카텝신
프로테아제
카텝신(cathepsin)(고대 그리스어로 kata-는 "down" 및 hepsein은 "boil"을 의미; 약칭 CTS)은 모든 동물에서의 것들을 비롯하여 다수의 세포 유형에서 발견되는 프로테아제, 즉 기타 단백질을 분해하는 단백질이다. 이 패밀리에는 대략 12종의 구성원이 있는데, 그들의 구조, 촉매 메카니즘, 및 그들이 어느 단백질을 절단하는가에 의해 구별한다. 구성원의 대부분은 리소좀에서 발견되는 낮은 pH에서 활성화된다. 따라서, 이 패밀리의 활성은 거의 전적으로 그들 세포소기관에 들어있다. 카텝신은 포유동물 세포 대사회전, 예를 들어 골 재흡수에서 지극히 중요한 역할을 한다. 그들은 폴리펩티드를 분해하며 그들의 기질 특이성에 의해 구별된다.

)
CTSK로 약칭되는 카텝신 K(진뱅크 수탁번호: NM_000396.3 (폴리뉴클레오티드) 및 NP_000387.1 (폴리펩티드))는 인간에서 CTSK 유전자에 의해 코딩되는 효소이다. 당해 유전자에 의해 코딩되는 단백질은 골 리모델링 및 재흡수에 관여하는 리소좀 시스테인 프로테아제이다. 당해 단백질은, 펩티다제 C1 단백질 패밀리의 구성원인데, 파골세포에서 우세하게 발현된다.
인간 카텝신 K는 대략 12.1 kb의 게놈 DNA에 의해 코딩되고 염색체 1q21에 맵핑된다. 게놈 DNA 서열의 분석에 따르면 8개 엑손과 7개 인트론이 유전자에 위치하고 있음을 보여준다. 전사산물은 1.7 kb 길이이다. 파골세포 분화 진행중에, 골모세포/기질 세포는 그 전구체의 성숙 파골세포로의 성장과 분화를 유도하고 조절하는 마크로파지-콜로니 자극 인자(M-CSF) 및 NF-jB 리간드의 수용체 활성화제(RANKL)를 비롯한 시토카인을 생성한다. OPG, IL-6, INF-c와 같은, 파골세포 분화와 활성화의 생리학적 억제제 또한 카텝신 K 발현을 직접 억제할 수 있다[1].

촉매
메카니즘
카텝신 K의 촉매작용 삼인조(Cys25, His159, Asn175, 파파인 넘버링)는 고전적으로 두 도메인을 분리하는 개열부에 수용되어 있는데, Cys25는 좌측 도메인의 길고 보존된 N-말단 a-나선에 위치하고, 반면에 His159은 다른 하나의 도메인에 위치한다. 조직 및 세포 분포

)

CTSK
억제제
카텝신 K 억제제는 골 질환의 치료(이에 한정되지 않음)에 대하여 문헌에 널리 기재되어 있다.
WO 2004/007477에서는 그 중에서도 특히 심혈관 질환의 치료를 위하여 대사 효소(즉, 카텝신 K)에 대한 억제제로서 아실 히드라지노 티오펜 유도체를 기재하고 있다. WO 2006/076796에서는 카텝신 K 억제제가 비만 및 관련 장애의 치료에 유용할 수 있음을 언급하고 있다.
선택적 카텝신 K 억제제인 오다나카티브 및 골다공증의 치료를 위한 그의 용도가 문헌 [J. Bone Miner. Res. 25 (5) 937-947 (2010)]에 기재되어 있다.
본 발명은 폐고혈압 및 심부전의 치료 및/또는 예방에 있어서 바람직하게는 선택적 카텝신 K 억제제의 용도, 및 폐고혈압 및/또는 급성 및/또는 만성 심부전의 치료 및/또는 예방에 있어서 그의 용도에 관한 것이다.

개에서의 고속 심실 페이싱에 의해 유발시킨 실험적 심부전은 저-박출량 심근병증 상태로 귀결된다. 심근 리모델링 및 심실 확장은 증가한 벽 응력을 상쇄하기 위하여 발생한다. 이들 변화와 더불어 저하된 심실 수축성이 존재한다. 이들 변화는 인간과 자연에서 발생하는 개의 확장형 심근병증(DCM) 둘 모두에서 관찰되는 것들과 유사하다. 심근병증은 심실벽 및 유두근 전역에 걸친 근육세포의 진행성 손실과 관련되어 있다 (문헌 [Cardiovascular Research 49 (2001) 127-134]).
폐고혈압 기니 피그 모델에서의 CTSK 억제제 (오다나카티브)
CTSK 억제제 오다나카티브를 실시예 섹션에 기재된 바와 같이 폐고혈압에 대한 기니 피그 모델에서 시험하였다. 대략 250 g 체중의 수컷 던킨 하틀리(Dunkin Hartley) 기니 피그를 3개의 상이한 처리군(n=7-8 동물/군; 대조군 + 위약, 저산소증 + 위약, 저산소증 + 오다나카티브)으로 무작위화하였다. 만성 저산소증에의 노출을 위하여, 기니 피그를 28일 동안 환기 챔버에서 정상기압(normobaric) 저산소증(10% O2에서)하에 유지시켰다. 대조군 동물은 실내 공기중에서 유지시켰다. 먹이와 물은 무제한으로 공급하였다. 기니 피그는 삼투압 미니펌프의 이식에 의한 연속 주입을 통해 0일차부터 28일차까지 오다나카티브 또는 위약을 투여받았다. 28일차에 동물을 방혈사시켜 심장을 적출하였다. 심장을 해부하고, 우심실 대 좌심실+중격 중량의 비율(RV/LV+S)을 우심실 비대증의 지수로서 계산하였다. 우심실을 RNA 추출 및 정량적 실시간 폴리머라제 연쇄 반응을 위해 드라이아이스상에서 순간 동결(snap-frozen)하였다. 저산소증 4주 후, RV/LV+S 비율은 0.28±0.01(평균±SEM, 산소 정상 상태 대조군)에서 0.37±0.01(평균±SEM, 저산소증 위약군)로 증가하였다. 오다나카티브 처리는 RV/LV+S 비율을 현저하게 및 놀랍게도 0.30±0.01(평균±SEM)로 감소시켰다. 결과를 도 24에 도시하였다. 대조군: 산소 정상 상태하에서 심장 우심실 중량 대 중격을 포함한 좌심실 중량의 비율; 위약: 저산소증하에서 심장 우심실 중량 대 중격을 포함한 좌심실 중량의 비율; 오다나카티브: 저산소증 및 오다나카티브 처리하에서 심장 우심실 중량 대 중격을 포함한 좌심실 중량의 비율. 유의한 변화는 별표로 표시하였다.
동물의 질환 상태를 분석하고 오다나카티브 처리의 효능을 측정하기 위하여, 마커 유전자의 발현을 수행하였다. ANP의 발현이 저산소증-유지 동물로부터의 심장에서 증가하는 반면에, 오다나카티브 처리는 (위약군과 비교하여) 저산소증하에서 현저히 감소된 발현을 초래한다. 결과를 도 25에 도시하였다. LTBP2의 발현이 저산소증-유지 동물로부터의 심장에서 증가하는 반면에, 오다나카티브 처리는 (위약군과 비교하여) 저산소증하에서 현저히 감소된 발현을 초래한다. 결과를 도 26에 도시하였다. CTSK의 발현이 저산소증-유지 동물로부터의 심장에서 증가되는 반면에, 오다나카티브 처리는 (위약군과 비교하여) 저산소증하에서 현저히 감소된 발현을 초래한다. 결과를 도 27에 도시하였다. 심부전 환자 및 동물 모델에서의 증가된 ANP 수준과 병기의 기지의 상관관계 때문에, CTSK의 증가된 발현은 폐고혈압과 상관관계가 있는 것으로 보인다. 저산소증-유발 폐고혈압 동물을 오다나카티브로 처리하면 위약-처리 동물과 비교하여 우심실 중량의 감소를 초래한다. 여기에서 본 발명자들은 본 발명이 제시한 CTSK 억제제에서 오다나카티브와 그 밖의 것들이 폐고혈압의 치료에 유용함을 보여준다.
개에서의 페이싱-유발 심부전에서 CTSK 발현
페이싱-유발 심부전견으로부터의 심장 샘플에서 CTSK의 발현을 수행하여 심부전에서 CTSK의 관련성을 평가하였다. 실험은 실시예 섹션(실시예 동물 모델 A-4)에 기재된 바와 같이 수행하였다.
ANP 및 CTSK의 발현 분석을 좌심방, 우심방, 좌심실 및 우심실에 대하여 실시예 섹션에 기재된 바와 같이 수행하였다. 결과를 CTSK의 발현에 대해서는 도 28에 및 ANP에 대해서는 도 29에 도시하였다. 비-페이싱 견으로부터의 조직과 비교하여, 페이싱 견의 좌심방, 우심방, 좌심실 및 우심실에서 CTSK의 발현이 증가하였다. 비-페이싱 견으로부터의 조직과 비교하여, 페이싱 견으로부터의 좌우심방에서 ANP의 발현이 증가하였다. 심부전 환자 및 동물 모델에서의 증가된 ANP 수준과 병기의 기지의 상관관계 때문에, CTSK의 증가된 발현은 심부전과 상관관계가 있는 것으로 보인다. 여기에서 본 발명자들은 CTSK 발현이 심부전에서 상향조절되고, 오다나카티브 같은(이에 한정되지 않음) 억제제(본 발명에서 제시한)에 의한 CTSK의 억제가 심부전의 치료에 유용함을 보여준다.
적응증
급성 저산소증은 강력한 폐동맥 혈관수축을 도출하고 폐동맥 압력을 증가시킨다[5]. 이러한 소위 율러-릴레스트란트(Euler-Liljestrand) 메카니즘은 폐의 환기와 혈액 순환(관류)간의 연관을 서술해주며 또한 저산소증 폐 혈관수축으로도 알려져 있다[6]. 만성 저산소증은 광범위한 혈관 리모델링, 폐고혈압, 및 폐성 심장(cor pulmonale)으로 귀결된다[7]. 혈관 리모델링 과정은 주로 폐동맥의 원위 브래치(distal braches)에 영향을 주며: 혈관 평활근 세포(VSMC)와 외막 섬유모세포 둘 모두 이들 조건하에서 증식한다[8].
폐고혈압(문헌 [Clinical Classification of Pulmonary Hypertension, Dana Point 2008])은 원인이 다양할 수 있으며 비-치료시에는 사망에 이르는 진행성 폐 장애이다. 폐고혈압은 사망에 이르게 할 수도 있는 펌프 실조로 진행하는 우측 심부전이 있는 우측 심장에 걸리는 과부하와 연관이 있다. 정의상, 만성 폐고혈압에서 평균 폐동맥 압력(mPAP)은 휴식시 >25 mmHg 및 운동중 >30 mmHg(정상치 <20 mmHg)이다. 폐 혈관의 폐동맥 혈관수축과 구조적 리모델링 둘 모두는 당해 질환에서 상승된 폐 압의 원인이 되는 병리학적 과정의 불가결한 특징이다. 리모델링은 신근육형성, 중막 비대증 및 외막 비후를 특징으로 한다. 폐 순환의 이러한 증가하는 폐색은 우측 심장에 진행성 스트레스를 가져오고, 그에 따라 우측 심장에 의한 감소한 박출량을 초래하고 종국에는 우측 심부전으로 귀결된다.
소위 특발성 폐동맥 고혈압(PAH)이라는 것은, 특정할 수 있는 원인 없이 발생하는데, 이환율이 백만명당 1 내지 2명인 극히 희귀한 장애이다[3]. 환자의 평균 연령은 36세인 것으로 산정되며, 환자의 10%만이 60세를 넘었다. 남성보다는 뚜렷하게 더 많은 여성이 이환된다. 폐고혈압의 2차 형태는 근본 원인의 다양성과 일치하여 상이한 경과를 나타내지만, 모든 사례에서 사망률이 높은 중증 장애이다.

좌측 심장 장애와 관련된 폐고혈압은 수축기능 장애, 확장기능 장애 및 판막 질환이 있는 장애를 포함한다. 폐 질환 및/또는 저산소증과 관련된 폐고혈압은 만성 폐색성 폐 장애, 간질성 폐 질환, 혼합된 제한적 및 폐색성 패턴을 갖는 기타 폐 질환, 수면시무호흡증후군, 폐포 저환기, 만성 고산병 및 체질이상을 포함한다. 만성 혈전색전증에 기인한 폐고혈압(CTEPH)은 근위 폐동맥의 혈전색전성 폐색, 원위 폐동맥의 혈전색전성 폐색 및/또는 비-혈전성 폐 색전증(종양, 기생충, 이물)을 포함한다.
불분명한 다인자성 메카니즘을 갖는 폐고혈압은 혈액학적 장애(골수증식성 장애, 비장 절제), 전신 장애(사르코이도시스, 폐 랑게르한스 세포 조직구증, 림프관평활근종증, 신경섬유종증, 혈관염), 대사 장애(갑상선 장애, 글리코겐 축적증, 고셔병) 및/또는 종양 폐색, 섬유화성 종격동염, 투석을 시행하는 만성 신부전 같은 그 밖의 장애를 포함한다.

조합 요법
본 발명은 추가로, 본 발명에 따른 화합물 및 1종 이상의 추가 활성 성분을 포함하는, 특히 전술한 장애의 치료 및/또는 예방을 위한 약제에 관한 것이다. 바람직하게 언급될 수 있는 적합한 조합 활성 성분의 예는 다음과 같다:

본 발명은 추가로, 유효량의 화학식 I 내지 XV의 적어도 1종의 선택적 카텝신 K 억제제, 또는 적어도 1종의 선택적 카텝신 K 억제제를 불활성 무독성인 제약상 적합한 부형제와 조합하여 포함하는 유효량의 약제를 투여함으로써 인간 및 동물에서 폐고혈압을 치료 및/또는 예방하는 방법에 관한 것이다.

진단
또 다른 실시양태에서, CTSK에 특이적으로 결합하는 항체는 폐고혈압 또는 심부전의 진단에, 또는 CTSK 억제제로 치료받는 환자를 모니터링하기 위한 검정에 사용될 수 있다. 진단 목적에 유용한 항체는 치료제에 대하여 전술한 것들과 동일한 방법으로 제조될 수 있다. CTSK에 대한 진단 검정은 항체 및 표지물을 사용하여 인간 체액에서 또는 세포 또는 조직의 추출물에서, 바람직하게는 심장 조직에서, 좀더 바람직하게는 심장 심실(좌심실 및/또는 우심실)에서 및 한층 더 바람직하게는 우심실에서 CTSK를 검출해내는 방법을 포함한다. 항체는 변형하여 또는 변형없이 사용될 수 있으며, 리포터 분자와의 공유 또는 비-공유 결합에 의하여 표지될 수 있다.

발현 분석
기니 피그 또는 개 조직을 액체 질소로 연마하여 분쇄시켰다. 총 RNA를 추출하고, DNase I 소화를 수행하여 잔류 게놈 DNA를 제거한 다음 RNA를 랜덤 헥사머 프라이머를 사용하여 역전사하였다. 어플라이드 바이오시스템즈 프리즘 7900(Applied Biosystems PRISM 7900) 서열 검출 시스템을 사용하여 정량적 택맨(TaqMan) RT-PCR 분석을 수행하였다. 열 프로토콜을 50℃에서 2 min, 이어서 95℃에서 10 min 및 이어서 95℃에서 15 s와 60℃에서 1 min의 40 사이클로 세팅하였다. 결과를 L32 (개) 또는 b-액틴 (기니 피그) 대조군에 대하여 정규화하고, 상대 발현을 하기 공식에 따라 계산하였다: 상대 발현 = 2(20-(CT(프로브)-CT(L32/b-액틴))). 파라미터 CT는 증폭 플롯이 기준선을 상회하는 고정 역치를 통과하는 사이클 수로 정의한다.

심혈관 질환(저산소증-유발 폐고혈압)에서 바이오마커, 치료 및 진단 표적으로서 LTBP2의 사용
저산소증-유발 폐고혈압 모델은 동물 모델 섹션(A-3)에 기재되어 있다.
총 세포 RNA를 제조업자의 설명서(인비트로젠(Invitrogen), 미국)에 따라 트리졸-시약(Trizol-Reagent) 프로토콜로 단리하였다. 트리졸-시약 프로토콜로 준비한 총 RNA를 DNAse I로 처리하여 게놈 DNA 오염을 제거하였다.
LTBP2의 mRNA 분포의 상대적인 정량을 위해, 각 샘플로부터의 총 RNA를 먼저 역전사하였다. 1 ㎍의 총 RNA를 제조업자의 프로토콜에 따라 임프롬-II(ImProm-II) 역전사 시스템(프로메가(Promega), 미국)을 사용하여 역전사하였다. 최종 부피를 물로 200 ㎕이 되도록 조정하였다.
LTBP2 mRNA 분포의 상대적인 정량을 위해, 제조업자의 설명서와 프로토콜에 따라 어플라이드 바이오사이언스(Applied Bioscience) ABI 7900HT 서열 검출 시스템을 사용하였다. PCR 반응을 LTBP2 및 하우스키핑 유전자 b-액틴을 정량하도록 셋업하였다. 피그에서의
저산소증-유발 폐고혈압의 동물 모델
CTSK
억제제로의 처리

형광투시경 안내하에(오이시 플렉시뷰 8800(OEC FlexiView 8800), 지이 헬스케어(GE Healthcare), 미국) 및 멸균 조건하에서 스테로이드-용출 페이스메이커 리드(세트록스 S60(Setrox S60), 바이오트로닉(Biotronik), 독일)를 액와 정맥을 통해 우심실에 삽입하고 페이스메이커(로고스(Logos), 바이오트로닉/독일)에 연결하였다. 정확한 안치를 확인하기 위하여, 포획 역치 및 심장내 신호를 측정하였다. 모든 동물은 페이스메이커 이식 후 3일의 기간에 걸쳐서 비경구용 항생제(엔로플록사신 (베이트릴(Baytril)®), 바이엘/독일; 5 mg/kg; s.c.) 및 진통제(메타미졸 (메타미졸-WDT®), WDT/독일; 50 mg/kg; i.m.) 처리를 받았다. 상처가 아문 뒤(7일차) 페이스메이커를 활성화시켰으며 심장은 분당 심박수(BPM) 220의 속도로 14일 동안 연속적으로 페이싱하였다. 이러한 페이싱 기간 동안 개는 견사에서 유지시켰고, 먹이와 물에의 접근은 자유롭게 하였으며, 개는 1일 2회 놀이 공간 출입이 허용되었다. 연구 지속기간 동안 매일 개를 관찰하고 임상평가하였다.
급성 실험 셋업
페이싱 14일 후에 동물을 전신마취하에 조사하여 각각 정맥내용 코니밥탄(Conivaptan)(0.1 mg/kg i.v.) 또는 톨밥탄(Tolvaptan) 볼러스(0.1 mg/kg i.v.)에 대한 그들의 혈류역학적 및 뇨 배출량 반응을 평가하였다. 조사 당일에, 마취 유발 1시간 전에 페이스메이커를 무력화시켰다. 멸균 조건하에서 동물에 대퇴 동맥 액세스를 장치하고(NaCl 0.9% 충진 시스(sheath) 도입기(코디스(Cordis), 벨기에 워털루)를 통해 동맥 혈압을 측정하기 위하여), LV 성능(속도, 수축성 및 이완)을 ECG 및 5F-마이크로팁 카테터(밀라 인스트루먼츠 인코포레이티드(Millar Instruments Inc.), 미국 휴스턴주)를 사용하여 평가하였다. 액와 정맥을 통해 스완 간쯔(Swan Ganz) 카테터(비질런스(Vigilance) 모니터를 구비한 CCOmbo, 에드워즈 라이프사이언스(Edwards Lifescience), 미국)를 도입시켜 심박출량, 폐동맥 압력, 중심정맥압 및 체온을 측정하였다. 모든 데이터는 굴드 앰플리화이어(Gould Amplifier)와 ACQ-16 액퀴지션 인터페이스 유닛(ACQ-16 Acquisition Interface Unit)으로 기록하였으며 폰마(Ponemah) 소프트웨어(이상 디에스아이(DSI) 제품, 미국 세인트 폴)로 추가 분석하였다. 방광 카테터를 삽입하고 뇨 배출량을 20분마다 측정하였다. 생리적 효과는 문헌 [Mondritzki et al., Am J Ther. 2010 Dec 29]에 기재되어 있다.

ANP
심방성 나트륨이뇨 펩티드(ANP), 심방성 나트륨이뇨 인자(ANF), 심방성 나트륨이뇨 호르몬(ANH), 다시 말해서 아트리오펩틴은 강력한 혈관확장제이고, 심근 세포에 의해 분비되는 단백질 (폴리펩티드) 호르몬이다[11]. ANP는 체내 수분, 나트륨, 칼륨 및 지방(지방 조직)의 항상성 조절에 관여한다. ANP는 높은 혈압에 반응하여 심장의 상부 챔버 (심방)에 있는 근육 세포(심방 근육세포)에 의해 방출된다. ANP는 순환 시스템에 과해지는 물, 나트륨 및 지방 부하를 감소시키도록 작용하며, 그에 따라 혈압을 감소시키게 된다. ANP는 수용체의 특이적 세트 - ANP 수용체에 결합한다. 수용체-효능제 결합은 혈액량의 감소 및 그 결과 심박출량과 전신 혈압의 감소를 초래한다. 지질 분해는 증가하고 신장 나트륨 재흡수는 감소한다. ANP가 신체에 미치는 전체적인 영향은 레닌-안지오텐신 시스템에 의해 초래된 혈압과 혈액량의 증가를 상쇄하는 것이다.

신장:
ANP는 수입사구체세동맥을 팽창시키고, 수출사구체세동맥은 수축시키며, 혈관사이세포는 이완시킨다. 이는 사구체 모세관의 압력을 증가시키고, 그에 따라 사구체 여과율(GFR)이 증가하게 되어, 나트륨과 물의 좀더 많은 배설이 일어나게 된다. ANP는 직행혈관을 통한 혈류를 증가시키고 이는 용질(NaCl 및 요소)을 수질 간질 바깥으로 씻어낼 것이다[6]. 수질 간질의 좀더 낮은 오스몰농도는 세뇨관액의 더 적은 재흡수 및 증가한 배설을 가져온다. ANP는 ENaC의 구아노신 3',5'-시클릭 모노포스페이트(cGMP)-의존성 인산화를 통해 네프론의 근위 세뇨관과 피질집합관에서의 나트륨 재흡수를 감소시킨다. ANP는 레닌 분비를 억제하고, 그에 따라 레닌-안지오텐신 시스템이 억제된다. ANP는 부신피질에 의한 알도스테론 분비를 감소시킨다.
혈관:
ANP는 혈관 평활근 cGMP의 막 수용체-매개 상승 및 카테콜아민의 효과의 억제에 의하여: 소동맥 및 소정맥에서 혈관 평활근을 이완시킨다.
심장:
ANP는 부적응성 심장 비대증을 억제한다. 심장 NPRA를 결여한 마우스는 증가한 심근 중량과 중증 섬유증이 발생하며 급사한다. NPRA의 재발현은 그 표현형을 구제한다. ANP는 고립성 심방성 아밀로이드증과 관련이 있을 수 있다.
LTBP2
LTBP2의 뉴클레오티드 서열은 수탁번호 Z37976(인간)에 의하여 데이터베이스로 입수가능하다. 프라이머 서열은 서열 9 내지 11(기니 피그)에 나타내었다.
형질전환 성장 인자 베타(TGFβ) 시토카인은 정상 및 형질전환 포유동물 세포 둘 모두에 광범위의 다양한 영향을 미치는 다기능성 패밀리이다. TGFβ의 분비 및 활성화는 잠복-관련 단백질 및 잠복성 TGFβ 결합 단백질(LTBP)과의 그들의 회합에 의해 조절된다. 형질전환 성장 인자 β(TGFβ)는 3가지 포유동물 이소형(TGFβ1, TGFβ2 및 TGFβ3)으로 존재한다. 이들의 각각은 보통은 생물학적 활성은 없고 3가지 성분을 포함하는 대형 잠복성 복합체(LLC)로 분비된다: 잠복-관련 단백질(LAP; 전구체 TGFβ의 N-말단 단편의 호모다이머) 및 잠복성 TGFβ 결합 단백질(LTBP)의 공유결합된 분자와 비-공유적으로 회합된 성숙 TGFβ의 디술파이드 결합된 호모다이머. 4종 LTBP 유전자가 동정되었다: LTBP1 내지 LTBP4. TGFβ 시토카인은 광범위의 다양한 포유동물 세포 타입의 성장과 기능을 조절한다. 최근에 와서는 LTBP가 TGFβ의 조립, 분비 및 그것이 저장 및/또는 활성화되는 부위로의 표적화에 관여될 수 있음이 분명해지고 있다. 따라서 이들 단백질은 TGFβ의 활성을 제어하고 지시하는 데에 중요한 역할을 할 수 있다. LTBP는 또한 TGFβ와 회합된 것들과 관계없이, 예를 들어 구조적 매트릭스 단백질로서 효과를 발휘할 수 있다.

심혈관 시스템에서의 LTBP2의 기능적 역할과 관련하여, 관상동맥 혈관성형술의 돼지 모델에서 동맥 손상에 반응하여 LTBP2 합성이 증가하였음이 입증되었다[9]. 따라서, 심부전의 발생에 있어서 TGFβ의 주지의 역할과 더불어[10], TGFβ-기능 변경 LTBP2이 LVAD 심장에서 및 심부전의 다양한 동물 모델에서 RNA 수준에 따라 조절된다는 본 발명자들의 발견은 LTBP2로 하여금 CHF에 대한 매력적인 후보 바이오마커가 되게 한다.
