Abstract

背景技术
目前,无论在欧美国家还是在我国,心血管疾病的发病率和死亡率己远远高于包括癌症在内的其他疾病,成为当今世界上威胁人类最严重的疾病之一。据卫生部公布的医学统计资料,我国心血管疾病在上个世纪50年代末发病率仅为5.1%,到90年代初己经增至13.6%,到2002时18岁以上人群为18.8%,而50-70岁的中老年人中患有高血压、血脂异常、糖尿病、肥胖症等心血管疾病的“危险状态人群”竟高达70%。一项关于我国人口死亡原因的调査分析资料显示,平均每3个人中,就有1个是死于心血管疾病。心血管疾病已成为我国人口的第一死因,成为危害国人身体健康的第一杀手。
心脏作为身体的供血器官,是一个高活动、高能量消耗的器官,无论心肌舒张或收缩都需要充足的能量供应。心肌能量代谢障碍是众多心血管疾病的共同特征。心脏疾病与能量代谢关系密切。
正常情况下,心脏通过代谢葡萄糖、乳酸、酮体和氨基酸获得大约40%的能量,剩下的60%来自脂肪酸代谢。这些物质以腺嘌呤核苷三磷酸(简称三磷酸腺苷,又叫腺苷三磷酸,adenosine triphosphate,简称ATP)的形式产能,但他们必须持续地从血液中获得,因为心脏储存能量的能力很低。
成人健康心脏中,线粒体氧化代谢产生大多数ATP(大约95%),其中70%-90%来自脂肪酸氧化,其余的来自于葡萄糖、乳酸、酮体和氨基酸(特别是支链氨基酸[BCAAs])代谢;剩下约5%的ATP由糖酵解产生。
线粒体是产生ATP的主要场所,心衰时ATP产生减少30%~40%,可能是由于心衰时线粒体氧化能力降低。衰竭的心脏中线粒体功能受损可能由于以下原因,包括:(1)线粒体的生物合成和脂肪酸氧化的转录调控下降;(2)活性氧(ROS)生成增多;(3)线粒体融合蛋白受损害;(4)持续的线粒体自噬;(5)心肌细胞中自噬性细胞死亡增加。在心肌肥厚、缺血性心脏病、糖尿病、心力衰竭等多种心脏疾病中均出现心肌能量代谢改变,包括心肌细胞表面分子表型的变化,这方面的研究相对较多且较明确,即不管何种类型的心脏疾病中均不同程度的出现心肌能量代谢障碍。心肌缺血时交感神经兴奋,儿茶酚胺水平升高,继而导致血中游离脂肪酸水平升高;心肌能源供给中游离脂肪酸氧化增加至80%~90%;葡萄糖酵解和葡萄糖氧化的耦联失调,致使心肌细胞内H、Ca2及Na超载,引起细胞酸中毒和损害,使心肌工作效率降低,同时耗能增加。心脏代谢通路依赖多种代谢酶和氧化磷酸化的调节,心脏耗氧量大,能量供应不足可导致心脏衰竭。心脏能量代谢稳态是保持心脏组织结构不断更新和内环境稳态的物质基础。慢性心脏衰竭的过程中往往伴随着心肌能量代谢的改变,而能量代谢的改变也会影响慢性心脏衰竭的病程进展。
心肌能量代谢治疗是指药物在不改变心率、血压和冠状动脉血流的前提下,通过改善心肌细胞的能量代谢过程,使心肌细胞获得更多的能量物质,来满足保存细胞完整性,实现其生理功能需要的一种治疗方法。心肌能量代谢治疗不是通过增加供能和减少耗能实现的,而是利用有限的氧气、底物资源来产生更多的能源物质,消除代谢产物的不良影响。因此,代谢治疗是对原有治疗的补充和完善,不是替代原有治疗,心肌能量代谢治疗已成为多种疾病治疗的新靶点。
目前,临床上有曲美他嗪,通过抑制游离脂肪酸代谢,使游离脂肪酸代谢减少,从而使心肌以葡萄糖代谢为主产生能量,在冠状动脉病变而心肌供氧受到限制时,提高氧的利用度,产生更多的高能磷酸键,以缓解心肌缺血症状并维持心肌的存活和心脏的功能,但是通过葡萄糖代谢途径产生的ATP是远远不够的。
公布号为CN101356971A的中国发明专利申请公开了一种用于抗疲劳耐缺氧的保健食品组合物,以重量百分比计,该保健食品组合物包含45%~80%D-核糖、15%~50%L-肉碱、0.5%~8%矫味剂。以上发明专利申请中,以D-核糖和L-肉碱为主功效成分,但实际上这样的组合物对于改善心脏能量代谢的效果有待提高。
发明内容
为了解决以上现有技术中的组合物对于改善心脏能量代谢的效果有待提高的问题,本发明提供一种营养支持剂,该营养支持剂能很好地增强心脏功能,同时恢复心脏活力,改善心衰,有效预防心功能急性衰竭。
为了达到上述目的,本发明采用如下技术方案:一种营养支持剂,其活性成分包括左卡尼丁、D-核糖,还包括辅酶Q10,所述左卡尼丁、D-核糖和辅酶Q10的用量分别为10-25份、20-60份、0.5-2份。
优选的是,所述营养支持剂的活性成分还包括5-15份的瓜氨酸和/或5-15份的牛磺酸。
上述任一方案优选的是,所述营养支持剂的活性成分还包括0.5-2份的钾离子和/或1-4份的镁离子。
上述任一方案优选的是,钾离子和/或镁离子以螯合形式存在。
上述任一方案优选的是,钾离子来自甘氨酸钾。
上述任一方案优选的是,钾离子来自甘氨酸镁。
上述任一方案优选的是,所述营养支持剂的活性成分还包括1-4份的维生素C。

各组分的作用如下:
1、左卡尼丁
左卡尼丁又名左旋肉碱,为一类维生素类生理活性物质,为哺乳动物能量代谢中必须的体内天然物质,是一种广泛存在于机体组织内的特殊氨基酸,为脂肪酸代谢所需。左卡尼丁是卡尼丁转位酶、脂酰卡尼丁转移酶Ⅰ和Ⅱ的辅助因子。这些酶的作用为促进长链脂肪酸转变成脂酰卡尼丁并转移入线粒体。左卡尼丁主要功能是将长链脂肪酸转运至线粒体进行β-氧化,促进脂类代谢,并促进其氧化分解,为细胞提供能量,其可将长链脂肪酸带入线粒体基质,并促进氧化分解为细胞提供能量,也可将线粒体代谢的短链酯酰基带出线粒体,有效地缓解体内脂质代谢紊乱,骨骼肌及心肌功能障碍。
正常心肌能量供应的60-80%来自脂肪代谢。正常人的心肌细胞含有大量的左旋肉碱,当患者发生急性心肌损伤时,心肌内的左旋肉碱含量减少,此时患者心肌细胞主要依靠葡萄糖的无氧酵解进行供能,最终由于能量供应不足,导致心肌细胞代谢发生障碍。正常生理状态下,细胞能够摄取代谢所需的左卡尼丁,但当左卡尼丁不足时将抑制脂肪酸的氧化,心肌细胞内会聚集能够对心肌细胞产生毒性作用的脂酞辅酶和游离脂肪酸,此时葡萄糖的无氧酵解成为心肌细胞的主要能量代谢方式,酸性和乳酸含量的堆积将导致心肌机械和代谢功能严重受损,在患者心肌损伤严重时,缺血、缺氧一方面使心肌转为无氧酵解为主,另一方面游离卡尼丁被大量消耗而减少。及时补充外源性左旋肉碱,可以使脂酰辅酶A转运入线粒体,使得心肌细胞可以进行正常的氧化磷酸化提供能量,足够量的游离卡尼丁,可以使心脏重新转回以脂肪酸氧化为主,使心肌细胞内能量代谢得以恢复。同时减少游离脂肪酸、长链脂酰CoA和长链脂酰卡尼丁等有害物质在心肌细胞内堆积,从而预防和减轻心肌损伤。
心脏疾病的患者长期心脏超负荷,导致心肌耗能增加,心肌细脆生长,患者心肌失代偿性增厚,进一步减少能量供应,导致心肌能量缺乏加重,同时脂肪酸氧化受限引起脂肪酸长期蓄积产生毒性物质,对线粒体酶活性进行抑制,导致氧化磷酸化,ATP降低,而采用左卡尼丁治疗能纠正心肌细胞能量代谢异常,提高心肌细胞能量产生和组织器官功能,对左室重构有帮助。其次,左卡尼丁对心肌细胞内Ca2+超载有抑制作用,降低细胞H+水平,对心脏再灌注损伤以及血管内皮细胞抗氧化作用进行保护,提高冠状动脉血流量,缓解心脏缺氧缺血功能。左卡尼丁通过降低患者血清NT-proBNP水平,改善左室射血分数促进患者心功能恢复。
左卡尼丁的促脂肪酸氧化功能,可以促进血脂,甘油三酯,HDL,Lp(a)的正常。当我们给与卡尼丁剂量为900mg/天时测定血脂浓度。随着卡尼丁的持续供给,血甘油三脂的浓度持续下降。给予为期10-15周的每日1g左卡尼丁可使血浆中HDL的浓度显著升高。
2、D-核糖
D-核糖是一种简单五碳单糖,为细胞能量代谢过程中重要且必须的物质。D-核糖具有手性碳原子特性,有L型和D型2种构型。自然界中L型核糖极不稳定,核糖主要以D型形态存在,即D-核糖。D-核糖是一种具有极强水溶性、有甜味的五碳糖,又名D-呋喃核糖,是存在于所有活细胞中的天然戊醛糖,以呋喃型广泛存在。虽然D-核糖是细胞里天然存在的物质,但是细胞没有储存D-核糖的功能。如果细胞没有足够的D-核糖供应,在缺氧状态下,细胞不能通过重新合成和补救途径有效合成ATP,因此会导致人或动物出现乏力、心肌细胞功能下降,进而对机体产生危害。外源D-核糖口服经肠吸收或通过静脉注射进入血液,可以被肌体组织高效利用,因此外源D-核糖是机体补充能量的一种方式。
在人体中,核糖是由葡萄糖(简单的6-碳糖)通过磷酸戊糖途径(PPP)或己糖磷酸支路(HMS)所合成。这径路与木糖醇的合成途径相同。但不幸的是,在心脏和肌肉的细胞中,调控这条途径的一些重要的酶是缺乏的。因此,导致了核糖在心脏和肌肉细胞中的合成变成了是一个缓慢的过程。其结果就是一旦因疾病或运动使得心肌能量储备耗竭时,心脏和肌肉组织无法迅速得到能量的补充。某些心脏疾病所致的心脏血流减少或剧烈运动可显著降低心肌能量水平。而心肌能量池的耗竭导致心脏应激增加,机体外周血流减少,疲倦和运动耐量降低。直接补充核糖绕过缓慢和速率限制的磷酸戊糖途径,形成PRPP,并迅速开始能量(ATP)合成过程,可快速恢复心脏能量储备。
D-核糖的生理功能形式被称为5-磷酸核糖基-1-焦磷酸(PRPP),它调节所有的存活组织中合成能量复合物的生成。如果该化合物不能提供充足的供应,能量合成过程就会减慢。如果因疾病或运动细胞造成心肌能量池耗尽,必须补充。机体需要开启代谢途径来生成PRPP来补充能量池。
ATP是细胞的“能量货币”。在高强度运动或疾病状态下,ATP的分解速度会大于合成速度,ATP浓度随之下降。大部分ATP的降解产物IMP可以通过补救途径重新合成ATP,小部分IMP会进一步降解,以嘌呤形式流失,进而导致核苷酸池不断下降。细胞可以通过重新和补救合成途径弥补流失的ATP,D-核糖的含量是其恢复速度的限制性因素。衰竭心脏内的ATP及核苷酸池含量会慢慢减少,它们的含量与衰竭的程度存在紧密的相关性,能量供应不足被认为是引起心力衰竭的重要原因。核糖可以通过加速ATP的从头和补救合成途径来补充ATP,大量临床实验表明核糖可以改善缺血性衰竭患者的心脏功能。
在体内,D-核糖可通过多个途径完成代谢,如磷酸戊糖途径、糖酵解和代谢成磷酸核糖焦磷酸(phosphoriboseyl pyrophosphate,PRPP)等。磷酸戊糖途径的第一个限速酶(葡萄糖-6-磷酸脱氢酶)因其催化反应速率慢,限制了整个代谢途径的反应进程。D-核糖可走旁路绕过这个限速过程,提高磷酸戊糖循环,提高ATP水平,产生还原型辅酶Ⅱ(triphosphopyridine nucleotide,NADPH),增强氧化酶的活性,提高清除氧自由基的能力,提高机体自身的防御能力。D-核糖在核糖激酶的作用下合成核糖-5-磷酸,核糖-5-磷酸在磷酸戊糖途径的非氧化阶段形成的果糖-6-磷酸和甘油醛-3-磷酸,进入糖酵解途径形成丙酮酸,产生ATP。核糖-5-磷酸在磷酸核糖焦磷酸合成酶的作用下合成PRPP,PRPP是重要的代谢中间产物,参与嘌呤核苷酸和嘧啶核苷酸的从头合成、补救合成、某些氨基酸、以及某些核苷酸类辅酶如辅酶I和辅酶II的合成。同时,核糖-5-磷酸可逆磷酸戊糖途径合成葡萄糖。D-核糖可提高组织中的ATP水平,为机体提供足够的能量;加快合成NAPDH,提高还原型谷胱甘肽(GSH)的含量;清除氧自由基,提高机体抗氧化能力;抑制细胞膜的脂质过氧化作用,保持细胞膜的结构和功能完整。而谷胱甘肽有利于细胞抵抗有毒物质,抗糖基化、预防和治疗心肌细胞缺氧复、氧损伤等作用。
3、CoQ 10:
辅酶Q10(CoQ10)是生物体内具有重要作用的辅酶之一,广泛存在的脂溶性醌类化合物,不同来源的辅酶Q10其侧链异戊烯单位的数目不同,人类和哺乳动物是10个异戊烯单位,故称辅酶Q10,又称泛醌。辅酶Q10是脂溶性,高分子量,由机体自身生成,用以维持细胞基本功能的大分子复合物。在细胞能量代谢生成ATP的过程中起到重要作用。在能量需求量大的器官如心脏和肝脏中,CoQ10的浓度最高。其富含于人心脏、肝脏、肾脏和胰腺中。人体内总含量为0.5~1.5g。在细胞内的分布为:细胞核内占25%~30%,线粒体内占40%~50%,微粒体内占15%~20%,细胞质内占5%~10%。
辅酶Q10具有促进氧化磷酸化反应和保护生物膜结构完整性的功能。辅酶Q在体内呼吸链中质子移位及电子传递中起重要作用,它是细胞呼吸和细胞代谢的激活剂,也是重要的抗氧化剂和非特异性免疫增强剂。在呼吸链中辅酶Q10是一个和蛋白质结合不紧密的辅酶,使它在黄素蛋白类和细胞色素之间能作为一种特殊灵活的载体而起作用;另外,其醌环中苯醌的羟基取代基使它倾向于极性,而结构中的聚异戊二烯侧链使它在疏水性环境中具有较低的自由能,能在线粒体内膜中迅速扩散,辅酶Q10从复合体Ⅰ和复合体Ⅱ接受氢,将质子释放至线粒体基质内,电子传递给细胞色素,通过这一过程促进氧化磷酸化及电子的主动转移,由此形成机体能量贮存的主要物质ATP,因此辅酶Q10可明显降低心肌缺血再灌注后丙二醛(MDA)的生成,减轻线粒体肿胀,维持细胞线粒体结构的完整性,防止细胞膜破裂,以及线粒体溶解与肌纤维的紊乱性排列。保持细胞膜结构的完整性,缩小心肌缺血范围。这些均表明辅酶Q10具有抗氧自由基,维持钙离子通道完整,防止心肌细胞线粒体畸变,维持细胞结构的完整性从而达到保护心肌的作用。心肌缺血、缺氧时,由于心肌细胞膜受到损伤导致膜内pH下降,并失去了结合Ca2+能力,大量Ca2+以丛状形式沉积于线粒体,即造成了所谓的Ca2+负荷过重。辅酶Q10能增加ATP合成,具有直接保护细胞膜稳定细胞内外环境的作用。
给予外源性辅酶Q10可使患者临床症状明显改善。在心力衰竭及扩张性心肌病变的患者中,心肌功能可能只有一般人的一半甚至不到辅酶CoQ10的心肌细胞能量活化作用,同时可以带动心律正常化及作功率的提升,使得心肌带氧量相对提升,这样的作用对于因为缺血性(缺氧)心脏病引起的心肌细胞缺损,往往具有一定程度的修补作用,同时能够让松弛无力的心肌慢慢紧缩收敛,并且减少心肌代偿性的作功。对于某些心肌肥大症及扩张性心肌病变的患者,可能会有明显的改善效果,不过这也会因个人病情而异。
CoQ10同时具有降血压作用,这也是CoQ10对心脏保护方面很重要的一环,一份针对患有高血压的46名男性及37名女性所做,为期12周的双盲试验结果发现,每天服用120毫克(分两次服用)的患者,血压平均下降约17毫米汞注(mmHg),所有服用CoQ10的患者,完全没有任何副作用发生。
CoQ1的抗氧化剂作用,可以抵制血液中自由基与低密度脂蛋白的副作用,是预防动脉硬化形成最直接有效的抗氧化成分,这项保护措施,更是预防心肌梗塞及血栓形成最重要的一道防线。多数的心血管疾病患者多合并有高血脂症的问题,于是服用降血脂药物当然是预防心脏病不可或缺的治疗守则,目前临床上效果最好且最被医学界广为使用的降血脂药物就是HMG-CoA还原酵素抑制剂的statin类药物,不过这类药物在阻断胆固醇合成机转的同时,却也同时阻断了体内自行合成CoQ10的能力,研究发现,服用statin类药物同时会使体CoQ10的浓度下降,这对本来就有心脏问题的高血脂患者,可能会是比较负面的副作用,所以建议服用statin类药物的同时,应该同时补充CoQ10以减少其诱发心脏功能减退的危险副作用,还具有减轻由statin类药物引起的肌痛症状的作用。
4、瓜氨酸
瓜氨酸(citrulline)一种不存在于蛋白质中的L-a-氨基酸,在血管内皮、肾脏和其他细胞内代谢成L-精氨酸。作为一种非蛋白质氨基酸,l-瓜氨酸很少在食物中发现,但高度集中在西瓜中,最早从西瓜中提取出来,故名为瓜氨酸。
众所周知,心力衰竭(HF)是一种临床综合征对心肌纤维的损伤。主要的病因是急性或慢性缺血、高血压和心律失常。涉及的分子机制有凋亡、氧化应激、炎症、内皮功能障碍和心脏重构等。特别对于心力衰竭中常见的CHF进行性恶化的原因,冠脉内皮舒张血管功能障碍是主要原因。HF与血管紧张素转化每抑制剂活性增加和外周血流量减少有关,两者都会降低一氧化氮的合成(NO),增加内皮素-1导致功能障碍。
内皮功能障碍导致血管舒张无力,无法适应血流增加等生理性刺激,反射性的损害血流依赖与内皮介导的血管舒张作用。这是HF全身性血液循环异常的表现,与NO生成量减少相关,同时也加速心力衰竭的进展过程。
瓜氨酸在肠道细胞中经鸟氨酸氨甲酰转移酶(OTC)的作用,利用谷氨酰胺合成瓜氨酸并释放入血,经肾脏中受精氨基琥珀酰合成酶(ASS)和精氨基琥珀酰裂解酶(ASL)的作用下转化为精氨酸;而在肝脏细胞中,经OTC的作用合成瓜氨酸后在ASS的作用下进入尿素循环;在产NO细胞中,瓜氨酸通过ASS参与精氨酸合成,并进一步将精氨酸合成NO,构成“Cit-NO”循环。作为在肾脏中合成精氨酸的前体,其能促进精氨酸进入内皮细胞和免疫细胞,在一定程度上提升精氨酸的作用,更重要的是瓜氨酸是能与精氨酸竞争抑制酶活性中心的结构相似的氨基酸,可通过负反馈调节NO的生成,同时瓜氨酸还具扩张血管、降血压、抑制血管内皮细胞增殖等作用。
目前国内外对瓜氨酸的治疗效果研究上,瓜氨酸的研究主要涉及炎症、肿瘤、自身免疫疾病等领域;但是瓜氨酸在心血管领域的研究相对较少。
一氧化氮(NO)是一种参与多种重要生物功能调控的气体信号分子,其生物利用度的降低导致多种年龄和生活方式相关疾病的发生和发展。虽然l-精氨酸是内皮型一氧化氮合酶(eNOS)合成NO的前体,但由于多种原因,口服l-精氨酸在增加NO合成和/或生物利用度方面基本无效。而瓜氨酸在西瓜中含量很高,是一种中性的氨基酸,由线粒体中的酶形成,同时也是l-精氨酸循环的底物。与l-精氨酸不同,l-瓜氨酸不能从胃肠道中定量提取。因此,肝脏及其补充物在提高l-精氨酸水平和NO合成方面更有效。补充l-瓜氨酸有希望作为一种降压干预(包括休息和压力诱导)的成人高血压前期/前期(动物)的动脉粥样硬化内皮保护的证据。l-瓜氨酸诱导的肌肉和代谢健康(通过血管和非血管途径)在易感/老年人群中也有不小的益处。越来越多的证据表明内皮功能障碍也有其原因由于缺乏l-精氨酸-no代谢,并给予了相对无效的l-精氨酸补充NO代谢。然而,与补充l-精氨酸相比,l-瓜氨酸缺乏胃肠道不适,这可能表明l-瓜氨酸的摄取利用了不同的转运系统。l-精氨酸主要通过与Na+无关的阳离子氨基酸转运体(cat-1,2和3)在肠膜上转运。l-瓜氨酸转运已在肠细胞、巨噬细胞、胶质细胞和主动脉平滑肌中得到证实。虽然尚未确定一种特定的l-瓜氨酸转运蛋白,但b0转运蛋白可能在l-瓜氨酸钠依赖转运的肠上皮细胞中发挥作用。自l-瓜氨酸代谢中肝脏在一定程度上被划分为尿素循环,外源性l-瓜氨酸通常会绕过尿素循环肝的新陈代谢。循环的l-瓜氨酸,从肠道释放,被近端小管吸收肾细胞。l-瓜氨酸被胞质精氨酸琥珀酸合成酶快速作用。
所以,l-瓜氨酸通过增加l-精氨酸的合成,间接增加NO的生物合成。已有文献证明,l-瓜氨酸,而不是l-精氨酸的补充,增加NO合成微循环(肠绒毛)血流量。此外,在啮齿类动物模型的自发高血压和化学物质破坏NO生物利用度,l-瓜氨酸恢复NO by水平增加l-精氨酸/非对称二甲基精氨酸(ADMA)比值。这些临床前研究也都支持l-瓜氨酸能增强内皮功能的假设。
综上所述,l-瓜氨酸改善内皮细胞功能的机制功能障碍,如动脉粥样硬化,很可能是通过直接减少介导的羟基自由基的形成,直接作用于血管平滑肌,间接作用于NO合成。
5、甘氨酸钾、甘氨酸镁、维生素C
镁离子是人体必需的微量元素,其含量是仅次于钠、钾、钙的人体第四位阳离子,它参与体内的物质代谢,并且是多种酶促反应的参与因子,包括葡萄糖利用、脂肪、蛋白、核酸合成代谢以及一些膜转运系统。在细胞内,其重要性仅次于钾离子。心肌细胞能量代谢通道的磷酸化和去磷酸化所需酶的激活都需镁参与。
在缺氧时,ATP生成减少,使得Na+-K+泵不能充分转运,进一步增多细胞内Na+,导致细胞水肿,血管内皮细胞水肿可堵塞微血管,加重组织缺氧。另外,心肌缺氧时钾离子外流使细胞内缺K+,将导致细胞内酶的合成代谢障碍,进一步影响ATP的合成和离子泵的功能。所有,补充甘氨酸钾、甘氨酸镁对ATP的合成和离子泵的功能有辅助重要的作用。
维生素C(vitaminC,VC)是体内抗氧化防御体系中唯一不能合成的组份。有研究显示维生素C能显著提高心肌超氧化物歧化酶(superoxidedismutas,SOD)的活性。维生素C清除心肌缺血后再灌注产生的氧自由基,起到心肌保护作用,这种保护作用是否与mitoKATP和mPTP有关尚未阐明。另一方面,维生素C和牛磺酸对改善心肌损伤有协同作用。
综上所述,左卡尼丁增强脂肪酸代谢途径产生ATP;D-核糖可多途径代谢产生ATP,包括磷酸戊糖途径、糖酵解和代谢成磷酸核糖焦磷酸;辅酶Q10促进氧化磷酸化反应和保护生物膜结构完整性;瓜氨酸改善血管内皮功能舒张血管。
甘氨酸钾和甘氨酸镁对Na+-K+泵功能转运,辅助ATP合成有重要的作用;维生素C的抗氧化可以清除心肌缺血后再灌注产生的氧自由基,起到保护以及对其他物质的协同作用。
左卡尼丁、D-核糖、辅酶Q10、甘氨酸钾、甘氨酸镁、维生素C,这几种物质作为能量代谢的底物和代谢通道的相互作用的中间物质,互相协调,增加ATP合成,增强心脏能量生产,同时,瓜氨酸改善血管内皮功能舒张血管,这几种物质联合使用,可以很好的改善心脏功能。
第二方面,本发明提供所述营养支持剂的制备方法如下:
1、将以上各原料(可叫做主料)与辅料粉碎至80-200目;
2、将上述原料和辅料共同置于混料罐中,充分混匀;
3、将各种原辅料经过30s紫外线轨道杀菌,然后进行分装,并用75%的酒精对外包装进行清洁消毒。
第三方面,本发明提供所述营养支持剂在用于治疗慢性心力衰竭、冠心病、心肌病、心率失常、高血压的药物的制备的用途。
本发明提供的营养支持剂,将各种重要的心肌能量代谢底物及中间产物进行有机融合并综合补充,维持ATP酶活性,同时增加ATP的代谢合成底物,增加ATP合成,增强心脏能量生产,改善心脏功能。组分之间发生协同作用,能有效减少心脏能耗,改善心脏功能,同时,积极改善血管内皮功能,全方位促进心脏恢复健康活力。
本发明的营养支持剂能间接增加NO的生物合成,为心脏增加能量底物代谢,减少心脏耗能的同时,改善血管内皮功能,维持血管的舒张状态;防止血小板在血管内皮粘附聚集;抑制血管平滑肌增殖;阻止白细胞粘附在血管内皮,使血管内皮免受害。
具体实施方式
为了更加清楚、正确地理解本发明的发明内容,下面结合具体实施例进一步对本发明的内容进行进一步的解释、说明。
以下实施例中,甘氨酸钾含有18%(重量)活性成分;甘氨酸镁含有18%(重量)活性成分;辅酶Q10来源于QAvail粉剂,14%的活性成分,其他原料购自美国健康设计公司(DFH)。每份重量以100mg计。
实施例1
一种营养支持剂,其活性成分(主料)由以下重量份的成分制成:左卡尼丁20份、D-核糖40份、辅酶Q10 0.5份。
本实施例中的营养支持剂的制备方法如下;
1、将各主料与辅料粉碎至80目;
2、将上述原料和辅料共同置于混料罐中,充分混匀;
3、将各种原辅料经过30s紫外线轨道杀菌,然后进行分装,并用75%的酒精对外包装进行清洁消毒。
按照GB 13432-2013《食品安全国家标准预包装特殊膳食用食品标签》各营养素均要在产品外包装上进行种类及含量标注,且最大检验误差不得低于标示值得80%。
此制备方法属于干法工艺,过程简单,且对维生素的稳定性较好,然后将原辅料经物理混合充分。所有操作在GMP实验室完成。
本实施例中,辅料为蔬菜纤维、糊精、天然桔子香料、甜叶菊。辅料一般不影响活性成分的效果,因此,辅料还可选择其他种类的辅料,且用量为本领域常规用量即可。

为了考察各组分的最佳用量及各实施例和对比例之间的差异性,下面结合动物实验进行说明。
动物实验
该营养支持剂对心衰大鼠的影响
(一)动物模型
1材料与方法
1.1实验动物健康雄性SD大鼠,体重230g~260g,由北京维通利华实验动物中心提供[许可证编号:SCXK(京)2006—0009],分笼饲养于拜欧(北京)生物科技有限公司SPF级动物房,室温保持在22-26℃之间,相对湿度50%-60%,日光交替照射,自由摄食饮水。基础饲料、高脂饲料(3.0%胆固醇,0.5%胆酸钠,10%猪油,5%蔗糖,0.2%丙基硫氧嘧啶和常规基础饲料)均购自上海普路腾生物科技有限公司。
造模前大鼠在SPF级动物饲养房适应环境1周。每6只大鼠饲养于一个鼠笼,动物饲养程序均依照《实验动物照顾和使用指南》执行,室温维持在22~25℃,空气湿度45%-50%,正常昼夜节律(12h光照,12h黑暗),给予自由饮水饮食,饮用水与饲料由北京中医药大学东直门医院动物实验中心提供。
1.2实验仪器
ALC—V8动物呼吸机(上海奥尔科特生物科技有限公司),心电示波仪(上海医学电子仪器厂XJ一11型),高分辨率小动物超声影像系统Ve—v02100(加拿大Visual Sonics公司,探头型号MS一201),16号套管针(针芯的针尖磨平)用于气管插管。
1.3动物模型制备
1.3.1动物麻醉及监测将动物称重后用1%戊巴比妥钠(40mg/kg,进13分装)腹腔注射麻醉,仰卧位固定。
1.3.2气管插管并连接呼吸机台灯下用纱布轻轻将大鼠舌头捏住提起,经口将套管针插入气管,拔出内芯。用胶布固定好,连接小动物呼吸机,调整潮气量为每次O.7mL~0.8mL,吸呼比为1:2,通气频率为80次/min。
1.3.3心肌梗死后心力衰竭模型的建立参照文献,大鼠胸骨旁左侧第3~4肋间,沿肋间方向切开皮肤,长约1.5cm,逐层分离皮下组织、肌肉,打开心包膜。用弯镊轻轻提起左心耳,可以看到左冠脉前降支的起始部,用5.0缝合线,在左心耳和肺动脉圆锥之间,心耳边缘下2mm,结扎左冠状动脉前降支,进针深度控制在1mm,宽度1mm~2mm,收线打结。术中肉眼可见结扎线远端左室前外侧壁心肌变白区,挤压胸腔以排出空气并逐层关闭胸腔。之后移除呼吸机,轻捏大鼠胸廓,辅助其恢复呼吸。术中观察动物呼吸状态及心电图变化情况。假手术组在相同位点只穿线不结扎。术后腹腔注射青霉素160IU/(kg·d),3d,以预防感染。
1.3.4动物分组
将312只大鼠,随机抽取30只作为假手术组,其余大鼠建立心衰模型,术后存活242只,10天余240只成模,将240只大鼠分组如下:
核糖对照组:共40只,分别空白对照组10只,低剂量核糖组10只,中剂量剂量核糖组10只,高剂量核糖组10只;
瓜氨酸对照组:共40只,分别空白对照组10只,低剂量瓜氨酸10只,中剂量剂量瓜氨酸组10只,高剂量瓜氨酸组10只;
辅酶Q10对照组:共40只,分别空白对照组10只,低剂量辅酶Q10组10只,中剂量剂量辅酶Q10组10只,高剂量辅酶Q10组10只;
左旋肉碱对照组:共40只,分别空白对照组10只,低剂量左旋肉碱组10只,中剂量左旋肉碱组10只,高剂量左旋肉碱组10只;
曲美他嗪对照组:共40只,分别空白对照组10只,低剂量曲美他嗪组10只,中剂量核糖组10只,高剂量核糖组10只;
该营养支持剂(实施例2.3):共40只,分别空白对照组10只,低剂量营养支持剂组10只,中剂量营养支持剂组10只,高剂量营养支持剂组10只。
(二)施用途径与剂量
1、施用途径
灌胃。灌胃给药,每天1次灌胃给药,灌胃体积10ml/kg。大鼠每周称体重1次,以便调整给药剂量。
2、剂量
D核糖:大鼠的实验用量一般为人摄食量的5倍,人口服摄入核糖的剂量为200mg/Kg·bw·dose,3/day。则大鼠的摄入剂量100mg/100g·bw·dose,即300mg/100g·day。设计剂量梯度:低剂量100mg/100gbw·day,中剂量300/100mg·bw·day;高剂量600/100mg·bw·day(按照平均体重计算)。
瓜氨酸:根据参考文献,设计剂量梯度:低剂量100mg/100gbw·day,中剂量200/100mg·bw·day;高剂量300/100mg·bw·day(按照平均体重计算)。
辅酶Q10:根据参考文献,设计剂量梯度:低剂量20mg/100gbw·day,中剂量25/100mg·bw·day;高剂量30/100mg·bw·day(按照平均体重计算)。
左旋肉碱:根据参考文献,设计剂量梯度:低剂量20mg/100gbw·day,中剂量30/100mg·bw·day;高剂量40mg·bw·day(按照平均体重计算)。
曲美他嗪:根据参考文献,设计剂量梯度:低剂量5mg/100gbw·day,中剂量10/100mg·bw·day;高剂量15mg·bw·day(按照平均体重计算)。
营养支持剂:设计剂量梯度:低剂量50mg/100g·bw·day,中剂量120/100mg·bw·day;高剂量150mg·bw·day(按照平均体重计算)。
(三)、实验方法
3.1大鼠灌胃
分别给每组大鼠灌胃核糖、瓜氨酸、辅酶Q10、左旋肉碱、曲美他嗪、该营养支持剂(实施例2.3)、CN101356971A发明专利申请实施例1口服液、蒸馏水,用量为1ml/100g*bw*day。实验期间,动物禁食不禁水,分别在灌胃后第10天和第60天进行超声心功能检测。
3.2主要溶液配制
核糖溶液:核糖粉末600g+双蒸水定容1000ml,配制成600mg/ml核糖溶液,121℃高温、高压灭菌20min,4℃保存。用时再分别稀释为300mg/ml核糖溶液和100mg/ml核糖溶液(现配现用)。
瓜氨酸酸溶液:分别使用10mL/kg双蒸水溶解的3000、4000、5000mg/kg瓜氨酸,配制成100mg/ml、200mg/ml、300mg/ml瓜氨酸溶液,121℃高温、高压灭菌20min,4℃保存。
辅酶Q10溶液:辅酶Q10粉末30g+双蒸水定容1000ml,配制成50mg/ml核糖溶液,121℃高温、高压灭菌20min,4℃保存。用时再分别稀释为20mg/ml辅酶Q10溶液和25mg/ml核糖溶液(现配现用)。
左旋肉碱溶液:左旋肉碱粉末40g+双蒸水定容1000ml,配制成40mg/ml核糖溶液,121℃高温、高压灭菌20min,4℃保存。用时再分别稀释为20mg/ml左旋肉碱溶液和30mg/ml核糖溶液(现配现用)。
曲美他嗪溶液:曲美他嗪15mg+双蒸水定容1000ml,配制成15mg/ml核糖溶液,121℃高温、高压灭菌20min,4℃保存。用时再分别稀释为5mg/ml核糖溶液和10mg/ml核糖溶液(现配现用)。
营养支持剂:营养支持剂4500mg+双蒸水定容1000ml,配制成150mg/ml核糖溶液,121℃高温、高压灭菌20min,4℃保存。用时再分别稀释为50mg/ml营养支持剂溶液和120mg/ml营养支持剂溶液(现配现用)。
3.3超声心动图检查
1%戊巴比妥钠(40mg/kg)腹腔注射麻醉,仰卧位固定胸部备皮。GEVivid7彩色多普勒超声仪进行超声心动图检查,记录超声图像,超声探头频率应用10S变频探头,主频10M H z。专人盲法检测,取左室乳头肌短轴切面及左室长轴切面,M型取样左室前壁和左室后壁,测定左室射血分数(ejection fraction,EF)和左室短轴缩短率(fractionalshortening,FS),每只动物重复测量3次,所有检测指标均取6个连续心动周期的平均值。
(四)统计学处理
所有数据均使用SPSS 20.0进行统计学分析,计量资料进行正态分布及方差齐性检验,符合正态分布的计量资料以均数±标准差(i t-S)表示,采用独立样本f检验,非正态分布的计量资料采用秩和检验。对超声心动图的EF值和FS值进行相关性分析,符合正态分布的资料采用Pearson检验,非正态资料采用Spearman检验,以P<0.05为差异有统计学意义。采用四格表计算敏感性、特异性和准确性,其中敏感性为模型组内所检出的心衰动物只数的比率(%),特异性为假手术组内所检出的非心衰动物只数的比率(%),准确性为模型组内检测出的心衰动物与假手术组检测出的非心衰动物只数占动物总数的比率(%)。组间比较采用方差分析,计量资料用t检验,P<0.05为差异有统计学意义。
(五)实验结果
5.1超声心动图结果
5.11与假手术组相比,模型组大鼠射血分数和短轴缩短率显著下降。
各组大鼠模型建立前超声检测左室射血分数(ejection fraction,EF)和左室短轴缩短率(fractional shortening,FS),每只动物重复测量3次,所有检测指标均取6个连续心动周期的平均值,各组之间差异无统计学意义(P>0.05)。血分数及短轴缩短率,造模型后10d,随机抽取模型组小鼠30只,与假手术组相比,模型组大鼠射血分数和短轴缩短率显著下降(P<0.05),详见表1。

5.12处理10和60天后,各组中超声心动图与空白组对照。
处理10天后,牛黄酸组、D核糖组、左旋肉碱组、辅酶Q10组、曲美他嗪组、该营养支持剂组分别与模型组对照组比较,左室射血分数(ejection fraction,EF)和左室短轴缩短率(fractional shortening,FS)有所增加,该营养支持剂组叫其他组的左室射血分数(ejection fraction,EF)和左室短轴缩短率(fractional shortening,FS)明显增加(P<0.05)。
处理60天后,与空白组相比,瓜氨酸、D核糖组、左旋肉碱组、辅酶Q10组、曲美他嗪组、该营养支持剂组左室射血分数(ejection fraction,EF)和左室短轴缩短率(fractional shortening,FS)有所增加,该营养支持剂组的EF值比较明显升高(P<0.05),低剂量组和中等剂量、高剂量组EF值相比,差异均无统计学意义(P>0.05),详见表2~7。

根据表2-7,可以知道,对于本发明的营养支持剂的各单组分及本发明的营养支持剂成品,均是中剂量为最佳用量。
5.13该营养支持剂与各组心脏射血分数EF值对照
为了证明本发明提供的营养支持剂的成分之间存在协同作用及瓜氨酸的重要性,用实施例1和2.3中的营养支持剂进行动物实验(实验方法同上),用量为中剂量(120/100mg·bw·day),并考察EF值,结果如表8所示。
为了进一步验证、证明本发明提供的营养支持剂的效果,设置以下对比例并进行动物实验(实验方法同上),考察各组的EF值并与实施例1和实施例2.3进行对比,详见表8。
对比例1
将动物实验中的营养支持剂替换成瓜氨酸,用量为瓜氨酸的最佳用量,即中剂量实验结果。
对比例2
将动物实验中的营养支持剂替换成辅酶Q10,用量为辅酶Q10的最佳用量,即中剂量实验结果。
对比例3
将动物实验中的营养支持剂替换成D-核糖,用量为D-核糖的最佳用量,即中剂量实验结果。
对比例4
将动物实验中的营养支持剂替换成左卡尼丁,用量为左卡尼丁的最佳用量,即中剂量实验结果。
对比例5
从CN101356971A发明专利申请的表5和表6中可知,其实施例1为最佳实施例,因此,本对比例中,将动物实验中的营养支持剂替换成实施例1中的组合物,用量为其中剂量,即2.0g/kg。

根据表8,对比各组小鼠10天后和60天后的心脏射血分数EF值,营养支持剂组(实施例1、2.3和2.4)干预效果明显,差异有统计学意义(P<0.05)。
同时,从表8可知,实施例1相对于对比例5(CN101356971A发明专利申请的实施例1)、空白对照组及对比例2(辅酶Q10)、对比例3(D-核糖)、对比例4(左卡尼丁),均具有统计学意义(P<0.05),说明本发明的营养支持剂(实施例1)提高小鼠的EF值的效果显著优于CN101356971A发明专利申请的组合物,且辅酶Q10、D-核糖及左旋肉碱(即左卡尼丁)之间发生了协同作用,能更好地提高小鼠心脏代谢能力。
从表8中,通过对比实施例2.3和2.4的数据可知,瓜氨酸的加入显著地提高了小鼠的超声EF值,可显著提高营养支持剂提高小鼠心脏代谢能力。
在本研究中,大鼠AMI模型建立后超声心动图检测发现心肌梗死后EF、FS等指标均发生了显著的改变,说明大鼠模型AMI模型建立非常成功。低剂量瓜氨酸、左旋肉碱、D核糖、辅酶Q10、曲美他嗪组、营养支持剂和中等剂量、高剂量组与假手术组相比指标明显偏低,出现明显的心室扩张和心功能下降。其中,使用营养支持剂的组较其他组明显,EF、FS等指标著增加,提示营养支持剂可以在一定程度上逆转心室结构的变化,改善小鼠心脏功能,其中以中等剂量营养支持剂组更为明显,提示营养支持剂可以改善小鼠心脏的收缩和舒张功能,一定程度上逆转小鼠心衰。
为了进一步验证本发明的营养支持剂的效果,下面结合典型病例进行说明。
典型病例列举以下典型病例均以实施例2.3中的营养支持剂为例,分装时每袋12g为例,包括本发明中要求保护的原料和辅料。
1、病例一
程某 男 46岁
主诉:心肌病十余年,常感心悸、乏力
病史:患者于2019年2月20日因心悸、心慌再次入院,既往多次临床用药治疗效果不佳。
入院查体:体温(T)37.0℃脉搏(P)80次/分呼吸(R)20次/分血压(BP)110/60mmHg慢性病容,双肺呼吸音清,未闻及干湿性罗音。心率86次/分,律不齐,有二尖瓣或三尖瓣区收缩期杂音。上腹部平坦,肝脾肋下未及。肠鸣音减弱,双下肢无水肿。舌质暗红,舌苔黄厚腻,脉弦。
辅助检查:心电图提示心室肥大,异常Q波,各种复杂的心律失常。超声心动图提示EF为30%。
病例特点:心肌病以及十余年,临床药物的治疗不佳,不仅患者心悸、心慌不能很好改善,而且心功能日益下降。
治疗经过:患者心肌病的时间已经较长,心功能下降明显(超声心动图提示EF为30%),既往临床用药疗效不佳,于2019年2月20日建议患者服用该营养支持剂12g/天,餐前服用,连续服用1个月后复查超声心动图。患者遵医嘱服用该营养支持剂3天后心率比之前稳定,心律不齐稳定的到明显改善,无心悸,偶有心慌,嘱继续服用该营养支持剂。患者于2019年3月14日复查超声心动图提示EF值为47%。
评价:本例患者临床表现典型,辅助检查,心电图提示心室肥大,异常Q波,各种复杂的心律失常。超声心动图提示EF为30%,给予心肌细胞能量支持,保护缺血、缺氧的心肌,改善充血性心力衰竭,病情得到控制,各项指标好转,生活质量明显提高。
2、病例二
张某 男 50岁
主诉:心功能欠佳,上楼气喘发虚
病史:患者于2019年2月21日因提重物上楼后身体发虚而入院,既往无心脏相关疾病史,但经常出现运动或劳作后身体是经常气喘发虚。
入院查体:体温(T)37.0℃脉搏(P)70次/分呼吸(R)18次/分血压(BP)130/80mmHg双肺呼吸音清,未闻及干湿性罗音。心率70次/分,律齐,无杂音。上腹部平坦,肝脾肋下未及。肠鸣音正常,双下肢无水肿。舌质淡红。
辅助检查:心电图提示心律不齐。超声心动图提示EF为48%。
病例特点:患者无明显的心脏病相关疾病史,但患者劳作后经常气喘发虚,心功能有所下降。
治疗经过:患者心功能有所下降(超声心动图提示EF为48%),既往无临床用药,于2019年2月21日建议患者服用该营养支持剂12g/天餐前服用,连续服用1个月。患者遵医嘱服用该营养支持剂7天后自觉体力恢复及佳,上楼无气喘发虚,可一次性坚持中低等轻度运动半个多小时。嘱继续服用该营养支持剂。患者于2019年3月16日复查超声心动图提示EF值为52%。
评价:本例患者自觉症状明显,超声心动图提示EF为48%,给予心肌细胞能量支持,并改善全身慢性疲劳的症状,患者短期内就能自觉到症状改善明显,心脏数学分数恢复正常值,生活质量改善。
3、病例三
樊某 男 63岁
主诉:糖尿病5年
病史:患者5年前诊断为II型糖尿病,目前空腹血糖控制在10~12左右,打胰岛素早32个单位,晚28个单位,二甲双胍4片/天。
入院查体:体温(T)37.0℃脉搏(P)70次/分呼吸(R)18次/分血压(BP)135/83mmHg双肺呼吸音清,未闻及干湿性罗音。心率65次/分,律齐,无杂音。上腹部平坦,肝脾肋下未及。肠鸣音正常,双下肢无水肿。
辅助检查:血常规:白细胞(WBC)5.9×109/L血红蛋白(Hb)90g/L血小板(PLT)90×109/L尿常规:尿蛋白(pro)(±)、葡萄糖+1;肝功能:丙氨酸氨基转移酶(ALT)58.2U/L天冬氨酸氨基转移酶(AST)45.9U/L r—谷氨酰转肽酶(GGT)215U/L;
病例特点:中老年男性,病程长,血糖控制不佳,出现肝肾功能的下降。
治疗经过:患者多年来血糖控制效果不佳,于2019年2月27日建议患者在继续使用胰岛素及二甲双胍的基础上,服用该营养支持剂12g/天餐前服用,连续服用3个月。患者遵医嘱服用该营养支持剂1周后空腹血糖控制在8~9左右,自觉体力较之前有很大改善,嘱继续服用该营养支持剂。患者于2019年3月27日复查肝肾功能,指标无明显异常。家庭测量血糖,空腹血糖一直控制在8左右。
评价:本例是典型的糖尿病患者,为细胞提供能量支持,并提高细胞对胰岛素的敏感性,患者短期内就能自觉到精神状态改善,空腹血糖降低,生活质量提高。
4、病例四
刘某 男 68岁
主诉:主动脉瓣伴升主动脉瓣置换术后一周
病史:患者因冠状动脉粥样硬化性心脏病、劳力性心绞痛、主动脉二瓣化、主动脉瓣中度狭窄,于2018年11月31日进行主动脉瓣伴升主动脉瓣置换手术,手术过程顺利,于2018年12月7日出院,住院诊断主动脉瓣二瓣化、主动脉瓣中度狭窄、升主动脉扩张、高血压三级、心功能III级(NYHN分级)。出院带药欣康片20mg每日两次14天,拜阿司匹林肠溶片100mg每日一次14天,匹伐他汀钙片4mg每晚一次14天,氨酰心安片12.5mg每日两次14天,泰勒宁1co每日三次5天,拜欣同控释片30mg每日一次14天,卡托普利片6.25mg每日三次14天,法华林钠片3mg(根据INR调整)每日一次14天。出院后患者体质极度虚弱,无食欲,自觉伤口疼痛不适。
病例特点:老年男性,心脏手术后,体质极度虚弱,伤口恢复较慢。
治疗经过:患者由于伤口疼痛不适,体质极度虚弱,于2018年12月7日主动要求术后康复,建议患者在继续服用出院所带药品的基础上,加该营养支持剂12g/天餐前服用,连续服用1个月,在服用期间定期检测凝血情况,并根据凝血情况调整华法林用量。患者遵医嘱服用该营养支持剂1周后空腹血压降低,自觉体力较之前有很大改善,伤口恢复好,再无不适,食欲增加,能自主做适当运动。
评价:本例是典型的心脏手术后老年患者,为细胞提供能量支持,抗氧化,心脏和肌肉组织的细胞能迅速得到能量供应,应对术后的机体应激状态,故患者短期内就能自觉到精神状态改善,伤口恢复好,食欲改善,血压有所下降。
二、病例回顾分析
射血分数(EF值),是指每搏输出量占心室舒张末期容积量的百分比。心室收缩时并不能将心室的血液全部射入动脉,正常成人静息状态下,心室舒张期的容积:左心室约为145ml,右心室约为137ml,搏出量为60-80ml,即射血完毕时心室尚有一定量的余血,把博出量占心室舒张期容积的百分比称为射血分数,一般50%以上属于正常范围,人体安静时的射血分数约为55%~65%。射血分数与心肌的收缩能力有关,心肌收缩能力越强,则每搏输出量越多,射血分数也越大。在心室异常扩大、心室功能减退的情况下,搏出量可能与正常人没有明显判别,但它并不与已经增大的舒张末期容积相适应,射血分数明显下降。低射血分数则是指射血分数低于正常百分比。病人在出现心功能不全时往往伴有射血分数的降低。当射血分数值降低至35%以下时发生恶性心律失常猝死的机会就大大增加。故,本次病例回顾分析以EF值作为心脏功能的评判标准。
选取2018年11月至2019年3月佳博泰和健康科技有限公司进行健康管理的心脏功能降低患者30例,其中男18例,女12例,随机分为观察组与对照组。其中观察组15例,男9例,女6例,年龄35~70岁,平均年龄(52.0±4.3)岁,心律失常7例,缺血性心肌损伤2例,冠状动脉粥样硬化性心脏病4例;心脏术后康复2例。对照组15例,男8例,女7例,年龄40~75岁,平均年龄(57.5±4.5)岁,缺血性心肌病7例,心律失常8例。两组患者在性别、年龄、所患疾病方面差异无统计学意义(P>0.05),具有可比性。
入选标准:①确诊为冠状动脉粥样硬化性心脏病、缺血性心肌病、不稳定性心绞痛等疾病;②患者自觉症状明显,包括心慌、乏力、心悸等;③心脏射血分数降低
治疗方法:所有入选人员,有病情需要者继续进行临床常规治疗,观察组在上述基础上增加使用该营养支持剂12g/天餐前服用,服用1月后复查心脏彩超。
临床观察:①胸痛、心悸等症状基本消失或缓解,观察组患者症状缓解明显;②对比两组患者治疗前后的心脏射血分数EF值;并比较。EF升高到50%以上为显效,EF升高为有效,EF没有升高判定为无效。
观察结果:
①两组患者治疗前后心脏射血分数EF值比较:两组患者治疗前后心脏射血分数对比;观察组治疗后心脏射血分数EF的变化优于对照组,差异有统计学意义(P<0.05),见表9。
②观察组治疗后心脏彩超显效8例,有效6例,无效1例,总有效人数为14例,总有效率为93.3%;对照组治疗后心脏彩超显效0例,有效3例,无效12例,总有效人数为3例,总有效率为20.0%。其中观察组总有效人数及显效人数均高于对照组,差异有统计学意义(P<0.05)。

从典型病例及表9可知,本发明提供的营养支持剂具有良好的临床效果。
应当理解的是,上述实施方式仅为达到目的的一种方式,本发明的保护范围不仅为上述实施方式,对于本领域技术人员来说,可以很容易的改变一种或者多种元件的结构特点来达到相同试验目的,皆因涵盖在本发明的保护范围之内,故本发明的保护范围应当以权利要求书的保护范围为准,即把权利要求的相同含义和范围内的所有变化包含在本发明之内。
Claims (3)
1.一种营养支持剂,活性成分由左卡尼丁20份、D-核糖40份、辅酶Q10 0.5份、瓜氨酸5份、甘氨酸钾0.5份、甘氨酸镁1份、维生素C1份组成。
2.一种如权利要求1所述的营养支持剂的制备方法,步骤如下:
(1)、将各原料与辅料粉碎至80-200目;
(2)、将上述原料和辅料共同置于混料罐中,充分混匀;
(3)、将各种原辅料经过30s紫外线轨道杀菌,然后进行分装,并用75%的酒精对外包装进行清洁消毒。
3.如权利要求1所述营养支持剂在用于治疗慢性心力衰竭、冠心病、心肌病、心率失常、高血压的药物的制备的用途。
