Description
骨関節症(OA)は変形性関節症としても知られており、老人や肥満者に多い疾病である。OAは関節の疾病であるが、慢性関節リウマチ(RA)と異なり、全身性ではなく、通常は1またはいくつかの関節のみに影響する。この疾病は関節軟骨の完全崩壊、下の骨の硬化、および骨増殖体の形成に繋がり、結果として動きが悪くなり痛みが出る。最終的には関節を完全に交換する必要があることもある。
様々な種類の疼痛が、米国の医師への相談において最も高頻度の理由であり、毎年全アメリカ人の半数が医療的ケアを求めている。それは、内科的疾患において、人間の生活の質及び一般的な機能を著しく妨げる主要な症状である。診断は、持続時間、強度、種類(鈍い、焼けるような又は刺すような)、原因又は体の位置により、色々な方法で疼痛を特徴的付けることに基づいている。一般に疼痛は治療をせずに治まり、又は休息や鎮痛剤服用などの簡単な対策に反応するものであり、それはその後急性疼痛と言われる。しかし、疼痛がもはや症状ではなく、疼痛そのものが病気である慢性疼痛と呼ばれる難治性の進行性の状態になる場合もある。
・疼痛を生じさせた刺激
・疼痛の持続時間
・疼痛の特徴(強さ、場所、等)
・絶え間ない疼痛。一定の強さが連続する。通常運動することによって悪化する。
・ズキズキする疼痛。片頭痛の典型である。脳の血管の拡張と収縮が原因である。
・突き刺すような疼痛。強く激しい疼痛である。機械的刺激が原因である。
・焼けるような疼痛。例えば胸焼けのような、絶えず焼けるような感覚である。
・圧迫されるような疼痛。血管又は筋肉の収縮が原因である。
・筋肉の疼痛。筋肉痛として知られており、筋肉に作用し過度の運動の後や炎症時に生じる。
・疝痛。月経期間の子宮のように特定の器官の筋肉収縮が原因である。
・関連痛。体から受けた情報を脳がどのように解釈するかにより、実際に生じた場所ではない場所で痛みが生じる。
・外科的手術後又は手術後の疼痛。外科的手術後に生じ、外科的措置の損傷によるものである。
・骨肉腫の疼痛。前立腺、乳房、又は他の軟部組織の腫瘍といった特定の種類の癌は、転移性骨疾患として知られる骨の痛みを伴う疾患に進行する場合がある。
痛みを治療するにはいくつもの方法がある。治療は痛みの原因によって変化する。主な治療オプションは以下の通りである:
アセトアミノフェン:タイレノール(アセトアミノフェン)を疼痛治療に用いる。他の痛み止めと異なり、タイレノールは抗炎症作用がない。しかしながら、慢性痛の場合はしばしば疼痛部位に炎症がないため、タイレノールが適切な治療となる。タイレノールは適切に用いれば安全であるが、過度に用いると危険となりうる。また、タイレノールは特定の他の薬剤と共に用いると、望まない副作用を生じることがある。
c−Fms(CSFR1、M−CSFRc−Fms)は、マクロファージの生成、分化、および機能を制御するサイトカインである、群体刺激因子1の受容体である(後述)。c−Fmsは、M−CSFの生物学上の作用の殆どあるいはすべてを媒介する。リガンド結合は、オリゴマー化とトランスリン酸化のプロセスを介してCSFR1を活性化する。コード化された蛋白質はチロシンキナーゼ膜受容体であり、チロシン−蛋白質キナーゼのCSF1/PDGF受容体ファミリのメンバーである。CSFR1遺伝子の第1イントロンは、転写不活性リボソーム蛋白質L7処理偽遺伝子を含み、これはCSFR1遺伝子の反対方向に配置されている。

検証するには、Ghose et al.,J Combin Chem:1:55−68,1999およびLipinski et al.,Adv Drug Del Rev 23:3−25,1997を参照されたい。
この実験において、c−Fmsに特異的な単クローン抗体を使用して、M−CSFアンタゴニストが骨関節症を治療するのに有効であり得ることを実証した。同様の実験が、疼痛の治療におけるc−Fmsアンタゴニストの有用性を実証する。
コラゲナーゼ誘発OAモデル(Blom et al.(2004)Osteoarthiritis Cartilage.12;627−635;Blom et al.(2007)Arthiritis Rheum.56,147−57)は、片側の関節内にコラゲナーゼを注射することにより、関節不安定性を誘発させるモデルである。これは、一般に関節の安定を補助する靭帯の弱体化の原因となり、誘導後6週間以内にOAの病状が発生する。このモデルでは、注入されたコラゲナーゼによる軟骨の直接の被害は観察されない。OAの病状の特徴は、軟骨破壊、滑膜線維症および骨棘形成が含まれる。骨棘形成を媒介する滑膜マクロファージに伴う滑膜の活性化がある。OA進行の初期段階では(0−14日)、骨棘形成および線維症がみられる。関節内で骨の形成が最初に起こる場所(軟骨表面に近い骨膜と、骨と靭帯の接合部位)は、ヒトのOAで見られるものと同様である。新骨形成のプロセスも同様であり、骨膜の活性化があり、続いて軟骨様組織の生成、さらに軟骨内骨化となる。疾病の最初の2週間には骨膜病変もみられ、滑膜の肥厚につながるマクロファージの流入、そして深い滑膜層の一部の炎症細胞がある。軟骨基質の侵食を含む完全なOAの病理は、OA誘導の6週間後までは見られない。
C57BL/6マウスに、関節不安定性を誘導するために0日目および2日目に右膝にコラゲナーゼタイプ関節内VIIを1単位与えた(Blom et al.(2004)Osteoarthritis Cartilage. 12627−35を参照)。
30匹のマウスを以下のように無作為に2つの群(15匹/群)に分けた:
・グループ1(n=15):抗c−Fms抗体
・グループ2(n=15):IgG2aアイソタイプコントロール抗体
OA導入後20日目の平均の疼痛の読みは、0日目(すべてのマウスでp<0.0001)に比べて顕著に高かった(すなわち、関節症の膝から体重が有意にシフトしている)。マウスは20日目で2つの治療群に分けられ、42日目まで、2x/週で適切なmAbで処置された。
実施例1のサンプルを、組織学的にも調べた。

抗c−Fms抗体治療が関節炎の発達に影響したかどうかを判定するために、42日目に膝関節に組織診断を行った(図2参照)。抗c−Fms抗体処置マウスは、コントロールmAb処置マウスと比較して内側頸骨の症状が顕著に穏やかだった(p≦0.01)。一般的に、抗c−Fms抗体で処置したマウスは、アイソタイプコントロール抗体と比較して、組織学的に分析されたすべての関節領域で重症度が低かった(図2参照)。
c−Fmsアンタゴニストを用いる処置の有効性を評価すべく、術後痛を想定した疼痛モデルを用いた。
220−240グラムの体重のオスのSDラットを、手術前に1週間動物施設でならしておく。
手術は、Brennan et al.,Pain 64:493−501,1996に記載の処置に基づいている。動物をノーズコーンを介して手術中は維持されている2%イソフルラン混合空気で麻酔する。右後足の底側面をポビドンヨードパッドで処理し、かかとの端0.5cmからつま先に向かって、皮膚と筋膜を通して1cmの中央の縦方向の切開を設ける。屈曲状態に保持された足を定規で測定する。湾曲鉗子を使用して足底筋を持ち上げ、縦に切開する。始点と挿入の間の深さ全体を通して筋肉が切開される。出血はガーゼパッドを介して圧力をかけて、手術を通じて制御される。傷は2つの刺し縫い縫合で閉鎖される(5−0ethilon黒モノフィラメント)。これらの縫合糸は最初のノットはゆるく結ばれ、5−6回結ばれる。創傷部位はバシトラシン液で消毒される。動物は、行動試験を開始する前に、清潔なケージで2時間以上休ませて回復させる。
蓄積した疼痛スコアは、体重荷重に関して疼痛を評価するために使用される。動物を持ち上げられたプラットフォーム(高さ18インチ)上の透明なプラスチックケージ内でプラスチックメッシュ(グリッド:8mm2)に載置し、足の下の検査を可能にする。20分の順応期間の後、0から2のスケールで体重荷重を測定する。スコア0は、足がメッシュに沈んでいるか押しつけられている場合であり、完全に荷重がかかっていることを示す。スコア1は、足がかばわれてメッシュに触れる程度であり、皮膚の沈みやくぼみがない。スコア2は、足が完全にメッシュから離して保持されている場合である。ラットがまだ休息している場合、足が畏縮していれば2と考えられる。動物はそれぞれ5分毎に1分間観察される。1/2時間で得られる6つのスコアの合計(0〜12)を用いて障害のある足が検査される。スコア2の頻度も計算され、激しい疼痛の影響あるいは動物が足を完全にかばっているのを判定するのに用いられる。動物はそれぞれ術前24時間(ベースライン)、術後2時間、24時間、48時間、72時間と試験される。この実験結果は、c−Fmsアンタゴニストで処置された動物で観察された安静時疼痛累積スコアが対照動物と比較して有意に減少していることを示す。荷重は動物がその肢を使用したいかによく相関しており、そのため疼痛緩和の測定に有効である。好ましくは、このc−Fmsアンタゴニストは、c−Fms、特にIL−34またはM−CSFの特異的な抗体である。このような抗体は、切開の15時間前に、様々な濃度(例えば動物の体重のキロ当たり0.004、0.01、0.02、0.1、0.6および1mg)で腹腔内(i.p.)に注射される。陰性コントロール群は抗体を受けないが、生理食塩水を腹腔内注入する。0.01mg\kgのフェンタニルを、手術後24時間かつ試験前の30分に、陽性対照として腹腔内注射する。各実験に各状況の8の動物を用い(群ごとにn=8)、コントロール群は56匹の動物を有する。手術が行われ、上記のように累積疼痛スコアを測定する。安静時の疼痛を、24時間の手術後に評価する。
熱痛覚過敏症は、Hargreaves et al.(1988)の改変方法の後のラットプランタ試験(Ugo Basile、イタリア)によって評価される。ラットを高架ガラステーブルの上に4つの個別のプレキシガラスのボックスで構成される装置に慣らしておく。モバイル式放射熱源をテーブルの下に位置し、後足にフォーカスする。動物は安静であるが寝ていない状態で、コントロールボックスのボタンを押して放射熱源を作動させ、熱源から動物が逃げようとする時間を自動記録する。この足撤退潜時(POOL)は、放射源の反射率の変化によって、放射熱源に組み込まれラットの足の動きを感知する光検出器によって検出される。足離脱潜時(PWL)は、秒単位で記録された。ここで、組織の損傷を防止するために、22.5秒の自動カットオフ点を設けた。動物のそれぞれの両後足毎に3〜4回PWLをとり、左右の後足についてベースラインを表す平均をとった。結果は、左足に対する右足(手術部位)の測定スコア比率で示される。装置は(研究の開始時に)一度較正され、6秒の通常PWLを得るために強度40に設定した。動物はそれぞれ、術前24時間(ベースライン)、術後3時間、24時間、48時間、72時間で試験された。熱痛覚過敏の測定は、接触異痛症の測定後に行われた。結果は、c−Fmsアンタゴニストによる治療が大幅に手術後の熱痛覚過敏症を減少させることを実証した。
例えば、M−CSF特異的抗体、IL−34特異的抗体またはc−Fms特異的抗体などのc−Fmsアンタゴニストは、骨転移を伴う癌の疼痛の治療に有効である。
動物は、腫瘍移植から10日目と14日目に痛みに関連する行動について試験する。動物の行動が以下の試験によりテストされた:継続的な痛み(自発的な防御と畏縮)、歩行痛(四肢の使用およびロータロッド)、および運動誘発痛(接触誘発による防御や、接触誘発による畏縮)。動物は、ワイヤメッシュ床の透明なプラスチック観察箱に入れ、30分間慣れさせる。順化後に、自発的防御、自発的畏縮、解放空間の通常の四肢使用歩行、および強制歩行時の防御を評価した。接触誘発による防御と畏縮が、肉腫注射および偽注射した動物における遠位大腿骨への通常は有害でない接触から2分間測定された。
6日目と13日目に、肉腫を注入した動物は、例えば抗M−CSF、抗IL−34または抗c−Fms受容体抗体といったM−CSFアンタゴニストを腹腔内に注入し(n=5)、あるいは肉腫注射と偽注射を行った胴部は、生理食塩水を腹腔内に注入した(各条件でn=5)。10日目と14日目のすべての動物の行動分析を行った。
(生理食塩水を投与した)肉腫注入動物は、(生理食塩水を投与した)偽注射した動物と比較して、統計的に有意に継続する疼痛を示しており、これは自発的防御および自発的畏縮により査定される。
(生理食塩水を投与した)肉腫注入動物は、(生理食塩水を投与した)偽注入動物と比較して、肢の使用と歩行防御で評価して(ロータロッド)歩行痛がみられた。肉腫移植から10日目と14日目において、生理食塩水を投与した肉腫注入マウスと比べて、c−Fmsアンタゴニストを投与した肉腫注入マウスにおける肢使用スコアと強制歩行防御スコアは大幅に増大する。これらの結果は、c−Fmsアンタゴニストは肉腫注入マウスにおける歩行痛を軽減することを示している。
(生理食塩水を投与した)肉腫注入動物は、(生理食塩水を投与した)偽注入動物と比較して、接触誘発防御および接触誘発畏縮の評価において、接触惹起疼痛行動がみられた。肉腫移植から10日目と14日目において、生理食塩水を投与した肉腫注入マウスと比べて、c−Fmsアンタゴニストを投与した肉腫注入マウスにおける接触誘発防御および接触誘発畏縮が大幅に低減する。これらの結果は、c−Fmsアンタゴニストは肉腫注入マウスにおける接触惹起疼痛を軽減することを示している。
ラットにおける完全フロイントアジュバント(CFA)誘発性慢性関節炎を、インドメタシンを基準物質とした場合と比較して、調査した。
以下の実験は、c−Fmsアンタゴニストは、炎症性疼痛の治療にも有効であることを示している。これを行うべく、mBSA/IL−1単関節関節炎をM−CSFノックアウトマウスおよびコントロールマウスに誘発させた。インキャパシタンステスタを使用して、様々な時点で、疼痛緩和物質であるインドメタシンの投与の有無について評価した。
24匹のオスのC57BL/6マウスと、24匹のオスのM−CSF−/−マウスを4つの処置群に用いた。

10μlのmBSA(20mg/ml)を膝関節に関節内注射して単関節関節炎を誘発させ、反対側の膝関節に10μlの生理食塩水を注射する。20μlのIL−1β(250ng)を3日間毎日皮下投与する。反応は一般にmBSA注射後4日から7日間で発生し、28日目までに鎮静する。2、3、4、5、および7日目にインキャパシタンスで試験される。
インキャパシタンステスタ(Dual Weight Averager)は、2本の後ろ足における体重分配を測定することで、鎮痛効果を自動的に再現性よく評価するのに使用した。使用者が選択した期間にわたり、各足によってもたらされる力(グラム単位で測定)の平均をとり、動物の片方の足からもう片方の足に体重移動する傾向を提示し、インキャパシタンスの定量的測定を提供する。
このモデルは、mBSAを注入されることにより、膝関節に滑膜炎が誘発される。7日目に膝関節は、視覚的に検査され、0(正常)から3(重度の炎症)のスコアが与えられる。mBSAが注入された左の膝は、右の膝(生理食塩水が注入された)と比較して、著しい炎症を起こした。実際、(生理食塩水が注入された)右膝はすべてスコア0だった。インドメタシンで治療したマウスとそれらで治療しない間には、顕著な違いはみられなかった。
以下の実験は、炎症性疼痛の治療において、c−Fmsアンタゴニストの有効性を明らかにする追加の実験である。ここで炎症性疼痛は、完全フロイドアジュバントで誘発されるものである。実施例6のように、疼痛を疼痛緩和物質であるインドメタシンの投与にかかわらず、インキャパシタンス検査装置を用いて様々な時点で評価した。
12匹のオスのC57BL/6マウス及び12匹のオスのM−CSF−/−マウスがそれぞれ3つの治療グループで使用された。
グループ1:C57BL/6野生型(n=12):CFA
グループ2:C57BL/6野生型(n=12):CFA+インドメタシン
グループ3:M−CSF KO(n=12):CFA
完全フロインドアジュバント(CFA)(Sigma)は、1mg/mlの濃度の鉱油中に、熱滅菌したヒト型結核菌株、H37Raを含む。熱滅菌したバクテリアが完全に懸濁液に組み込まれるように、ボルテックスでCFAを完全に混合した(Kamala T Hock immunization:a humane alternative to mouse footpad injections.J Immunol Methods 328:204−214、2007)。ボルテックスの後すぐに、アジュバントを、19ゲージ針を用いたガラスシリンジで吸い取った。気泡をシリンジから丁寧に取り除いてから、針を取り去った。各マウスは、CFAエマルション20μlを左後ろ足(足蹠)に皮下注射された。疼痛評価1時間前にグループ2のマウスの腹腔内に、1mg/kgのインドメタシンを投与した(実施例6参照)。
実施例6のように、インキャパシタンステスタ(Dual Weight Averager)を、2本の後足の体重分配を計測することで自動的で再現性のある鎮痛効果の評価をするために使用した。各後足が体重をかけた場所を5秒にわたって測定した。マウス1匹につき各時点で3回別個に測定し、次いで平均をとった。結果は、注入した足/コントロールの足×100として表示する。値が100であれば、右足と左足にかけられた体重が等しいことを意味する。100以下の値は、コントロールの足(右)と比較すると、注入された足(左)に体重がかかっていないことを意味する。CFA注入の24、48、72時間後にインキャパシタンスによる検査をした。
左の足蹠にCFAを皮下注射した後、C57BL/6(グループ1)とM−CSF−/−(グループ3)マウスで同様な大きさの腫れが、左の足蹠に示された。インドメタシンで治療されたC57BL/6マウス(グループ2)も、腫れの程度に違いはなかった。いずれのグループも対側(右)の足に腫れは見られなかった。
膝の骨関節症を患っている成人患者に臨床試験を行った。ランダムで二重盲式のプラセボ対照臨床試験の目的は、膝の骨関節症(OA)と診断された患者30人の総サンプルにおいて、本発明のc−Fmsアンタゴニストとプラセボの疼痛緩和とクオリティオブライフ全般の比較差を判定することである。別の目的は、本発明のc−Fmsアンタゴニストの安全性および忍容性を、有害事象、身体検査およびバイタルサインによるように決定することである。
30人の患者(およそ15人の成人男性と15人の成人女性)は、40歳以上であり、膝の骨関節症と診断されており、研究試験の前に、月に少なくとも15日間膝の痛みが証明されている。患者は、c−Fmsアンタゴニストまたはプラセボの治療上有効な量を受ける(約6ヶ月間、例えば2週間に1回)。
身体の痛みの変化:プラセボと比較して、本発明のc−Fmsアンタゴニストによる治療を受けた患者では、SF−36v2の身体の痛みの改善が統計的に顕著である。高いスコアは、患者が製品を用いた後に疼痛が弱まっていることを意味するため、優れている。プラセボ群に対して本発明のc−Fms拮抗を受けた群では、身体・疼痛スコアにおいて統計学的に顕著な改善がある。
臨床試験は、膝の骨関節症患者の生活の質を改善するのに、本発明のc−Fmsアンタゴニストの有効性を示した。臨床試験の結果はまた、重篤な有害作用は見られず、製品の安全性および耐性を示している。
実施例1を、M−CSFに特異的な抗体を用いて繰り返した。本書で上述したように、コラゲナーゼ誘発性のOAモデルは、Blom et al.(2004)Osteoarthritis Cartilage.12;627−35 and Blom et al.(2007)Arthritis Rheum.56;147−57に基づくものである。特に示さない限り、実験の詳細は実施例1で概説したものと同様である。

グループ2(n=15):IgG2aアイソタイプコントロール抗体。
Claims (7)
- 骨関節痛または疼痛の治療用の医薬組成物であって、前記医薬組成物が、c−Fmsアンタゴニストを含み、前記アンタゴニストが、M−CSFに特異的な抗体、c−Fmsに特異的な抗体またはIL−34に特異的な抗体であり、前記医薬組成物が、薬学的に許容される担体、希釈剤または賦形剤をさらに含むことを特徴とする医薬組成物。
- 請求項1に記載の医薬組成物において、前記疼痛が、外科的手術後の疼痛であることを特徴とする医薬組成物。
- 請求項1に記載の医薬組成物において、前記疼痛が、骨肉腫の疼痛であることを特徴とする医薬組成物。
- 請求項1に記載の医薬組成物において、前記疼痛が、リウマチ性関節痛であることを特徴とする医薬組成物。
- 請求項1に記載の医薬組成物において、前記疼痛が、骨関節症の疼痛であることを特徴とする医薬組成物。
- 請求項1に記載の医薬組成物において、前記疼痛が、炎症性疼痛であることを特徴とする医薬組成物。
- 請求項1乃至6のいずれかに記載の医薬組成物において、前記医薬組成物がヒトに用いられることを特徴とする医薬組成物。
