Claims (1)
- 【特許請求の範囲】 1.生体身体(3)の組織(8)の治療装置であって、 少なくとも1つの治療超音波交換器(1、2、59)と、超音波変換器(1、2 、59)により形成された超音波を生体身体(3)に入力結合するための手段( 29)と、超音波の治療作用領域(W)と生体の身体(3)とを相対的相互に移 動するための手段(22、23、32、34、61)と、超音波強度、有利には 超音波の振幅を測定するための手段(34、35、47、48、57)とを備え 、 前記超音波変換器(1、2、59)は、治療作用領域(W)を有する超音波を送 出し、 前記超音波強度測定手段(34、35、47、48、57)は、作用領域(W) の瞬時の位置に依存しないで、治療すべき組織(8)の領域中の超音波強度を測 定し、 超音波の治療作用領域(W)と生体身体(3)との相対的相互の移動は、超音波 強度測定手段(34、35、47、48、57)によって測定された超音波強度 に基づき、治療すべき組織(8)の少なくとも一部が超音波の治療作用領域(W )内にあるように行うことを特徴とする治療装置。 2.前記超音波強度測定手段(34、35、47、48、57)は少なくとも1 つのハイドロフォン(47、48)を有する請求の範囲第1項記載の治療装置。 3.超音波の治療作用領域(W)中の超音波強度を調整するための手段(32、 34)が設けられている請求の範囲第1項または第2項記載の治療装置。 4.治療すべき組織(8)の領域の温度を測定するための手段(34、35、3 8、39、57)が設けられており、該手段は少なくとも1つの温度センサ(3 8、39)を有する請求の範囲第1項から第3項までのいずれか1項記載の治療 装置。 5.超音波強度測定手段(34、35、47、48、57)により測定された超 音波強度を目標値と比較するための手段(34)および/または温度測定手段( 34、35、38、39、57)により測定された温度を目標値と比較するため の手段(34)が設けられている請求の範囲第3項または第4項記載の治療装置 。 6.治療すべき組織(8)に少なくとも隣接する組織領域を冷却するための手段 (35、44、45、46)が設けられている請求の範囲第1項から第5項まで のいずれか1項記載の治療装置。 7.超音波の治療作用領域(W)中の超音波強度を調整するための手段(32、 34)によって超音波強度は、治療すべき組織(8)が温熱治療法で通常の温度 よりも高い温度にもたらされるように調整される請求の範囲第4項から第6項ま でのいずれか1項記載の治療装置。 8.少なくとも2つの治療超音波変換器(1、2、59)が設けられており、当 該超音波変換器は超音波をそれぞれ主伝播方向に送出し、 当該超音波変換器は、その主伝播方向が相互に交差し、当該超音波変換器から送 出された超音波が治療すべき組織(8)の領域で重畳するように配置されている 請求の範囲第1項から第7項までのいずれか1項記載の治療装置。 9.超音波変換器(1、2、59)からそれぞれ発射される超音波の強度は、治 療作用領域(W)が超音波の重畳される領域内にあるように選択される請求の範 囲第8項記載の治療装置。 10.超音波変換器(1、2、59)は超音波を集束して放射し、それぞれのフ ォーカスゾーンは超音波の重畳領域にある請求の範囲第8項または第9項記載の 治療装置。 11.フォーカスゾーンは少なくとも一部相互に重なっている請求の範囲第10 項記載の治療装置。 12.フォーカスゾーンは種々異なる寸法を有し、および/または種々異なる形 態である請求の範囲第10項または第11項記載の治療装置。 13.比較的に小さなフォーカスゾーンと比較的に大きなフォーカスゾーンとが 設けられており、超音波の治療作用領域(W)と生体身体(3)との相対的相互 の移動は、比較的に小さなフォーカスゾーンが比較的に大きなフォーカスゾーン 内で移動されるように行われる請求の範囲第12項記載の治療装置。 14.超音波の重畳によって治療作用領域(W)は少なくとも実質的に球形状に となる請求の範囲第8項から第13項までのいずれか1項記載の治療装置。 15.超音波変換器(1、2、59)は種々異なる周波数の超音波を放射する請 求の範囲第8項から第14項までのいずれか1項記載の治療装置。 16.超音波変換器(1、2、59)からそれぞれ発射される超音波の振幅は超 音波の重畳領域で種々異なる大きさである請求の範囲第8項から第15項までの いずれか1項記載の治療装置。 17.超音波変換器(1、2、59)のアパーチャ開口角は種々異なる大きさで ある請求の範囲第8項から第16項までのいずれか1項記載の治療装置。 18.超音波変換器(1、2、59)は種々異なる集束度を有する請求の範囲第 8項から第17項までのいずれか1項記載の治療装置。 19.超音波変換器(1、2、59)は移動手段(22、23、32、34、6 1)によって、2つの相互に有利には直角に交差する軸線(A,B)を中心にし て旋回可能である請求の範囲第1項から第18項までのいずれか1項記載の治療 装置。 20.治療超音波変換器(59)は直腸適用のために設けられたゾンデ(60) に収容されている請求の範囲第1項から第19項までのいずれか1項記載の治療 装置。 21.超音波変換器(59)は移動手段(32、34、61)によって、ゾンデ (60)の長手軸線を中心に回転可能であり、ゾンデ(60)の長手軸線に対し て有利には直角に交差する軸線を中心に旋回可能である請求の範囲第20項記載 の治療装置。 22.治療装置は良性前立腺肥大を治療するために設けられており、 超音波強度測定手段(34、35、47、48、57)および/または温度測定 手段(34、35、38、39、57)および/または冷却手段(35、44、 45、46)は尿道に挿入されるカテーテル(35)に、および/または直腸に 挿入される測定ゾンデ(57)に組み込まれている請求の範囲第1項から第21 項までのいずれか1項記載の治療装置。 23.超音波変換器(1、2、59)は緩和された強度のパイロット信号を送出 するために制御され、超音波変換器(1、2、59)と生体身体((3)との相 対的相互の配向はパイロット信号に基づいて行われる請求の範囲第1項から第2 2項までのいずれか1項記載の治療装置。 24.それそれ1つの超音波変換器(1、2、59)だけが同時にパイロット信 号の制御のために制御される請求の範囲第23項記載の治療装置。 25.直腸適用に適した超音波診断ゾンデ(52)が設けられており、 該超音波診断ゾンデ(52)によって2つの相互に交差する、治療すべき組織( 8)の領域を含む生体の身体層((X,Y)の超音波断層画像を形成することが でき、 超音波の治療作用領域(W)と生体身体(3)との相対的相互の移動は、超音波 診断ゾンデ(52)によって形成された超音波断層画像を考慮して行う請求の範 囲第1項から第24項までのいずれか1項記載の治療装置。 26.少なくとの1つの超音波変換器(1)には診断超音波アプリケータ(64 )が組み込まれており、該診断超音波アプリケータ(64)により超音波の治療 作用領域を含む生体の身体層の超音波断層面像を形成することができ、 超音波の治療作用領域(W)と生体身体との相対的相互の移動は、診断超音波ア プリケータ(64)により形成された超音波断層画像を考慮して行われる請求の 範囲第1項から第25項までのいずれか1項記載の治療装置。 27.生体身体(3)、例えば泌尿生殖器領域および/または腸領域の組織(8 )の治療装置であって、治療作用領域(W)を有する超音波を送出する少なくと の1つの治療超音波変換器(1、2)と、該超音波変換器(1、2)により形成 された超音波を生体身体(3)へ入力結合するための手段(29)と、超音波の 治療作用領域(W)と生体身体(3)とを相対的相互に移動するための手段(2 2、23、32、34)と、直腸適用に適した超音波診断ゾンデ(52)と治療 作用領域(W)と超音波診断ゾンデ(52)との相互の空間的配置関係を検出す るための手段とを有し、 前記超音波診断ゾンデ(52)によって2つ相互に交差する、治療すべき組織( 8)の領域を含む生体の身体層(X,Y)の超音波断層画像が形成され、超音波 の治療作用領域(W)と生体身体(3)との相対的相互の移動は、超音波診断ゾ ンデ(52)により形成された超音波断層画像に基づき、検出された治療作用領 域(W)と超音波診断ゾンデ(52)との相互の空間的配置関係を考慮して、治 療すべき組織(8)の少なくとも一部が超音波の治療作用領域(W)にあるよう に行うことを特徴とする治療装置。 28.生体身体(3)、例えば泌尿生殖器領域および/または腸領域の組織(8 )の治療装置であって、少なくとも2つの治療超音波変換器(1、2、59)と 、超音波変換器(1、2、59)により形成された超音波を生体身体(3)へ入 力結合するための手段(29)とを備え、 前記治療超音波変換器(1、2、59)は超音波をそれぞれ主伝播方向に送出し 、 前記治療超音波変換器(1、2、59)は、それらの主伝播方向が相互に交差し 、かつ当該治療超音波変換器(1、2、59)から発射された超音波ガ重畳する ように配置されしてお、 超音波変換器(1、2、59)からそれぞれ発射された超音波の超音波強度は、 超音波の治療作用領域(W)が超音波の重畳する領域内に生じるように選択し、 比較的小さなフォーカスゾーンと比較的大きなフォーカスゾーンとが設けられて おり、 超音波の作用領域(W)と生体身体(3)との相対的相互の移動は、比較的小さ なフォーカスゾーンを比較的大きなフォーカスゾーン内で、治療すべき組織(8 )の少なくとも一部が超音波の治療作用領域(W)内にあるように移動させて行 うことを特徴とする治療装置。 29.超音波変換(1、2、59)の入力結合は、腹部おにび/または会陰およ び/または直腸で行う請求の範囲第1項から第28項までのいずれか1項記載の 良性前立腺肥大の治療装置のための駆動方法。 30.腹部適用は超音波領域の側方から行う請求の範囲第29項記載の駆動方法 。
