Description
Thermoplastisch materiaal voor orthopedische en chirurgische toepassingen.
De onderhavige uitvinding heeft betrekking op een thermoplastisch polymeer materiaal ten behoeve van orthopedische en chirurgische spalken verbandmiddelen en van hulpmiddelen voor protheses.
Het materiaal dient in geschikte vorm als steunmiddel en dient daarom aan speciale eisen te voldoen, die niet aan andere kunststofartikelen worden gesteld.
Om de nadelen van gipsspalken te ondervangen,werden reeds polymere kunststoffen voor deze doeleinden voorgesteld.
In het U.S. Patent 3,692,023 werden daartoe polymeren of mengsels van poly- <£.-caprolacton en polyvinylethylether beschreven, die op weefsels of gaas kunnen worden aangebracht.
Dit materiaal moest dan in verwarmde toestand om gebroken ledematen worden gewikkeld en vormde na afkoeling een betrekkelijk harde steun^die door verwarming manueel weer kon worden afgenomen.
In het U.S. Patent 4,175,177 werden orthopedische verbanden beschreven, bestaande uit copolymeren van £-caprolacton en acrylaten.
Om buig- en treksterkte en elasticiteit van het materiaal reproduceerbaar te beheersen,werd naar geschikte materialen gezocht.
De reeds bekende materialen werden bestudeerd en vergeleken.
Hieruit bleek,dat thans de volgende eisen moeten worden gesteld: 1) Thermoplastische kunststof zonder steunmateriaal toe te passen.
2) Gemakkelijk te vervormen en aan te brengen.
3) Materiaal opnieuw te gebruiken.
4) Drukvastheid.
5) Elasticiteit.
6) Reksterkte.
Het verschijnsel van elasticiteit en reksterkte hangt evenals de drukvastheid af van de kristalliniteitsgraad van het polymeer, van de verknoping van de molecuulketens en van de vorm daarvan.
Een geeigend materiaal voor chirurgische en orthopedische toepassingen, bestaande uit een composiet van lagen van poly-£-caprolacton en een polyester met een deklaag van polyurethanschuim met open cellen werd reeds beschreven in Europ.Octrooi 0 263 552 t.n.v. aanvrager.
Een nieuw mengsel van copolymeren met weekmaker werd ontwikkeld.
Hierbij werd gebruik gemaakt van uit fundamenteel onderzoek verkregen inzichten in de structuur van het materiaal.
Het materiaal,als in de aanhef vermeld,is volgens de onderhavige uitvinding gekenmerkt,doordat men equimolaire gewichtshoeveelheden £-caprolacton, polyurethan van polyoxypropeenglycol en tolueen-di-iso-cyanaat, en etheen-buteen-styreen-copolymeer onder verwarming co-polymeriseert,en tijdens de vorming van het copolymeer triallyl-cyanuraat in een hoeveelheid van 0.3 tot 3.5 gew.-%, betrokken op de totale massa, alsmede ca. 1 gew.-% oliezuurimide als weekmaker toevoegt, en het product in de vorm van blokken of platen met gammastralen tot 2.5 Mrad bestraalt.
Met deze werkwijze wordt een materiaal verkregen,dat door een partiële verknoping (cross-linking) van gebundelde ketenvormige macromoleculen en een eveneens partiële kristalliniteit (5-10%), als gevolg van een isotactische structuur gemakkelijk in Teflongecoate spuitgietmatrijzen kan worden verwerkt tot de voor orthopedie en chirurgie gewenste vormen.
In een gedeelte van de grondstoffen kan een weinig water worden toegevoegd, waardoor een zacht opgeschuimd product ontstaat,dat op het oppervlak kan worden gekleefd.
De verwerkingstemperatuur ligt, afhankelijk van het weekmakergehalte, tussen b5° en 180° C.
Elasticiteit, kristalliniteit en rek van het materiaal hangen af van de deformatie-enthalpie.
Een maat hiervoor is door Mooney en Rivlin aangegeven (Journal of Applied Physics 11 (1940), 582 e.v.).
Een overzicht hiervan wordt weergegeven in "Elasticity and Structure of Polyurethane Networks", Dissertatie van R.Blokland, T.H. Delft (1968).
Een gerekt elastomeer als het onderhavige copolymeer beantwoordt aan de vergelijking van Mooney en Rivlin: F = (C,+ r'.C2). (λ- λ'2), waarin: I- 2 1 F = trekkracht J^dyn/cm J- C/ = constante voor basispolymeer j^kg/cm^ ^ ·* C^= constante voor copolymeer ^kg/cm^J.
= rek en de deformatie-enthalpie van elk bestanddeel is: aG = |.C. (λ* + Xy +λ2ζ -3), waarin £ G= deformatie-enthalpie.
C= constante voor bestanddeel, rek in richting x.
^ = rek in richting y.
^ = rek in richting z.

De elastische deformatie leidt tot verhoging van de kristalliniteit en van de deformatie-enthalpie (verlaging van de entropie).
Voor de polyurethan-copolymeren met etheen-buteen-styreen en caprolacton gelden bij een temperatuur van 25° C waarden voor C,=3,3 kg/cm^ en 2 C2= 2.00 kg/cm , hetgeen door de toevoeging van triallylcyanuraat en oliezuurimide gevarieerd kan worden.
Door de bestraling met gammastralen wordt een tegengesteld effect verkregen, zodat elke gewenste mechanische eigenschap kan worden bereikt. Belangrijk is,dat het materiaal bij toepassing goed op de vereiste plaats blijft (niet schuift en niet deformeert,als er geen kracht op wordt toegepast), en dat het niet op de huid kleeft.
Het volgens de onderhavige uitvinding ontwikkelde materiaal heeft door zijn in het voorafgaande beschreven bijzondere moleculaire en kristallijne structuur een "plastisch geheugen", zodat het zich na aanpassing in de vereiste vorm (van b.v. spalken e.d.) bij iets verhoogde temperatuur weer in zekere mate soepel en zacht aandrukt aan het betreffende lichaamsdeel. Het product is in zijn toepassing niet beperkt tot de orthopedische en chirurgische gebruiksmogelijkheden,doch kan ook worden gebruikt voor technische doeleinden,zoals de vervaardiging van filters, membranen en zeven, omdat het in dunne lagen naar wens poreus en specifiek doorlatend kan worden gemaakt.
Belangrijk is voorts de mogelijkheid van recycling,omdat het als thermoplastisch polymeer gegranuleerd en door extrusie of spuitgieten geheel opnieuw gevormd kan worden.
De onderhavige uitvinding heeft betrekking op vervaardiging, verwerking · en toepassing van het beschreven materiaal.
Voor speciale paramedische toepassingen (zoals steunzolen en dergelijke hulpmiddelen) kan men de kunststofschuimlaag geheel of gedeeltelijk vervangen door het oppervlak te behandelen met een suspensie van een neutraal schuurmiddel, zoals bij voorbeeld puimsteenpoeder.
De chemische structuren van de organische bouwstenen van het copoly-meermengsel worden weergegeven op het bijgaande formuleblad met de formules (1) t/m (5).
Aangezien polycaprolacton aeroob-biodegradeerbaar is, is het product volgens de onderhavige uitvinding duidelijk milieuvriendelijk.
Claims (3)
1. Werkwijze voor het bereiden van een vervormbaar materiaal voor orthopedische en chirurgische toepassingen op basis van poly-£-caprolacton, met het kenmerk, dat men equimolaire gewichtshoeveel-heden £-caprolacton, polyurethan van polyoxy-propyleenglycol en tolueendiisocyanaat, en etheen-buteen-styreen-copolymeer onder verwarming met elkaar laat reageren en tijdens de reactie triallyl-cyanuraat in een hoeveelheid van 0.3 tot 3.5 gew.-% en oliezuur-imide in een hoeveelheid van ca. 1 gew.-%, betrokken op de totale massa, toevoegt, en het product in de na de reactie verkregen vorm aan bestraling met gammastralen tot 2.5 Mrad blootstelt.
2. Werkwijze voor het vervaardigen van orthopedische en chirurgische spalk- of steunmiddelen van thermoplastische kunststof, met het kenmerk, dat men een product,verkregen volgens conclusie 1,in de gewenste vorm brengt.
3. Orthopedische en chirurgische hulpmiddelen voor het fixeren van lichaamsdelen na fracturen en operaties, met het kenmerk, dat deze geheel of ten dele uit materiaal,vervaardigd volgens conclusie 2, bestaan. Formuleblad I
