本発明は、一般に、前立腺癌を検出およびモニタリングするために用いることができる新規バイオマーカーおよびバイオマーカーの組合せに関する。本発明はまた、一般に、本発明のバイオマーカーの検出を含む、前立腺癌を診断、モニタリング、および処置するための方法にも関する。
前立腺癌は、男性の癌関連死の主因であり、肺癌に次いで2位であり、65歳を超える年齢の男性の9人のうち1人が罹患する。American Cancer Societyによれば、241,000の新しい事例の前立腺癌が、同じ年に約30,000件の前立腺癌関連死と共に報告されている。この疾患は65歳を超える年齢の男性において典型的に診断されるが、前立腺癌で死亡する男性の平均生存期間は平均でほぼ10年減少するという点で、その影響は依然として大きい。しかしながら、前立腺癌が早期発見される場合、90%の事例は外科手術で治癒し得る。腫瘍が前立腺の領域外に広がり、遠隔転移を形成すると、疾患の処置はより困難となる。従って、早期検出が、介入療法の成功にとって、および前立腺癌と関連する死亡率を低下させるために非常に重要である。

前立腺癌と関連する腫瘍マーカーまたは抗原の同定は、前立腺癌のスクリーニング、診断、予後診断(prognosis)、臨床管理および処置可能性、特に、前立腺癌の早期検出のための有望なツールとしてかなりの興味を刺激してきた。実際、早期検出は、癌が転移する危険性を軽減する。転移していない、局所的な前立腺腫瘍は、前立腺全摘出術または放射線療法によって治癒することが多いが、遠く拡散した疾患を有する患者については、治癒的処置は利用できない。これは、正確な早期検出のための機会を改善し得る新しい前立腺(癌)特異的診断ツールの必要性を強調するものである。

本発明は、少なくとも部分的には、フィラミンAが前立腺癌細胞中で示差的に調節されるという発見に基づく。特に、本発明は、フィラミンAレベルが、前立腺癌を有する患者の血清中で有意に上昇するという驚くべき発見に基づく。従って、本発明は、哺乳動物における、腫瘍疾患状態、例えば、前立腺癌を診断および/またはモニタリング(例えば、疾患進行もしくは処置のモニタリング)および/または予後診断するための方法を提供する。本発明はまた、腫瘍疾患状態、例えば、前立腺癌を有する被験体の血清中のフィラミンAのレベルに関する診断情報に基づいて処置する、または処置レジメンを調整するための方法も提供する。本発明はさらに、本発明の方法を実施するためのパネルおよびキットを提供する。
本発明を、検出可能な疾患バイオマーカー、例えば、ポリペプチドバイオマーカー、核酸バイオマーカー、mRNAバイオマーカー、マイクロRNAバイオマーカーを潜在的に含有する、発現する、含む任意の好適な生物試料を用いて実行することができる。例えば、全血および血清を含む供給源から、疾患を有する、および/または健康な組織、例えば、前立腺腫瘍の生検まで、生物試料を取得することができる。本発明の方法は、任意の前立腺組織試料、すなわち、前立腺の組織または液体、ならびにそのような組織または液体から単離された細胞(またはその子孫)の試料の試験に特に適用することができる。別の実施形態においては、本発明を、新鮮に単離された、または被験体から収集された後、凍結もしくは保存された任意の好適な前立腺組織試料、または例えば、診断、処置および/または結果の履歴が知られた保管組織試料を用いて実行することができる。前立腺組織を、例えば、微細針吸引および針生検などの任意の非侵襲的な手段により、またはあるいは、例えば、外科生検などの侵襲的な方法により収集することができる。
本発明は、本発明のバイオマーカーを検出および/または測定するための任意の好適な手段、技術、および/または手順を企図する。当業者であれば、本発明のバイオマーカーを測定するために用いられる方法は、検出または測定されるバイオマーカーの型(例えば、mRNAバイオマーカーまたはポリペプチドバイオマーカー)および生物試料の供給源(例えば、全血対前立腺生検組織)に少なくとも依存することを理解できる。特定の生物試料はまた、本発明のバイオマーカーを測定する前に、特定の特殊な処理、例えば、mRNAバイオマーカーが測定される場合、生検組織からのmRNAの調製も必要とし得る。

本発明は、本発明のポリペプチドバイオマーカーを検出するための任意の好適な方法を企図する。特定の実施形態においては、検出方法は、本発明の1種以上のバイオマーカー、例えば、フィラミンAのみ、またはフィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、PSA、PSM、PSCA、TMPRSS2、PDEF、HPG-1、PCA3、およびPCGEM1からなる群より選択される少なくとも1種の他の前立腺癌関連マーカーと組合わせたフィラミンAに特異的に結合する抗体を含む免疫検出方法である。様々な有用な免疫検出方法のステップが、例えば、参照により本明細書に組込まれるNakamuraら(1987)などの科学文献に記載されている。

1. 単離されたポリペプチドバイオマーカー
本発明の一態様は、単離されたマーカータンパク質およびその生物学的に活性な部分、ならびにマーカータンパク質またはその断片に対する抗体を上昇させるための免疫原としての使用にとって好適なポリペプチド断片に関する。一実施形態においては、天然のマーカータンパク質を、標準的なタンパク質精製技術を用いる適切な精製スキームにより細胞または組織源から単離することができる。別の実施形態においては、マーカータンパク質の全部またはセグメントを含むタンパク質またはペプチドは、組換えDNA技術により生成される。組換え発現とは別に、そのようなタンパク質またはペプチドを標準的なペプチド合成技術を用いて化学的に合成することができる。
本発明の一態様は、マーカータンパク質またはその一部をコードする核酸を含む、単離された核酸分子に関する。本発明の単離された核酸はまた、マーカー核酸分子、およびマーカー核酸分子の断片を同定するためのハイブリダイゼーションプローブとしての使用にとって十分な核酸分子、例えば、特定の生成物の増幅またはマーカー核酸分子の突然変異のためのPCRプライマーとしての使用にとって好適なものも含む。本明細書で用いられる用語「核酸分子」は、DNA分子(例えば、cDNAもしくはゲノムDNA)およびRNA分子(例えば、mRNA)ならびにヌクレオチド類似体を用いて生成されるDNAもしくはRNAの類似体を含むことが意図される。核酸分子は、一本鎖または二本鎖であってもよいが、好ましくは、二本鎖DNAである。
本発明は、被験体における、異常な前立腺状態、例えば、BPHまたは腫瘍疾患状態、例えば、前立腺癌を診断するための方法を提供する。本発明はさらに、異常な前立腺状態、例えば、BPHまたは前立腺癌の進行を予後診断もしくはモニタリングする、または積極的処置もしくは経過観察中の治療的処置に対する異常な前立腺状態、例えば、BPHまたは前立腺癌の応答をモニタリングするための方法を提供する。
1. 診断アッセイ
生物試料中のマーカータンパク質または核酸の発現レベルの存在または非存在または変化を検出するための例示的方法は、試験被験体から生物試料(例えば、腫瘍性障害関連体液)を取得し、生物試料と、ポリペプチドまたは核酸(例えば、mRNA、ゲノムDNA、もしくはcDNA)を検出することができる化合物または薬剤と接触させることを含む。かくして、本発明の検出方法を用いて、例えば、in vitroならびにin vivoで、生物試料中のmRNA、タンパク質、cDNA、またはゲノムDNAを検出することができる。

本発明の特定の実施形態においては、マーカーは、核酸である。核酸は、例えば、成分の1つが自然に存在する化合物ではないため、検出しようとするマーカー核酸と、マーカー特異的プローブとの複合体が自然に存在しないいくつかのアッセイを用いて検出される。特定の実施形態においては、分析物は核酸を含み、プローブは1種以上の合成一本鎖核酸分子、例えば、DNA分子、DNA-RNAハイブリッド、PNA、または1個以上の人工塩基、糖、もしくは骨格部分を含有する改変された核酸分子を含む。特定の実施形態においては、合成核酸は一本鎖であり、蛍光標識を含むDNA分子である。特定の実施形態においては、合成核酸は、長さ約12〜約50ヌクレオチドの一本鎖オリゴヌクレオチド分子である。特定の実施形態においては、検出しようとする核酸はmRNAであり、形成される複合体はmRNAと相補的である一本鎖DNA分子にハイブリダイズしたmRNAである。特定の実施形態においては、RNAは、最初にプライマー、例えば、ポリA RNAを転写するための一般的なポリTプライマーとしてRNAにハイブリダイズする一本鎖DNAを用いる、RNA鋳型からのDNA分子(すなわち、cDNA分子)の生成により検出される。次いで、cDNAを、マーカー特異的プローブを用いる増幅反応、例えば、PCR、プライマー伸長アッセイのための鋳型として用いることができる。特定の実施形態においては、標識された一本鎖DNAを、蛍光in situハイブリダイゼーション(FISH)によるRNAの検出のため、またはノーザンブロットによるRNAの検出のために試料中に存在するRNAにハイブリダイズさせることができる。
マーカーレベルを、絶対発現レベルまたは正規化もしくは相対発現レベルに基づいて検出することができる。絶対マーカーレベルの検出は、被験体の処置をモニタリングするか、または被験体の前立腺癌状態の変化が存在するかどうかを決定する場合に好ましい。例えば、1種以上のマーカーの発現レベルを、例えば、毎月の間隔などの規則的間隔で、前立腺癌のための処置を受けている被験体においてモニタリングすることができる。1種以上のマーカーのレベルの調節を経時的にモニタリングして、マーカーレベルの変化における傾向を観察することができる。被験体における本発明のバイオマーカー、例えば、フィラミンAのみ、または前立腺特異的抗原(PSA)、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、PSM、PSCA、TMPRSS2、PDEF、HPG-1、PCA3、およびPCGEM1のうちの1種以上と組合わせたフィラミンAの発現レベルは、正常な試料中のこれらのマーカーの発現レベルよりも高くてもよいが、以前の発現レベルより低くてもよく、かくして、被験体のための処置レジメンの利益を示す。同様に、マーカーレベルの変化の速度は、前立腺癌のための積極的な処置を受けていない被験体(例えば、待機療法)において重要であり得る。マーカーレベルの変化があるかないかは、集団中に存在するマーカーレベルよりも被験体のための処置決定とより関連し得る。さもなければ正常な前立腺を有すると考えられる被験体におけるマーカーレベルの急速な変化は、そのマーカーが集団にとって正常な範囲にある場合でも、異常な前立腺状態を示し得る。
本発明は、
(1)被験体に由来する生物試料と、各検出試薬が1つの前立腺癌関連タンパク質に特異的であり、前立腺癌関連タンパク質が、以下のような前立腺癌関連タンパク質:フィラミンAのみ、または前立腺特異的抗原(PSA)、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、PSM、PSCA、TMPRSS2、PDEF、HPG-1、PCA3、およびPCGEM1のうちの1種以上と組合わせたフィラミンAから選択される、1種以上の検出試薬のパネルとを接触させること;
(2)各検出試薬によって生物試料中で検出された各前立腺癌関連マーカーの量を測定すること;ならびに
(3)被験体から得られた生物試料中の1種以上の前立腺癌関連タンパク質の発現レベルと、対照試料中の1種以上の前立腺癌関連タンパク質の発現レベルとを比較することによって、異常な前立腺状態を検出すること
により、被験体において異常な前立腺状態を検出するための方法を提供する。
(1)被験体に処置レジメンの少なくとも一部を施す前に被験体から得られた第1の生物試料と、検出試薬のパネルとを接触させることであって、ここでそれぞれの検出試薬が1つの前立腺癌関連タンパク質に特異的であり、前立腺癌関連タンパク質が以下の前立腺癌関連タンパク質セット:フィラミンAのみ、または前立腺特異的抗原(PSA)、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、PSM、PSCA、TMPRSS2、PDEF、HPG-1、PCA3、およびPCGEM1のうちの1種以上と組合わせたフィラミンAから選択され;

により、前立腺癌の処置をモニタリングするための方法を提供する。

(4)第1の試料中の1種以上の前立腺癌関連マーカーの発現のレベルと、第2の試料中の1種以上の前立腺癌関連マーカーの発現レベルとを比較すること
による、被験体における前立腺癌の積極的処置の施行の選択または積極的処置の施行に反対する選択のための方法であって、前立腺癌の積極的処置の施行の選択または積極的処置に反対する選択が、第1の試料と第2の試料との1種以上のマーカーの発現のレベルの変化の存在または非存在に基づくものである、前記方法を提供する。
(1)第1の時点で前立腺癌を有する被験体から得られた第1の試料中のフィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、およびチューブリン-ベータからなる群より選択される1種以上のマーカーのレベルを検出することであって、ここで被験体は前立腺癌について積極的に処置されておらず;
(2)第2の時点で被験体から得られた第2の試料中のフィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、およびチューブリン-ベータ3からなる群より選択される1種以上のマーカーのレベルを検出することであって、例えば、ここで被験体は積極的に処置されておらず;

を含み、前立腺癌の積極的処置の施行または積極的処置の施行への反対を選択することは、第1の試料と第2の試料との間の1種以上のマーカーの発現のレベルの変化の存在または非存在に基づくものである、前記方法を提供する。
本発明のマーカーの発現のレベルに対する薬剤(例えば、薬物化合物)の影響のモニタリングを、単一の被験体の処置の基礎的薬物スクリーニングまたはモニタリングだけでなく、臨床試験においても適用することができる。例えば、マーカー発現に影響する薬剤の有効性を、腫瘍性障害のための処置を受けている被験体の臨床試験においてモニタリングすることができる。好ましい実施形態においては、本発明は、薬剤(例えば、アゴニスト、アンタゴニスト、ペプチド模倣物質、タンパク質、ペプチド、核酸、低分子、または他の薬物候補)を用いる被験体の処置の有効性をモニタリングするための方法であって、(i)薬剤の投与の前に被験体から投与前試料を取得するステップ;(ii)投与前試料中の本発明の1種以上の選択されたマーカー(例えば、場合によりPSAと組合わせた、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3)の発現のレベルを検出するステップ;(iii)被験体から1種以上の投与後試料を取得するステップ;(iv)投与後試料中のマーカーの発現のレベルを検出するステップ;(v)投与前試料中のマーカーの発現のレベルと、投与後試料中のマーカーの発現のレベルとを比較するステップ;ならびに(vi)それに応じて被験体への薬剤の投与を変化させるステップを含む、前記方法を提供する。例えば、処置の経過中のマーカー遺伝子の発現の増加は、用量が無効であること、および用量を増加させることが望ましいことを示してもよい。逆に、マーカー遺伝子の発現の低下は、処置が有効であること、および用量を変化させる必要がないことを示してもよい。
本発明は、被験体、例えば、哺乳動物、例えば、ヒトにおける疾患状態を処置するための、フィラミンA、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、フィラミンB(FNLB)、およびリンパ球抗原9(LY9)からなる群より選択される1種以上(例えば、1、2、3、4、5、6、7、8、または9種)のマーカーの使用のための方法を提供する。
核酸治療剤は、当業界で周知である。核酸治療剤は、細胞中の標的配列と相補的である一本鎖および二本鎖の両方(すなわち、1つまたは2つの核酸鎖であってもよい少なくとも15ヌクレオチドの長さの相補的領域を有する核酸治療剤)の核酸を含む。核酸治療剤を、例えば、核酸を、培養培地に単独で、または薬剤と一緒に添加して、細胞中への核酸の取込みを促進することにより、培養中の細胞に送達することができる。核酸治療剤を、任意の投与経路により、被験体中の細胞に、すなわち、in vivoで送達することができる。特定の製剤は、投与経路に依存する。
典型的には、約16〜30ヌクレオチド長である、アンチセンス核酸治療剤一本鎖核酸治療剤は、培養物または生物中で、標的細胞中の標的核酸配列と相補的である。
多くの実施形態において、二本鎖領域は、長さ15〜30ヌクレオチド対である。いくつかの実施形態においては、二本鎖領域は、長さ17〜23ヌクレオチド対、長さ17〜25ヌクレオチド対、長さ23〜27ヌクレオチド対、長さ19〜21ヌクレオチド対、または長さ21〜23ヌクレオチド対である。
上述の通り、発現レベルが1種以上の選択された前立腺疾患特徴(例えば、前立腺癌の進行)と相関するバイオマーカーのセットは、これらのバイオマーカー遺伝子および/またはその産物の発現を阻害または増強する化合物または実体を検出するためのスクリーニングによる新しい治療剤の同定のための魅力的な標的である。従って、本発明は、前立腺癌進行を調節するのに潜在的に有用な化合物の同定のための方法を提供する。特に、本発明は、化合物が、フィラミンA、ならびに/または前立腺特異的抗原(PSA)、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、およびチューブリン-ベータ3などの他のバイオマーカーと組合わせたフィラミンAの発現を調節する(例えば、増加させる、または減少させる、好ましくは、減少させる、または阻害する)前立腺癌進行を調節するのに潜在的に有用な化合物の同定のための方法を提供する。
本発明はまた、疾患もしくは障害、障害の再発、または障害(例えば、異常な前立腺状態、BPH、腫瘍障害、例えば、前立腺癌)について処置される被験体の生存を診断、予後診断、またはモニタリングするための組成物およびキットも提供する。これらのキットは、1つ以上の以下のもの:本発明のマーカーに特異的に結合する検出抗体、本発明のマーカーに特異的に結合する検出抗体、染色のための被験体組織試料を取得および/または調製するための試薬、ならびに使用のための説明書を含む。

(実施例)
本発明は、限定と解釈されるべきではない以下の実施例によってさらに例示される。本出願を通して引用される全ての参考文献、GenBank受託番号および遺伝子番号ならびに公開された特許および特許出願の内容は、参照により本明細書に組込まれるものとする。当業者であれば、開示された構造、材料、組成物および方法に対する変更を用いて本発明を実行することができ、そのような変更が本発明の範囲内にあると見なされることを認識できる。
細胞外ケラチンは、上皮由来前立腺癌の細胞増殖および転移に影響することが知られている。アンドロゲン不応性前立腺癌は、正常組織と比較した場合、ケラチン8(K8)の示差的発現を示す。次に、ケラチンの調節および分解は、活性酸素種(ROS)のミトコンドリアでの生成により媒介される。これらの進歩にも拘らず、前立腺癌の転移および増殖におけるケラチンおよび他のECタンパク質の理解に対する体系的アプローチは不足している。
疑問システム生物学に基づく探索プラットフォームを用いて(すなわち、Interrogative Platform Technologyまたは別名、Interrogative Biology(商標))、前立腺癌細胞の挙動におけるミトコンドリアの役割の理解における機構的洞察を得た。WO2012119129に詳述されるこのプラットフォーム技術は、in vitroのヒト細胞に基づくモデルおよび前立腺癌患者に由来するヒト血清試料を含むシステムの階層にわたる探索ならびに人工知能(AI)に基づく情報モジュールを用いる下流のデータ統合および数学的モデリングを含む。
プラットフォーム技術を用いて前立腺癌マーカーとしてフィラミンBを同定したら、正常な被験体および前立腺癌を有する被験体から得られたヒト血清試料を用いて、前立腺癌マーカーとしてのフィラミンBを確認した。
実施例3において用いた同じヒト血清試料をさらに試験して、LY9の存在を検出した。LY9のための商業的に入手可能なELISA試験を、商業的供給源から入手した。アッセイを、製造業者の説明書を用いて実施した。アッセイから得られた結果を、図7に示す。この結果は、前立腺癌を有さない対照被験体と比較した、前立腺癌の診断を有する患者におけるLY9のレベルの差異を示す。示されるように、前立腺癌を有する被験体から得られた試料は、正常被験体と比較してより高レベルのLY9を有することがわかった。ヒト血清中のフィラミンBとLY9の両方の発現レベルのアッセイから得られた結果を、タンパク質のng/mlとして表した結果と共に、図8に示す。
フィラミンBのレベルが前立腺癌を有する被験体の血清中で増加することが証明されたら、その結果を同じ試料中のPSAレベルの試験と共に分析して、フィラミンBとPSAの一緒になった予測値が、いずれかのマーカー単独よりも良好であるかを決定した。PSA、フィラミンB、およびPSAとフィラミンBとの組合せの感度および偽陽性率(FPR)の受信者操作特性(ROC)曲線分析を生成した。この曲線と曲線下面積(AUC)値を、図9AおよびBに示す。この分析の目的は、特異的カットオフとは無関係の試験の予測力を測定することであった。ROC分析を用いた場合、正常状態と疾患状態との間の完全な識別または精度をもたらす試験はAUC=1を有するが、無作為な機会より良好でない識別をもたらす非常に弱い試験はAUC=0.5を有する。
それぞれのフィラミンB、LY9、およびPSAが前立腺癌を有する被験体から得られた血清試料中で全て上昇することを証明したら、線形スコアリング関数を用いて、3つのマーカーのそれぞれを個別に3つ全部のマーカーの組合せと比較し、非線形スコアリング関数を用いて、3つ全部のマーカーの組合せに対する、フィラミンBとLY9との組合せ、およびフィラミンBとPSAとの組合せを比較するROC曲線分析を実施して、マーカーの組合せが被験体における前立腺癌の検出のためのそれぞれ単一のマーカーよりも有効であることを決定した。示されるように、3つ全部のマーカーの組合せは、いずれかのマーカー単独よりも予測性が高かった(図10A)。LY9を含むか、または含まない、フィラミンBとPSAとの組合せは、フィラミンBとLY9との組合せよりも予測性が高かった(図10B)。追加の試料を分析して、結果をさらに改良することができる。AUCの結果を、表にまとめる。
それぞれ実施例3および4で示されたように、フィラミンBレベルとLY9レベルとを用いて、前立腺癌に罹患しているか、または罹患していない被験体を識別することができる。さらに、実施例5および6で示されたように、場合によりLY9とさらに組合わせた、フィラミンBとPSAの両方の分析は、いずれかのマーカー単独に基づく分析よりも高感度である。同様に、マーカーであるケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、およびチューブリン-ベータ3を、ヒト試料中で、実施例3〜6に記載されたように分析する。
前立腺癌を有するとの診断の時点で、被験体は試験に参加するよう招かれる。被験体試料、例えば、血液を取得する。定期的に、モニタリング、待機療法、または被験体の積極的処置、例えば、化学療法、放射線療法、外科手術、ホルモン療法を通じて、新しい被験体試料を取得する。試験の終わりに、全ての被験体試料を、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3のうちの少なくとも1種、好ましくは、ケラチン7、ケラチン15、およびケラチン19のうちの少なくとも1種;場合によりさらにフィラミンB、LY9、およびPSAのうちの少なくとも1種の発現レベルについて試験する。被験体試料を、被験体の医療記録と一致させて、マーカーレベルを、診断時の前立腺癌状態、疾患の進行速度、1つ以上の介入に対する被験体の応答、およびアンドロゲン依存的と非依存的状態の間の移行と相関させる。前立腺癌に罹患していない被験体から得られた正常試料と比較した、ケラチン19、フィラミンB、LY9、およびPSAのうちの1種以上の発現レベルの増加は、被験体における前立腺癌を示す。ケラチン7、8および15の発現レベルはまた、前立腺癌を有する被験体の診断およびモニタリングにおいて特に有用であり得る。
わずか25〜40%の正の予測値などのその限界にも拘らず、PSAは依然として唯一の一般に受け入れられている前立腺癌のためのバイオマーカーである。さらに、前立腺癌は最も一般には、高齢男性においてゆっくり増殖する腫瘍であるため、この癌の処置は、腫瘍自体よりも被験体にとってより有害である可能性がある。従って、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3のうちの少なくとも1種、好ましくは、ケラチン7、ケラチン15、およびケラチン19のうちの少なくとも1種;場合によりさらにフィラミンB、LY9、およびPSAのうちの少なくとも1種の発現レベルに関する試験を一緒に、前立腺癌の検出およびモニタリングのために用いる。単独の、および様々な組合せのマーカーの発現レベルを、前立腺癌の危険性が高い男性(例えば、高齢、家族歴、民族など)における日常的な、防止的、スクリーニング方法などの検出において、またはさらなる、潜在的により侵襲性の診断試験、例えば、前立腺検査もしくは生検、デジタル直腸検査、またはより積極的な処置を必要とする被験体を良好に同定するのに有用であり得る、処置前もしくは処置中の前立腺癌と診断された被験体のモニタリングにおいて用いる。マーカー、またはその様々な組合せの発現のレベルの検出はまた、他の兆候または症状の変化、例えば、ホルモン療法に対する腫瘍応答の喪失の前に、特定の処置レジメンに対する良好な、または弱い応答を示してもよい。
それぞれ実施例3および4に示されたように、フィラミンBレベルおよびLY9レベルを用いて、前立腺癌に罹患しているか、または罹患していない被験体を識別することができる。さらに、実施例5および6に示されたように、場合によりさらにLY9と組合わせた、フィラミンBとPSAの両方の分析は、いずれかのマーカー単独に基づく分析よりも高感度である。

ELISAアッセイを市販のヒト血清試料上で行って、前立腺癌を有する、および有さない患者に由来する試料中のフィラミンAおよびケラチン19のレベルを検出した。
本発明者らは、アンドロゲン非依存的PC3およびアンドロゲン依存的LNCaP前立腺癌細胞系などのいくつかの癌および正常細胞系に基づいて前立腺癌のin vitroモデルを開発した。Berg Interrogative Biology(商標)により推測された重要な調節結節点を、前立腺癌のバイオマーカーとしてヒト血清中での概念実証の検証のために選択した。
正常な個体および前立腺癌を有する個体に由来するヒト血清試料を、実施例13に従って、商業的供給源から獲得した。バイオマーカーのパネルを、実施例13に従って、市販のELISAキットによって測定した。

この概念実証試験の結果に基づいて、KRT19およびFLNAは、前立腺癌を有する患者と正常な個体とを区別する能力を有する。これらの2つのバイオマーカーを、市販の、または社内で開発されたELISAアッセイによって、より大きい臨床検証試験において評価することができる。上記の結果は、FLNaおよびKRT19が前立腺癌の統計的に有意なバイオマーカーであり、従って、現在のスクリーニングツールを超える有意な改良であり得ることを示している。

Mt. Sinai Hospital、Toronto、Canadaから、回顧的および臨床的に注釈された試料セットを取得した。試料マトリックスは血清であり、その収集およびプロセッシング以来凍結されている。
血清試料を、662人の男性患者から収集した。全患者を、前立腺生検のために前立腺センターに照会した。これらの患者は、臨床症状、デジタル直腸検査に基づいて、または、より頻繁には、血清PSA上昇(3ng/mlを超える)のため、前立腺癌について疑わしかった。患者は、個人面接によってこの研究に参加することに同意し、臨床操作または前立腺生検の前に採血した。

検定および検証試験は、PCAパネルの4つの臨床適用における有用性を示す。開発されたモデルは、侵攻性の低い形態を侵攻性の高い形態から識別する能力を有意に改善した。グリソンスコア8以上を有する患者から採取した試料を識別するためのAUCは0.8であった(PSA単独に関する0.67に対する)。患者値がモデルのカットオフより低い場合、負の予測値はこのモデルについて非常に高く、患者が疾患を有さない確率は95%であった。
前立腺癌は、男性の間で最も多い癌診断であり、癌関連死の第2位の主因である。前立腺癌スクリーニングのためのデジタル直腸検査(DRE)および前立腺特異的抗原(PSA)の血液に基づくスクリーニングの幅広い使用にもかかわらず、その特異性および予後診断値における有意な制限がある。i)良性前立腺過形成(BPH)からPSAが低い前立腺癌を識別する、およびii)遅発性と侵攻性の疾患経過を識別するバイオマーカーは、満たされない臨床必要性の代表である。実験的に、前立腺癌細胞系および非腫瘍性ヒト一次細胞のパネルを、酸素供給不足、低pH、栄養微小環境の縮小、および代謝摂動(24〜48h)を刺激するように設計されたin vitro条件に曝露した後、細胞溶解物のiTRAQプロテオミック分析を行った。次いで、疑問生物学プラットフォームを用いて、プロテオミックデータをベイジアンネットワーク学習にかけて、分子相互作用をマッピングし、細胞骨格および足場タンパク質フィラミンA(FLNA)、フィラミンB(FLNB)、およびケラチン19(KRT19)を候補前立腺癌バイオマーカーとして同定した。

Claims (22)
- 被験体における前立腺癌の存在を診断するための方法であって、前記被験体は増大した前立腺特異的抗原(PSA)レベルを示す被験体であり、(a) 前記被験体の生物試料中のフィラミンAのレベルを検出すること、および(b)生物試料中のフィラミンAのレベルと、正常な対照試料中のフィラミンAのレベルとを比較することを含み、正常な対照試料中のフィラミンAのレベルより上のフィラミンAのレベルが、被験体における前立腺癌の存在を示す、前記方法。
- 前立腺癌の1種以上のさらなるマーカーのレベルを検出することをさらに含む、請求項1に記載の方法。
- 前立腺癌の1種以上のさらなるマーカーが、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、および前立腺特異的抗原(PSA)からなる群より選択される、請求項2に記載の方法。
- ステップ(a)が、生物試料中のフィラミンAタンパク質のレベルを決定することを含む、請求項1に記載の方法。
- フィラミンAタンパク質のレベルがイムノアッセイ、ELISAまたは質量分析によって決定される、請求項4に記載の方法。
- 被験体における前立腺癌の存在を診断するための方法であって、前記被験体は増大した前立腺特異的抗原(PSA)レベルを示す被験体であり、(a)生物試料と、フィラミンAに選択的に結合する試薬とを接触させること;(b)試薬とフィラミンAとの間で複合体を形成させること;(c)複合体のレベルを検出すること、および(d)複合体のレベルと、正常な対照試料中のフィラミンAのレベルとを比較することを含み、正常な対照試料中のフィラミンAのレベルより上の複合体のレベルが被験体における前立腺癌の存在を示す、前記方法。
- 前立腺癌の1種以上のさらなるマーカーのレベルを検出することをさらに含む、請求項6に記載の方法。
- 前立腺癌の1種以上のさらなるマーカーが、フィラミンB、LY9、ケラチン4、ケラチン7、ケラチン8、ケラチン15、ケラチン18、ケラチン19、チューブリン-ベータ3、および前立腺特異的抗原(PSA)からなる群より選択される、請求項7に記載の方法。
- 1種以上のさらなるマーカーのレベルが所定の閾値と比較して増加するか、または1種以上のさらなるマーカーのレベルが所定の閾値と比較して低下する、請求項7または8に記載の方法。
- 生物試料が血液、血清、尿、臓器組織、生検組織、糞便、皮膚、毛髪、および頬組織からなる群より選択される、請求項1または6に記載の方法。
- 前立腺癌が前立腺上皮内新生物、腺癌、小細胞癌、または扁平上皮癌であるか、または前立腺癌がアンドロゲン依存的前立腺癌、アンドロゲン非依存的前立腺癌、侵攻性前立腺癌、または非侵攻性前立腺癌である、請求項1または6に記載の方法。
- 被験体の治療的抗癌処置のための組成物であって、該被験体は、請求項1〜6のいずれか1項に記載の方法により被験体における前立腺癌の存在が示される被験体であり、前記組成物は、化学療法物質、ホルモン、抗体、サイトカイン、増殖因子、およびそのいずれかの組合せからなる群より選択される物質を含む、前記組成物。
- 前立腺癌を有すると疑われる、または有する危険性がある被験体を選択することをさらに含む、請求項1〜11のいずれか1項に記載の方法。
- 請求項1〜6のいずれか1項に記載の方法が、前立腺癌を有すると疑われる、または有する危険性がある被験体を選択することをさらに含む、請求項12に記載の組成物。
- 生物試料中の1種以上の前立腺癌関連マーカーのレベルと、生物試料よりも早い時点で同じ被験体から得られた試料、良性前立腺過形成(BPH)を有する被験体に由来する試料、非転移性前立腺癌を有する被験体に由来する試料、転移性前立腺癌を有する被験体に由来する試料、アンドロゲン感受性前立腺癌を有する被験体に由来する試料、アンドロゲン非感受性前立腺癌を有する被験体に由来する試料、侵攻性前立腺癌を有する被験体に由来する試料、および非侵攻性前立腺癌を有する被験体に由来する試料からなる群より選択される対照試料中の1種以上の前立腺癌関連マーカーのレベルとを比較することをさらに含むか、または
正常な前立腺と前立腺癌、良性前立腺過形成と前立腺癌、良性前立腺過形成と正常前立腺、アンドロゲン依存的前立腺癌とアンドロゲン非依存的前立腺癌、侵攻性前立腺癌と非侵攻性前立腺癌、および転移性前立腺癌と非転移性前立腺癌からなる群より選択される2つの前立腺癌状態の間を区別することをさらに含む、請求項1〜11及び13のいずれか1項に記載の方法。 - 請求項1〜11のいずれか1項に記載の方法、又は請求項13に記載されている方法が、生物試料中の1種以上の前立腺癌関連マーカーのレベルと、生物試料よりも早い時点で同じ被験体から得られた試料、良性前立腺過形成(BPH)を有する被験体に由来する試料、非転移性前立腺癌を有する被験体に由来する試料、転移性前立腺癌を有する被験体に由来する試料、アンドロゲン感受性前立腺癌を有する被験体に由来する試料、アンドロゲン非感受性前立腺癌を有する被験体に由来する試料、侵攻性前立腺癌を有する被験体に由来する試料、および非侵攻性前立腺癌を有する被験体に由来する試料からなる群より選択される対照試料中の1種以上の前立腺癌関連マーカーのレベルとを比較することをさらに含むか、または
正常な前立腺と前立腺癌、良性前立腺過形成と前立腺癌、良性前立腺過形成と正常前立腺、アンドロゲン依存的前立腺癌とアンドロゲン非依存的前立腺癌、侵攻性前立腺癌と非侵攻性前立腺癌、および転移性前立腺癌と非転移性前立腺癌からなる群より選択される2つの前立腺癌状態の間を区別することをさらに含む、請求項12または14に記載の組成物。 - 被験体に由来する生物試料中のフィラミンAのレベルを測定するための少なくとも1つの試薬、および被験体に由来する生物試料中のフィラミンAのレベルを測定するための説明書のセットを含む、請求項1〜11、13及び15のいずれか1項に記載の方法に使用するためのキット。
- 説明書が、生物試料中のフィラミンA mRNAの量を決定するためのハイブリダイゼーションアッセイを説明する、請求項17に記載のキット。
- キットが、フィラミンA mRNAの一部と相補的な少なくとも1つのオリゴヌクレオチドをさらに含み、かつ場合により説明書が、被験体に由来する生物試料中のフィラミンAのレベルと、フィラミンAの閾値とを比較することをさらに説明する、請求項18に記載のキット。
- 説明書が、生物試料中のフィラミンA mRNAまたはタンパク質の量を決定するためのハイブリダイゼーションアッセイを説明する、請求項17に記載のキット。
- キットが、フィラミンA mRNAの一部と相補的な少なくとも1つのオリゴヌクレオチド、またはフィラミンAタンパク質またはその断片と結合する少なくとも1つの抗体をさらに含み、かつ場合により説明書が、被験体に由来する生物試料中のフィラミンAのレベルと、フィラミンAの閾値とを比較することをさらに説明する、請求項20に記載のキット。
- 被験体の年齢を決定するための説明書をさらに含む、請求項17〜21のいずれか1項に記載のキット。
