Description
Foreliggende oppfinnelse vedrører .en., fremgangsmåte til fremstilling av en stabil, i det vesentlige ikke-toksisk, olje-i-vannemulsjon som inneholder minst tre hovednæringsstoffer,
dvs. fett, karbohydrat og aminosyrer,-. f or parenteral injeksjon.
Et daglig inntak av matvarer eller næringsmidler hos pasienter er av stor betydning for opprettholdelse av en god helsetilstand. Når oral tilførsel av mat er umulig som tilfelle er for geriatriske pasienter eller pasienter som befinner seg i en pre- eller post-operasjonstilstand, må nødvendige næringsmidler og kalorikilder gis parenteralt.
Flere fettemulsjoner, vandige karbohydrat-infusjons-oppløsninger og vandige aminosyre-infusjonsoppløsninger har hittil vært tilgjengelige for intravenøs administrasjon av næringsmidler eller kalorikilder, og en blanding av disse preparater har hatt klinisk anvendelse. Deri kliniske anvendelse av blandingen av nevnte preparater er imidlertid ufordelaktig på grunn av at en stor mengde av blandingen må .administreres til pasientene for å opprettholde den nødvendige tilgang på næringsstoffer og kalorier. Intravenøs administrasjon av en stor mengde av nevnte blanding forårsaker dessuten flere bivirkninger slik som kulde, oppkast og smerte i brystet og ryggen. Vidére har nesten alle kjente fettemulsjoner blitt fremstilt
ved å anvende et ikke-ionisk overflateaktivt middel (f.eks. polyoksyetylensorbitanoleat, sorbitanmonooleat) eller soya-bønne- fosfolipider som emulgeringsmiddel. Disse kjente emul-sjoner er ikke stabile fordi fettpartikler i emulsjonen bevirker koalescering eller delvis nedbrytning i nærvær av elektrolytt slik som L-arginin, L-lysin, L-aspartinsyre eller L-glutaminsyre. Di=se fettemulsjoner kan følgelig ikke blandes med en vandig
aminosyre-infusjonsoppløsning uten betydelig forringelse av emulsjonen eller økning av fettpartiklenes diameter. Det er kjent at fosfolipider fra eggeplomme har dårlig emulgeringsevne skjønt deres bivirkninger slik som hypotensiv aktivitet, er mindre enn bivirkningene fra soyabønne-fosfolipider.
Vest-tysk patent nr. 1.792.294 beskriver fremstilling av en nærende, pyrogenfri fettemulsjon ved emulgering av en vandig blanding av fett, karbohydrat og aminosyrer under anvendelse av soyabønne-fosfolipider under høyt trykk. Denne emulsjon er imidlertid ikke tilgjengelig som en bedre fett-emuls jon for fullstendig intravenøs næringstilførsel. F.eks., selv om det er kjent at L-arginin gir den kraftigste eller beste detoksikering overfor blodammoniakk hos levende individer, kan denne forbindelse ikke tilsettes til emulsjonen som fremstilles i ovennevnte patent fordi den har en forringende virkning på emulsjonens fettpartikler. Ifølge metoden i det tyske patent må derfor L-ornitin, som er dårligere enn L-arginin med hensyn . til detoksikering overfor blodammoniakk, anvendes i steden for L-arginin som en av de aktive bestanddeler. L-glutaminsyre og
L-aspartinsyre tilsettes dessuten til emulsjonen i patentet.
Det er nå funnet at langkjedede fettsyrer eller deres basiske aminosyresalter er egnet for forbedring og forøkning av den emulgerende evne hos eggeplomme-fosfolipider. Det er også funnet at det kan fremstilles en stabil olje-i-vannemulsjon hvori fettpartiklene har en gjennomsnittlig diameter, på mindre ennO,? ym,ved emulgering av en blanding av vann og fett under anvendelse av en langkjedet fettsyre eller dens basiske aminosyresalt samt eggeplomme-fosfolipider. Det er videre funnet at den således oppnådde olje-i-vannemulsjon er meget stabil i og med at den ikke ødelegges i nærvær av elektrolytt slik som L-arginin, L-lysin, L-aspartinsyre eller L-glutaminsyre.
Formålet med foreliggende oppfinnelse er å tilveiebringe en fremgangsmåte hvorved det anvendes en blanding av en langkjedet fettsyre eller dens basiske aminosyresalt og eggeplomme-fosfolipider som emulgeringsmiddel. Et annet formål med oppfinnelsen er å tilveiebringe en fremgangsmåte til fremstilling av en olje-i-vannemulsjon som inneholder minst tre hovednærings-stof fer, dvs. fett, karbohydrat og aminosyrer, i et passende næringsforhold. Et annet formål er å tilveiebringe en stabil fettemulsjon hvori fettpartiklene har en gjennomsnittlig diameter på under 0,5 ym, spesielt mindre enn 0,3 ym. Et ytterligere formål er å tilveiebringe en vesentlig ikke-toksisk fettemulsjon"som er egnet for bruk som kilde for næringsstoffer og kalorier hos
pasienter som krever intravenøs ernæring.
Ifølge foreliggende oppfinnelse er det således tilveie-brakt en fremgangsmåte til fremstilling av en olje-i-vannemulsjon for parenteral injeksjon, hvorved man dispergerer en vandig blanding inneholdende 1-15 vekt/volum % fett, 1-30 vekt/volum % karbohydrat og 1-15 vekt/volum % aminosyrer, respektivt be-regnet på emulsjonen, under anvendelse av en homogenisator under et trykk på 100-550 kg/cm 2 hvorved det oppnås en olje-i-vannemulsjon hvis féttpartikler har en gjennomsnittlig diameter på 0,2-0,5 ym, og denne fremgangsmåte er kjennetegnet ved at emulgeringen foretas ved hjelp av en blanding av 0,05-0,30 g eggeplomme-fosfolipider pr. g fett og 0,02-3,0 g fettsyre med 12-20 karbonatomer eller dens arginin-, lysin- og ornitinsalter pr.
g fosfolipider.
Egnede langkjedede fettsyrer er f.eks. mettede fettsyrer slik som palmitinsyre, stearinsyre, laurinsyre og myristin-syre, og umettede fettsyrer slik som oljesyre, linolsyre, linolensyre og arachidonsyre. De umettede fettsyrer som er nevnt ovenfor er spesielt egnet for oppfinnelsens formål.
Den nærende fettemulsjon som fremstilles ifølge oppfinnelsen kan oppnås på konvensjonell måte. Fett, eggeplomme-fosfolipider og en langkjedet fettsyre eller dens basiske aminosyresalt kan f.eks. dispergeres i vann, og den vandige dispergerte oppløsning emulgeres. Karbohydrat og aminosyrer tilsettes til emulsjonen og den således oppnådde vandige blanding blir emulgert på nytt. Den nærende fettemulsjon ifølge oppfinnelsen oppnås derved. Den nærende fettemulsjon kan alternativt fremstilles ved dispergering av fett, karbohydrat, langkjedet fettsyre eller dens salt, samt eggeplomme-fosfolipider i vann, og deretter foreta emulgering av den vandige blanding, tilsetning av aminosyrer til blandingen og deretter emulgere den således oppnådde vandige blanding. En foretrukket mengde av eggeplomme-fosfolipider er 0,1-0,25 g pr. g fett. Fra et praktisk syns-punkt er det foretrukket å benytte godt rensede, pyrogenfrie, eggeplomme-fosfolipider. En foretrukken mengde langkjedet fett syre eller dens basiske aminosyresalt er 0,05-2,5 g pr. g fosfolipider. Emulgeringen kan utføres i en vanlig homogenerings-innretning. Anordninger hvori dispergering f.eks. bevirkes ved å føre blandingen som skal emulgeres gjennom et lite hull under høyt trykk, kan f.eks. benyttes for oppfinnelsens formål. Egnede eksempler på slike anordninger er de som beskrives i "Emulsion: Theory and Practice", side 227-230 (1957) (utgitt av Teinhold Publishing Corporation, New York, U.S.A.).
Fettpartikler i en olje-i-vannemulsjon for intravenøs
bruk bør ha en gjennomsnittlig diameter på under 0,5um, spesielt mindre enn, 0,3ym, fordi emulsjonen ellers kan forårsake ugunstige virkninger ved mikrosirkulering i blodkapilærene.
Alle aminosyrer som vil være nødvendige for pasienter
ut fra et ernæringssynspunkt, kan tilsettes til den nærende fremstilte fettemulsjon i hvilke som helst mengdeforhold. To eller flere aminosyrer valgt fra gruppen bestående av L-isoleucin, L-leucin, L-lysin, L-valin, L-metionin, L-treonin, L-tryptofan, L-fenylalanin, L-alanin, L-prolin, L-glutaminsyre, L-aspartinsyre,
L-serin, L-arginin, L-histidin, L-tyrosin, L-cystein, L-cyctin
og glycin, kan f.eks. med fordel tilsettes. Ved fremstilling av den nærende fettemulsjon kan dessuten forskjellige fetter slik som sesamfrøolje, peanøttolje, olivenolje, safflorolje og soyabønne-olje anvendes som en av de tre hovednæringsstoffer. Blant disse oljer er soyabønneolje mest egnet på grunn av dens lave toksisitet sammenlignet med de andre oljetypene. Karbohydratet som an-
vendes i foreliggende oppfinnelse bør være et som ikke reagerer med aminosyrer. For dette formål er det foretrukket å anvende sukkeralkohol, slik som xylitol eller sorbitol. Konsentrasjoner av aminosyrer, fett og karbohydrat i den nærende fettemulsjon er som nevnt 1-15 vekt/vol ■%, 1-15 vekt/vol % og 1-30 vekt/vol %, respektivt, basert på emulsjonens totale volum. De optimale vektforhold mellom disse tre hovednæringsstoffer og mellom de ovenfor nevnte aminosyrer som skal være tilstede i emulsjonen,
kan lett bestemmes ut fra forskjellige undersøkelser med hensyn til ernærende og kalorimessige forhold hos pattedyr, slik som beskrevet i Nutr. Metabol., 14, 1-170 (1972) (A. Wretlind).
Om nødvendig kan vitaminer, mineraler, stabilisatorer og tera-peutisk verdifulle stoffer tilsettes til emulsjonen. I alle til-feller viser den fremstilte nærende fettemulsjon tilstrekkelig
stabilitet over et lengere tidsrom.
Praktiske og foretrukne utførelsesformer av foreliggende oppfinnelse illustreres i følgende eksempler. Den benytt-ede betegnelse "nærende fettemulsjon" gjelder en olje-i-vannemulsjon som inneholder minst tre hovednæringsstoffer, dvs. fett, karbohydrat og aminosyrer. Når det snakkes om vann i de nedenstående forsøk og eksempler, menes destillert vann for injeksjon som tilfredsstiller den standard som er angitt i U.S. Pharma-copoeia, 18. utgave.
Forsøk I
120 g eggeplomme-fosfolipider og 1000 g xylitol dispergeres i 3 liter vann under kraftig omrøring. 1000 g soyabønneolje tilsettes til den dispergerte oppløsning. Den dispergerte oppløsning omrøres kraftig og vann tilsettes for å bringe det totale volum til 10. Den dispergerte oppløsning emulgeres deretter under anvendelse av en homogenisator (Maton-Gaulin Manufacturing Co. Inc., Model M3). Trykket reguleres til 350 kg/cm p i emulgeringens første trinn og til 100 kg/cm i det andre trinnet. 10 liter fettemulsjon oppnås på denne måte.
Vandige aminosyreoppløsninger fremstilles ifølge de nedenstående sammensetninger A, B, C, D og E, respektivt. 10 liter av hver av de vandige oppløsninger blandes med 10 liter av den ovenfor oppnådde fettemulsjon. Blandingen emulgeres på samme måte som beskrevet ovenfor, påfylles på prøveflasker og steriliseres deretter ved 120°C i 30 minutter. Prøver A, B,
C, D og E av nærende fettemulsjoner blir dermed oppnådd. Umiddel-bart etter at prøvene A - E er oppnådd, blir størrelsen av fettpartiklene studert mikroskopisk. De resulterende observasjoner er angitt i tabell 1.
S ammensetning B
0,2 vekt/vol % av L-glutaminsyre tilsettes til sammensetning A.
Sammensetning C
0,2 vekt/vol % av L-aspartinsyre tilsettes til sammensetning A.
Sammensetning D
0,2 vekt/vol % L-arginin tilsettes til sammensetning A.
Sammensetning E
0,2 vekt/vol % L-lysinacetat tilsettes til sammensetning A.
Som det fremgåi? fra det ovenfor anførte observeres mange store fettpartikler med diameter på opptil 12y i prøvene B-E, skjønt diameteren på fettpartikler i prøve A er mindre enn 0,5ym. Dette skyldes at de nærende fettemulsjoner som er oppnådd ovenfor, ikke er stabile nok til å unngå at emulsjonens fettpartikler koalescerer eller delvis ødelegges i nærvær av L-glutaminsyre, L-aspartinsyre, L-arginin eller L-lysinacetat.
Forsøk II
2000 g sorbitol og 240 g eggeplomme-fosfolipider dispergeres i 3 liter vann under kraftig omrøring. an bestemt
mengde fettsyre eller dens aminosyresalt som angitt i tabell 2. og 1000 g soyabønneolje tilsettes til den dispergerte oppløsning. Den dispergerte oppløsning omrøres kraftig og vann tilsettes for å bringe det totale volum til 10 liter. Den dispergerte opp-løsning emulgeres deretter ved hjelp av en homogenisator (Manton-Gaulin Manufacturing Co. Inc., Model M3). Trykket reguleres til 350 kg/cm i emulgermgens første trinn og til 100 kg/cm 2 i det andre trinnet. Derved oppnås 10 liter fettemulsjon.
En vandig aminosyreoppløsning fremstilles og har følgende sammensetning: 10 liter av den vandige aminosyreoppløsning blandes med 10 liter av den ovenfor oppnådde fettemulsjon. Blandingen emulgeres på samme måte som beskrevet ovenfor, fylles på prøve-flasker som deretter steriliseres ved 120°C i 20 minutter.
Når en nærende fettemulsjon fremstilles som angitt ovenfor observeres fettpartiklenes størrelse mikroskopisk. Resultatene fra denne observasjon er angitt i tabell 2.
Fra de ovenfor angitte data fremgår det klart at når
en vandig dispergert oppløsning av soyabønneolje og sorbitol emulgeres ved hjelp av fettsyre eller dens salt samt eggeplomme-fosfolipider, blir den oppnådde nærende fettemulsjon stabil nok til at dens fettpartikler ikke koalescerer eller ødelegges i nærvær av L-arginin, L-lysin, L-aspartinsyre eller L-glutaminsyre .
Eksempel 1
40 g eggeplomme-fosfolipider og 1000 g xylitol dispergeres i 3 liter vann under kraftig omrøring. 50 g'linolsyre og 400 g soyabønneolje tilsettes til den dispergerte oppløsning. Oppløsningen omrøres kraftig og vann tilsettes for å bringe det totale volum til 5 liter. Den dispergerte oppløsning emulgeres deretter ved bruk av homogenisatoren (Marton-Gaulm Manufacturing Co. Inc., Model M3) . Trykket reguleres til305 kg/cm 2 i emulgeringens første trinn og til 100 kg/cm i det andre trinnet. Dette gir 5 liter fettemulsjon. 5 liter av en vandig aminosyreoppløsning fremstilles ut fra følgende sammensetning: 5 liter av den vandige aminosyreoppløsning blandes med 5 liter av den ovenfor oppnådde fettemulsjon. Blandingen emulgeres på samme måte som beskrevet, fylles på prøveflasker og steriliseres deretter ved 120°C i 20 minutter. Dette gir en nærende fettemulsjon. Fettpartikler i emulsjonen viser en gjennomsnittlig diameter på 0,3um.
Eksempel 2
En nærende fettemulsjon tilveiebringes på samme måte som beskrevet i eksempel 1 med unntagelse av at 75 g L-lysin-oleat anvendes i steden for 50 g linolsyre. Fettpartikler i den nærende fettemulsjon viser en gjennomsnittlig diameter på 0,3um.
Eksempel 3
50 g eggeplomme-fosfolipider dispergeres i 3 liter vann under kraftig omrøring. 61,5 g L-arginin oppløses i den dispergerte oppløsning. 20 g linolsyre, 20 g linolensyre og 500 g
soyabønneolje dispergeres i nevnte dispergerte oppløsning under kraftig omrøring og vann tilsettes for å bringe det totale volum til 5 liter. Den dispergerte oppløsning emulgeres deretter under anvendelse av en homogenisator (.Manton Gaulin Manufacturing Co. Inc., Model M3). Trykket reguleres til 400 kg/cm<2>i emulgeringens første trinn og til 100 kg/cm^ i det andre trinnet. Dette gir 5 liter fettemulsjon. 5 liter av en vandig karbohydrat- og aminosyreoppløsning fremstilles ut fra følgende sammensetning:
5 liter av den vandige karbohydrat- og aminosyreopp-løsning blandes med 5 liter av den ovenfor oppnådde fettemulsjon. Blandingen emulgeres på samme måte som beskrevet, påfylles
på prøveflasker og steriliseres deretter ved 120°C i 20 minutter. Dette gir en nærende fettemulsjon hvis fettpartikler viser en gjennomsnittlig diameter på 0,3um.
Claims (2)
1. Fremgangsmåte til fremstilling av en olje-i-vannemulsjon for parenteral injeksjon, hvorved man dispergerer en vandig blanding inneholdende 1-15 vekt/volum % fett, 1-30 vekt/volum % karbohydrat og 1-15 vekt/volum % aminosyrer, respektivt be-regnet på emulsjonen, under anvendelse av en homogenisator under et trykk på 100-550 kg/cm 2, hvorved det oppnås en olje-i-vannemulsjon hvis fettpartikler har en gjennomsnittlig diameter på 0,2-0,5um,karakterisert vedat emulgeringeh foretas ved hjelp av en blanding av 0,05-0,30 g eggeplomme-fosfolipider pr. g fett og 0,02-3,0 g fettsyre med 12-20 karbonatomer eller dens arginin-, lysin- og ornitinsalter pr. g fosfolipider .
