Abstract

Description
본 발명은 생명공학의 일반 분야에 관한 것으로, 구체적으로는 골관절 및 자가면역 질환의 치료를 위한 재생 의학 분야 및 인간 중간엽 세포, 특히 세포외 소포를 기반으로 한 치료제의 제조에 관한 것이다.
현재, 세포 치료는 연구와 임상 적용 사이의 가교 역할을 하며, 기능을 회복하기 위해 인간 세포, 조직, 또는 기관을 대체, 재생 또는 변형하기 위한 조직 공학과 관련하여 진보를 이루고 있다. 이러한 변화는 줄기 세포, 주로 중간엽 줄기 세포(MSC)를 사용하는 임상 시험의 수가 증가함에 따라 동반되고 장려된다. 세포 치료 분야가 다양한 의료 응용 분야에서 급속히 발전했지만, 한편으로는 많은 사람들과 새로운 분야에 도움을 줄 수 있도록, 이러한 기술의 비용을 낮추고, 더 효율적이고 안전하게 만들 수 있도록 개발 과정에 더 나은 솔루션이 필요한 몇 가지 기술적 문제가 있다.
이와 관련하여 보다 안전하고 비용이 적게 드는 최근의 접근법은 세포 배양 중에 세포에서 자연적으로 분비되는 소포를 사용하는 것인데. 소포가 무세포 치료법에 대한 큰 잠재력을 가지고 있는 것으로 나타났기 때문이다. 연구자들은 세포 간 의사소통에서 중요한 역할을 하고, 근본적으로 단백질과 RNA에 해당하는 내용물을 통해 질환의 형성 또는 조절에 영향을 미치는 것을 포함하여, 다양한 분야에서의 이러한 소포의 기능을 연구했다. 특히 이러한 소포에는 2000 내지 3000가지 유형의 마이크로RNA 또는 miRNA가 많이 포함되어 있는 것으로 밝혀졌다. 이러한 분자는 비암호화 RNA 가닥으로, 유전자 발현 조절에 중요한 역할을 한다. 세포외 소포의 하위 집합은 엑소좀(30 내지 200 nm)이며, 이미 다양한 약물 전달을 위한 소포로서 사용되어 왔다.
현재, 엑소좀 또는 작은 세포외 소포(sEV)를 포함하는 세포외 소포를 기반으로 한 치료법의 개발은 전임상 연구에서 발전을 이루었으며, 본 발명에서 제안한 것과 같은 임상 적용 가능성이 매우 높다.
최신 기술로는 다양한 진단 적용과 분리된 엑소좀을 사용한 일부 치료법이 있으며, 예를 들어, 국제공개 WO2019038660A1호(Seattle Childrens Hospital DBA Seattle Childrens Res Inst, 2019-02-28)는 환자의 체액에 존재하는 엑소좀을 검사하여 가와사키병을 진단하는 방법을 보호한다.
녹내장과 같은 일부 질환의 치료를 위해 중간엽 세포의 엑소좀을 사용하는 일부 특허 출원(미국 특허 US20190224242A1 호, 문헌[Tomarev et al, 2019-07-25])이 있지만, 해당 문서에서 관찰된 특성과 사용된 조성물은 본 발명의 조성물과는 매우 다르며, 예를 들어, 응용에서 엑소좀은 마커 CD11c+ 및 CD63+를 갖는 반면, 본 발명에서는 단지 한 가지 차이점을 언급하기 위해 마커 CD63, CD81, 및 CD9가 필요하다.

실시예
실시예 1: 본 발명의 조성물 수득
제대 중간엽 세포를 수득하였고, N-카데린, CD44, RANKL, CD105, CD56, 또는 CD90 마커를 갖는 것으로 확인되었다. 3개 배치의 세포를 얻었으며, 하기 표 1에 중간엽 세포의 마커와 이들 각각에 대한 본 발명의 선택 조건이 나타나 있다.

이렇게 확인된 세포를 상용 배지(고글루코스 DMEM; 5% hPL, 1% L-Glut, 1% Pen/Strep 보충)를 사용하여 총 부피 650 mL의 컨플루언스까지 임의의 중간엽 세포 배양의 일반적인 조건에서 배양했다. 이어서, 세포를 PBS(인산염 완충액, pH 7.2; 290 mOsm/L)로 2회 세척한 후, 유도 배지(고글루코스 DMEM; 1% L-Glut만 보충) 650 mL를 첨가하였다.
48시간 배양 후, 세포로부터 모든 상등액 배지를 수집하고, 650 mL의 유도 배지를 다시 배양액에 첨가하여 48시간의 두 번째 배양을 수행하였다.
각 배양 1 또는 2 후에 수집된 상등액을 600 g 4℃에서 10분간 원심분리하여 불순물을 제거했다.
이전 원심분리의 상등액을 초원심분리기에서 100,000 g 4℃에서 1시간 10분 동안 처리했으며, 상등액의 전체 부피가 처리될 때까지 이 단계를 반복했다. 상등액을 제거하고, 볼텍스를 사용하여 펠렛을 방출했다. 그 후, 가능한 오염 물질을 제거하기 위해 펠릿을 두 번 세척했다. 이를 위해 펠릿을 여과된 PBS에 재현탁하여 최종 부피가 10ml가 되도록 하고, 100,000 g 4℃에서 밤새 원심분리했다. 이어서, 마지막 세척을 수행하여 상층액을 제거하고 펠렛을 최종 부피 10 ml의 링거 젖산(pH 6.5, 273 mOsm/L)에 재현탁시키고, 100,000 g 4℃의 초원심분리기에서 1시간 10분 동안 처리했다. 마지막으로, 상등액을 제거하고, 펠릿을 볼텍스한 후 보관을 위해 분취했다.
다음에, 분취량 입자를 정량화했다. NanoSight NS300을 사용하여 여과된 PBS(1 : 100)로 펠렛을 희석하여 입자 정량화를 수행했다.
3번의 초기 배양으로부터, 본 발명의 분취량 입자 또는 조성물의 4개 배치를 얻었다. 하기 표 2는 수득된 농도와 검증 마커의 존재 측면에서 이러한 입자의 특성을 나타낸다.

수득된 4개 배치를 포함된 miRNA의 동일성 및 상대적 농도와 관련하여 추가로 분석하였으며, 이들 각각에서 약 2500개의 miRNA가 서열 분석되었다. 표 3은 본 발명에서 가장 관련이 높은 miRNA의 농도를 나타낸다. 도 7은 Lot A의 농도를 그래프로 나타낸 것이다.

실시예 2: 골관절염 치료에 대한 본 발명의 조성물의 적용.
골관절 질환(OA)에 대한 본 발명의 조성물의 효과를 평가하기 위해, 실시예 1에서 수득된 조성물을 뮤린 골관절염 모델에서 구별없이 사용하였다.
이러한 모델에서는 0일째와 2일째에 관절(이 경우 무릎)에 콜라게나제를 주사하여 질병을 유발한 다음 7일째와 14일째에는 각각의 치료법을 관절에 주사하여 적용한다. 그런 다음 40일째에 안락사를 실시하고 관절을 분석한다.
이 경우, 총 65마리의 동물을 치료한 3개의 독립적인 연구가 수행되었다(연구 1: 17마리의 동물, 연구 2: 20마리의 동물, 연구 3: 28마리의 동물).
각 그룹은 3개로 세분화하였으며, 위약(PBS, 5μL )을 투여한 첫 번째 대조군, 병변이 유발되었으나 치료제를 적용하지 않아 골관절염 질환(OA)을 유지하는 두 번째 대조군, 그리고 최종적으로 병변이 유발되어 본 발명의 치료법을 적용한 그룹으로 세분화하였다. 부상을 유발하기 위해 5 μL 콜라게나제 유형 VII, 1 U/5 μL를 실험 시작 시와 실험 2일째에 부상 전용(OA) 그룹과 후속 치료 그룹에 대해 관절에 주입했다. 이후, 세 번째 그룹의 동물은 2 × 108 엑소좀의 양이 적용되도록 조정된 본 발명의 조성물을 치료제로 투여 받았고, 실험 시작 후 7일째와 14일째에 5 μL의 용량으로 2회에 걸쳐 치료제를 도포하였다.
생체 내 연구가 완료된 후, 위약, 병변 단독 또는 치료제와 함께 치료된 관절에 대한 분석이 수행되었다. 관절의 상태를 분석하기 위해 2가지 기술이 사용되었는데, 첫 번째는 골 무기질 밀도의 결과를 전달하는 마이크로CT 장비를 이용한 영상 연구이다. 사용된 두 번째 옵션은 골관절염의 뮤린 모델의 임상 징후 지표에 해당하는 OA 조직학 점수가 확립된 치료된 무릎의 조직학적 분석을 수행하는 것이다(문헌[Cosenza, S., Ruiz, M., Maumus, M., Jorgensen, C. & No l, D. Pathogenic or Therapeutic Extracellular Vesicles in Rheumatic Diseases: Role of Mesenchymal Stem Cell-Derived Vesicles. Int. J. Mol. Sci. 18, (2017)]).
지적한 바와 같이, 본 발명자들은 3가지 생체 내 연구를 수행하였고, 실험 요약은 하기 표 4에 제시되어 있다. 생체 내 N°1에서는 결과가 조직학적 분석으로 분석되었고 생체 내 2 및 3의 결과는 마이크로CT 분석을 사용하여 평가되었다.

결과
생체 내 연구 No. 1에 대한 결과는 도 1에 나타냈다. 본 발명의 조성물을 사용한 치료는 병변에 효과가 있는 것으로 관찰되었으며, 즉 관절의 OA 점수는 치료하지 않은 대조군에 비해 감소했으며(도 1a), 이는 각각 연골 교체 또는 연골 재생을 나타내며, 이는 생체 내 모델이 작동하고, 엑소좀이 예방 또는 재생 활성을 나타냄을 모두 검증할 것이다.
이러한 생체 내 연구가 분리된 실험에 해당한다는 것을 이해하면, 이러한 데이터는 마이크로CT로 결과를 분석한 다른 연구를 통해 재확인되었다.
생체 내 연구 N°2(도 2)와 관련하여, 마이크로CT 분석에 따르면, 본 발명의 조성물, 즉 본 발명자들이 생산한 엑소좀은, 병변만 있는 대조군에 비해 골 무기질 밀도 BMD 수치가 더 낮다는 사실(도 2a)로 인해, 골관절염으로 인한 손상을 예방할 수 있음을 알 수 있으며, 위약으로 치료한 건강한 관절과 유사한 값을 얻었다. 특히, 골 무기질 밀도의 영상 분석(도 2b)은 사용된 유도 모델이 상당한 부상을 생성했음을 보여주었지만, 이에도 불구하고 엑소좀은 재생 효과를 생성할 수 있다.
마지막 생체 내 연구인 연구 3의 경우, 도 3에서 볼 수 있듯이, 병변(OA)에 관해 본 발명의 조성물과 UC-MSC 세포의 실시예 1에 따라 수득된 sEV 사이에 유의한 효과가 있음을 관찰할 수 있다. 이전의 생체 내에서와 마찬가지로, 본 발명에 따른 조성물이 골관절염 질환에 대한 예방 또는 재생 효과를 갖는다는 것이 입증되었다.
이러한 결과는 골관절염과 같은 골관절 병변에 본 발명의 조성물을 적용하는 것이 관절의 조직학 연구 및 OA 점수에서 볼 수 있는 병변의 정도뿐만 아니라 골 무기질 밀도에도 긍정적인 영향을 미친다는 것을 입증한다.
실시예 3: 본 발명의 조성물을 이용한 면역억제 분석.
말초 혈액에서 단핵 세포를 분리하고, PHA(20 μg/mL)로 활성화한 후 CTV(1 : 1000)로 염색하여 증식을 평가했다. a) 10 μL의 PBS 대조군 및 b) 10 μL의 비히클에 용해된 총 입자 양이 2 × 108인 본 발명의 조성물(sEV's)의 단일 용량으로 처리한 두 개의 실험 그룹을 고려했다. 두 그룹 모두에서 헬퍼 T 림프구 세포(Th로 약칭; CD4+)의 증식과 조절 T 세포(Treg로 약칭; CD4+FOXP3+CD25+)의 백분율이 나타났다. 도 4는 배양 3일째(도 4a) 및 배양 5일째(도 4b)에 Th 림프구 세포의 증식 백분율을 보여주며, 여기서 본 발명의 조성물로 처리하면 증식이 덜 일어나는 것으로 관찰된다. 도 5는 Treg 세포의 백분율을 보여주며, 이는 본 발명의 조성물(sEV's)로 처리하면 배양 3일째에 이 집단의 백분율이 증가했음을 보여준다.
Th 세포는 적응 면역 반응에서 중요한 역할을 하는 세포 집단에 해당한다. 이러한 세포는 Th1, Th2, 및 Th17로 나뉘며; 이들 모두 사이토카인을 분비해 면역 반응에 관여하는 세포의 증식과 분화를 자극한다. 건강한 대조군보다 OA가 있는 기관에 T 세포가 더 많이 존재한다는 증거가 있다. 반면 Treg는 면역 체계 내에서 항상성과 내성을 유지하고, 다른 CD4+ 및 CD8+ T 세포와 B 림프구 및 수지상 세포도 조절하거나 억제하는 CD4+ T 세포의 하위 집단이다. Treg 세포는 TFG-β, IL-10, 및 아데노신과 같은 면역억제 기능을 가진 분자를 생산할 수 있다.
실시예 4: 본 발명의 조성물을 이용한 대식세포 분극 분석
말초 혈액으로부터 단핵 세포를 분리한 후 음성 선별 키트를 이용하여 존재하는 단핵구를 수득하였다. 이러한 단핵구를 M-CSF 및 GM-CSF 인자를 사용하여 배양하고 대식세포로 분화시켰다. 6일간의 배양 후, a) 대조군 및 b) 총 입자 양이 1 × 109인 본 발명의 sEV 조성물의 단일 용량으로 처리된 두 개의 실험 그룹을 확립하였다.
자극 24시간 후, 유세포 분석을 사용하여 전염증성 대식세포(M1이라고 함; CD86+HLA-DR+) 및 항염증제(M2라고 함; CD206+HLA-DR+)의 백분율을 평가했다. 도 6은 M1(도 6a) 및 M2(도 6b) 마커에 대한 이중 양성 세포의 백분율을 보여준다.
결과는 sEV 처리가 M1 대식세포의 개체수를 감소시키고, M2 대식세포의 개체수를 증가시킨다는 것을 보여준다.
