Abstract
본 발명은 골다공증을 치료하는 신규한 치료 방법을 설명한다. 이독시펜이 바람직한 화합물이다.
Description
폐경기에 발생하는 에스트로겐의 감소는 폐경기후 여성에게 있어서 골다공성 골절 및 심혈관 질환의 발생률을 증가시키는 중요한 병인학적 요인이다. 에스트로겐 대체 요법 (ERT)에 의해 폐경기 후의 골 손실을 예방할 수 있다 하더라도, 무저항 ERT는 자궁내막암의 위험을 증가시킨다. 생식 조직에 원치않는 영향을 주지 않으면서 에스트로겐의 뼈 및 심혈관에 대한 바람직한 효과를 유지하는 치료법이 이상적이다. 타목시펜 (tamoxifen)은 폐경기후의 여성에게 있어서 콜레스테롤 수치를 저하시키고 골 손실을 방지한다고 입증된 항에스트로겐제이다. 타목시펜은 또한 난소를 적출한 쥐의 골다공증 모델에서도 효과적이다. 그러나, 타목시펜은 원치않는 부작용, 특히 자궁내막 증식증 및 자궁내막암을 일으키는 것으로 나타났다. 문헌 [1. 〈발명의 요약〉
본 발명은 자궁에 대한 명백한 영향을 주지 않으면서 폐경기후 질환을 예방 및 치료하는 방법을 제공한다. 이 방법은 하기 화학식 I의 화합물 또는 그의 제약학상 허용가능한 산부가염의 유효량을 필요로하는 사람에게 투여하는 것을 포함한다.
식 중, X는 3- 또는 4-요오도 또는 브로모를 나타내고, 동일하거나 상이할 수 있는 R1및 R2는 C1-3알킬, 특히 메틸 또는 에틸기를 나타내거나, 또는 R1은 수소 원자를 나타내고 R2는 C1-3알킬기를 나타내거나, 또는 R1및 R2는 그들이 결합되어 있는 질소 원자와 함께 통상 5 또는 6개의 고리 원자를 갖는 포화 헤테로시클릭기를 나타내며, 특히 피롤리디노, 피페리디노, 4-메틸피페리디노 또는 모르폴리노기를 나타낸다.
본 발명은 골다공증 치료에 유용하다고 알려져 있는, 에스트로겐 수용체에 결합하는 치료제에 관한 것이다.
본 발명은 종래에 제조되어 에스트로겐 수용체-양성 유방암의 치료에 효과적이라고 평가된 일군의 화합물을 사용하여, 폐경기후 질환을 예방 및 치료하는 치료법에 관한 것이다. 이런 화합물들이 상기의 화학식 I 및 미국 특허 제4,839,155호에 개시되어 있다.
설명된 치료법에 있어 바람직한 화합물은 하기의 (E)-1-[2-[4-[1-(4-요오도페닐)-2-페닐-1-부테닐]페녹시]피롤리딘이다.
이런 화합물들은 에스트로겐 수용체에 결합하여, 연구되고 있는 조직에 따라 에스트로겐 아고니스트 작용 또는 길항 작용을 일으킨다고 알려져 있다.
"폐경기후 질환"이란 용어는 골다공증, 및 심근경색증 및 발작과 같은 동맥경화성 심혈관 질환 및 혈장내 콜레스테롤의 증가를 말한다. 본 발명의 방법은 골 손실을 방지하고, 동맥경화증의 위험 감소와 관련된 혈장 지질 프로파일을 만드는데 유용하다.
골 손실을 예방하는 능력은 난소를 적출한 쥐의 골다공증 모델의 연구 및 폐경기후 여성의 연구에 의해 평가된다.
조직형태 연구를 통해 성숙한 암컷 쥐로부터 난소를 제거했을 때, 기부 경골의 골간단에서 점진적인 골 손실이 일어난다는 것을 알 수 있었다. 난소를 적출 (OVX)하고 3개월 후에는, 과도한 골 흡수가 나타나는 골 교체 회전수의 증가로 해면질 골의 60 내지 70 %가 제거된다. 골 손실은 느린 속도지만 요부 척추에서도 일어난다. (에스트로겐의 대체 복용에 의해 완전히 방지할 수 있는) 쥐의 OVX-유도 골 손실은 가장 광범위하게 사용되고 최상의 특성화된 골다공증 동물 모델의 기초를 이룬다.
7 내지 8 개월된 스프라크-더리 (Spraque-Dawley) 쥐를 사용하였다. 기준 골 무기물 밀도 (BMD)를 척추의 3 내지 6군데의 요추 영역 및 기부 경골의 골간단에서 이중 에너지 x-선 흡광도법 (DXA)에 의해 측정하였다. 이 쥐들을 요추 BMD의 평균값 및 표준 편차값이 대략 동일한 8 내지 10개의 그룹으로 나누었다. 모든 쥐들을 양쪽 난소 적출 수술 (OVX)을 하고, 각 실험에서 하나의 그룹은 가짜로 수술을 하였다.
이독시펜은 1 % 카르복시메틸 셀룰로오스 수용액 중의 현탁액제로 경구 복용용으로 제조되었다. 쥐들에게 일일 1회 경구로 가바즈 (gavage)를 통해 복용시켰다. 각 실험에서는 하나의 OVX 그룹 및 모의 그룹에게 가바즈를 통해 일일 1회 경구 복용 부형제를 주었다. 하기의 외과 수술을 한 다음날, 복용을 시작하였다.
처치하고 2주 후에 혈장내 콜레스테롤 수치를 측정하였다. 요추 및 경골의 BMD를 1달 간격으로 측정하였다. 상기 동물들을 치사시켜 자궁을 제거하고, 습윤 중량을 측정하였다. 사후에 경측골을 모아서 끼워 넣고, 조직형태측정 (15)을 위해 박편으로 절단하였다. 성장 판으로부터 1.2 mm의 제2의 해면질에서 해면질의 골 영역 및 주변을 측정하였다. 해면질 골 영역 (Cn.B.Ar)은 수질 영역의 %로 표현하였다. 2차적인 구조 파라미터인 소주 폭, 소주 수 및 소부 분리도를 파피트 (Parfitt) 등이 개발한 식을 사용하여 제1의 영역 및 주변측정치로부터 계산하였다.
〈난소를 적출한 쥐의 골다공증 모델에서 골 손실, 혈장 콜레스테롤 및 자궁 중량에 대한 이독시펜 영향에 대한 초기 복용량 범위 연구〉
이 연구의 목적은 OVX 쥐의 골다공증 모델에서 골 손실을 방지하는 이독시펜의 최적 복용량을 결정하는 것이었다.
처치한지 1, 2 및 3 달 후에 BMD를 측정하였다. 조직형태측정을 위해 경측골을 제거하는 것과 함께, (대퇴골만의) BMD의 체외 측정 및 기계적 시험을 위해 대퇴골 및 척추 (L1 및 2)를 제거하였다. 이독시펜을 2, 8, 40 및 200 ㎍/kg/d로 복용시켰다.
측정된 모든 파라미터에 따르면 이독시펜의 무효과 복용량은 2 ㎍/kg이었다.
200 ㎍/kg의 이독시펜 복용량이 요부 척추에서 OVX에 의해 유도되는 BMD의 감소를 상당히 방지하였다. 처치하고 3달 후에 요부 척추에서, 200 ㎍/kg의 이독시펜은 약 50 %의 골 손실을 억제하였다.
1달째에 기부 경골에서는, 8 내지 200 ㎍/kg의 이독시펜의 복용량은 골 손실을 약 50 % 억제하였다. 이런 보호 정도는 3달째는 무의미한 수준인 약 25 %로 떨어졌다. 이는, 이독시펜의 200 ㎍/kg이 상기 골격 부위에서는 적정량이 아니라는 것을 시사했다.
체외에서 측정된 골 무기물 밀도를 통해 이독시펜 200 ㎍/kg이 기부 대퇴골의 BMD를 모의 대조군의 수준으로 유지한다는 것을 알 수 있었다. 또한, 이는 대퇴골 중골간 수질의 횡단면적을 모의 대조군의 수준으로 유지시키는데, 이는 이독시펜이 해면질 골 손실 뿐만 아니라 피질 손실도 방지한다는 것을 나타낸다. 이독시펜은 삼점 벤딩 시험에서의 대퇴부 골간 또는 축상 압축 시험에서의 L2 척추체의 기계적 강도에 악영향을 미치지 않았다.
조직형태측정 결과는, 처치한지 3달 후에 기부 경골 해면질 골 영역에서 이독시펜의 작으나 무의미한 효과를 나타냈고, 이는 BMD 측정 결과와 일치하였다. 소주 폭에 있어서 상기 그룹 간에는 어떠한 차이도 없었다. 장기간의 경골 해면질 골 면적에 대한 이독시펜의 효과가 부족함에도 불구하고, 40 ㎍/kg 정도의 낮은 복용량으로도 이독시펜의 활성이 소주 수 및 소주 분리에 비추어 명백하다.
이독시펜 (200 ㎍/kg)은 혈장 콜레스테롤을 상당히 감소시켰다 (도 5). 처치하고 3달 후에는 모든 복용량의 이독시펜이 자궁 중량을 매우 조금이지만 통계학적으로 유의하게 증가시켰다.
〈난소를 적출한 쥐의 골다공증 모델에서 골 손실, 혈장 콜레스테롤 및 자궁 중량에 대한 이독시펜 영향에 대한 복용량 세밀화 연구〉
이 연구의 목적은 OVX 쥐의 골다공증 모델에서 이독시펜의 골 손실을 방지하는 적정량을 결정하는 것이었다. 1달째에 BMD를 측정하였고, 자궁 및 경골을 모으기 전에 2주 더 처치를 계속하였다.
이독시펜 200 ㎍/kg 및 500 ㎍/kg은 요부 척추에서 골 손실을 완전히 방지하였다. 요부 척추에서 1000 ㎍/kg의 이독시펜이 중요한 영향을 주지는 않았다. 기부 경골 골간단에서 200 내지 1000 ㎍/kg의 이독시펜이 골 손실을 완전히 방지하였다.
조직형태측정 결과는, 500 ㎍/kg의 이독시펜이 해면질 골의 OVX-유도 손실을 최적으로 방지한다는 것을 증명하였다. OVX에 의해 유도된 소주 폭의 감소가 200 및 500 ㎍/kg의 이독시펜에 의해 상당히 방지되었다. 소주 수는 500 및 1000 ㎍/kg의 이독시펜에서 상당히 보존되었다. OVX에 의해 유도된 소주 분리의 증가가 200 내지 1000 ㎍/kg의 이독시펜에 의해 상당히 방지되었다.
모든 복용량의 이독시펜이 혈장 콜레스테롤 수치를 상당히 감소시켰다 (도 11). 시험된 이독시펜의 복용량 중 어느 것도 자궁의 습윤 중량에 영향을 미치지 않았다.
이독시펜의 복용량 500 ㎍/kg이 OVX 쥐를 처치하고 6주 후 최적임이 모든 측정된 파라미터에 의해 일관되게 증명되었다.
6주의 처치 기간에 걸쳐, 이독시펜의 적정 복용량은 500 ㎍/kg인 것으로 밝혀졌다. 요부에서 골 손실 방지에 대한 최소 유효 복용량은 200 ㎍/kg였고, 콜레스테롤-저하 효과에 대해서는 100 ㎍/kg였다. 요약하면, 이독시펜의 골-보호 및 콜레스테롤-저하 효과는 자궁에 대한 분명한 영향이 없이 폐경기후 질환의 방지에 유용하다.
폐경기후 여성에게 3달 동안 이독시펜을 3가지 다른 복용량 (2.5, 5 및 10 mg/일)으로 투여하여 위약을 투여하였을 때와 비교하였다. 이 여성들은 연구 초기에는 골무기질 밀도가 낮다는 증거를 갖고 있다. 골 손실의 감소 속도와 일치하는 것인 골에 대한 효과의 존재는 골 흡수 (요를 통한 콜라겐 가교물 배출; C-텔로펩티드, 유리 가교물 및 총 가교물의 배출량으로 측정) 및 골 형성 (혈청 오스테오칼신)의 생화학 표지에서의 변화를 측정함으로써 탐지하였다.
이런 생화학 표지 수치 (하기 참조)의, 조사한 복용량과 관련된 감소는 골 교체 회전수에서의 감소로 나타내어지며, 폐경기후 여성의 골 교체 회전수에 대한 에스트로겐의 효과와 일치한다. 모든 변화들을 기준값에 대한 변화량의 %로 나타내었다. 위약과의 통계학상 유의 수준 차는*=p〈0.01,**=p〈0.001과 같은 표기법으로 나타내었다.

경구 투여시 활성인 본 발명의 화합물 및 그의 제약학상 허용가능한 염을 예를 들어 시럽, 현탁액제 또는 유제와 같은 액상제, 정제, 캡슐 및 로젠지로 제제화할 수 있다.
액상 제제는 일반적으로 에탄올, 글리세린, 비수성 용매 (예를 들면, 폴리에틸렌 글리콜, 오일), 또는 현탁제, 보존제, 향미료 또는 색소를 함유한 물과 같은 적당한 액상 담체를 포함할 것이다.

비경구 투여시 (즉, 주입물의 주입) 활성인 본 발명의 화합물 및 그의 제약학상 허용가능한 염은 용액 또는 현탁액제로 제제화할 수 있다.
비경구 투여용 조성물은 일반적으로 무균 수성 담체 또는 비경구 허용가능한 오일, 예를 들면 폴리에틸렌 글리콜, 폴리비닐 피롤리돈, 레시틴, 아라키스유 또는 참기름 중의 활성 성분의 용액 또는 현탁액을 포함할 것이다. 별법으로, 이 용액은 동결 건조 후, 투여 직전에 적당한 용매로 재구성될 수 있다.
통상의 좌약 조성물은 좌약 투여시 활성인 본 발명의 화합물 또는 그의 제약학상 허용가능한 염을 고분자 글리콜, 젤라틴 또는 코카 버터 또는 다른 저융점 식물성 또는 합성 왁스 또는 지방과 같은 결합제 및(또는)윤활제와 함께 포함한다.
통상의 경피성 제제에는 통상적인 수성 또는 비수성 부형제, 예를 들면 크림, 연고 로션 또는 페이스트, 또는 약용 고약, 패치 또는 막의 형태가 포함된다.
국소 투여용으로 제약학상 허용되는 조성물로는 용액제, 현탁액제, 연고 및 고상 삽입제가 포함된다. 통상의 제약학상 허용가능한 담체로는 예를 들어, 물, 물과 저분자량 알칸올과 같은 수혼화성 용매 또는 식물성유의 혼합물, 및 수용성 안과학상 허용가능한 비독성 고분자, 예를 들면 메틸 셀룰로오스와 같은 셀룰로오스 유도체가 있다. 제약학적 제제는 또한 유화제, 보존제, 습윤제 및 바딩제 (bodying agent)와 같은 비독성 보조 물질, 예를 들면 폴리에틸렌 글리콜; 4급 암모늄 화합물과 같은 항박테리아성 성분; 염화 알칼리 금속과 같은 완충 성분; 메타중아황산 나트륨과 같은 항산화제; 및 소르비탄 모노라우레이트와 같은 다른 통상적인 성분들을 포함할 수도 있다.
상기 조성물은 단위 복용 형태인 것이 바람직하다. 제약학상 복용 단위의 본 발명의 화합물의 복용량은 활성 화합물을 0.01 내지 200 mg/kg, 바람직하게는 0.1 내지 100 mg/kg의 범위 중에서 선택된 비독성 유효량일 것이다. 선택된 복용량은 심혈관 질환의 치료 또는 예방이 필요하거나 혈장 콜레스테롤 수치가 낮은 환자에게 경구, 비경구, 국소 투여, 주사에 의해 일일 1 내지 6회, 또는 주입에 의해 연속으로 투여될 수 있다. 사람에게 투여하는 경구 복용 단위는 화합물 성분을 10 내지 500 mg 함유하는 것이 바람직하다. 비경구 투여에서는 더 적은 복용량을 사용하는 것이 일반적이다. 경구 투여는 안전하고, 효과적이고, 환자에게 편리할 경우 사용된다.
본 발명의 화합물을 본 발명에 따라 투여할 때, 허용가능하지 않은 독물학상 작용이 전혀 나타나지 않을 것이라고 기대된다.
〈실시예 1〉
화학식 I의 활성 화합물의 경구 투여를 위한 경구 복용 형태를 예를 들어, 하기에서 제시된 비율로 사분, 혼합 및 경질 젤라틴 캡슐에로의 충전에 의해 제조한다.

(E)-1-[2-[4-[1-(4-요오도페닐)-2-페닐-1-부테닐]페녹시]피롤리딘 50 mg을 정상 염수 25 ml 중에 분산시켜서 주사용 제제를 제조하였다.
〈실시예 4〉
이 실험은 골아세포에서 이독시펜 및 라록시펜 (raloxifen)의 작용 메카니즘을 비교하기 위해 수행하였다. (하기에서 설명되는) 루시페라제 리포터 유전자의 상류에 에스트로겐 반응 요소 (ERE)를 함유하는 작제물을 사용하여, 이독시펜이 골아세포에서의 ERE를 통해 에스트로겐과 같이 순수한 아고니스트라는 것이 이제 입증되었다. 천연 스테로이드 호르몬 에스트로겐 및 이독시펜 간의 아고니스트 작용 효능은 유사하였다. 이독시펜과 동일한 농도 (0.01 내지 10 μM)의 라록시펜은 ERE를 통해 부형제의 대조군과 유사한 정도의 매우 약한 신호를 나타내었다.
이런 반응 요소을 통한 작용 메카니즘을 확인하기 위해 경쟁 실험을 수행하였다. 라록시펜은 ERE를 통해 에스트로겐 및 이독시펜 모두의 아고니스트 활성을 저해하였다. 리간드 (에스트로겐 또는 이독시펜)의 복용량 100 nM에서, 아고니스트 반응이 최대였다. 라록시펜 500 nM과의 동시 처치에 의해서는 리포터 유전자 활성을 부형제 대조군의 수준으로 저하시켰다. 반대로, 이독시펜 500 nM는 골아세포에서 에스트로겐 100 nM의 최대 아고니스트 작용을 감소시키지 않았다. 이독시펜 및 에스트로겐의 최대 농도 보다 적게 (〈100 nM) 사용하면, 골아세포에서 ERE를 통한 이 2가지 아고니스트의 상가 작용이 있었다.
〈실험 방법〉
6-웰 플레이트에는 페놀 레드-무함유 배지 중의 1.5×105세포/웰 또는 24-웰 플레이트에는 1.5×104세포/웰로 세포를 파종했다. 글루코코르티코이드 반응 요소를 33개의 염기쌍 비텔로제닌 에스트로겐 반응 요소의 5개의 카피 (copy)로 대체한 마우스 유방 종양 바이러스 프로모터를 포함하는 DNA 작제물을 사용하였다. 이는 루시페라제 리포터 유전자 (MMTV-ERE-Luc)의 상류에 있다 (Wen, D.X., Y-F. Xu, M.K. Goldman 및 P. McDonnell. 1994). 사람의 프로게스테론 수용체의 A 및 B 이소폼은 표적 세포 내에서 구별되는 신호 통로를 통해 작용한다. 문헌 [Molec. Cell. DNA를 리포펙션 (lipofection) 방법 (메릴랜드주 게이더스버그 소재의 라이프 테크날로지스 (Life Technologies)사)에 의해 쥐의 골육종 (Roc 17/2.8) 세포에 도입하였다. MMTV ERE-Luc를 6-웰 플레이트에서는 웰 당 2 ㎍ 및 24-웰 플레이트에서는 웰 당 140 ng, 및 대조 레닐라-루시페라제 벡터 (pRL-CMV) 25 ng으로 세포를 동시에 트랜스펙션시켰다. 트랜스펙션 효율을 레닐라-루시페라제 벡터로 동시-트랜스펙션에 의해 보정하였고, 이 벡터는 생체발광 정보 판독 (위스콘신주 매디슨 소재의 프로메가사)을 위해 다른 기질인 코엘렌테라진 (coelenterazine)을 이용한다. 세포들을 하룻밤 동안 배양시켰다. 이어서, 트랜스펙션 배지를 제거하고, 세포를 호르몬의 존재 또는 부재하에서 48 시간 동안 배양하였다. 세포를 인산 완충된 염수로 세척한 다음, 교반대에서 샘플을 흔들면서 15 분 동안 능동 용해 완충액 (PLB)으로 웰 당 500 ㎕로 1회 용해시켰다. 용해액을 12,000 g에서 30 초 동안 원심분리하고, 투명한 용해액을 리포터 효소 분석 전에 튜브로 옮겼다. 샘플 (20 ㎕)을 96 웰 발광 검출 플레이트로 옮기고, 각각의 분석 시료 (위스콘신주 매디슨 소재의 프로메가사) 100 ㎕와 반응시켰다. 각 분석 시료를 루시페라제 활성을 측정하는 마이크로루매트 (microlumat) LB96P 루미노미터 (luminometer) (메릴랜드주 게이더스버그 소재의 월락 (Wallac)사)에 의해 주입하였다. 루시페라제 활성은 에스트로겐 반응 요소 (ERE)를 함유하는 에스트로겐 반응 유전자의 전사에 의한 활성화를 제공한다. 따라서, 루시페라제 활성의 상향 조절이 아고니스트 작용을 나타내는 반면, 하향 조절은 ERE을 통한 길항 작용을 나타낸다.
결론:
이독시펜은 골아세포에서 에스트로겐 반응 요소 (ERE)를 통한 아고니스트이다. 이독시펜과는 반대로, 라록시펜은 시험된 복용량에서 골아세포에 있는 ERE를 통한 길항 물질이고, 이는 라록시펜으로 제시되는 골 부족 효과에 대한 명백한 메카니즘을 제안하고 있다. 따라서, 라록시펜은 사람의 TGFβ3 프로모터의 5'-비번역 영역에 존재하는 서열을 포함하는 non-ERE를 통한 생물학적 효과를 발휘할 수 있다. 동일한 세포계에서, 라록시펜은 ERE-함유 비텔로제닌 프로모터 발현을 억제해서 순수한 에스트로겐 길항 작용을 보였다. 이런 반응 요소를 통한 ERE 제안 조절이 다른 유전자에 작용할 것이기 때문에, 라록시펜 반응 요소 (Yang, N, N., Venugopalan, M., Hardikar, S., 및 Glasebrook, A.
