Abstract
Fremgangsmåte for å oppioseliggjare hydrofobe organiske forbindelser, spesielt terapeutisk aktive forbindelser, i en vandig fase. Fremgangsmåten går ut på at man forst oppløser den hydrofobe forbindelse i én eller flere polyethylenglycoler med molekylvekt mellom 300 og 700 og deretter tilsetter denne oppløsning en vandig oppløsning med en fastsatt pH-verdi.De resulterende vandige oppløsninger er anvende-lige for fremstilling av medisiner for parenteral administrering, administrering gjennom fordøyeisesapparatet eller administrering gjennom slimhinner eller gjennom huden.
Description
Oppfinnelsen angår en ny fremgangsmåte for å oppløse-liggjøre en hydrofob, terapeutisk aktiv, organisk forbindelse, spesielt en N-acyl-indol.
Tidligere undersøkelser av Mack (J. Pharm. Sei. 5_2_
( 1963) 694, av Swarbrick (J. Pharm. Sei. 5_4 (1965) 1229)
og av N.E. Webb (Bull Parenteral Drug Assoc. 30 (1976) 180)
har understreket betydningen av det farmasøytiske problem som knytter seg til bruk av hydrofobe substanser og vanskelighetene med å gjøre disse synbart oppløselige i vann.
De oppløseliggjøringsmetoder som disse forfattere
har benyttet, går vanligvis ut på å danne miceller eller å benytte blandede oppløsningsmidler i hvilke den aktive bestanddel har tilbøyelighet til å oppløses.
Vanskelighetene tilspisses når de hydrofobe forbindel-
ser ikke lar seg overføre i en ionisk form, såsom et salt
av en syre eller et salt av en base.
Dessuten vil enkelte derivater som kan la seg overføre
i ionisk form, meget lett hydrolyseres (f.eks. salter av en svak base eller av en svak syre) eller gi en pH-verdi som er uforlikelig med den som kreves for parenteral administrering (meget sure oppløsninger eller meget basiske opplos-ninger).
Derfor har Swarbrick (loe. eit.) beskrevet en generell metode for å oppløse forbindelsene i et vandig system, hvil-
ken metode går ut på å dispergere den aktive bestanddel i form av miceller ved hjelp av ioniske eller ikke-ioniske overflateaktive midler. Dette system medfører imidlertid to vesentlige ulemper. På den ene side vil den aktive be-
standdel ha tendens til å endres eller" hydrolyseres hurtigere enn i det system hvor den er uoppløselig, og på den annen side taper den aktive bestanddel en del av sin biologiske aktivitet, enten gjennom en modifikasjon av dens absorpsjon i fordøyelsessystemet, eller fordi micellene bærer en elek-
trisk ladning som endrer deres bindeevne på mottagerstedet.
Således vil f.eks. de fenoliske antibakteriéllé midier
tape sin aktivitet som følge av interaksjon med de anioniske overflateaktive midler.
Dette er grunnen til at metoden med å danne miceller, selv om den muliggjør oppløseliggjøring av hydrofobe forbindelser i vann, ikke løser de generelle farmasøytiske vanskeligheter som er forbundet med administrering av et oppløselig produkt under bibeholdelse av aktiviteten, stabiliteten eller inn-trengningen i organismen eller helst under oppnåelse av en forbedring av disse egenskaper, sammenlignet med den uopplø-selige form.
Mer nylig har undersøkelser utført av Webb vist hvor vanskelig det er å fremstille et injiserbart vandig preparat på basis av en vannuoppløselig forbindelse, nemlig metiapin. Forekomsten av krystaller i preparatet dannet med propylen-glycol/vann eller ethanol/vann og forekomsten av tromber forårsaket ved injeksjon av oppløsningen av den aktive bestanddel i eddiksyre som er gjort isotonisk med mannitol, har gjort det nødvendig å benytte et annet medium for å gjøre dette ikke-ioniserbare molekyl oppløselig i vann.
Bruken av dette andre medium innebærer at den aktive bestanddel oppløses i en umettet fettsyre, og at den oppnådde lipidiske oppløsning emulgeres i vann etter tilsetning av et ikke-ionisk overflateaktivt middel. Den resulterende opp-løsning er klar, dens pH-verdi lar seg innstille innenfor
et fysiologisk akseptabelt område og den lar seg sterilisere ved filtrering gjennom et steriliserende filter. Injeksjon
av denne oppløsning forårsaker forekomst av blødende områder og farvede områder. Ved undersøkelser av disse områder etter autopsia er det blitt påvist forekomst av områder med necro-sis i muskelfibrene og dannelse av granuloma i de store cel-lene .
Forfatteren konkluderer med at bruken av denne opplø-seliggjøringsmetode øket muligheten for irritasjon forårsaket av den aktive bestanddel, som følge av en mer utstrakt diffu-sjon av oppløsningen på injeksjonsstedet som følge av til-stedeværelsen av et overflateaktivt middel.
En tilsvarende undersøkelse utført med terfenadin har vist nødvendigheten av å tilsette en vesentlig mengde ikke-ionisk overflateaktivt middel for å opprettholde en klar opplesning. Den påviste også grensene for konsentrasjonen av den aktive bestanddel i slike oppløsninger.
Derfor har denne type av olje-i-vann-dispersjon heller ikke muliggjort løsning av det tekniske problem som er forbundet med fremstillingen av vandige farmasøytiske former.
En annen metode for fremstilling av et oppløselig preparat er den metode som beskrives av J. Gessing og P.J. Toumlin (British J. of Anaesthesia 89 (1977) 954). Den aktivt bestanddel oppløses ved denne metode i en 20%ig lipidisk emulsjon (Intralipid. Forfatterne konstaterte med overras-kelse at det for å oppnå den samme grad av beroligelse under medullær anaesthesia med denne oppløsning var nødvendig å benytte en mengde aktiv bestanddel som var dobbelt så stor som den som krevdes for en vandig oppløsning (8,6 mg isteden-for 4,92 mg for en vandig oppløsning).
Forfatterne trakk av dette den konklusjon at det fant sted interaksjon med de farmacokinetiske egenskaper av den aktive bestanddel "Diazepam".
Sluttelig kan det antas at problemet med å oppløselig-gjøre aktive bestanddeler i et vandig medium oppviser to aspekter:
- Et fysikalsk-kjemisk aspekt:
Den resulterende oppløsning må være stabil ved en pH-verdi som er forlikelig med injeksjon eller annen admini-streringsmetode, og den aktive bestanddel må ikke nedbrytes som følge av at den oppløses.
- Et biologisk aspekt:
Oppløsningen må bibeholde all farmakologisk eller kli-nisk aktivitet av den aktive bestanddel og må ikke endre ab-sorpsjonsegenskapene eller de farmacokinetiske egenskaper 1 ugunstig retning.
Det er forøvrig i SE utlegningsskrift nr. 434 457,
DE utlegningsskrift nr. 2 102 889, FR patentskrift nr.
2 040 194 og GB patentsøknad, publ. nr. 2 069 334 foreslått å oppløseliggjøre en terapeutisk aktiv forbindelse i oppløs-ningsmiddelkombinasjoner omfattende polyethylenglycol og vann, men de der beskrevne metoder løser ikke det problem som løses ved fremgangsmåten ifølge oppfinnelsen.
Det er nu funnet en ny og uventet løsning på det oven-for omtalte problem, som har eksistrert i mange år og som hittil er forblitt uløst. Det dreier seg om en fremgangsmåte hvor det benyttes ko-oppløsningsmidler, og hvor den terapeu-tiske forbindelse dispergeres i én eller flere ethylenglycol-polymerer med moderat molekylvekt, uten noen tilsetning av vann, hvoretter den resulterende pseudo-oppløsning settes til et vandig medium med en bestemt pH-verdi.
Med oppfinnelsen tilveiebringes det således en fremgangsmåte for å oppløseliggjøre en hydrofob, terapeutisk aktiv, organisk forbindelse, som lett hydrolyseres eller oxyderes i vandig medium, spesielt en N-acyl-indol som f.eks. indometacin, cinmetacin, acemetacin, nicametacin og sermetacin. Fremgangsmåten er særpreget ved at
a) den aktive hydrofobe organiske forbindelse først oppløses i én eller flere polyethylenglycoler med molekylvekt
fra 300 til 700, som kan være tilsatt fysiologisk godtagbare konserveringsmidler, stabiliseringsmidler, smaksstoffer, farvestoffer og/eller søtningsmidler, og
b) det til denne oppløsning deretter settes en vandig bufferoppløsning som har en pH-verdi i området fra 4,5 til
8, og som eventuelt inneholder fysiologisk godtagbare mineral-sålter og konserveringsmidler og eventuelt en annen terapeutisk aktiv forbindelse, slik at det fåes en endelig oppløsning som inneholder fra 0,1 til 25% av den eller de terapeutisk aktive forbindelser, og som har en pH-verdi bestemt av buffer-oppløsningen.
Det er tenkelig at dispergeringen av den aktive bestanddel i polyethylenglocol av høy molekylvekt forårsaker dannelse av micelle-partikler hvor hvert molekyl av den aktive bestanddel beskyttes kjemisk av polyethylenglycolmole-kylene og den aktive bestanddels reaktive kjemiske funksjoner således blokkeres. Dette fører ti.l at den aktive bestanddel beskyttes mot hydrolyse eller auto-oxydasjon.
Oppfinnelsen er som nevnt særlig anvendelig på proble-
met med oppløseliggjøring av N-acyl-indoler, som f.eks. indometacin, cinmetacin, acemetacin, nicametacin eller sermetacin. Forbindelser som har en N-acylfunksjon, spaltes meget lett,
og hydrolyse av denne gruppe i et nøytralt eller svakt basisk medium medfører tap av aktiviteten. Dette gjelder for de fleste tilfeller hvor det foretas saltdannelse med en sterk base.
Oppfinnelsen er også anvendelig for vandige oppløsnin-
ger hvor konsentrasjonen av aktiv forbindelse er høy eller for vandige oppløsninger av lite volum.
Blant de konserveringsmidler som er oppløselige i
eller blandbare med den organiske fase, kan nevnes methyl-
eller propyl-p.hydroxybenzoat, isopropylgallat eller tertbu-tyl-p.hydroxyanisol.
Blant mineralsaltene som kan settes til den vandige
fase, kan nevnes dem som kreves for å gjøre den vandige opp-løsning isotonisk, såsom f.eks. natriumklorid eller lithium-klorid.
Midlet som bestemmer pH-verdien, er fortrinnsvis et buffersystem såsom f.eks. en blanding av mononatriumfosfat og dinatriumfosfat, en blanding av borsyre og natriumborat eller en blanding av eddiksyre og natriumacetat. Administre-
ring via fordøyelsesapparatet gjør det mulig å administrere oppløsninger hvis pH-verdi er mellom 4,5 og 8.
Ved parenteral administrering kan det benyttes oppløs-ninger hvor pH-verdien er mellom 5,5 og 7,5.
Ved adminstrering gjennom slimhinner eller gjennom
huden kreves pH-verdier mellom 5,5 og 7,0.
Valget av buffersystem avhenger av forlikeligheten
og effektiviteten av administreringsmetoden.
Arten av den hydrofobe organiske forbindelse som inn-lemmes i slike preparater, kan variere innenfor et meget bredt område. Det kan dreie seg om steroidforbindelser såsom horrnonforbindelsene (estradiol, estron, testosteron, testos-teronpropionat, nor-ethynodiolacetat, trenbolonacetat, ethy-nylestradiol). Det kan likeledes dreie seg om analgetiske forbindelser, såsom paracetamol, benorylat, glafenin, floe-tafenin, nicafenin eller anthrafenin. Likeledes kan det dreie seg om antiinflammatoriske midler, såsom piroxicam, oxicam, tenoxicam, ibuprofen, flurbiprofen, tiaprofensyre eller di-flunisal.
Videre kan det dreie seg om uricosuriske midler såsom allopurinol, benzbromaron eller bromaron.
Det kan videre dreie seg om kardiovasculære midler, såsom khellin, amiodaron, amrinon eller erythritolpentani-trat.
Det kan også dreie seg om diuretiske midler, såsom klorthiazid, cyclothiazid, ethacrynsyre eller triamteren.
Videre kan det dreie seg om antibakterielle midler, såsom sulfamider, trimethoprim, halvsyntetiske penicilliner, forestrede eller til salter overførte macrolider, såsom erythromycin-dipropionat, josamycin, fosfonomycin, tetracyc-liner, enniatiner, rifampiciner eller cefalosporiner.
Det kan likeledes dreie seg om hypnotiske eller antiepileptiske forbindelser, såsom barbiturater, ureaforbin-delser eller oxazolidoner.
Likeledes kan det dreie seg om antidepressiva, såsom imipramin, clomipramin, des-methylklorimipramin, mianserin eller amineptin.
Videre kan det dreie seg om neuroleptiske midler, såsom haloperidol, domperidon, clorprometazin, metoclopramid, sulpirid, diazapam, lorazepam, bromazepam eller nitrazepam.
Videre kan det dreie seg om antifungus-midler eller anti-trichomonas-midler, såsom ternidazol, secnidazol, mico-nazol, econa-zol eller chlotrimazol.
Videre kan det dreie seg om antivirusmidler, såsom jod-desoxy-uridin eller citarabin eller anti-mitotiske midler, såsom "Ftorafur" eller "Carmofur".
Den prosentvise mengde aktiv bestanddel i disse preparater er som nevnt i området fra 0,1 til 25%, og den er fortrinnsvis fra 1 til 10%.
Oppløsningen som fåes ved fremgangsmåten ifølge oppfinnelsen, kan pakkes i enhetsdoser eller lyofiliseres, slik at det kan foretas ny oppløsning på anvendelsestidspunktet.
Den aktive bestanddel kan også bare ganske enkelt oppløses i polyethylenglycolen med molekylvekt mellom 300
og 700, idet den vandige fase tilsettes på tidspunktet for administreringen. Den vandige fase kan da inneholde oppløst en annen aktiv bestanddel som er fullt oppløselig i vann. Dette er tilfelle for nesedråper eller for øyedråper. Det foretrekkes å oppløse i polyethylenglycolen en aktiv bestanddel som er uoppløselig i vann, som f.eks.-et antiinflammato-risk middel og å oppløse i den vandige fase en annen aktiv bestanddel, såsom et middel som motvirker congestio, såsom f.eks. neosynefrin eller fenylpropanolamin i form av hydro-kloridet eller et antibakterielt middel såsom chlorhexidin. Blandingen av de to oppløsninger foretas umiddelbart før administreringen og muliggjør således samtidig administrering av de to aktive bestanddeler med virkninger som kompletterer hverandre eller med synergistisk virkning.
De følgende eksempler illustrerer oppfinnelsen.

Claims (1)
- Fremgangsmåte for å oppløseliggjøre en hydrofob, terapeutisk aktiv, organisk forbindelse, som lett hydrolyseres eller oxyderes i vandig medium, spesielt en N-acyl-indol som f.eks. indometacin, cinmetacin, acemetacin, nicametacin og sermetacin,karakterisert ved ata) den aktive hydrofobe organiske forbindelse først oppløses i én eller flere polyethylenglycoler med molekylvektfra 300 til 700, som kan være tilsatt fysiologisk godtagbare konserveringsmidler, stabiliseringsmidler, smaksstoffer, farvestoffer og/eller søtningsmidler, ogb) det til denne oppløsning deretter settes en vandig bufferoppløsning som har en pH-verdi i området fra 4,5 til8, og som eventuelt inneholder fysiologisk godtagbare mine-ralsalter og konserveringsmidler og eventuelt en annen terapeutisk aktiv forbindelse, slik at det fåes en endelig opp-løsning som inneholder fra 0,1 til 25% av den eller de terapeutisk aktive forbindelser, og som har en pH-verdi bestemt av bufferoppløsningen.
