CONSTITUTION:Conductor electrodes 7a and 7b formed on nesa electrodes 6 are led out alternately to both sides of photoelectric conversion film 8 formed on contact fiber substrate 3 and are formed and arranged. Substrate 10 for fitting of light source 1 which consists of the same materials as substrate 3 is fixed at the approximately same height as substrate 3 at the tip part of substrare 3, and light source 1 is fixed to slope 10a of substrate 10. Further, plural terminals 12 for external connection are formed in the longitudinal direction of film 8 in prescribed positions on upper face 10b of substrate 10 by adhesion, and tape carrier 13 is arranged between these terminals 12 and conductor electrodes 7a by the bonding method, and integrated circuit element 14 for the driving circuit is mounted on carrier 13.
