Description
本発明は、治療有効性がより大きくかつ毒性がより少ない腫瘍標的指向方法及び試薬に関する。
併用療法は事実上全ての種類のがんでよく行われており一般に認められている処置手段であり、数十年にわたって標準的な治療手段である。併用療法を採用する根拠は、単一の薬剤のみが使用された場合にがんの高い突然変異率が腫瘍細胞の耐性株の急速な発生を可能にすると判断された初期の化学療法の経験であった。併用療法の目標は、有効性を高め、腫瘍の耐性またはエスケープの発現を最小限にすることである。これは一般的に、それぞれが異なる作用機序を有する2種以上の抗がん剤を使用することにより達成され、これによって耐性腫瘍細胞の発生がより困難になり、また可能性がより低くなる。2種以上の薬剤を組み合わせることによる相加効果または相乗効果は、患者の処置の成功と失敗の違いになり得る。

内部移行解析。LNCaP及びCWR22Rv1細胞をトリプシン処理し、1本のチューブあたり1×106細胞でチューブに分注した。細胞をRPMI(FBSなし)で1回洗浄した。1本のチューブあたり200μLのRPMI(FBSなし)中に入っている177Lu-J591(150,000CPM)及び/または177Lu-PSMA-617(110,000CPM)を、1本のチューブあたり200μLのRPMI(FBSなし)の中に添加した。サンプルを37℃で1時間インキュベートし、15分ごとに細胞を再懸濁した。インキュベートした後、細胞を0.1%BSA/PBSを用いて2回洗浄することで、結合していない/内部移行していない結合薬剤を除去した。サンプルをカウントした。
個々の薬剤、J591-177Lu、及びPSMA-617-177Luは、それぞれ与えられたいずれの細胞株(LNCaP及びCWR22Rv1)内でも同程度に内部移行した(図1A~1B)。内部移行はCWR22Rv1よりもLNCaPで大きく、LNCaP細胞株でPSMA発現がより高いことと一致した。両方の薬剤を同時インキュベーションすると、両方が効果的に同時に内部移行し、細胞内に相加的な量の放射標識が得られた。
LNCaP異種移植片において、PBSで処理した動物(プラセボ対照)は、25日目までに直径約1cmの腫瘍を発症し、数日後に殺処分を必要とした。最も優れた抗腫瘍効果は、150μCiのJ591-177Luによって達成された(図2)。同等の抗腫瘍効果は、半分の用量のJ591-177Lu(すなわち、75μCi)で、MTDを大幅に下回る用量のPSMA-617-177Lu(すなわち200μCi)を追加することによっても達成できた(図2)。
総合すると、これらのデータは、細胞集団に存在する標的分子(例えば細胞表面受容体分子)の異なる部位に結合する2つの薬剤を同時投与すると、それらの薬剤の相加的な結合が得られることを示している。そのため、標的分子/受容体が内部移行する場合、相加的な量の2つの薬剤が内部移行する。さらに、2つの標的指向薬剤が異なる特性(例えば分子量、電荷、親水性/疎水性、薬物動態、生体内分布等)を有しておりその結果としてそれぞれの副作用が異なり重複しない場合、2つの薬剤は、同時投与されることで、付加的な毒性を生じさせることなく標的細胞に相加的に結合させる/取り込ませることができる。当然の結果として、各薬剤の用量は、それぞれの最大耐量未満に適度に(5~50%)減らすことができ、これにより、2つの標的指向薬剤の同時投与は、標的細胞に相加的な用量を与えながらも対象が経験する毒性を実質的にまたは完全に減らすことができる。
前立腺特異的膜抗原(PSMA)により例示される腫瘍関連分子標的は、抗体または小分子リガンドのいずれかによりインビトロ及びインビボで標的とされ得る。これらの2つのクラスの薬剤は、分子サイズと血漿中半減期の両方で大きく異なる。

例えばJ591-Lu177などのAb上のβ(例えば177Lu)を標的指向化してPSMAを標的とする場合、鍵となる(及び用量制限の)副作用は、血小板減少症(血小板数の減少)であり、用量の増加に伴って血小板減少症の相対的な程度が次第に大きくなる。J591-Lu177で処置された患者はいずれも出血症状を経験しなかったものの、何人かは血小板数が自然に回復するまで血小板輸血を必要とした。30mCi/m2×2の線量(約2.0m2の男性で合計約120mCi)または35mCi/m2×2の線量(合計約140mCi)の用量で、血小板輸血が必要な患者はいなかった一方で、それぞれ40mCi/m2×2の線量(合計約160mCi)及び45mCi/m2×2の線量(合計約180mCi)の用量で、16人中5人(31%)及び15人中9人(60%)の患者が、骨髄の回復の間に彼らをサポートするために血小板輸血を必要とした。
α粒子は、それらの大幅に大きい原子量、線形エネルギー移動、及び放射線生物学的効果のため、βよりも有意に多くの細胞を死滅させる可能性を提供する。PSMA-617-Ac225を用いて前立腺がん患者を処置する場合、かなりの抗腫瘍効果の可能性が報告されている。残念なことに、この処置は唾液腺に与えられる容認し得ない損傷により制限される。50kBq/kgの処置活性では毒性は認められなかったものの、腫瘍量が多い患者では不十分な抗腫瘍応答を生じた。しかし、処置活性が1サイクルあたり100kBq/kgを超えると、投与された活性の増加は、用量制限毒性になる重度の口腔乾燥症をもたらした(Kratochwil et.al.,“Targeted Alpha-Therapy of Metastatic Castration-Resistant Prostate Cancer With(225)Ac-PSMA-617:Dosimetry Estimate and Empiric Dose Finding,”J.Nucl.Med.58(10):1624-1631(2017)、これはその全体が参照により本明細書に組み込まれる)。用量が高いほど大きな抗腫瘍活性が生じたものの、副作用が大きすぎであった。しかしながら、これは、唾液腺を標的としないAbを介して追加的なAc225を腫瘍に送達することによって改善することができる。J591-Ac225の適切な用量は、MTDが決定されるまで全ての患者が100Kbq/kgの許容耐量でのPSMA617-Ac225に加えて漸増用量のJ591-Ac225を受ける標準的な第1相試験で決定することができる。これにより、PSMAリガンド-Ac225+抗PSMA Ab-Ac225の組み合わせの用量が決定される。
α粒子ならびにその高エネルギー及び高細胞毒性及びその集中した短い距離を、より長い距離を有するβと共に用いる組み合わせ処置は、後者が腫瘍縁部で抗腫瘍活性を与えるため、患者の利益も提供する。容認し得ない唾液腺の副作用を回避するためにAbを使用してαを標的指向化し、深刻な血液毒性を避けるためにPSMAリガンドを使用してβを標的指向化することが好ましい。全ての患者は、2週間サイクルで合計400mCiでPSMA617-Lu177を受ける(または2週間サイクルで合計300~500mCiの範囲)。J591-Ac225の適切な用量は、MTDが決定されるまで漸増用量のJ591-Ac225を上述した用量のPSMA-617-177Luと組み合わせる標準的な第1相試験形式で決定することができる。
Claims (31)
- 第1のがん治療成分に結合している第1の標的指向成分を含む第1の薬剤であって、前記第1の標的指向成分が前立腺特異的膜抗原(PSMA)受容体を標的とする抗体であり、かつ前記第1のがん治療成分が最大耐量を有する第1の放射性核種である、第1の薬剤と、
第2のがん治療成分に結合している第2の標的指向成分を含む第2の薬剤であって、前記第2の標的指向成分がPSMA受容体を標的とする小分子であり、かつ前記第2のがん治療成分が最大耐量を有する第2の放射性核種である、第2の薬剤と
を含む、前立腺がんを処置するための併用療法剤であって、
前記第1及び第2の標的指向成分が、対象内で異なる生体内分布及び薬物動態を有し、
前記第1および第2の薬剤が前立腺がんを処置するのに有効な条件下で前立腺がんを有する対象に投与され、かつ
前記投与が、前記第1の薬剤または第2の薬剤のいずれか単独の最大耐量の投与に起因するものを超えて正常組織における毒性を増加させることなく、前記第1の薬剤または第2の薬剤のいずれか単独の最大耐量の投与によって達成されるものよりも多い放射線生物学的に有効な用量で、前立腺がん組織に、前記第1の薬剤および第2の薬剤を送達するために行われる、
併用療法剤。 - 前記第1及び第2の標的指向成分が、同じ分子標的を標的とする、請求項1に記載の併用療法剤。
- 前記第1及び第2の標的指向成分が、同じ細胞上の異なる分子標的を標的とする、請求項1に記載の併用療法剤。
- 前記第1及び第2のがん治療成分が、86Re、90Y、67Cu、169Er、121Sn、127Te、142Pr、143Pr、198Au、199Au、161Tb、109Pd、188Rd、166Dy、166Ho、149Pm、151Pm、153Sm、159Gd、172Tm、169Yb、175Yb、177Lu、105Rh、111Ag、131I、177Sn、225Ac、227Th、211At、及びこれらの組み合わせからなる群から独立して選択される放射性核種である、請求項1に記載の併用療法剤。
- 前記第1の標的指向成分が、PSMA受容体に対するPSMA受容体抗体であり、かつ前記第2の標的指向成分が、PSMA受容体阻害剤である、請求項1に記載の併用療法剤。
- 前記第1の標的指向成分が、J591、J415、J533、及びE99からなる群から選択される抗体であり、一方で、前記第2の標的指向成分が、PSMA 617、PSMA I&T、DCFBC、DCFPyL、グルタメート-尿素-リシン、ホスホルアミダート、2-(ホスフィニルメチル)ペンタン二酸、及び他のPSMA受容体リガンド/阻害剤からなる群から選択される、請求項5に記載の併用療法剤。
- 前記第1の薬剤がJ591-177Luであり、かつ前記第2の薬剤がPSMA 617-177LuまたはPSMA I&T-177Luである、請求項5に記載の併用療法剤。
- 前立腺がんを処置するための併用療法剤の製造における、第1のがん治療成分に結合している第1の標的指向成分を含む第1の薬剤、および第2のがん治療成分に結合している第2の標的指向成分を含む第2の薬剤の使用であって、
前記第1の標的指向成分が前立腺特異的膜抗原(PSMA)受容体を標的とする抗体であり、かつ前記第1のがん治療成分が最大耐量を有する第1の放射性核種であり、
前記第2の標的指向成分がPSMA受容体を標的とする小分子であり、かつ前記第2のがん治療成分が最大耐量を有する第2の放射性核種であり、
前記第1及び第2の標的指向成分が対象内で異なる生体内分布及び薬物動態を有し、
前記第1及び第2の薬剤が前立腺がんを治療するのに有効な条件下で前立腺がんを有する対象に投与され、かつ
前記併用療法剤が、前記第1の薬剤または第2の薬剤のいずれか単独の最大耐量の投与に起因するものを越えて正常組織における毒性を増加させることなく、前記第1の薬剤または第2の薬剤のいずれか単独の最大耐量の投与によって達成されるよりも多い放射線生物学的に有効な用量を前立腺がん組織に送達することによって、前立腺がんを処置するように製剤化されている、
使用。 - 前記第1及び第2のがん治療成分がそれぞれ最大耐量を有し、かつ前記第1及び第2のがん治療成分の前記最大耐量が前記投与中に投与される、請求項8に記載の使用。
- 前記第1及び第2のがん治療成分がそれぞれ最大耐量を有し、かつ前記第1及び第2のがん治療成分の前記最大耐量より少ない量が前記投与中に投与される、請求項8に記載の使用。
- 前記第1の標的指向成分が、PSMA受容体に対するPSMA受容体抗体であり、かつ前記第2の標的指向成分が、PSMA受容体阻害剤である、請求項8に記載の使用。
- 前記第1の標的指向成分が、J591、J415、J533、及びE99からなる群から選択される抗体であり、一方で、前記第2の標的指向成分が、PSMA 617、PSMA I&T、DCFBC、DCFPyL、グルタメート-尿素-リシン、ホスホルアミダート、2-(ホスフィニルメチル)ペンタン二酸、及び他のPSMA受容体リガンド/阻害剤からなる群から選択される、請求項11に記載の使用。
- 前記第1の薬剤がJ591-177Luであり、かつ前記第2の薬剤がPSMA 617-177LuまたはPSMA I&T-177Luである、請求項11に記載の使用。
- 前記第1の放射性核種及び前記第2の放射性核種が同じである、請求項8に記載の使用。
- 前記第1の放射性核種及び前記第2の放射性核種が異なるものである、請求項8に記載の使用。
- 前記第1の薬剤が、その最大耐量の100%の用量で投与される、請求項8に記載の使用。
- 前記第2の薬剤が、その最大耐量の100%の用量で投与される、請求項8に記載の使用。
- 前記第1の薬剤がJ591-225Acであり、かつ前記第2の薬剤がPSMA 617-177Luである、請求項8に記載の使用。
- 前記第1の薬剤がJ591- 225 Acであり、かつ前記第2の薬剤がPSMA I&T-177Luである、請求項8に記載の使用。
- 前記第1の薬剤がJ591-225Acであり、かつ前記第2の薬剤がPSMA 617-177Luである、請求項1に記載の併用療法剤。
- 前記第1の薬剤がJ591- 225 Acであり、かつ前記第2の薬剤がPSMA I&T-177Luである、請求項1に記載の併用療法剤。
- 前記第1の放射性核種及び前記第2の放射性核種が同じである、請求項1に記載の併用療法剤。

- 前記J591-225Acおよび前記PSMA I&T-177Luの適切な全身レベルを達成するために、前記J591-225Acおよび前記PSMA I&T- 177 Luが対象に同時投与される、請求項26に記載の使用。
- 前記PSMA 617- 177 LuまたはPSMA I&T- 177 Luが、2週間サイクルで合計300~500mCiの線量で投与される、請求項26に記載の使用。
- 治療的有効量のJ591-225Acおよび治療的有効量のPSMA 617- 177 LuまたはPSMA I&T- 177 Luを含む、前立腺がんを処置するための併用療法剤。
