Abstract
본 발명은 의료용 약제에 관한 것이다. 본 발명의 약제는 생물학적 활성물질로 이루어지는데, 이들에는 인슐린, 에리트로포이에틴, 칼시토닌 및 성장호르몬 등이 있다. 본 발명의 약제는 위의 활성물질과 결합하는 인지질을 함유할 수 있는데, 이는 소장 상피조직벽에서 발견되는 레시틴을 형성하는 알파-글리세롤 또는 다른경로에 관현하는 물질을 생성시킨다. 본 발명에 따른 약제로 경구투여되는 생물학적 활성 단백질은 임상적 효능과 활성을 나타낸다.
Description
본 발명은 의료약제에 관한 것이다. 더욱 상세하게는 생물학적 활성물질, 특히 단백질 성분 물질의 경구 또는 직장투여 가능한 약제에 관한 것이다.
여러가지 질환 또는 병세를 예방하고 치료하는데 여러종류의 생물학적 활성물질을 투여하여온 것이 과거 오랜동안의 의료 관행이었다. 처방 시용된 생물학적 활성물질가운데 가장 잘 알려져 있는 것중의 하나가 인슐린인데 이것은 당뇨병 치료에 사용되는 것이다. 투약방식에 있어 가장 쉬운것은 구강에 의한 경구흡수라 할 수 있다. 그 투약경로는 대개 시럽, 엑릭시르(elixir), 정제, 캡슐, 과립, 산제 그밖에 간편한 제제를 통한 단순 간단한 방식이지만 때로는 환자 입장에서 볼때 불편하고 고통스러운 경우도 있다. 따라서, 단백질성분 약제나 그밖에 생물학적 활성 물질이 위장을 통과하게 되고 이러한 환경은 단백질 성분을 포함하여 여러 유효성분에 이롭지 못하므로 치료나 예방의 관점에서 환자에게는 매우 불리하다. 산성, 가수분해성 및 단백질 분해성 조건하의 위장 내에서는 단백질성분 물질이 쉽게 아미노산이나 올리고펩티드로 분해되므로 간편하게 경구 투여할 경우 생물학적 활성 단백질 성분 물질은 극히 소량 또는 그 분해물질이 위장을 통과하여 소장에서 흡수된다는 것은 쉽게 이해할 수 있다.
많은 당뇨병 환자가 실증하는 바와 같이 여러가지 단백질 성분 치료약은 비경구적으로 투여하여야 하고 경우에 따라서는 피하, 근육 또는 정맥주사로 투여하지 않으면 안되는데 이것은 환자를 불편하고 고통스럽게 한다. 이러한 현상은 개별적인 문제가 아니라 단백질 성분 약제 투여로 치료하는 질환에 있어서는 흔히 부수되는 문제이다.
예컨데 당뇨병의 경우 세계 각국에서 수많은 환자를 발생시켜 그러한 문제가 야기된다. 한가지 또는 다른 형태의 당뇨병에 이환된 수많은 환자는 위장의 유해환경에 지탱할 수 있는 인슐린의 경구용 제제의 개발을 요망하게 되었다. 이렇한 약제의 개발은 여러가지로 시도되었으나 그 조성성분을 시판약제로 부터 추정해 낼 수 없었다. 그에대한 종래의 참고할만한 제의는 다음과 같다.
WO-A-8701035는 지용성 약제와 비타민의 비경구 투여제제에 관한 것으로 이 제제는 슈도 미셀(Pseudomicelles)로 되어 있다.
WO-A-8705505는 액상제제로 부터 얻은 고체입자를 피복한 경구투여용 인슐린 제제에 관한 것으로 인슐린 피복입자는 다시 지질(Lipid)로 피복처리된다.
US-A-4849405는 경구투여용 인슐린 조성물에 대하여 기재하고 있는데 조성물은 2상 제제(twophase preparations)로 모두 액상이고 공동군생계(coacervate system)에 의해 따로 따로 보관된다.
EP-A-0140085는 유효약 함유 지질 소낭 제제(lipid vesicle preparation)에 대하여 개시하고 있다.
시치리외 수인은 물과 기름의 혼합제 인슐린 미셀(micells)에 대하여 보고한바 있다.[참조:Acta diavet. lat. 15 175-183(1978)].
US-A-4784845 및 US-A-4816247은 비경구 투여용 소수성 유체 조성물(emulsion composition)약제에 대하여 기재하고 있다.
JP-A-50017328은 경구투여용 인슐린 함유 유성 혼탁액제에 대하여 공개하고 있다.
EP-A-0366277(1990. 5. 2 공개)은 경구 또는 직장 투여 가능한 개량약제에 대하여 기재하고 있다. 더욱 상세하게 EP-A-036277은 소수성상(hydrophobic phase)과 친수성상(hydrophilic phase)으로 이루어진 미세 현탁액을 함유하는 약제에 관하여 기재하고 있는데, 이 약제는 (A) 소수성상을 친수성상에 분산시키는데, (B) 친수성상은 생물학적 활성물질을 함유하고, (C) 소수상은 카일로미크라(chylomicra) 또는 카일로미크라를 생체내에서 합성할 수 있는 물질을 함유한다. 친수성상은 생물학적 활성물질과 상용할 수 있는 생리적 용매(예; 물)를 함유할 수 있다. 생물학적 활성물질은 카일로미크라 또는 카일로미크라 구성성분과 함께 투여되면 소장의 빌라(villae)또는 마이크로빌라로 향하고 여기서 유미관(lacteal)또는 장관림프(intestinal lymph)로 분비되어 이어서 흉관으로 도입된 후 마지막으로 혈액순환하게 되는것으로 추정된다.
주지하는 바와 같이 카일로미크라는 지질/콜레스테롤 중심체(core) 또는 매트릭스(matrix)로 이루어져 있고 그 둘레에 단백질이 산재하는 인지질 단일층으로 된 막이 쌓여 있다(참조 : Redrave in Gastrointestinal Physiology IV, International Review of Physiology, Vol. 28, 103-130, Young, J. A., Ed., University Park Press. Baltimare, 1983). 선행기술의 유럽특허출원 기재내용은 소수성 중심체 내의 생물학적 활성물질을 소개하고 있다.
본 발명은 경구(또는 직장) 투여용 생물학적 활성물질, 특히 단백질에 관한 문제점을 해결함에 있어 다른 접근 방법을 시도한 것이다. 단백질 성분화합물, 즉 인슐린(소, 돼지, 사람 또는 기타), 성장호르몬(사람 기타), 칼시토닌(연어 기타), 에리트로포이에틴(사람 기타) 등을 레시틴 형성에 관련된 하나 또는 그 이상의 인지질(레시틴 전구체)이나 그밖의 화합물과 화합하여 구강투여할 수 있다. 이러한 화합은 생물학적 활성물질이 단백질인 경우 인지질-단백질(phospholico-protein)(예; 인지질-인슐린)이다.
본 발명의 한 구현으로 단백질성 물질이나 그밖의 생물학적 활성 물질과 레시틴 또는 레시틴 전구체(lecithin precursor)로 이루어진 약제조성물이 제공된다. 단백질성 화합물은 다른 생화학적 활성물질로 치환되거나 보충될 수 있다. 이러한 경우 작용 방식은 상기 기작(mechanism)과 유사한 것으로 보인다. 생물학적 활성물질 화합물과 레시틴 전구체는 상호 화합하고 있는 형태를 일반적으로 취하고 있다.
레시틴은 카일로미크라, 특히 그 격막내로 통합될 수 있다. 이와같이 통합되는 다른 화합물 또는 그들 전구체의 사용은 본 발명의 범위에 속한다.
레시틴 또는 그 전구체에 관련된 본 명세서의 기재내용은 유사 화합물에도 적용할 수 있다.
레시틴 전구체는 사람이나 동물의 장관 상피조직(epithelium)내에서 레시틴으로 형성되어 이 단백질성 물질은 레시틴과 화합하여 레시틴-단백질 복합체(예; 레시틴-인슐린)을 형성하게 된다. 이와같이 장관 상피조직내에서 형성된 레시틴은 카일로미크라의 표면막을 형성하고, 아포프로테인-A, -B, -C- 및 -E와 같은 아포리포프로테인(Apolipoprotein)의 표면중 80% 이상을 구성한다.
따라서 인지질-단백질 복합체를 위한 적당한 흡착증강제를 사용하여 복합체를 장관의 상피조직내로 흡수시킬 수 있는데, 이어서 레시틴이 합성되고(복합체는 레시틴-단백질 복합체로 됨) 레시틴은 카일로미크론 중심체와 함게 카일로미크라에 부착한 아포단백체를 덮어 씌울 수 있게 된다. 그후 레시틴은 림프관내로 방출되어 흉관으로 도입(카일로미크라에 아직 부착되어 있는 레시틴 단백질 복합체는 잔여 카일로미크라 부위를 형성할 수 있음)되고 간을 경유하여 순환혈액중에 방출된다. 이와같이 레시틴은 단백질을 순환계통으로 운반하는 기능을 보유하고 있는 것으로 믿어진다. 레시틴은 또한 여러 다른 경로를 통하여 생체내 합성이 이루어질 수 있다. 이를 설명하면 다음과 같다. 첫째로 α-글리세롤 경로를 이용할 수 있다. 이 경로에서 sn-글리세롤-3-포스페이트는 포스파티데이트(Phosphatidate)및 디글리세라이드와 같이 전구체로 이용된다. 둘째로 레시틴은 콜린포스포트랜스페라제(Cholinphospho transferase)에 의하여 합성될 수도 있다. 이 경로에서 콜린, 포스포콜린, 시티딘디포스포콜린 및 디글리세라이드는 전구체로 이용될 수 있다. 셋째로 레시틴은 포스포티딜 에탄올아민과 같은 다른 포스퍼타이드(phosphatide)로 부터 합성될 수도 있다. 넷째로 레시틴은 트리글리세라이드 및 레시틴 분해물질로 부터 합성되거나 트랜스에스테르화 과정에 의해 합성될 수 있다.
생물학 활성물질이라 함은 특히 약제학적으로 활성을 나타내는 단백질성 물질을 포함하는 것이다. 단백질성 물질은 순수 단백질이거나 단백질과당(糖)잔기를 함유하는 글리코프로테인 같은 단백질 유도체일 수 있다. 이들 물질은 사람의 진료나 동물의 진료에서 질병 또는 징후의 예방 치료에 이용될 수 있다.
본 발명에 따라 제조할 수 있는 경구 또는 직장 투여용 약제에 이용될 수 있는 단백질성분의 생물학적 물질은 예컨데 인슐린, 칼시토닌, 성장호르몬 같은 단백질성 호르몬(사람 또는 동물은 이들 물질을 전부 또는 일부를 생합성 한다.), 에리트로포이에틴 헤마토포이에틴, 플라스미노젠 활성화제(plasminogen activator) 즉, t-PA, 유로키나제, 프로-유로키나제, 스트렙 토키나제와 인터페론(예; 인터페론 알파, 인터페론 베타, 인터페론 감마등), 인터로이킨(inter leukins)(예; IL-1, IL-2, IL-3, IL-4), 콜로니자극인자(예; G-CSF, GM-CSF)및 인자 VIII와 같은 혈액인자(blood factor)등이다.
그러나 본 발명은 이들 단백질성 화합물에만 국한되는 것이 아니라, 많은 종류의 비단백질성 약제 또한 본발명의 방법에 따라 효과적으로 제조할 수 있다. 예컨데 비스테로이드성 항염제(NSAIDs)를 적합하게 제조할 수 있다.
그러나 본 발명의 인지질과 생물학적 활성물질의 공동형성을 고려하여 활성 물질은 인지질 또는 그 밖의 약제 조성물질과 비가역적으로 공유결합을 형성하지 않는 것이 바람직하다.
본 발명의 방법에 따라 상기 활성물질을 제조하는데 아무 문제가 제기되지 않음을 확인하였다. 활성 화합물과 레시틴 또는 그 전구체와의 화합은 비공유성 복합체 형상으로 이루어진다. 이들 복합체는 수소결합, 반데르발스결합, 이온성결합 및/또는 지방-지방 결합으로 이루어진다.
본 발명에 따라 제조할 수 있는 생물학적 활성물질의 분자크기에 특별히 제한을 할 필요는 없으므로, 본 발명은 마크로분자성 제제를 제조하는데 특히 적합하며 이것은 상기 생물학적 활성물질을 비제한적으로 임의 선택할 수 있는 것으로 부터 명백히 알 수 있다. 그러한 마크로 분자의 분자량은 약 1K Da, 약 5K Da, 약 10K Da 또는 그 이상이거나 15K Da 또는 그 이상이다. 한편, 생물학적 활성물질의 친수성 또는 소수성(친지방성)은 특별한 제한이 따르지 않기 때문에 본 발명은 인슐린, 칼시토닌(특히 연어의 칼시토닌)및 성장호르몬 또는 소마토트로핀(특히 돼지 소마토트로핀)등과 같은 친수성 분자를 제조하는데 쉽게 적용할 수 있는데, 이들 모두(특히 연어 칼시토닌)는 검습기의 기능처럼 친수성을 나타낸다.
본 발명의 제제 조성 성분중의 생물학적 활성물질 함량은 물질성질에 따라 자연 결정되며 실제 투여에 적합한 처방으로 임의 조절할 수 있다. 이러한 고려하에 본 발명의 제제함량은 1㎍, 10㎍, 0.1mg 또는 1mg/1 내지 1, 10g 또는 100g/1으로 할 수 있다.
본 발명은 인지질 화합물과 그 유도체 또는 그 밖에 사람 또는 동물의 장관 상피조직 중에서 레시틴 생체합성을 위한 전구체로 작용할 수 있는 화합물을 포함한다. 레시틴은 카일로미크론 중심체의 막 구성일부를 형성하게 된다. 투여조건하에서 막-통합 화합물은 생물학적 활성물질을 레시틴 막(카일로 미크론 중심체를 피복하고 있는것)속으로 파묻히게 한다. 레시틴 막은 상술한 바와 같이 인지질이 주 구성성분이다. 카일로미크론 막은 인지질이 풍부하고, 인지질은 본 발명의 생물학적 활성물질을 제조하는데 매우 적합한 물질이다.
레시틴 전구체는 생물학적 활성물질을 변형시킬 수 있는 것이어서는 안된다. 예를들면 일부 지방산(예; 올리에이트, 스테아레이트)은 돼지 스마토 트로핀과 역으로 작용하기 때문에 사용될 인지질, 인지질 유도체 또는 다른 막-통합체 또는 전구체를 선택하는 경우에는 상당한 검토가 요망된다. 그러나 이러한 선택은 당분야에서 통상의 기술을 가진 사람이라면 쉽게 할 수 있다.
인지질은 선택대상의 레시틴 전구체이다. 인지질은 글리세릴 트리에스테르인데 그중 에스테르 관능기의 하나는 임의 치환된 인산이다. 본발명에 사용되는 대표적 인지질은 다음 일반식의 화합물이다.
위의 일반식에서, R1및 R2는 각각 탄소수 10, 12 또는 14 내지 26의 아실그룹인데, 단일 또는 복수-불포화기를 가질 수 있고, X는 수소원자 또는 인지질 선두그룹이다. 인지질 선두그룹은 생리적으로 작용할 수 있는 인지질을 형성할 수 있는 그룹이면 어느거나 선택될 수 있다. 이들 인지질을 예시하면 다음과 같다.
디아실 포스파티딜 글리세롤 류: 디미리스토일 포스파티딜 글리세롤(DPMG),디팔미토일 포스파티딜 글리세롤(DPPG), 디스테아로일 포스파티딜 글리세롤(DSPG); 디아실 포스파티딜 콜린 류: 디미리스토일 포스파티딜 콜린(DPMC), 디팔미토일 포스파티딜 콜린(DPPC), 디스테아로일 포스파티디 콜린(DSPC); 디아실 포스파티딜 산 류: 디미리스토일 포스파티딜산(DPMA), 디팔미토일 포스파티딜산(DPPA), 디스테아로일 포스파티딜산(DSPA); 디아실 포스파티딜 에탄올 아민 류: 디미리스토일 포스파티딜 에탄올아민(DPME), 디팔미토일 포스파티딜 에탄올아민(DPPE), 디스테아로일 포스파티딜 에탄올아민(DSPE); 그밖의 예시(이들만에 한정되는 것은 아님)로서, 에탄올아민 유도체(상기 포스파티딜에탄올아민 또는 세팔린), 세린(예; 포스파디딜세린) 및 3'-0-리실 글리세롤(예; 3'-0-리실-포스파디딜 글리세롤).
하나 이상의 포스파디딜 그룹이 단일 인지질 선두 그룹에 결합할 수 있다.
예컨데 두 포스파디딜 부위가 디포스파디딜 글리세롤 또는 카르디올리핀에서와 같이 단일 글리세롤 잔기에 결합할 수 있다. X가 수소원자일 경우, 인지질은 포스파디딜산(예; l-α-포스파디딜산 비미리스토일)이다.
본 발명에 유용한 인지질은 합성 및 천연의 모든 인지질로서 단일성분이나 둘이상의 혼합성분으로 이용된다. 순수한 천연 인지질을 제조하는 경우 단일종류의 인지질이 함유될 지 모르나 이러한 인자는 본 발명의 목적에 영향이 미치지 않는 것으로 보인다.
특히 바람직한 인지질은 1,2-디미리스토일-sn-글리세롤-3-포스포콜린(1수 화물 형태로 있을수도 있음), L-α-포스파디딜산 비미리스토일(나트륨형 형태로 있을 수 있음)등이다.
본 발명의 약제조성물에 있어서, 생물학적 활성물질은 레시틴 전구체와 화합할 수 있다. 이 화합의 정확한 성질은 특정될 필요는 없으나, 비공유성결합, 특히 수소결합, 소수성결합 등의 형태로서, 카일로미크론 또는 인지질 막에 통상 존재하는 인 단백질이 결합되고 있는 형태를 이루고 있는 것이 많다.
고도의 친수/친지방 평형가(HLB)를 가진 계면활성제, 즉 10 이상 또는 14이상의 HLB 값을 가진 계면활성제의 존재하에, 레시틴 전구체와 화합하는 생물학적 활성물질이 쉽게 장관 벽세포인 엔테로사이트(enterocyte)를 통과할 수 있는 친수성 복합체를 형성할 수 있다. 엔테로사이트 내에서 레시틴 전구체는 레시틴 합성에 이용된다. 이와같이 전구체 및 화합한 생물학적 활성물질은 리소솜을 피하여 활성물질 복합체로 전환되고, 막은 이들 화합물(예; 레시틴)을 통합시킨다. 카일로미크론 중심체 표면의 약 80% 이상을 피복한 막 외층을 형성하는 레시틴을 상기 복합체는 대체시키거나 보충시킨다.
순환혈액중에서 표면 단백질 및 인지질은 다른 지방-단백체(lipo-protein)로 치환된다. 이같이 하여 본발명에 따라 투여된 단백질 성분은 당초 화합하고 있었던 카일로미크론으로 부터 분리된 인 단백질(phosphoprotein)로서 혈액중에 섞여 순환된다. 인단백질의 일부는 유리형태로 순환혈액중에 방출되고 그 일부는 카일로미크론 잔부가 부착된 간을 통과할 수 있게 된다. 인단백질/지질의 교환 속도 및 정도는 인지질쇄의 길이 변경 등 여러인자로 영향 받는다.
본 발명에 따른 약제는 친수성 액체를 함유할 수도 있는데 이로서는 통상물이 사용될 수 있고, 생리적 또는 인산염 완충염액도 만족스럽게 사용될 수 있다. 약제의 조제로서 친수성 용매를 혼용할 수도 있다. 사용하는 용매의 혼합액으로 V/V 비가 0.5:1 내지 2:1 인데, 비액성 용매는 꼭 필요하지는 않다. 조성성분의 백분율(중량/중량%가 일반적이나, 중량/체적 또는 체적/체적% 일수도 있음)을 광역 및 협역(바람직한 범위)으로 다음에 나타낸다. 총량은 100%를 초과하지 않는다.
본 발명의 약제는 친수성 계면활성제(예컨데 HLB 10 이상의 것)를 함유할 수있다. 이것은 생물학적 활성 화합물과 레시틴 전구체(특히 사람 또는 돼지 같은 다른 동물의 장관 상피조직중에서 합성된 레시틴 또는 레시틴 전구체)사이의 복합체 형성을 촉진 및/또는 이들 복합체에 친수성 특성을 넓게 부여하는 기능을 한다. 친수성 계면활성제는 10%(W/V 또는 V/V)이하, 바람직하기로는 1 내지 5%, 대표적으로 1.5 내지 4%, 예컨데 약 2.4% 또는 2.5%의 함량으로 할 수 있다.
그밖에 종종 고도로 요청되는 추가성분으로 프로테아제 억제제(protease inhi bitor)가 있다. 이것은 한가지 또는 그 이상의 개별 프로테아제 억제제로 구성되어 있다.
본 발명에 유용한 프로테아제 억제제는 넓게 두가지로 나눌 수 있다. 첫째, 생물학적 활성물질이 단백질성 물질일때 이들의 분해를 제한하고 예방하는데 효능이 있는 프로테아제 억제제이다. 이들 억제제는 위장의 관(tract)에서 발견되는 가수분해효소(예; 트립신, 카이모트립신, 카르복시펩티다제)를 억제하는 기능을 갖고 있다. 인슐린의 경우, 인술리나제(예;트랜스 술파타제)로 알려진 효소종류를 일반적으로 저해하는 프로테아제 억제제이다. 트립신 억제제로서 적당한 억제제는 콩이나 계란 흰자위(오보뮤코이드)로 부터 추출할 수 있다. 둘째, 본발명의 약제에 아포단백체가 함유될 경우에는 장관 점막에 도달되기 전에 아포단백체의 분해를 약화시키기 위하여 프로테아제 억제제를 첨가하는 것이 바람직하다. 일반적으로 말하면, 생물학적 활성을 가진 단백질성 물질의 보호를 위해 사용되는 프로테아제 억제제는 유사할 것이므로 단일 프로테아제 억제제로서 양기능을 수행할 수 있다. 프로테아제 억제제는 생물학적 활성물질 및 막 통합체(membrane integrator)또는 전구체(예;인지질) 사이의 복합체나 화합물에 첨가할 수 있는데, 이때 두개의 상(phase)이 존재하면 친수성 상에 첨가하는 것이 편리하다. 프로테아제 억제제의 첨가향 선택은 통상의 기술을 가진 사람에게 잘 알려진 범위의 것이고, 일반적으로 약 0.1% W/V 이하이거나 0.5% W/V이다. 아포단백체는 10,000,000 T.U., 바람직하기로는 0.5 내지 5,000,000 I.U. 대표적으로는 1.5 내지 4,000,000 I.U., 예컨데 3,000,000 I.V. 인데 정확한 사용량은 생물학적 활성물질의 활성에 따라 결정할 수 있다. 임의의(될수 있으면 바람직한 것을 선택) 친수성 계면활성제 및 프로테아제 억제제와 함께, 생물학적 활성물질 및 막 통합체 또는 전구체의 복합체(바람직한 구현예로서 인지질-단백질 복합체)를 투여하는 것이 이로운 경우가 많은데, 이들은 현탁시키거나 낮은 HBL값(HLB4이하)을 가진 계면활성제를 함유하는 액으로 유화(emulsion) 또는 미세유화(microemulsion)시킨다.
친지방성 물질은 카일로미크라를 생체내 합성할 수 있는 것으로 알려진 물질을 함유할 수도 있는데(필연적인 것은 아님), 이들 물질로는 콜레스테롤을 위시하여, 콜레롤 에스테르 레시틴 및/또는 다른 인지질 또는 포화 또는 모노- 또는 폴리- 포화지방산(예; C내지 C의 탄소로 이루어 진것)등을 들 수 있는데 이들은 임의로 글리세롤 에스테르로 에스테르화하여 모노-, 디- 또는 트리글리세라이드를 형성한다. 한편, 필수적 친수성 인지질 단백체(또는 다른 복합체)는 적당한 유류, 특히 식물성 기름[예; 중간길이쇄의 트리글리세라이드(Medium Chain Glyceride : MCT)기름 또는 그밖의 적당한 기름]과 쉽게 혼합할 수 있다. 상기 혼합액에는 낮은 HLB 값(예컨데 HLB이하의 것)을 가진 적당한 계면활성제를 첨가할 수 있다. 적합한 계면활성제는 리소레시틴(lysolecithin) 유도체와 그밖에 친지방성 물질등이다.
친수성 인지질 단백체(또는 다른 복합체)는 장관 흡수를 대비하여 적당한 피복(coationg) 처리한 후 경구투여할 수 있다. 그러나 실험결과에 의하면 복합체를 적당한 기름 또는 전구체와 혼합하여 소장 장관의 빌라(villae)로 활성물질을 이송하고 여기서 빌라를 통하여 흡수되어 임파계로 도입된다.
일반적인 목적을 위한 친지방성 물질의 광역 및 협역(바람직한 범위)의 조성성분 100분률(일반적으로 W/W이나, W/V 또는 V/V 일수도 있음)을 아래에 나타낸다. 총량은 100%를 초과하지 않는다.
약제의 조제로서 친지방성 상(lipophilic phase)과 상용할 수 있는 유기 용매를 혼용할 수 있다. 용매의 성질은 다른 조성성분의 성질에 따라 결정된다. 경우에따라 에탄올이 적당하다. 용매의 양은 친지방성 상의 양에 따라 결정되며 예컨데 5내지 50% V/V이다. 인지방 단백질 또는 다른 복합체를 상술한 바와 같이(통상 물-기름 계 상태로서) 친지방성 상과 함께 유화액 또는 미세유화액으로 조제할 경우, 다른 성분을 꼭 첨가할 필요는 없으나, 통상 편의에 따라 다른 성분을 추가할 수 있다. 임의의 추가성분은 생물학 활성물질을 위한 안정제이다. 추가할 경우 그 안정제의 성질은 활성물질의 성질에 따라 결정된다. 예컨데 인슐린의 안정제로 여러종류가 있는데, 본 발명의 방법에 따라 효과적으로 인슐린 함유제제에 조성시킬 수 있다. 이들은 예컨데 히드록시프로필셀루로스(HPC), 칼슘염 및 시트르산과 그 염 등이다. 칼슘은 인슐린의 안정제로서만 아니라 세포막의 투공성(透孔性)을 증가시키는 추가 효능이 있는 것으로 알려져 있다. 이러한 기능에 의해 활성물질의 장관벽 세포 투입이 용이하여 진다. 친수성 상에 생물학적 활성물질을 첨가할때 안정제는 통상 같은 상으로 하는 것이 바람직하다.
사용할 안정제의 양은 활성물질의 성질에 따라 결정한다. 그 양의 선택은 통상의 기술을 가진 사람이면 쉽게 할 수 있으며, 경우에 따라 1 또는 2% W/V로 할 수 있다.
한편 추가성분으로 유화조제(emulsification aid)를 혼용할 수 있는데, 이는 유화액 생성에 사용되는 유화조제이다. 일부 유화조제는 계면활성제인데, 이들 계면활성제에 특별한 HLB값의 제한은 없다. 유용한 유화제제는 콜레스테롤, 스테아린산, 나트륨 스테아레이트, 팔미틴산, 나트륨 팔미테이트, 올레인산, 나트륨 올레이트, 폴리옥시에틸렌 40, 스테아레이트, 폴리소르베이트 20, 폴리소르베이트 40, 폴리소르베이트 60, 폴리소르베이트 80, 프로피렌글리콜 디아세테이트 및 폴리프로필렌 글리콜 모노스테아레이트 이다.
유화조제는 친지방성 또는 친수성 상으로 하거나 두상모두 함께 하는 상태로 혼용할 수 있다. 유화조제의 사용량은 안정한 제제를 형성하는데 보조적으로 적합한 양이면 충분하다. 통상의 기술을 가진 사람은 쉽게 그 정확한 양을 결정할 수 있는데 통상 0 내지 15% W/V, 예컨데 0.1 내지 5% W/V이다. 동일 또는 이종 계면 활성제의 친수성상 중에 1 내지 5%로 사용하는 것이 적합하다. 친지방성 상에는 폴리소르베이트 80을 가하는 것이 좋고 친수성 상에는 폴리옥시에틸렌 40 스테아레이트를 가하는 것이 적합한 것으로 확인되었다.
본 발명에 따른 약제는 각종 보존제를 함유할 수 있다. 두가지 유용한 종류의 보존제는 산화방지제 및 향균제(antimicrobial agent)이다. 항산화제가 특히 유용한데, 본 발명의 약제에 적합하게 사용되는 화합물중 일부는 산화작용으로 분해되기 쉽기 때문이다. 그런데 이러한 문제는 불활성 대기 (예; 질소 분위기)하에서 약제를 제조하므로서 해소시킬 수 있기는 하나, 매우 불편하고 비용이 많이 들어, 화학제인 산화방지제를 첨가하는 방식을 택하는 경우가 많다. 약제학적으로 적합한 산화방지제는 프로필갈레이트, 부틸화한 히드록시아니솔, 브틸화한 히드록시톨루엔, 아스코르빈산, 나트륨 아스코르베이트, DL-또는 D-α-토코페롤 및 DL-또는 -α-토코페릴아세테이트 등이다. 산화방지제를 첨가할 경우, 그 양은 예컨데 0.1%(W/V), 바람직하기로는 0.0001 내지 0.3%로 할 수 있다. 일반적으로 친지방성 산화방지제(예:α-토코페롤)를 소수성 상과 혼합하고, 친수성 산화방지제(예; 아스코르빈산)는 친수상과 혼합시킬 수 있다.
참기름(바람직하기로는 화학적으로 정제된것)을 본 발명의 약제에 첨가할 수 있는데 이는 산화방지 작용이 있기 때문에다. 더우기 참기름은 약제의 방향성을 높여 환자의 불만을 해소할 수 있는 이점이 따른다. 이때 참기름의 사용량은 0.1 내지 3% W/V, 바람직하기로는 5 내지 20% W/V(최종 액체제 제량 대비)인데 보통 친지방성 상에 첨가된다.
한편 항균제의 사용량은 일반적으로 약 3% W/V 이하, 바람직하기로는 약 0.5 내지 2.5%(약제 총량대비)인데, 이에는 메티파라벤, 프로필파라벤, 부틸파라벤, 페놀, 디히드로아세트산, 페닐에틸알코올, 나트륨벤조에이트, 소르빈산, 티몰, 티메로살, 나트륨 디히드로아세테이트, 벤질 알코올, 크레솔, P-클로로-m-크레솔, 클로로 부탄올, 페닐머큐릭 아세테이트, 페닐머큐릭 보레이트, 페닐머큐릭 니트레이트, 벤질 알코늄 클로라이드 등이 있다.
필수적인 것은 아니지만 본 발명의 약제가 두가지 상(phase)의 형태로 존재하는 경우, 사람 또는 그밖의 동물에 있어 인지질단백체 또는 다른 복합체의 임파관 통과흡착(trans-lymphatic absorption)을 개량시키는 것이 통례이고 편의로운 일이다. 본발명에 따른 2상계(two-phase system)는 물-기름(즉 친수성-소수성), 물-기름-물, 기름-물 및 기름-물-기름 계의 상이 있다.
2 상계는 통상 친수 및 소수성 상을 균질하게 혼합하여 제조할 수 있다. 본발명에 따른 2상계는 유화액 또는 미세유화액(microemulsion)으로 할 수 있다. 친수성 상 : 소수성 상(V/V 비)의 비율은 일반적으로 0.2:1 내지 5:1, 대표적으로는 0.5:1 내지 2:1 이다. 유화액 또는 미세유화액을 생성시키려면 경우에 따라 두가지 상이한 계명활성제를 사용할 필요가 있는데, 그 하나는 친수성인 것으로 HLB 값이 높고, 다른 하나는 친지방성(상기한바와 같음)의 것으로 HLB값이 낮은 것이다.
본 발명에 유용한 친수성 계면활성제는 HLB값이 10이상 또는 17이상 그리고 경우에 따라서는 20가까이 될 수 있다. 한편 본발명에 사용되는 친지방성 계면활성제는 HLB값이 낮은것으로서 예컨데 10 이하의 것이다. 바람직하기로는 친지방성 계면활성제의 HLB값은 7이하 또는 경우에 따라 4이하의 것이 좋다.
본 발명의 약제를 제조하는데 사용되는 계면활성제를 선정하는 일반적 기준은 음이온성 계면활성제 또는 비이온성 계면활성제에 두고 있다. 이들 계면활성제는 약제 계통에 있어 상용성, 안정성 및 비독성이 높게 평가되어 특히 유용하다.
본 발명에 있어 여러 목적에 일반적으로 적합한 계면활성제로는 다음과 같은 것이 있다.
장쇄(C내지 C)의 지방산 : 예컨데 팔미틴산, 스테아린산 및 올레인산 ;
장쇄(C내지 C)의 지방산 에스테르 : 예컨데 나트륨 팔미테이트, 나트륨스테아레이트 및 나트륨 올레에이트 ;
나트륨 라우릴 술페이트 ;
폴리에틸렌글리콜의 지방산에스테르 : 예컨데 폴리에틸렌글리콜 모노- 또는 디-스테아레이트.
프로필렌글리콜 및 프로필렌 글리콜의 지방산 에스테르 : 예컨데 프로필렌 글리콜 모노스테아레이트 ;
글리세린 및 지방산 모노- 또는 폴리글리세라이드 : 예컨데 글리세릴 모노스테아레이트 ;

선택된 계면활성제는 약제학 용도의 목록에 들어 있는 것이며 적당히 낮은 LD값을 예시한다.
적당히 높은 HLB 값의 계면활성제는 다음과 같다.

본 발명의 단일 계면활성제 대신 계면활성제 혼합물을 종종 사용할 수 있음을 유의하여야 한다. 예컨데 단일 친수성 계면활성제 대신 둘 이상의 친수성 계면활성제를 사용할 수 있는데 혼합물의 효과적 HLB는 10 이상이어야 한다. 효과적 HLB란 계면활성제 혼합물의 친수-친지방성 평형가가 HLB10 이상의 단일 계면활성제와 동가 이어야 함을 의미한다. 비슷하게 친지방성 계면활성제의 혼합물을 단일 친지방성 계면활성제 대신 사용할 수 있다. 또 친지방성 계면활성제의 효과적 HLB는 10 이하이지 않으면 안된다.
본 발명의 약제에 사용되는 계면활성제의 양을 선택하는 것은 당분야에서 통상의 기술을 가진 사람에게 일임된다. 물론 각 경우에 있어 최적량은 사용되는 계면활성제의 성질과 약제에 함유된 다른 조성성분에 따라 결정된다. 일반적인 지침에 의한 친수성 계면활성제의 양은 0.1 내지 50g/1(약제 총량기준), 바람직하기로는 0.5 내지 25g/1, 가장바람직한 것은 1 내지 10g/1이다. 친지방성 계면활성제에 대하여는 기름상(oil phase)의 미세유화액에 관련하여 위에서 검토한 바와 같다. 그양은 0.1 내지 100g/1, 바람직하기로는 0.5 내지 50g/1, 가장 바람직하기로는 2 내지 25/g1 이다(약제총량기준).
본 발명의 약제 조성물은 여러 조성성분을 광범위하게 혼합하여 제조할 수 있다. 본 발명의 둘째 의의는 약제조성물의 제조방법을 제시하는데 있으며, 각 조성성분 및 그 제조방법은 상술한 바와 같다.
조성성분으로 첫째로 바람직한 것은 활성화합물(통상 단백질성 물질) 및 레시틴 전구체이다. 이것은 바람직한 구현체로서 인지질 단백체의 복합체를 형성할 수 있게 한다.
상술한 바와 같이 본발명에 따른 조성성분은 두개의 상(phase)을 포함할 수 있다. 이러한 조성성분의 약제 조성물을 제조하는 바람직한 방법은 다음과 같다.
(i) 생물학적 활성물질 및 레시틴 전구체를 함유하는 친수성 상을 형성하고,
(ii) 임의의 하나 이상의 흡수 증강제와 함께 친수성 상을 함유하는 2상계(two-phase system)의 제제를 생성시킨다.
유화액 및 미세 유화액(microemulsion)은 고유한 열역학적 안정성을 보유하고 있기 때문에, 본발명에 따른 액체 약제는 친수성 상과 친지방성 상을 혼합하므로서 쉽게 제조할 수 있으며, 이어서 그밖의 다른 조성성분은 함께 투입하여 제제를 완성시킬 수 있다. 그러나 화학적 결합의 동력학적 고려를 바탕으로, 본발명에 따른 유화 또는 미세유화액상의 약제를 보다 효과적으로 신속히 생성시키려면 실제적인 조치를 하여야 할 것으로 추정된다.
특히 친수성과 친지방성 상을 함께 혼합하는 과정과 그후 미세유동액 조성제(microfluidiser)를 사용하여 미세유화액을 신속히 형성시킬 수 있고 적당한 기계장치를 이용하여 효과적으로 유화액을 생성시킬 수 있다.
본 발명의 약제에 있어, 특히 2상 형태의 약제가 액상인 경우가 대부분인 것은 유의할 만하다. 그러나 이들은 통상의 방법에 따라, 고체형태나 산제로 전환시킬 수 있다. 일반적으로 액상 형태의 조성제는 유동액화기구(fluidiser bed) 또는 그와 유사한 기구(예; SPIR-A-FLOW 기구)를 사용하여 고체 부형제인 분말조성제 위에 피복시킬 수 있다.(SPIR-A-FLOW는 상표임). 위와 같은 처리로 생성된 산제 또는 과립은 경질 젤라틴 캡슐에 충전하여, 필요에 따라 코팅처리할 수 있다.
한편, 액체제제는 연질 젤라틴 캡슐에 충전하여 필요에 따라 장내 흡수를 대비하여 코팅처리할 수 있다[참조 ; Remington's Pharmaceutical Sciences, 15th Edi t i on, PP. 1614-1615(1975); 2nd Edit., PP 116-117, 371-374(1976); Hagers Handbuch der Pharmazeutischen Praxie, 4h Edit., Vol. 7a(Springer Velaf 1971), PP 739-742, 776-778].
본 발명에 따라 제조한 약제는 경구 투여할 수 있으며, 그 투여방식은 다양하게 조절할 수 있는데, 예컨데 액제, 젤라틴 캡슐(연질 또는 경질 캡슐), 정제(코팅처리할 수 있음) 그밖에 여러형태의 약제로 형성시킬 수 있다. 또 본발명에 따른 방법으로 투여한 생물학적 활성 물질은 높은 혈액 농도를 나타내는 강한 활성을 나타내어 소기의 치료효과를 나타낸다.
직장투여를 위하여는, 약제 또는 고체약제를 관장 또는 좌제 형태로 실시할 수 있다.
본 발명의 또다른 의도에 따라, 본 발명의 경구 또는 직장투여용 약제를 투여하여 사람과 그밖에 동물에 대한 치료방법이 제시된다. 특히 본 발명은, 생물학적 활성성분이 인슐린인 본 발명의 약제를 직장투여, 바람직하기로는 경구투여하여 당뇨병을 치료하는 방법에까지 범위가 포함된다.
본 발명은 또한 생물학적 활성물질에 의하여 치료조절될 수 있는 질환의 예방 또는 치료를 위한 경구 또는 직장투여용 약제를 제조함에 있어서 본 발명에 따른 약제조성성분을 사용하는것도 그 범위에 포함된다.
특히 당뇨병을 치료 또는 조정하는 약제를 제조하는데 인슐린을 사용할 수 있다. 연어 칼시토닌(calcitonin)은 골전복심화증[예컨데 파겔씨 골질환 (Parget's disease of the bone)]과 악성 골다공증과 병발하는 급성 과칼슘증(hypercalcaemia)의 치료에 사용할 수 있다. 에리트로단백질은 체외 신장 투석기구의 만성사용, 항암치료의 화학요법, 그밖의 원인에 의해 야기되는 빈혈증 치료에 사용할 수 있다. 돼지 소마토트로핀은 돼지에 투여하여 생장 기간을 감소하고 등에고인 지방두께를 줄일 수 있다. 사람성장호르몬은 성장 장애의 소아를 치료하는데 사용할 수 있다.
다음 실시예를 통하여 본 발명을 구체적으로 설명하고자 한다. 그러나 본 발명이 이들에 의하여 제한되는 것은 아니다.
[실시예 1]
본 실시예에선 생물학적 활성성분의 인슐린이고, 인지질-인슐린 복합체 (pho s pholipo-insulin complex)를 함유하는 약제를 제조한다.
실온에서 소인슐린(약 22내지 261U 활성/mg의 효능을 가진 소인슐린결정 또는 분말 2 내지 20mg을 1.0 내지 3.0g의 콩레시틴 난황레시틴, L-α-포스파티딜산 비미리스토일(나트륨염) 및/또는 1.2-디미리스토일-sn-글리세롤-3-포스포콜린 1 수화물과 혼합하여, 400mg의 아프로티닌(3,000,000 칼리크레인 불활성화제 단위) 존재하에 0.9% 벤질알코올 또는 0.9% 염화나트륨용액(150ml)에 용해시키고, 실온에서 시트르산 용액으로 pH2.3 되도록 조절한다. 비이온성 계면활성제, 바람직하기로는 폴리옥시-40-스테아레이트 4g을 상기 인지질-인슐린화합물에 가한다.
실온하의 완만한 상태에서 상기 인지질-인슐린 용액을 프리-카일로미크론 복합체(포스파티딜콜린 ; 모노-, 디-, 및/또는 트리-글리세리드 ; 콜레스테롤 및 그밖의 화합물복합체), 4.0 이하의 HLB값을 가진 비이온성 계면 활성제 및 하나이상의 산화방지제로 이루어진 기름상(oil-phase)용액에 서서히가 한다.
상기 물-기름 유화액을 5℃ 내지 10℃의 마이크로 플루다이저 (microflui d izer)로 2시간 연속하여 100,000 PSI 이상의 상태에서 통과시킨다. 이같이하여 물-기름 미세유화액 상태의 인지질-인슐린 복합체를 제조한다. 이 미세유화액 400ml 중에는 각각 다음 조성성분이 함유되어 있다.
용해시킨후, 3000내지 4000 IU의 아프로티닌(1000-15000 IU EPO당) 존재하에 0.5M 인산염 완충용액(pH7.8)으로 pH를 7.3으로 조절한다.
잔여등분의 EPO는 100ml의 생리적 식염수에 용해시키고, pH를 7.3으로 조절한후, 아프로티닌 및 HLB값 7.0 이상의 비이온성 계면활성제(예;폴리옥시-40-스테아레이트)를 1000 IU EPO 당, 0.00044 내지 0.0044mg의 농도로 가하고 이어서 실온에서 유화안정제 및 점도증가제(예;히드록시프로필셀루로스-SL을 용해시킬 수 있다).
실온하 완만한 교반을 계속하면서, 인지질-EOP 복합체 및 EOP함유 수상용액(water phase solution)을, 콜레스테롤 레시틴, 글리세롤 모노스테아레이트(4.0 이하의 HLB값을 가진 비이온성 계면활성제) 및 산화방지제가 함유된 유상용액(oil phase solution)중에 서서히 가한다.
상기 물-기름 유화액을 유동화-유화기(microfludizeremulsifier)에 냉각 하면서 1회 통과시킨다.
1000 IU의 각 EPO에대한 생성 W/O EPO 미세유화액은 다음 조성성분을 함유한다.
[실시예 2]
본 실시예에서는 생물학적 활성화합물이 에리트로포이에틴(erithropoietin : EPO)이고, 인지질-에리크로포이에틴 복합체는 위 실시예 1의 경우와 같이 제조한다. 또 EPO의 절반은 2상(미세유화액)계의 수상(water-phase)에 직접 가한다. 따라서 본 실시예에 있어서는 단백질성 화합물은 인지질과 결합하고 있을 뿐아니라, 미세유화액의 수상계에 직접 가하여진다.
에리트로포이에틴(EPO)는 상업용 [참조 : 일본국 쥬가이 약품주식회사 (Lot NO. R9H05)제품]의 것을 수득할 수 있다. 1등분량은 단백질 농도가 0.936(아미노산 분석법에 의한 것)이고, 그 양은 1.01mg(RP-HPLC 분석에 의한 것)이고 생체내 활성도 (in vivo specific activity)는 pH7.2(인산염완충용액)에서 180,000 IU 이다. 실온에서 EPO 소정량을 양분한다. 첫째 등분의 EPO에 0.004 내지 0.007mg의 1,2-디미리스토일-sn-글리세롤-포스포콜린 1수화물/1000 IU EPO를 가하고, 생성된 혼합물을 50ml 의 0.9% 생리적 식염수에 용해시킨후, 3000 내지 4000 IU의 아프로티닌(1000-15000 IU EPO당) 존재하에 0.5M 인산염 완충용액(pH7.8)으로 pH를 7.3으로 조절한다.
잔여등분의 EPO는 100ml의 생리적 식염수에 용해시키고, pH를 7.3으로 조절한후, 아프로티닌 및 HLB값 7.0 이상의 비이온성 계면활성제(예;폴리옥시-40-스테아레이트)를 1000 IU EPO 당, 0.00044 내지 0.0044mg의 농도로 가하고 이어서 실온에서 유화안정제 및 점도증가제(예; 히드록시프로필셀루로스-SL을 용해시킬 수 있다).
실온하 완만한 교반을 계속하면서, 인지질-EPO 복합체 및 EPO함유 수상용액(water phase solution)을, 콜레스테롤 레시틴, 글리세롤 모노스테아레이트(4.0 이하의 HLB값을 가진 비이온성 계면활성제) 및 산화방지제가 함유된 유상용액(oil phase solution)중에 서서히 가한다.
상기 물-기름 유화액을 유동화-유화기(microfludizeremulsifier)에 냉각하면서 1회 통과시킨다.
1000 IU의 각 EPO에 대한 생성 W/O EPO 미세유화액은 다음 조성성분을 함유한다.
[실시예 3]
본 실시예에서는 인지질-단백질 소마토트로피(PST) 복합체와 약제를 제조한다.
돼지 소마토트로핀(Poreine somatotropin:pST) 결정분말(pST Lot NO. 73 68C-25Q)를 상업용으로부터 수득한다. (제품참조 ; American Cynamid Company, Agricultural Research Division, Princeton, New Jersey, USA).
실온에서 pST를 인지질(예 ; 1,2-디미리스토일-sn-글리세롤-3-포르포콜린 1 수화물)을 함유하는 0.9% 염수(saline solution), L-알파-포스포티딜산 비미리스토일(나트륨염), 난황레시틴 및/또는 콩레시틴의 혼합물에 가한다.
HLB값 7.0 이상의 비이온성 계면활성제를 아프로티닌, 트립신 억제제가 존재하는 생리적 식염수와 함께 가한다.
수상(water phase)용액의 pH를 필요에따라 0.1M Nacl/10mM 인산나트륨 완충액으로 pH7.2로 조절한다. 실온에서 완만한 교반을 계속하면서 상기 인지질-pST 용액을 서서히 레시틴-콜레스테롤-글리세롤-모노올레에이트 혼합물이 함유된 기름상 용액에 가한다. 생성 혼합액을 냉각하면서 유동화-유화기에 두번 통과시킨다. pST 미세 유화액은 각각 다음 조성성분을 함유한다.
[실시예 4]
재조합한 사람 성장호르몬(r=hGH ; Bath No.9-08 P-508-2, 1989년 8월 15일)을 상업용 제품으로부터 수득한다(제품참조 ; Smithklein Beecham of Philadelphia). r-hGH를 다른실시예 경우와같이 약제 조성물로 혼합한다.
특히 r-hGH(500mg 분말)를 0.9% Nacl 용액에 녹인다. 수중의 인지질유도체를 가하여 수용성 인지질-r-hGH-복합체를 형성시킨다음, 서서히 유상(oil phase)액에 가한다. 유상액은 난황레시틴, 콜레스테롤, 글리세롤-1-모노올레에이트, δ-알파-토코페롤, 산화방지제용액 및 트윈-80으로 이루어져있다. 이물-기름유화액을 냉각하면서 유동화-유화기에 1회 통과시킨다.
r-hGH 미세유화액의 각 ml는 다음조성성분을 함유한다.
10mM 인산나트륨 완충액으로 pH를 7.2로 조절한다. (유의 : 본 실시예에서는 트립신억제제를 가하지 않는다.)
[실시예 5]
본 실시예에서는 연어 칼시토닌(제품참조 : Rorer Central Research of Hirsham, Pennsylvania, USA ; NPD #8906046 NPP #211)을 0.9% 염수증의 아프로티닌 존재하에 난황레시틴과 혼합한다음, 시트르산 및 아스콜빈산으로 pH를 2.0으로 조절한다. 이같이 형성된 수용성 인지질-sCT-아프로티닌 복합체를 실시예 1의 경우와 같이 물-기름 미세유화액으로 만든다. 100IU의 sCT를 함유하는 제제의 각부분은 다음 조성성분을 함유한다.
[실시예 6]
실시예 1의 분말 또는 과립상 인슐린은 약제학적 불활성담체(예; 카르복시메틸셀룰로스-Ca, 젤라틴, 히드록시프로필셀루로스-L, 아르기닌산 또는 그들혼합물)에 상기 미세유화액을 분무피복(spray coating)시켜 제조할 수 있다. 분말(또는 과립) 인슐린 함유제제를 NO.1 크기의 경질캡술에 충전시켜 다음 조성성분으로 형성시킨다.
NO.1 크기의 경질캡술의 각개 중량 : 250mg
* 1,2-디미리스토일-sn-글리세롤-3-포스포콜린 1 수화물
+ 이들은 사람에 특히 비흡수성이므로 약제학적으로 불활성물질이다.
[생물학적 실시예 A]
실시예 1의 방법에 따라 제조한 구강투여약제를 사용하여, 당뇨병환자에 대한 임상시험을 한다. 실시예 1의 약제는 다음과 같이 혈당치를 저하시키는 효과를 나타낸다.
일반적으로 레시틴-소 인슐린 약제의 저혈당화효과는 경구투여후 30 내지 90분 사이에 신속히 나타나는데, 투여후 120분 정도 경과되면 혈당치는 약간 상승하나 3시간정도 경과후에는 다시 떨어진다. 따라서 인슐린의 경구투여는 중복효능을 나타낸다. 소인슐린의 친지방성 부위를 인지질(예 ; 레시틴)과 결합시킴으로서 인지방-인슐린복합체[다른지방단백질(lipoprotein)과 같이 체내에서 친수성 화합물이됨]를 형성하게된다. 인지질-인슐린복합체 (phospholipo-insulin complex)부위가 사람의 위장관계기관으로부터 즉시 흡수되어 저혈당화작용을 신속히 나타내는 것으로 볼 수 있다. 흡수되지 아니한 잔여복합체는 간장으로 이송되어 그곳에서 잠시 보존되었다가 순환혈액중으로 방출되어 인슐린 수용체내에서 2차 작용을 나타내는 것으로 추정된다.
[생물학적 실시예 B]
건강한 남자지망생 8명이 본 시험연구에 참여한다. 하루밤 절식한후, 시험 제1일에 피검자에 다음 시험약제를 투여한다.
혈액시료를 약제투여후 0, 0.5, 1, 2, 3, 4, 6, 10 및 14시간 경과시마다 채취하며, 망상적혈구(reticulocyte)를 산정하고, 방사면역학적 분석방법에 따라 EPO 값을 측정한다.
시험제2일 및 제3일에 각 피검자엑 08:00, 14:00 및 22:00의 각시간에 예정된 시험약제를 투여하고 하루밤 절식시킨후 시험제4일에 상기한 바와같이 시험약제의 경시적 효과를 검사한다.
특히 시험제4일째의 망상적혈구수는 이들 피검자의 시험결과, 10 IU/kg EPO 정맥주사후와 함께 경구용 EPO 15IU/kg 투여후 그리고 20IU/kg 투여후 증가함을 보여준다. 상기적혈구수는 의약 그릅(placebo group)에서는 점차로 감소된다.
시험제1일에 플라스마 RIA(방사면역학적분석)측정 EPO 값을 10 IU/kg EPO 정맥주사후, 그리고 경구용 EPO 15 IU/kg 투여후 및 20 IU/kg 투여후 현저히 증가한다. 의약그룹에 있어서는 다시 EPO값이 점차로 감소한다. 실시예 2의 약제에 의한 EPO는 사람에 경구투여할 수 있고 활성이 나타난다.
[생물학적 실시예 C]
실시예 3에서 제조한 약제(디미리스토일글리세롤-포스포콜린-포스파티딜 비미리스토일-pSCT복합체)를 시험돼지에 십이장내 흡수시킨다. 실시예 3의 약제를 투여하기 전과 후의 혈당치 및 플라스마 pST값(방사면역학적분석으로 측정한것)을 검사한다. 아래에 그결과를 나타낸다. 혈당치는 상대적으로 오르고 RIA(방사면역학적분석)-측정 pST값을 증가함을 나타낸다.
[생물학적 실시예 D]
실시예 4에서 제조한 약제(인지질-감마-사람성장호르몬복합체를 함유)를 사용하여, 건강한 남자 피검자 9명에 대하여, 경구투여한 r-hGH의 임상적활성(경구투여의 r-hGH에 의한 혈당치변화를 측정) 및 임상적 효능을 검사한다.
표 : 피검자인의 명세내용(모두 남자피검인)
9명의 피검인에게 r-hGH를 0, 7, 15 및 30ml(0, 7, 15 및 30mg의 경구 r-hGH약제) 투여하고, 유도되는 혈당치 및 hGH 값을 측정한다. 측정은 EDTA-처리플라스마를 위한 시판-측정기구로 실시한다. 각 피검인에 대한 시험결과를 제1도 내지 제9도에 각각 나타내고, 방사면역학적 분석에 의한 플라스마 hGH에 대한 r-hGH의 경구투여효과를 나타내는 평균투여량을 제10도에 나타낸다. 플라스마 hGH에 있어서, RIA-측정평가 및 당뇨병 유발성을 관찰한다. 모든 피검자에 활성약제를 투여하고 관찰한 결과는 본 발명의 다른약제(예; 돼지의 경구 pST 및 당뇨병의 경구인슐린)을 투여한경우와같이 중복 효능을 보이는 변화를 한다.
명확한 사실은, 경구투여한 r-hGH(인지질-r-hGH로서)은 흡수되어, 사람에 있어 hGH의 생물학적 작용을 유도한다는 것이다. 순화혈액중에는 통상 경구투여하여 0.5 내지 4 시간 경과후에 효능이 발현되는데, hGH는 간으로 이송되어 그곳에서 방출되고, 순환혈액중에서는 경구투여 8 내지 12(또는 그이상)시간후 다시 효과가 발현되는 것으로 보인다.
hGH의 효능(및 활성)에 있어 중복성 효과는 투여량에 따라 영향을 받는데, 비교적 적은 투여량에 있어서 경구투여 hGH는 0.5 내지 2시간 경과후 효능이 나타나고, 다시 8시간정도 경과후 한번도 효능이 나타난다. 한편 높은 투여량(30mg의 r-hGH)의 경우는 최초의 효능피크(peak)가 경구투여 약 4시간후 일어나고 두번째 효능피크는 약 11시간후에 나타난다.
적당한 투여량은 사람에 있어 15mg이다. 본 시험에서는 7mg도 현저한 효능을 나타낸다. 사람에게는 적합한 치료효과를 얻을 수 있는 투여량이 1 내지 10mg, 특별하게는 3 내지 5mg이다.
[생물학적 실시예 E]
실시예 5의 약제(경구투여형태의 연어칼시토닌 : ODDS-sct)를 젊은남자 피검인 그룹에 투여하고 검사한다. 이들 지원피검자들의 명세내역은 다음과 같다.
각 피검인을 하루밤 절식시킨후, 시험당일 오전 6시에 ODDS-sCT 캡슐을 투여하거나 CLASYNAR(상표의 표시)(주사용 연어 칼시토닌제제)를 피하주사한다. 팔뚝정맥으로부터 내재카테터(indwelling catheter)를 통하여 정맥혈액시료를 시험약제투여후, 0, 60, 90, 120, 150, 180, 210, 240, 300 및 360분 경과시마다 수집한다. 혈액시료채취후 즉시 혈청인산염 농도를 측정하는 한편, EDTA-처리한 혈장시료에 대한 방사면역학적 분석에 의해 혈장내 연어 칼시토닌 농도를 검사한다.
300IU 또는 600IU의 ODDS-sCT 캡슐을 투여받은 모든 피검인에 있어 혈청인산염 농도는 현저히 감소하였는데, 실시예 5의 연어 칼시토닌 약제를 경구투여한후 일어나는 혈정인산염 농도의 그러한 변화는 연어 칼시토닌을 피하주사한 피검인에 있어서도 유사하게 나타난다. 300IU 및 600IU의 ODDS-sCT를 경구투여한 경우는 100IU의 sCT를 피하주사한 것과 유사한 혈청인산염 농도의 효과를 나타낸다. 따라서 본 발명의 약제는 그 효과가 현저함을 나타낸다.
[실시예 7]
0.9%의 등장성 염 용액(isotonic saline solution)중의 아프로티닌 존재하에 1,2-디미리스토일-sn-글리세롤-3-포스파디딜산 1 수화물과 L-α-포스파티디콜린 비미리스토일(나트륨염)을 혼합하여, 인지질-연어칼시토닌 복합체를 제조하고, 30분간 화학적 상호작용을 유도한다. 소장 상피조직에서 포스포티딜콜린 합성의 L-α-포스파티딜글리세롤 경로에서 참여하는 것으로 알려진 인지질(phospholipid)은 명백하게 생체의 연어 칼시토닌의 친지방성 부위에 비공유결합을 하고 있다.
상기 인지질-연어 칼시토닌 복합체를 점도증가제[증량제(thickner)] 및 현탁 인지질 칼시토닌의 안정제(예컨데 5% 이하, 바람직하기로는 2% 이하의 히드록시프로필셀루로스) 존재하에, HLB14 이상의 계면활성제(폴리옥시-40-스테아레이트)를 함유하는 용액과 혼합한다. 시트르산 및 아스코르빈산의 농축액으로 pH2.0 근처로 조절한다(다른 산성물질도 pH 조절에 사용할 수 있다).
위에서 생성된 용액을 HLB4 이하의 계면활성제 존재하에 3 내지 4배량의 올레인산(그밖에 탄소수 16 이상의 지방산)과 함께 현탁시킨다. 탄소수 16이상의 지방산은 증량제로 작용함과 동시에, 인지질-연어 칼시토닌의 막투과 흡수를 위한 증강제로 작용할 수도 있다. 또 인지질-연어 칼시토닌의 장내흡수대비를 위한 피복처리기능도 발휘할 수 있다. 그러나 올레인산/계면 활성제 조합은 절대 필수적인 것은 아니다.
본 실시예의 인지질-연어 칼시토닌의 화학적 조성을 다음에 기재한다.
약제를 만드는 방법은 다음과 같다.
A부를 잘 혼합하고 실온에서 30분 이상 방치한다. B부를 따로 혼합하여 조제하고, 이에 A부를 실온하에 서서히 교반하면서 가한다. 시트르산 또는 아스콜빈산으로 pH를 2.0으로 조절한다.
유상용액(oil solution)의 C부를 혼합하여 조제한다. 이를 서서히 교반하면서 A부 및 B부 제제를 가한다.
이혼합용액을 동일중량의 다음분말, 즉 카르복시메틸-셀루로스-Ca 및 알지닌산을 함유하거나 알지닌산 및 젤라틴을 함유하는 분말상에 분무코팅(spray coating) 처리할 수 있다. 피복건조된 분말은 경질캡슐에 충전하여, 피검인 또는 환자에 투여할 수 있다. 한편 액체형태의 약제는 그대로 경구 투여하거나 연질캡슐로 경구투여할 수 있다.
[생물학적 실시예 E]
건강한 남자 지원 피검인(volunteer) 6명이 본검사에 참여한다. 하루밤 절식시킨후 시험당일 05시 30분에 실시예 7의 연어 칼시토닌 약제를 4명의 피검인에 경구투여한다. 두 피검인에게는 실시예 7의 연어 칼시토닌 캡슐의 300IU를 주고, 다른 두 피검인에게는 실시예 7의 경구용 연어 칼시토닌을 600IU투여한다. 두피검인에게는 연 칼시토닌 주사액(CALCYNAR 상표의 주사액) 100IU를 피하 주사한다. 혈청 인산염 및 혈장칼시토닌 농도를 방사면 역학적 분석방법으로 투약30분전, 투약시 그리고 투약후 30, 60, 90, 120, 150, 180, 210, 240 및 300분 경과시마다 검사한다.

건강한 남자 피검인 그룹에게 경구투여한 실시예 7의 약제인 연어 칼시토닌은 연어 칼시토닌(salmon caleitonin)을 피하주사한 효과와 같거나 그보다 큰 정도로 혈청인산염농도를 감소시키는 생물학적 활성을 나타낸다. 그러나 실시예 7의 연어 칼시토닌을 300 또는 600IU 경구 흡수시킨후 적용되는 현재의 방법에 의하여 RIA-측정성 혈장 sCT는 항상 검정되지는 않는다.
sCT는 인지질과 결합하여 sCT에 어떤 항원성 변화(antigenic change)가 일어난, 방사면역학적 분석에 사용되는 sCT-특수항제로 인하여 쉽게 sST가 검정되지 않는 경우도 있다.
[실시예 8]
경구 투여용 인슐린(소 인슐린)약제를, 적당한 계면활성제 및 이소부탄 알코올 중의 아프로티닌(0.9%)의 존재하에 인슐린 및 L-α-포스포콜린(레시틴전구체)을 혼합하여 제조한다. 이같은 형성된 비공유 결합성 복합체를 물과 MCT[중간길이 쇄의 트리글리세라이드 (medium chain-length trigly ceride)] 기름중에 현탁시킨후 당뇨병 환자 그룹에 경구 투여한다.
약제의 화학적 조성을 다음에 나타낸다.
약제는 다음과 같이 제조한다.
A부를 실온에서 서서히 교반하여 충분히 혼합한다. 100ml의 탈이온수에 HPC 및 폴리옥시-40-스테아레이트를 완전히 용해시켜 B부를 제조한다. B부를 실온에서 서서히 교반하면서 여기 A부를 가하여 충분히 혼합한다.
이 인슐린약제는 당뇨병환자에 그대로 경구투여하거나, 10 내지 30ml의 MCT(중간길이쇄의 트리글리세라이드)기름에 현탁시킬 수 있는데, 이것은 장내흡수대비 코팅효과를 나타내고 중량제작용을 하여, 약제가 유문을 통하여 십이지장-공장-회장내로 이송되도록 촉진한다.
[생물학적 실시예 G]
당뇨병환자 8명이 본 시험연구에 첨가한다. 절식하면서 시험당일 이른아침 각 피검인에 실시예 8의 경구용 인슐린약제를 투여한다. 혈당치를 2시간 동안 측정한다.
실시예 8의 인슐린약제를 경구투여하면, 양 당뇨병형태의 과혈당증을 조절하는 효능을 보인다.
Claims (19)
- (a)물, (b)생물학적 활성 단백질성 물질 및 (C)활성단백질 물질과 관련된 인지질을 포함하는 친수성상과 기름, 인지질 및 10이하의 친수성/친지방성 평형가를 갖는 친지방성 계면활성제를 포함하는 친지방성상을 포함하는 비리포좀 의료용 약제.
- 제1항에 있어서, 단백질성 물질이 인슐린, 에리트로포이에틴(erythropoietin), 돼지 소마토트로핀, 사람 성장호르몬 또는 연어 칼시토닌인 약제.
- 제1항에 있어서, 인지질이 다음 일반식 화합물인 약제.여기에서, 각 R1및 R2는 독립적으로 모노- 또는 폴리- 불포화된 탄소수 14내지 26의 아실기를 나타내고, X는 수소원자 또는 인지질 선두그룹이다.
- 제3항에 있어서, 인지질 선두그룹이 에탄올아민, 콜린, 세린 또는 글리세롤 잔기인 약제.
- 제3항에 있어서, 인지질이 아래 화합물로 이루어진 약제. 디미리스토일 포스파티딜 글리세롤(DPMG), 디팔미토일 포스파티딜 글리세롤(DPPG), 디스테아로일 포스파티딜 글리세롤(DSPG), 디미리스토일 포스파티딜콜린(DPMC), 디팔미토일 포스파티딜콜린(DPPC), 디스테아로일 포스파티딜콜린(DSPC), 디미리스토일 포스파티딘산(DPMA), 디팔미토일 포스파티딘산(DPPA), 디스테아로일 포스파티딘산(DSPA).
- 제3항에 있어서, 인지질이 아래 화합물로 이루어진 약제. 디미리스토일 포스파티딜 에탄올아민(DPME), 디팔미토일 포스파티딜 에탄올아민(DPPE) 또는 디스테아로일 포스파티딜 에탄올아민(DSP트).
- 제1항에 있어서, 인지질이 레시틴인 약제.
