Claims (1)
- 【特許請求の範囲】 1.患者の動脈系から心臓を隔絶しておく間に、患者の心臓の大動脈弁から離れ る方向に通じる大動脈通路を通って心臓にアクセスする大動脈用血管カテーテル であって、 a)患者の心臓の大動脈弁から離れる方向に通じる大動脈通路を通って前進 可能な細長いカテーテルシャフトを有し、該カテーテルシャフトは、患者の体外 に配置される近位側部分と、患者の上行大動脈及び左心室内に配置される遠位側 部分と、カテーテルシャフト内で遠位側部分のポートから近位側部分の一位置ま で延びている内部管孔とを備え、 b)カテーテルシャフトの遠位側部分のポートから近位側に間隔を隔てて遠 位側部分に設けられた第1膨張可能手段を有し、該手段は、拡大したときに上行 大動脈内の大動脈通路を閉塞する形状を有し、 c)カテーテルシャフトの遠位側部分のポートから近位側で且つ第1膨張可 能手段より遠位側の位置で、カテーテルシャフトの遠位側部分に設けられた第2 膨張可能手段を有し、該手段は、拡大したときに左心室内で着座してカテーテル を固定し、大動脈通路内でのカテーテルの移動を最小にする形状を有することを 特徴とする血管内大動脈内カテーテル。 2.第1膨張可能部材は膨張可能バルーンであり、該バルーンの内部は、第1膨 張可能部材の内部の一位置から、患者の体外に配置される近位側部分の一位置ま で延びている膨張用管孔と流体連通していることを特徴とする請求の範囲第1項 に記載のカテーテル装置。 3.第2膨張可能部材は膨張可能バルーンであり、該バルーンの内部は、第1膨 張可能部材の内部の一位置から、患者の体外に配置される近位側部分の一位置ま で延びている膨張用管孔と流体連通していることを特徴とする請求の範囲第1項 に記載のカテーテル装置。 4.カテーテルシャフトの第1膨張可能手段と第2膨張可能手段との間には第2 ポートが設けられており、流体供給管孔が、カテーテルシャフト内で、前記第2 ポートから、患者の体外に配置されるカテーテルシャフトの近位側部分内の 一位置まで延びていることを特徴とする請求の範囲第1項に記載のカテーテル装 置。 5.第1及び第2膨張可能部材は内部を備えた膨張可能バルーンであり、カテー テルシャフトは更に、第1及び第2膨張可能部材の内部と流体連通している膨張 用管孔を備えていることを特徴とする請求の範囲第1項に記載のカテーテル装置 。 6.膨張用管孔は、患者の体外に配置される近位側シャフトセクション内の一位 置まで延びていることを特徴とする請求の範囲第5項に記載のカテーテル装置。 7.患者の上行大動脈を通って心臓にアクセスする血管内カテーテル装置におい て、 a)細長いカテーテルシャフトを有し、該カテーテルシャフトは、この内部 で延びている少なくとも1つの内部管孔を備え且つ上行大動脈を通って患者の心 臓から離れる方向に通じる大動脈通路を通って前進できる寸法及び形状を有し、 b)カテーテルシャフトの遠位側部分に設けられた第1膨張可能手段を有し 、該手段は、大動脈通路内で拡大したときに大動脈通路を閉塞する形状を有し、 c)第1膨張可能手段より遠位側の位置でカテーテルシャフトの遠位側部分 に設けられた第2膨張可能手段を有し、該手段は、拡大したときに大動脈弁に隣 接する左心室内で着座して、大動脈通路内でのカテーテルの移動を最小にする形 状を有し、 d)第2膨張可能部材より遠位側の位置でカテーテルの遠位側部分に設けら れたポートを更に有し、該ポートが、カテーテルの外部の流体を内部管孔内に吸 引できる内部管孔と流体連通していることを特徴とする血管内カテーテル装置。 8.第1膨張可能部材は膨張可能バルーンであり、該バルーンの内部は、カテー テルシャフト内で延びている膨張用管孔と流体連通していることを特徴とする請 求の範囲第7項に記載のカテーテル装置。 9.第2膨張可能部材は膨張可能バルーンであり、該バルーンの内部は、カテー テルシャフト内で延びている膨張用管孔と流体連通していることを特徴とする 請求の範囲第7項に記載のカテーテル装置。 10.カテーテルシャフトの第1膨張可能手段と第2膨張可能手段との間には少な くとも第2ポートが設けられており、該第2ポートは、カテーテルシャフト内で 延びている第2内部管孔と流体連通しており且つ第1膨張可能部材と第2膨張可 能部材との間の大動脈通路内に流体を注入できることを特徴とする請求の範囲第 7項に記載のカテーテル装置。 11.第1膨張可能手段は膨張可能バルーンであり、該バルーンの内部はカテーテ ルシャフト内で延びている第1膨張用管孔と流体連通しており、第2膨張可能手 段は膨張可能バルーンであり、該バルーンの内部はカテーテルシャフト内で延び ている第2膨張用管孔と流体連通しており、第1及び第2膨張可能部材は独立し て膨張可能であることを特徴とする請求の範囲第7項に記載のカテーテル装置。 12.第1膨張可能手段は、第2膨張可能手段が患者の大動脈弁の上流側に隣接し て着座するときに、第1膨張可能手段が冠状動脈口と腕頭動脈との間に配置され るように選択された距離だけ第2膨張可能手段から離れていることを特徴とする 請求の範囲第7項に記載のカテーテル装置。 13.患者の解剖学的冠状構造にアクセスする方法において、 a)近位端及び遠位端と、遠位端のポートまで延びている内部管孔と、遠位 側部分に設けられた第1及び第2膨張可能部材とを有する細長いカテーテルを、 患者の上行大動脈及び左心室内の一位置へと前進させ、 b)冠状動脈口と腕頭動脈との間の上行大動脈内で第1膨張可能部材を拡大 して、上行大動脈を通る血液の流れを阻止し、 c)左心室内で第2膨張可能部材を拡大し、拡大された第2膨張可能部材を 左心室内で着座させることを特徴とする方法。 14.患者の心筋層を麻酔するステップを更に有することを特徴とする請求の範囲 第13項に記載の方法。 15.麻酔ステップは、少なくとも1つの冠状動脈を通る心臓麻酔液で患者の心筋 層を灌流するため、心臓麻酔液を、カテーテルの第1膨張可能部材と第2膨張可 能部材との間の少なくとも1つのポートを通して上行大動脈内に注入するこ とからなることを特徴とする請求の範囲第13項に記載の方法。 16.流体を、カテーテルの遠位端のポート及び内部管孔を通して患者の体外の位 置に抽出するステップを更に有することを特徴とする請求の範囲第13項に記載 の方法。 17.手術器具を、カテーテルの内部管孔を通して患者の左心室内の一位置に導く ステップを更に有することを特徴とする請求の範囲第13項に記載の方法。 18.患者に血管内心臓外科処置を行なう方法において、 a)近位端及び遠位端と、遠位端のポートまで延びている内部管孔と、遠位 側部分に設けられた第1及び第2膨張可能部材とを有する細長いカテーテルを、 患者の上行大動脈及び左心室内の一位置へと前進させ、 b)冠状動脈口と腕頭動脈との間の上行大動脈内で第1膨張可能部材を拡大 して、上行大動脈を通る血液の流れを阻止し、 c)左心室内で第2膨張可能部材を拡大し、拡大された第2膨張可能部材を 左心室内で着座させ、 d)心臓外科処置を行なう1つ以上の器具を、デリバリカテーテルの内部管 孔に通すことを特徴とする方法。 19.患者の心筋層を麻酔するステップを更に有することを特徴とする請求の範囲 第18項に記載の方法。 20.麻酔ステップは、少なくとも1つの冠状動脈を通して患者の心筋層を灌流す るため、心臓麻酔液を、カテーテルの第1膨張可能部材と第2膨張可能部材との 間の少なくとも1つのポートを通して上行大動脈内に注入することからなること を特徴とする請求の範囲第19項に記載の方法。 21.患者の心臓外科処置を行なう装置において、 a)患者の上行大動脈内の一位置に前進させることができる細長い大動脈閉 塞カテーテルを有し、該カテーテルは、近位端及び遠位端と、カテーテル内でカ テーテルの遠位端のポートまで延びている第1内部管孔と、カテーテルの遠位側 部分の第1膨張可能部材とを備え、該第1膨張可能部材は、上行大動脈内で拡大 したときに大動脈通路を閉塞し、第1膨張可能部材より先端側に配置された第2 膨張可能部材を更に備え、該第2膨張可能部材は、拡大したときに患 者の左心室内で着座し、大動脈通路内でのカテーテルの移動を最小にする形状を 有し、 b)患者の静脈系から血液を抽出する手段と、抽出された血液を酸素化する 手段と、酸素化血を動脈系に導く手段とを備えた心肺バイパス装置と、 c)患者の心筋層に心臓麻酔物質を供給する手段とを更に有することを特徴 とする装置。 22.細長い大動脈閉塞カテーテルの第1膨張可能部材は膨張可能部材であること を特徴とする請求の範囲第21項に記載の装置。 23.細長い大動脈閉塞カテーテルは、第1膨張可能部材の内部に膨張流体を導く ことができる第2内部管孔を有することを特徴とする請求の範囲第22項に記載 の装置。 24.第2膨張可能部材は膨張可能部材であることを特徴とする請求の範囲第21 項に記載の装置。 25.細長い大動脈閉塞カテーテルは、第2膨張可能部材の内部に膨張流体を導く ことができる第3内部管孔を有することを特徴とする請求の範囲第24項に記載 の装置。 26.細長い大動脈閉塞カテーテルは、左心室から第1内部管孔を通して血液を抽 出し且つ抽出した血液を患者の体外に導くことができることを特徴とする請求の 範囲第21項に記載の装置。 27.抽出した血液を患者の血管構造に戻す手段を有することを特徴とする請求の 範囲第26項に記載の装置。 28.心臓麻酔物質を供給する手段は、第1膨張可能部材と第2膨張可能部材との 間に、第2内部管孔と連通している少なくとも第2ポートを有し、該第2ポート は、第1膨張可能部材と第2膨張可能部材との間の上行大動脈内に心臓麻酔液を 灌流させることができることを特徴とする請求の範囲第21項に記載の装置。
