Abstract
본 발명은 다발성 경화증관련 항원에 지향된 Tr1 세포를 함유하는 조성물 및 다발성 경화증의 치료 방법에 관한 것이다.
Description
본 발명은 다발성 경화증과 같은 자가면역 질환의 치료 분야에 관한 것이다. 더욱 구체적으로, 본 발명은 다발성 경화증 관련 항원에 지향된 Tr1 세포를 포함하는 의약에 관한 것이다.
다발성 경화증은 중추신경계의 탈미엘린화(demyelinating) 및 만성 염증 질환이다. 이 질환의 조직병리학적 특징은 어느 정도의 축색 손상의 증거와 함께, 탈미엘린화 및 백색질에서의 기타 염증 세포와 함께 CD4+ 및 CD8+ T 세포가 두 가지 모두 국소 침윤된다는 것이다. 다발성 경화증의 면역 반응의 표적으로서 여겨지고 있는 미엘린 단백질으로는 미엘린 염기성 단백질 (MBP), 단백지질 단백질 (PLP), 미엘린 관련 당단백질 (MAG) 및 미엘린 희소돌기아교세포 당단백질 (MOG)을 들 수 있다.
현재 인간의 다발성 경화증을 치료하기 위한 치료적 접근방법이 다양하게 이용되고 있다. 그러나, 다발성 경화증을 완치시킬 수 있는 치료법은 존재하지 않는다. 코르티코스테로이드와 개질 β 인터페론을 비롯한 몇몇 화합물들은 다발성 경화증의 몇몇 증상을 경감시킬 수 있지만, 이들은 심각한 부작용을 나타내거나 또는 장기간 사용 하기에는 바람직하지 않은 것으로 나타났다.
다발성 경화증의 한 가지 유망한 치료법이 WO 02/077025에 기재되어 있는데, 이 문헌은 미엘린 염기성 단백질 (MBP)의 펩타이드 유사체의 용도를 개시하고 있다. 이러한 유사체를 함유하는 조성물은 심한 부작용 없이 MS의 증상을 완화시킬 수 있는 것으로 보고되고 있다. 또한, 미엘린 구성 단백질에 대한 펩타이드 유사체를 사용할 경우에도 MS용 모델 마우스에 있어서 흔히 사용되는 장기 특이적 면역 질환인 실험실 알레르기성 뇌척수염 (EAE)을 치료하는데 효과적인 것으로 나타났다. 그러나, 투여량 수준에서의 개질된 MBP 단백질의 관용성이 나빴기 때문에 몇몇 II상 임상 실험은 중단되어야만 하였다 (Bielekova et al., nature medicine, 2000, (6) 10: 1167 et Kappos et al., nature medicine, 2000, (6) 10: 1176).
또 다른 유망한 다발성 경화증의 치료 방법이 EP0587735에도 기재되어 있다. 상기 치료 방법은 자가반응성 TCR 단편과 매우 유사한 펩타이드에 면역학적으로 노출될 경우 Th2 세포의 프라이밍/인식이 증강됨으로 해서 Th1-매개형 염증에 대한 사이토킨의 조절적 제어를 유지하는데 도움이 된다는 이론에 근거한다. 임상 연구 결과 이러한 치료법은 환자들에게 안전하고 관용될 수 있는 것으로 입증되었다; 그러나, 이 치료 방법은 면역 처리된 환자의 50%에서만 효과가 있을 뿐이다.
US2004/0087018은 다발성 경화증 치료를 필요로 하는 환자에게 항원 특이적인 IL-10 생산 세포를, 바람직하게는 가용성 항원과 동시에 투여하는 것을 포함하여 이루어지는, 다발성 경화증의 치료 방법을 설명하고 있다.
본 발명자들은 놀랍게도, 가용성 항원을 함께 투여하지 않고도, 다발성 경화증 관련 항원에 지향된 Tr1 세포를 투여할 경우, 면역화된 마우스에 있어서 EAE의 진행이 극적으로 억제된다는 것을 발견하였다.
발명의 개요
본 발명은 다발성 경화증 관련 항원에 대한 적어도 하나의 Tr1 세포 집단을 함유하는 조성물에 관한 것이다. 상기한 다발성 경화증 관련 항원은 바람직하게는 미엘린 염기성 단백질, 미엘린 관련 당단백질, 미엘린 희소돌기아교세포 단백질, 단백지질 단백질, 희소돌기아교세포 미엘린 올리고단백질, 미엘린 관련 희소돌기아교세포 염기성 단백질, 희소돌기아교세포 특이 단백질, 열충격 단백질, 희소돌기아교세포 특이 단백질, NOGO A, 당단백질 Po, 말초성 미엘린 단백질 22, 2'3'-시클릭 뉴클레오타이드 3'-포스포디에스테라제로 이루어진 군으로부터 선택되는 것이 좋다.
본 발명의 또 다른 목적은 본 발명의 조성물을 함유하는 의약 또는 약학적 조성물을 제공하는데 있다.
본 발명은 또한 다발성 경화증의 치료를 필요로 하는 환자에게, 본 발명의 의약 또는 약학적 조성물의 유효량을 투여하는 것을 포함하여 이루어지는, 다발성 경화증의 치료 방법에 관한 것이기도 하다. 바람직한 실시 상태에서, 치료를 요하는 환자에게 투여될 의약 또는 약학적 조성물은 상기 환자의 세포에 자가적인 (autologous) Tr1 세포를 함유하는 것이 좋다.
본 발명의 또 다른 실시 상태에서, 다발성 경화증의 치료를 필요로 하는 환자의 다발성 경화증의 치료 방법은 상기 환자에게 본 발명의 의약 또는 야각적 조성물의 유효량을 다발성 경화증의 치료에 사용되는 또 다른 치료제와 조합하여 투여하는 것을 포함한다.

다발성 경화증 환자들은 임상적으로 명확한 다발성 경화증 진단을 수립하는 판단 기준에 의해 동정될 수 있다. 간략히 설명하면, 임상적으로 명백히 다발성 경화증에 걸린 개체는 두 번의 발병 (two attacks) 경험이 있고, 2개 병소의 임상적 증거 또는 1개 병소의 임상적 증거와 또 다른 별개 병소에 대한 준의료적인 증거를 갖는다. 또한 두 번의 발병과 뇌척수액 중 IgG의 올리고클론 밴드가 존재한다는 증거 또는 한 번의 발병, 2개 병소의 및 퇴척수액 중 IgG의 올리고클론 밴드의 임상적 증거가 있어도 명백히 다발성 경화증에 걸린 것으로 진단할 수 있다. 이 맥도날드 기준은 또한 다발성 경화증을 진단하는데도 이용될 수 있다. 맥도날드 기준은 다발성 경화증의 임상적 발병 부재시, 다발성 경화증 진단에 사용될 경시적인 CNS 손상의 MRI 증거의 사용을 포함한다. 다발성 경화증의 효과적인 치료는 여러 가지 방법으로 평가될 수 있다. 치료 효율을 평가하기 위해 다음의 변수들을 사용할 수 있다. 두 가지 예시적인 기준으로는: EDSS (심층 장애도: extended disability status scale)과 MRI (자기공명영상화) 상 악화된 외견을 들 수 있다. EDSS는 다발성 경화증에 기인한 임상적 손상 정도를 등급화하기 위한 수단이다. 신경 손상의 유형과 위중도에 따라 8 가지 기능 시스템 별로 평가된다. 간단히 설명하자면, 치료에 앞서, 환자들을 다음의 시스템, 즉: 추체 (pyramidal), 소뇌, 뇌간, 감각, 장 및 방광, 시각, 뇌 및 기타에 따라 평가한다. 점수 범위는 0 (정상) 내지 10 (다발성 경화증으로 인한 사망)이다. 완전한 하나의 단계가 감소하면 치료가 유효한 것이다.
악화란 다발성 경화증에 기인한 새로운 증상의 출현과 적절한 새로운 신경학적 비정상 상태를 수반하는 것으로 정의된다. 이에 더하여, 악화는 적어도 24 시간 동안 지속되어야 하며 이에 앞서서 적어도 30일간 안정 또는 개선이 선행되어야 한다. 간략히, 임상의가 환자에게 표준 신경 검사를 실시한다. 악화는 신경학적 등급 점수의 변화에 따라 약하거나, 중간 정도거나 또는 심한 것이다. 연간 단위의 악화율과 증상이 악화되지 않은 환자의 비율을 측정한다.
이들 두 가지 측정 방식 중 어느 것에서든지 치료군과 위약군 사이에 악화되지 않거나 재발되지 않은 환자들의 비율에 통계적으로 유의적인 차이가 있으면 그 치료법은 유효한 것으로 간주할 수 있다. 또한, 최초의 악화까지 걸린 시간과 악화 기간 및 위중도 역시도 측정할 수 있다. 이와 관련하여 치료 효율의 척도는 대조군과 비교할 때 치료군에서 최초 악화까지 걸린 시간 또는 악화 기간 및 악화 위중도에 있어서의 통계적으로 유의적인 차이이다. 악화되지 않거나 재발되지 않은 기간이 1년 이상, 18개월 이상 또는 20개월 이상이면 특히 효과가 현저한 것이다.
화학적 측정 방법에는 1년 및 2년 간격에 있어서의 재발률 및 6개월 이상 지속된 EDSS에 있어서 기본 1.0점부터 증상이 진행되기까지의 시간을 비롯한, EDSS의 변화가 포함된다. 카플란-마이어 곡선 상에서, 장애도의 일관된 진전이 지체되는 경우에는 효과가 있는 것이다. 그 밖의 기준으로는 MRI 상의 T2 영상의 면적 및 체적의 변화와, 가돌리늄 개선 영상에 의해 측정되는 병소의 갯수와 체적의 변화를 들 수 있다. MRI는 가돌리늄-DTPA-개선형 영상화를 이용함으로써 활성 병소를 측정하거나 또는 T2-칭량 기술을 이용하여 병소의 위치와 정도를 측정하는데 이용할 수 있다. 간단히 기본 MRI를 얻는다. 각각의 후속 연구에서는 동일한 영상 플레인과 환자 위치를 이용한다. 병소의 검색을 최대화하고 병소 추적을 용이하게 하기 위해서 위치 잡기와 영상화 순서를 선택할 수 있다. 후속 연구에서 동일한 위치 잡기 및 영상화 순서를 이용할 수 있다. 다발성 경화증 병소의 존재 여부, 위치 및 정도를 방사선 전문의가 측정할 수도 있다. 총 병소 면적에 있어서는 병소 단편들을 합하여 병소의 면적을 대략 추정할 수 있다. 세 가지 분석, 즉 새로운 병소의 증거, 활성 병소의 출현율, 병소 면적 변화의 백분율에 대한 분석을 수행할 수 있을 것이다. 치료에 따른 증상 개선은 치료군 대 위약군 사이의 비교 또는 베이스라인과 비교한 개별 환자에 있어서 통계적으로 유의적인 개선 여부에 의해 수립될 수 있다.
다발성 경화증은 각 경우 마다 그 양태와 후속적인 경과에 있어서 몇몇 패턴 중 한 가지를 나타낸다. 가장 흔한 경우는, 우선, 다발성 경화증이 몇 차례 발병하여 증상을 보인 다음, 알 수 없는 이유로 증상이 호전되어 완전히 또는 부분적으로 관해(remission)를 보이다가, 안정기가 지난 후 다시 증상이 재발하는 것이다. 이는 재발 관해형 (RR: relapsing-remitting) 다발성 경화증으로 칭해진다.
일차성 진행형 (PP: primary-progressive) 다발성 경화증은, 비록 일시적인 고조기 (plateaus) 또는 약간의 증상 호전이 있을 수 있으나, 뚜렷한 관해 없이 임상적으로 점차 쇠퇴 (decline)한다는 특징을 갖는다.
이차성 진행형 (SP: secondary-progressive) 다발성 경화증은 재발 관해형 경로로 출발하여 나중에는 일차성 진행형 경과가 이어진다. 드물지만, 질병이 진행성 경로로 일어나지만 중간, 중간 급성 발병이 일어나는, 진행 재발형 (PR: progressive-relapsing) 경과를 겪는 환자들도 있다.
PP, SP, 및 PR은 때로는 합쳐져서 만성 진행형 다발성 경화증으로 불리기도 한다. 소수의 환자들은 급격하고 가차없이 진행되는 악성 다발성 경화증을 겪는데, 이 경우 발병 개시 후 심각한 장애가 오거나 심지어는 얼마 안 되어 사망에 이르기도 한다.
본 발명
본 발명은 다발성 경화증 관련 항원에 대한 Tr1 세포 집단을 하나 이상 함유하는 조성물에 관한 것이다.
본 발명의 한 가지 실시 상태에서, Tr1 세포는
a) 대상자로부터 전구세포 집단 (progenitor cell population)을 분리하는 단계,
b) 상기 전구세포 집단을 IL-10의 존재 하에 배양함으로써 수지상 세포 집단을 얻는 단계,
c) 상기 단계 b)에서 얻은 세포를 다발성 경화증 관련 항원 존재 하에 상기 대상자로부터 분리된 CD4+ T 임파구 집단과 접촉시킴으로써, 상기 항원에 지향된 CD4+ T 세포를 Tr1 세포 집단으로 분화시키는 단계, 및
d) 상기 단계 c)에서 얻은 Tr1 세포 집단을 회수하는 단계
에 의하여 얻을 수 있다.
단계 b)에서, IL-10은 배양 배지 중 50 내지 250 U/ml, 바람직하게는 100 U/ml의 농도로 존재한다. Tr1 세포를 얻기 위한 상기 방법은 Wakkach 등의 문헌 (Immunity 2003 May; 18(5):605-17)에 알려져 있다.
상기 방법은 또한 덱사메타손과 비타민 D3를 이용하여 수행할 수도 있고, 또는, b) 단계의 DS 대신 관용화 (tolerogenised) 되거나 미성숙한 DC를 이용하여 수행할 수도 있다.
본 발명의 또 다른 실시 상태에서, Tr1는 다음 단계, 즉:
a) 대상자로부터 분리된 다발성 경화증 관련 항원에 대한 CD4+ T 세포 집단을 적절한 양의 IFN-α와 함께 배지에서 배양하는 단계;
b) Tr1 세포 집단을 회수하는 단계
를 포함하는 방법에 의해 얻을 수 있다.
IFN-α는 배지 중에 5 ng/ml의 농도로 존재하는 것이 좋다. 단계 a)에서, 배지는 IL-10을 적절한 양으로, 바람직하게는 100 U/ml의 양으로 더 함유할 수도 있다.
단계 b)에서는 Tr1 세포 집단을 IL-15를 함유하는 배지에서 배양하여 증식시키는데, 여기서 IL-15는 배지 중 5 ng/ml의 양으로 존재하는 것이 좋다. Tr1 세포를 얻기 위한 전술한 방법은 미국특허 제6746670호에 설명되어 있다.
본 발명의 또 다른 실시 상태에서, Tr1는 다음 단계, 즉:
a) 인공 항원 제시 세포에 의해 제시된 다발성 경화증 관련 항원의 존재 하에 CD4+ T 세포 집단을 시험관내 활성화시키는 단계; 및
b) Tr1 세포를 10% 이상 함유하는 활성화된 CD4+ T 세포를 회수하는 단계
를 포함하는 방법에 의하여 얻을 수 있다.
바람직하게는, 인공 항원 제시 세포는 HLA II계 분자와 인간의 LFA-3 분자는 발현하지만, 공자극 (co-stimulation) 분자인 B7-1, B7-2, B7-H1, CD40, CD23 및 ICAM-1은 발현하지 않는 것이 좋다.
Tr1 세포를 얻기 위한 상기 방법은 특허출원 WO02/092793호에 설명되어 있다.
본 발명의 또 다른 바람직한 실시 상태에서, Tr1 세포는 다음 단계, 즉:
a) 다발성 경화증 관련 항원과 적절한 양의 IL-10의 존재 하에 CD4+ T 세포를 시험관내 활성화시키는 단계; 및
b) Tr1 세포 집단을 회수하는 단계
를 포함하는 방법에 의하여 얻을 수 있다.
바람직하게는, 배지 중에 존재하는 IL-10은 100 U/ml인 것이 좋다. 상기 방법은 Groux 등의 문헌 (Nature 1997, 389(6652):737-42)에 설명되어 있다.
본 발명의 또 다른 실시 상태에서, 항원 특이적인 Tr1 세포는 다음 단계, 즉:
a) 백혈구 집단 또는 말초 혈액 단핵 세포 (PBMC) 집단을 다발성 경화증 관련 항원으로 자극하는 단계,
b) 상기 자극된 집단으로부터 항원 특이적인 Tr1 세포 집단을 회수하는 단계,
c) 필요에 따라 상기 항원 특이적인 Tr1 세포 집단을 증폭시키는 단계
를 포함하는 방법에 의하여 얻을 수 있다.
백혈구는 그의 중요도, 분포 상황, 갯수, 수명 및 잠재능력에 따라 특징지어지는 여러 가지 유형의 세포들을 포괄한다. 이러한 유형으로는 다음을 들 수 있다: 다핵형 또는 과립형 백혈구 (이들 중에는 다시 호산구성 백혈구, 호중구성 백혈구 및 호염기성 백혈구가 있다) 및 큰 백혈구 세포이면서 면역계의 세포 유형을 구성하는 (임파구 및 단핵구) 세포 유형인 단핵 세포 또는 말초 혈액 단핵 세포 (PBMC). 백혈구 또는 PBMC는 본 발며이 속한 기술 분야에 잘 알려져 있는 방법에 의해 말초 혈액으로부터 분리될 수 있다. PBMC의 분리를 위해서는, 원심분리법을 사용하는 것이 유리하고, 밀도 구배 원심분리법이 바람직하며, 특히 좋기로는 불연속형 밀도 구배 원심분리법이 좋다. 별법으로 특이적인 모노클로날 항체를 사용하는 방법이 있다. 특정 실시 상태에서 PBMC는 일반적으로 표준 공정을 이용하는 피콜-하이파크에 의하여 전혈 생성물로부터 분리된다. 또 다른 실시 상태에서 PBMC는 백혈구성분 채집술 (leukapheresis)에 의해 회수된다.
상기 방법은 특허출원 WO2007/010406에 설명되어 있다.
또 다른 실시 상태에서, Tr1 세포는 다음 단계, 즉:
a) 백혈구 집단 또는 말초 혈액 단핵 세포 (PBMC) 집단을 다발성 경화증 관련 항원 존재 하에 중간엽 줄기 세포와 함께 배양하는 단계,
b) Tr1 세포 집단을 회수하는 단계
를 포함하는 방법에 의하여 얻을 수 있다.
상기 방법은 또한 PBMC나 백혈구 대신에 나이브 T 세포 또는 메모리 T 세포를 이용하여 수행할 수도 있다.
이렇게 얻은 Tr1 세포 집단은 인터류킨-2 및 인터류킨-4와 같은 사이토킨의 존재 하에 배양시킴으로써 더 증폭시킬 수도 있다. 다른 한편, Tr1 세포 증폭 배양시 인터류킨-15 및 인터류킨-13도 사용할 수 있다.
전술한 방법에서, Tr1 세포는 WO2005/000344에 설명된 동정 방법에 의하여 특징화될 수 있다. Tr1 세포의 상기 동정 방법은 CD18 및/또는 CD11a, 및 CD49b으로 이루어진 군으로부터 선택되는 분자들과 CD4 분자를 코딩하는 유전자의 발현 산물의 동시 존재 여부를 검출하는 것에 기반한다. Tr1 세포는 상기 마커에 대한 항체를 이용하는 면역친화법, Elisa, 또는 유동 세포계수법에 의해 동정 및/또는 정제될 수 있다.
Tr1 세포는 또한 유동 세포계수법 또는 마그네틱 비드를 이용하여 양성 선별 또는 음성 선별법에 의해 농축 (enrich)시킬 수도 있다. 이러한 방법은 WO2005/000344에도 설명되어 있다.
본 발명의 또 다른 실시 상태에서, 다발성 경화증 관련 항원에 대한 Tr1 세포를 WO2006/108882에 설명된 시험관내 방법에 의해 증폭시킬 수도 있다. 상기 방법은 다음 단계, 즉:
a) 35℃ 미만의 온도 T1 하에, 배양 배지 Mf 중에서 곤충 공급자 세포 (feeder cell)와 같은 공급자 세포를 배양하는 단계, 단, 상기 온도 T1은 공급자 세포의 증식을 허용하는 온도이고, 상기 공급자 세포는 하기 세포 표면 단백질과 상호반응하는 인자를 발현하는 세포이다:
- CD3/TCR 복합체,
- CD28 단백질,
- IL-2 수용체,
- CD2 단백질,
- IL-4 수용체,
b) 세척되거나 또는 배양 배지 Mf가 포함되지 않은, 상기 단계 a)로부터 얻어진 공급자 세포를, 배양 배지 Mp에 함유된 Tr1 세포 집단과 접촉시키는 단계, 단, Tr1 세포 집단, 공급자 세포 및 배양 배지를 함유하는 혼합물을 얻기 위해, 상기 배양 배지 Mp는 처음에는 단계 a)에서 인용된 인자들을 함유하지 않는다,
c) Tr1 세포 집단은 증식하도록 하지만 공급자 세포는 증식하지 않도록 선택된 온도 범위인 35℃ 이상의 온도 T2에서 상기 단계 b)에서 얻은 혼합물을 배양하는 단계,
d) 이렇게 증폭된 Tr1 세포 집단을 회수하는 단계
를 포함하여 이루어지는 방법에 의하여 얻을 수 있다.
전술한 세포 표면 단백질과 상호반응하는 인자들의 예로는 다음을 들 수 있다:
- CD3 중쇄의 항CD3 세포질내 도메인이 막투과 도메인에 의해 대체되도록 변형된 항CD3 항체,
- CD80 또는 CD86 단백질,
- 공급자 세포에 의해 분비된 IL-2,
- CD58 단백질,
- IL-4 및IL-13을 포함하는 군으로부터 선택된 인터류킨.
본 발명의 바람직한 실시 상태에서, 다발성 경화증 관련 항원에 대한 상기 Tr1 세포는 통상적인 T 세포 클로닝 방법에 의해 클로닝시킬 수 있다.
본 발명의 바람직한 실시 상태에서, 다발성 경화증 관련 항원에 대해 지향된 하나 이상의 Tr1 세포 집단 또는 다발성 경화증 관련 항원에 대해 지향된 Tr1 세포의 하나 이상의 클론을 포함하는 상기 조성물은 냉동 보관할 수 있다.
본 발명의 바람직한 실시 상태에서, 상기 다발성 경화증 관련 항원은 미엘린 염기성 단백질 (MBP), 미엘린 관련 당단백질 (MAG), 미엘린 희소돌기아교세포 단백질 (MOG), 단백지질 단백질 (PLP), 희소돌기아교세포 미엘린 올리고단백질 (OMGP), 미엘린 관련 희소돌기아교세포 염기성 단백질 (MOBP), 희소돌기아교세포 특이 단백질 (OSP/클라우딘 11), 열충격 단백질, 희소돌기아교세포 특이 단백질 (OSP), NOGO A, 당단백질 Po, 말초성 미엘린 단백질 22 (PMP22), 2'3'-시클릭 뉴클레오타이드 3'-포스포디에스테라제 (CNPase), 이들의 단편, 변형체 및 혼합물을 포함하는 군으로부터 선택된다.
바람직하게는, 상기 다발성 경화증 관련 항원은 미엘린 염기상 단백질 (MBP), 단백지질 단백질 (PLP) 및 미엘린 희소돌기아교세포 단백질 (MOG) 펩타이드 및 이들의 단편, 변형체 및 혼합물로 이루어진 군으로부터 선택되는 것이 좋다.
더욱 바람직하게는, 상기 다발성 경화증 관련 항원은 HLA-DR2 양성 대상자의 경우, MBP 82-98, MBP 83-99, MBP 151-170를 포함하는 군으로부터 선택되는 것이 좋다.
더욱 바람직하게는, 상기 다발성 경화증 관련 항원은 HLA-DR2 양성 대상자의 경우, MOG 35-55, MOG 21-40, MOG 41-60, MOG 71-90, MOG 81-100, MOG 111-130, MOG 63-37를 포함하는 군으로부터 선택되는 것이 좋다.
더욱 바람직하게는, 상기 다발성 경화증 관련 항원은 HLA-DR4 양성 대상자의 경우, MBP 111-129, MBP 116-123를 포함하는 군으로부터 선택되는 것이 좋다.
더욱 바람직하게는, 상기 다발성 경화증 관련 항원은 HLA-DR4 양성 대상자의 경우 MOG 21-40, MOG 97-108, MOG 71-90, MOG 181-200를 포함하는 군으로부터 선택되는 것이 좋다.
본 발명의 또 다른 목적은 전술한 조성물을 포함하는 의약을 제공하는 것이다.
본 발명은 또한 전술한 조성물을 1종 이상의 약학적으로 허용되는 담체와 복합적으로 함유하는 약학적 조성물을 제공하는 것이다.
본 발명에서 유용한 약학적으로 허용되는 담체는 통상적인 것들이다. 본 발명의 조성물을 약학적으로 전달하는데 적합한 조성과 포뮬레이션은 Remington's Pharmaceutical Sciences [제6판, Osol, A. Ed. 본 발명의 한 가지 실시 상태에서는 대상자에게 104/kg 내지 109/kg 세포를 투여한다. 바람직하게는 105/kg 내지 107/kg 세포를, 더욱 바람직하게는 대상자에게 약 106/kg 세포를 투여하는 것이 좋다.
본 발명의 한 가지 실시 상태에서는, 대상자의 임상 상태의 쇠퇴에 의해 입증되는 돌발적 증상 악화 (flare-up)시, 또는 중추신경계 내에서 염증성 병소가 예컨대 MRI에 의해 시각화될 수 있는 경우, 대상자에게 의약을 투여한다.

본 발명의 네번째 실시 상태에서는, 본 발명에 따른 의약 또는 약학적 조성물을 1년에 1회 내지 2회 대상자에게 투여한다.
본 발명의 또 다른 실시 상태에서, 다발성 경화증의 치료를 요하는 대상자에게 투여될 의약 또는 약학적 조성물은 상기 대상자의 세포에 자가적인 Tr1 세포를 함유한다.
이것은 Tr1 세포 자신이 유래된 대상자에게 Tr1 세포를 투여하거나 또는 Tr1 세포의 생산에 이용된 전구체가 유래한 대상자에게 Tr1 세포를 투여한다는 것을 의미하는 것이다.
본 발명의 또 다른 실시 상태에서, 다발성 경화증의 치료를 필요로 하는 대상자에게 본 발명의 의약 또는 약학적 조성물의 유효량을, 다발성 경화증을 치료하는데 사용되는 1종 이상의 다른 치료제와 조합하여 투여하는 것을 포함하는, 다발성 경화증의 치료 방법이 제공된다.

- BHT-3009와 아토바스타틴.
본 발명의 바람직한 실시 상태에서, 다발성 경화증의 치료 방법은 상기 치료를 요하는 대상자에게 본 발명의 의약 또는 약학적 조성물의 유효량을 인터페론-β, 글라티라머 아세테이트, 미톡산트론, 시클로포스파미드, 메토트렉세이트, 아지트로핀 또는 나탈리주맙을 포함하는 군에서 선택된 1종 이상의 치료제와 복합적으로 투여하는 것을 포함한다.
본 발명은 상기 대상자에게, 본 발명의 약학적 조성물 또는 의약의 유효량을 인터페론-β, 글라티라머 아세테이트, 미톡산트론, 시클로포스파미드, 메토트렉세이트, 아지트로핀 또는 나탈리주맙을 포함하는 군에서 선택된 1종 이상의 치료제와 복합적으로 투여하는, 본 발명에 따른 약학적 조성물 또는 의약의 용도에 관한 것이다.

"부적절한 반응", "적절하게 반응하지 않다" 또는 "적절하게 반응할 것 같지 않다"라는 표현은, 그 대상자에 관한 한, 해당 치료법이 효과가 없었거나 (또는 효과가 없을 것 같거나), 유독하였던가 (또는 유독할 것 같던가), 또는 잘 관용되지 않았던 (또는 잘 관용되지 않을 것 같은) 것을 의미한다.
인터페론-β, 글라티라머 아세테이트, 미톡산트론, 시클로포스파미드, 메토트렉세이트, 아지아트로핀 또는 나탈리주맙으로 이루어진 군 중 1종 이상의 치료제를 이용한 치료와 같은 다발성 경화증의 통상적인 치료에 대하여 적절하게 반응하지 않거나, 또는 적절하게 반응할 것 같지 않은 대상자들은 본 발명이 속한 기술 분야의 당업자에게 통상적으로 잘 알려진 EDSS (심층 장애도: Expanded Disability Status Scale) 점수를 이용함으로써 확인할 수 있다.

분화 요법에 제공된 나이브 CD4+ 세포를 48 시간에 걸쳐 항CD3 + 항CD28 모노클로날 항체로 활성화시켰다. 이어서, 배양체 상등액에 있어서의 IL-4, IL-10 및 IFN-γ의 존재 여부를 ELISA에 의하여 시험하였다.

실시예
다음의 설명에서, 구체적인 프로토콜이 적시되지 않은 실험들은 모두 표준 프로토콜에 따라 실시된 것이다.
다음의 실시예들은 본 발명의 바람직한 실시 상태를 입증하기 위해 포함된 것이다. 후술되는 실시예에 기재된 기술들은 본 발명을 수행시 본 발명자에 의해 잘 기능하는 것으로 발견된 기술을 대표적으로 나타낸 것이고, 따라서 바람직한 형태를 구성하는 것으로 간주되어야 한다. 그러나, 본 발명의 개시 내용에 비추어, 당업자들은 본 발명의 정신과 범위로부터 일탈함이 없이, 본 명세서에 기재된 특정 실시 상태에 대하여 다양한 변형을 가할 수 있음과 그럼에도 불구하고 여전히 유사하거나 균등한 결과를 얻을수 있음을 이해할 것이다.
실험 공정
마우스
C57Bl/6 마우스를 쟝비에 (Le Genest-St-Isle, France)로부터 입수하였다. C57Bl/6 백그라운드 상의 MOG35-55 특이-TCR 형질전환 마우스를 Pr. Liblau (Inserm U563, Hopital Purpan, Toulouse)의 연구실에서 하우징하였다. 모든 마우스들은 7주 내지 8주령의 암컷이었다.
항체 및 시약
마우스 세포의 정제 및 특징화를 위해 다음의 항체들을 이용하였다: 항CD4 (H129-19) 항CD62L (Mel-14), (BD-Pharmingen, Le Pont de Claix, France). MOG35 -55 펩타이드를 바켐사 (Voisin-le-Bretonneux, France)으로부터 구입하였다. IL-2를 카이론 코포레이션사 (Emmeryville, CA, USA)으로부터 입수하였다. IL-4, IL-12 및 항IL12를 알앤드 시스템스사 (Minneapolis, USA)로부터 구입하였다.
T 세포 정제 및 배양
T 세포 배양에 사용된 배지는 FCS, Yssel 배지 및 β2-머캅토에탄올 (Sigma)이 보강된 FCS Iscove 배지 (Invitrogen)였다. MOG35 -55 - 특이 TCR 형질전환 마우스의 비장세포를 먼저, FITC-컨쥬게이트된 항CD62L 및 PE-컨쥬게이트된 항CD4로 표지시켰다. 이어서, CD4+CD62L+ T 세포를 FACStar SE (Becton Dickinson, France) 상에서 소팅시켰다. MOG35 -55에 지향된 마우스의 Th1, Th2 및 Tr1 세포를 다음과 같이 시험관내 분화 후에 얻었다: 2.5x105 Sorted CD4+CD62L+ T 세포를 24웰 플레이트 중, 4.106 조사된 동계 (syngeneic) 비장세포 및 MOG35 -55 펩타이드(10mg/ml)의 존재 하에 배양하였다. T 세포가 각각 Th1, Th2 및 Tr1 세포로 분화되도록, IL-12 (20ng/ml), IL-4 (40ng/ml) 플러스 항IL-12 (5mg/ml) 또는 IL-10 (50ng/ml)을 첨가하였다. 세포들을 37℃, 5% CO2에서 배양하고, Th1의 경우 IL-2 (100UI/ml)가 보강된 배지에서, Th2 및 Tr1 세포의 경우, IL-2 플러스 IL-4 (20ng/ml)가 보강된 배지에서 필요에 따라 분할하였다. 별법으로, Tr1 세포를 IL-10 (5ng/ml)이 보강된 배지에서도 분할하였다. 2주 또는 3주의 기간 동안 1주일에 1회 T 세포 집단을 재자극하였다.
사이토킨
분석
항CD3 (10㎍/ml) + 항CD28 (1㎍/ml)로 자극된 T-세포 집단의 상등액에 대하여 샌드위치 ELISA를 48 시간 동안 수행하였다. 간단히 설명하면, 평평한 바닥의 96 웰 플레이트 중, 코팅된 항CD3 및 가용성 항CD28 모노클로날 항체로 5.105 세포를 활성화시키고 37℃, 5% CO2에서 48 시간 동안 배양하였다. 항IL-4 (11B11), 항IL-10 (2A5), 항IFN-g (XGM1.2), 바이오틴 항IL-4 (24G2), 항IL-10 (SXC1), 항IFN-g (R4-6A2) (Pharmingen Becton Dickinson)을 이용하여 ELISA를 수행하였다.
실험용 자가면역 뇌척수염
Cua 등의 문헌 (J. Exp. Med, 1999)에 설명된 프로토콜에 따라 자가면역 뇌척수염 실험을 수행하였다. 간략히 설명하면, 완전 프로인트 아쥬반트 중에 제조된 마우스 척수 균질물 2.5 mg을 C57BL/6 마우스에게 피내 주사하였다. 2일 후, 백일해 독소 200 ng을 마우스에게 복강내 주사에 의해 투여하였다. 동일한 면역 요법을 J8 및 J9에서 각각 척수 및 백일해 독소에 대해 반복하였다. 제10일부터 매일 임상 점수를 평가하였으며 점수는 다음과 같다: 0= 질병 없음; 1 = 꼬리 마비; 2 = 뒷다리 약화; 3 = 뒷다리 마비 - 4 - 뒷다리와 앞다리 마비; 5 = 다 죽어가는 상태. 정맥 투여 경로로 제 9일에 T 세포 집단 (3.105 세포/마우스)를 주사하였다.

본 발명자들은 먼저 항MOG35 -55 TCR 형질전환 마우스로부터 분리된 나이브 CD4+ T 임파구로부터 항MOG35 -55 CD4+ T 임파구 집단을 분화시켰다. 이들 마우스로부터 얻은 모든 임파구는 H-2b 분자 측면에서 제시된 펩타이드 MOG35 -55를 특이적으로 인식하는 특이 TCR을 산생한다. 나이브 세포 (CD4+CD62L+)를 소팅하고 조사된 동계 비장 세포 및 MOG35 -55 펩타이드를 이용하여 시험관내에서 활성화시켰다. Th1 세포를 분화시키기 위해, IL-12를 첨가하고, Th2 세포를 분화시키기 위해 IL-4와 항IL12 모노클로날 항체를 첨가하였으며, Tr1 세포를 분화시키기 위해 IL-10을 첨가하였다. 2주 또는 3주 자극시킨 후, 세포를 수확하고 항CD3 + 항CD28 자극 하에서 이들의 사이토킨 생산 여부를 시험하였다.
본 발명자들은 IL-10의 존재 하에 분화를 위해 제공된 세포들이 IL-10은 많이, IL-4는 적게 생산하는, Tr1 세포의 전형적인 사이토킨 생산 패턴을 획득하였음을 관찰하였다. 분화중인 세포를 IL-4 자극하자 Tr1 세포는 IL-4와 IL-10을 동등하게 생산하였으나 IFN-γ는 생산하지 않았다. 상당량의 IL-10 생산에도 불구하고, 분화하는 세포를 IL-12로 자극하자 IFN-γ는 많이 생산하고 IL-4는 생산하지 않는 Th1 사이토킨 생산 프로파일을 나타내는 세포가 얻어졌다.

본 발명자들은 다음으로, 분화된 항MOG T 세포 집단이 실험용 자가면역 뇌척수염에 미치는 효과를 평가하였다. 이 목적을 위해, C57Bl/6 마우스를 완전 프로인트 아쥬반트 중 마우스 척수 균질물 (msch)로 면역화시킨 다음 하루 후에 백일해 독소를 복강내로 투여하였다. 제8일과 제일에 동일한 요법을 msch와 백일해 독소에 대해 각각 반복하였다. 제9일에 면역화된 마우스에게 항MOG Th1, Th2 및 Tr1 세포 집단을 정맥 주사하고 하루 한번씩 임상 점수를 평가하였다. 도 2는 항MOG CD4 양성 T 세포가 실험용 뇌척수염 발병에 미치는 충격을 보여준다. 본 발명자들의 관찰에 의하면, MOG35 -55 펩타이드에 대해 지향된 Th1 및Th2 세포들은 msch 면역화에 의해 유도된 EAE에 대해서는 유의적인 효과를 나타내지 않았다. 이와 대조적으로, 항MOG35-55 Tr1 세포를 투여하자, 면역화된 마우스에서 EAE의 진행이 크게 억제되었다. 실제로, 미엘린 항원에 대해 지향된 Tr1 세포로 치료된 마우스들은 가벼운 꼬리 마비를 일으킨 반면, 대조군 마우스는 뒷다리의 운동 기능이 상실되었다. 중요한 것은, Tr1 투여가 질병의 발병을 억제할 뿐만 아니라, 비T 세포로 치료된 마우스에서도 재발을 예방한다는 사실이다. 이전의 연구 결과 (Barrat 외, J Exp Med, 2002)는 항오브알부민 Tr1 세포가 마우스에서 EAE를 방지한다는 것을 보여주었다. 이러한 억제 효과는 오브알부민의 두개내 점적주입 후에야 달성되었는데, 이는 뇌 내의 Tr1 세포의 특이 항원 활성화가 그들의 이펙터 기능에 있어서 전제 조건이 됨을 가리키는 것이다. 본 발명자들은 따라서 중추신경계의 고유 성분으로서 자가 항원이 억제자 세포 활성화제와 같은 역할을 수행할 수 있는지를 평가하고자 하였다. 본 발명자들의 실험 결과는 이러한 의문에 긍정적인 답변을 제시하였는데, 즉, 미엘린 항원에 대하여 지향된 Tr1 세포가 생체내에서 뇌척수염을 억제할 수 있음을 보여주었다.
