrevet. En serie artikler som behandler forbindelsens farmakologi, toksikologi og kliniske studier av tramadol, er funnet i "Arzneim. Forsen" 28(1), 114 (1978). Driessen et al. beskriver i "Årch. Pharmacol.", 341, R104 (1990) at tramadol oppnår sin analgetiske virkning via en mekanisme som hverken er helt ut opioid-lignende og heller ikke helt ikke-opioid-lignende. (1990) beskriver at tramadolhydroklorid er et oralt aktivt rent agonist opioid analgetikum. Imidlertid antyder kliniske forsøk at tramadol mangler mange av de typiske sideeffektene til opioide agonister, for eksempel respiratorisk depresjon (W. Vogel et. al., "Arzneim. Opioider har i mange år vært benyttet som analgetikum for å behandle alvorlig smerte. Imidlertid gir de uønskede bivirkninger som gir bruksbegrensninger. For å redusere bivirkningsproblemet har opioider vært kombinert med andre ikke-opioide analgetiske midler for å redusere mengden opioid som var nødvendig for å fremstille ekvivalente analgetika. Den reduserte mengde av opioid reduserer generelt antallet og graden av bivirkninger. Det er krevet at noen av disse kombinasjonsprodukter også har fordelen av å gi en synergistisk analgetisk virkning. For eksempel beskriver A. Takemori i "Annals New York Acad. A. Pircio et. al., rapporterer i "Arch. Int. Pharmacodyn.", superadditive analgetika med en 1:125 blanding av butorfanol, nok et opioidanalgetikum, og acetaminofen, APAP, et ikke-opioidanalgetikum, mens en 1:10 blanding ikke viser noen statistisk signifikant super-additiv analgetisk virkning.
Imidlertid hverken foreslår eller beskriver den kjente teknikk at tramadol, et "atypisk" opioid analgetikum, kan eller skulle kombineres med et annet analgetikum, spesielt opioidanalgetium, for å redusere bivirkningene for hver av dem, eller for å gi et preparat som viser super-additiv analgetisk virkning.
Det er nu funnet at tramadol kan kombineres med visse opioider, kodein, oksykodon og hydrokodon, og derved gi en analgetisk virkning. Hvert av disse opioider er medlemmer av en klasse opioider som har den samme kjernestruktur, det vil si at hver har en 3-metoksysubstituent på den aromatiske del. Kombinasjonen benytter mindre mengder av både tramadol og opioid enn det som skulle være nødvendig for å gi den samme mengde analgetisk virkning hvis hver av dem ble benyttet alene. Ved å benytte mindre mengder av begge medikamenter blir bivirkningene forbundet med hver av forbindelsene redusert både i mengde og antall. Overraskende er preparatene omfattende tramadol og en hvilken som helst av kodein, oksykodon eller hydrokodon, funnet å vise synergistiske analgetiske virkninger når de kombineres i visse forhold. Preparatene ifølge oppfinnelsen kan også være brukbare ved å behandle tussiv-tilstander og diaré.
Oppfinnelsen vil forstås bedre under henvisning til de ledsagende tegninger der: Figur I er et isobologram som viser den analgetiske virkning av tramadolhydroklorid og kodeinfosfat-preparater på haleslag-latensen hos mus; og Figur II er et isobologram som viser den analgetiske virkning av tramadolhydroklorid og oksykodonhydroklorid-preparatet på haleslag-latensen hos mus.
Foreliggende oppfinnelse angår, som antydet ovenfor, et farmasøytisk preparat som karakteriseres ved at det omfatter tramadol og et opioid valgt blant kodein, oksykodon og hydrokodon.
Med tramadol menes ifølge oppfinnelsen er en hvilken som helst av (IR, 2R eller IS, 2S )-(«dimetylaminometyl )-l-(3-metoksyfenyl)-cykloheksanol (tramadol), men også de individuelle stereoisomerer, blandinger av stereoisomerer inkludert racematene, farmasøytisk akseptable salter av aminene som hydrokloridsaltet, solvater og polymorfer derav. Tramadol er kommersielt tilgjengelig fra Grunenthal eller kan fremstilles ved den prosess som er beskrevet i US-PS 3 652 589.
Kodein, oksykodon og hydrokodon omfatter også deres basiske former og farmasøytisk aksepterbare salter som fosfat, sulfat, hydroklorid og bitartrat. Fremstillingen av hver av kodein, oksykodon og hydrokodon er beskrevet av Goto et al. i "Proe. Japan Acad.", 30, 769 (1954), Freund et al. i "J. Prakt. Chem." , 94, 135 (1916 ) samt i US-PS 2 715 626.
Tramadol og det opioide materiale er generelt til stede i et vektforhold tramadol:opioid-materiale fra ca. 200:1 til 1:20.
Dette forhold varierer innen det beskrevne området avhengig av de spesielle komponenter i preparatet. Visse forhold innen dette området vil resultere i et preparat som viser synergistiske analgetiske virkninger, for eksempel i et preparat omfattende tramadol og kodein med et forhold mellom komponentene fra 20:1 til 1:20, helst fra 2:1 til 1:2 og aller helst fra ca. 2:1 til 1:1. I et preparat omfattende tramadol og oksykodon bør forholdet mellom komponentene være fra 200:1 til 1:1 og helst fra 20:1 til 2:1.
Farmasøytiske preparater omfattende tramadol og en hvilken som helst av kodein, oksykodon eller hydrokodon som aktive bestanddeler i grundig blanding med en farmasøytisk bærer kan fremstilles i henhold til konvensjonelle, farmasøytiske galeniske teknikker. Bæreren kan ha et vidt antall former avhengig av den ønskede preparatform for administrering, for eksempel intravenøs, oral eller parenteral. Preparatet kan også administreres ved hjelp av en aerosol. Ved fremstilling av preparatene i oraldoseform kan en hvilken som helst av de vanlige farmasøytiske media benyttes. Når det gjelder orale flytende preparater som suspensjoner, elexirer og oppløs-ninger, kan vann, glykoler, oljer, alkoholer, smaksmidler, preserveringsmidler og farvestoffer og lignende benyttes. Når det gjelder faste orale preparater som for eksempel pulvere, kapsler og tabletter, kan bærere som stivelser, sukkere, fortynningsmidler, granuleringsmidler, smøremidler, binde-midler, disintegreringsmidler og lignende, benyttes. På grunn av den enkle administrering representerer tabletter og kapsler den mest fordelaktige orale enhetsdoseform, i hvilket tilfelle selvfølgelig faste farmasøytiske bærere benyttes. Hvis ønskelig kan tabletter være sukkerbelagte eller enteriske belagte ved hjelp av standard-teknikker. For parenterale preparater vil bæreren vanligvis omfatte sterilt vann selv om andre bestanddeler kan innarbeides for å understøtte oppløselighet eller for preserveringsformål. Injiserbare suspensjoner kan også fremstilles, i hvilket tilfelle egnede flytende bærere, suspensjonsmidler og lignende kan benyttes. De farmasøytiske preparater vil generelt foreligge i form av en enhetsdose, for eksempel tablett, kapsel, pulver, injeksjon, teskje og lignende, i mengder fra 0,001 til 50 mg/kg og fortrinnsvis fra 0,003 til 25 mg/kg aktiv bestanddel.
De følgende eksperimentelle forsøk beskriver oppfinnelsen i større detalj og skal illustrere oppfinnelsen uten å begrense den.
Eksempel 1: Fremstilling av de kombinerte doser av tramadol
og kodein.
For hver av forholdene av de individuelle medikamenter som ble benyttet i medikamentkombinasjonene som ble prøvet, se tabell 1, ble det fremstilt lageroppløsninger som tilsvarte to ganger dosen av hvert medikament med en konsentrasjon uttrykt i 5 mg medikament pr. 5 ml destillert vann. Når det gjelder 1:20 tramadol:kodeinforholdet, ble 9,1 mg tramadolhydroklorid (= 8 mg tramadol som base) og 217,1 mg kodeinfosfat (= 160,0 mg kodein som fri base) hver oppløst i separate 5 ml ampuller med destillert vann for å gi individuelle forrådsoppløsninger. Like volumer av hver av forrådsoppløsningene ble så kombinert for å gi den endelige ønskede medikamentkombinasjon pr. 10 ml destillert vann. For eksempel ble 5 ml tramadol-forrådsoppløsning kombinert med 5 ml kodein-forrådsoppløsning for å gi l:20-dosen, altså 8 mg tramadol: 160 mg kodein, i 10 ml vann. Derefter ble 10 ml/kg av forrådsoppløsningen injisert i mus. Det henvises til tabell 1. Hvert forhold ble preparert separat på tilsvarende måte og injisert i et volum på 10 ml/kg pr. mus.
Eksempel 2: Fremstilling av de kombinerte doser av tramadol
og oksykodon.
Fremstillingen av forskjellige forhold av en tramadol:oksykodon-kombinasjon bevirkes ved å fremstille en forråds-oppløsning med en konsentrasjon, uttrykt i mg medikament pr. 10 ml oppløst vann. For eksempel ble 80 mg tramadol som fri base og 4 mg oksykodon som fri base oppløst i 10 ml vann for derved å gi forrådsoppløsningen (80 mg: 4 mg) tramadol:oksykodon-kombinasjon pr. 10 ml vann. Forrådsoppløsningen av medikamentkombinasjonen ble så fortynnet med en tilstrekkelig mengde destillert vann til å gi de lavere doser av medikamentkombinasjonen pr. 10 ml oppløst vann. For eksempel ble 5 ml av forråds 20:1 tramadol:oksykodon-kombinasjonen fortynnet med et likt volum destillert vann for å gi den lavere 20:1 dose, det vil si (40 mg:2 mg) kombinasjonen pr. 10 ml vann. Hvert forhold ble preparert på tilsvarende måte og injisert i et volum av 10 mg/kg pr. mus.
Eksempel 3: Fremstilling av de kombinerte doser av tramadol
og hydrokodon.
Fremstillingen av forskjellige forhold av en tramadol:hydrokodon-kombinasjon gjennomføres ved å fremstille en forråds-oppløsning med en konsentrasjon uttrykt i mg medikament pr.
10 ml oppløst vann. For eksempel ble 160 mg tramadol som fri base og 160 mg hydrokodon som fri base oppløst i 10 ml vann for derved å gi forrådsoppløsningen av (160 mg:160 mg) tramadol:hydrokodon-kombinasjon pr. 10 ml vann. Forråds-oppløsningen av medikamentkombinasjonen ble så fortynnet med en tilstrekkelig mengde fortynnet vann.til å gi de lavere doser av medikamentkombinasjonen pr. 10 ml oppløst vann. For eksempel ble 5 ml forråds 1:1 tramadol:hydrokodon-kombinasjon fortynnet med et likt volum destillert vann for derved å gi den lavere l:l-dose, det vil si 80 mg:80 mg-kombinasjon pr.
10 ml vann.
Eksempel 4: Analgetisk aktivitet.
CDl-hannmus med en vekt fra 18-24 g ble benyttet for å bestemme de analgetiske virkninger forbundet med preparatene ifølge oppfinnelsen. Noen mus ble dosert oralt med preparatene som fremstilt i eksempel 1 og som vist i tabell 1 og andre mus ble dosert oralt med preparatene som fremstilt i eksempel 2 og som vist i tabell 2. Doseringsvolumet var 10 ml/kg.
Den analgetiske aktivitet ble fastslått ved å benytte haleslagprøven som beskrevet av F.D. Amour et al. i "J. Pharmacol. Exp. Ther.", 72, 74 (1941). Prøven involverte å underkaste musehalen en fokusert varmestimulus og å regi-strere responsen, det vil si "slaget" eller fjerningen av halen fra stimuluskilden. Reaksjonstiden eller latensen for responsen ble først målt uten administrering av medikament-preparatet og så gjentatt 15 minutter efter administrering av preparatene ifølge oppfinnelsen der komponentene av preparatene er i de angitte forhold. Hver mus ble kun underkastet en for- og postadministreringsprøve.
Analysen av et eventuell superadditivitet for preparatene ved hvert gitte forhold ble bestemt som beskrevet av R.J. Tallarida et al. i' "Life Sei.", -45, 947 (1989). Denne prosedyre involverte bestemmelse av den totale meifgde av blandingen som er nødvendig for å gi et spesifisert effekt-nivå, for eksempel 50$ (EDsø-blanding), og den tilsvarende totale mengde som skulle ventes under enkel additivitet, ED5ø-add. Der det ble fastslått at ED5Q-mix mindre enn ED50<->add for et spesifikt gitt forhold, er preparatforholdet superadditivt. Både en mengde EDsQ-mix og EDsg-add var tilfeldige variabler; EDsQ-mix ble anslått fra dosisrespons-kurven for et spesifikt gitt forhold; EDsg-add ble oppnådd ved å kombinere ED5Q-estimatene for de to medikamenter under additivitet. ED5Q-mix ble så sammenlignet med ED5Q-add via en t-prøve. ED5Q-verdiene (95$ konfidensintervall) for tramadol alene var 76,8 (59,2-99,7) mg/kg. ED50-verdien (og 95$ konfidensintervallet) for kodeinfosfat alene var 71,3 (52,0-97,7) mg/kg, og for oksykodon var ED5Q-verdien 4,2 (3,0-5,8).
Interaksjonen mellom tramadol og kodein eller hydrokodon ble bestemt ved nøyaktige doseringsforhold av tramadolhydroklorid og kodein eller oksykodon. Flere, karakteristisk 4-6, kodede doser av hver valgte kombinasjon ble undersøkt med henblikk på analgetisk effektivitet efter 15 minutter ved bruk av en eksperimentell design som tillot fullstendig randomisering av de separate doseformer som ble prøvet.
Interaksjonen for tramadolhydroklorid og kodeinfosfat eller oksykodonsulfat på haleslaglatensen hos mus ble vist ved de data som er gitt i tabellene 1 og 2 og som vist i figurene I og II, Loewe-isobologrammer (se S. Loewe, "Pharm. Rev.", 9; 237 (1957) når det gjelder oppsetting og basis for et isobologram).
I figurene representerer den diagonale linje som forener ED5Q-verdiene for de to medikamenter gitt separat, den enkle additivitet av en blandings virkninger ved forskjellige komponentforhold. Den stiplede nærliggende linje til diagonallinjen definerer 95$ konfidensintervallet. ED5Q-verdier som faller under kurven (mellom linjen og origin) Indikerer superadditivitet, det vil si uventet økning av virkningene. Den diagonale stiplede linje fra origin representerer doseforholdet tramadolhydroklorid:kodeinfosfat
(figur I) eller tramadolhydroklorid:oksykodonsulfat (figur II) som benyttet i mus som mottok de kombinerte medikament-doseringer. Strekene gjennom EDsg-punktene for tramadol- og kodein-strek eller tramadol- og oksykodonpreparatene viser 95$ konfidensintervallene for EDsg-verdiene.
De forsøksdata som er vist i figur I viser at preparater med et forhold tramadolrkodein fra 1:20 til 20:1, representert ved den krumme linje, gir uventet øket aktivitet fordi ED5Q-mix er mindre enn EDsg-add. Forsøksresultatene som vist i figur II fastslår på samme måte at preparater med et forhold tramadol:oksykodon fra 200:1 til 1:1 også gir uventet forbedret aktivitet. Videre er bruken av hydrokodon i et preparat ifølge oppfinnelsen ventet å gi et preparat med tilsvarende aktivitet som en blanding omfattende oksykodon på grunn av de like aktivitetsprof iler og den like kjemiske struktur for hydrokodon og oksykodon.
Claims (7)
- Farmasøytisk preparat, karakterisert ved at det omfatter tramadol og et opioid valgt blant kodein, oksykodon og hydrokodon.
- 2. Farmasøytisk preparat ifølge krav 1, karakterisert ved at tramadol og opioidet foreligger i forholdet 1:20 til 20:1 for kodein og 200:1 til 1:1 foroksy-kodon og hydrokodon.
- 3. Farmasøytisk preparat ifølge krav 1, karakterisert ved at opioidet er kodein.
- 4 . Farmasøytisk preparat ifølge krav 3, karakterisert ved at tramadol foreligger som tramadolhydroklorid.
- 5 . Farmasøytisk preparat ifølge krav 4, karakterisert ved at tramadolhydrokloridet er racemisk.
- 6. Farmasøytisk preparat ifølge et hvilket som helst av kravene 1 til 5, karakterisert ved et vektforholdet tramadol:kodein er fra 1:2 til 2:1.
- 7. Farmasøytisk preparat ifølge krav 6, karakterisert ved et vektforhold fra "1:1 til 2:1. 8.Farmasøytisk preparat ifølge krav 1, karakterisert ved at opioidet er oksykodon. 9.Farmasøytisk preparat ifølge kravene 1 til 5 samt 8, karakterisert ved at vektforholdet tramadol:oksykodon er fra 20:1 til 1:1. 10.Farmasøytisk preparat ifølge krav 1, karakterisert ved at det videre omfatter en farmasøytisk akseptabel bærer.
