Description
本出願は、2010年11月8日出願の「組織領域を通るチャネルを形成するための電気外科手術デバイスおよびその使用方法」と題された米国出願12/926,292、2011年10月31日に出願された米国特許出願13/286,041、及び2011年11月1日に発行された米国特許8,048,071の内容を、それら全体を参照することにより組み込まれている。本出願は、2013年3月12日に出願された、出願番号61/777,368の仮出願「支持ワイヤを有する医療用デバイス」を参照することにより組み込まれている。
(ii)前記電気外科手術デバイスを使用して、少なくとも部分的に閉塞を横断するエネルギを送達し、
(iii)ステップ(ii)の前後の少なくともいずれか一方において、前記電気外科手術デバイスの位置を決定するために、前記電気外科手術デバイスによって規定された圧力送達ルーメンを使用して圧力を測定する、方法である。いくつかの実施形態は、ステップ(i)で、第1の所望の位置にあることを確認するために電気外科手術デバイスを介して造影流体を送達することを含む。いくつかの実施形態は、さらに、(iv)第2の位置へ電気外科手術デバイスを進め、(v)電気外科手術デバイスによって規定された圧力送達ルーメンを介した圧力測定値を、1つまたは複数使用して、または電気外科手術デバイスを介して造影流体を供給することによって、電気外科手術デバイスが第2の位置にあることを確認する、のステップを備える。
Claims (12)
- 電気エネルギを送達するための導電性の壁を規定する細長い部材と、
前記細長い部材は、遠位部と近位部とを備え、
前記遠位部は、柔軟性を有しており、
前記導電性の壁は、前記細長い部材の前記遠位部から前記近位部に亘って延設されたルーメンを規定するものであり、
前記細長い部材の導電性の壁に電気的に連結された電極を含み、前記細長い部材の前記遠位部より遠位にある遠位先端部を規定するエネルギ送達デバイスと、
前記細長い部材の壁への支持するための支持スパインと、を備え、
前記細長い部材の前記近位部および前記エネルギ送達デバイスの前記遠位先端部は、前記細長い部材の前記遠位部よりも柔軟性が低く、
前記支持スパインは、前記ルーメン内において前記導電性の壁に固着せずかつ接触可能な状態で前記エネルギ送達デバイスから近位に延び、前記遠位先端部に剛性を提供する、電気外科手術デバイス。 - 前記細長い部材の前記遠位部は、少なくとも部分的に貫通する切り込みを有する金属層を含む、請求項1に記載の電気外科手術デバイス。
- 前記支持スパインは、導電性であると共に長手方向のいずれかの位置で前記細長い部材に接触可能となるように長手形状かつ柔軟であり、かつ前記エネルギ送達デバイスを前記導電性の壁に電気的に結合するように動作が可能である、請求項2に記載の電気外科手術デバイス。
- 前記細長い部材を覆う絶縁層と、前記電極と前記細長い部材との間の電気絶縁性熱シールドと、をさらに備え、
前記電気絶縁性熱シールドの少なくとも一部は、前記絶縁層を熱から保護するために、前記絶縁層と前記電極との間に配置されている、請求項3に記載の電気外科手術デバイス。 - 前記絶縁層の先端部は、前記細長い部材の遠位端の遠位にあり、前記絶縁層の前記先端部は、前記電気絶縁性熱シールドの近位部を覆っている、請求項4に記載の電気外科手術デバイス。
- 前記電気絶縁性熱シールドは、前記細長い部材の前記遠位部よりも高い剛性を有する、請求項5に記載の電気外科手術デバイス。
- 前記電気絶縁性熱シールドは、セラミックで構成されている、請求項6に記載の電気外科手術デバイス。
- 前記エネルギ送達デバイスは、前記細長い部材の前記導電性の壁に電気的に前記電極を結合するエネルギ送達デバイスカプラを含む近位部を有する、請求項7に記載の電気外科手術デバイス。
- 前記エネルギ送達デバイスは、前記電気絶縁性熱シールドを支持する中間部分を有する、請求項8に記載の電気外科手術デバイス。
- 前記エネルギ送達デバイスカプラは、少なくとも部分的に前記ルーメンの遠位部を閉塞する導電性スペーサを備え、前記導電性スペーサは前記導電性の壁と電気的に連結している、請求項8に記載の電気外科手術デバイス。
- 前記エネルギ送達デバイスは、さらに、前記導電性スペーサと前記電極との間に延在する中間導電性素子を有する中間部分を含み、前記電気絶縁性熱シールドは、前記中間導電性素子に取り付けられている、請求項10に記載の電気外科手術デバイス。
