Abstract
본 발명은 동맥류를 치료하는에 이용되는 대동맥 또는 장골이동맥 관강보철을 삽입하는데 이용되는 기구장치가 보철과 결합된 두 개의 팽창스텐츠 기구를 장치한 카테테르를 이용하는 것을 특징으로 한다.

본 발명은 동맥류(동맥 또는 정맥의 관벽이 이완확대되어 생기는 낭상물, 외출혈 또는 내출혈의 원인)의 치료하기 위한 동맥 또는 이장골동맥 관강 보철을 이식하는데 이용되는 기구장치에 관계하며 더 구체적으로는 대동맥벽에 튜브형 보철을 부착장치를 적응시키는데 있어 카테테르에 장착된 팽창형 기구에 관계한다.
이미 공지된 것으로는 혈관내 장치로 스텐츠(stents)라는 것으로써 기구 카테테르에 유입장치를 이용하여 기관내에 장치는 것이다.
혈관내 보철을 부착시키기 위한 수단으로써 이용되는 스텐츠는 대동벽의 내부에 죄여질때까지 기구가 팽창하여 직경이 증가되는 것이다.
팔마즈가 출원한 미국출원 제 535,745 호(1990.6.11)와 본발명은 동맥류가 형성된 동맥을 따라 이식될 수 있는 팽창형 공축튜브가 연결된 스텐츠 또는 두 개의부착수단으로 구성된 보철에 관계한다.
본 기구장치와 함께 보철-말단에 스텐츠의 부착은 매우 지루하고 시간이 많이 드는 작업이다.
스텐츠의 부착을 설계한 장치는 로버트 에르섹이 1972. 4. 25. 일자로 출원한 미국특허 No.3,657,744에도 설명되어 있으나 카테테르를 이용하는 것이 아니라 치료할 부위를 절개해야 한다.
팽창형 기구와 카테테르의 삽입기술은 판형성술과 혈관 형성술 분야에 공지된 것이며 하나이상의 관강에 팽창 카테테르를 이용한다.
투개로된 접착수단 또는 스텐츠와 팽창튜브에 의해 공축으로 연결된 상기 언급한 보철과 카테테르를 삽입하고자 할 때 두 개의 카테테르를 사용하여 근접 스텐츠와 외접 스텐츠가 연속적으로 팽창 될 수 있도록 한번에 하나씩 사용해야 한다.
이러한 작업은 매우 힘들고 시간이 많이 소요되어 추천할만한 것이 못된다.
본 발명의 장치는 동맥류 치료에서 상기 언급한 보철을 이식하는데 있어서 세 개의 관강으로 한 개 카테테르에 있는 두 개의 기구를 사용하여 작업을 단순화시킨 것이다.
또한 본 발명의 장치는 횡복동맥이 갈라진 부분에 바로 동맥류가 위치할 때 보철삽입에서 발생되는 문제점을 발견할 수 있다.
본 발명의 주된 목적은 동맥류 치료에 이용되는 대동맥 또는 횡복이동맥 관강내에 보철을 삽입하는데 이용되는 기구장치로써, 그 구성은 신규한 것으로써 카테테르위에 일정거리를 두고 두 개의 팽창형 기구로 구성되고, 카테테르에는 세갈래의 통로가 있고, 각 기구는 적어도 이내부 통로중 어느하나와 연결되어 있는 것을 특징으로 한다.
상기 언급된 기구의 실행에 있어서, 적절한 유형은 말초기구는 카테테르의 삽입말단에 근접하게 위치하고, 기부기구는 삽입된 보철의 길이에 비례하여 그거리만큼 멀리 위치하도록 한다.
실행에 있어서, 상기 적절한 모양으로 기부기구는 카테테르의 입구가되고(말초기구의 반대편)이 기구는 기구의 평행축에 대해 약 30°정도의 각으로 옮겨져 있게된다.
본 발명의 장치의 장점과 그 목적은 첨부된 도면과 다음의 상세한 설명에서 가치를 인식할 수 있다.
제1도 윗부분에는 흉부대동맥(1)과 이와 떨어진 신장동맥(2)가 연결된 치료된 복부대동맥부분을 나타낸 것이다. 복부대동맥에 위치한 동맥류(5)는 흉부대동맥(1)에서 횡복동맥(1)의 두갈래부위까지 거의 전부에 퍼져있다.
치료방법은 동맥류(5)내에 보철을 주입하는데 보철은 비활성재질로 만들어진 탄성라이너의 양쪽말단에 봉합된 말초스텐츠(4)와 기부스텐츠(8)로 구성되어 있다.
상기 설명한 것과 같이 스텐츠(4,8)은 매우 미세한 벽으로 구성되어 있고 압력은 원튜브 직경보다 큰 직경으로된 튜브를 형성할 수 있도록 벽은 압력을 받아 팽창할 수 있다.
스텐츠(4,8)의 직경을 증가시키는 수단은 그내부에 관강이나 라인이 있는 카테테르(6)에 의해 동맥으로 기구(3,9)를 유입시킴으로써 가능한데, 기구(3,9)로 유체를 불어넣을 수 있다.
유도자나 가아드 와이어(guide wire)는 대퇴도맥에 절개하여 통과할 수 있는 것으로 구성된 금속선으로 이루어진 유닛을 이용하고, 라디오그래프로 감지할 수 있다.
치료할 동맥류부위에 유도자를 장치한후 유도자에 의해 스텐츠(4,8)이 기구(3,9)에 장착된 카테테르(6)를 삽입시키고 이 카테테르의 라인은 직경이 최소한이 되도록 압축시킨다.
튜브 또는 라이너(7)는 기구(3,9)에 있는 두 개의 스텐츠(4,8)사이에 위치한다.
말초스텐츠(4)와 그기구(3)이 동맥류(5)위에 있는 탄탄한 벽에 담으면 유도자를 빼내고 기구(3)은 최대직경이 되도록 팽창시키고, 스텐츠(4)가 제1도에서처럼 대동맥벽에 삽입될 수 있게된다.
스텐츠(8)과 그 기구(9)의 삽입과정은 동일하다.
기구(3)과 (9)의 팽창은 동시에 또는 차례로 실시될 수도 있다.
스텐츠(4)(8)이 제자리에 장치된후에 기구(3,9)에서 공기를 빼고 카테테르도 빼낸다.
제1도에서처럼 장골동맥(1) 분기부위에 인접하여 동맥류(5)가 위치하고 있을 때 기부스텐츠(8)의 정확한 연결이 어렵게 되는데 그이유는 카테테르(6)가 복대동맥과 공축을 이루지 않고 스텐츠(8)의 내벽에 대한 기구(9)에 의해 압력이 동일하지 않기 때문이다.
이러한 경우 기구(9)를 제2도에서와 같이 특이한 형태를 갖도록 한다.
기부기구(9)는 기구(9) 횡축에서 d 만큼 떨어진 부위에 카테테르(6)의 출구(13)를 가지고 그크기는 약 기구(9)반경의 반이 되며 카테테르(6)는 기구(9) 횡축에 대해 약 30°정도의 α각의 위치에 자리한다.
기구(9)의 내부는 거의 안장모양(12)이 되는데 장골동맥의 분지(10) 부위에 위치하기 때문이다.
동맥류(5)가 횡복동맥(11)에 관계할 경우 분지된 튜브형태의 라이너(7)이 이용되는데 기부기구(9)의 직경이 횡복동맥(11)의 직경과 같게되고 카테테르(6)와 말초기구(3)은 공축을 형성하게 된다.
장치의 실행에 있어서, 적합한 형식은 카테테르(6)의 길이가 50내지 75㎝ 두께가 프렌치 눈금으로 5내지 10이 되고 말단부가 블런트(blunt) 모서리의 형태로된 원뿔정을 가진 폴리비닐클로라이드로 만들어진다.
카테테르(6)의 중앙통로 또는 관강은 가이드코드 삽입과 대비물질 주입시에 이용된다. 다른 두개통로는 각 기구(3)(9)의 팽창과 수축에 이용된다.
또한 카테테르는 매 15㎝ 마다 표식과 각 기구(3)(9)의 시부와 말부에 방사선 불투과 표식을 하고 있다.
좀더 적절한 실행에는 기구(3)(9)는 폴리비닐클로리드 또는 폴리에틸렌으로 만들어지고 양말단부가 뭉퉁한 실런더형으로 그길이가 3내지 5㎝, 그 직경이 16내지 30mm가 된다.
본 발명은 상기 설명된 구조, 작용, 재질과 구체예에 한정하지 않으며 첨부된 청구범위에만 본 발명이 한정되고 공지된 기술 분야에서 가능한 수정과 변형은 본 발명에 속함이 명백하다.
Claims (5)
- 대동맥 관강내 또는 장골이동맥 보철을 삽입하는데 이용되는 기구(balloon)장치에 있어서; -카테테르는 적어도 세 개의내부 통로를 가지며;-카테테르에는 두 개의 팽창 가능한 기구(balloon)가 있는데 두 개의 기구는 일정거리를 두고 떨어져 있으며 각 기구는 적어도 한 개의 내부 통로와 연결된 것을 특징으로 하는 기구(balloon) 장치.
- 제1항에 있어서, 카테테르에는 삽입단부가 있고, 제1기구는 카테테르 삽입 단부 가까이 있고, 제2기구는 제1기구와 보철 길이에 해당되는 거리만큼 떨어져 카테테르에 배치되어 있는 것을 특징으로 하는 기구 장치.
- 제2항에 있어서, 카테테르는 제2기구를 통과하고, 이때 제2기구에는 종축으로 하여 제1단부와 제2단부가 있으며, 제2기구의 제1단부는 제2기구의 제2단부보다는 제1기구와 가깝고, 카테테르는 제2기구의 종축에 대해 약 30도 각을 이룬 위치에서 제2기구의 제2단부로 들어가는 것을 특징으로 하는 기구 장치.
- 제3항에 있어서, 카테테르가 제2기구의 단부를 통과할 때, 카테테르는 제2기구의 종축방향에서 제2기구 반경의 1/2인 d위치에서 제2기구 출구로 통과하는 것을 특징으로 하는 기구 장치.
- 제2항에 있어서, 제1기구와 제2기구는 각각 종축에 대해 단면직경을 가지는데, 제1기구의 직경이 제2기구의 직경보다 크고, 카테테르는 제1기구와 제2기구의 공축을 통과하는 것을 특징으로 하는 기구 장치.
