本開示は、がん患者の予後情報を決定するための遺伝子発現プロファイルに関する情報を提供する分子診断アッセイに関する。具体的には、本開示は、遺伝子または共発現される遺伝子を含むアルゴリズムを提供し、その発現レベルを使用して、腎臓がん患者が正または負の臨床転帰を経験する可能性を決定することができる。本開示は、腎臓がん患者の再発スコアの計算に有用な遺伝子発現情報を提供する。
米国がん協会(American Cancer Society)は、米国において2013年に、腎臓がんの約65,150例の新たな症例と、腎臓がんによる約13,680名の死亡が存在することになると推定する(米国がん協会、腎臓がん(成人)腎細胞癌概要(Kidney Cancer (Adult) Renal Cell Carcinoma Overview)、http://www.cancer.org/acs/groups/cid/documents/webcontent/003052-pdf.pdfにてオンライン利用できる)。腎腺癌または副腎腫(hypernephroma)とも呼ばれる腎細胞癌(RCC)は、腎臓がん10症例のうち9症例超を占める最も一般的な種類の腎臓がんであり、全悪性腫瘍のおよそ2~3%を占める(Id.; National Comprehensive Cancer Network Guidelines (NCCN) Clinical Practice Guidelines in Oncology, Kidney Cancer、バージョンI、2013年)。未知の理由から、RCCの率は、過去65年間で年間2%増加した(NCCN Clinical Practice Guidelines in Oncology, Kidney Cancer)。
医師は、RCC患者のための情報に基づく処置決断や、治験の検定力を増加させるための臨床治験への適切な高リスク患者のリクルートに役立つ予後情報を要求する。現存する方法は、主観的尺度に基づいており、したがって、不正確な予後情報を提供し得る。
本願は、腎臓がん患者から得られた生体試料由来の1種または複数の遺伝子または遺伝子サブセットの発現レベル(複数可)の測定が関与する分子アッセイを開示する。例えば、臨床転帰の可能性は、疾患の観察される臨床特色または無再発間隔に基づく定量的スコアの観点から記載することができる。
本発明の実施形態において、例えば以下の項目が提供される。

腎臓がん患者の再発スコア(RS)を得るための方法であって、
前記患者から得られた腫瘍試料における、APOLD1、EDNRB、NOS3、PPA2B、EIF4EBP1、LMNB1、TUBB2A、CCL5、CEACAM1、CX3CL1およびIL-6から選択される少なくとも1種のRNA転写物またはその発現産物のレベルを測定するステップと、

前記正規化されたレベルを遺伝子サブセットに割り当てるステップと、
がん再発に対するその寄与に従って前記遺伝子サブセットに重みを付けるステップと、

この方法において、目的のポリヌクレオチド配列(cDNAおよびオリゴヌクレオチドを含む)は、基材上にアレイ化される。次に、アレイ化された配列を、特異的ハイブリダイゼーションに適した条件下で、試料のRNAから作製された検出可能に標識されたcDNAと接触させる。RNAの供給源は、典型的に、腫瘍試料と、任意選択で、内部対照として同じ患者の正常組織または細胞株から単離された全RNAである。RNAは、例えば、凍結または保管されパラフィン包埋および固定された(例えば、ホルマリン固定された)組織試料から抽出することができる。

本開示は、患者から得られた腫瘍試料由来のある特定の予後遺伝子の発現レベルをアッセイすることにより、腎臓がん患者の再発スコアを得るための方法を提供する。そのような方法は、遺伝子産物の類似機能に基づき作製された遺伝子サブセットの使用が関与する。例えば、本明細書に開示されている予後診断方法は、血管正常化群、免疫応答群および細胞成長/分裂群ならびにIL-6のそれぞれに由来する少なくとも1種の遺伝子を含む遺伝子サブセットの発現レベルをアッセイするステップと、がん再発に対するこれらそれぞれの寄与により遺伝子サブセットのそれぞれの発現レベルに重みを付けることにより、患者の再発スコア(RS)を計算するステップが関与する。重み付けは、遺伝子サブセット毎に異なることができ、正であっても負であってもよい。例えば、血管正常化遺伝子群スコアは、-0.45の因数を乗じることにより重み付けることができ、免疫応答遺伝子群スコアは、-0.31の因数を乗じることにより重み付けることができ、細胞成長/分裂遺伝子群スコアは、+0.27の因数により重み付けることができ、IL-6の値は、+0.04の因数を乗じることができる。
本明細書に開示されている方法において使用される発現データは、正規化することができる。正規化は、例えば、アッセイされるRNAの量における差および使用されるRNAの品質における可変性を補正(正規化して排除)して、CT測定値における体系的変動の望まれない供給源等を除去するプロセスを指す。保管され固定されパラフィン包埋された組織試料が関与するRT-PCR実験に関して、体系的変動の供給源は、患者試料の採取後の時間経過および試料の貯蔵に使用した固定液の種類に対する、RNA分解の程度を含むことが公知である。体系的変動の他の供給源は、検査室の処理条件に起因し得る。
本明細書に開示されている方法において使用される発現データは、標準化することができる。標準化は、全遺伝子を比較可能なスケールに効果的に配置するプロセスを指す。一部の遺伝子が、その他よりも大きい変動(より広い範囲の発現)を呈するため、この操作が行われる。標準化は、各発現値を該遺伝子の全試料にわたるその標準偏差で割ることにより行われる。次に、ハザード比は、発現における1標準偏差増加毎の臨床エンドポイント(臨床再発、生物学的再発、腎臓がんによる死亡またはいずれかの原因による死亡)に対するハザードの比例的変化として解釈される。
オリゴヌクレオチドセットは、プライマーおよびプローブ(P3)プールならびに遺伝子特異的プライマー(GSP)プールを築くために使用される、フォワードプライマー、リバースプライマーおよびプローブを表す。RT-PCRサイクル閾値(CT)測定値における体系的な差は、固有の変動オリゴヌクレオチド合成により、異なるオリゴヌクレオチドセットの間に生じ得る。例えば、オリゴヌクレオチドセットにおける差は、開発、産生(検証のために使用)および将来的な産生ヌクレオチドセットの間に存在し得る。よって、RSの計算において使用される遺伝子係数への翻訳をもたらすオリゴヌクレオチドセットにおける体系的な差を調整するための統計的較正手順の使用が望ましくなり得る。例えば、アルゴリズムにおける使用のためにアッセイされる遺伝子毎に、産生オリゴヌクレオチドセットのCT測定値、対、異なるオリゴヌクレオチドセットにおいて使用される相当する試料に由来するCT測定値の散布図を使用して、ロット間の差の効果をランダム効果として処理する線形回帰モデルを作成することができる。そのようなプロットの試験は、CT測定値の分散が、平均CTの関数として指数関数的に増加することを明らかにする。ランダム効果線形回帰モデルは、対数線形分散により評価して、線形較正方程式を得ることができる。スコアの計算された平均平方誤差(MSE)は、較正スキームが全く使用されない場合のMSEと比較することができる。
分析または試料の差による患者RSの分布における体系的な差は、初期開発、臨床検証および商業的試料の間に存在し得る。アルゴリズムにおいて一定センタリング調整パラメータを使用して、そのような差を説明することができる。

臨床再発に関連する遺伝子を同定するための遺伝子同定試験は、2010年1月11日に出願された米国特許仮出願第61/294,038号および2010年5月19日に出願された同第61/346,230号ならびに2011年1月7日に出願され2011年7月14日に公開された米国特許出願公開第2011/0171633号(その全てが参照により本明細書に組み込まれる)に記載されている。簡単に説明すると、保管されパラフィン包埋された腎摘除術試料を有する、1985年~2003年の間のCleveland Clinicにおける腎摘除術を受けたステージI~III ccRCC患者を同定した。6×10μmの切開腫瘍切片からRNAを抽出し、RT-PCRを使用して、732種の遺伝子(5種の参照遺伝子を含む)のRNA発現を定量化した。一次エンドポイントは、RCCの手がかりとなる、腎摘除術から最初の再発または死亡までの時間として定義される無再発間隔(RFI)であった。完全な臨床/病理学データおよび組織ブロックを有する931名の患者が評価可能であった。患者特徴:63%男性、年齢中位数61歳、ステージI(68%)、II(10%)およびIII(22%)、経過観察中位数5.6年間、ステージI、IIおよびIIIにおける5年間の再発率は、それぞれ10%、29%および45%であった。RFIに有意に関連する臨床/病態共変量は、顕微鏡レベルの壊死、Fuhrmanグレード、ステージ、腫瘍サイズおよびリンパ節関与を含んだ(全てp<0.001)。
アルゴリズム開発
実施例1に記載されている試験において同定された遺伝子は、再発スコアにおける算入に関して考慮された。次の通り、72種の遺伝子のより小型のセットを選択した:
・ 共変量調整と、Storeyの手順(Storey JD(2002年)A direct approach to false discovery rates. Journal of the Royal Statistical Society: Series B 64巻:479~498頁;Storey JD, Taylor JE, Siegmund DO(2004年)Strong control, conservative point estimation and simultaneous conservative consistency of false discovery rates: a unified approach. Journal of the Royal Statistical Society、Series B 66巻:187~205頁)を使用した10%FDR制御後にRFIに関連する29種の遺伝子。
・ 共変量調整前に最も有意な14種の遺伝子。
・ VEGF/mTOR経路の12種の遺伝子メンバー。
・ 主成分分析により17種の遺伝子を選択して、追加的な経路由来の遺伝子を同定した。
定量的RT-PCRを使用して、選ばれた遺伝子のmRNA発現レベルを決定した後に、遺伝子をサブセットに群分けした。血管および/または血管新生機能に関連することが公知の遺伝子を「血管正常化」遺伝子群に群分けした。免疫機能に関連することが公知の遺伝子を「免疫応答」遺伝子群に群分けした。重要な細胞成長および細胞分裂経路(複数可)に関連する遺伝子を「細胞成長/分裂」遺伝子群に群分けした。

アルゴリズムの性能
再発のエンドポイントに関する平均値への回帰の補正のための調整ありおよびなしのアルゴリズムに含まれる最終遺伝子の性能を、表3にまとめる。
不均一性試験
組織不均一性による可変性を調べる内部試験を、固定されパラフィン包埋された組織(FPET)ブロックの腎がん試料において行った。上述の実施例に示す方法およびアルゴリズムを使用して、患者毎に2個のブロックおよび各ブロック内に3枚の切片を有する8名の患者を査定した。ブロック間の可変性およびブロック内の可変性を査定することにより、不均一性を測定した。ブロック間の可変性は、同じ患者内のFPETブロック間の生物学的可変性を測定する。これにより、集団レベル可変性の推定がもたらされる。ブロック内の可変性は、ブロック内の組織不均一性と、技術的なアッセイ関連の可変性の両方を捕捉する。正規化された個々の遺伝子スコアと再発スコアを計算し、ブロック内、ブロック間および患者間の可変性推定を作成した。分析の結果を下の表4および表5に示す。ブロック間および内の可変性に対する患者間の可変性の高い率が一般に有利である。これは、個々の遺伝子測定値および再発スコアにおける臨床的に情報価値のある患者間可変性と比較して、組織不均一性および技術的なアッセイ関連の可変性が低いことを示す。
追加的な多重遺伝子組合せ
遺伝子同定試験由来のデータセットまたは検証試験由来のデータセットのいずれかを使用して、いくつかの代替的多重遺伝子モデルも評価した。遺伝子同定試験由来のデータセットにおいて検査された追加的な代表的遺伝子組合せを表6に示す。検証試験由来のデータセットにおいて検査した追加的な代表的遺伝子組合せを表7に示す。表6に示すモデル1~4は、検証試験由来のデータセットにおいて検査されておらず、表7から省略されている。これらの表は、両者共に、がん再発リスクを予測するためのアルゴリズムにおける各遺伝子の相対的な重さを反映する計算された係数を示す。検査した様々なモデルにおいて使用された特異的遺伝子(例えば、モデル11は、APOLD1、NOS3、PPAP2BおよびCEACAM1を含んだ)をコードする各mRNAの測定された腫瘍レベルに、示されている係数を乗じて、代替的スコアを得た。絶対的標準ハザード比および相当する95%信頼区間により測定される、各代替的スコアの性能も表に示す。2種の遺伝子がヘッダー行に示されている場合(例えば、APOLD1-EDNRB、IL6-IL8)、この列は、これら2種の遺伝子をコードするmRNAの平均測定腫瘍レベルの係数を示す。
Claims (9)
- 腎臓がんを有する患者の再発スコア(RS)結果を得るための方法であって、
前記患者から得られた腫瘍試料において、遺伝子APOLD1、EDNRB、NOS3、PPA2B、EIF4EBP1、LMNB1、TUBB2A、CCL5、CEACAM1、CX3CL1およびIL-6のそれぞれのRNA転写物のレベルを測定するステップであって、前記遺伝子が、APOLD1、EDNRB、NOS3およびPPA2Bを含む血管正常化遺伝子群と、CCL5、CEACAM1およびCX3CL1を含む免疫応答遺伝子群と、EIF4EBP1、LMNB1およびTUBB2Aを含む細胞成長/分裂遺伝子群と、IL-6とを含む、ステップと、
前記RNA転写物のレベルを、前記腫瘍試料における少なくとも1種の参照RNA転写物のレベルに対して正規化して、正規化された前記RNA転写物レベルをもたらすステップと、
前記正規化されたRNA転写物レベルを、それらのそれぞれの遺伝子サブセットに割り当てるステップと、
がん再発に対するその寄与に従って、以下:
