Description
Foreliggende oppfinnelse vedrører fremstilling av nye, terapeutisk aktive benzimidazolderivater som eksogent eller endogent inhiberer stimulert magesyresekresjon og således kan anvendes ved hindring og behandling av peptisk ulcus. I mer generell forstand kan disse forbindelsene anvendes for hindring og behandling av mage- og tarmlnflammatoriske sykdommer, og magesyrerelaterte sykdommer i pattedyr og mennesker, slik som gastritt, grastrisk ulcus, duodenal ulcus og refluks esofagitt. Videre kan forbindelsene anvendes for behandling av andre mage- og tarmforstyrrelser hvor gastrisk antisekretorisk effekt er ønskelig f.eks. i pasienter med gastrinomas, og i pasienter med akutt øvre mage- og tarm-blødning. De kan også benyttes i pasienter som er i situa-sjoner med intensiv pleie, og pre- og postoperativt for å hindre syreaspirasjon og stress-ulcerasjon. Farmasøytiske preparater inneholder minst en av de fremstilte forbindelsene som aktiv bestanddel.
Benzimidazolderivater beregnet for inhibering av magesyresekresjon er beskrevet i flere patenter, blant disse kan nevnes GB 1 500 043, GB 1 525 958, US 4 182 766, EP 0 005 129 og BE 890 024. Benzimidazolderivater foreslått for bruk i behandlingen eller hindringen av spesielle mage- og tarm-inflammatoriske sykdommer er beskrevet i EP-A-0 045 200. N—substituert-2-(pyridylalkylensulfinyl)benzimidazoler er beskrevet i EP-A-0 176 308.
For visse farmasøytiske bruksformer er det et stort behov for høy vannoppløselighet av forbindelsen som skal benyttes. F.eks., for intravenøse og intramuskulære injeksjons-formuleringer må dosen oppløses i et lite volum av et vandig medium. Dette krever naturligvis en høy vannoppløselighet. En høy vannoppløselighet er imidlertid ofte en stor fordel også
i andre tilfeller, f.eks. for orale formuleringer.
De tidligere kjente forbindelsene har generelt en lav vannoppløselighet hvilket ikke tillater fremstilling av slik sterk konsentrert vannoppløsning som er nødvendig for intravenøse og intramuskulære injeksjoner. Forbindelsene som er angitt i EP-A-0176308, som beskriver N-substituerte benzimidazolderivater, har f.eks. lav vannoppløselighet og er således ikke egnet for den ovennevnte parenterale bruk.
Det er funnet at forbindelsene med den nedenfor angitte formel I er effektive som inhibitorer for magesyresekresjon i pattedyr og menneske, og at nevnte forbindelser I viser en uventet høy oppløselighet i vann sammenlignet med forbindelser tilhørende den tidligere teknikk.
Forbindelsene med formel I viser generelt høyere kjemisk stabilitet i vannoppløsninger ved den pH-verdi hvor de viser optimal stabilitet sammenlignet med de tilsvarende forbindelsene uten N-l-substitusjonen, ved den samme pH-verdien. Noen av forbindelsene viser meget høy kjemisk stabilitet i oppløsning. Forbindelsene med formel I er derfor særlig egnet for parenteral, spesiell intravenøs og intramuskulær administrasjon. Den høye oppløseligheten og kjemiske stabiliteten gjør også forbindelsene ifølge oppfinnelsen egnet for andre administrasjonsveier, slik som f.eks. oral og rektal administrasjon.
Forbindelsene med formel I som er sulfoksyder, har et asymmetrisk senter i svovelatomet, dvs. disse forbindelsene eksisterer som to optiske isomerer (enantiomerer), eller dersom de også inneholder ett eller flere asymmetriske karbonatomer har forbindelsene to eller flere diastereomere former, som hver eksisterer i to enantiomere former.
Både de rene enantiomerene, racemiske blandinger (50$ av hver enantiomer) og ulike blandinger av de to, omfattes av foreliggende oppfinnelse. Det skal forstås at alle de diastereomere formene som er mulige (rene enantiomerer eller racemiske blandinger) omfattes av oppfinnelsen.
Det antas at forbindelsene med formel I metaboliseres før de utøver sin effekt. Slik metabolisme kan forekomme i N-substituenten i stilling 1 i benzimidazolkjernen.
De nye, terapeutisk aktive benzimidazolderivatene som tilveiebringes ifølge oppfinnelsen har den generelle formel:

Forbindelsene med formel I fremstilles ifølge foreliggende oppfinnelse ved omsetning av en forbindelse med formelen: hvor R<2> og R<3> har de ovenfor angitte betydninger, og Z er halogen slik som Cl eller en funksjonelt ekvivalent gruppe, med en forbindelse med formelen: hvor RP er en beskyttende gruppe, og M er et motion; hvoretter, etter eventuell nødvendig fjerning av beskyttende gruppe, den oppnådde forbindelse omdannes til dinatriumsaltet.
RP i formler VI og IX som er en beskyttende gruppe kan være cyanoetyl, benzyl eller p-nitrofenyl. Motionet M kan ha hentydningen Na<+>, K<+>, Ag<+> eller trialkylammonium. I det tilfellet hvor en beskyttende gruppe RP anvendes, så blir denne fjernet etter koblingsreaksjonen. Fjerningen av den beskyttende gruppen foretas ved metoder som er velkjent innen teknikken. Således kan f.eks. fosfatene beskyttes som dibenzyl- eller difenylestre, som kan spaltes ved basisk hydrolyse. Dibenzylestrene kan også spaltes med natriumjodid i aceton. Den cyanoetyl-beskyttende gruppen kan fjernes ved behandling med en base slik som NaOH.
Avhengig av prosessbetingelsene og utgangsmaterialene oppnås sluttproduktene med formel I enten i nøytral eller saltform. De oppnådde frie basene kan omdannes til dinatriumsaltene med organiske eller uorganiske syrer. I fremstillingen av disse kan det benyttes slike syrer er hydrohalogensyrer, sulfon-syre, fosforsyre, salpetersyre og perklorsyre; alifatiske, alicykliske, aromatiske eller heterocykliske karboksyl- eller sulfonsyrer, slik som maursyre, eddiksyre, propionsyre, ravsyre, glykolsyre, melkesyre, eplesyre, vinsyre, sitron-syre, askorbinsyre, maleinsyre, hydroksymaleinsyre, pyrodrue-syre, fenyleddiksyre, benzosyre, p-aminobenzosyre, p-hydroksybenzosyre, salicylsyre eller p-aminosalicylsyre, emboninsyre, metansulfonsyre, etansulfonsyre, hydroksyetan-sulfonsyre, etylensulfonsyre, halogenbenzensulfonsyre, toluensulfonsyre, naftylsulfonsyre eller sulfanilsyrer, metionin, tryptofan, lysin eller arginin.
Baseaddisjonssalter av dikarboksylsyre - eller fosforholdige forbindelser kan på tilsvarende måte omdannes til syreformen, og deretter omdannes til det terapeutiske dinatriumsaltet.
Oppnådde racemater kan separeres ifølge kjente metoder, f.eks. omkrystallisering fra et optisk aktivt oppløsnings-middel.
I tilfelle for diastereomere blandinger (racematblandinger) kan disse separeres til stereoisomere (diastereomere) rene racemater ved hjelp av kromatografi eller fraksjonert krystall isering.
Utgangsmaterialer med formel IV er nye og slike forbindelser hvor Z er Cl kan fremstilles ved omsetning av en forbindelse med formelen:
hvor R 2 og R<3> har de ovenfor angitte "betydninger og R<4> er hydrogen eller C^_4 alkyl, med et kloreringsmiddel slik som SOCI2 i nærvær av en egnet hase slik som trietylamin i et egnet oppløsningsmiddel slik som CH2CI2, toluen, acetonitril, tetrahydrofuran eller blandinger derav. For fremstilling av forbindelser med formel IV hvor Z er Br eller I anvendes anloge metoder som benytter egnede reagenser inneholdende brom, slik som BBr3, eller jod.
For klinisk bruk blir forbindelsene med formel, I formulert til farmasøytiske preparater for oral, rektal, parenteral eller annen administrasjon. Det er spesielt foretrukket å formulere forbindelsene til farmasøytiske preparater for parenteral administrasjon. Det farmasøytiske preparatet inneholder en forbindelse med formel I i kombinasjon med en farmasøytisk akseptabel bærer. Bæreren kan være i form av et fast, halvfast eller flytende fortynningsmiddel, eller kapsel. Vanligvis er mengden av aktive forbindelser mellom 0,1 og 95 vekt-# av preparatet, mellom 0,2 og 20 vekt-# i preparater for parenteral bruk og mellom 1 og 50 vekt-# i preparater for oral administrasjon.
Ved fremstilling av farmasøytiske preparater inneholdende en forbindelse med formel I i form av doseringsenheter for oral administrasjon kan den valgte forbindelse blandes med en fast, pulverformig bærer, slik som laktose, sakkarose, sorbitol, mannitol, stivelse, amylopektin, cellulosederivater, gelatin, eller en annen egnet bærer, samt med smøremidler slik som magnesiumstearat, kalsiumstearat, natriumstearylfumarat og polyetylenglykolvokser. Blandingen bearbeides deretter til granuler eller presses til tabletter. Granuler og tabletter inneholdende sulfoksydene kan belegges med et entérisk belegg som beskytter den aktive forbindelsen fra syrekatalysert nedbrytning så lenge doseringsformen forblir i magen. Det enteriske belegget velges blant farmasøytisk akseptable enterisk-beleggmaterialer, f.eks. bivoks, skjellakk eller anioniske filmdannende polymerer slik som celluloseacetatftalat, hydroksypropylmetylcellulose-ftalat, delvis metylforestrede metakrylsyrepolymerer og lignende, om foretrukket i kombinasjon med en egnet mykner. Til dette belegget kan forskjellige fargestoffer tilsettes for å sjeldne blant tabletter eller granuler med forskjellige aktive forbindelser eller med forskjellige mengder av tilstedeværende aktiv forbindelse.
Myke gelatinkapsler kan fremstilles med kapsler inneholdende en blanding av den aktive forbindelse, vegetabilsk olje, fett, eller annen egnet bærer for myke gelatinkapsler. Myke gelatinkapsler kan også være enterisk-belagte som beskrevet ovenfor. Harde gelatinkapsler kan inneholde granuler eller enterisk-belagte granuler av den aktive forbindelsen. Harde gelatinkapsler kan også inneholde den aktive forbindelsen i kombinasjon med en fast pulverformig bærer slik som laktose, sakkarose, sorbitol, mannitol, potetstivelse, amylopektin, cellulosederivater eller gelatin. De harde kapslene kan være enterisk-belagte som beskrevet ovenfor.
Doseringsenheter for rektal administrasjon kan fremstilles i form av suppositorier som inneholder den aktive substans blandet med en nøytral fettbase, eller de kan fremstilles i form av en rektal gelatinkapsel som inneholder den aktive substans i en blanding med en vegetabilsk olje, parafinolje eller annen egnet bærer for rektale gelatinkapsler, eller de kan fremstilles i form av et ferdiglaget mikrolavement, eller de kan fremstilles i form av et tørt mikrolavementpreparat for rekonsituering i et egnet oppløsningsmiddel like før administrasjon.
Flytende preparat for oral administrasjon kan fremstilles i form av siruper eller suspensjoner, f.eks. oppløsninger eller suspensjoner inneholdende 0,2-20 vekt-# av den aktive bestanddel og resten bestående av sukker eller sukker-alkoholer og en blanding av etanol, vann, glyserol, propylen-glykol og polyetylenglykol. Om ønsket, kan slike flytende preparater inneholde fargestoffer, smaksstoffer, sakkarin og karboksymetylcellulose eller andre fortykningsmidler. Flytende preparater for oral administrasjon kan også fremstilles i form av et tørt pulver for rekonstituering med et egnet oppløsningsmiddel før bruk.
Oppløsninger for parenteral administrasjon kan fremstilles som en oppløsning av en forbindelse med formel I i et farmasøytisk akseptabelt oppløsningsmiddel, fortrinnsvis i en konsentrasjon på 0,1-10 vekt-#. Disse oppløsningene kan også inneholde stabiliseringsmidler og/eller buffermidler og kan fremstilles i forskjellige enhetsdoseampuller eller -flasker. Oppløsninger for parenteral administrasjon kan også fremstilles som et tørt preparat for rekonstituering med et egnet oppløsningsmiddel etter bestilling før bruk.
Den typiske daglige dose av den aktive substans varierer innen et vidt område og vil avhenge av forskjellige faktorer slik som f.eks. det individuelle krav for hver pasient, administrasjonsveien og sykdommen. Generelt vil orale og parenterale doser være i området 5-500 mg pr. dag aktiv substans.
Oppfinnelsen illustreres av følgende eksempler.
Eksempler 1 og 2
a) Fremstilling av en blanding av fosforsyre, [6-metoksy-[2-[ [ ( 4-metoksy-3 , 5-dimetyl-2-pyridinyl )metyl] sul f inyl] -1H-benzimidazol-l-yl]metylester, dinatriumsalt og fosforsyre, [5-metoksy-[2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl)metyl]-sulfinyl]-lH-benzimidazol-l-yl]metylester, dinatriumsalt, 2:1
Tributylamin (14 ml, 0,059 mol) ble tilsatt under omrøring til en oppløsning av fosforsyre, 85% (2 ml, 0,030 mol) i etanol (10 ml). Oppløsningsmidlet ble inndampet og resten opptatt i metylenklorid (25 ml). Den organiske fasen ble tørket over natriumsulfat, filtrert og inndampet. En blanding av l-klormetyl-6-metoksy-2-[[(4-metoksy-3,5-dimetyl-2-pyridi-nyl)metyl]sulfinyl]-lH-benzimidazol og l-klormetyl-5-metoksy-2-[[(4-metoksy-3, 5-dimetyl-2-pyridinyl )metyl] sulf inyl] -1H-benzimidazol, 2:1 (1,0 g, 0,0025 mol) og tributylammonium-saltet av fosforsyre, fremstilt ovenfor, ble oppløst i metylenklorid (50 ml). Metylenkloridet ble avdestillert og resten ble oppvarmet på et vannbad i 5 minutter ved 60° C. Resten ble oppløst i metylenklorid (50 ml) og igjen ble metylenkloridet avdestillert og den oljeaktige produkt-blandingen ble oppvarmet på et vannbad i 5 minutter ved 60°C. Denne metoden ble gjentatt fire ganger inntil reaksjonen var fullført. Resten ble oppløst i metylenklorid og vasket med tre porsjoner vann. Det organiske laget ble tørket, filtrert og inndampet. Den urene blandingen av tributylammoniumsalter ble oppløst i metylenklorid (25 ml) og vann (10 ml) ble tilsatt. En oppløsning av natriumhydroksyd ble tilsatt under omrøring mens pH-verdien til det vandige laget ble nøye regulert. Da pH-verdien nådde 9,0-9,5 i den vandige fasen (0,005 mol NaOH tilsatt) ble blandingen sentrifugert. Det vandige laget ble vasket med tre porsjoner (25 ml) metylenklorid og deretter frysetørket til dannelse av 760 mg, 61$ av tittelforbindelsene. NMR-spektrum av blandingen var i overensstemmelse med spektraene for de rene isomerene gitt i tabell 1.
b) Fremstilling av fosforsyre, [5-metoksy-[2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl)metyl]sulfinyl]-lH-benzimidazol-1-vllImetylester. dinatriumsalt
Fra blandingen av tributylammoniumsalter fremstilt i trinn a) ovenfor ble fosforsyre, [5-metoksy-[2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl )metyl] sul f inyl] -lH-benzimidazol-1-yl]]metylester, tributylammoniumsalt oppnådd gjennom kromatografi på en kolonne med reversert fase, eluert med 15$ acetonitril i vann. Etter frysetørking ble forbindelsen oppløst i metylenklorid og syren behandlet med natriumhydroksyd som beskrevet ovenfor. Rent fosforsyre, [5-metoksy-
[ 2 - [ [ ( 4 -me tok sy-3 , 5-dimetyl-2-pyridinyl )metyl] sulf inyl] -1H-benzimidazol-l-yl]]metylester, dinatriumsalt ble oppnådd i et utbytte på ca. 1$. NMR-spektrum i tabell 1.
c) Fremstilling av fosforsyre, [6-metoksy-[2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl)metyl]sulfinyl]-lH-benzimidazol-1-yll] metylester. dinatriumsalt
Tittelforbindelsen ble fremstilt som beskrevet i trinn a) ovenfor for den isomere blandingen, med utgangspunkt i ren 1-kl ormetyl-6-metoksy-2-[[(4-metoksy-3,5-dimetyl-2-pyri-dinyl)metyl]sulfinyl]-lH-benzimidazol (1,9 g, 0,0048 mol), fosforsyre, 85$ (4 ml, 0,060 mol), tributylamin (28 ml, 0,118 mol) og 100 ml metylenklorid. Produktet ble krystallisert fra en etanol-vann-blanding hvilket 0,85 g, 35 timer. NMR-spektrum av forbindelsen var identisk med spekteret gitt i tabell 1.
F. jerning av beskyttende gruppe
Til resten fra fremstillingen av fosforsyre, cyanoetyl-6-metoksy-[2-[[(4-metoksy-3 , 5-dimetyl-2-pyr idinyl)metyl]-sulfinyl]-lE-benzimidazol-l-yl]metyldiester, trietylammonium-salt ble det tilsatt en oppløsning av NaOH (0,30 g, 0,0075 mol) oppløst i vann (25 ml). Etter oppvarming av blandingen på et vannbad (60°C) i 15 minutter ble den vasket to ganger med metylenklorid (15 ml) og inndampet til tørrhet, hvilket ga den ønskede forbindelsen (0,25 g). Identiteten til produktet ble bekreftet med NMR, utbytte 22%.
Fremstilling av mellomprodukter
Eksempler I og II
l-klormetyl-(5-metoksy) og (6-metoksy)-2-[[(4-metoksy-3,5-dimetyl- 2- pyridinyl) metyl1sulfinyll- lH- benzimidazol l-hydroksymetyl-(5-metoksy) og -(6-metoksy)-2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl)metyl]sulfinyl]-lH-benzimidazol (30 g, 0,08 mol, forhold 1:2) ble oppløst i toluen (500 ml) og oppløsningen ble avkjølt til -30°C. En oppløsning av tionyl-klorid (19 g, 0,16 mol) i toluen (100 ml) ble tilsatt dråpevis ved —30°C og blandingen ble omrørt i 10 minutter ved -30°C. Deretter ble en oppløsning av trietylamin (45 g, 0,45 mol) i toluen (200 ml) tilsatt dråpevis. Etter tilsetningen ble temperaturen hevet og blandingen omrørt ved romtemperatur i 30 minutter. Blandingen ble inndampet og resten (120 g) ble kromatografert på silisiumdioksydgel (etylacetat-metylenklorid 50-50) hvilket ga tittelforbindelsen som en isomer blanding (forhold 1:3). Utbytte 11,9 g.

Den rene l-klormetyl-6-metoksy-2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl)metyl]sulfinyl]-lE-benzimidazolforbindelsen kunne om ønsket oppnås når den isomere blanding ble krystallisert i acetonitril.

Fremstilling av fosforsyre, cyanoetyl-[6-metoksy-[2-[[(4-metoksy-3,5-dimetyl-2-pyridinyl)metyl]sulfinyl]-lH-benzimidazol- l- yl]] metyldiester. trietvlammoniumsalt 1 - klormetyl-6-metoksy-2-[[(4-metoksy-3,5-dimetyl-2-pyri-dinyl)metyl]sulfinyl]-lH-benzimidazol (0,90 g, 0,0023 mol) ble tilsatt under omrøring til en oppløsning av mono-trietylammoniumsalt av fosforsyrecyanoetylester (0,70 g, 0,0028 mol) og trietylamin (0,65 g, 0,0064 mol) i metylenklorid (20 ml). Blandingen ble tilbakeløpskokt natten over. Metylenkloridet ble avdestillert og resten ble oppvarmet på et vannbad i 10 minutter ved 60°C og deretter benyttet uten ytterligere rensing i den etterfølgende reaksjonen.
Isomere former
Den isomere identiteten til den rene eksempelforbindelsen 1 ble fastsatt ved bruk av NOE NMR-teknikker på dens syntetiske forløper i eksempel II. Alle de andre benzimidazolisomerene har blitt strukturelt bestemt ved å relatere deres NMR-spektra til det for eksempel 1.
Fremstillingen av forløperene i eksempler I og II resulterer i dannelsen av en isomer blanding i forholdet 1:2. Som en følge av dette, dannes målf orbindelsene nr. 2 og 1 som isomerer i det samme forholdet 1:2. I de andre eksemplene er de isomere forhold noen ganger forskjellige fra 1:2 og er avhengige av substituentene, spesielt substituentene på benzimidazolkjernen.
NMR-data for eksempler 1 og 2 er angitt i nedenstående tabell 1.
Biologiske tester
Inhiberende effekt in vivo på magesyresekresjon i bevisst hund
Testmetode
Hunder med kronisk gastrisk fistel ble benyttet. Disse hundene har fått operert inn en gastrisk kanyle i magen og en duodenal fistel, benyttet for direkte intraduodenal administrasjon av testforbindelser. Etter en 4 ukers restitusjons-periode etter kirurgi, ble tester foretatt en gang i uken på hver hund. Mat og vann ble fjernet 18 timer før hver test.
Magesyresekresjon ble indusert ved en kontinuerlig 4 timers infusjon av histamin ved individuelle doser (400-600 nmol/kg*t, iv)k resulterende i omtrent 90% av maksimal sekresjon av magesyre. Etter 1 times histaminstimulering ble testforbindelser eller bærer administrert, enten iv eller im (bærer = 0,5$ saltoppløsning) eller id (bærer = 0,5$ Methocel®, 90 HG 15000, Dow Chem Corp.) via et kateter gjennom duodenalfistelen. Magesaften ble oppsamlet ved fri strømning fra den gastriske kanylen i etter hverandre følgende 30 min. prøver under varigheten av histaminin-fusjonen. Prøvene ble titrert til pH 7,0 med 0,1M eller 1,0M NaOH ved bruk av en automatisk titreringsanordning og syreutgangen ble beregnet. Syreutgangen i løpet av perioden etter administrasjon av testforbindelse eller bærer ble uttrykt som fraksjonen av utgangen i løpet av perioden forut for disse adminstrasjonene, og den prosentvise inhibering av syresekresjon ble beregnet ved sammenligning i hver hund av de fraksjonale reaksjoner indusert av testforbindelsen til de tilsvarende bærer-induserte fraksjonale reaksjoner.
Testresultatene gitt i tabell 2 nedenfor representerer inhibering i løpet av den andre timen etter dose.
Oppløselighet
Oppløseligheten i vann ved romtemperatur for forbindelsene ifølge eksempel 1 (dinatriumsalt) ble målt og ble vist å være høyere enn 60 mg pr. ml. Ved sammenligning har natriumsaltet av omeprazol, 5-metoksy-2-[[(4-metoksy-3,5-dimetyl-pyridi-nyl)metyl]sulfinyl]-lH-benzimidazol, en oppløselighet på 0,4 mg pr. ml ved pH 9, en pH-verdi som ansees egnet for i. v. administrasjon.
K. iemisk stabilitet
Den kjemiske stabiliteten til forskjellige forbindelser fremstilt ifølge oppfinnelsen har blitt fulgt kinetisk ved lav konsentrasjon ved 37 'C i vandig bufferoppløsning ved forskjellige pH-verdier. Resultatene vist i tabell 3 nedenfor illustrerer at forbindelsene med formel I har en høy kjemisk stabilitet.

- Analogifremgangsmåte for fremstilling av terapeutisk aktive benzimidazolderivater med formelen:hvor R<2> og R<3> er forskjellige og er hydrogen eller metoksy, karakterisert vedomsetning av en forbindelse med formelen: hvor R<2> og R<3> har de ovenfor angitte betydninger, og Z er halogen slik som Cl eller en funksjonelt ekvivalent gruppe, med en forbindelse med formelen:hvor RP er en beskyttende gruppe, og M er et motion; hvoretter, etter eventuell nødvendig fjerning av beskyttende gruppe, den oppnådde forbindelse omdannes til dinatriumsaltet.
