Description
適応障害は、環境の変化に適応できずに心身に不調をきたして適応不全となっている状態であり、ICD-10(世界保健機構の診断ガイドライン)によると「ストレス因により引き起こされる情緒面や行動面の症状で、社会的機能が著しく障害されている状態」と定義されている。適応障害の症状としては、不安、怒り、焦りや緊張などの情緒面の症状、及び暴飲暴食、無謀な運転などの攻撃的な行動や子供の場合の「赤ちゃん返り」などの行動面の症状が挙げられる。また、体の症状としては、自律神経失調症(発汗、動悸、めまい、神経症反応など)の症状が観られることもある。適応障害は、抑うつ反応、うつ病、神経症反応、躁鬱病、不安症、パニック障害、睡眠障害、てんかん、統合失調症、非定型精神病などとして現れる(診断される)こともある。
急性ストレス障害は、フラシュバックなどの侵入症状、陰性気分、解離症状、回避症状、覚醒症状などを含む、「Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition」(DSM-5)で推奨されている基準などに基づいて診断される。患者は、外傷的出来事に直接的又は間接的に曝露したことがあり、心的外傷の記憶を反復的に想起し、睡眠障害や集中困難などの症状を呈する。急性ストレス障害は、3日間以上持続する著しい苦痛を引き起こすが、心的外傷後ストレス障害(PTSD)とは異なり、1カ月以上持続することはない。患者の症状は、ホメオスタシスの障害(自律神経系、内分泌系、免疫系による心身症、過剰適応状態、神経性食思不振症、神経性過食症など)、精神障害(抑うつ反応、神経症反応、躁鬱病、不安症、パニック障害、睡眠障害など)としても現れることがある。
心的外傷後ストレス障害は、直接的又は間接的に経験される、恐怖感、無力感、又は戦慄を感じさせる出来事(例えば、自己又は他者が重傷を負ったり死の脅威に曝されたりする、戦闘、性的暴行、自然災害、人災)などに起因する、あまりにも受け入れがたいストレスによって、不適応になった状態である。心的外傷後ストレス障害では、外傷的出来事の悪夢やフラッシュバック、感情的な麻痺、うつ病、他の不安症、薬物乱用、睡眠障害などの症状が観られることがあり、急性ストレス障害の続きとして現れる場合もある。診断は、DSM-5の基準に基づいて臨床的に行われる。
〔1〕 乳酸菌ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティス又はその乳酸菌由来成分を有効成分とするストレス障害の予防又は改善用剤;
〔2〕 前記ストレス障害が、適応障害、自律神経失調症、抑うつ反応、うつ病、神経症反応、躁鬱病、不安症、パニック障害、睡眠障害、てんかん、統合失調症、非定型精神病のいずれか1以上である、前記〔1〕記載のストレス障害の予防又は改善用剤;
〔3〕 前記乳酸菌が、ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティスKLAB-4株及びその変異体からなる群から選択される乳酸菌を含む、前記〔1〕又は〔2〕記載のストレス障害の予防又は改善用剤;
〔4〕 水溶性食物繊維成分をさらに含有する、又は水溶性食物繊維成分と併用される、前記〔1〕~〔3〕のいずれか1項記載のストレス障害の予防又は改善用剤;
〔5〕 前記水溶性食物繊維成分が、グルコマンナン、ペクチン、グアーガム、アルギン酸、ポリデキストロース、βグルカン、フルクタン、イヌリン、レバン、グラミナン、アラビアガム、マルチトール、サイリウム、難消化性オリゴ糖、難消化性デキストリン、アガロース、アルギン酸ナトリウム、カラギーナン、フコイダン、ポルフィラン、ラミナラン、海藻、及び寒天からなる群から選択される、前記〔4〕記載のストレス障害の予防又は改善用剤;
〔6〕 前記水溶性食物繊維成分が、グルコマンナンである、前記〔5〕記載のストレス障害の予防又は改善用剤;
〔7〕 前記〔1〕~〔6〕のいずれか1項記載のストレス障害の予防又は改善用剤を含む、組成物;
〔8〕 前記〔1〕~〔6〕のいずれか1項記載のストレス障害の予防又は改善用剤を含む、医薬品組成物;
〔9〕 前記〔1〕~〔6〕のいずれか1項記載のストレス障害の予防又は改善用剤を含む、飲食品組成物;
〔10〕 前記〔1〕~〔6〕のいずれか1項記載のストレス障害の予防又は改善用剤を含む、動物用飼料又は動物用医薬品組成物;
〔11〕 前記〔1〕~〔6〕のいずれか1項記載のストレス障害の予防又は改善用剤、あるいは前記〔7〕~〔10〕のいずれか1項記載の組成物を、対象に投与することを含む、ストレス障害の予防又は改善方法;
が提供される。
本発明において使用される乳酸菌ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティスは、発酵乳又はチーズなどの発酵食品などから単離することができ、あるいは、単離された菌株を入手して使用することもできる。本発明において使用される乳酸菌としては、ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティス KLAB-4株(Lactobacillus delbrueckii subsp. lactis KLAB-4、以下「KLAB-4株」ともいう)及びその変異体からなる群から選択される乳酸菌を含むことが望ましい。
本発明のストレス障害の予防又は改善用剤には、上記のとおりの乳酸菌又は乳酸菌由来成分に加えて、水溶性食物繊維成分を含ませることができる。このような成分をさらに含むことにより、本発明の剤の効果が増強されることが見出された。
本発明のストレス障害の予防又は改善用剤は、上記のとおりの乳酸菌又は乳酸菌由来成分を唯一の必須成分とするが、水溶性食物繊維成分のような他の有効成分を含有又は併用することができる。本発明のストレス障害の予防又は改善用剤を含む組成物は、本発明の剤に加えて、一以上の他の成分を含む。
乳酸菌ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティスとして、KLAB-4乳酸菌粉末(商品名「乳酸菌LAB4」(株式会社カネカ製))を、水溶性食物繊維成分としてグルコマンナン(商品名「レオレックスRS」(清水化学株式会社製))を使用し、以下の3種のカプセル剤を製造した。
1カプセル当たり、乳酸菌粉末100mg、グルコマンナン粉末50mg、結晶セルロース粉末17.5mg、ステアリン酸カルシウム粉末2mg、二酸化ケイ素粉末0.5mgを混合、撹拌して均一にした後、市販のカプセル(商品名「ブタゼラチンクリア3号」(クオリカプス株式会社製))に充填した。
製造例2:
1カプセル当たり、乳酸菌粉末100mg、乳糖50mg、結晶セルロース粉末17.5mg、ステアリン酸カルシウム粉末2mg、二酸化ケイ素粉末0.5mgを混合、撹拌して均一にした後、製造例1で使用したものと同じ市販のカプセルに充填した。
比較例:
乳酸菌粉末及びグルコマンナン粉末の代わりに、乳糖150mgを含む以外は製造例1と同じカプセル剤を製造した。
本研究は、(公財)国際全人医療研究所の倫理委員会の承認のうえで、十分なインフォームド・コンセントを得て実施した(2017年、研究期間:2017年12月~2019年6月)。
専門医によりストレス障害と診断された患者64例(男性32例、女性32例)を、封筒法によって、プラセボ(P)群、乳酸菌群(L)群、及び乳酸菌+グルコマンナン(M)群の3群に分け、二重盲検法により、本発明の剤の効果を試験した。
評価は、CHCWによって行った。投与開始前及び終了後に、各被験者にCHCWを記入してもらい、変化を比較した。個人情報保護のため、質問紙の個人名は消去し、番号で示した。
図1は、投与開始前及び終了後に採点したCHCWの変化を比較した図である。投与前の各群間の値には有意差はなかった。また、P群については、投与後に有意に悪化した。一方、L群及びM群では、投与前と比較して投与後において有意に改善した。L群(**:p<0.01)とM群(***:p<0.001)とを比較すると、M群の方が改善の有意差が大きかった。

図2に示すとおり、P群に対し、L群(**:p<0.01)及びM群(***:p<0.001)では、有意にCHCWが上昇した。L群とM群とを比較すると、M群ではL群に対し、有意にCHCWが上昇した(※※:p<0.01)。
各群について、投与開始前及び投与終了後に、血液及び尿検体を採取し、血算(赤血球、白血球、ヘモグロビン、ヘマトクリット、血小板)、Alb、GOT、GPT、LDH、AlP、γ-GTP、アミラーゼ、BUN、クレアチニン、血糖値、ヘモグロビンA1c、尿(蛋白、糖、ウロビリノゲン)の検査を行って、結果を比較した。
Claims (10)
- 乳酸菌ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティスを有効成分とするストレス障害の予防又は改善用剤。
- 前記ストレス障害が、適応障害、自律神経失調症、抑うつ反応、うつ病、神経症反応、躁鬱病、不安症、パニック障害、睡眠障害、てんかん、統合失調症、非定型精神病のいずれか1以上である、請求項1記載のストレス障害の予防又は改善用剤。
- 前記乳酸菌が、ラクトバチルス・デルブリッキー・サブスピーシーズ・ラクティスKLAB-4株を含む、請求項1又は2記載のストレス障害の予防又は改善用剤。
- 水溶性食物繊維成分をさらに含有する、又は水溶性食物繊維成分と併用される、請求項1~3のいずれか1項記載のストレス障害の予防又は改善用剤。
- 前記水溶性食物繊維成分が、グルコマンナン、ペクチン、グアーガム、アルギン酸、ポリデキストロース、βグルカン、フルクタン、イヌリン、レバン、グラミナン、アラビアガム、マルチトール、サイリウム、難消化性オリゴ糖、難消化性デキストリン、アガロース、アルギン酸ナトリウム、カラギーナン、フコイダン、ポルフィラン、ラミナラン、海藻、及び寒天からなる群から選択される、請求項4記載のストレス障害の予防又は改善用剤。
- 前記水溶性食物繊維成分が、グルコマンナンである、請求項5記載のストレス障害の予防又は改善用剤。
- 請求項1~6のいずれか1項記載のストレス障害の予防又は改善用剤を含む、ストレス障害の予防又は改善用組成物。
- 請求項1~6のいずれか1項記載のストレス障害の予防又は改善用剤を含む、ストレス障害の予防又は改善用医薬品組成物。
- 請求項1~6のいずれか1項記載のストレス障害の予防又は改善用剤を含む、ストレス障害の予防又は改善用飲食品組成物。
- 請求項1~6のいずれか1項記載のストレス障害の予防又は改善用剤を含む、ストレス障害の予防又は改善用動物用飼料又は動物用医薬品組成物。
