クレアチン供給デボ−の増加法
発明の説明
本発明は、概して、クレアチン代謝に障害を持たない哺乳動物における筋肉運動
能力を増加させ、それによって筋肉強度を増加させ、労作後のエネルギーにおけ
るリン化合物の再確保時間を短縮し、そして筋肉本体を増加させるための製剤に
関する。これは、該哺乳動物に対してクレアチンを1日に少なくとも15グラム
、すなわち0.2〜14g/kg(体重)、好ましくは約0.3g/kg(体重
)の量で少な(とも2日間投与することによって達成される。本発明は、前述の
日用量を供給する量のクレアチンを含む製剤の製造のためのクレアチンの使用、
並びにクレアチン代謝に障害を持たない哺乳動物において、場合により数回の用
量に分割された少なくとも15グラム、すなわち0.3g/kg (体重)の日
用量を供給することによって筋肉運動能力を増加させる方法を記載する。
クレアチンリン酸は、ATP (アデノシン三リン酸)を最も速く再合成する筋
組織中の基質であり、このために最大または近最大の能力生産に用いられている
ということは十分に確証されている。ATPの再合成速度は、グリコーゲン(筋
肉中炭水化物)からよりもクレアチンリン酸からの方がほぼ2倍高く、本発明者
は、最大能力による筋収縮がクレアチンリン酸供給を空にすることを示すことが
できた(ハルトマン(Hu I t ma n)ら、Biochem、5ocT
rans 1991:19.347〜354)、労作後のエネルギー基質の酸化
的再合成でさえも、筋組織中の増加したクレアチン量に積極的に影響される。
したがって、決定的な関係は、筋肉中のクレアチン量と、休止のための短い中断
を伴う反復労作での力の生産との間に明確な関係が存在すると示唆されてきた。
クレアチンは筋組織中で合成されないが、血流によって、一部分は肝臓、腎臓お
よびすい臓での合成から並びに一部分は食物による摂取から筋肉に対して供給さ
れる。クレアチンの血中1度は、血液1リットル当り50gモル程度であり、こ
の1度から、筋組織に対するクレアチン取込みが能動輸送によって引き起こされ
る。クレアチニンの形でのクレアチン排出は、筋肉質量の大きさによって変化し
、普通の体重のヒトでは0. 2〜2g/日に達する。
筋肉能力を向上させる試みが、筋肉中のクレアチン含量を増加させることによっ
て行なわれてきた。クレアチンは、種々の形でおよび他の基質、例えば、ローヤ
ルゼリー、カルノシン、ビタミンおよびアミノ酸化合物と一緒に投与されてきた
。
更に、クレアチンリン酸が投与されてきた。これらの試みにおいて、100mg
から最大数グラムまでに相当する日用量が投与されてきた。このクレアチン投与
法は、クレアチンの血中濃度を穏やかに増加させるが、(実験自体の)筋肉中ク
レアチン含量の増加は測定されていない。
欧州特許第199117号明細書は、心筋梗塞の治療用のおよび心臓外科手術中
に心筋(心筋層)を防護するための非経口用製剤中の活性成分としてのホスホク
レアチンを開示している。該製剤は、3〜35ミリモルのホスホクレアチン二ナ
トリウムを含むと記述されている。実施例3では、1日目にホスホクレアチン6
gを使用した後、1〜7日目の間に2g/日で治療することが開示されている。
欧州特許第222257号明細書は、心臓疾患に対する療法において200〜4
00mg/kg/日の量で用いるためのホスホクレアチン含有組成物を開示して
いる。

筋中クレアチン含量を達成するためのクレアチン前駆体としてのグアニジノ酢酸
の使用に関する。しかしながら、出願人は、第2頁14行〜18行において、[
外因性クレアチンの投与は、投与されたクレアチンの量と等しい量の内因性クレ
アチンの合成を外因性クレアチンが阻害するので決定的な結果を全くもたらさな
い」と記述している。
米国特許第5091404号明細書により、2g/70kg(体重)〜6g/7
0kg (体重)より大、好ましくは8〜12g/70kg (体重)の投与量
の注射または注入によるシクロクレアチンの投与を含む、虚血しているインビボ
被験動物筋組織の生理学的機能を保存するおよび/または修復するための方法が
従来知られている。
EP A2第0449787号明細書は、カルノシンまたはそれに関連したべプ
チドを活性成分として含む薬剤、治療食用または獣医学的組成物に関する。有益
な効果の例としては、運動競技能力、長期間労働した人の向上並びに年配の若し
くは衰弱した被験者および子供等の筋肉機能の増進がある。任意の活性成分は、
適当な供給を与えるように存在しつる。例えば、クレアチンは0.5〜10g/
日の投与量で投与しつる。製剤の例としては、場合により発泡性でありうるし且
つ使用前に水または他の液体中に溶解しつる散剤または顆粒剤を含む1回用量す
ッ/工;錠剤、軟質および硬質カプセル剤;シロップ剤;キャンデー等がある。
Cardiology、80巻(3〜4)、184〜95 (M、S、オフスパ
ッケン(Obsbakken)ら)は、クレアチンではないが、シクロクレアチ
ンによる予備処置が、虚血中かおよび/または後に心筋を防護することを示した
データを示している。更に、シクロクレアチンによる処置は、クレアチンによる
処置よりも長い虚血エピソード後に機械的機能を復帰させるということに影響し
ていると記述されている。これらはラットでの実験結果である。

0〜200は、クレアチンリン酸が虚血性心臓病に対して防護効果を有すること
を示している。前記論文の197頁において、示されてはいないが、従来述べら
れたことに反してホスホクレアチンは細胞膜に浸透することができ、そして更に
、遊離クレアチンは防護効果を与えないということが記述されている。
したがって、従来、主として虚血性組織を防護する治療状況において、ホスホク
レアチン、クレアチン類似体であるシクロクレアチンおよび種々のクレアチン前
駆体を用いることが知られている。
ホスホクレアチンは完全な細胞膜を通過することができないので、いずれの治療
状況においてもそれはほとんど用いることができない。虚血性組織に対するホス
ホクレアチンの積極的な効果は、ホスホクレアチンが損傷組織によって取込まれ
たことに依ることがある。
シクロクレアチンは、ヒトの体内においては天然に存在しない合成物質である。
それは実験動物中においてのみ用いられてきた。クレアチンキナーゼ作用の最大
活性は、基質としてのホスホクレアチンと比較して、リン酸化シクロクレアチン
を用いると350倍低い。該物質はヒト被験者に用いられていないし、しかも中
枢神経系に対する作用はまだ知られていない。
クレアチン前駆体を添加後のヒト筋肉中の増加したクレアチン含量を示す実験結
果は得られていない。増加した前駆体含量の効果は、肝臓および腎臓中のメチル
化酵素の活性に依るものである。
いくつかの公開論文が、クレアチンは改善された医学的結果を全くもたらさない
と記述している。しかしながら、これは、供給される量が低すぎることに関係し
ていると考えられる。
したがって、クレアチン代謝に障害を持たない哺乳動物に対して経口、腸溶性ま
たは非経口投与された少なくとも15gの日用量のクレアチンすなわち0.2〜
0.4g/kg (体重)、好ましくは釣13g/kg(体重)の供給は、示強
活動中に筋中ホスホリルクレアチン貯蔵の減少作用を防止することによって筋肉
の能力を向上させるために用いて筋肉疲労を防止し且つ回復期間を短縮すること
ができるし、または心臓外科手術に関係した予備処置のために、アンギナ患者の
処置、呼吸機能不全、低下した肺機能、気腫に対して、酸素療法の必要な患者に
対して、人口呼吸で処置された患者に対して、術後および一般的な栄養失調のた
めに、線維筋痛のために並びに種々の型の筋障害の患者に対して、解糖またはミ
トコンドリアによるエネルギー生産能力が制限された筋組織において直ちに利用
可能なエネルギーデポ−を増加させるように用いることができるという知見を当
業者にもたらした上述の先行文献においては何も開示されないしまたは示唆され
ない。サンステッド(Sands t ed t)ら、C11nicalNut
rition、10巻、1991.97〜104頁によると、特に101頁を参
照されたいが、ホスホクレアチン濃度は、概して、損傷されたかまたは外科手術
を施された後の筋組織中において減少する。
したがって、上記で確認された機能不全に苦しむ哺乳動物または正確で且つ有効
な機能の供給を必要とする何等かの理由を有するその筋組織に対して副作用を伴
うことな(与えることができる安全で且つ簡単な製剤が要求されてきた。
クレアチンホスファゲン系が、筋肉および他の組織におけるエネルギー供給に中
心的な役割を果たしていることは知られているが、その取込みおよび全クレアチ
ンブールの調節に関しては、比較的僅かしか知られていない。
クレアチン補足効果についてのユニの報告はあったが、全クレアチンの組織中含
量の直接測定は行なわれていない。
更に、示強運動中の筋中ホスホリルークレアチン貯蔵の減少は、一般的に、筋肉
疲労の開始と関係があることが知られている(ハルトマン(Hu l tman
)ら;5cand J、Cl1n、Lab、Invest、1967.19.5
6本発明の目的は、クレアチン代謝に障害を持たない哺乳動物に対して副作用を
伴うことなく与えることができる、安価で、簡単で且つ安全な製剤を提供するこ
とである。前記製剤は、上記で確認された障害に関して用いることができるし、
更に、示強活動中に筋中ホスホリルークレアチン貯蔵の減少作用を防止し、それ
によって筋肉の能力を向上させ、そして更に回復期間を短縮するのに用いること
ができる。
本発明のもう一つの目的は、筋肉運動能力を増加させることによってエネルギー
量および労作能力を向上させるような用量を哺乳動物に対して供給する製剤の製
造のためにクレアチンを用いること、並びにこれらの作用をもたらす方法である
。
これらの目的は、エネルギーに富む化合物が制限されている状態、例えば、術後
疲労、呼吸機能不全および/または心不全において重要である。
前記目的は、特許請求の範囲に記載の特徴によって達成される。
1日に少なくとも15グラム、すなわち0. 2〜0.4g/kg (体重)、
好ましくは約0.3g/kg (体重)のクレアチンを少な(とも2日間にわた
って腸溶性または非経口投与することにより、副作用をもたらすかもしれない他
の活性成分を全く加えることなく、最大クレアチン濃度並びに意外にも高い血漿
濃度が得られる。
クレアチン約0.3g/kg (体重)の量に基く投与は少なくとも6日間にわ
たって行なうことができるが、しかしながら、その量は70kgの被験者におい
ては15g/日以上であるべきである。
クレアチン代謝に障害を持たない哺乳動物に対するクレアチン15〜30グラム
/日の少なくとも2日間にわたる補足により、血漿の中クレアチン濃度の増加を
伴うことな(、筋肉中の全クレアチンブールの予期しない増加が得られた。
クレアチンの毎日の供給は、数回の分割用量で与えられるのが好ましい。
好ましくは、クレアチンは、1日に15〜30グラム、すなわち0.2〜0゜4
g/kg(体1)、好ましくは約0.3g/kg (体重)の量で4〜7日間に
わたって供給され、しかも腸溶性または非経口投与に適した任意の剤形で与えら
れることができる。
非経口投与用に、クレアチンは、100m1溶液当り2〜4gの濃度で与えられ
るのが好ましい。
更に、クレアチンは、慣用的な栄養素、例えば、脂質、炭水化物、アミノ酸、電
解質、微量元素およびビタミンと一緒に供給されてよい。
発明の好ましい実施態様
クレアチン5gの反復用量を70kgの被験者に対して毎日与えた場合、血中濃
度は血液1リットル当り800〜1200μモル、すなわち正常濃度の10〜2
0倍で維持されることができる。次に、筋組織中のクレアチンの急速な取込みは
基礎1度を越えて生じる。クレアチン20〜30g/日の3〜4日間の供給は、
供給前の正常クレアチン1度の120〜150%のクレアチン濃度、すなわち2
0〜50%のa度の増加を生じた。針生検技術によって得られた筋組織中のクレ
アチン濃度の直接測定に助けられて実施されたこれらの研究は、ヒトに対するク
レアチン供給に関して実施されてきた唯一の測定である。その研究は、筋肉中の
増加したクレアチン濃度の効果は、2種類の系列で実験された。「ダブルブライ
ンド」実験において、プラシーポかまたはクレアチンが供給された1週間の期間
の前後に5回のイソキネテインク最大運動を行なった。ブラシーボ群では、5回
の収縮系中の能力生産は未変化であったが、クレアチンが供給さた群においては
意外に高い能力が生産された。クレアチンおよびブラシーボ供給はそれぞれコー
ド化され、したがって、被験者も試験監視者も、実験終了後まで供給の性状につ
いての情報を与えられなかった。その結果は、Cl1nical 5cienc
e 1993.84号、556〜571に公開1000メ一トル走者の二つのグ
ループを含む更に別の実験を行なった。更に、この研究は、ブラシーポ群と、上
記と同用量(20〜30g/日で1週間)のクレアチンを与えられた群とから成
った。結果は同一であった。すなわち、4x1000メートルの競争時間は、ク
レアチンを供給された群で有意に向上したが、プラシーボ群でのそれは変化がな
かった。更に、クレアチン群においては体重が1.85±0.5kg増加したが
、ブラシーボ群では変化がなかったことが確認された。高クレアチン供給によっ
て他の群でも観察された体重増加は、クレアチン含量が高い筋系での増加したタ
ンパク質合成として説明される。
試験用にクレアチンHIO(Cr、HtO)5gを、クレアチニンの生成が検出
されない温〜熱水300m1中に溶解させた。1日4回の投薬回数を二人の被験
者に与え、そして他の者には1日6回まで増加させた。
図1は、200m1の温水中に溶解したCr、HtO5グラムを0時間口に投与
された1回用量後の3人の被験者の血漿中C「濃度を図示する。被験者の年齢は
28才(PG)〜62才(EH)であり、体重は76kg (RH) 、83k
g (PG)および87kg(EH)であった。
図2は、Cr、HtOを補足前(◆)および後(■、・)の大腿口頭筋の全りレ
アチンTCr含量を図示する。
4人5gを41/2日間(EHおよびRHI) 、7日間(ISおよびSL)お
よび10日間(KS)。
6人5gを7日間(Al、ESおよびJV、3日目、5日目および7日目に生検
を行なう)および1日おきに21日間(HHSHL、JSおよび00)。
更に、側副脚で1日1時間の激しい運動を行なった5人の被験者の対照脚からの
結果を含む。この場合の補足量は、
4人5gを31/2日間(RH,)
6人5gを4日間(AT、MLおよびNC,2日目および4日目に生検を行なう
)および7日間(SK)
であった。被験者を初期TCr含量の増加順に配列した。グラフ上の数字は、生
検の時点での補足の日数を示す。

料中のTCr対ATP含量の比率を図示する。与えられた用量の詳細は、図2に
対する説明で理解しつる。

図4は、大腿口頭筋のTCr含量に対する運動およびC「補足の効果を図示する
。補足期間中に、被験者は、一方の脚のみを用いて自転車エルゴメータ−で1時
間の激しい運動を行なった(労作脚=WL)。この時間中、対照脚は休止してい
た(体止脚=RL)。それ以外の時間、被験者は通常の日常活動を行なった。
用いられたCr、H,Oの投与量は、

5x5gを4日間(AT、MLおよびNC,2日目および4日目に生検を行なう
)および7日間(SK)
であった。得られた生検数を最小限にするために、補足前に1回だけ採取した。
これは体止脚からであって、側副脚の前補足TCr含量も示すと仮定される。被
験者を、初期TCr含量の増加順に配列した。被験者は全て男性であった。グラ
フ上の数字は、生検の時点での補足の日数を示す。

図5は、1日に更に1時間の激しい運動を行なった被験者において、Cr、H,

Claims (18)
- 1.クレアチン代謝に障害を持たない哺乳動物の筋肉運動能力を増加させるため の製剤であって、経口、腸溶性または非経口投与された場合に、少なくとも15 gすなわち0.2〜0.4g/kg(体重)、好ましくは約0.3g/kg(体 重)の日用量のクレアチンを該哺乳動物に供給することを特徴とする上記製剤。
- 2.15〜30gの日用量のクレアチンを哺乳動物に供給することを特徴とする 請求項1に記載の製剤。
- 3.慣用的な栄養素を更に含むことを特徴とする請求項1および2に記載の製剤 。
- 4.慣用的な栄養素が、脂質、炭水化物、アミノ酸、電解質、微量元素およびビ タミンから選択されることを特徴とする請求項1〜3に記載の製剤。
- 5.グルコースを含むことを特徴とする請求項3〜4に記載の製剤。
- 6.非経口用製剤であることを特徴とする請求項1〜5のいずれかに記載の製剤 。
- 7.100ml溶液当り2〜4gのクレアチンを含む溶液であることを特徴とす る請求項6に記載の製剤。
- 8.クレアチン代謝に障害を持たない哺乳動物の筋肉運動能力を増加させる製剤 の製造のためのクレアチンの使用であって、該製剤が、少なくとも15g/日す なわち0.2〜0.4g/kg(体重)、好ましくは約0.3g/kg(体重) の供給のためのクレアチンを含有する上記使用。
- 9.15〜30gの日用量のクレアチンを供給する製剤を製造するための請求項 8に記載の使用。
- 10.100ml溶液当り2〜4gのクレアチンを含む溶液の形の非経口用製剤 を製造するための請求項8および9に記載の使用。
- 11.少なくとも2日間および多くとも7日間の製剤の請求項8〜10のいずれ かに記載の使用。
