Claims (36)
- 1.その長さに平行な縦の軸を有し、且つ遠位チップを有する、すべての心臓の チャンバーに入り且つナビゲートする、一般的に高フレキシブルな管状遠位カテ ーテルセクシヨン、 遠位チップ近傍の管状遠位カラーテルセクションによって担われた電極手段、遠 位カラーテルセクションの動きを、遠位チップが入ったチャンバーの壁の全部分 にアクセスできるような方法で全方向に行い且つ制御する二要素バイプラナー制 御システムを含み、そして、該二要素バイブラナー制御システムがさらに、カラ ーテルの遠位チップを制御された方法で、管状カラーテルの縦の軸に実質的に平 行な面で偏向する操作ができる垂直偏向制御要素、カラーテルの遠位チップを制 御された方法で、管状カラーテルの縦の軸に実質的にパーペンディキュラーな面 で偏向する操作ができる横の偏向制御要素を含む心臓の不整脈切除用カラーテル 。
- 2.該電極手段がマッピング及び切除電極手段として操作可能に機能する請求項 1の装置。
- 3.該遠位カラーテルセクションの少なくとも一つの部分が事前決定され、特別 化された形状を想定するように形成された請求項1の装置。
- 4.該横の偏向制御ワイヤが遠位カラーテル内腔を横切るセグメント中でより大 きなフレキシビリティーを有することを特徴とする請求項2の装置。
- 5.垂直制御要素及び横の制御要素がワイヤ要素である請求項1の装置。
- 6.遠位カラーテルセクションの遠位チップ部分がさらに、遠位カソードセクシ ョンの遠位チップに接触した内部心臓組織の電気的活性を示すインパルスを受け るマッピング電極手段、高周波電流を受け、切除エネルギーを接触した組織に供 給する切除電極手段、マッピング及び切除手段に接続した、電気インパルスを受 け転送するインプット/アウトプット手段、及び インプット/アウトプット手段を制御する制御手段を含む請求項1の装置。
- 7.該電極手段を位置づけし且つ安定化することを目的とする遠位カラーテルセ クションの遠位チップの延長として又は延長に付着したテイルセグメントをさら に含む請求項1の装置。
- 8.適切に制御するために、主カラーテル管を伸長し、遠位カラーテル内腔を記 述し且つ稼動遠位チップを有するフレキシプルな、管状遠位カラーテルセクショ ンを有するカラーテルであって、 横の偏向制御要素に回転トルクを与えて対応する遠位チツプの偏向を与えられた 回転トルクの方向に実質的に平行な面で作るように、遠位カラーテル内腔内に通 し、フレキシプルなカラーテルセクションの遠位チップの近傍にアンカーされた 遠位端を有する横の偏向制御要素、及び、該横の偏向制御要素に回転トルクを与 える手段を含むカラーテルチップ制御システム。
- 9.該横の偏向制御要素が、トルク伝達制御ワイヤであり、横の偏向制御ワイヤ への該回転トルク適用手段が、セルフロックで手動回転できるノプである請求項 8の装置。
- 10.セルフロックで手動回転できるノプがさらに、回転できるノプの回転が、 横の偏向制御ワイヤへの回転トルクの対応量を与えるような方法で、横の偏向制 御ワイヤを付着する手段、ノプの回転位置をその解除のときに維持するためのス プリングー口ードしたロックスプライン手段、 横の偏向制御ワイヤに与えられた回転トルクを制限するためのノプの回転をどち らの方向にも制限する回転ノプの回転制限手段、を含む請求項9の装置。
- 11.該遠位カラーテルセクションの少なくとも一つの部分が事前に定められ、 且つ特定化された形状を想定するように形成されている請求項8の装置。
- 12.遠位カラーテル内腔をデスクライプし、適切に制御すると考えられる遠位 チップを有する、高フレキシプルな、管状遠位カラーテルセクションを有するカ テーテノレ、 遠位カラーテル内腔中で縦に沿った垂直偏向制御要素の動きが、回転トルクの方 向に実質的に平行な面でカラーテルの遠位チップを偏向するような方法で、遠位 カラーテル内腔内に通し、フレキシプルなカラーテルセクションの遠位チップの 近傍にアンカーされた遠位端を有する垂直遠位制御要素、垂直偏向制御ワイヤを フレキシプルな遠位カラーテル内腔に沿って縦にディスプレイスする手段、 カラーテル偏向制御要素に回転トルクを与えて対応する遠位チップの偏向を与え られた回転トルクの方向に実質的に平行な面で作るように、遠位カラーテル内腔 内に通し、フレキシプルなカラーテルセクションの遠位チップの近傍にアンカー された遠位端を有する横の偏位制御要素、及び該横の偏向制御要素に回転トルク を与える手段、を含む2要素カラーテルチップ制御システム。
- 13.垂直偏向制御ワイヤをフレキシプルな遠位カラーテル内腔に沿って縦にデ ィスプレイスする手段がさらに垂直偏向制御要素に付着した縦にディスプレイス できるセクションを有する手動操作制御ハンドルで、その縦の動作が垂直偏向制 御要素の対応する縦のディスプレイスをもたらし、それによって遠位チップの垂 直偏向を制御するもの、及び、 該横の偏向制御要素への回転トルクの付与手段が、さらにノプの左右回転が対応 する回転トルクを、遠位チップに回転トルクを伝達しそれによって遠位チップを 所望の横に偏向させる横の偏向制御要素に与えるような方法で、制御ハンドルか らスペースされ、横の偏向制御要素に付着した自己一口ック、手動回転可能横の 調整制御ノプを含む請求項12の装置。
- 14.該垂直制御要素及び横の制御要素がワイヤ要素である請求項12の装置。
- 15.遠位カラーテルセクションの遠位チップがさらに複数のスペースされたマ ッピング及び切除に適当な電極を含む請求項12の装置。
- 16.遠位カラーテルセクションの遠位チップがさらに、遠位カラーテルセクシ ョンの遠位チップに接触した内部組織の電気活性を示すインパルスを受ける複数 の順次にスペースされたマッピング電極手段、高周波電流を受け、接触組織に切 除エネルギーを供給する複数の順次にスペースされた切除電極手段、 マッピング及び切除手段に接続した、電気インパルスを受け転送するインプット /アウトプット手段、及び 各電極へのインプット/アウトプットを別々に制御する制御手段、を含む請求項 12の装置。
- 17.マッピング電極手段及び切除電極手段が同一の電極により達成される同一 の機能である請求項15の装置。
- 18.該遠位カラーテルセクションの少なくとも一つの部分が事前に定められ、 且つ特定化された形状を想定するように形成された請求項15の装置。
- 19.該縦の偏向制御要素が制御ワイヤであり、該横の偏向制御要素が制御ワイ ヤであり、 円筒状リセスを有する及び円筒状リセス中で縦にディスプレイス可能なピストン を有する手動操作制御ハンドルで、そのレシプロ的な動作が垂直偏向制御ワイヤ の対応する縦のディスプレイスをもたらし、それによって遠位チップの垂直偏向 を制御する、及び 該横の偏向制御要素への回転トルクの付与手段がさらに横の偏向制御ワイヤに接 続した内部ピストンギアに関連した回転環ギア手段、環ギアの回転がピこオンギ アの対応する反対の回転をもたらし、ピニオンギアの回転が横の偏向制御ワイヤ に対応する量の回転トルクを与え、該トルクを遠位チップに伝達し該トルクを制 御に翻訳して、遠位チップを所望するだけ横に偏向するような方法で、横の偏向 制御ワイヤに接続した内部ピこオンギアに関連した回転可能環ギア手段を含む請 求項12の装置。
- 20.環ギアの回転がピニオンギアの対応する多数の回転をもたらす請求項19 の装置。
- 21.ピストンメンバーと円筒状リセスとの間にシーリング手段を含む請求項1 9の装置。
- 22.電極手段を位置づけし且つ安定化することを目的とする遠位カラーテルセ クションの遠位チップの延長として又は延長に付着したテイルセグメントをさら に含む請求項12の装置。
- 23.遠位カラーテル内腔をデスクライプし、適切に制御すると考えられる遠位 チップを有する、高フレキシプルな、管状遠位カラーテルセクションを有するカ ラーテル、 遠位カラーテル内腔中でディポーズされ且つエクステンドし、フレキシプルなカ ラーテルセクションの遠位チップの近傍にアンカーされそして遠位端を有する遠 位カラーテル内腔中で縦に沿った制御手段のレシプロ的運動が、制御手段の運動 の方向に実質的に平行な垂直面内でカラーテルの遠位チップを偏向するような方 法で、そして制御手段への回転トルクの適用が与えられた回転トルクの方向に実 質的に平行な横の面内でカラーテルの遠位チップの対応する偏向を生み出すよう な方法で操作可能な、垂直及び横の単一制御手段、該偏向制御ワイヤをフレキシ プルな遠位カラーテル内腔に沿って縦にディスプレイスする手段、及び 偏向制御手段に回転トルクを与える手段を含む2要素カラーテルチップ制御シス テム。
- 24.該制御要素が制御ワイヤである請求項23の装置。
- 25.該遠位カラーテルセクションの少なくとも−つの部分が事前に定められ、 且つ特定化された形状を想定するように形成された請求項23の装置。
- 26.遠位カラーテルセクションの遠位チップがさらに、遠位カラーテルセクシ ョンの遠位チップに接触した内部心臓組織の電気活性を示すインパルスを受ける 複数の順次にスペースされたマッピング電極手段、高周波電流を受け、接触組織 に切除エネルギーを供給する複数の順次にスペースされた切除電極手段、 マッピング及び切除手段に接続した、電気インパルスを受け転送するインプット /アウトプット手段、及び 各電極へのインプット/アウトプットを別々に制御する制御手段、を含む請求項 23の装置。
- 27.該遠位カラーテルセクションの少なくとも一つの部分が事前に定められ、 且つ特定化された形状を想定するように形成された請求項26の装置。
- 28.電極手段を位置づけし畳っ安定化することを目的とする遠位カラーテルセ クションの遠位チップの延長としての又は延長に付着したテイルセグメントをさ らに含む請求項26の装置。
- 29.マッピング電極手段及び切除電極手段が同一の電極手段である請求項27 の装置。
- 30.該遠位カラーテルセクションの少なくとも一つの部分が事前に定められ、 且っ特定化された形状を想定するように形成された請求項28の装置。
- 31.垂直偏向制御ワイヤをフレキシプルな遠位カラーテル内腔に沿って縦にデ ィスプレイスする手段がさらに、 垂直にディスプレイスできるリセス、シリンダー状リセス中でディスポーズされ 且つレシプロ的に操作可能でかつ制御ワイヤに付著したレシプロ的ピストン手段 を含む制御ハンドル手段で、ピストンの縦の動作が垂直偏向制御要素の対応する 縦のディスプレイスをもたらし、それによって遠位チップの垂直偏向を制御する ハンドル手段、及び ここで、ハンドル手段に関するピストン手段の回転が制御ワイヤに遠位チップに 伝達し、それによって遠位チップを所望の横のに対応して偏向する回転トルクを 与えるように、該ピストン手段がハンドル手段に関して手動回転可能な関係で保 持されるピストン手段を含む請求項12の装置。
- 32.ピストン手段が、ピストン手段の動きを制御する外部の、手動操作可能な ピストンハンドル手段をさらに含む請求項31の装置。
- 33.ピストン手段の予期しない相対的縦の動作を防止するロック手段をさらに 含む請求項31の装置。
- 34.ハンドル及びピストン手段に関連して相対回転制限手段をさらに含む請求 項31の装置。
- 35.すべての心臓のチャンバーに入り且つナビゲートし、マッピング及び切除 チップ電極システムを担う遠位チップセグメントを有する高フレキシプルな管状 遠位カラーテルセクション、 遠位カラーテルセクションの動きを、遠位チップが入ったチャンバーの壁の全部 分にアクセスできるような方法で全方向に向けさせ且つ制御する2ワイヤ2平面 制御システム、及び 該2ワイヤ2平面制御システムがさらに、カラーテルの遠位チップを制御された 方法で管状カラーテルの縦の軸に実質的に平行なプレーンで偏向する操作ができ る垂直偏向制御ワイヤ、カラーテルの遠位チップを制御された方法で管状カラー テルの縦の軸に実質的にパーペンディキュラーなプレーンで偏向する操作ができ る横の偏向制御要素を含む偏向できる心臓のマッピング及び切除カラーテルシス テムを導入し、垂直偏向制御ワイヤ及び横の偏向制御ワイヤの張力を調整して目 的のチャンバーの組織表面をアクセス及びマップすべく電極システムを偏向及び ステアし、チップ電極システムを操作して指示された早期活性部位を切除する、 工程を含む電気的心臓の脱分極ポテンシャルを写し且つ心臓の組織を切除する方 法。
- 36.高フレキシプルな円筒状遠位カラーテルセクションの遠位チップセグメン トが事前に決定された特定の形状で形成され、且つさらに該事前に決定された形 状をマこ一バーして特異的にデザインされた心臓の領域の組織表面をマップする 工程をさらに含む請求項35の方法。
