Abstract

Description

말단 5-HT를 증가시키는 다른 접근법은 5-HT1B 자가수용체의 차단을 통한 것 이었다. 레트에서의 미세투석 실험은 시탈로프람에 의한 해마 5-HT의 증가가 실험용 5-HT1B 수용체 길항제인 GMC 2-29에 의해 강화된다는 것을 실제로 나타냈다.

5-HT2C 리간드가 레트 전두 피질에서 도파민(DA) 및 노르아드레날린(NE)의 방출에 영향을 미칠수 있다는 것이 입증되었다. 따라서, 5-HT2C 효현제 Ro 60-0175 및 선택적 5-HT2C 길항제 SB-242084는 세로토닌(5-HT)의 레벨을 변화시키지 않고 각각 DA 및 NE의 레벨을 모두 억제 및 증가시켰다(Millan, M. 현재, 미세투석 실험에서 측정된 바, 세로토닌 재흡수 저해제와 5-HT2C 길항제 또는 역성 효현제 효과를 가지는 화합물(5-HT2C 수용체에서 부정적 효능을 가지는 화합물)의 조합이 말단 영역에서 5-HT의 레벨에 상당한 증가를 제공한다는 것이 밝혀졌다.
특히, SSRI와 조합된 케탄세린, 리탄세린, RS 102221, (+)-트랜스-1-(5-클로로-3-(4-플루오로페닐)-1-인단일)-4-(2-(3-이소프로필-2-이미다졸리디논-1-일)에틸)-피페라진 및 2,5-디메틸-3-(4-플루오로페닐)-1-[1-[2-(이미다졸리딘-2-온-1-일)에틸]-피페리딘-4-일]-1H-인돌은 상승적으로 작용하여 세포외 세로토닌의 레벨을 증가시킨다는 것이 밝혀졌다. 사람에게 적용되었을 때, 이것은 클리닉에서 항우울 효과의 더 짧은 개시 및 증강, 또는 세로토닌 재흡수 저해제(SRI)의 치료 효과의 강화를 내포한다.

본 발명에 따라서, 동물 연구는 5-HT2C 수용체 길항제 또는 역성 효현제가 세로토닌 재흡수 저해제의 치료 효과의 빠른 개시를 제공하고, 세로토닌 재흡수 저해제의 불안치료의 잠재력을 강화할 수 있다는 것을 나타냈다.
5-HT2C 수용체 길항제, 역성 효현제 또는 부분 효현제와 세로토닌 재흡수 저해제 조합의 사용은 우울증 및 다른 정동적 장애를 치료하는데 필요한 세로토닌 재흡수 저해제의 양을 대단히 감소시킬 수 있으며, 따라서 세로토닌 재흡수 저해제에 기인하는 부작용을 감소시킬 수 있다. 특히, 감소된 양의 SRI와 5-HT2C 수용체 길항제, 역성 효현제 또는 부분 효현제의 조합은 SSRI-유도 성기능장애 및 수면장애 의 위험을 감소시킬 수 있다.
또한, 5-HT2C 수용체 길항제, 역성 효현제 또는 부분 효현제와 세로토닌 재흡수 저해제의 공동-투여는 불응성 우울증, 즉 세로토닌 재흡수 저해제의 단독 투여에 의해 적합하게 치료될 수 없는 우울증의 치료에 유용할 수 있다. 전형적으로, 5-HT2C 수용체 길항제, 역성 효현제 또는 부분 효현제는, 적어도 40% 내지 60%의 증상 감소가 SRI를 사용한 치료의 첫 6주 동안 달성되지 않는 경우에, 환자에서 SRI에 대한 반응을 증강시키기 위해 치료법에 더해져서 사용될 수 있다.

미세투석 실험
Wistar-유래 균주의 수컷 알비노 레트(285-320g; Harlan, Zeist, 네델란드)를 실험에 사용했다. 레트를 플라스틱 우리(35x35x40cm)에 가두고, 물과 음식에 자유롭게 접근하게 했다. 실험은 헬싱키의 선언과 일치하고, 그로닝겐 대학의 수학 및 자연 과학부의 동물 관리 위원회에 의해 승인되었다.

케탄세린 분석: 케탄세린을 액체-액체 추출한 후, 형광 검출기와 함께 역상 크로마토그래피하여 측정했다. 간단히 말해서, 150㎕ 혈장 샘플에 0.1N NaOH 30㎕를 첨가했다. 샘플을 디에틸 에테르 3mL와 함께 3분간 기계로 흔들어 2번 추출했다. 다음에, 에테르층을 10mL 증발 튜브로 옮기고, 0.1N HCl 150㎕를 첨가했다. 에테르를 질소 흐름하에 40℃ 수욕에서 증발시켰다. HCl층을 0.5mL 에테르로 1번 세척했다. 샘플 50㎕를 칼럼 위에 주사했다.

데이타 프레젠테이션 및 통계: 편차가 20% 미만인 4개의 연속 미세투석 샘플을 대조표준으로 하고, 100%로 설정했다. 데이타는 대조표준 레벨에 대한 퍼센 트로 나타낸다(평균±SEM). 윈도우용 Sigmastat(Jandel Corporation)를 사용하여 통계 분석을 수행했다. 치료 효과를 반복 측정에 대한 투웨이 ANOVA를 사용하여 비교한 후, Dunnett's 시험을 수행했다. 유의한 레벨을 p<0.05로 설정했다. 약물동력학 데이타는 Multifit(카피라이트 Dr. J. 결과
리탄세린 공동-투여
10μmol/kg 리탄세린의 복강내 투여는 레트 복측 해마에서 5-HT 레벨에 대한 어떤 효과도 없었다. 10μmol/kg 시탈로프람의 피하 투여는 살린 주사와 비교하여 5-HT 레벨을 유의하게 증가시켰다(F((1,131)=18.8, p<O.05). 이후의 분석은 t=45분에서 t=150분까지의 유의성을 밝혔다. 10μmol/kg 시탈로프람을 피하 주사하기 60분 전에 제공된 10μmol/kg 리탄세린은 시탈로프람의 효과를 유의하게 증강시켰다(F(1,105)=8.90, p<O.05). 이후의 분석은 t=60분에서 t=150분까지의 유의성을 밝혔다.

10μmol/kg 시탈로프람과 상이한 용량의 케탄세린의 동시 투여는 SSRI의 케 탄세린에 의한 용량 의존성 증강이 5-HT 레벨의 증가를 유도했음을 나타냈다. 케탄세린 1nmol/kg의 공동-투여는 시탈로프람의 효과를 증강시키지 않았고(F(1,118)=0. 574), 반면 케탄세린 10 및 100nmol/kg의 공동-투여는 시탈로프람의 효과를 유의하게 증진시켰다(각각 F(1,117)=6.95, p<0.05, F(1,120)=5.66, p<0.05). 이후의 분석은 10nmol/kg 케탄세린 공동-투여에 대한 t=45분에서 t=150분까지의 유의성, 및 100nmol/kg 케탄세린 공동-투여에 대한 t=45분에서 t=150분까지의(90 및 105 제외) 유의성을 밝혔다.

약물동력학 실험
시탈로프람 투여
10μmol/kg 시탈로프람의 피하 투여시, 대략 0.7μM의 초기 혈장 레벨이 발 생했고, 이 후 레벨은 대략 2시간의 겉보기 반감기를 보이며 감소했다.
케탄세린 투여

시탈로프람 및 RS 102221, 시탈로프람 및 (+)-트랜스-1-(5-클로로-3-(4-플루오로페닐)-1-인단일)-4-(2-(3-이소프로필-2-이미다졸리디논-1-일)에틸)-피페라진, 시탈로프람 및 2,5-디메틸-3-(4-플루오로페닐)-1-[1-[2-(이미다졸리딘-2-온-1-일)에틸]-피페리딘-4-일]-1H-인돌, 및 플루옥세틴 및 케탄세린.
결과:
1μmol/kg RS 102221의 피하 투여는 레트 복측 해마 5-HT 레벨에 대한 효과가 없었다. 10μmol/kg 시탈로프람의 피하 투여는 대조표준과 비교하여 5-HT 레벨을 유의하게 증가시켰다. 10μmol/kg 시탈로프람과 1μmol/kg RS 102221의 동시 투여는 시탈로프람 효과의 유의한 증진을 나타냈다(F(1,119)=3,34).
1μmol/kg (+)-트랜스-1-(5-클로로-3-(4-플루오로페닐)-1-인단일)-4-(2-(3-이소프로필-2-이미다졸리디논-1-일)에틸)-피페라진의 피하 투여는 레트 복측 해마 5-HT 레벨에 대한 효과가 없었다. 10μmol/kg 시탈로프람의 피하 투여는 대조표준과 비교하여 5-HT 수준을 유의하게 증가시켰다. 10μmol/kg 시탈로프람과 1μmol/ kg Lu 27121의 동시 투여는 시탈로프람 효과의 유의한 증진을 나타냈다(F(1,120) =4,25).
1μmol/kg 2,5-디메틸-3-(4-플루오로페닐)-1-[1-[2-(이미다졸리딘-2-온-1-일)에틸]-피페리딘-4-일]-1H-인돌의 피하 투여는 레트 복측 해마 5-HT 레벨에 대한 효과가 없었다. 10μmol/kg 시탈로프람의 피하 투여는 대조표준과 비교하여 5-HT 레벨을 유의하게 증가시켰다. 10μmol/kg 시탈로프람과 1μmol/kg Lu 29066의 동시 투여는 시탈로프람 효과의 유의한 증진을 나타냈다(F(1,120)=7,29).
100μmol/kg 케탄세린의 피하 투여는 레트 복측 해마 5-HT 레벨에 대한 효과가 없었다. 5mg/kg 플루옥세틴과 100nmol/kg 케탄세린의 동시 투여는 플루옥세틴 효과의 유의한 증진을 나타냈다(F(1,69)=2.28).
