Description
[0001] 本出願は、その内容は参照により本明細書に援用される、2017年10月30日に出願された米国仮特許出願第62/579,124号「医療機器用熱可塑性ポリマーシステム」の優先権を主張する。
技術分野
[0002] 本発明の実施形態は、形状記憶ポリマー医療装置の分野である。
[0048] いくつかの実施形態では、血管内塞栓装置及び他の移植可能な組織足場用のX線可視化SMP発泡体システムがあげられる。一実施形態では、モノマーとして造影剤を組み込んだ熱硬化性生体形状記憶ポリウレタン発泡体システムがあげられる。透視技術を用いて、発泡体を所望の生体構造内に移植すると、体積が膨張して組織内で充填されて、塞栓とその後の細胞浸潤及び治癒が促進される。SMPがX線可視化できると、医師は発泡体が膨張する際の真の容積充填を観察しうる。これにより、装置を適当に配置でき、組織に過剰充填されるリスクが低減し、処置安全性が向上する。

[0058] いくつかの実施形態では、脳動脈瘤治療用のX線可視SMP塞栓発泡体材料システムがあげられる。一実施形態では、放射性ヨウ素原子のポリマーネットワークへの取り込みを介してX線造影を行いうる。当該化学的アプローチにより、非可視化配合物の限界に対処することで、SMP塞栓発泡体の用途を広げうる。トリヨードベンゼンを含む造影剤をモノマーとして形状記憶ポリウレタンバックボーンに組み込むと、放射性物質であるポリ尿素アミドを獲得しうる。この造影剤モノマーにより、X線が可視化され、かつ、発泡時に発泡剤として、また、形状記憶をもたらす化学的架橋剤として機能する。

[0065] 出血性脳卒中予防の従来の究極の判断基準は、ベアプラチナコイル(BPC)による頭蓋動脈瘤の治療である。BPCにより、米国における年間の出血性脳卒中の数が約10万3000件まで減少したが、この治療法にはまだ臨床的に制限されている。例えば、臨床的基準では非常に高いとされる充填密度(30~35%)でも、塞栓は完全でない。コイル数を多くして充填密度を高めると、過剰充填や動脈瘤破裂が生じる可能性がある。さらに、初期では有効な塞栓でも、大動脈瘤及び巨大動脈瘤での再疎通率は各々、35%及び50%という高い比率が示される。BPCでの必要なコイル数、手術時間、及び複雑性のため、大動脈瘤の充填に用いる費用は法外である。したがって、より効率的な容積充填のための解決策として、BPCの有効性及び費用限界が望まれる。
[0070] いくつかの実施形態では、質量密度0.015gcm-3の超低密度SMP発泡体と、形状が97%回復しうる高架橋密度を備える超低密度SMP発泡体があげられる。当該特性により、7発泡体の体積膨張が70倍となりえた。当該低密度SMP発泡体を従来のコイリング技術と組み合わせて利用すると、動脈瘤の充填に必要なコイル数を大幅に削減し、処置時間と費用が削減されうる。さらに、SMP発泡体の膨張力は、BPCの適用力よりも大幅に小さく、移植時の動脈瘤剥離のリスクを低減しうる。他のコイル系塞栓装置と同様、塞栓発泡体は血流を乱し、発泡体内で血栓形成を誘導して、動脈瘤を閉塞させる。
[0074] 一実施形態では、SMP重合用に同定された造影剤は、5-アミノ-2,4,6-トリヨードイソフタル酸(ATIPA)である。図2(a)及び(b)に示すように、ATIPA分子のX線造影は、3つの高z型ヨウ素原子を含むトリヨードベンゼンモチーフ由来である。ATIPA分子のX線造影は、3つの高z型ヨウ素原子を備えるトリヨードベンゼンをモチーフにしており、第一級芳香族アミンと2つのカルボン酸で末端が結合し、イソシアネートとの架橋反応に3つの官能基をもたらす。さらに、イソシアネートとカルボン酸との反応により、アミド結合と二酸化炭素が得られ、ATIPAは発泡体重合の化学発泡剤となる。
[0077] ATIPAを重合する障壁は、他のモノマー、特にジイソシアネート類との溶解性が限定されることである。これまでに、ATIPA、DEG、BEP、HDIを用いて様々な組成物を重合し、図3に示すような重合性発泡体を得ることに成功している。当該発泡体は、ATIPA(15%モル当量)ジカルボン酸とジイソシアネートを反応させてブローし、133mgI/mlでX線造影を行った。発泡体密度及び孔径は、予備重合体の水酸当量比の比率を変えて、プレミックス粘度を変化させて制御した。
[0079] いくつかの実施形態では、非イソシアネート成分の20~30mol%でATIPAを変化させたポリマー試料があげられる。この目的はバルク材料の特性に特化することであり、試料は密度にかかわらず自然状態で試験する。モノマーとしてATIPAを用いる場合、20~30mol%で約175~275mgI/mlのヨウ素が得られる。このヨウ素負荷密度は、従来の血管造影注入濃度に匹敵し、最小限のパス長で十分な材料可視化をもたらす。
[0083] 発泡体形態学的には、界面活性剤濃度、触媒濃度(アミンとスズ)、予備重合体の水酸基比(粘度)、及び物理的な発泡剤濃度(Enovate(登録商標)245fa)を変化させて制御する。触媒と界面活性剤は全て、AirProducts(登録商標)をから入手できる。温度依存性プレミックス粘度プロファイルは、コーン型及びプレート型粘度計を用いて得られうる。走査型電子顕微鏡により、完成発泡体の孔径及び形態が定量化しうる。密度は、均一な(1cm3)材料ブロックの重量分析により測定できる。密度及び孔径の成功基準は、試作品の神経血管塞栓装置の仕様に基づいてよい。
[0089] いくつかの実施形態では、バルク硬化アプローチを改善する溶媒系合成アプローチがあげられる(上記の共有結合ヨード系モノマーの代わりに、又はそれに加えて)。造影剤は、ポリウレタン合成に適さない水やアルコール等の極性の高い溶媒に溶解するように設計される。しかしながら、ATIPAは、テトラヒドロフラン(THF)、ジメチルホルムアミド(DMF)、及びジメチルスルホキシド(DMSO)に可溶である。図7は、固体ATIPAモノマーをポリウレタンSMP発泡体に組み込む代替合成戦略の概要を示す。まず、造影剤は、無水THF溶液中で過剰なHDIで予備重合される。当該第1予備重合により、溶解性の問題が解決し、最終材料でのTgと架橋密度を操作するポリオールを多数用いることができるようになる。さらに、当該工程は、発泡工程から二酸化炭素の発生を効果的に除去して、発泡を、ATIPA含有量に応じて制御しうる。予備重合後、ロータリーエバポレーションを用いてオリゴマー溶液からTHFを除去する。いくつかの実施形態では、前重合体溶液を化学量論的に等価なポリオールと混合し、ニートフィルムに硬化させる。いくつかの実施形態では、予備重合体溶液とポリオール架橋剤、界面活性剤、ポリウレタン触媒、物理的発泡剤とを組み合わせて、ポリマー発泡体を合成してもよい。いくつかの実施形態では、カルボン酸反応用のアミド触媒を用いて、発泡体をさらに最適化しうる。

[0108] 図9は、いくつかの実施形態で合成した発泡体組成物の概要である。ポリウレタン合成のA側では、反応性のある水酸基、アミン、カルボン酸官能基を考慮して当量%で算出する。発泡体混合時の周囲の水質汚染を考慮し、2%モル過剰イソシアネートを各発泡体合成に添加した。HDI、TMHDI、BEP、MPD、ATIPA、及びHTは、VWR Scientific社及びSigma Aldrich社から入手して用いた。
[0113] キーエンス(登録商標)VHX-5000計測システム(可変照明アダプター付)を用いて、倍率50及び100倍で取得した光顕微鏡画像から孔径を求めた。発泡体試料は、縦及び横方向平面で2~4mmの薄片に切断した。各発泡体画像で10個の孔径測定を行った。
[0116] 発泡体試料を1cm×1cmの試料を厚さ8、4、2、1mmの薄片に切断し、ポリプロピレンシートに付着させて、X線イメージング用の試料を調製した。各発泡体アレイに、X線基準として白金製の塞栓コイルを設けた。発泡体を直径8mmの円柱に切断し、0.006インチのステンレス鋼ワイヤーに縦方向にネジを通して、周辺部の塞栓試作品を準備した。一の試作品を膨張状態で画像化し、他方の試作品を、MachineSolutionsの加熱式ステントクリンパーSC250を用いて縦方向に圧着した。試料は、圧着内径で100℃15分間平衡化され、放射状に圧縮され、周囲温度に冷却しながら回収した。直径2mmでバックボーンワイヤー非含有神経血管試作品も準備し、縦方向に圧着した。血管造影及び蛍光透視法画像を、Philips(登録商標)AlluraXperFD20 X線システムを用いて取得した。
[0118] 通気性のあるアルミパンに入れた発泡体試料5~10mgの乾燥Tgを、TA Q200(登録商標)示差走査熱量計を用いて測定した。発泡体試料を-40℃で5分間平衡化した後、120℃まで加熱し、-40℃まで冷却し、10℃/分の温度勾配で120℃まで再加熱した。Tgは、第2加熱曲線の変曲点で算出した。
[0122] TA Q800(登録商標)を用いて動的力学解析を行った。発泡体円柱を8mmの生検用パンチで調製し、カミソリで約5mmの長さに切断した。試料を0℃で5分間平衡化し、1Hzで40μmに変形しながら、3℃/分で120℃に加熱した。
[0124] 発泡体を直径2mmの円柱に切断し、0.006インチのステンレス鋼ワイヤーに縦方向にネジを通した。試料は、MachineSolutions(登録商標)SC250加熱式ステントクリンパーを用いて放射状に圧縮した。圧着試料は、37℃の水浴中で膨張させる前に24時間弛緩させた。試料は5分間隔で合計45分間画像化した。発泡体の長さに沿って、ImageJ(登録商標)ソフトウェアを用いて直径を5回測定した。
[0126] ダイヤモンドATR(登録商標)結晶を用いて、BrukerALPHAInfraredSpectrometer(登録商標)を用いてATR FTIRスペクトルを得た。データ解析はBrukerOPUS Spectroscopy software(登録商標)を用いて行った。
[0128] ASTM TypeIVドッグボーンパンチを用いて乾燥発泡体試料を調製した。一軸引張試験は、Insight30 Material Tester(登録商標)(MTS Systems Corporatio,Eden Prairie,MN)を用いて、50mm/分の一定のひずみ速度、室温で行った。各試料の応力-ひずみ曲線から、最大抗張力(kPa)、破断ひずみ(%)、弾性率(kPa)を算出した。
[0130] 約1立方センチメートルに測定した発泡体試料を洗浄し、イソプロピルアルコール中に希釈比20:1で30分間の超音波洗浄を3回行って残留界面活性剤を除去した。試料を100℃12時間真空下で乾燥させた。乾燥した発泡体試料を質量分析し、THFでショルダーまで充填した20mLバイアルに添加し、50℃で1Hzの振動で48時間加熱した。THFを除去し、試料を60℃24時間真空下で乾燥させた。ゲル分率は、最終的な試料質量を元の試料質量で除算して報告する。
[0132] Enovate(登録商標)の体積が異なる図11の組成物1~5には、物理的発泡剤の体積の増加に伴い材料密度が減少するという明らかな傾向がある。乾燥Tg値には統計的な有意差があるが(1way ANOVA,α=0.05)、この変動は装置設計基準に有意な影響は与えないと考えられる。材料密度と孔形態の関係は、以下で記載する光顕微鏡画像でよりよく示される。
[0139] 図12は、発泡剤の増加が発泡体形態に与える影響を示す。物理的発泡剤を用いない「0ml」組成物では、ATIPA発泡反応による気孔が残存する。当該気孔の大きさは0.5mlであり、1.0mlの発泡体に匹敵するものの、ストラットの形態で、バルク材料密度の有意な違いが説明される。同様に、1.5mlと2.0mlの小孔発泡体は、ストラット構造が薄いために材料の密度は最低となる。

[0154] 示差走査熱量測定を用いて、Tgと熱遷移の幅に対する組成効果を決定した。このデータは、発泡体拡張率を含む特定の装置設計基準に合わせて調整された材料を開発する重要な情報源である。図19は、物理的発泡剤により、密度が変化する発泡体の一貫した熱特性を示し、遷移温度からは独立して発泡体形態制御能をさらに促進する。発泡体密度を下げると、より多くの材料を、カテーテルを介して送達可能な断面に圧着しうる。これにより、生体内での容積膨張がより高まり、塞栓を容易にするより効果的な充填が可能となる。

[0167] 図26の赤外線スペクトルは、HT含有量が増加しても大きな変化を示さない。しかし、当該スペクトルはポリウレタン発泡体に特徴的なピークを示す。3310cm-1を中心とした広いピークはN-H振動を強調する。また、2852cm-1と2923cm-1のピークは、MPDメチル基からのC-Hの対称及び非対称伸縮に由来する。2260cm-1には未反応のNCOピークが見られなかった。1685cm-1では、C=Oウレタンのピークが水素結合により大きく右にシフトする。これは、セグメント化されたポリウレタンに比べて架橋間の分子量が比較的低い他のポリウレタンのSMP発泡体の水素結合ウレタンピークと一致する。強い水素結合アミドIIのピークが、1515cm-1に見られる。

[0242] 発明者は、ペンダントビニル基を備える分子量の異なる直鎖状の熱可塑性鎖を合成した。次に、発明者は、PUケミストリーを調整して、ジオールとジイソシアネートの選択により、疎水性とTgに対する依存性を制御した足場を達成した(図29)。要すれば、図30の反応は、脂肪族ジイソシアネートモノマーがジオールと反応して直鎖を成長させることに基づく。ジイソシアネート選択は、ポリマーの疎水性を調整する方法である。配合物として選択される2つの例のジイソシアネートモノマーは、TMHD及びHDIである(図29)。ペンダントビニル基を備えるジオール(例えば、図29のTMPAE及びHDD)は、架橋密度を制御する。アリルアルコール(AA)等の単官能アルコールを反応に添加すると、鎖を末端化し、様々なモル量で添加すると、分子量(MW)を制御しうる。アリルアルコールは、架橋用ビニル基をさらに提供するが、エチルアルコール等のアルキルアルコールを添加すると、電子線架橋密度依存性の熱可塑性分子量を制御しうる。

- 前記混合工程の前に、前記トリヨードベンゼンモノマーの単官能イソシアネートによる官能化工程を含む、請求項8に記載の方法。
- 前記ジイソシアネートが、アリルイソシアネート、エチルイソシアネート、又はメタクリル酸イソシアノエチル(IEMA)のうちの少なくとも1つを含む、請求項10に記載の方法。
- 熱可塑性ポリマーの形成工程が、電子線架橋を介して、熱硬化性ポリマーを架橋する、請求項8に記載の方法。
- 官能基を2より多く備える前記トリヨードベンゼンモノマーの、アミンペンダント基を官能化した後の官能基は、2である、請求項8又は9に記載の方法。
- 前記トリヨードベンゼンモノマーのカルボン酸官能基は、アミンペンダント基を官能化した後は、2である、請求項8又は9に記載の方法。
- 前記トリヨードベンゼンモノマーは、5-アミノ-2,4,6-トリヨードイソフタル酸(ATIPA);ジアトリゾ酸;トリヨードフェノール;イオヘキソール;イオタラミン酸(iothalmic acid);2-{3-[アセチル(プロピル)アミノ]-2,4,6-トリヨードフェノキシ}ヘキサン酸;2,3,5-トリヨード安息香酸(取り込みはCOOH基との単官能のみ);又は3-(3-アミノ-2,4,6-トリヨードフェニル)プロパン酸のうち少なくとも1つを含み、
前記脂肪族モノマーは、2-ブテン-1,4-ジオール;1,4-ブタンジオール;1,6-ヘキサンジオール;1,8-オクタンジオール;1,10-デカンジオール;1,6-ヘキサンジイルビス[オキシ(2-ヒドロキシ-3,1-プロパンジイル)]ビスアクリレート;ビスフェノールAグリセロール酸(glycerolate)ジメタクリレート;3,4-ジヒドロキシ-1-ブテン;7-オクテン-1,2-ジオール;ペンタエリトリトールトリアクリレート;ジエチレングリコール(DEG);ジエタノールアミン;ハイドロキノンビス(2-ヒドロキシエチル)エーテル;トリエチレングリコール(TEG);1-(ベンジルオキシメチル)トリエチレングリコール;2,2’-エチルイミノジエタノール;ブタンジオール;ペンタンジオール;ヘキサンジオール;ブチンジオール;ペンダントビニル基を含むジオール;1,5-ヘキサジエン-3,4-ジオール(HDD)、又はトリメチロールプロパンアリルエーテル(TMPAE)のうちの少なくとも1つを含み、及び、

請求項8~12のいずれか一項に記載の方法。 - 溶液鋳造、溶液紡糸、エレクトロスピニング、超臨界発泡、ディップ被覆、熱成形、圧縮成形、射出成形、押出成形、フィルムブロー、又は添加剤製造のうちの少なくとも1つを用いた、熱可塑性ポリマーの発泡体成形工程;
発泡体成形工程後に、電子線架橋を介して熱可塑性ポリマーを架橋して熱硬化性ポリマーを形成する工程、
をさらに含む、請求項8~11のいずれか一項に記載の方法。 - 熱可塑性ポリマーを架橋して、熱硬化性ポリマーが永久的な一次形状としての発泡体を備えさせる工程;
熱硬化性ポリマーの作動転移温度以上の温度で応力又はひずみを印加して、熱硬化性ポリマーを安定した二次形状に形成する工程;を含み、
前記熱硬化性ポリマーが二次形状の間、前記熱硬化性ポリマーを作動遷移温度以下の温度に冷却する、
請求項16に記載の方法。 - 熱可塑性ポリマーを、凍結乾燥、相分離、エマルジョン発泡、エマルジョンテンプレーティング、ケミカルブロー、又は物理ブローのうちの少なくとも1つを介して発泡体に加工する工程をさらに含む、請求項8~12のいずれか一項に記載の方法。
- 熱硬化性ポリマーを、前記トリヨードベンゼンモノマーである発泡剤に基づく発泡を介して発泡体に加工する工程をさらに含む、請求項12に記載の方法。
