実施例1 組換えレンチウイルスベクターの構築

その形質導入効率はGFPの発現により評価する(9)ことができ、scFvとBCMAタンパクの結合効率はBCMA-Fcにより研究する(10)ことができ、抗HA抗体はCARの発現を検出し、リッチCAR-T細胞に用いて、機能性分析を行う(11)。
GFPタンパクを検出する以外、CAR分子の発現は他の方法により行うこともできる。例えば、適量のビオチン化(biotinylated)BCMAとPEストレプトアビジン(streptavidin)でCAR分子を標識し、そしてPEシグナルによりCAR分子の発現を反映することができる。別の例として、適量のビオチン化抗HAモノクローナル抗体(biotinylated anti-HA mAb)とPEストレプトアビジン(streptavidin)でCAR分子を標識して検出に用いることができる。

図2Aに示されるように、PEIをトランスフェクション試薬として用い、実施例1で調製されたPXL0009、PXL0041、PXL0037のCARプラスミドを293T細胞に一過性トランスフェクトして、それぞれPXL0009-293T細胞、PXL0041-293T細胞、PXL0037-293T細胞を得た。一過性トランスフェクションの72時間後のPXL0009-293T細胞、PXL0041-293T細胞、PXL0037-293T細胞は、候補CAR分子の発現能力の評価に用いられた。図2Aにおいて、1はプラスミドとトランスフェクション試薬を表し、2は一過性トランスフェクションを表し、3は発現とT2A切
断ペプチドの切断を表す。
ビオチン化BCMA(biotinylated BCMA)の用量を変化させ、PEシグナルの変化(図2Bに示さ
れる)を得た。その中で、X軸は細胞で発現されるGFPタンパクのシグナルであり、Y軸は
勾配希釈したビオチン化BCMA(biotinylated BCMA)と所定量のPEストレプトアビジン(PE-streptavidin)でCAR分子を標識して得られたPEシグナルである。
使用して、候補CARプラスミドで一過性トランスフェクトされた293T細胞のPEシグナル変
化曲線(図3に示される)を検出した。カーブフィッティングから計算されたBCMAタンパ
クと細胞表面のCAR分子が結合するEC50値は表2に示される。
タグをコードする遺伝子を含まず、配列がSEQ ID NO:45に示される。細胞サンプルではGFPシグナルが検出されず、CARがビオチン化抗HAモノクローナル抗体(biotinylatedanti-HA mAb)と結合するPEシグナルもないはずである。したがって、PXL0009-293T細胞サンプ
ルはフローサイトメトリーの対照サンプルとして使用できる。PXL0009とは異なり、参照
プラスミドとしてのPXL0041とPXL0037はGFPタンパクとHAタグをコードするため、PXL0041-293T細胞サンプルではGFPシグナルが検出され、CARとビオチン化BCMA(biotinylated BCMA)タンパク又はビオチン化抗HAモノクローナル抗体(biotinylatedanti-HA mAb)との
結合(PEシグナル)もそれぞれ検出でき、陽性対照として使用できる。
たCAR分子は細胞上で発現でき、且つBCMAタンパクと正常に結合できることを示している
。
3.1 レンチウイルスのパッケージング
実施例1で調製された番号がPXL0009、PXL0041、PXL0037であるCARプラスミドは、293T
細胞をシャトルプラスミドおよび他のパッケージングプラスミドで同時トランスフェクトし、細胞内でレンチウイルスのパッケージングを行う必要がある。具体的な手順は以下の通りである。
×104個/cm2の密度で、10% FBSを含有するDMEM培地に接種し、37℃、5% CO2、飽和湿度の環境で培養し、接種の3日後にトランスフェクションを行った。トランスフェクションの
前に2本のEP管を取り、それぞれ500μlopti-MEMを加え、一方の管にレンチウイルスヘルパーベクター3μg PSPAX2、2 pMD2.G及び実施例1で調製されたベクター(番号がPXL0009
、PXL0041、又はPXL0037であるCARプラスミド)5μgを加え、均一に混合し、プラスミド
を含む管を得た。他方の管に濃度1mg/mlの PEI 30μlを加え、十分に混合した。その後、上記の他方の管におけるPEIを加えた溶液を、前記プラスミド含有管に滴下し、加えなが
ら均一に混合し、室温で30min静置した後、前述293T細胞に均一に滴下した。トランスフ
ェクション後の24時間に6mlの10% FBSを含有するDMEM培地を交換した。
レンチウイルスパッケージングの過程で収集した上清における形質導入活性を有するウイルスタイター(バイオタイター)を検出することにより、レンチウイルスのパッケージング効率を評価することができる。具体的な検出手順は以下の通りである。
イルス形質導入を行い、それぞれ100μl、50μl、25μl、12.5μlの上記上清を6ウェルプレートに加えた(各サンプル量を2つのウェルに設置した)。その後、37℃、5% CO2、飽
和湿度の環境で続けて細胞を培養した。レンチウイルス形質導入の72時間後、293T細胞を消化して再懸濁後にフローサイトメトリーに用いた。
ルスバイオタイター(GFPタイターと呼ばれる)を計算することができる。
又は、LV0007により形質導入された293T細胞をビオチン化BCMA(biotinylated BCMA)
とPEストレプトアビジン(PE-streptavidin)で標識し、CAR陽性率(CARタイターと呼ば
れる)をフローサイトメトリーで検出した。
バイオタイター(TU/ml)=(CAR陽性率×293T細胞数)/ウイルスサンプル体積
上述したパッケージング過程で収集された上清のバイオタイターの検出結果は図4と表4に示される。
に、ビオチン化BCMAとPEストレプトアビジン(PE-streptavidin)によりウイルス形質導
入された293T細胞におけるCAR陽性率を検出し、計算により得られたタイターである。
初日に、約65 mlの健康なドナーの末梢血を採取し、得られたPBMCをFicollで分離し、
さらに得られたT細胞をCD3 MicroBeadsで分別した。分別した後のT細胞はCD3/CD28 Dynabeadsで活性化した。約24時間活性化した後(2日目)、実施例3で調製されたLV0007とLV0011のレンチウイルス形質導入(MOI = 4)を加え、形質導入するとき、T cells密度が約1.5×106細胞/mlであった。3日目に、形質導入した後のT細胞に対して培地交換を行った。
その後、毎日カウントし、細胞密度を(0.6~2.0)×106細胞/mlの間に維持し、細胞の成長曲線をプロットした。
性率(GFP%)、CD4/CD8比率を検出した。10日目に、CAR-T細胞の体外機能評価を行った。
さらに、所定濃度且つ過剰のPEストレプトアビジン(PE streptavidin)の条件下で、
勾配希釈したビオチン化BCMA(biotinylatedBCMA)タンパクでCAR分子を標識する方法(
実施例2の方法と同じ)により、実施例4で調製されたCAR-Tで発現されるCAR分子とBCMAタンパクとの結合能力を検出し、結果は図6と表7に示される。
6.1 CD107a脱顆粒実験
CD107a脱顆粒実験によりCAR-T細胞の体外生物学的有効性の評価を行った。CD107aは細
胞内微小胞のマーカーであり、グランザイムを担持した微小胞が細胞膜と融合すると、細胞膜上のCD107aが増加し、モネンシン(monesin、BioLegendから購入)で回収を阻害するとき、微小胞放出の強度を定量的に反映できる。CAR-Tが標的細胞上の標的抗原に刺激さ
れると、グランザイム放出の引き起こし、CD107aの増加をフローサイトメトリーにより検出することでT細胞の活性化を判断することができる。
胞と標的細胞の細胞密度がいずれも5×105細胞/mlであった。その後、CD8、PD1抗体でサ
ンプル標識した後、フローサイトメトリーを行った。候補CAR-T細胞におけるCAR陽性細胞はGFPの共発現により検出され、対照としてのLV0011-CAR-T細胞はビオチン化BCMA-FcとPEストレプトアビジンによりCAR陽性細胞を標識した。実験の陰性対照はK562細胞とCAR-T細胞を共インキュベートし、陽性対照は標的細胞の代わりに混合物(cocktail)でCAR-T細
胞を活性化した。
合がそれぞれ得られる。
本実施例のセクション6.1の実験操作に従って、CAR-T細胞サンプル(LV0007-CAR-TとLV0011-CAR-T細胞)を、それぞれ標的細胞U266(BCMA陽性)又はK562(BCMA陰性)と3時間
共インキュベートした後、フローサイトメトリーを行った。細胞サンプルのCD107a脱顆粒実験データの結果は表8と図8に示され、表8と図8はいずれもCD8+/PD1-/CAR+及びCD8+/PD1-/CAR-の2つのサブグループ細胞における発現CD107a陽性細胞比率を示す。
さらに、多発性骨髄腫(MM)細胞の表面で発現するBCMAタンパクの細胞外部分はγ-セ
クレターゼ(γ-secretase)によって切断でき、可溶性BCMA(sBCMA)を形成する。可溶
性BCMAはMM患者の血清で上昇し、且つ濃度が腫瘍の悪性度と正の相関がある。したがって、CAR-T細胞を標的細胞と共インキュベートするとき、培地に1μg/ml BCMAタンパクを加
えて、可溶性BCMAのCAR-T細胞活性化に対する影響を評価した。
表9と図9に示される。図9に示されるように、LV0007形質導入細胞サンプルについて、U266細胞のCAR-T細胞に対する活性化はBCMAタンパクの競合性によって阻害されなかったが、対照としてのLV0011-CAR-T細胞サンプルの活性化は明らかに阻害された。
サイトカイン放出測定実験は、測定されるCAR-T細胞(5×105細胞、100μl)と標的細
胞(5×105細胞、100μl)をRPMI-1640培地で24時間共インキュベートした後、細胞培養
上清を収集し、そしてCBA法によりIL-2、IL-4、IL-6、IL-10、IL-17A、TNF-α、及びIFN-γなどの因子の分泌状況を測定した。
の検出結果は表10と図10に示される。図10に示されるサイトカイン放出量はサンプルで検出される最大値に対する百分率である。表10と図10に示されるように、BCMA陽性の標的細胞U266に刺激された後(+U226)、LV0007-CAR-T細胞から分泌するTNF-α、IFN-γ、及びIL-2はいずれも大幅に増加した。BCMA陰性のK562に刺激される場合(+K562)、LV0007-CAR-TはTNF-α、IFN-γ、及びIL-2の分泌増加を示しなかった。
実施例4に類似する実験方法により、実施例3で調製されたレンチウイルスLV0002とLV0021を形質導入することにより、LV0002-CAR-T細胞とLV0021-CAR-T細胞を調製し、LV0021ウイルス(PXL0087プラスミドによってコードされるCAR分子に対応し、その共刺激因子が4-
1BBである)とLV0002ウイルス(PXL0009プラスミドによってコードされるCAR分子に対応
し、その共刺激因子がCD28である)を用いて、異なるドナーのT細胞を形質導入し、CAR-T細胞を作成した。ただし、LV0002ウイルスでドナー1のT細胞を形質導入したものをLV0002-D01-CAR-T細胞と命名し、LV0002ウイルスでドナー1のT細胞を形質導入したものをLV0002-D02-CAR-T細胞と命名し、LV0002ウイルスでドナー3のT細胞を形質導入したものをLV0002-D03-CAR-T細胞と命名し、LV0021ウイルスでドナー1のT細胞を形質導入したものをLV0021-D01-CAR-T細胞と命名し、LV0021ウイルスでドナー2のT細胞を形質導入したものをLV0021-D01-CAR-T細胞と命名し、LV0021ウイルスでドナー3のT細胞を形質導入したものをLV0021-D03-CAR-T細胞と命名した。ドナー自身のT細胞を対照とした。ドナー1からのT細胞をT細胞-D01と命名し、ドナー2からのT細胞をT細胞-D02と命名し、ドナー3からのT細胞をT細胞-D03と命名した。これらのCAR-T細胞の機能も実施例6におけるCD107a脱顆粒実験で検出し、結果は表11と図11に示される。
実施例8で調製されたCAR-T細胞LV0021-D02-CAR-Tを用いて体外殺傷機能を検出し、具体的にカルセイン蛍光法により検出できる。その検出手順は以下の通りである。BCMA陽性のU266細胞とBCMA陰性のK562細胞をそれぞれ5×105個取り、PBS+4%FBS溶液中で1×106個/mlの細胞懸濁液に再懸濁させた。そして、カルセイン(Calcein-AM)25μMでU266とK562細
胞をそれぞれ標識した。標識された後のU266とK562細胞を各ウェル5000細胞の量でそれぞれU底96ウェルプレートに接種した。その後、対応するウェルに、それぞれエフェクター
細胞と標的細胞の比(E:T値)が50:1、25:1、5:1である割合で、測定されるCAR-T細胞又
は対照としてのT細胞を添加し、各ウェルの溶液体積が200μlであった。また、エフェク
ター細胞の代わりに、PBS溶液をU266又はK562細胞に加え、検出する陰性対照とした。エ
フェクター細胞の代わりに、細胞分解液溶液をU266又はK562細胞に加え、検出する陽性対照とした。その後、U底96ウェルプレートを遮光して37℃で3時間インキュベートし、そして各ウェルから上清溶液を抽出して(細胞を抽出しないように)蛍光検出した(励起光波
長485/20nm、発光波長530/25nm)。標的細胞(U266又はK562)がエフェクター細胞によって殺され分解されてカルセインを放出する相対的な割合は下記の式で計算できる。
放出するように表現される。対照として、CAR-T細胞はBCMA陰性のK562細胞に対して殺傷
効果が弱い。また、T細胞はU266細胞に対して一定の非特異的殺傷効果を有し、且つ非特
異性殺傷はE:T値の増大に伴って変化しない。したがって、CAR-T細胞は、明らかなBCMAに対する特異性殺傷効果を有する。
実施例8で調製されたドナー2からのCAR-T細胞(LV0021-D02-CAR-T)を用いて動物モデル
実験を実施した。前記LV0021-CAR-T細胞をXL103-07と命名した。一方、ドナー2からのT細胞(T細胞-D02)を対照T細胞とした。各マウス2×106細胞の量で、増幅中のU266細胞を皮下注射により免疫不全NSGマウスに接種し、U266皮下担癌モデルを作った。腫瘍のサイズが100-150 mm3に達すると、表12に示されるように、担癌マウスを5組に分け、それぞれCAR-T細胞(XL103-07)、対照T細胞(T細胞-D02)、PBS、又は細胞凍結保存液(7.5%のDMSO、23%のヒトアルブミン、32.5%の複合電解質液、35%のブドウ糖注射液、および2%の生理食塩水を含む)を注射した。組み分け及び投与方法は表12に示される。
与量組でも低投与量組でも良好な腫瘍殺傷効果を有し、腫瘍は注射の19日後に完全になくなった。対照T細胞、PBS、又は細胞凍結保存液を注射したマウス体内の腫瘍はいずれも続けて成長した。
実施例10と同じCAR-T細胞XL103-07を、フロービーズアレイ法(cytometric beads array, CBA)法(具体的な方法はBDTM CBA Flex Set Reagents及びBD FACSArrayTM Bioanalyzer
参照)により投与した後、マウス末梢血プラズマ中のサイトカイン(IFNγ、IL2、IL10、IL7、IL6、TNF-α)の変化を検出した。担癌マウスを、対照T細胞組、XL103-07高投与量
組、XL103-07低投与量組の三組に分け、各組5匹であった。その中で、XL103-07-Hは高投
与量組を表し、投与量が10×106細胞(XL103-07細胞)/動物であった。XL103-07-Lは低投与量組を表し、投与量が2×106細胞(XL103-07細胞)/動物であった。
定量PCR法により、実施例10の動物モデル担癌マウスの末梢血細胞ゲノムDNAにおけるXL103-07のCAR分子のDNAコピー数の変化を検出し、マウス体内でのCAR-T細胞の増幅状況を
分析し、結果は図16に示される。類似的に、担癌マウスを、対照T細胞組、XL103-07高投
与量組、XL103-07低投与量組の三組に分け、各組5匹であった。その中で、XL103-07-Hは
高投与量組を表し、投与量が10×106細胞(XL103-07細胞)/動物であった。XL103-07-Lは低投与量組を表し、投与量が2×106細胞(XL103-07細胞)/動物であった。
薬学研究を通じて、安定なPXL0085プラスミド、LV0020レンチウイルスベクター、及びCART細胞の生産プロセスを取得した後、探索的臨床研究を行い、その安全性、忍容性、お
よび初歩的な有効性を考察しながら、PK/PDの特徴を探索した。この研究は、試験サンプ
ルの調製、研究者と研究機構、試験方法、倫理審査、及びインフォームドコンセント、被験者の登録選択と診断治療、副反応の報告と処理、試験データの集計と統計分析などの面ではGCP原則に従った。

液滴デジタルPCR法(Bio-Rad QX200説明書参照)により、実施例13の5人の被験者の末
梢血におけるCARのDNAコピー数を検出し、CAR-T細胞の薬物動態特性を評価した。結果は
図18に示される。投与後、製品が体内で迅速に増幅し、60日目でも検出可能であった。ほとんどの被験者体内のCAR-Tのピークまでの時間は10日程度であるが、001番被験者のピークまでの時間は17日であった。
R 3.5.0ソフトNonCompartパックで、実施例14のCARのDNAコピー数変化をさらに分析し
、薬物動態の関連パラメーターを計算し、結果は表13に示される。表13において、T0は最初に非ゼロ濃度が観測された時間を表し、ピーク薬物濃度(Cmax)は薬品濃度が達する最大ピーク値を表し、ピークまでの時間(Tmax)はピーク薬物濃度に達するのに必要な時間を表し、AUC(0-28)は0から28日の曲線下の積分面積を表し、AUC(0-CLST)は0から最後の観察時間までの曲線下の積分面積を表す。
均AUC(0-28)が(1306653.3±100000)であり、第二投与量組の平均Cmaxが(32487.5±2000)コピー数/μg DNAであり、平均AUC(0-28)が(317816.95±150000)である。

- キメラ抗原受容体(CAR)であって、
前記CARは、ヒトBCMAに特異的に結合する抗体又はその断片を含むBCMA結合ドメインと、膜貫通ドメインと、共刺激ドメインと、細胞内シグナル伝達ドメインとを含み、前記抗体は、重鎖相補性決定領域1(HCDR1)と、重鎖相補性決定領域2(HCDR2)と、重鎖相補性決定領域3(HCDR3)とを含み、前記HCDR1はSEQ ID NO:9に示されるアミノ酸配列を含み、前記HCDR2はSEQ ID NO:10に示されるアミノ酸配列を含み、及び前記HCDR3はSEQ ID NO:11に示されるアミノ酸配列を含み、並びに、
