Description
Foreliggende oppfinnelse vedrører et tannpleiemiddel med antikaries-virkning, og det særegne ved tannpleiemidlet er at det inneholder: minst ett fluorid og minst ett monofluorfosfat som
fluorforbindelser, og
en kombinasjon av minst to forskjellige fosfatforbindelser hvorav minst den ene er et cyklisk trimetafosfat, og minst ett kalsiumsalt av en organisk syre som avgir
kalsiumioner til tannemaljen.
Disse og andre trekk ved tannpleiemidlet i henhold til oppfinnelsen fremgår av patentkravene.
Tannpleiemidlet i samsvar med oppfinnelsen har en forbedret antikaries-virkning og er også istand til helt eller delvis å helbedre karies-angrepne tanndeler.
Betegnelsen "tannpleiemiddel" som anvendt heri betegner produkter som bare innføres i munnen i en forholdsvis kort tidsperiode hvor de bringes i intim kontakt med hovedsakelig alle overflater på tennene hvorpå de fjernes.
Eksempler på slike produkter er tannpastaer, profilaktiske pastaer, tannpoleringsmidler, munnvann, påføringsvæsker og geler, spesifikke tyggegummier, etc.
Det har lenge vært kjent at tilsetning av fluorforbindelser til slike tannpleiemidler i vesentlig grad kan redusere ut-viklingen av karies.
Ved innblandingen av fluorforbindelser støtte man imidlertid på det problem at disse materialer viste seg å være ikke-blandbare eller dårlig blandbare ved et antall av de hyppigst anvendte utgangsmaterialer.
Spesielt ved anvendelse i tannpastaer støtte man på mange problemer. For å gi god blandbarhet mellom fluorforbindelsen på den ene side og de andre bestanddeler på den annen side ble det utviklet nye fluorforbindelser og nye råmaterialer (spesielt poleringsmidler).
Grunnleggende forskning vedrørende blandbarheten av enkelte fluorforbindelser med de mest vanlige tannpasta-bestanddeler ble gjennomført av Ericsson i 1962 . Hans arbeid vedrørte spesielt en fluorforbindelse som var ganske ny ved dette tidspunkt, nemlig natrium-monofluorfosfat.
I denne substans er fluorionet komplekst bundet og som et resultat mindre reaktivt i forhold til de andre komponenter.
Da natrium-monofluorfosfat har en ytterst god antikaries-virkning, er denne fluorforbindelse nå den hyppigst anvendte i tannpleiemidler. Andre fluorforbindelser, spesielt NaF og SnF^ har også vist seg å være effektive ved kliniske for-søk, men disse fluorforbindelser kunne først anvendes etter at spesielle råmaterialer var blitt utviklet som kunne kombineres med disse fluorforbindelser. Blant disse er spesielt poleringsmidlene, som f.eks. spesielle poleringsmidler av syntetisk harpiksmaterial for tannpleiemidler inneholdende NaF, og spesielle polymere bestanddeler og pyrofosfater for tannpleiemidler inneholdende SnF^.
I tillegg til disse tre hyppigst anvendte fluorforbindelser, er mange andre fluorforbindelser egnet for anvendelse. Blant disse kan nevnes fluorider som f.eks. aluminiumfluorid, cesiumfluorid, kaprikfluorid, ferrifluorid, magnesiumfluorid, fluorfosfater som f.eks. monofluorfosfater, di-fluorfosfater, og polyfluorfosfater, f.eks. kalsium-monofluorfosfater, natriumdifluorfosfat, vanadium-monofluorfosfat og litium-monofluorpolyfosfat.
Andre egnede uorganiske fluorforbindelser er heksafluor-sili-kater, siliko-fluorsyre og zirkonium-germaniumfluorider, mens organiske fluorforbindelser, som f.eks. aminhydrofluorider, imidazolhydrofluorider, kvaternære ammoniumfluorider, amino-fluorfosfater, hydrofluorider av aminosyrederivater og fluo-rerte biguanidforbindelser også er egnet.
Vitenskapelige undersøkelser vedrørende virkningen av fluorforbindelser har vist at forskjellige reaksjonsmekanismer spiller en rolle. Den klassiske forklaring for den kariostatiske virkning av fluorforbindelser er at hydroksyapatitt omsettes med fluoridionet og omdannes derved til fluorert hydroksyapati tt.
Dette material er mindre oppløselig i surt medium enn hydroksyapatitt. Som en følge derav blir tannen etter en fluor-tilførsel bedre beskyttet mot syrepåvirkningene som initierer kariesutviklingen.
I tillegg til denne mekanisme har andre vært foreslått, f.eks. en blokade av uregelmessigheter i apatitt-krystallen, doping av knutesteder, etc. En viktig teori som er blitt gitt en vitenskapelig basis i de senere år, går ut på innvirkningen av fluoridion på remineraliseringsprosessen.
Remineralisering av tannemalje betyr det forhold at hydroksyapatitt kan dannes fra substanser som naturlig forekommer i spytt, spesielt på steder hvor hydroksyapatit er forsvunnet i en foregående fase.
I munnen er der en naturlig likevekt mellom hydroksyapatitt som oppløses fra tennene på den ene side og hydroksyapatitt som dannes på eller i tennene fra spyttet på den annen side. Denne likevekt forandres kontinuerlig. Blant andre faktorer bestemmes den av kostholdet og den fysiske tilstand. Hvis likevekten er slik at hydroksyapatitt oppløses, betegnes dette som demineralisering og karies-tilstanden oppstår. Hvis likevekten er slik at hydroksyapatitt dannes betegnes dette som remineralisering. Ved remineralisering kan ini-
tialt tannsvinn avhjelpes ved hjelp av naturlige midler.
Man har funnet at fluorforbindelser, selv i små konsentra-sjoner, fremmer remineralisering. Denne mekanisme gir om mulig ennå bedre beskyttelse mot tannsvinn enn nedsettelsen i syreoppløselighet som skyldes fluorapatitt-dannelse, referert til i det foregående.
Den grad som de forskjellige fluorforbindelser fremmer remineraliseringsprosessen varierer i sterk grad. Effektiviteten som fluorid innlemmes med varierer også med de forskjellige fluorforbindelser som anvendes for fluoridtilfør-selen. Det er ønskelig at en optimal virkning oppnås fra de kjente beskyttelsesmekanismer, og dette kan blant annet oppnås ved å anvende en kombinasjon av minst to eller flere fluorforbindelser. De undersøkelser som ligger til grunn for den foreliggende oppfinnelse har vist at kombinasjonen av minst ett fluorid, f.eks. natriumfluorid, sammen med minst ett monofluorfosfat, f.eks. natrium-monofluorfosfat, gir de beste resultater.
Et forhold mellom natriumfluorid og natrium-monofluorfosfat på 3 :1, uttrykt som F, foretrekkes spesielt.
For å bestemme det optimale fluoridforhold ble det blant annet annvendt en såkalt RES-test (redusert emaljeoppløse-lighet). Ved denne test behandles en fullstendig renset tann fra storfe i en tannpastasuspensjon (1:3 fortynning) ved en temperatur på 37°C i 24 timer. Etter vasking og rensning av tannen anbringes denne i en acetat-buffer (pH 5,0) i 24 timer hvoretter mengden av oppløst kalsium og fosfat måles i mikromol pr. cm<2> tannemalje. En tann behandlet med vann anvendes som kontroll. For en pasta inneholdende 1500 ppm fluor (beregnet som F), ble følgende reduksjoner funnet: Det er kjent at cykliske fosfater og spesielt cykliske trimetafosfater synergistisk øker den kariostatiske virkning av monofluorfosfater. For å oppnå en optimal virkning av monofluorfosfatet i tannpleiemidlene er det derfor ønskelig å tilsette en mindre mengde cyklisk fosfat. Natriumtrimetafosfat foretrekkes for dette formål, idet dette i kombinasjon med natrium-monofluorfosfat i en tannpasta ble undersøkt ved kliniske forsøk, hvor det ble funnet meget høye kariesreduk-sjoner. Den vanlige konsentrasjon var fra 0,1 til 5% og foretrukket ble det anvendt mellom 0,25 og 2% cyklisk fosfat.
Den totale mengde fluorforbindelser som tilsettes til tannpleiemidler i samsvar med oppfinnelsen avhenger spesielt av det produkt hvori de anvendes. Et kriterium for den maksimale konsentrasjon av fluorid som anvendes er den gjennomsnittlige mengde av produkt som anvendes for hver behandling, og også hyppigheten av anvendelsen. Fluoridkonsentrasjonen bør velges for å sikre at den giftige dose ikke overskrides.
I tannpastaer er den totale mengde fluorforbindelser ofte til-svarende 1000 til 1500 ppm F. I profilaktiske pastaer er denne konsentrasjon omtrent 5 ganger høyere, mens påførings-væsker og geler ofte inneholder 10.000 til 15.000 ppm F.
I fluoridholdige munnvann for daglig bruk er den totale mengde fluorid lavere enn i tannpasta. En passende konsentrasjon i slike væsker er 250 - 500 ppm F.
Så langt det har kunnet fastslås synes konsentrasjonen av fluorid som tilføres å være noe for høy for optimal remineralisering. Det er imidlertid av betydning av en slik mengde fluorid stilles til disposisjon at en buffer-mengde dannes på tannen, ofte i form av kalsiumfluorid, som er istand til grad-vis å frigi fluor til spyttet, slik at den nødvendige ganske lave fluoridkonsentrasjon er tilstede i spyttet over en lengre tidsperiode for optimal remineralisering.
I tillegg til fluoridioner er det av vesentlig betydning at tannpleiemidlene har en remineraliserende virkning slik at de er istand til å frigi både kalsiumioner og fosfationer.
I tannpleiemidlene i samsvar med oppfinnelsen anvendes to typer fosfater. For det første er cykliske fosfater tilstede, som synergistisk øker innvirkningen av fluorforbindelser, og videre lineære fosfater som er tilstått for å øke remineraliseringsprosessen.
Egnede lineære fosfater er fosforsyre, natriumortofosfat, natriumtripolyfosfat, natriumtetrapolyfosfat, natriumpyrofos-fat, lineært natriumheksametafosfat, etc.
Fosfater med en vannoppløselighet på mer enn 5% er spesielt egnet.
De lineære fosfater anvendes ofte, men ikke nødvendigvis i konsentrasjon mindre enn 5%. Foretrukket anvendes de i et støkiometrisk forhold til kalsium, som forekommende i hydroky-apatitt, det vil si 1,66 Ca : 1P.
Kalsiumionene er også vesentlige for remineraliseringsprosessen, endog mer enn fosfationene, på grunn av at i deminerali-seringsprosessen oppløses kalsium først fra hydroksy-apatit-ten. Demineralisert tannemalje, og også svakt kariesangrepet emalje har også faktisk, i forhold til et nedsatt fosfatinn-hold, et mye mer nedsatt innhold av kalsiumioner.
Tilførselen av kalsiumioner i en effektiv form ved hjelp av tannpleiemidler er ikke en enkel sak. Kalsiumioner er for-foldsvis reaktive med de konvensjonelle bestanddeler i disse produkter og kan miste sin virkning tilsynelatende fullstendig ved kjemisk kombinering med disse bestanddeler.
Kalsium kan således ikke enkelt tilsettes til tannpleiemidler i nærvær av fosfationer, på grunn av at de to vil reagere med hverandre til å danne kalsiumfosfat, som neppe er effektivt. Av samme grunn kan kalsiumioner ikke enkelt kombineres med fluoridioner, på grunn av utfellingen av kalsiumfluorid, som er tilsynelatende ueffektiv. Problemet med å tilsette kalsiumioner i en effektiv form til tannpleiemidler blir ennå større hvis natriumtrimetafosfat er tilstede, på grunn av at denne forbindelse er en meget god kalsiumbindende bestanddel.
Følgelig er et overskudd av kalsiumholdige forbindelser ikke særlig mulig, på grunn av at fluoridet og fosfatet da blir innaktivert, men lav konsentrasjon av en kalsiumdonor er også problematisk, på grunn av at i dette tilfelle vil alt kalsium bli innfanget av trimetafosfatet.
Andre bestanddeler er også istand til å deaktivere kalsium, men det foregående er nevnt på grunn av at deres skadelige virkning er dobbelt. Kalsium gjør det kariostatiske fosfat og fluoridionene innaktive og motsatt gjør disse materialer de remineraliserende kalsiumioner ineffektive.
Det er overraskende funnet at spesifikke kalsiumkomplekser er istand til å opprettholde kalsium i en aktiv form i tannpleiemidler uten derved at andre aktive materialer deaktiveres.
Det er et formål for den foreliggende oppfinnelse å tilveie-bringe tannpleiemidler med optimale remineraliseringsegenskaper og inneholdende kalsiumioner så vel som fosfationer og fluorforbindelser i aktiv form.
Det er funnet at kalsiumsaltet av organiske syrer, som f.eks. kalsiumsaltene av sitronsyre, adipinsyre, og vinsyre tilfred-stiller de anførte krav. Kalsiumforbindelsene anvendes foretrukket i et mengdeforhold på 0,05 til 5 vekt%.
Det er videre funnet at pH i tannpleiemidlene påvirke remineraliseringsprosessen. Optimal utbytting er mulig i svakt surt medium. Beste resultater er oppnådd ved pH 6-7. Effektiviteten av de forskjellige tannpleiemidler ble målt ved hjelp av måling av kritisk vinkel på tannoverflaten. Ved denne metode uttrykkes graden av overflate-remineralisering tannmessig.
Ved denne måling bestemmes den kritiske vinkel av en vanndråpe påført et fullstendig rengjort stykke av tannemalje som er blitt slipt til ensartet jevnhet. En første måling foretas før testen. En annen måling foretas etter at tannen er blitt demineralisert med en 0,1 M melkesyre/hydroksy-etylcellulose-oppløsning i 10 minutter. Denne demineralisering nedsetter den kritiske vinkel gjennomsnittlig 3 0 til 3 5°. Deretter bringes tannen i 30 minutter i kontakt med en 3 0% suspensjon av tannpasta eller en oppløsning av et eller flere tannpleiemidler, hvoretter den kritiske vinkel på nytt måles.
For å bestemme den resterende effekt etter behandlingen legges tannen deretter i en kunstig spyttoppløsning og prosessen med overflate-remineralisering bestemmes med regulære tidsmellom-rom ved hjelp av målinger av den kritiske vinkel.
Ved å gå.ut fra en pasta som angitt i det følgende eksempel 1 ble følgende pastaer sammensatt: A. Pasta i samsvar med eksempel 1, men uten de aktive komponenter .
B. Pasta A pluss natrium-monofluorfosfat.
C. Pasta A med natrium-monofluorfosfat og natriumfluorid.
D. Pasta C pluss natriumfosfat og fosforsyre.
E. Pasta C med natriumtrimetafosfat.
F. Tannpasta i henhold til C med natriumfosfat/fosforsyre og
kalsiumci trat.
G. Tannpasta i henhold til C med natriumtrimetafosfat og kalsiumci trat.
H. Tannpast i samsvar med C med fosfat/fosforsyre og natrium-trimetaf osf at og kalsiumcitrat.
Resultater
1. Endring av kritisk vinkel eller 3 0 minutters behandling med tannpastaen som testes. 2. Endring av kritisk vinkel etter en times lagring i en kunstig spyttoppoløsning. 3 . Endring i kritisk vinkel målt fra demineralisering opptil og inklusive behandling med kunstig spytt.
I kolonne 3 er den totale virkning indikert. Blandingen F, G og H i samsvar med oppfinnelsen synes å gi overlegne resultater .

Claims (3)
1. Tannpleiemiddel med antikaries-virkning, karakterisert ved at det inneholder: minst ett fluorid og minst ett monofluorfosfat som
fluorforbindelser, og en kombinasjon av minst to forskjellige fosfatforbindel
ser hvorav minst den ene er et cyklisk trimetafosfat, og minst ett kalsiumsalt av en organisk syre som avgir
kalsiumioner til tannemaljen.
2. Tannpleiemiddel som angitt i krav 1, karakterisert ved at de ovennevnte fluorforbindelser er foretrukket natriumfluorid og natrium-monofluorfosfat, de ovennevnte fosfatforbindelser er foretrukket cyklisk natrium-trimetafosfat og et lineært ortofosfat og/ eller et lineært tri-polyfosfat og ovennevnte kalsiumsalt av en organisk syre er foretrukket kalsiumcitrat, kalsiumadipat eller kalsiumtartrat, eller en blanding av to eller flere av disse forbindelser.
3. Tannpleiemiddel som angitt i krav 1-2, karakterisert ved at forholdet mellom natriumfluorid og natrium-monofluorfosfat beregnet som F er mellom 1:1 og 4:1, foretrukket 3 :1.
