Abstract
Ny og forbedret bærer for administrering av farma-søytiske bestanddeler til menneskehud på en slik måte at biotilgjengeligheten, den subcutane absorpsjon og effektiviteten av den aktive bestanddel økes i bemerkelsesverdig grad. Bæreren består av et flyktig, silikon, en-fettalkohol med fra 12 til 22 carbonatomer, en forhåndsvalgt aktiv bestanddel og slike preserverings- eller emul-ger ingsmidler som kan være berettiget..
Description
Foreliggende oppfinnelse vedrører en basis for avgivelse av en farmasøytisk aktiv forbindelse valgt fra gruppen bestående av hydrokinon, benzoylperoxyd, hydrocortison, providon-jod, kulltjæreekstrakt, bactirnycin og 13-cis-retinoinsyre. Videre vedrører oppfinnelsen et kosmetisk preparat for motvirkning av uskjønt utseende ved hyperpigmentering.
Ved den tidligere kjente teknikk har topisk virk-somme farmasøytiske midler tradisjonelt blitt administrert i kremer, lotions og geler som alle krever at brukeren søler fingrene og hendene sine ved påføringen av slike medika-menter og under den etterfølgende "innsmøring". Videre er slike produkter fettet, sentørket og etterlater uunngåelig en rest som ikke bare er synlig, men smitter av på klær og lignende, alt sammen brysomt for brukerens sosiale behagelig-het.
En variasjon fra de tradisjonelle kremer, lotions og geler oppsto ved utviklingen av de såkalte stiftavleverings-systemer som ble brukt til å påføre antiperspiranter, deodo-ranter, lebebalsam, lebefarge og lignende bestanddeler som vanligvis påføres overflaten av huden.
Antiperspirantstifter basert på kombinasjonen av et flyktig silikon, eh fettalkohol og en pulverformet antiperspirant ble nylig utviklet og markedsført. Disse stifter har imidlertid ikke blitt tatt i bruk ut over enkle antiperspi-rantstifter under anvendelse av vannoppløselige uorganiske antiperspirantsalter. Denne begrensede bruk er resultatet av den kjente mekanisme til antiperspirantbærere og behovet for med hensikt å utforme slike administrasjonssystemer for å begrense mengde aktiv bestanddel (antiperspirant) som kan trenge inn i huden. For at en antiperspirant skal være virk-som, må en høy konsentrasjon av den aktive bestanddel beholdes på hudoverflaten hvorfra den sakte oppløses og sprer seg inn i svettekanalene, og ikke absorberes gjennom huden. For å utvide bruken av denne tidligere kjente teknikk til andre farmakologisk aktive, topiske midler, er det nødvendig å øke mengden av legemidlet som er i stand til å trenge gjennom huden for å nå de ønskede virkningssteder, svært mye. Denne absorpsjon av et legemiddel gjennom huden betegnes som percutan absorpsjon.
Heller ikke det typiske lebebalsampreparat løser problemet ettersom slike produkter vanligvis inneholder store mengder voks (som f.eks. bivoks, karnauba og lignende) og oljer (som f.eks. laxérolje, lanolin og lignende), hvoretter det frembringes en fettet, oljeaktig fornemmelse på huden som ikke bare er ubehagelig, men gir brukeren følelsen av å være "skitten". Denne type preparat, som tilfellet også er med antiperspiranter, begrenser dertil i alvorlig grad den percutane absorpsjon av de fleste legemidler gjennom huden.
På samme måte makter de konvensjonelle kremer og lotions ikke å imøtekomme probleireie som er oppregnet ettersom kremer og lotions er overordentlig fettet og har mulig-het for å avsondre den aktive bestanddel slik at den percutane absorpsjon av legemidlet og dets virkningsfullhet redu-seres .
Et formål ved foreliggende oppfinnelse er å utvikle en basis for avgivelse av bestemte farmasøytisk aktive forbindelser på menneskehud, som er akseptabel for forbrukeren og som unngår de ovenfor nevnte ulemper ved de tidligere kjente kremer, lotions, geler og stifter, mens den gjør det mulig for brukeren lett å kontrollere anbringelsen på et bestemt sted med maksimal effektivitet, minimalt tap og uten noe søl.
Et annet formål ved foreliggende oppfinnelse er å tilveiebringe en ny og forbedret basis som er forenlig med de farmasøytisk aktive forbindelser hydrokinon, benzoylperoxyd, hydrocortison, providon-jod, kulltjæreekstrakt, bactimycin og 13-cis-retinoinsyre, og som er i stand til å administrere slike farmasøytiske forbindelser med effektivitet og økt percutan absorpsjon for derved å øke både den aktive bestanddels biotilgjengelighet og dens styrke.
Foreliggende oppfinnelse vedrører således en basis for avgivelse av en farmasøytisk aktiv forbindelse valgt fra gruppen bestående av hydrokinon, benzoylperoxyd, hydrocortison, providon-jod, kulltjæreekstrakt, bactimycin og 13-cis-retinoinsyre kjennetegnet ved at den består av en blanding som inneholder fra 20 og opp til 40 vekt% fettalkohol med 12 til 22 carbonatomer eller blandinger derav, fra 30 og opp til 60 vekt% av et flyktig silikon, og fra 1 til 10 % av en bestanddel valgt fra et preserveringsmiddel, et emulgeringsmiddel eller en blanding derav.
Videre vedrører foreliggende oppfinnelse et kosmetisk preparat for motvirkning av uskjønt utseende ved hyperpigmentering. Preparatet er kjennetegnet ved at det består av fra 0,1 og opp til 10 vekt% hydrokinon som den vesentlige bestanddel, idet bestanddelen er dispergert i en halvfast bærer med fra 20 og opp til 40 vekt% stearyl- og cetylalkohol, fra 30 og opp til 60 vekt% av et flyktig silikon og fra 1 til 10 vekt% av en bestanddel valgt blant et preserveringsmiddel, et emulgeringsmiddel eller en blanding derav.
En viktig faktor ved foreliggende oppfinnelse er bruken av et flyktig silikon som f.eks. dimethicon, cyklo-methiconene eller ekvivalente cykliske silikoner, som gjør at man unngår de tunge oljer og vokser som hittil har vært brukt, og i stedet tilveiebringer et silkebløtt og ikke-fettet smøremiddel som setter den aktive bestanddel i stand til å bli spredd jevnt og glatt på huden. Så snart den aktive bestanddel er blitt administrert til det utvalgte sted, vil silikonene fordampe og ikke etterlate noen rest på huden. Egnede silikoner for slik anvendelse er tilgjengelig kommer-sielt som "200 fluid" (0,65 cs), "344 fluid" (tidligere "Q2-1053") og "345 fluid" (tidligere "Fl-3597").
Bruken av dodecanol, tridecanol, tetradecanol, hepta-decanol, octadecanol, nonadecanol, eicosanol, heneicosanol og docosanol (stearylalkohol) eller blandinger derav utgjør et annet viktig trekk ved oppfinnelsen.
Nærmere bestemt inneholder basisen ifølge foreliggende oppfinnelse fra 20 og opp til 40 vekt% fettalkohol som f.eks. en blanding av stearyl- og cetylalkohol, fra 30 og opp til 60 vekt% flyktig silikon som f.eks. dimethicon eller et cyklomethicon og fra 1 til 10 % av et preserveringsmiddel og/eller emulgeringsmiddel.
Den således beskrevne basis har den ytterligere over-raskende fordel at den ikke er begrenset kun til avgivelse av vannoppløselige legemidler, men også kan anvendes på en svært gunstig måte sammen med mange uoppløselige bestanddeler når disse suspenderes i form av små partikler (mindre enn ca. 30 um) slik at det dannes et akseptabelt produkt.
De bestemte enkeltheter ved hvilke vår basis anvendes sammen med bestemte farmakologiske midler, er beskrevet nedenunder. Det rettes en spesiell oppmerksomhet til denne basis' evne til å stabilisere legemiddelbestanddeler som er kjent for å være ustabile i de fleste vanlige bærere, og til å akseptere de aktive bestanddeler som hittil ikke har vært akseptable i noe virksomt bærersystem.
De aktive farmasøytiske forbindelser som effektivt inkorporeres i foreliggende basis, og som, slik det vil bli påvist, oppviser forbedret stabilitet og økt biotilgjengelighet, er oppstilt i tabell I. Den aksepterte bruk av hver forbindelse og dens forventede holdbarhet, slik den ble bestemt ved hjelp av fremskyndede stabilitetsundersøkelser, er også angitt i tabell I.
Av tabell I kan det lett sees at basisen ifølge foreliggende oppfinnelse er basert på flyktig silikon, gir en utmerket grunnmasse for stabiliseringen av mange legemidler som er kjent for å dekomponere i vandige bærere eller lett oxydere i kremer og lotions.
Silikon/fettalkohol (C12-C22)~ Dasisen ifølge foreliggende oppfinnelse gir videre en bærer med lavt irritasjons-potensial. Dette er viktig ved behandling av hudsykdommer eller følsom hud med hvilket som helst kosmetisk middel eller legemiddel. Mange av bærerne som hittil har vært brukt, inneholder ethylalkohol, isopropylalkohol, glycoler og andre svakt irriterende stoffer. Silikon/fettalkohol-systemet gir en ikke-sviende, ikke-irriterende og svakt allergen bærer.
Den økte biotilgjengelighet for hydrokinonstiften ifølge foreliggende beskrivelse sammenlignet med de mer tradisjonelle bærerne som f.eks. kremer og lotions, er et ene-stående resultat. Det er alminnelig kjent at ved å sammen-
ligne effektiviteten av forskjellige antiperspirantbærere,
slik som kremer, aerosoler, roll-ons og stifter, er den be-stemmende faktor nivået av den aktive bestanddel (antiperspirant) . Det vil si at den samme prosent aktiv antiperspirant-bestanddel vil gi det samme nivå av antiperspirantvirkning, uansett hvilken bærer som brukes.
Som vist i tabellene II og III, oppviser foreliggende oppfinnelse en vesentlig forbedret legemiddel-biotilgjengelighet, og derved effektivitet, sammenlignet med andre vanlige brukte bærere som inneholder den samme prosent aktiv bestanddel (f.eks. hydrokinon). I tillegg oppviser foreliggende oppfinnelse en vesentlig større biotilgjengelighet enn en standard krembærer som inneholder over to ganger så
mye av den aktive bestanddel. Disse resultater er bemerkelses-verdige sammenlignet med det som ville være forventet ved ganske enkelt å utnytte den tidligere kjente antiperspirant-teknikk.
Ved én utførelsesform av foreliggende oppfinnelse
hvor det brukes stearylalkohol, blandes alle de ønskede bestanddeler med unntak av det flyktige silikon og blandingen opp-varmes deretter forsiktig til ca. 80°C inntil alle bestand-
deler er grundig blandet ut i den smeltede stearylalkohol (smeltepunkt = 8 0°C)• Den smeltede oppløsning eller suspen-
o
sjon avkjøles så til 60 til 70 C og det flyktige silikon til-settes til den avkjølte blanding og røres inn i denne. Når silikonet er grundig blandet ut, helles den resulterende blanding i egnede former eller beholdere og får avkjøles inntil den er størknet. En gunstig effekt av denne praksis er at den aktive bestanddel beholder sin homogene dispersjon i den størknede masse og det er lite eller ingen bunnfelling i den.
Oppfinnelsen har med hell vært brukt til å fremstille stiftpreparatene gjengitt nedenunder som så er i stand til å administrere den angitte aktive farmasøytiske bestanddel til de ønskede steder på en lettvint og effektiv måte.
Brukeren anvender stiften ved å gripe beholderen hvori bæreren og dens aktive bestanddel er blitt anbragt. Den konvensjonelle "push-up"-beholder, dvs. en beholder med en grunn-plate som er flyttbar oppover som følge av en kraft påført denne av brukeren, som igjen tvinger bæreren oppover inntil en påføringsoverflate eksponeres over den øvre kant på beholderen, kan ansees for å være typisk for beholderen som her brukes. Brukeren håndterer deretter beholderen på en strykende måte for å belegge det ønskede sted med blandingen og derved administrere den aktive bestanddel til stedet hvor dens virkning er ønsket eller påkrevet. Dette er selvfølgelig ikke ment å skulle begrense bruken av foreliggende oppfinnelse til bare stiftformer, men er gjengitt som et eksempel på en mulig anvendelse.
For ytterligere å hjelpe på forståelsen av foreliggende oppfinnelse, og ikke for å begrense den, fremlegges følgende eksempler.
Eksempel 1
Det ble fremstilt en porsjon av en hydrokinonholdig bærer for å tilveiebringe et produkt for behandling av hyperpigmentering. Legg merke til at et slikt produkt, for å være effektivt, må være i stand til å trenge inn i epidermis for å nå de hyperaktive melanocyter som befinner seg ved den dermale/epidermale overgang.
En blanding av hydrokinon, benzofenon-3-, octyl-di-methyl-para-amino-benzosyre, "Laureth-4", stearylalkohol, cetylalkohol, "PEG-1000" og preserveringsmidler ble rørt sammen og forsiktig varmet opp til 80°C hvor den ble holdt mens alle bestanddeler ble grundig utblandet i den smeltede masse. Den smeltede oppløsning/suspensjon ble deretter av-kjølt til en temperatur mellom 60 og 70°C og et flyktig silikon (cyklomethicon) ble tilsatt og iblandet. Etter at silikonet var grundig blandet med alle de øvrige bestanddeler, ble blandingen helt over i en sylindrisk form eller beholder og avkjølt ytterligere inntil den var fullstendig størknet.
Det resulterende produkt, som her er betegnet "hydrokinonstift", har følgende sammensetning (i vekt%):

Hydrokinonstiftene fremstilt ifølge eksempel 1 ble testet for biotilgjengelighet mot de viktigste kommersielle OCT-produkter som selges for hudbleking under anvendelse av standard fremgangsmåten for slike tester basert på FICK'S
lov (se: Franz, T.J. "On the bioavailability of topical formulations " J. Amer. Academy of Dermatology, St. Louis, Vol. 9, nr. 1, s. 63 - 73, spesielt 68 - 69). Resultatene
er gjengitt i tabell II og uttrykt som den prosent av den påførte dose som ble absorbert i løpet av 24 timer.
Eksempel 4
Hydrokinonstiften ifølge eksempel 1 ble testet for biotilgjengelighet mot den viktigste reseptpliktige farmasøy-tiske (Rx) krem for behandling av hudhyperpigmentering ved å gjenta fremgangsmåten ifølge eksempel 3. Resultatene fra denne test fremgår av tabell III.

Av det ovenfor anførte er det helt åpenbart at det her er blitt beskrevet et nytt og unikt bærersystem som av-leverer aktive farmasøytiske og kosmetiske bestanddeler i en form hvor biotilgjengeligheten og den percutane absorpsjon er økt i bemerkelsesverdig grad og ved hvilket de fysiske ulemper med tidligere kjente systemer er unngått. Selvsagt er slike modifikasjoner, endringer og tilpasninger som er åpenbare for fagfolk innen teknikken når de står overfor foreliggende beskrivelse, ment å ligge innenfor foreliggende opp-finnelses ånd som bare er begrenset av kravenes omfang.

1. Basis for avgiveles av en farmasøytisk aktiv forbindelse valgt fra gruppen bestående av hydrokinon, benzoylperoxyd, hydrocortison, providon-jod, kulltjæreekstrakt, bactimycin og 13-cis-retinoinsyre, karakterisert ved at den består av en blanding som inneholder fra 20 og opp til 40 vekt% fettalkohol med 12 til 22 carbonatomer eller blandinger derav, fra 30 og opp til 60 vekt% av et flyktig silikon, og fra 1 til 10 % av en bestanddel valgt fra et preserveringsmiddel, et emulgeringsmiddel eller en blanding derav.
