Abstract
본 발명은 심장 줄기 세포 내부에서 심장 세포의 재생을 향상하기 위한 줄기 세포 자극제로, 적어도 2의 줄기 세포 자극제와 적어도 하나의 약학적으로 용인가능한 첨가제를 포함하는 줄기 세포의 자극을 위한 인간 또는 동물의 약학적 조성물(B)에 관한 것이다.
Description
본 발명은 일반적으로 조직 및/또는 기관을 목표로 하는 적합한 약학 조성물에 관한 것으로, 심장 세포 재생이 필요해짐에 따라 인간의 심장에 줄기 세포 자극제의 투여를 통한 심장병의 치료에 관한 것이다. 더 상세하게는, 심장 줄기 세포 내부에서 심장 세포의 재생을 향상하기 위한 줄기 세포 자극제의 사용을 밝힌 것이다.
심근경색증(Myocardial Infraction, MI)은 심근세포(cardiomyocytes)의 감소, 흉터 형성, 심실개조(ventricular remodeling) 및 결과적으로 심부전을 야기한다. 약학, 카테터(catheter)기반 및 외과적 개입(surgical interventions)은 비록 죽은 심근 재생은 실패했지만 관상 동맥 질환(Coronary Artery Disease, CAD)을 앓는 환자의 생존 향상을 이끌었다. 결과적으로, 허혈성 심부전으로 인하여 감소된 사망자 수는 증가된 사망자 수를 동반한다. 최근 심장 재생(repair)을 위한 잠재적인 새로운 계획으로써 줄기 세포 기반 치료법이 생겨났다(Dimmler S. et al., J Clin Invest 2005, 11, 572-583). 손상된 심근의 기능 재생을 위한 세포의 최적의 소스는 집중적인 연구 주제이다. 심장의 재생을 위한 줄기 세포의 중요한 특징은 자가 재생, 클론원성(clonogenicity) 및 심근세포, 내피 세포 및 혈관 평활근 세포(vascular smooth muscle cells)로 분화되는 능력이다.
과거 10년동안 연구자들은 혈맥 음성(lineage negative) c-kit 양성 골수 줄기 세포(Orlic et a/., Nature 2001 ; 410: 701 -705; Kajstura et a/., Circ. Res. 2005; 96: 127-137; Rota et a/., Proc Natl Acad Sci USA 2007 ; 104: 17783-17788), 골수-유래 간충직 줄기 세포(Mesenchymal Stem Cells, MSCs)(Min et a/., Ann Thor Surg 2002; 74: 1568-1575; Amado et a/., Proc Natl Acad Sci USA 2005 ; 102: 1 1474-1 1479) 및 내피 모세포(Endothelial Progenitor Cells, EPCs)(Cho et a/., J Exp Med 2007; 204: 3257-3269; Schuh et a/., Basic Res Cardiol 2008; 103:60-77)와 같은 동물 연구에서 심장 회복 치료를 위해 다양한 골수(Bone Marrow, BM)-유래 줄기/전구(progenitor) 세포를 적용해왔다. 이러한 연구는 골수-유래 줄기/전구 세포에서 심근세포 및 혈관 세포의 전형적인 특징을 가지는 세포로의 분화를 보여주나, 다른 연구는 이식된 골수 줄기 세포는 손상된 심장에서 심장의 표현형을 쉽게 얻을 수 없을 것이라고 제시하였다(Balsam et a/., Nature 2004; 428: 668-673; Murry et a/., Nature 2004; 428: 664-668; Nygren et a/., Nat Med 2004; 10: 494-501 ).
따라서, 골수-유래 줄기/전구 세포의 이식 후에 이들의 분화를 높이는 것은 연구자들에게 심장 재생 치료에서 이 세포를 효과적으로 사용하기 위한 중요한 과제로 남아있다. 다른 줄기 세포 소스는 골수-유래 줄기/전구 세포로부터 떨어져 심장 재생 치료를 위해 사용될 수 있다. 특히, 심장 줄기 세포(CSCs)는 심장 그 자체에서 발견되었다(Beltrami et a/., Cell 2003; 1 14: 763-776; Uranek et a/., Proc Natl Sci USA 2006; 103: 9226-9231). 심장 줄기 세포는 이미 심장 내에 존재하고, 심장 세포 조직을 발생시키기 위하여 프로그램되어 있으며, 심장 재생 치료에 활용하기 위한 타당한 소스를 대표한다. 심장 줄기 세포가 고유의 특징을 가지고 있음을 고려하면, 심장 줄기 세포의 인지는 엄청난 흥분을 만들어냈고, 심장 그 자체에서 나오며 이로 인하여 본질적으로 심장 조직을 원상태로 만들 수 있도록 프로그램되어 있는 세포로 하여금 심근 재생에 대한 강렬한 희망을 촉발시켰다.
심근 재생은 새로운 근세포와 관상 혈관의 형성을 요구하고, 이는 근세포 혈맥에 이미 완전하게 쓰이는(committed) 세포에 의해 수행되지 않는다. 경색(infarct)의 존재 하에서, 근세포의 생성은 단독으로 무동원체 영역(akinetic region)에서 수축성능을 회복할 수 없다; 근세포는 혈관 형성의 결핍에서는 성장하거나 살아남지 못한다. 소동맥은 혈관 공급에 중요하고, 산소 운반은 모세혈관 그물에 의하여 조절된다. 유사하게, 혈관신생(neovasculogenesis)은 단독으로 죽은 심근을 회복하거나 심실벽(ventricular wall)의 경색 부분에서 수축 활동을 다시 시작하지 못할 것이다. 동물의 경색된 심근에 국부적으로 주사된 심장 줄기 세포가 괴저성 조직을 재생하고 심실 기능을 향상시킨다는 관찰은 심장 줄기 세포가 근세포, 내피 세포, 평활근 세포 및 섬유아세포의 일반적인 재생의 원인임을 보여주는 새로운 패러다임의 기본을 형성한다. 환자에게 미래의 치료를 위한 적절한 계획을 세우려는 시도로, 개인적 견지에서 임상적으로 심장 줄기 세포 치료를 구체화하는 방향으로 분야를 이동시키기 위하여 새로운 가설이 나타나야 한다.

본 발명은 적어도 2의 줄기 세포 자극제와 적어도 하나의 약학적으로 용인가능한 첨가제(pharmaceutically acceptable excipient)를 포함하는 줄기 세포의 자극을 위한 인간 또는 동물의 약학적 조성물(B)에 관한 것이다.

실험예
실험은 심하게 경색된 돼지로 수행되었다. 다음과 같은 계획서가 세워졌다. 경색은 T0에서 90분 좌전하행(Left Anterior Descending, LAD) 폐색에 뒤이어 30분 재관류(reperfusion)에 의해 수행되었다. 재관류의 끝에서(T1), 첫 번째 조성물이 경색 면적에 관상동맥 먼 쪽을 통해 전달에 의해 비경구적으로 동물에게 투여되었다. 삼투 펌프가 있는 BrdU 또한 T1에서 피하에 주입된다. 14일 후에(T2), 두 번째 조성이 동물에 비경구적으로 투여된다. 양 조성물의 투여는 정맥을 통한 주입, 근육 내 주입 또는 관상동맥을 통한 주입과 같이 투여에 있어서 다른 방법으로 이루어질 수 있다. 마지막으로 42일째(T3)에 돼지의 안락사가 수행되었다.
구성성분의 농도는 괄호에서 언급되었다. 두 조성물, 단독 또는 결합하여, 실험되었다.
- IGF-1(인산완충액(Phosphate Buffer Solution, PBS) 15ml 내에 8㎍) 및 HGF(PBS 15ml 내에 2㎍)로 구성된 조성물(A) 및
- TGFβ-1(PBS 15ml 내에 0.5㎍), BMP4(PBS 15ml 내에 1㎍), IGF-1(PBS 15ml 내에 10㎍), 액티빈-A(PBS 15ml 내에 2㎍), 카르디오트로핀 1(PBS 15ml 내에 0.2㎍) 및 카르디오제놀 C(PBS 15ml 내에 5.2㎍)으로 구성된 조성물(B).
두 조성물은 약학적으로 용인가능한 첨가제(pharmaceutically acceptable excipient) 내에 있다. 약학적으로 용인가능한 첨가제는 인산완충액(PBS), 하드만액(hartmann's solution), 링거액(Ringer's lactate), 알부민 또는 임의의 적절한 단백질 안정제 조성물이 추가되거나 추가되지 않은 생리식염수(physiological NaCl)(0.9% NaCl)일 수 있다.
다섯 마리 동물에 대하여 다섯 치료 그룹이 평가되었다.
- 치료 그룹 1은 대조군이며, 단지 T1과 T2에 오로지 생리식염수만 받았다.
- 치료 그룹 2는 T1에 조성물(A)가 포함된 용액을, T2에 생리식염수를 받았았다. 믹스A(mix A)로 언급한다.
- 치료 그룹 3은 T1에 조성물(A)가 포함된 용액을, T2에 조성물(B)가 포함된 용액을 받았다. 믹스A+B로 언급한다.
- 치료 그룹 4는 T1과 T2에 조성물(B)가 포함된 용액을 받는다. 믹스B+B로 언급한다.
- 치료 그룹 5는 T1에 조성물(B)을 포함하는 용액을, T2에 생리식염수를 받는다. 믹스B로 언급한다.
계획서(protocol)은 표 1에 요약하였다.

혈액 분석이 다른 시간적 간격(interval)로 이루어졌다. 혈액 샘플은 각 치료 그룹마다 두 마리의 돼지의 목정맥을 통하여 관상동과 귀정맥을 통하여 정맥혈로부터 채취되었다. 첫 번째 투여 이후에, 샘플은 T1+5분; T1+1시간; T1+6시간에 채취되었다. 두 번째 투여 이후에, 샘플은 T2+5분; T2+1시간; T2+6시간 및 T2+24시간에 채취되었다. IGF-1 및 카르디오트로핀 1 농도의 정량을 위하여 ELISA 면역분석이 샘플로 수행되었다.
또한, 자기공명영상(MRI)이 T1+3일; T2 및 T3에 모든 동물에 대해 수행하여 흉터 부분, 전반적인 좌심실 기능(global left ventricular function), 국소적인 기능(벽 운동 및 비후) 및 심실의 국소관류법(regional perfusion)을 연구하였다.
또한, 조직병리학이 수행되어 흉터 부분, c-키트(c-kit) 양성 심장 줄기 세포의 확인 및 정량을 결정한다. 조직병리학은 또한 새로운 혈관과 심근세포의 분포, 크기 및 밀도에 대한 데이터를 제공한다. 조직병리학은 조직과 세포적 레벨에서 재생 과정을 기록하게 한다.
심장 재생의 분석에서 중요한 변수가 고려되어져 왔다: (1)원상태로 만들어진 조직 또는 심근세포의 덩어리(mass)와 관상동맥의 맥관구조의 양; (2) 회복된 근세포 및 혈관의 수와 크기; (3) 새롭게 형성된 심근 주위의 근세포와 혈관의 통합(integration); 및/또는 (4) 재생된 심근 구조의 기원.
경색 결과
MRI로부터의 이미지는 경색 크기, 경색 무게 및 경색 면적을 평가하기 위해 사용되었다. 결과는 아래의 표 2에 기재하였다.

실험은 경색 면적이 대조군은 약 19.8%이고, 믹스A인 그룹 2와 믹스B+B인 그룹 4는 약 19%가 각각 사용되었음을 보여준다. 놀랍게도, 본 발명에 따른 조성물(B)를 사용한 그룹 5(믹스B)는 경색 면적이 13.7%까지 제한되었다. 이러한 이유에서, 본 발명에 따른 조성물(B)는 다른 조성물과 비교하여 심근경색증과 같은 경색을 치료하는데 매우 효과적이다.
또한, 놀랍게도 믹스A의 일차 주사 후에 믹스B의 주사가 이루어질 때, 경색 면적은 약 9.6%의 값까지 더욱 제한되는 것이 관찰되었다. 이는 다른 그룹에 대하여 관찰된 결과에 근거하여, 예상할 수 없는 결과이다. 확실히 그룹 2에서 사용된 믹스A 단독으로는 거의 비효율적이다. 상승적인 효과는 본 발명에 따른 약학 혼합제를 사용함으로써 관찰된다. 이 결과는 조직병리학 및 면역조직화학 실험에 의해 확인된 것이다.
조직병리학
경색의 경계대(border zone) 또는 경색의 중심 면적 내에 취한 부분에 대하여 결과는 별도로 작성하였다. 모든 그룹에 대한 결과는 아래의 표 3에 열거하였다. 표 3의 데이터는 각 그룹의 동물에 대하여 심장 절편(heart slice)으로부터의 평균이다.

경색과 흉터 크기 사이의 비율은 경색 크기를 나타낸다. 경범침범도(transmurality)은 경색이 심근의 외부 표면에서 심하게 국부화되거나 심근의 내부 표면 전역에 확대되는지 규명하는 파라미터인데 반면에, 경색과 흉터 크기 사이의 비율은 경색 크기를 나타낸다. 경범침범도 값이 높을수록, 경색 크기는 크다.
경색의 경계대와 관련하여, 표 3은 믹스A, 믹스B+B 또는 대조군은 경색 크기에 거의 영향이 없다는 것을 보여준다. 이들의 경우, 경색과 흉터 크기 사이의 비율이 31.4% 내지 36.0%이다. 이와는 반대로, 본 발명에 따른 조성물(예, 믹스B)이 사용되었을 때, 경색과 흉터 크기 사이 비율은 놀랍게도 26%까지 감소하였다. 이 값은 본 발명에 따른 약학 혼합제(예, 믹스A+B)를 사용하였을 때 20%까지 더 감소될 수 있다. 이 실험은 약학 조성물 및 약학 혼합제는 심장 질환 치료에 효과적임을 나타낸다. 또한 이는 경색 영역에서 실험을 수행한 경우 확인되었다.

면역조직화학
실험을 수행하여 경색 부분 내에서, 미세혈관 밀도(vWF-양성 혈관/mm2), BrdU 양성 세포(BrdU positive cell) 및 c-키트 양성 세포(c-kit positive cell)를 평가하였다. 폰빌레브란트 인자(von Willebrand Factor, vWF)를 사용한 미세혈관 밀도의 정량은 경색 부분에서 생성된 새로운 혈관의 양을 결정하게 한다. BrdU 양성 세포 테스트는 심장 세포를 포함한 세포의 증식을 나타낸다. c-키트 양성 세포 테스트는 선택된 경색 부분 내에서 심장 줄기 세포의 양을 보여준다. 결과는 표 4에 나타난다. 이 실험은 그룹 1(대조군), 그룹 3(믹스A+B) 및 그룹 5(믹스B)에 대해서만 수행하였다.

결과는, 본 발명에 따른 조성물(A)와 조성물(B)이 혼합되거나 조성물(B) 단독으로 심장으로 주사될 때, 이는 심장 줄기 세포 자극 또는 심장 세포 증식에 영향력이 큰 효과가 있음을 보여준다. 확실하게, 본 발명의 조성물 또는 혼합제에 의해 자극으로 미세혈관 밀도가 향상되고 새로운 혈관이 생성되었다. 대조군의 27.9와 비교하여, 그룹 3 또는 5는 각각 34.2 및 34.3의 결과를 얻었다. 이는 BrdU 양성 세포 테스트로써 확인되었고, 본 발명의 조성물에 의해 세포 증식이 향상되었음과 활발한 세포적 활성이 관찰되었음을 보여준다. 대조군에 대해 BrdU 양성 세포 22.1%에 비하여, 믹스B가 주사되는 경우, 36.0%가 관찰되었다. 본 발명에 따른 약학 조성물이 세포 증식과 결과적으로 심근 주위에 새로운 근세포와 혈관의 형성을 촉진한다는 점을 분명히 밝힌다. 이는 본 발명에 따른 약학 혼합제를 사용하는 경우 더욱 향상될 수 있다. 52.7%의 값은 이러한 혼합제로써 도달되었다. 이러한 이유에서, 본 발명에 따른 두 약학 조성물과 약학 혼합제는 심장 조직 재생의 향상에 적합하다.
약학 혼합제의 심장 줄기 세포 활성과 증식을 유도 및 촉진하는 능력은 c-키트 양성 세포 테스트로 확인되었다. c-키트 양성 세포 테스트는 대조군과 비교하여, 믹스A+B를 사용한 경우 심장 줄기 세포의 양이 상당히 줄어들기 때문에 고정(resident) 심장 줄기 세포가 소비됨을 설명한다. 이러한 이유로, 재생된 심근 구조는 고정 심장 줄기 세포로부터 기원된다. 본 조성물 및/또는 혼합제는 고정 심장 줄기 세포 내부 자극에 효과적이다.

자극받는 상기 줄기 세포는 고정 심장 줄기 세포, 순환성 줄기 세포 또는 주입된 줄기세포인 것을 특징으로 하는 약학 조성물. - 제 1항 내지 제 7항 중 어느 한 항에 있어서,
일차 입자를 포함하는 것을 특징으로 하는 약학 조성물. - 제 8항에 있어서,
상기 일차 입자는 알긴산, 키토산, 덱스트란, 셀룰로오스, 리포솜 및 PLGA와 같은 폴리에스터의 마이크로스피어 또는 나노스피어, 폴리카프롤락톤 또는 코폴리에스터로 구성된 군으로부터 선택되는 것을 특징으로 하는 약학 조성물. - 제 8항 또는 제 9항에 있어서,
상기 일차 입자는 상기 약학 조성물의 줄기 세포 자극제를 캡슐화한 것을 특징으로 하는 약학 조성물. - 제 1항 내지 제 10항에 의한 약학 조성물(B)과 적어도 하나의 약학적 활성 물질을 포함하는 조성물(A)를 포함하는 것을 특징으로 하는 약학 혼합제.
- 제 11항에 있어서,
상기 적어도 하나의 약학적 활성 물질은 인슐린 유사 성장 인자 1(IGF-1), 간세포 증식 인자(HGF) 및/또는 NK1, 1K1, 1K2, HP11, 또는 HP21, 및 이들의 조합의 이형인 것을 특징으로 하는 약학 혼합제. - 제 11항 또는 제 12항에 있어서,
적어도 하나의 상기 약학적 활성 물질은 SCF-1을 더 포함하는 것을 특징으로 하는 약학 혼합제. - 제 11항 내지 제 13항 중 어느 한 항에 있어서,
상기 조성물(A)는 이차 입자를 더 포함하고, 상기 이차 입자는 알긴산, 키토산, 덱스트란, 셀룰로오스, 리포솜 또는 PLGA와 같은 폴리에스터의 마이크로스피어 또는 나노스피어, 폴리카프롤락톤 또는 코폴리에스터로 구성된 군으로부터 선택되는 것을 특징으로 하는 약학 혼합제. - 제 14항에 있어서,
상기 이차 입자는 적어도 하나의 상기 약학적 활성 물질을 캡슐화한 것을 특징으로 하는 약학 혼합제. - 제 14항 또는 제 15항에 있어서,
일차 입자에 캡슐화된 물질의 전달 전에 상기 이차 입자에 캡슐화된 물질이 전달되게 하기 위해서 상기 이차 입자가 배열되는 것을 특징으로 하는 약학 혼합제. - 약으로 사용하기 위한 제 1항 내지 제 10항 중 어느 한 항에 의한 약학 조성물 또는 제 11 항 내지 제 16항 중 어느 한 항에 의한 약학 혼합제.
- 심장 조직의 재생에 사용하기 위한 제 1항 내지 제 10항 중 어느 한 항에 의한 약학 조성물 또는 제 11항 내지 제 16항 중 어느 한 항에 의한 약학 혼합제.
- 심장 조직 변질의 치료에 사용하기 위한 제 1항 내지 제 10항 중 어느 한 항에 의한 약학 조성물 또는 제 11항 내지 제 16항 중 어느 한 항에 의한 약학 혼합제.
- 심부전, 심장 허혈 또는 심근경색증을 포함하는 심장 질환의 치료에 사용하기 위한 제 1항 내지 제 10항 중 어느 한 항에 의한 약학 조성물 또는 제 11항 내지 제 16항 중 어느 한 항에 의한 약학 혼합제.
- 인간 또는 동물의 심장 줄기 세포 내부 또는 외부에서 작용하기 위해 제 1항 내지 제 10항 중 어느 한 항에 의한 상기 약학 조성물 (B)가 상기 인간 또는 동물에 투여되는 것을 특징으로 하는 방법.
- 제 21항에 있어서,
상기 조성물 (B)의 투여가 적어도 하나의 약학적 활성 물질을 포함하는 조성물 (A)의 일차 투여 후에 이루어지는 것을 특징으로 하는 방법. - 제 21항 또는 제 22항에 있어서,
상기 투여는 주사에 의해 수행되는 것을 특징으로 하는 방법. - 제 21항 내지 제 23항 중 어느 한 항에 있어서,
상기 투여는 순차적인 주사인 것을 특징으로 하는 방법. - 제 21항 내지 제 24항 중 어느 한 항에 있어서,
제 1항 내지 제 10항 중 어느 한 항에 의한 상기 약학 조성물 (B)의 두 번의 연속적인 투여 사이의 기간은 1 시간 내지 180일인 것을 특징으로 하는 방법. - 제 21항 내지 제 25항 중 어느 한 항에 있어서,
각각의 상기 투여가 반복 실시되는 것을 특징으로 하는 방법. - 제 21항 내지 제 26항 중 어느 한 항에 있어서,
상기 조성물 (A) 또는 조성물 (B)는 비경구 투여되는 것을 특징으로 하는 방법. - 제 21항 내지 제 27항 중 어느 한 항에 있어서,
상기 조성물 (A) 또는 (B)는 인간 또는 동물의 순환계에 투여되는 것을 특징으로 하는 방법. - 제 21항 내지 제 28항 중 어느 한 항에 있어서,
상기 조성물 (A) 또는 조성물 (B)는 정맥 및/또는 동맥에 투여되는 것을 특징으로 하는 방법. - 제 21항 내지 제 29항 중 어느 한 항에 있어서,
상기 조성물 (A) 또는 조성물 (B)는 심장 조직으로 투여되는 것을 특징으로 하는 방법. - 제 21항 내지 제 30항 중 어느 한 항에 있어서,
