様々な種類の疼痛が、米国の医師への相談において最も高頻度の理由であり、毎年全アメリカ人の半数が医療的ケアを求めている。それは、内科的疾患において、人間の生活の質及び一般的な機能を著しく妨げる主要な症状である。診断は、持続時間、強度、種類(鈍い、焼けるような又は刺すような)、原因又は体の位置により、色々な方法で疼痛を特徴的付けることに基づいている。一般に疼痛は治療をせずに治まり、又は休息や鎮痛剤服用などの簡単な対策に反応するものであり、それは急性疼痛と言われる。しかし、疼痛がもはや症状ではなく、疼痛そのものが病気である慢性疼痛と呼ばれる難治性の進行性の状態になる場合もある。

・筋肉の疼痛。筋肉痛として知られており、筋肉に作用し過度の運動の後や炎症時に生じる。
・疝痛。月経期間の子宮のように特定の器官の筋肉収縮が原因である。
・関連痛。体から受けた情報を脳がどのように解釈するかにより、実際に生じた場所ではない場所で痛みが生じる。
・外科的手術後又は手術後の疼痛。外科的手術後に生じ、外科的措置の損傷によるものである。
・骨肉腫の疼痛。前立腺、乳房、又は他の軟部組織の腫瘍といった特定の種類の癌は、転移性骨疾患として知られる骨の痛みを伴う疾患に進行する場合がある。
疼痛治療には、多くの方法がある。疼痛の原因によって治療が変わる。主な治療は以下の通りである。
・アセトアミノフェノン:タイレノール(アセトアミノフェン)は、疼痛を治療するのに用いられている。他のいくつかの疼痛用の薬物とは違い、タイレノールは抗炎症作用がない。しかし、慢性疼痛であって疼痛の部位に炎症がない場合は、しばしばタイレノールは適切な治療の選択となりうる。タイレノールは、適切に使用すれば安全であるが、過度に使用すると危険な場合がある。さらに、タイレノールは、他の特定の薬物と共に使用すると、望ましくない効果をもたらすことがある。
・非ステロイド性抗炎症薬(NSAIDs):NSAIDs(イブプロフェン、モトリン、アリーブ等のような)は、急性疼痛又は慢性疼痛の患者が再発した場合に最も有益である。NSAIDsはまた、腱炎、滑液包炎、及び関節炎などの炎症性疾患の治療にも優れている。胃の疾患の進行を懸念して、一般に、NSAIDは慢性疼痛を患う患者に使用を限定している。セレブレックスなどCOX−2阻害剤と呼ばれる新商品が、この合併症を回避する目的で作られたが、これらの薬を長期間にわたり使用する際には、依然として注意が必要である。
・コルチコステロイド:NSAIDsと同様に、コルチコステロイドは、強力な抗炎症薬であり、急性疼痛又は慢性炎症疾患の再発の使用に最適である。コルチコステロイドは、経口投与(メドロール、プレドニゾン等)か、軟部組織又は関節へ注入(コルチゾン注射)が可能である。
・麻薬:麻薬を使用しないと疼痛が制御できない場合に麻薬の使用を考慮すべきである。多くの麻薬は、危険であり中毒になる。麻薬の投与は、急性疼痛に有用であるが、重大な副作用も有している。これらの薬の短時間作用型は、乱用と耐性を生じさせることがある。長時間作用型の選択は副作用が少なく、慢性疼痛を制御するのにより優れている。麻薬は、投与量を徐々に減らさずに長期間使用したり、疼痛以外の理由で薬を摂取した場合に中毒になる場合がある。
・抗けいれん誘発剤:抗けいれん誘発剤は、神経痛を緩和するカテゴリーの薬剤である。これらの薬は、神経や脳に送られる信号の機能を変化させる。神経痛の抗けいれん誘発剤で最も一般的に処方されているのは、Neurontin(ガバペンチン)と呼ばれるものである。線維筋痛治療専用で近年浮上している別の選択は、リリカ(プレガバリン)と呼ばれるものである。
・局所麻酔薬:局所麻酔薬はある部位の一時的な疼痛を軽減する。慢性疼痛に使用するときに、局所麻酔薬はしばしば局所パッチとして疼痛部位に貼付される。リドダームは、皮膚に貼付し、この部位の感度を低下させるパッチとして販売されている。
顆粒球マクロファージコロニー刺激因子(GM−CSF)は、白血球成長因子として機能するサイトカインである。GM−CSFは、幹細胞を刺激して顆粒球(好中球、好酸球、及び好塩基球)と単球を生成する。単球は、血液循環を出て、組織に移動するとすぐに成熟してマクロファージになる。これは、少数のマクロファージを活性化し、その数を急速に増加させる自然免疫/炎症カスケードの一部であり、感染症と戦う不可欠な過程である。活性化型GM−CSFは、細胞外でホモ二量体として存在する。特に、GM−CSFは、炎症性サイトカイン、ケモカイン、及びプロテアーゼを増産させ、それにより最終的に関節破壊をもたらす、リウマチ(RA)のような自己免疫疾患の炎症性メディエータとして同定される。
・分子係数logPが、−0.4から+5.6の範囲である。
・モル屈折率が、40から130である。
・原子数が20から70である。
GM−CSF−/−マウスの生成は、Stanleyら、(1994)、「Proc.Natl.Acad.Sci.USA 91」5592頁に記載されている。簡単にいうと、分裂したGM−CSF遺伝子を有する129/OLA由来のES細胞(H−2b)をC57BL/6(H−2b)宿主胚盤胞にマイクロインジェクションして、キメラマウスを生成する。変異GM−CSF対立遺伝子の生殖細胞系伝達物質は、11世代にわたりC57BL/6マウスと交配され、実験用のGM−CSF−/−、GM−CSF+/−、及びGM−CSF+/+を生み出すために異種交配されたGM−CSF+/−マウスを与える。GM−CSF遺伝子型の状態は、テールDNAをPCR分析することにより決定する。動物は、標準的なげっ歯動物用の飼料と水が適宜与えられ、おがくずが並べられたケージの中で同性の同腹子と収容された。オス、メス共に生後8−15週のマウスが実験の対象とされた。
疼痛モデルは、外科的手術後に生じる疼痛のGM−CSFアンタゴニストを用いた治療の有効性を評価するために用いられる。
体重220−240グラムのオスのSprague Dawleyラットを外科的手術前1週間、動物施設に順応させる。
外科的手術は、Brennanら、「Pain64」、493−501頁、1996年、に記載された手順に従った。動物を、ノーズコーンを通じて外科的手術の間維持した空気混合中の2%のイソフルレンで麻酔した。右後ろ足の足底表面をポビドンヨードパッドを用いて調整し、皮膚と筋膜を貫通するように1cm中心長さの切開をし、踵の端0.5cmからつま先に向かって広げた。測定は屈曲位に保った足に定規を用いて行った。足底の筋肉を湾曲した鉗子を用いて持ち上げて縦方向に切開した。基点から差し込み部分の間、その深さを貫通するように筋肉を切開した。外科的手術の間、ガーゼパッドを通して圧迫することにより出血をコントロールした。創傷は、2本の縫合糸(5−0エチロンブラックモノフィラメント糸)で閉じた。これらの縫合糸は5−6回結び、最初の結び目は緩く結んだ。創傷部位をバシトラシン溶液で消毒した。行動実験を開始する前に2時間又はそれ以上動物を清潔なケージ内で休ませ、回復させた。
累積的な疼痛スコアを、体重負荷に関連する疼痛を評価するために使用した。土台を高くした(h:18”)清潔なプラスチックのケージのプラスチックメッシュ(格子:8mm2)上に動物を置き、足の底裏面を検診した。20分の順応時間の後、体重負荷を0から2のスケールで評価した。0スコアは、足が白くなったか、メッシュに押し付けられたときに与え、これは、完全な体重負荷を示している。1スコアは、メッシュに触れているだけの皮膚で足がかばわれており、足が白くなってないか皮膚の圧痕がないときに与えた。2スコアは、足がメッシュから完全に離れて保たれている場合に与えた。ラットがまだ休んでいる場合には躊躇無く足を2とみなした。各動物を数分間にわたり5分おきに1分間観察した。1/2時間に得られた6つのスコアの和(0−12)を使用して、切開した足の疼痛を評価した。2スコアの頻度も計算し、動物による激痛の発生率又は足の全体的な防御を評価するのに使用した。各動物を外科的手術の24時間前(ベースライン)、並びに手術2時間、24時間、48時間、及び72時間後に検査した。GM−CSFアンタゴニストで治療された動物は、コントロールの動物と比較して累積の安静時の疼痛が著しく減少したことをこの実験の結果は示している。体重負荷は、動物がどれくらい足を使用する意思があるかにかなり相関するので、疼痛緩和の効果的な測定である。GM−CSFアンタゴニストは、GM−CSF受容体に特異的であるGM−CSFに特異的な抗体であるのが好ましい。これらの抗体を、切開15時間前に様々の濃度の抗体を(例えば、動物の体重キログラムあたり0.004、0.01、0.02、0.1、0.6、及び1mg)腹腔注入した。陰性対照グループには、抗体を投与せず生理食塩水を腹腔に注入した。外科的手術の24時間後の検査開始30分前に0.01mg/kgのフェンタニルを、陽性対照として腹腔に注入した。各実験は、各条件について8匹の動物(グループあたりn=8)を含め、コントロールグループは56匹とした。外科的手術を行い、累積疼痛スコアの測定を上記の通り行った。安静時の疼痛を外科的手術の24時間後に評価した。
温熱性知覚過敏を、下記のHargreavesらにより変更された方法(1988)である、ラットの足底表面検査(Ugo Basile、イタリア)により評価する。ラットは、ガラス製のテーブルの上にある4つの個々のプレキシガラスの箱から成る装置で順応させられる。モバイル放射熱源は、テーブルの下に配置されており、後ろ足に焦点を当てる。動物が寝てないが動いていないときに、コントロール箱上のボタンを押すと、放射熱源が作動し、動物が熱源から足を引っ込めるに要する時間を自動的に記録する。この足撤退待ち時間(POOL)は、放射源の反射率の変化によりラットの足の動きを感知する放射熱源に内臓された光検出器によって検出される。足撤退待ち時間(PWL)は、数秒で記録される。組織の損傷を防止するため、22.5秒の自動カットオフポイントを設けている。各動物の両後ろ足のPWLを3−4回計り、その平均を右及び左後ろ足のベースラインとする。結果は右足(外科的手術の部位)と左足において測定されたスコアの比として表示する。約6秒の正常のPWLを与えるために、装置は1度(調査開始時点)調整され、40の強度に設定される。各動物を外科的手術の24時間前(ベースライン)、並びに手術の3時間、24時間、48時間、及び72時間後に検査した。温熱性知覚過敏の測定は、接触性アロディニアの測定後に行われる。
GM−CSFに特異的な抗体、又はGM−CSF受容体に特異的な抗体などのGM−SCFアンタゴニストは、骨転移に関連する骨肉腫疼痛の治療に有効である。
動物の腫瘍移植10日及び14日後の疼痛関連行動を検査する。以下の検査を用いて動物の行動検査を行う。継続的な疼痛(自発的防御及び収縮)、歩行時の疼痛(足の使用や回転)、及び運動により誘発された疼痛(触診により誘発された防御及び触診により誘発された収縮)。動物は、ワイヤメッシュの床を有する透明なプラスチックの観察箱に配置され、30分間順応させられる。順応後、オープンフィールドでの通常の歩行時に自発的に防御、自発的な収縮、足の使用、及び歩行を強制された時の防御を評価する。触診により誘発された防御及び収縮は、肉腫及びシャム注入動物の大腿遠位の正常な非有害触診の2分後に測定される。
6日目と13日目に、肉腫注入動物に、抗GM−CSF又は抗GM−CSF受容体抗体(n=5)のようなGM−CSFアンタゴニストを腹腔内(i.p.)に注入し、又は肉腫及びシャム注入動物に、腹腔内(i.p.)に生理的食塩水を注入した(n=5、各条件で)。10日目と14日目に全ての動物の行動面における分析を行う。
肉腫注入動物(生理的食塩水と共に投与)は、シャム注入動物(生理的食塩水と共に投与)と比較して自発的防御及び自発的により評価すると統計的に著しい持続性疼痛行動を示す。
肉腫注入動物(生理的食塩水と共に投与)は、シャム注入動物(生理的食塩水と共に投与)と比較して、足の使用及び歩行を強制されたときの防御(ロータロッド)により評価すると、歩行時に疼痛行動を示す。肉腫移植の10日と14日後に肉腫注入マウスにGM−CSFアンタゴニストを投与すると、生理的食塩水を投与された肉腫注入マウスと比較して、足の使用スコアと歩行を強制されたときの防御のスコアが著しく増加する。これらの結果は、GM−CSFアンタゴニストが肉腫注入マウスの歩行時の疼痛を減少させることを示している。
肉腫注入動物(生理的食塩水と共に投与)は、シャム注入動物(生理的食塩水と共に投与)と比較して、触診により誘発される防御及び触診により誘発される収縮により評価すると、接触により誘発された疼痛行動を示す。肉腫移植の10日と14日後に肉腫注入マウスにGM−CSFアンタゴニストを投与すると、生理的食塩水を投与された肉腫注入マウスと比較して、触診により誘発される防御及び触診により誘発される収縮が著しく減少する。これらの結果は、GM−CSFアンタゴニストが肉腫注入マウスの接触により誘発される疼痛を減少させることを示している。
完全フロインドアジュバント(CFA)により誘発されるラットの慢性関節炎のGM−CSFアンタゴニストの鎮痛効果を基準物質として使用したインドメタシンと比較して、発声検査を用いて研究する。
この実験では、GM−CSFアンタゴニストが変形性関節症の治療に効果があることを明らかにすべく、GM−CSFに特異的なモノクローナル抗体を使用した。
C57BL/6マウスに関節不安定性を誘発させるため、0日目と2日目に右膝にコラゲナーゼVII型を1ユニット関節内投与した(Blomら、Osteoarthritis Cartilage.12、627−35頁、2004年を参照されたい)。
20匹のマウスを不規則に2つのグループに分けた(10匹/グループ)。

最終注入の6週後に組織構造はマウスの膝関節に形成された。膝関節を回収、固定、脱石灰化し、パラフィン内に包埋し、ミクロトームで7μmにカットした。スライドをサフラン−O/ファストグリーン、ヘマトキシン、及びエオシンで染色し、関節病変を明らかにした。研究された病変は、軟骨損傷、滑膜炎、骨棘変形、及び関節変形を伴っていた。

変形性関節症モデルの疼痛指標は、インキャパシタンスメータ(Incapacitance Meter)を用いて測定される体重分配の差異である。この器具は、手術された後ろ足と、反対側のすなわち手術されていない後ろ足の間の体重分配の変化を計測する。マウスは、実験前に3回、装置に順応させられる。各後ろ足が体重をかけた場所を5秒にわたって測定した。マウス1匹につき各時点で3回別個に測定し、次いで平均をとる。測定は、実験の間、1週間に2回行われた。結果は、コラゲナーゼを注入した足/コントロールの足×100として表示する。
組織構造において分析されたすべての領域(中央大腿骨を除く)、すなわち外側大腿骨、外側脛骨、及び中央脛骨では、抗GM−CSF抗体で治療されたマウスの疾患が少ない明確な傾向がある。結果は図1に示されている。
以下の実験は、GM−CSFアンタゴニストが炎症性疼痛の治療にも有効であることを示す。GM−CSFノックアウトマウス及びコントロールのマウスにmBSA/IL−1単関節関節炎が誘発された。疼痛は、インドメタシン、疼痛緩和物質の投与にかかわらず、インキャパシタンス検査装置を用いて様々な時点で評価された。
24匹のオスのC57BL/6マウス及び24匹のオスのGM−CSF−/−マウス(実施例1を参照されたい)が4つの治療グループで用いられる。

単関節痛関節炎は、生理食塩水中のmBSA(20mg/ml)を10μl膝関節に、生理食塩水10μlを対側の膝関節に関節内注射することにより誘発される。IL−1β20μl(250ng)を、3日間毎日皮下投与された。一般に、mBSA注入の4日及び7日後に反応が現れ、28日後に消散する。2、3、4、5、及び7日目にインキャパシタンスの検査をした。
インキャパシタンステスター(Dual Weight Averager)は、2本の後ろ足における体重分配を測定することで、鎮痛効果を自動的に再現性よく評価するのに使用した。使用者が選択した期間にわたり、各足によってもたらされる力(グラム単位で測定)の平均をとり、動物の片方の足からもう片方の足に体重移動する傾向を提示し、インキャパシタンスの定量的測定を提供する。
このモデルは、mBSAを注入されることにより、膝関節に滑膜炎が誘発される。7日目に膝関節は、視覚的に検査され、0(正常)から3(重度の炎症)のスコアが与えられる(図3を参照)。mBSAが注入された左の膝は、右の膝(生理食塩水が注入された)と比較して、著しい炎症を起こした(p>0.001 C57マウス及びp=0.02 GM−CSF−/−マウス)(図3参照)。実際、生理食塩水が注入された任意の右膝は、スコア0だった。インドメタシンで治療したマウスとそれらで治療しない間には、損傷に顕著な違いはみられなかった。

以下の実験は、炎症性疼痛の治療において、GM−CSFアンタゴニストの有効性を明らかにする追加の実験である。ここで炎症性疼痛は、完全フロインドアジュバントで誘発されるものである。実施例5のように、疼痛をインドメタシン、疼痛緩和物質の投与にかかわらず、インキャパシタンス検査装置を用いて様々な時点で評価した。
12匹のオスのC57BL/6マウス及び12匹のオスのGM−CSF−/−マウス(実施例1を参照されたい)がそれぞれ3つの治療グループで使用された。
グループ1:C57BL/6野生型(n=12):CFA

完全フロインドアジュバント(CFA)は、1mg/mlの濃度の鉱油中に、熱滅菌したヒト型結核菌株、H37Raを含む。熱滅菌したバクテリアが完全に懸濁液に組み込まれるように、ボルテックスしてCFAを完全に混合した(Kamala T Hock immunization:a humane alternative to mouse footpad injections.J Immunol Methods 328:204−214、2007)。ボルテックス後すぐに、アジュバントは、19ゲージ針を用いるガラスシリンジに吸い込まれた。気泡をシリンジから丁寧に取り除いてから、針を除去した。各マウスは、CFAエマルション20μlを左後ろ足(足蹠)に皮下注射された。疼痛評価1時間前にグループ2のマウスの腹腔内に、1mg/kgのインドメタシン(実施例5参照されたい)を投与した。
実施例5にあるように、インキャパシタンステスター(Dual Weight Averager)を、2本の後ろ足の体重分配を計測することで自動的で再現性のある鎮痛効果の評価をするために使用した。各後ろ足が体重をかけた場所を5秒にわたって測定した。マウス1匹につき各時点で3回別個に測定し、次いで平均をとった。結果は、注入した足/コントロールの足×100として表示する。100の値は、右足と左足にかけられた体重が等しいことを意味する。100以下の値は、コントロールの足(右)と比較すると、注入された足(左)に体重がかけられていないことを意味する。CFA注入の24、48、72時間後にインキャパシタンスによる検査をした。
左の足蹠にCFAを皮下注射した後、C57BL/6(グループ1)とGM−CSF−/−(グループ3)マウスと同様な大きさの腫れが、左の足蹠に示された。インドメタシンで治療されたC57BL/6マウス(グループ2)も、腫れの程度に違いはなかった(図5を参照されたい)。いずれのグループも対側(右)の足に腫れは見られなかった。

実施例2−7を繰り返す。GM−CSFアンタゴニストとして、配列番号3に示される重鎖可変領域のアミノ酸配列を含む又は配列番号4に示される軽鎖可変領域のアミノ酸配列を含むGM−CSF特異抗体が用いられる。マウスとは別の種が用いられてもよく、特にこの実験で用いられる抗体は交差反応性である。この実験で特に好ましい動物種はラットである。
実施例2−7を繰り返す。GM−CSFアンタゴニストとして、配列番号5−16のいずれかから選択されるH−CDR3配列を含むGM−CSF特異抗体が使用される。前記抗体はさらに配列番号16のH−CDR1配列、及び/又は配列番号18の配列H−CDR2、及び/又は配列番号19のL−CDR1配列、及び/又は配列番号20のL−CDR2配列、及び/又は配列番号21のL−CDR3配列を含むのが好ましい。マウスとは別の種が用いられてもよく、特にこの実験で用いられる抗体は、交差反応性である。この実験で特に好ましい動物種は、ラットである。
実施例2−7を繰り返す。GM−CSFアンタゴニストとして、配列番号22のL−CDR1配列、及び/又は配列番号23のL−CDR2配列、及び/又は配列番号24のL−CDR3配列、及び/又は配列番号25のH−CDR1配列、及び/又は配列番号26のH−CDR2配列、及び/又は配列番号27のH−CDR3配列を含むGM−CSF特異抗体である。抗体は、配列番号22−28のCRDsを全て含むのが好ましい。マウスとは別の種が用いられてもよく、特にこの実験で用いられる抗体は交差反応性である。この実験で特に好ましい動物種はラットである。
