Abstract
Description
本発明の一態様は、体内の2つの腔を隔てる体壁を一方の前記腔側から切開するときに使用される医療用デバイスであって、他方の前記腔内に挿入可能な細長い長尺部と、該長尺部の先端に設けられ、前記体壁の表面に対向して配置され該体壁の表面を覆うシート部と、該シート部の周囲に全周にわたって設けられ、前記体壁の表面に密着する取付部と、前記シート部に設けられ、前記体壁が前記一方の腔側から前記他方の腔側まで切開されたことを、前記体壁の切開部位から前記シート部が前記一方の腔側に露出することに基づいて術者に報知する報知部とを備える医療用デバイスである。
このようにすることで、シート部を閉形態に配置することによって医療用デバイスの径寸法が小さくなるので、内視鏡のチャネルのような細い空間を介して腔内に医療用デバイスを挿入および抜去することができる。
このようにすることで、切開部位を介して一方の腔側に露出する着色層の色によって、体壁の貫通を術者に報知することができる。
このようにすることで、体壁の表面とシート部との間に距離を確保しながら患部を他方の腔内において隔離することができる。
このようにすることで、生体接着剤の粘着力によって体壁の表面にシート部を固定することができる。
このようにすることで、吸引装置によって吸引管内を介して吸引口に吸引力を発生させることによって取付部を体壁を吸着させることができる。また、吸引装置による吸引を停止することによって、取付部を体壁から容易に取り外すことができる。
このようにすることで、反射体によって一方の腔側へ反射される光によって、体壁の貫通を術者に報知することができる。
このようにすることで、発光体から一方の腔側へ発せられる光によって、体壁の貫通を報知することができる。
このようにすることで、体壁を貫通した処置具がシート部に接触したときに、その接触が接触検知手段によって検知され、認識手段によって聴覚的にまたは視覚的に術者に報知される。これにより、処置具によるシート部の破損をさらに確実に防止することができる。
このようにすることで、絶縁層は電気メスのような電気処置具によって切開されないので、電気処置具によるシート部の破損を絶縁層によってさらに確実に防止することができる。
このようにすることで、体壁を貫通した電気メスのような電気処置具が導電層に接触したときに電気処置具から出力される電流がアース線を介してアースへ流れるので、導電層は電気処置具によって切開されない。これにより、電気処置具によるシート部の破損を絶縁層によってさらに確実に防止することができる。
本実施形態に係る医療用デバイス1は、図1Aおよび図1Bに示されるように、細長い長尺部2と、該長尺部2の先端に設けられ長尺部2の径方向に拡張および収縮可能なシート部3と、該シート部3を変形させる開閉機構4と、シート部3に設けられた報知部5および取付部6とを備えている。
長尺部2は、管腔内視鏡のチャネル内に挿入可能な直径を有している。また、長尺部2は、体内での管腔内視鏡の湾曲に従って湾曲可能な可撓性を有している。
複数本の受骨8は、長尺部2の周方向に間隔をあけて長尺部2を中心とする放射状に配列されている。各受骨8の先端は、シート部3の基端面の周縁に固定されている。
本実施形態に係る医療用デバイス1は、図3に示されるように、管腔A内に挿入された管腔内視鏡30と体腔B内に挿入された体腔内視鏡40とを使用して、管腔Aと体腔Bとを隔てる管腔Aの壁Cに形成された腫瘍のような患部Dを切除する手術に使用される。管腔Aは、例えば胃または腸であり、体腔Bは、例えば腹腔である。
なお、図5Aから図9において、図面を簡略にするために開閉機構4の図示を省略している。
さらに、絶縁層32も電気メス50によって切開されないので、シート部3の破損をさらに確実に防止することができる。
例えば、報知部が、図9に示されるように、導電層(接触検知手段)31と、長尺部2内を通る電線19を介して導電層31と接続された高周波検知部(接触検知手段)20と、該高周波検知部20に接続されたスピーカ(認識手段)21とを備えていてもよい。

Claims (11)
- 体内の2つの腔を隔てる体壁を一方の前記腔側から切開するときに使用される医療用デバイスであって、
他方の前記腔内に挿入可能な細長い長尺部と、
該長尺部の先端に設けられ、前記体壁の表面に対向して配置され該体壁の表面を覆うシート部と、
該シート部の周囲に全周にわたって設けられ、前記体壁の表面に密着する取付部と、
前記シート部に設けられ、前記体壁が前記一方の腔側から前記他方の腔側まで切開されたことを、前記体壁の切開部位から前記シート部が前記一方の腔側に露出することに基づいて術者に報知する報知部とを備える医療用デバイス。 - 前記シート部が、前記長尺部の長手方向に交差する方向に拡張した開形態と、前記長尺部の長手方向に沿って配置されるように折り畳まれた閉形態との間で変形可能であり、
前記シート部を前記開形態と前記閉形態との間で変形させる開閉機構を備える請求項1に記載の医療用デバイス。 - 前記報知部が、前記シート部の前記体壁の表面と対向して配置される面に設けられ、前記体壁の補色または該補色に近い色を有する着色層を備える請求項1に記載の医療用デバイス。
- 前記取付部が、前記シート部の前記体壁の表面と対向して配置される面よりも突出している請求項1に記載の医療用デバイス。
- 前記取付部が、前記シート部の周囲に全周にわたって塗布された生体接着剤からなる請求項4に記載の医療用デバイス。
- 前記取付部が、周方向に沿って設けられた吸引口を備え、
前記長尺部の基端側に配置され吸引力を発生する吸引装置と、
前記長尺部を介して前記吸引口と前記吸引装置とを接続する吸引管とを備える請求項4に記載の医療用デバイス。 - 前記報知部が、前記シート部の前記体壁の表面と対向して配置される面に設けられ、光を反射する反射体を備える請求項1に記載の医療用デバイス。
- 前記報知部が、前記シート部の前記体壁の表面と対向して配置される面に設けられ、光を発する発光体を備える請求項1に記載の医療用デバイス。
- 前記報知部が、
前記シート部に設けられ、前記体壁を切開する処置具との接触を検知する接触検知手段と、
