Abstract
Description
このような問題を解決するために、癌細胞の分裂と正常細胞の分裂がどの様に異なるのかを理解することは、興味深いことである。
細胞周期による細胞の増殖は、細胞周期を通じてその濃度が変動するサイクリンと呼ばれるタンパク質と、結合して活性型となるサイクリン依存性キナーゼ(以下「CDK」という。)との組合せによって形成されるホロ酵素により決定される。また、サイクリン/CDK複合体である該ホロ酵素は、タンパク質p21 WAF1/CIP1(p21)を含むサイクリン依存性キナーゼインヒビター(以下、「CDKI」という。)と呼ばれるタンパク質によって阻害される。
サイクリンE遺伝子、D遺伝子及びB1遺伝子のタンパク質並びに各サイクリン依存性キナーゼであるCDKについては、従来から種々の研究がなされている。サイクリン/CDK2複合体及びサイクリンD/CDK4又はサイクリンD/CDK6複合体は、G1期とDNA合成期であるS期の間の細胞周期チェックポイントを通じて細胞の進行を制御する。また、サイクリンB1/CDK1複合体は、有糸分裂直前の細胞周期チェックポイントを制御する。
上記サイクリン/CDK複合体に着目した癌の診断方法及び治療方法に関する発明が既にすでに開示されており(特表2002−504683号公表特許公報、特表2002−519681号公表特許公報)、細胞周期調節因子を、放射性同位元素(以下「RI」という。)を使用せずに測定する方法も開示されている(特開2002−335997号公開特許公報)。
細胞周期のうち主にG1期の構造が破壊されると、異常な細胞増殖が誘導される。細胞周期のG1期の制御において中心的役割を果たすタンパク質としてp53分子(腫瘍抑制分子)が知られている。様々な癌組織において、該p53遺伝子の突然変異が高い発生率で生じることは、既に報告されている。正常なp53分子は、転写因子として機能し、細胞増殖、DNA修復、分化、及びアポトーシスを制御する。また、DNAが損傷した非癌細胞において、p53分子はCDKI分子であるp21 cip1/kip1の発現を促進し、内在するCDK2キナーゼ活性を抑制し、細胞周期をG1/S境界であるR点で止める。一方、突然変異したp53分子は、細胞周期を調節することができず、制御不能な細胞の増殖及びゲノムの不安定性を導き、その結果細胞を悪性腫瘍に至らしめると考えられる。
一連の分子病理学的調査により、G1期におけるCDK制御分子の臨床的意義が明らかになっている。細胞周期調節タンパク質であるCDK2に対応するサイクリンE及びCDK4並びにCDK6に対応するサイクリンD1の過剰発現、及びCDKI分子、例えばCDK2に対応するp21、CDK2,4,6に対応するp27、及びCDK2に対応するp16の不活性化は種々の組織における腫瘍発生において重要な役割を果たしている。
しかし、抗体毎の特異性に依存する免疫組織学的な技術がいまだ確立されていないこと等から、これらのサイクリンE、サイクリンD1等の癌組識における発現率は、報告ごとに大きくばらついている。
本発明者らは、各種細胞周期調節因子の存在と癌細胞の状態に関連があることに着目し、鋭意研究を重ねた結果、細胞周期関連タンパク質の発現及び活性を測定し、細胞周期プロファイルを分析することにより、より正確な癌の診断が判断可能となることを見出し、本発明を完成した。
すなわち本発明は、
1.細胞周期関連タンパク質を少なくとも2種以上測定し、細胞周期プロファイリングを行うことを特徴とする組織及び/又は細胞の検査方法、
2.細胞周期プロファイリングが、1つの細胞周期関連タンパク質の測定値を基準とした他方の相対測定値を解析することにより、組織及び/又は細胞の情報を得ることである前項1に記載の検査方法、
3.細胞周期関連タンパク質がサイクリン依存性キナーゼである前項1に記載の検査方法、
4.細胞周期関連タンパク質がCDK1,CDK2,CDK4及びCDK6のいずれかから選択される少なくとも2種以上である前項2に記載の検査方法、
5.基準とする細胞周期関連タンパク質測定項目の1種がCDK1活性値である前項2に記載の検査方法、
6.細胞周期関連タンパク質の測定が、サイクリンの発現測定を含む前項1に記載の検査方法、
7.細胞周期関連タンパク質が、サイクリン依存性キナーゼインヒビターである前項1に記載の検査方法。
8.前項1に記載の組織及び/又は細胞の検査方法を行うことによる癌検査方法、
9.前項1に記載の検査方法のための検査システム、よりなる。

また、本発明において、細胞周期プロファイリングとは、細胞周期関連タンパク質の発現又は活性を最低2個以上、好ましくは3個以上同時測定することをいい、好ましくは1つの細胞周期関連タンパク質の測定値を基準とした他方の相対測定値を解析することにより、測定対象とする組織及び細胞の状態や性質に関する情報(プロファイル)を得ることをいう。測定項目は、例えばCDK1活性、CDK2活性、CDK4活性及びCDK6活性などのCDK活性、CDK1タンパク質発現、CDK2タンパク質発現、CDK4タンパク質発現及びCDK6タンパク質発現などのCDK発現、cyclin Bタンパク質発現、cyclin Dタンパク質発現及びcyclin Eタンパク質発現などのサイクリン発現、p16タンパク質発現、p21タンパク質発現及びp27タンパク質発現などのCDKI発現、p53タンパク質発現、Rbタンパク質発現などである。
また、相対値を得る為の基準値としては、例えばCDK1活性値、CDK1タンパク質発現値が考えられるが、上記項目以外の測定値を用いても良い。
(細胞周期関連タンパク質の測定)
本発明の検査方法において行う細胞周期関連タンパク質の測定は、公知の又は今後開発されるあらゆる測定方法を採用することができる。例えば、CDK酵素活性は従来の放射性同位元素(RI)を用いて測定することもできるし、RIを使用せずに測定することもできる(特開2002−335997号公開特許公報)。臨床検査の現場において、複数の患者等からバイオプシーや外科切除等により採取した組織を簡便迅速に測定するには、RIを使用しない方法により測定することが好ましい。
また、タンパク質発現量の測定も、公知又は今後開発されるあらゆる測定方法を採用することができる。例えば、ドットブロット技術に基づいたタンパク質発現分析によるシンプルな定量分析システムにより測定することができる。
(細胞周期プロファイリングのための試料の調製)
本発明の検査方法を行うために、測定用試料を調製する必要がある。具体的な細胞の可溶化方法、CDKを含む試料の単離方法は、特開2002−335997号公開特許公報に記載されている方法を採用することができる。
(細胞周期プロファイリング分析)
正常細胞において、細胞の増殖は、G1期において調節因子により厳格に制御されている。一方、制限ポイント(R)を通過した後の細胞周期の進展は、G1期に比較して制御が緩やかである。それゆえにG1期CDK(CDK2,4,6)活性プロファイリングを、G2/Mキナーゼ又はCDK1活性値を基準として行うことは妥当である。
癌細胞における異常増殖の原因は、細胞周期関連タンパク質の発現によっては単純に分析することができない。つまり、これらの分子の不活性化又は活性化の原因として、タンパク質の発現量の増減に加え、分子の突然変異等が報告されている。上記、様々な原因を一義的なアッセイシステムで測定することは困難である。そこで、細胞周期関連蛋白質の多くがその調節にかかわっているCDK活性を測定することは、癌診断に有用であると考えた。
表1に、後述する実施例で測定した胃腸癌を保有する患者の臨床病理学的特徴を示した。研究集団のメジアン年齢は65歳(23歳〜86歳の範囲)であった。大腸癌22例中の1例、胃癌8例中の5例、食道癌7例中の3例が臨床病期第IV期であった。
表2及び表3に癌大腸癌患者、表4及び表5に胃癌患者、並びに表6及び表7に食道癌患者由来の癌組織及び正常粘膜組織における4種類のCDKのキナーゼ活性及び9種類の細胞周期関連蛋白質の発現結果を示した。また、表8に癌組織及び正常粘膜組織での各CDK分析結果を示した。
ここで、表2乃至表7において平均値+2SD(standard deviation:標準偏差)を超える測定値には(+)、平均値−2SD未満の測定値には(−)を付している。なお、測定は、後述する実施例の方法により行った。
CDKの発現は、CDK1に関し、大腸、胃及び食道の3種の癌組織で、正常粘膜に比較し高値を示した。総タンパク質あたりのCDK1発現量のカットオフ値を、相当する正常組織における平均値+2SDとして決定したところ、大腸では1.99ng/μg、胃では1.60ng/μg、食道では0.91ng/μgであった。
このカットオフ値を用いて解析したところ、大腸癌組織の27%(6/22)、胃癌組織の75%(6/8)、食道癌組織の57%(4/7)について、CDK1の発現が陽性と判断された。これらの相違はすべて統計上有意であった(大腸;P<0.05,胃;P<0.05,食道;P<0.05、Wilcoxon signed−rank試験)。
これとは対照的に、CDK2,4,6の発現に関して、癌組織と正常組織間に統計的有意差は認められなかった。
CDK活性のうち、CDK2については、癌組織と正常組織間で統計上著しい相違が認められた(大腸;P<0.01、胃;P<0.01、食道;P<0.01、Wilcoxon signed−ranks試験)。
各組織について、タンパク質重量あたりのCDK2活性のカットオフ値を、正常組織で示されるCDK2活性の平均値+2SDとすると、大腸;0.054U/μg、胃;0.102U/μg、食道;0.033U/μgであった。これにより、大腸癌組織の72%(16/22)、胃癌組織の75%(6/8)、及び食道癌組織の100%(7/7)が陽性と判断された。例えば乳癌については、過剰な活性を有するサイクリンEの異性体(isoform)の発現が、患者生存率と強力に相関することが報告されており、このことは、高いCDK2活性がゲノムの不安定性及び癌の悪性化の主な原因であることを示唆する。
CDK4活性及びCDK6活性については、正常粘膜と比較すると癌組織の方が低値であった(大腸;CDK4 P<0.05、大腸;CDK6 P<0.05、Wilcoxon signed−ranks試験)。
胃の組織では、他の臓器に比較して、CDK1及びCDK2の両方に高い発現と活性が見られた。これらの結果は、他の器官よりも胃の粘膜組織が活発に再生していることを、又は、日本人にしばしば観察されるHelicobacter Pyroli感染による肥厚増殖を示唆する。
本発明の技術的範囲は、組織検査方法、細胞検査方法に使用する試薬、検査システム、本検査方法による癌の検査方法並びにプロファイルを分析することにより選択された癌の治療薬、治療方法にまで及ぶものである。
以下に実施例を示して本発明を具体的に説明するが、本発明は、これらに限定されるものではない。
(実施例1)細胞検査方法
1)測定用組織試料の調製
外科的に採取した各組織(2mm3)を、ノニデットP−40(NP−40)(カルビオケム社製)を含む溶解緩衝液とともにすりつぶし、ホモジナイザーを用いて可溶化した。不溶性物質は、フィルターにより除去した。得られた細胞可溶化液を測定用組織試料とした。
2)CDK活性測定
総タンパク質量100μgを含む試料を、2μgの抗体(抗−CDK1,2,4及び6抗体、サンタ−クルーズ製)及び20μlのプロテインAビーズ(バイオラッド社製)に加えて4℃で1時間反応させ、CDK分子を沈降させた。
緩衝液(0.1%NP−40,50mM Tris−HCl,pH7.0)で3回洗浄したのち、タンパク質を含む基質混合物を50μl加え、37℃で10分間振盪してインキュベートした。
基質混合物は、CDK1及びCDK2に対応する10μgのヒストンH1(アップステイトバイオテクノロジー製)、CDK4及び5に対応する10μgの遺伝子組換えRbタンパク質のC末端ドメイン(a.a.769−921)、5mMのアデノシン5’−O−(3−チオ3リン酸)(ATP−YS、シグマ社製、USA)、及び緩衝液(20mM Tris−HCl,pH7.4,0.1% Triton X−100)である。
ビーズの除去後、基質中に誘導されたモノチオリン酸は、10mMのヨードアセチル−ビオチン(Pierce製、USA)でさらにラベルされた基質とともにカップリング緩衝液(100mM Tris−HCl,pH8.5,1mM EDTA)中で暗所において90分間室温にてインキュベートした後、反応をβ−メルカプトエタノールで停止した。
反応後の基質0.4μgを、スロットブロッターを用いてPVDF膜上に添加し、吸引した。得られた膜を4%のウシ血清アルブミン(BSA)で30分間ブロックし、アビジン−FITC(ベクター製)を37℃で1時間反応させた。
膜を洗浄した後、膜上のイメージを蛍光イメージアナライザー(バイオラッド社製)により分析した。活性は、K−562慢性骨髄性白血病細胞株に含まれる0,12.5,25,50,100,150μgのCDKに対応する標準曲線に基づいて活性を算出した。
なお、1Uは、K−562細胞の総タンパク質1μgのときの酵素活性と同等のものをいう。
3)タンパク質発現測定
外科的に採取した組織(2mm3)を、ノニデットP−40(NP−40)を含む溶解緩衝液とともにすりつぶし、ホモジナイザーを用いて可溶化した。
不溶性物質は、フィルターにより除去した。そして、合計2.5μgのタンパク質を、疎水性膜(PVDF,ミリポア社製)をセットしたスロットブロッターのウェル(2×2×3mm、許容量:50μl)に加えた。膜に結合した粗製試料中の標的物質は、次の反応である抗細胞周期関連タンパク質抗体、ビオチン化した2次抗体及び蛍光ラベルしたストレプトアビジンの反応により定量的に検出した。
各反応の間に、ウェルはTBS(25mM Tris−HCl、pH7.4、150mM NaCl)により自動的に洗浄された。
膜上の蛍光イメージをイメージアナライザー(バイオラッド社製)によって分析し、ドットの蛍光強度を測定した。細胞周期関連タンパク質の定量は、標準遺伝子組換えタンパク質を用いた標準曲線により行った(CDK1;2.5−25ng/dot,CDK2;1.0−10ng/dot,CDK4;1.0−10ng/dot,CDK6;2.5−25mg/dot)。
(実施例2)CDK活性について
胃腸癌組織及び正常粘膜について、CDK1、2、4及び6の各酵素活性を測定した。活性の測定は、実施例1に記載の方法を用いて、大腸癌(A)22症例、胃癌(B)8症例、及び食道癌(C)7症例及び正常粘膜の各組織について行った。
その結果を第1図A〜Cに示した。その結果、癌組織と正常な粘膜において、各酵素活性の相違は、大腸、胃及び食道組織のCDK2活性の上昇において、統計的有意性が認められた(P<0.01)。一方、大腸及び胃癌組織のCDK4及びCDK6は、正常な粘膜よりも比較的低い活性を示した。統計的分析は、Wilcoxon signed−ranks試験により行った。CDK2酵素活性の増加は、ほとんどの大腸、胃、食道癌組織において観察された。
(実施例3)CDK活性プロファイルについて
G1期のCDK(CDK2、4、6)について、G2/MキナーゼであるCDK1活性値を基準にしてCDK2、4、6の活性値を標準化し、プロファイルを調べた。その結果、図2A実線で示すように、正常大腸粘膜組織8例中7例について、G1期のCDKプロファイルは、CDK2/CDK1では0.024〜0.43、CDK4/CDK1では1.2〜39、CDK6/CDK1では1.9〜26のフォールド幅に位置しており、共通していた。これは、正常大腸組織の間においてCDK活性の相対値が類似していることを示すものである。
次に、大腸癌組織9例の症例から無作為に選ばれものについて、同様にG1期のCDK(CDK2、4、6)について、G2/MキナーゼであるCDK1活性値を基準にしたプロファイルを調べた。その結果、第2図Bに示すように、正常粘膜組織に見られた共通プロファイルと一致したプロファイルは観察されなかった。
本発明の方法により、活性測定を含む細胞周期プロファイリングを行うと、より正確な癌組織及び癌細胞の検査が可能であることが見出された。各細胞周期調節因子の発現、活性、及びプロファイリング等の分析は、レトロスペクティブ又はプロスペクティブ試験において、予後因子としての価値が見出される可能性が高いと予想される。
また、活性測定を含む細胞周期プロファイリングを行う本発明の方法は、テーラーメイドの医療に適応しうる非常に貴重なものとなるであろう。なぜならば、乳癌については、CDK2に過剰な活性を与えるサイクリンEの異性体の発現が、患者生存率と高度に相関することが報告されているからである。
1.患者から採取した細胞の2種以上の細胞周期関連タンパク質の活性値又は発現量を測定し、得られた測定値に基づいて細胞周期プロファイリングを行い、このプロファイリングの結果に基づいて、患者から採取した細胞が癌細胞か否か判定することを特徴とする細胞の検査方法、
2.細胞周期プロファイリングが、1つの細胞周期関連タンパク質の測定値を基準とした他方の相対測定値の解析を含む請求項1に記載の細胞の検査方法、
3.細胞周期プロファイリングが、得られた測定値のプロファイルと正常細胞のプロファイルとの比較を含む請求項1又は2に記載の細胞の検査方法、
4.細胞周期関連タンパク質がサイクリン依存性キナーゼである請求項1乃至3のいずれか1項に記載の細胞の検査方法、
5.細胞周期関連タンパク質が、CDK1, CDK2, CDK4及びCDK6のいずれかから選択される少なくとも2種である請求項1乃至4のいずれか1項に記載の細胞の検査方法、
6.基準とする細胞周期関連タンパク質測定値がCDK1活性値である請求項2に記載の細胞の検査方法、
7.細胞周期関連タンパク質がサイクリンである請求項1に記載の細胞の検査方法。
8.細胞周期関連タンパク質がサイクリン依存性キナーゼインヒビターである請求項1に記載の細胞の検査方法、
9.請求項1乃至8のいずれか1項に記載の細胞の検査方法のための検査システム、よりなる。
本発明の検査方法において行う細胞周期関連タンパク質の測定は、公知の又は今後開発されるあらゆる測定方法を採用することができる。例えば、CDK酵素活性は従来の放射性同位元素(RI)を用いて測定することもできるし、RIを使用せずに測定することもできる(特許文献3)。臨床検査の現場において、複数の患者等からバイオプシーや外科切除等により採取した組織を簡便迅速に測定するには、RIを使用しない方法により測定することが好ましい。
本発明の検査方法を行うために、測定用試料を調製する必要がある。具体的な細胞の可溶化方法、CDKを含む試料の単離方法は、特開2002−335997号公開特許公報に記載されている方法を採用することができる。

総タンパク質量100μgを含む試料を、2μgの抗体(抗-CDK1, 2, 4及び6抗体、サンタ-クルーズ製)及び20μlのプロテインAビーズ(バイオラッド社製)に加えて4℃で1時間反応させ、CDK分子を沈降させた。
外科的に採取した組織(2mm3)を、ノニデットP-40(NP-40)を含む溶解緩衝液とともにすりつぶし、ホモジナイザーを用いて可溶化した。不溶性物質は、フィルターにより除去した。そして、合計2.5μgのタンパク質を、疎水性膜(PVDF,ミリポア社製)をセットしたスロットブロッターのウェル(2×2×3mm、許容量:50μl)に加えた。膜に結合した粗製試料中の標的物質は、次の反応である抗細胞周期関連タンパク質抗体、ビオチン化した2次抗体及び蛍光ラベルしたストレプトアビジンの反応により定量的に検出した。
胃腸癌組織及び正常粘膜について、CDK1、2、4及び6の各酵素活性を測定した。活性の測定は、実施例1に記載の方法を用いて、大腸癌(A)22症例、胃癌(B)8症例、及び食道癌(C)7症例及び正常粘膜の各組織について行った。
G1期のCDK(CDK2、4、6)について、G2/MキナーゼであるCDK1活性値を基準にしてCDK2、4、6の活性値を標準化し、プロファイルを調べた。その結果、図2Aの実線で示すように、正常大腸粘膜組織8例中7例について、G1期のCDKプロファイルは、CDK2/CDK1では0.024〜0.43、CDK4/CDK1では1.2〜39、CDK6/CDK1では1.9〜26のフォールド幅に位置しており、共通していた。これは、正常大腸組織の間においてCDK活性の相対値が類似していることを示すものである。
Claims (9)
- 細胞周期関連タンパク質を少なくとも2種以上測定し、細胞周期プロファイリングを行うことを特徴とする組織及び/又は細胞の検査方法。
- 細胞周期プロファイリングが、1つの細胞周期関連タンパク質の測定値を基準とした他方の相対測定値を解析することにより、組織及び/又は細胞の情報を得ることである請求の範囲1に記載の検査方法。
- 細胞周期関連タンパク質がサイクリン依存性キナーゼである請求の範囲1に記載の検査方法。
- 細胞周期関連タンパク質がCDK1,CDK2,CDK4及びCDK6のいずれかから選択される少なくとも2種以上である請求の範囲2に記載の検査方法。
- 基準とする細胞周期関連タンパク質測定項目の1種がCDK1活性値である請求の範囲2に記載の検査方法。
- 細胞周期関連タンパク質の測定が、サイクリンの発現測定を含む請求の範囲1に記載の検査方法。
- 細胞周期関連タンパク質が、サイクリン依存性キナーゼインヒビターである請求の範囲1に記載の検査方法。
- 請求の範囲1に記載の組織及び/又は細胞の検査方法を行うことによる癌検査方法。
