気泡の造影剤を得る方法。 2 請求の範囲第1項に記載の方法において、前
記溶液は粘性のある水溶液であることを特徴とす
る方法。 3 請求の範囲第1項または第2項に記載の方法
において、前記微小気泡の直径がほぼ6〜20ミク
ロンの範囲であることを特徴とする方法。 4 請求の範囲第1項、第2項または第3項に記
載の方法において、前記超音波処理にはほぼ
20000Hzのエネルギーが用いられることを特徴と
する方法。 発明背景 発明の分野 本発明は、超音波映像発生技術の分野に関する
ものであり、更に詳しくいえば、超音波微小気泡
の造影剤を得る方法に関するものである。 先行技術の説明 診断および治療の補助となるように動物または
人体ような哺乳類の部分を映像化して示す各種の
技術が存在する。 最も良く知られている映像発生技術のうちの1
つは、動物の体および人体内部の骨格構造および
内部構造を視覚化するために、X線を用いるもの
である。しかし、X線の使用には、いくつかの問
題がある。第1に、体の部分によつては、X線を
安全に照射できない。また、照射量が過大である
と、X線は危険である。更に、吸収されたX線
は、一生にわたつて累積される。最後に、X線は
骨格構造およびその他の内部構造の映像を発生で
きるが、ある種の器官系および血管の詳細を見る
ためには、X線はあまり満足できるものでないこ
とが判明している。 別の広く用いられている技術は、血管造影法で
ある。この血管造影法では、X線に対して不透明
な色素が動脈内に注入される。色素が流れている
動脈は際立つて視覚化されるから、主な大動脈お
よびそれらの主要な岐脈の像を得るためにX線を
使用できる。しかし、血管造影法では、組織の満
されていない局所貧血領域、心臓の筋肉、また
は、微小循環系は、視覚化できない。また、ある
種の血管造影法による観察は、使用する装置と、
レンズの場所および角度、オペレータ技能、およ
び、それらに類似の要因に応じて変り得る測定値
を基にして行われる。その上に、血管造影法は、
静脈ではなくて動脈の中にカラーテルを入れる必
要があるから、有害である。入院を必要とするだ
けでなく、血管造影法には危険性がある。 しばしばラジオ−ニユウクライド映像発生
(radio―nuclide imaging)と呼ばれる別の技術
では、タリウムのような放射性物質を血流中に注
入する。この技術は、血管造影法とは異なつて動
脈への注入の必要はないが、非常に高度で高価な
機械を使用する必要がある。更に、ラジオ―ニユ
ウクライド映像発生は、心臓の限られた数面の像
を発生し、それらの像は例外的に明確でないもの
がある。最後に、この種の放射は一生を通じて累
積し、したがつて危険性がある。 最近、体の種々の部分を超音波により映像で示
す技術が進歩してきた。それらの技術は、とくに
心臓に対して用いられた場合には、「エコーカー
ジオグラフイ(echocardiography)」として知ら
れている。音波の発生および受波のために、超音
波スキヤナが用いられる。超音波スキヤナは、映
像化すべき領域の外部の体表面上に置かれる。ス
キヤナで発生された音波は、映像化したい領域へ
と向けられる。スキヤナは、領域内部から反射さ
れた音波を検出し、そのデータを映像に変える。 そのような超音波スキヤナはこの分野で知られ
ているが、本発明を一層良く説明するために、そ
れについて簡単に説明する。超音波エネルギーが
物質を通じて送られるとき、その物質の音響特性
は、伝送速度と物質の密度に依存する。物質の音
響特性((すなわち音響インピーダンス)の変化
は、異なる物質(すなわち、固体、液体および気
体)の境界において最も著るしい。したがつて、
超音波物質が種々の媒体を通じて送られると、音
響特性の変化により反射特性が変化して、より強
い反射音響信号が超音波スキヤナにより受信され
る結果となる。 エコーカージオグラム(echocardiogram)の
ような初期の超音波映像技術は、明瞭さに欠けて
いた。そのため、超音波スキヤナおよび関連する
機器を改良するために広範な努力が払われてき
た。また、1968年の初めに、より明瞭なすなわち
「強められた」超音波映像を得るために、血流中
に「コントラスト」剤が注入された。従来のコン
トラスト剤は、微細な気泡を含む液体であつた。
その気泡は、時にはゼラチンまたは糖質のカプセ
ルの中に封じられ、または、時には種々の混合液
体を機械的にかき回す、すなわち、手で振る、こ
とにより発生された。他の従来のコントラスト剤
がアカデミツク・プレス社(Academic Press,
Inc.)により1980年に発行された超音波映像発生
(UItrasonic Imaging)におけるオーフアイア
(J.Ophir)他の「コラーゲン微小球体により発生
されたコントラストからの超音波後方散乱
(Ultrasonic Backscatter from Contrast
Produced by Collagen Microspheres)」と題す
る論文に示されている。 コントラスト剤自体は、液体とその中に溶解さ
れている微小気泡との間の音響的な差のために、
強列な音波反射体となる。したがつて、コントラ
スト剤が組織の微小血管の中に注入されて、それ
らの微小血管を満すと、その組織の明らかな映像
を発生できる。しかし、そのようなコントラスト
剤を使用するにもかかわらず、たとえば心筋組織
の発生された映像は、比較的低品質で、極めて変
化しやすく、定量化し難い。なぜなら、従来の微
小気泡は、寸法と持続性が変化するからである。 発明の概要 本発明の一実施例は、より小さくて、より一様
な微小気泡を作ることより、従来のコントラスト
剤の改良を目ざすものである。 本発明のコントラスト剤は、(1)エコーを発生す
る(すなわち、音響を反射できる)、(2)末梢静脈
側に注入された従来のコントラスト剤が入ること
ができなかつた組織を満すように、毛細血管を通
るために十分に小さく、それによりそのような組
織および器官の強調された映像を生じて、良く満
された組織と、良く満されていない組織を区別で
きるようにし、(3)定量的に再現できるものであ
る。 本発明の方法は、(1)従来の超音波技術を用いて
は映像を得ることができなかつた器官系の映像を
生じさせることができ、(2)そのような先行技術を
用いて見ることができたある部分はそれをより明
確かつより細かに映像化できる。 本発明の実施例においては、粘性のある溶液
(たとえば、70%ブドウ糖(Dextrose)50%ブド
ウ糖、70%ソルビトール(Sorbitol)、レノグラ
チン(Renogratine)―76、それらの物質の混合
物など)に、5000〜30000Hzの高周波超音波物質
をかける。その結果、哺乳類に注入できる微小気
泡が、発生させられる。参照を容易にするため
に、そのような微小気泡をここでは「超音波で処
理した(sonicated)」微小気泡と呼ぶことにす
る。以下に詳しく説明するように、そのような超
音波で処理された微小気泡は、改良されたコント
ラスト剤であることが見出された。 本発明は、先行技術に伴う問題の多くを解決し
て、種々の器官系の独特の映像の発生を可能にす
るものである。本発明の技術は種々の動物および
人の身体の器官系に適用できるが、それの新規な
諸特徴と諸利点は、心筋組織および潅流すなわち
血流パターンの映像を得るのに本発明を使用する
ことについての以下の説明から、より良く理解で
きる。説明と添附図面は、説明だけを目的とする
もので、それらの説明および図面は発明の範囲を
定めることを意図するものではないことを明確に
理解されたい。 説明を読み、図面を見る際には、心臓は多くの
血管により養なわれている「ポンプ」であり、そ
れらの血管は、時間が経過するにつれて、部分的
または全面的に詰るようになることがあり、それ
により心臓の組織に損傷を与える可能性に心され
たい。過去においては、心臓組織についての情報
はラジオ―ニユウクライド映像発生法または手術
を用いて得られていた。発生された血管造影像

な血管および心臓の壁の動きについて得られたデ
ータから推定することを必要とした。

る(すなわち、音響を反射できる)、(2)末梢静脈
側に注入された従来のコントラスト剤が入ること
ができなかつた組織を満すように、毛細血管を通
るために十分に小さく、それによりそのような組
織および器官の強調された映像を生じて、良く満
された組織と、良く満されていない組織を区別で
きるようにし、(3)定量的に再現できるものであ
る。 本発明の方法は、(1)従来の超音波技術を用いて
は映像を得ることができなかつた器官系の映像を
生じさせることができ、(2)そのような先行技術を
用いて見ることができたある部分はそれをより明
確かつより細かに映像化できる。 本発明の実施例においては、粘性のある溶液
(たとえば、70%ブドウ糖(Dextrose)50%ブド
ウ糖、70%ソルビトール(Sorbitol)、レノグラ
チン(Renogratine)―76、それらの物質の混合
物など)に、5000〜30000Hzの高周波超音波物質
をかける。その結果、哺乳類に注入できる微小気
泡が、発生させられる。参照を容易にするため
に、そのような微小気泡をここでは「超音波で処
理した(sonicated)」微小気泡と呼ぶことにす
る。以下に詳しく説明するように、そのような超
音波で処理された微小気泡は、改良されたコント
ラスト剤であることが見出された。 本発明は、先行技術に伴う問題の多くを解決し
て、種々の器官系の独特の映像の発生を可能にす
るものである。本発明の技術は種々の動物および
人の身体の器官系に適用できるが、それの新規な
諸特徴と諸利点は、心筋組織および潅流すなわち
血流パターンの映像を得るのに本発明を使用する
ことについての以下の説明から、より良く理解で
きる。説明と添附図面は、説明だけを目的とする
もので、それらの説明および図面は発明の範囲を
定めることを意図するものではないことを明確に
理解されたい。 説明を読み、図面を見る際には、心臓は多くの
血管により養なわれている「ポンプ」であり、そ
れらの血管は、時間が経過するにつれて、部分的
または全面的に詰るようになることがあり、それ
により心臓の組織に損傷を与える可能性に心され
たい。過去においては、心臓組織についての情報
はラジオ―ニユウクライド映像発生法または手術
を用いて得られていた。発生された血管造影像
は、組織にいての直接的なデータを発生せず、主
な血管および心臓の壁の動きについて得られたデ
ータから推定することを必要とした。
以下、添附図面を参照して本発明を説明する。
第1図はエコーカージオグラフイにおいて超音
波スキヤナを用いる様子を示す略図、 第2図乃至第5図は心臓の中を流れるコントラ
スト剤を用いて鮮やかにされた心臓の断面図であ
る。

第1図は、心臓および肺の略図と、スキヤナ1
0および映像発生装置12より成る超音波走査装
置の略図である。この超音波走査装置は、所定の
領域、この場合には人体の心臓領域の視覚映像を
生ずる。典型的には、スキヤナ10は、映像化す
べき部分16の上方の皮膚14の上に直接置かれ
る。スキヤナ10の中には、超音波トランスデユ
ーサを含めて種々の電子部品が納められる。スキ
ヤナ10は、心臓領域16を扇形に走査する超音
波18を発生する。超音波18は心臓領域16の
種々の部分により反射され、その反射波は発生ト
ランスデユーサにより受けられてから、この分野
で知られているパルスエコー法に従つて処理され
る。処理の後、表示のために信号は映像発生装置
12(これもこの分野で知られている)へ送られ
る。 患者が「準備され」、スキヤナ10が所定場所
に置かれた後で、本発明に従つて超音波で処理さ
れた微小気泡コントラスト剤が、たとえば24で
全体的に示されている腕の静脈を通じて注入され
る。コントラスト剤は静脈24の中を矢印26の
向きへ流れ、心臓30の右静脈側28と、肺32
へ導かれる主肺動脈29を通り、肺32を毛細血
管33を通つて横切り、肺静脈35に入り、最後
に左心房36に入り、心臓30の左心室37を出
る。超音波で処理された微小気泡は、強いコント
ラストの領域を有する映像を生ずることが見出さ
れた。どのような理論によつても束縛されるもの
ではないが、本発明の超音波で処理された微小気
泡は、従来のコントラスト剤と比較して、心筋組
織および微小血管の著るしく明確で、詳しく映像
を生ずる。 次に第2図乃至第5図を参照する。これらの図
から、本発明の微小気泡コントラスト剤を用いて
発生されたコントラスト―エコーカージオグラム
を見ることができる。図において、馬蹄形部分5
0は、左心室(left ventricular cavity)37を
囲む左心室壁筋肉(または組織)を表す。微小気
泡は犬の肺動脈に注入され、肺の毛細血管床を横
切つて左心房36と左心室37に入り、冠動脈を
通つて大動脈に入り、最後に左心室組織50に入
つてそれの映像を強調する。 とくに、第2図は、超音波で処理した微小気泡
を入れる前の左心室37と左心房36の二次元エ
コーカージオグラム(「2―DE」)映像を示す。
第3図は、挿入された肺動脈カテーテルを通じて
10mlの超音波で処理したレノグラフイン
(Renografin)/NaClの混合物注入した様子を
示す。図からわかるように、コントラスト剤は、
左心房36に現われ、左心室37へ流れる。第4
図においては、左心室37のほぼ完全な不透明化
が生じている。第5図においては、心筋組織50
の引き続く不透明化が見える。その理由は、コン
トラスト剤を運ぶ血液が、心筋組織50へ血液を
供給する冠状動脈へ大動脈を通じて流れたからで
ある。したがつて、血液が肺を通るのに要した時
間、血液流パターン、左心房の寸法、僧帽弁(こ
れは左心室と左心室を隔てている)の能力、左心
室の室寸法、壁運動の異常についての観察と診断
を行うことができる。コントラスト剤が左心室か
ら放出されると、大動脈弁の能力も解析でき、か
つ、左心室から放出された量の放出割合も解析で
きる。最後に、組織のコントラスト・パターン
は、どの領域(もしあれば、)が適切に満されて
いないかを示す。 要約すると、そのような映像パターンは、心臓
内部の異常な血流特性、弁の能力、心室の寸法お
よび壁の運動をを診断する際の補助をなし、心筋
潅流(myocardial perfusion)を潜在的に指示
するものである。 本発明においては、微小気泡は、レノグラフイ
ン―76(この分野で周知であり、毒性が比較的な
く、生体に適合し、放射線を通さない色素)と塩
(saline)とを1対1の割合で混合したもので作
られる。この混合物を超音波で処理する、すなわ
ち、ヒート・システム(Heat System)375ワツ
ト超音波処理器により約30秒間高周波エネルギー
を照射する。そのような超音波処理器は、他の用
途のために良く知られているもので、20000Hzの
超音波エネルギー通常放出する。もつとも5000〜
30000Hzまたはそれより高いエネルギーも本発明
の範囲内である。上記混合物、砂糖溶液などのよ
うな選択されたコントラスト剤に応じて、寸法の
異なる気泡が発生されるが、通常は、直径が約6
〜20ミクロンの希望の範囲である。 先に簡単に述べたスキヤナ10の外に、他の超
音波スキヤナもある。その例が米国特許第
4143554号、第4315435号に開示されている。基本
的には、それらの米国特許は、動いている器官の
動的な視覚化を可能とするに十分なフレーム速度
で、超音波エネルギーにより動脈または人の解剖
学的構成の断層面の二次元映像を逐次発生するた
めの、動的断面図エコーグラフイー
(dynamiccross―sectional echography,DCE)
を含む種々の技術に関するものである。DCEに
おいて用いられる種類の装置は、一般にDCEス
キヤナと呼ばれ、細いビームすなわち線状の短い
音響パルスを送り、かつ受ける。反射信号の強さ
は、時間の関数として変化する。その時間軸を、
公称音速を用いて位置軸へ変えて反射信号の強さ
を、レーダまたはソナーの表示器に多少類似する
やり方で陰極線管その他の適当な装置上に表示す
る。DCEは、肝臓、胆のう、膵臓、腎臓を含む
多くの器官の映像を発生するために使用できる
が、心臓の組織と主な血管を視覚化するためにも
DCEはしばしば用いられる。 従来のDCEスキヤナの分類は、その視野(直
線走査または扇形走査)、その視野を走査するた
めに用いられる手段(機械的走査または電子的走
査)、および、トランスデユーサが水浴を介して
患者または物体を走査するか、適切な接触ゲルま
たは接触油を用いて物体の表面たとえば患者の皮
膚に直接接触して走査するかに従つて、行うこと
ができる。直線スキヤナは、装置に用いられるト
ランスデユーサにより主として決定される各走査
線の実効幅にほぼ匹敵する線間隔で互いに移動さ
せられる1組のほぼ平行な走査線より成る組織の
走査を行う。したがつて、そのような走査装置に
より映像化された断面の形は、ほぼ長方形であ
る。その幅は線の総数とにより決定され、深さは
超音波エネルギーの組織内への浸透深さにより決
定される。直線スキヤナは、腹部の臓器における
ような、そこから解剖学の対象とする部分への接
近が可能であるような、体表面の比較的広がつた
領域が存在するような部位で一般に用いられる。 扇形スキヤナは、互いにある角度で隔てられて
いるが、1点で交わる(公称)扇形状に拡がる線
より成る解剖学的組織の走査を生ずる。角度間隔
は装置に応じて一定または不定で、各線の実効角
度幅にほぼ匹敵する。扇形スキヤナは、成人の心
臓、脳、および、眼におけるような、そこから対
象とする解剖学的部分へ入口が比較的小さいよう
な体表面の解剖学的窓または領域で一般に使用さ
れる。 別の種類の扇形スキヤナは、機械的な性質のも
のであつて、2つの下位種類、振動トランスデユ
ーサ・スキヤナと回転トランスデユーサ・スキヤ
ナとに更に分けることができる。振動トランスデ
ユーサ・スキヤナは、前方平面においてほぼ一線
上にあり、かつトランスデユーサの中心を通る軸
を中心として、1個のトランスデユーサが振動さ
せられるものであつて、任意の時刻におけるトラ
ンスデユーサの角度位置を監視するために適切な
角度センサが用いられる。典型的な回転トランス
デユーサ・スキヤナにおいては、液体が満されて
いるドームの内部にいくつかのトランスデユーサ
がピンで留められ、1度に1個のトランスデユー
サが対象領域を走査する。それらのスキヤナおよ
びその値のスキヤナを本発明に組合わせることが
できる。 前記したように、調査対象である組織の強調さ
れた超音波映像を再生するために使用する安全
で、再現性ある、定量化可能なコントラスト剤を
探すために、研究者たちは、平均寸法が約75ミク
ロンで、窒素または二酸化炭素ガスがサツカリン
またはゼラチンの中に封入された微小気泡、液体
中に圧入されたガス(たとえばH2O2)、および、
液体溶液の機械的に撹拌された(手で振動させ
た)混合物を使用してきた。しかし、肺動脈の毛
細血管の直径は約8〜10ミクロンであるから、75
ミクロンの封入された微小気泡は毛細血管を通る
ことができず、その結果として、そのような微小
気泡を使用するには、映像化すべき部分へ微小気
泡を直接注入することを必要とし、あるいは、前
記した血管造影法の浸襲的やり方と同じ危険を含
む動脈直接注入を必要とする。更に、種々の液体
を超音波で処理する以外の、撹拌することにより
作られる微小気泡は、寸法が大幅に分散する。そ
のように、物質中に封入されているのではなく
て、撹拌により生じた微小気泡は、その不定量が
毛細血管の中を通ることができるが、現在の技術
状態では、上記のように不定量を制御できないた
めに、定性的なデータが得られるだけである。そ
れらのコントラスト剤は、ある程度までは十分良
く機能するが、気泡の寸法が一定でないというこ
とを含めて、いくつかの問題がある。それらの問
題およびその他の問題は、本発明の超音波処理さ
れた微小気泡により解決される。 以上本発明を説明したが、その他の変更を当業
者が行えることは明らかである。したがつて、本
発明は、添附した請求の範囲と精神のみによつて
限定されるべきである。

れた微小気泡により解決される。 以上本発明を説明したが、その他の変更を当業
者が行えることは明らかである。したがつて、本
発明は、添附した請求の範囲と精神のみによつて
限定されるべきである。
