Description
Foreliggende oppfinnelse vedrører en fremgangsmåte for fremstilling av sårhelingsmidler, nærmere bestemt angiogene og vekstfaktorer.
Angiogenese, som er celledelingen og den rettede veksten av kapillært endothel, sammen med fibroplasia og kollagensyntese er viktige komponenter av en vertsrespons på sårdannelse. Aktiveringen av plater og koaguleringskaskaden er blandt de første reaksjonene på skade.
Plater aktivert ved trombin frigir et mitogen, eller en vekstfaktor, for fibroblaster og glattmuskelceller og stimulerer forøket kollagensyntese ved glattmuskelceller in vitro. Mitogenet, (plate-avledet vekstfaktor, heretter betegnet PDGF) består av to polypeptider. En artikkel som beskriver PDGF ble publisert i 1982 av G. R. Grotendorst, T. Chang, H. E: J: Seppa, H. K. Kleinman og G. R. Martin i Journal of Cellular Physiology med tittelen "Platelet-Derived Growth Factor is a Chemoattractant for Vascular Smooth Muscle Celles", bind 113, side 261-266. Et ikke-mitogent stoff, kalt angiogenfaktor, fremstilles også ved trombinaktiverte plater og stimulerer kapillarvekst. Forskjellige angiogenesefaktorer er kjente innbefattende tumor, netthinne og sårfluidangiogenesefaktorer. Det er ukjent om alle angiogenesefaktorer har en felles virkningsme-kanisme på kapillærendothelceller.
Angiogenesefaktorer ble isolert og beskrevet av M. S. Banda, D. R. Knighton, T. K. Hunt og Z. Werb i Proe. Nat'l. Acad. Sei. U.S.A. (7773-7777, des. 1982) i en artikkel med tittelen "Isolation of a nonmitogenic angiogenesis factor from wound fluid", denne beskrivelsen er innbefattet heri som referanse.
Angiogenese og plateavledede vekstfaktorer er beskrevet av D. R. Knighton, T. K. Hunt, K. K. Thakral og W. H. Goodson III, i "Role of Platelets and Fibrin in the Healing Sequence", Annals of Surgery 196: 379-388 (1982), denne beskrivelsen er innbefattet heri som referanse.
I denne artikkelen beskrives den vellykkede behandlingen av et ikke-helende sår hos en pasient hvorved det ble gitt en enkelt, ti-enhetsplatetransfusjon. Såret helet i løpet av tre uker.
En senere undersøkelse har vist at når kroppens normale helingsprosess fungerer har den et effektivitetsnivå på bare ca. 50$.
En human angiogenfaktor fremstilt fra fibroblaster fra humanforhud er beskrevet i U.S. patent nr. 4,273,871, Tolbert et al. En offentlig tilgjengelig forhudfibroblastcellelinje er benyttet for å fremstille en angiogenfaktor.
I Antoniades's U.S. patent nr. 4,479,896, som er innbefattet heri som referanse, er plate-avledede vekstfaktorer karakterisert og ekstrahert for studium ved hjelp av gelelektro-forese.
Trombinaktiverte plater har evnen til å stimulere angiogenese, øke kollagensyntesen og celledelingen og veksten. Det er funnet at prøver av heltblod kan benyttes til å fremstille plate-anriket plasma, som når det aktiveres ved trombin, inneholder angiogenfaktor og vekstfaktor som kan benytttes til å akselerere helingsprosessen for sår.
Fremgangsmåten ifølge oppfinneslen for fremstilling av fysiologisk aktive sårhelingsstoffer, for anvendelse ved lokal påføring, som er kjennetegnet ved at den innbefatter at en blodprøve;
a) eventuelt, tilsettes et antikoagulasjonsmiddel av typen som benyttes for å forhindre koagulering inne i et legeme; b) eventuelt, blandes med en citratfosfat-dekstroseopp-løsning; c) et platerikt plasma med en konsentrasjon på minst 1.000.000 plater pr. milliliter blod isoleres fra blodprøven; d) platene aktiveres, fortrinnsvis med trombin, for eksempel med 1-10 enheter trombin pr. milliliter platerikt plasma; e) eventuelt det aktiverte, platerike plasmaet blandes med en mikrokrystallinsk kollagenbærer.
Fremgangsmåten er videre kjennetegnet ved at det platerike plasmaet under pkt. e) (heretter PRP) isomeres fortrinnsvis igjen inntil en høy konsentrasjon av plater oppnås. Platene plasseres deretter i en platebuffer. Konsentrasjonen av plater bør være minst 1.000.000 plater pr. milliliter. Fortrinnsvis bør konsentrasjonen være av størrelsesorden 1.000.000.000 plater pr. milliliter.
Trombin tilsettes til PRP for å aktivere platene. Fortrinnsvis anvendes 1 til 10 enheter trombin pr. milliliter PRP. De trombin-aktiverte platene frigir plate-avledede vekstfaktorer (heretter PDGF) og plate-avledede angiogenesefaktorer (heretter PDAF). Platene og trombin får inkubere ved romtemperatur i 5 til 10 minutter.
Det aktiverte PRP som inneholder PDGF og PDAF tilsettes fortrinnsvis til et biologisk kompatibelt, makromolekylært stoff som virker som en bærer. Først sentrifugeres platene ved ca. 950 x g, og den platefrie supernatanten blandes med bæreren. Fortrinnsvis anvendes et mikrokrystallinsk kollagen så som "Avitene", typen kollagen, markedsført av FMC Corp., Avicel Dept., Marcus Hook, PA 19061, som den biologisk kompatible bæreren. Mikrokrystallinske kollagener er biologisk kompatible i legemet. Nok bærer tilsettes til å suge opp alt det platerike plasmaet som oppnås fra blodet. F.eks. krever en blodprøve på 40 ml typisk ca. 25 ml bærer etter anrikning. Pastaen som derved oppnås lagres fortrinnsvis på is eller i kjøleskap.
De farmasøytiske preparatene for anvendelse som sårforbindinger som selges av Pharmacia Fine Chemicals, Inc., Piscataway, New Jersey, under betegnelsen "Debrisan" er en egnet bærer.
Det aktiverte PRP i bæreren kan deretter påføres på et sår. Det sterkt anrikede og aktive PDGF og PDAF assisterer derved helingen ved celledeling og rettet vekst av kapillært endothel, ved dobling av hastigheten for kollagensyntese og ved å produsere leukosytt-kemotaksis. Mitogenaktivitet resulterer i celledeling og vekst for å erstatte det tapte vevet.
Daglig påføring av det aktiverte PRP på sår stimulerer og understøtter helningsprosessen. Mengden PRP som bearbeides fra 40 ml blod er nok til å produsere påføringer for syv dager. Materialet plasseres over hele såret i relativt uniform tykkelse, ca. 2 mm tykt. Granulering, kontraksjon og epitilisering kan initieres ved anvendelsen av aktivert PRP i tilfeller hvor kroppens egne reparasjonssignaler er utilstre-kkelige til å stimulere god heling.
Når trombin benyttes refereres det til trombin som et biologisk frigivelsesmiddel for platefrigjøring. Andre biologiske frigivelsesmidler kjent innen teknikken, innbefattende kollagen, ADP og serotonin, kan benyttes istedenfor, eller i tillegg til, trombin for å aktivere platene, selv om trombin er foretrukket.
Blod oppnådd fra individet som skal behandles med sarhelings-faktorene fremstilt ifølge oppfinnelsen, stabiliseres i silikonbelagte rør inneholdende syre-citratdekstrose (0,15 M citrat, 2% glukose, pH 4,2) (heretter betegnet CDP) og sentrifugeres for å separere ut det platerike plasmaet. 40 til 60 ml blod blandet med 4 - 6 ml CDP sentrifugeres deretter ved ca. 135 x g i 20 min ved ca. 4°C for å oppnå platerikt plasma. Det platerike plasmaet fjernes og plasseres i et annet sterilt rør på 50 ml. En platetelling foretas deretter. CPD benyttes for å forhindre aktivering av koaguleringssekvensen ved kontakt mellom blodet og plasten i sprøyten. CPD er til stede i sprøyten mens blod tas fra pasienten. Blodet blandes kontinuerlig med CPD for å forhindre koagulering. Det platerike plasmaet i røret sentrifugeres deretter ved 750 x g i 10 minutter ved 4°C.
Det platefrie plasmaet fjernes og avhendes. Platepelleten resuspenderes i en mengde platebuffer slik at det dannes et antall ml. En lavere konsentrasjon på ca. 1.000.000 plater pr. ml er nyttig, men mindre foretrukket. Platebufferen som benyttes inneholder 0,05 M HEPES (N-2-hydroksyetylpiperazin-n-2-etansulfonsyre), 0,03 M glukose, 0,004 M KC1, 0,1 M NaCl og ca. 0,35$ humant serumalbumin innstilt på en pE på ca. 6,5. En prøve nedfryses ved ca. -20"C for senere undersøkelse av mitogenaktivitet. en annen prøve stripes på blodagar som et sterilitetsforsøk.
Det platerike plasmaet er den eneste blodfraksjonen som benyttes i fremgangsmåtene og preparatene ifølge oppfinnelsen. PRP'et aktiveres deretter med renset trombin ved et omfang på 1 til 10 enheter trombin pr. ml PRP. Fortrinnsvis anvendes ca. 1 enhet trombin pr. ml platerikt plasma. Aktiviteten av trombinet koagulerer fibrinogenet og aktiverer plater hvilket forårsaker at de frigir a-korn inneholdende plate-avledet vekstfaktor og plate-avledet angiogenesefaktor. Trombinet som ble benyttet var "Thrombinar" fra Armour Pharmaceutical Co. of Kankakee, Illinois. Platene og trombin får inkubere ved romtemperatur i 5-10 minutter.
PRP'et underkastes deretter en fjernelse av plater og fibrin ved sentrifugering. Den resulterende supernatanten inneholder både PDAF og PDGF etter sentrifugering ved 950 x g i 5 minutter ved 4°C. Pelleten kastes idet PDAF og PDGF er ekstrahert inn i supernatanten. PDGF er isolert og karakterisert. Det er et protein av molekylvekt 30.000 som brytes ned i to spesies med molekylvekt på 15.000 og 14.000.
For å påføre PDAF'et og PDGF'et i den platefrie supernatanten som dermed er oppnådd på et sår er det ønskelig å benytte et bærerstoff som er biologisk kompatibelt og virker som et temporært "depot". Et makromolekylært stoff så som mikrokrystallinsk kollagen utgjør en egnet bærer. En spesielt foretrukket bærer er "Avitene"-type mikrokrystallinsk kollagen fra FMC Corp., Avicel Dept., Marcus Hook, PA 19061. Det resulterende preparatet er tykkere og vil vise tendens til å forbli i posisjon i kontakt med såret. "Debrisan" sårforbindinger som inneholder "Sepharose"-perler, varemerker for Pharmacia Fine Chemicals, Inc., Piscataway, New Jersey, kan benyttes som en alternativ bærer. Fortrinnsvis anvendes 8-10 ml supernatant pr. gram bærer for å fremstille en pasta.
Påføring av sårbehandlingspreparatet foregår ved fysikalsk påføring av materialet over og inn i såret som ved påføring av en medisinsk salve. Behandlinger bør gjentas på en daglig basis så lenge som såret forblir åpent. En foretrukket behandling er å påføre et ca. 1 mm tykt lag av platefak-tor/bærerkomplekset på såret om morgenen. Det forbindes deretter med en steril, tørr forbinding. Om kvelden fjernes forbindingen og stoffet fjernes ved vasking med steril saltvannsoppløsning.
Selv om den kliniske utprøvingen innbefattende sarbehandlings-prepatatene fremstilt ifølge oppfinnelsen har vært rettet mot sar på utsiden av kroppen, kan preparatene også benyttes til å behandle indre sår. Suturer kan impregneres med sårbehand-lingspreparatene for å akselrere indre heling. Sårbehand-lingspreparatene kan også benyttes sammen med bionedbrytbare forbindinger, som et belegg over implanterbare innretninger og bionedbrytbare innretninger som benyttes ved kirurgiske fremgangsmåter. Generelt kan et hvilket som helst fremmed legeme som skal innføres i en pasient belegges med preparatet for å akselrere helingsprosessen. Alternativt kan preparatet påføres direkte over det skadede vevet.
Innledende kliniske forsøk har vært utført på åtte pasienter, alle med ikke-helende sår i et tidsrom fra 1 til 5 år. Alle pasienter hadde fått maksimal standardisert pleie i forsøk på å hele sårene. Denne behandlingen hadde mislykkes. I alle tilfellene initierte administrering av plate-avledede faktorer en helingsrespons som fremgikk ved granulasjons-vevdannelse (granulasjonsvev inneholder fibroblaster, endothelceller og kollagen). Sårene lukket seg ved kontraksjon og epitilisering eller ved hudoverføring. Stimulering av heling og eventuell gjenoppbygning fant sted i alle tilfellene.
Selv om det er foretrukket å fremstille aktivert PRP for sårbehandlingsformål direkte fra det skadede dyrets eget blod, kan fordelene med foreliggende oppfinnelse oppnås ved å anvende blod eller eldre plater fra dyr av det samme spesies. Anvendelse av blod fra det skadede individet som skal behandles er spesielt foretrukket siden man unngår eksponer-ing mot mulig hepatitt eller andre forurensninger fra lagret blod. Anvendelse av en pasients eget blod vil også eliminere eventuelle mulige allergiske reaksjoner. En tilsvarende kilde for materialet kan oppnås fra vaskede, eldre humane plater. Stoffene kan også anvendes innenfor veterinærmedisin ved å anvede plater avledet fra dyret selv eller et annet dyr tilhørende samme spesies.
Eksempel I
En pasient som hadde et åpent sår på venstre fot etter debridement av dødt vev og transmetatarsalamputering ble gitt PDGF og PDAF oppnådd som beskrevet ovenfor fra hans eget blod. Etter behandlingen ble såret fylt med nytt granulasjonsvev. Et etterfølgende debridement viste fullstendig dekkede metatarsalben og kontraktur av sårstørrelsen.
Eksempel II
En pasient gjennomgikk amputasjon av sin høyre storetå og ble behandlet med standard behandling i tre uker uten at granulasjonsvev akkumulerte i såret. Han fikk deretter platefaktorbehandling Etter tre ukers behandling hadde såret trukket seg sammen ca. 30-40$ og helingen var rask.
Eksempel III
En pasient som hadde to store sår på den mediale og laterale delen av den transmetatarsale amputasjonsstumpen, var behandlet i fire måneder uten heling ved konvensjonell behandling. I løpet av to ukers behandling med PDAF og PDGF som beskrevet ovenfor, var såret fritt for en synlig infeksjon og begynte å produsere granulasjonsvev.
Trettiåtte ikke-helende ulcuser hos 28 diabetiske pasienter ble behandlet med PRP-pastaen. Den gjennomsnittlige varig-heten av ulcusene før behandling var 6 1/2 år. En pasta fremstilt fra PRP med en konsentrasjon på ca. 10^ plater/ml ble blandet med "Avitene"-type kollagen. Pasientene påførte den PDGF- og PDAF-holdige pastasen daglig i 12 timers tidsrom i gjennomsnittlig åtte uker. Hver dag ble dødt vev fjernet fra sårene. Alle sårene produserte granulasjonsvev og lukket seg i et omfang på gjennomsnittlig 83$ sammenlignet med det opprinnelige sårarealet. 95$ av ulcusene var vellykket behandlet, hvilket resulterte i enten fullstendig sårepitili-sering eller vellykket hudoverføring. Bare to av disse ikke-helende sårene helet ikke. De helede ulcusene forble lukkede uten noen tegn på hypertropisk arrdannelse eller neoplasma-dannelse.
Ved vurdering av foreliggende oppfinnelse bør det holdes i minne at beskrivelsen utelukkende er illustrerende, og at omfanget av oppfinnelsen er bestemt av de følgende kravene.
Claims (4)
1.
Fremgangsmåte for fremstilling av fysiologisk aktive sårhelingsstoffer, for anvendelse ved lokal påføring, karakterisert ved at den innbefatter at en blodprøve; a) eventuelt, tilsettes et antikoagulasjonsmiddel av typen som benyttes for å forhindre koagulering inne i et legeme; b) eventuelt, blandes med en citratfosfat-dekstroseopp-løsning; c) et platerikt plasma med en konsentrasjon på minst 1.000.000 plater pr. milliliter blod isoleres fra blodprøven; d) platene aktiveres, fortrinnsvis med trombin, for eksempel med 1-10 enheter trombin pr. milliliter platerikt plasma; e) eventuelt det aktiverte, platerike plasmaet blandes med en mikrokrystallinsk kollagenbærer.

Fremgangsmåte ifølge krav 1, karakterisert ved at den platerike plasmafraksjonen under pkt. c) isoleres i en konsentrasjon på ca. 1.000.000.000 pr. milliliter og at det under pkt. d) fortrinnsvis tilsettes 1 enhet trombin pr. milliliter platerikt plasma.

Fremgangsmåte ifølge krav 1, karakterisert ved at det oppnås en prøve av plater hvoretter et platerikt plasma isoleres fra eldre plater og platene vaskes for å fjerne serumholdig materiale før platene underkastes aktivering ifølge trinn d).

Fremgangsmåte ifølge krav 3, karakterisert ved at platene oppnås fra eldre plater i en blodbank.
