Abstract
항결핵균감염에서의 켈리마이신 응용의 주요 과정은, 임상 1선 항결핵약물 아이소나이아지드와 리팜피신을 대조그룹으로 사용하여, 절대농도법으로 켈리마이신 항결핵균활성을 측정한다. 그 결과, 켈리마이신의 약제내성균을 포함한 임상분리된 결핵균에 대한 활성이 임상 1선 대조약물 아이소나이아지드와 리팜피신보다 우수하다고 나타났고, 켈리마이신의 결핵균감염질병치료약물제조에 있어서의 용도발굴을 기대할 수 있을 것으로 나타났다.
Description
본 발명은 결핵균 감염치료에서의 매크로라이드계(Macrolides) 항생제의 응용에 관한 것이다.
결핵은 결핵균(MTB)감염으로 인해 발생하는 만성전염병으로, 주로 폐와 연관되며, 전염병 중에서 사망자 수가 가장 많은 단일 질병이다. 결핵은 보통 면역기능이 낮은 사람들에게서 많이 발견되며, AIDS와 관계되는 가장 흔한 기회주의적 감염성 질병의 일종이다. WHO의 보고서에 따르면, 전 세계에서 매년 800-1000만 건의 새로운 결핵 발병 사례가 발생하며, 300~400만 명은 결핵으로 인해 사망하고, 그리고 개발도상국에서 발생하는 환자가 가장 많다고 한다. 또 다른 예측에 따르면, 2000-2020년 기간 중에 약 10억 명의 인구가 결핵에 감염될 것이며, 3500만 명이 결핵으로 사망할 것이라고 한다. 이와 동시에, MTB의 약제내성이 해마다 증가하고 있기에, 이는 전세계적으로 결핵을 제어하는데 있어서 큰 위협이 될 것이다. 중국은 전세계 결핵 발병률이 높은 22개 국가 중 하나로, 활동성 결핵환자의 인구가 세계에서 2위이다. 그리고 높은 감염률, 높은 유병률, 높은 약제내성률과 높은 사망률이 유행의 특징이다. 전국 200만 명의 MTB양성환자 중에서, 1/4이상이 결핵균 약제내성환자다.
현재 임상 상용되고 있는 결핵치료 1선 약물(명확한 항결핵균활성이 있는)은 리팜피신(Rifampicin), 아이소나이아지드(Isoniazid), 스트렙토마이신(Streptomycin), 에탐부톨(Ethambutol), 피라진아미드(Pyrazinamide) 등 5종류가 있다. 그러나 이 약물들의 부작용이 많고, 살균작용이 한계가 있으며, 치료과정이 긴 편으로 일반적으로 6개월 이상 사용해야되기 때문에 환자의 약물 복용 준수성이 낮다. 2선 항결핵약물(결핵균에 대해 억제작용을 한다)은 카프레오마이신(Capreomycin), 에티오너마이드(Ethionamide), 아미노살리실산(Aminosalicylic acid), 사이클로세린(Cycloserine), 시프로플록사신(Ciprofloxacin), 아미카신(Amikacin) 및 카나마이신(Kanamycin) 등이 있다. 그러나 이 약물들의 부작용이 많고, 치료시간이 더욱 길고(18~24개월), 가격이 매우 비싸고 치료율도 더 낮다. 주목할 만한 점은, 임상연구결과, 1선의 항결핵약물 모두 간손상을 유발하며, 예를들면, 아이소나이아지드의 불량 반응의 경우 말초신경염, 간독성, 중추신경계장애 및 과민반응이 있다; 리팜피신의 불량반응은 간독성, 소화불량, 신경계증상 및 과민반응이 있다; 에탐부톨의 불량반응은 주로 시신경독성이다; 피라진아미드의 불량반응은 피부의 황색변색증과 혈뇨산의 증가를 유발한다; 스트렙토마이신의 독성과 세균 약제내성 문제는 이의 응용을 심각하게 제한시키고 있다. 그러나 기타 약물과 함께 사용시 세균 약제내성의 발생율을 감소시킬 수 있기에, 임상적인 응용이 많지는 않지만 여전히 항결핵 1선 약물로 사용되고 있다.[ZHU Shanmei 하이샤 약학 2010, 22 (2); 123-125]. 또 다른 보도에 따르면, 2010년에 518주(株)의 임상분리된 결핵균에 대한 연구에서 1선 약물중 아이소나이아지드에 대한 약제내성율은 최고 53.67%에 달했고, 스트렙토마이신에 대한 약제내성율은 45.95%에 달했다고 한다. 2선 약물중에서는, 오플록사신(Ofloxacin)에 대한 약제내성율은 39.77%, 아미카신에 대한 약제내성율은 15.83%, 카프레오마이신에 대한 약제내성율은 21.81%에 달했다고 한다. 321주에서 1종(蘆) 및 2종 이상의 1선 약물의 약제내성 균주에 대해, 오플록사신에 대한 약제내성율은 최고 57.01%에 달했고, 아미카신과 카프레오마이신에 대한 약제내성율은 각각 25.55%, 33.02%에 달했다고 한다. 217주의 다중 약제내성 균주에서 오플록사신에 대한 약제내성율은 72.35%에 달했다고 한다[LIU Yidian 등 2010 중국 폐결핵예방협회 임상기초전문할술대회총집 p274-275]. 이런 결과는 중국의 결핵제어작업, 특히 다중약제내성결핵제어작업 실행에 있어 매우 중대한 도전이 된다. 그러므로 항결핵균 특히 항다중약제내성 결핵균의 대체제품을 최대한 빨리 찾는 일을 더 이상 미룰 수 없다.
새로운 매크로라이드계 약물 클래리스로마이신(Clarithromycin), 아지트로마이신(Azithromycin), 록시스로마이신(Roxithromycin)은 14-원자 고리 erythromycin의 파생물로, 이들의 항균 작용 메커니즘은 위의 1선, 2선 항결핵약물과 다르며, 이들은 균체내 리보솜의 50S 서브유닛과 가역적으로 결합하여 단백질합성을 방해한다. 중국국내의 어떤 보도에 따르면, 클래리스로마이신의 민감한 결핵균에 대한 MIC가 0.25-2.0 μg/ml, 약제내성균에 대한 MIC가 2.0-32 μg/ml라고 하며, 아지트로마이신 항결핵균의 MIC는 128 μg/ml라고 한다[TANG Shenjie, 항결핵약물연구 최신진전, 중국 결핵예방잡지, 2006년 28권 증간 p1-3]. 해외보도에 따르면, 클래리스로마이신의 M. tuberculosis H37Rv (ATCC 27294)에 대한 MIC는 6 μg/ml이며, 아지트로마이신의 MIC는 95 μg/ml라고 한다[Kanakeshwari Falzari et al: In vitro and in vivo activities o macrolide derivatives against Mycobacterium tuberculosis. Antimicrob. Agents and Chemother. 2005, 49(4): 1447-1454]; 록시스로마이신의 MIC는 64 μg/ml이상이다. 이 종류의 약물은 결핵균감염 임상치료약물에 포함되지 않았지만, 단독 또는 항결핵약물과 함께 사용하여 다중약제내성결핵균 감염치료를 한 임상보도가 있다[XU Li 등, 선젼시 입원환자 항결핵관련 항균약물응용조사 중국 결핵예방잡지 2010, 32(3): 151-154].
연구결과, 16-원자 고리와 14-원자 고리 매크로라이드계 항생제는 유사한 항균 작용 메커니즘을 가지고 있다는 것이 증명되었다. 이 종류의 약물은 결핵균에 대해 활성이 낮다. 예를들어 M. tuberculosis H37Rv (ATCC 27294)의 타일로신(Tylosin)에 대한 MIC는 58.6 μg/ml이며, 스피라마이신(Spiramycin)과 미데카마이신(midecamycin)은 모두 100μg/ml을 초과한다[Kanakeshwari Falzari et al: In vitro and in vivo activities of macrolide derivatives against Mycobacterium tuberculosis. Antimicrob. Agents and Chemother. 2005, 49(4): 1447-1454]. 그러므로, 오늘날까지 국내외적으로 이런 류의 항생제를 결핵임상치료에 사용했다는 관련 보도가 없다.
본 실험실에서 연구제조한 신형 16-원자 고리 매크로라이드계 항생제-켈리마이신(kelimycin)(원 명칭: shengjimycin, bitespiramycin)은 이소발레릴(Isovaleryl) 스피라마이신이 주요 구성요소인 4"-아실화된(Acylated) 스피라마이신으로, 이의 작용 메커니즘은 세균리보솜과 결합하여 단백질합성을 억제하는 것이다. 체내외 실험결과, 켈리마이신은 그람양성균, 특히 특정 약제내성균(예: 베타락탐 내성 황색포도상구균(β lactam resistant Staphylococcus aureus), 에리트로마이신 내성 황색포도상구균(Erythromycin-resistant Staphylococcus aureus) 등)에 효과가 있고, 동종 약물과 뚜렷한 교차내약성이 없다. 이와 동시에, 이는 마이코플라즈마, 클라미디아에 대해 우수한 항균활성이 있고, 일부 그람음성균에도 항균작용이 있다. 또한 톡소플라스마, 레지오넬라균 등에게도 우수한 항균효과를 나타낸다[WANG Yiguang 등, "bitespiramycin 및 이의 항감염성질병에서의 응용", 2003년 12월 23일, 중국발명특허: ZL 200310122420.9]. 켈리마이신은 우수한 조직 삼투성을 보유하고 있어서, 체내에서의 항균활성이 체외에서의 항균활성보다 확연히 우수하며, 잠재적인 면역조절작용도 보유하고 있다. I, II, III기의 임상연구를 통해 켈리마이신은 치료효과가 좋고 안전하게 사용할 수 있는 항생제임을 증명하였다. 본 실험실은 켈리마이신의 잠재적인 효능을 더욱 발굴하여, 켈리마이신의 임상적응증(Clinical indications)과 사용범위를 확대했다.
본 발명의 목적은, 의 임상분리결핵균활성에 대한 일련의 측정검사실험연구를 제공하여, 켈리마이신이 결핵균감염질병을 치료하는 새로운 용도가 될 수 있다는 것을 증명하는 것이다.
본 발명의 주요절차는 절대농도법을 사용하여 임상분리된 결핵균에 대해 켈리마이신의 항결핵균 활성을 측정하고, 임상사용되는 항결핵 1선 약물 아이소나이아지드와 리팜피신을 대조약물로 삼는다. 실험결과, 임상분리된 240주(株)의 결핵균에 대하여 켈리마이신이 활성을 띤 균주는 172주였고 총 유효율은 71.66%였다. 그 중에서 활성이 아이소나이아지드보다 뛰어난 균주가 37주였고, 유효균수에서 21.5%를 차지했다. 활성이 리팜피신보다 뛰어단 균주는 39주였고, 유효균수에서 22.7%를 차지했다. 활성이 이 2개보다 모두 우수했던 균주는 23주였고, 유효균수에서 13.4%를 차지했다. 본 발명의 결과, 켈리마이신이 아이소나이아지드와 리팜피신에 대해 약제내성이 있는 결핵균감염질병치료에 새로운 치료용도를 발굴할 수 있는 가능성을 제시할 수 있음을 나타낸다.
본 발명은 켈리마이신을 유효성분으로 하며 약학적으로 수용할 수 있는 담체와 함께 구성된 조합물의 항결핵균감염약물 제조에서의 응용을 제공하고 있다.
본 발명에서 언급한 응용은 경구 투여 약제형태, 주사 약제형태 또는 기타 적절한 약제형태를 사용할 수 있다.
아래에 제공된 실시예는, 당업자가 본 발명에 대해 더욱 잘 이해할 수 있게 돕기위한 것이며, 어떠한 방식으로도 본 발명을 제한하지 않는다.
구체적인 실시방식 중, 임상 상용되고 있는 항결핵 관련 1선 약물을 대조약품으로 삼고, 켈리마이신 항결핵균 활성 측정에 관한 일련의 연구를 진행하였다. 연구결과, 켈리마이신이 임상분리된 결핵균에 대해 활성이 나타나는 균주의 유효수가 대조그룹보다 모두 우수하였다. 이는 특정 약제내성 결핵균감염질병치료에서 응용되었다.
<실시예 1> 결핵균 표본의 획득과 처리
WHO에서 발표한 국제 WS288-2008 <페결핵진단표준> 규정에 근거하여, 임상표현증상, 병증 및 흉부영상학검사확진 또는 고도 결핵의심환자를 선택한다. 선택된 환자의 침, 흉수, 뇌척수액, 고름 표본을 약 2mL 수집하여 스크루 캡이 달린 50mL 원심관(centrifugal tube)에 넣는다. 표본과 똑같은 양의 N-아세틸-L-시스테인 수산화 나트륨(NaOH-NALC) 전처리액를 넣고, 20초간 소용돌이 진탕한다. 실온에서 18분 간 가만히 둔다. PBS(pH 6.8)를 40mL까지 넣고, 20분 3000g 원심분리한 후, 위의 맑은 액체를 버리고 침전물만 남긴다. 2mL PBS(pH6.8)을 넣고 현탁액으로 만든다. 처리된 표본을 배양기에 접종하여 고체배양한다.
<실시예 2> 결핵균 표본의 분리 배양 감정
1. 배양기의 제조:
배양기성분은 표 1에 나와있다. 표에 나와있는 시제의 양에 각 성분마다 증류수를 넣고, 충분하게 용해시킨다. 30분간 끓이거나 15분간 121℃ 고압상태에 둔다.
신선한 계란을 물로 헹구고, 비눗물로 깨끗하게 씻는다. 계란이 건조되면 75% 알코올로 닦아 소독한다. 무균작업 하에 달걀액을 멸균된 눈금이 있는 법랑컵에 넣고, 충분히 섞어준 후, 멸균된 거즈로 여과하고, 1000mL를 취하여 충분히 섞어준다; 2% 말라카이트 그린 20mL를 넣고 충분히 섞는다; 분할저장 시험관(18mmⅹ180mm)에 배양기 7mL를 넣고, 85℃ 스팀 인큐베이터에 50분간 두어 응고시킨다. 5%대로 시험관으로 제조된 배양기를 37℃에서 48시간 배양하고, 무균검사를 한다. 무균검사합격 후 4℃냉장고에 비축해두어, 한 달 안에 사용한다.
2. 결핵균의 접종 배양:
<실시예 1>에서 처리된 표본 0.1mL을 취하여, 배양기 경사면에 균일하게 접종한다. 이때 현탁액은 전체 경사면을 덮어야 한다. 37℃ 인큐베이터에 두고 배양한다. 제3일, 제7일에 세균의 성장상황을 관찰하고, 그 후부터 매주마다 1회 관찰한다. 그러나 양성으로 의심되는 것이 발견되면, 적시에 골라내고, 매일마다 관찰한다. 양성성장은 꼭 스메어 스테이닝(smear staining)법으로 검증한다. 8주까지 배양한 후 세균이 자라지 않은 것이 결핵균 음성 배양이다.
3. 결핵균의 항산성염색(acid-fast staining)
상기 결핵균 양성 균태(菌苔)를 조금 취하여, 슬라이드 글라스에 골고루 바르고, 불로 가열하여 고정시킨다. 그 후 카르볼푸크신으로 염색한 후 5분간 불로 가열하고, 3% 염산알코올로 1분 정도 탈색하고 물로 세척한다. 알칼리성 메틸렌블루 용액으로 1분 재염색하고 물로 세척한다. 수분을 다 건조하고, 오일 현미경으로 관찰한다. 붉은색으로 염색된 것이 결핵균이다.
<실시예 3> 켈리마이신의 항결핵균활성 실험
1. 절대농도법
1) 항결핵약물
켈리마이신 표준품: 중국 국립 제약 생물 제품 관리 연구소의 제품을 사용했다. 대조약품: 아이소나이아지드, 리팜피신은 Sigma회사의 표준품을 사용했다. 항결핵약물을 일정 농도로 제조하여 모액으로 만들고 일정량을 배양기에 넣어 필요한 시제량을 만든다(표 2).
2) 균주의 접종
임상표본을 분리한 균주를 스미어를 통해 항산성균의 배양물임을 확인하고, Tween80 10%를 함유한 생리식염수로 희석하여, McFarland의 표준 비탁튜브(Guangdong Huankai Microbial Sci. & Tech. Co.,Ltd)와 비탁분석하여 10-2mg/mL의 균액을 준비하고, 측정검사약물이 함유된 배양기에 접종한다.
매 실험마다 양성 및 음성 대조그룹을 설정한다. 음성대조그룹은 약물을 넣지 않은 배양기이다. 양성대조그룹은 혈핵균 표준균주 H37Rv이다. 매 실험관에 0.1mL를 접종하고, 접종된 배양기를 37℃에서 배양시킨다.
비고: 본 발명실험에서의 1선 약물사용량은 실제 임상사용량에 따라 설계됨.
3) 결과관찰과 판정
균액 접종 후 제3일에 1회 관찰한다. 이후 매주마다 1회 관찰하고 4주 후 결과를 보고한다.
배양결과는 하기의 기준에 따라 판정한다:
① 8주간 배양시 균락이 자라지 않는 경우, 결핵균 음성(-)배양으로 보고한다.
② 배양기 경사면에 균락이 20개 이하로 자라는 경우, 균락수와 결핵균 양성배양임을 보고한다.
③ 배양기 경사면에 균락이 분산되어 자라고, 균락수 20개 이상에 경사면 면적의 1/4이하를 차지하는 경우, 결핵균 양성(1+)로 보고한다.
④ 배양기 경사면에 균락이 분산되어 자라고, 경사면 면적의 1/4이상에서 1/2이하를 차지하는 경우, 결핵균 양성(2+)로 보고한다.
⑤ 배양기 경사면에 균락이 밀집되어 자라거나 부분적으로 융합되어 있고, 경사면 면적의 1/2이상에서 3/4이하를 차지하는 경우, 결핵균 양성(3+)으로 보고한다.
⑥ 배양기 경사면에 균락이 밀집되어 자라고, 경사면 면적의 3/4이상을 차지하는 경우, 결핵균 양성(4+)로 보고한다.
2. 켈리마이신 항결액균활성
1) 켈리마이신에 민감한 임상결핵균
240주(株)임상분리 감정된 결핵균에서 켈리마이신 1 또는 20(μg/mL)에 민감한 균주는 총 172주이다(표3). 표 3 결과에 따르면, 임상분리된 172주의 민감균주 중, 92주에 대한 켈리마이신의 민감도가 실험실품질관리균주 H37Rv와 비슷하다는 것을 알 수 있고, 유효균주의 53.48%를 차지한다는 것을 알 수 있다. 기타 80주에 대한 민감도는 실험실품질관리균주보다 눈에 띄게 높다. 54주는 1 μg/mL의 켈리마이신에 대해 민감하며, 총 유효균의 31.39%를 차지한다. 이는 켈리마이신이 임상분리된 결핵균에 좋은 활성이 있다는 것을 나타낸다.
품질관리: 결객균실험실표준균주H37Rv를 양성대조그룹으로 사용함. 대조: 약물을 첨가하지 않은 배양기. 모든 분리균주는 무약물배양기에서의 성장결과가 모두 4+이다.
2) 켈리마이신과 아이소나이아지드의 일부 임상결핵균에 대한 활성 비교
켈리마이신과 임상 1선 항결핵약물 아이소나이아지드의 임상결핵균활성에 대한 활성을 비교하였다. 그 결과, 켈리마이신 20 μg/mL이 37주의 아이소나이아지드 약물내성이 있는 임상결핵균에 효과가 있는 것으로 나타났다(표 4).
3) 켈리마이신과 리팜피신의 일부 임상결핵균에 대한 활성 비교
켈리마이신과 임상 1선 항결핵약물 리팜피신의 임상결핵균활성에 대한 활성을 비교하였다. 그 결과, 켈리마이신 20 μg/mL이 39주의 리팜피신 약물내성이 있는 임상결핵균에 효과가 있는 것으로 나타났다(표 5).
켈리마이신의 일부 임상결핵균에 대한 활성이 250 μg/mL 리팜피신보다 우수하다.
4) 켈리마이신과 일부 아이소나이아지드내성 및 리팜피신내성 임상결핵균 간의 활성을 비교하였고, 그 결과, 켈리마이신 20 μg/mL의 23주 임상결핵균활성이 1μg/mL 아이소나이아지드와 250μg/mL 리팜피신보다 우수한 것으로 나타났다(표 6).
본 발명실험연구결과, 켈리마이신은 임상 1선 항결핵약물 아이소나이아지드와 리팜피신 민감균에 활성이 있을 뿐만 아니라, 일부 아이소나이아지드 및 리팜피신에 대해 약제내성이 있는 균에게도 활성이 있어서, 약제내성결핵균 감염질병에 세로운 응용을 기대할 수 있다.

- 환자에게 유효한 양의 켈리마이신과 약학적으로 수용할 수 있는 담체와 함께 구성된 조합물을 제공하는 것을 특징으로 하는 항결핵균감염의 방법.
- 제4항 또는 제5항에 있어서,
