Description
本発明は、医学の分野に関する。より具体的には、本発明は、プラスミノーゲンを含む医薬組成物、及びその治療のための使用に関する。
プラスミノーゲンは、図1Aに示すプラスミンの酵素前駆体(チモーゲン)である。ヒトプラスミノーゲン前駆体のアミノ酸配列を図1Bに示す。ヒトプラスミノーゲンは、約90kDaの分子量及び約7.0のpIを有する791個のアミノ酸(前駆体=810個のアミノ酸)を含むが、N末端活性化ペプチドの異なるグリコシル化及び/または除去は、6.2〜8.0の範囲のpIをもたらし得る。それは、24の鎖内ジスルフィド架橋、5つのクリングル・ドメイン(フィブリンとインヒビターα2−アンチプラスミンとの結合に関与する)、セリンプロテアーゼドメイン(P)、及び最初の77個のアミノ酸から構成される活性化ペプチド(AP)を含む一本鎖タンパク質である。第2のO−結合部位が同定されているが(Goldberg,2006)、1つのN結合グリコシル化部位及び1つのO−結合部位が存在する。循環中のプラスミノーゲンの約70%はO−結合グリコシル化のみを含み、残りはN−及びO−結合糖の両方を含む。

約3.0〜約10.0、好ましくは約5.0〜約8.0;より好ましくは約6.0〜約8.0;さらに好ましくは約6.5〜約7.5のpHを有する医薬組成物が記載される。
本発明の化合物(複数可)は、キットの一部として容器(例えば、包装、箱、バイアル等)を任意に含んで包装することができる。キットは、本明細書に記載される方法に従って市販のものを使用してもよく、本発明の方法で使用するための説明書を含み得る。
「安定な」組成物としては、その中のタンパク質(プラスミノーゲンまたはその変異体)が、保存時の、その物理的安定性及び/または化学的安定性及び/または生物学的活性を実質的に保持する組成物が挙げられる。タンパク質安定性を測定するための種々の分析技術が利用可能であり、例えば、Lee、V.,1991,Peptide and Protein Drug Delivery,247−301(Marcel Dekker,Inc.,New York,NY)及びJones,A. 1993,Adv.Drug Delivery Rev. 10:29−90に記載されている。選択された期間、選択された温度で安定性を測定することができる。一実施形態では、組成物は、室温(約25℃)または40℃で、少なくとも1、2、3、4、5または6ヶ月間安定であり、及び/または約2−8℃で少なくとも1、2、3、4、5または6ヶ月安定である。さらに、特定の実施形態では、組成物は凍結(例えば、−30℃)後に安定である。特定の実施形態において、安定性の基準は、以下の通りである:(1)組成物は、視覚的分析によって透明にとどまっている;(2)組成物の濃度、pH及び重量オスモル濃度が約±10%以下の変動しかない;(3)SEC−HPLCで測定した凝集体形態が約10%以下、約5%以下、または約1%以下である;(4)当技術分野で公知の方法及び十分に確立された分析方法によって測定される、プラスミノーゲンの分解が10%以下、約5%以下、または3%以下である。
安定化百分率=100×[S−T]/[T]
以下の医薬組成物の安定性試験結果を、WO2006/120423に記載の結合親和性技術に従って、血漿から抽出した5mg/mlのヒトプラスミノーゲン、10mMクエン酸塩、150mM NaCl、pH6.5(Lot:2002.pc02_Pg140210.01)を含む医薬組成物を−20℃、−30℃、−80℃で6ヶ月間保存して、得た。安定性データは、これらの凍結条件下でプラスミノーゲンが安定であることを示した。プラスミノーゲンlot:2002.pc02_Pg140210.01は、−20℃、−30℃及び−80℃で試験した。凝集レベルをSEC−HPLCにより、純度レベルを還元条件下及び非還元条件下でのSDS−PAGEにより、活性レベルをBCS活性により、そしてタンパク質濃度を280nmにおける吸光度(ABS280)により、調査した。6ヶ月後の安定性データの結果は、全ての安定性指示アッセイが、所望の仕様を達成したことを示した。
製剤1〜6のオスモル濃度及び製剤1〜9についての凍結乾燥後の固体の全パーセンテージを表2に報告する。製剤1〜9の含量を表1Aに定義する。
本発明の医薬組成物中で精製され製剤化されたプラスミノーゲンの分子量は、プラスミノーゲンのアミノ酸の一次配列の計算によって決定され、質量分析によって確認された分子量87,000(MW)である。
製剤1〜5は、表5に詳述されたサイクルプロセスの後に凍結乾燥された。製剤1〜5の含量は表1Aに規定されている。
プラスミノーゲン単独では水中では、FDAによって許容されない、即ち100mLの組成物当たり10μm以上の粒子6000個超の、多数のプラスミノーゲン凝集体が生成する(表6のサンプルF0406参照)。プラスミノーゲンを製剤化するために、種々のスクロースに基づく製剤が開発されている。これらの製剤の大部分は、100mL当たり10μm以上の6000粒子の閾値を満たすことに成功したが、より低い粒子含量を含む製剤が開発され、本明細書ではプラスミノーゲンを安定化するアミノ酸の効果が研究されている。これらのさらなる製剤は、10mgのプラスミノーゲン及び35mMのNaClを含有しており、3つの緩衝液/pH、即ち10mMクエン酸ナトリウム(pH6.5)、10mMリン酸ナトリウム(pH7.2)及び10mMトリスHCl(pH8.0)が試験された。アルギニン、アラニン及びグリシンから選択されるアミノ酸の存在及び非存在を0.5%の濃度で比較した。試験された製剤は新鮮なもの、即ち凍結乾燥の前である。表6は、10μm以上及び25μm以上の粒子のPMC数を報告している。アミノ酸の存在は、安定化効果が凝集レベルの低下に寄与していることを示している。
10mMのクエン酸ナトリウム(pH6.5)、10mMのリン酸ナトリウム(pH7.2)または10mMのトリスHCl(pH8.0)のいずれかを含む10mgのプラスミノーゲン及び35mMのNaClを含有し、且つアミノ酸(0.5%アルギニン塩酸塩、アラニンまたはグリシン)を含有または含有しない製剤に対して、増量剤の0.5%マンニトールの存在(の効果)について試験し、表7に報告した。この研究では新鮮な製剤(lyo前)を試験した。一般に、マンニトールの存在は、粒子数を低下させた。一般に、マンニトールとアミノ酸の組み合わせは、粒子数を減少させ、特にアルギニンとマンニトールとの組み合わせにおいてそうである。
0.1%フェノール等の防腐剤の存在は、10mMのクエン酸ナトリウム(pH6.5)、10mMのリン酸ナトリウム(pH7.2)または10mMトリスHCl(pH8.0)のいずれかを含み、且つ0.5%アルギニン塩酸塩または0.5%アルギニン塩酸塩と0.5%マンニトールとの組み合わせを含む、9または10mgのプラスミノーゲン及び35mMのNaClを含有する製剤において試験した。この研究では、新鮮な製剤(lyo前)を試験した。粒子数(PMC)は表8に報告されている。有利なことに、フェノールの存在は粒子数に有意に影響しなかった。
凍結乾燥及び再構成の後、プラスミノーゲン10mg/mL、35mM NaCl、10mMリン酸ナトリウム(NaPi)、pH7.2の製剤で、0.5%及び1%のアルギニンを試験した。粒子数(PMC)を表10に報告されており、アルギニンの両方の濃度が低いPMC値を維持するのに非常に良好であることを示している。
10mg/mLのプラスミノーゲンのいくつかの製剤のオスモル濃度、及びこの製剤を調製するために使用される固形分のパーセンテージが、表12に報告されている。20mg/mLのプラスミノーゲンのいくつかの製剤のオスモル濃度、及びこの製剤を調製するために使用される固形分のパーセンテージが、表13に報告されている。試験された製剤の結果として得られるオスモル濃度は、有利にも、オスモル濃度の生理学的範囲内にあるか、または非常に近い。固形分の全(合計)パーセンテージは、その中のプラスミノーゲン、張性調整剤及び増量剤の含量から計算され、凍結乾燥した後、得られたケーキの大きさの良好な指標である。

この研究によって示されるように、プラスミノーゲン凝集は可逆的である。dH2O中で透析され、大量に沈殿した凍結プラスミノーゲンサンプル(Pgバルク)5mg/mlを融解し、1.2μmシリンジフィルタで濾過し、対照(Pg対照)のための出発物質として使用した。2mlのPg対照のアリコートを、1Mのクエン酸ナトリウムpH6.5(サンプル1)、0.5Mリン酸NapH 7.2(サンプル2)または1Mのトリス−HCl pH8.0(サンプル3)の緩衝液ストックを予めスパイク(混合)したガラスバイアルに加え、各pHの最終10mM緩衝液濃度をそれぞれ得る。サンプル1、2及び3を図15に示す、ここで非スパイクPg対照はサンプル0である。サンプル1、2及び3をさらに5M NaCl及び0.8Mアルギニンのストック溶液でスパイクして、各サンプルにおいて35mM NaCl及び28.5mM(0.5%)のArgの最終濃度を得、そして図16に示されている。各々0.02、20、100、800NTUの濁度標準が、参照として図15及び16で使用された。図15から、pH調節がかなりの量の凝集物を溶解したことが分かる。図16において、サンプル1、2及び3は、アルギニン及びNaClの添加によって透明にされ、凝集物は視覚的に検出され得ない。
濁度は、0.5%グリシン、アルギニン、アラニンを含み、及びアミノ酸を含まない10mgプラスミノーゲン及び35mM NaClを含有する組成物において、クエン酸ナトリウム緩衝液pH6.5(図17)で、リン酸ナトリウム緩衝液pH7.2(図18)で、及びトリス緩衝液pH8.0(図19)で測定した。
高濃度のプラスミノーゲンを含有する組成物を研究した。10mMまたは100mMのクエン酸ナトリウム、35mMのNaCl、pH6.5(アルギニン1%を含む及び含まない)中の20mg/mlのプラスミノーゲンの組成物、及びアルギニン1%を含む100mMクエン酸ナトリウム、35mM NaCl、pH6.5中の37及び56mg/mlのプラスミノーゲンの組成物について、10μm以上、及び25μm以上の粒子のPMC数が表17に報告されている。
表17は、本発明の製剤が、許容できない粒子レベルを引き起こさずにプラスミノーゲンを高濃度(37及び56mg/ml)で含む組成物の調製を有利に行うことができることを示している。
本明細書に例示した各製剤のプラスミノーゲン活性を試験し、初期製剤またはバルク製剤のプラスミノーゲン活性(Pgバルク)と比較した。 プラスミノーゲン活性は、これらのいずれの製剤によっても影響されないことが示された。
Claims (28)
- − 約0.01mg/ml〜約80mg/mlの濃度のプラスミノーゲン、または天然プラスミノーゲンポリペプチドとのアミノ酸配列同一性が少なくとも90%である、その生物学的に活性な変異体;
− 約30mM〜約100mMの濃度の塩化ナトリウムである張性調整剤;そして
− 約10mM〜約100mMの濃度の、アルギニンまたはグリシンである安定化剤
を含み、且つ
約5.0〜約8.0のpHを有する液体医薬組成物。 - 前記pHが6.0〜8.0である請求項1に記載の医薬組成物。
- 前記pHが6.0、6.1、6.2、6.3、6.4、6.5、6.6、6.7、6.8、6.9または7.0である請求項2に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体の濃度が、約5mg/ml〜約60mg/mlである請求項1〜3のいずれか1項に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体の濃度が、約40、30、20,10または5mg/mlである請求項4に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体の濃度が、約2mg/ml〜約20mg/mlである請求項1〜3のいずれか1項に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体が、当該組成物の全タンパク質含量の少なくとも80%であるか、または当該組成物の全タンパク質含量の90%を超過する請求項1〜6のいずれか1項に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体が、組成物の全タンパク質含量の95%または98%を超過する請求項7に記載の医薬組成物。
- 前記安定化剤が、アルギニンである請求項1〜8のいずれか1項に記載の医薬組成物。
- 前記安定化剤が、グリシンである請求項1〜8のいずれか1項に記載の医薬組成物。
- 前記安定化剤が約25mM〜約75mMの濃度を有する請求項1〜10のいずれか1項に記載の医薬組成物。
- 前記張性調整剤が、約34mM、約35mM、約68mMまたは約75mMの濃度で存在する請求項1〜11のいずれか1項に記載の医薬組成物。
- 前記張性調整剤が、約68mM〜約75mMの間の濃度で存在する請求項1〜11のいずれか1項に記載の医薬組成物。
- さらにスクロースである非還元糖を含む請求項1〜13のいずれか1項に記載の医薬組成物。
- 実質的にアプロチニンを含まない請求項1〜14のいずれか1項に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体が、ヒトプラスミノーゲンである請求項1〜15のいずれか1項に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体が、約80%を超えるGlu−プラスミノーゲンから構成されている請求項1〜16のいずれか1項に記載の医薬組成物。
- 前記プラスミノーゲンまたはその生物学的に活性な変異体がGlu−プラスミノーゲンである請求項1〜16のいずれか1項に記載の医薬組成物。
- 当該組成物が、容器あたり6000個未満の10μm以上の粒子量を含み、該容器が100ml以下の名目上の含量を有する請求項1〜18のいずれか1項に記載の医薬組成物。
- 前記粒子量が、容器あたり2000粒子未満であり、該容器が100ml以下の名目上の含量を有する請求項19に記載の医薬組成物。
- 静脈内、皮下、局所、皮内、眼科及び/または筋肉内投与に適している請求項1〜20のいずれか1項に記載の医薬組成物。
- 再構成組成物である請求項1〜21のいずれか1項に記載の医薬組成物。
- 薬剤として使用される請求項1〜22のいずれか1項に記載の医薬組成物。
- 対象において創傷を治癒する、鼓膜穿孔を治癒する、歯周創傷を治癒する、感染性疾患を治療する、口腔の健康を増強または維持する、後部硝子体剥離(PVD)を治療する、糖尿病性潰瘍を治療する、プラスミノーゲン欠乏症を治療する、血栓症を治療する、組織への再灌流障害を治療する、虚血を治療する、梗塞を治療する、または脳浮腫を治療するための薬剤を製造するための請求項1〜22のいずれか1項に記載の医薬組成物の使用。
- 前記薬剤が、対象において創傷を治癒するためのものである請求項24に記載の使用。
- 前記薬剤が、対象においてプラスミノーゲン欠乏症を治療するためのものである請求項24または25に記載の使用。
- 前記プラスミノーゲン欠乏症がI型プラスミノーゲン欠乏症である請求項26に記載の使用。
- 前記薬剤が、対象において糖尿病性潰瘍を治療するためのものである請求項24に記載の使用。
