本明細書で使用する「活性剤」とは、身体において局所的、および/または全身的に作用する、生理的または薬理学的な活性物質を指す。活性剤は、疾患または障害の処置(例えば、治療剤)、予防(例えば、予防剤)、または診断(例えば、診断剤)のために、患者に投与される物質である。本明細書で使用する「点眼薬」または「点眼活性剤」とは、眼の疾患または障害の1つもしくは複数の症状を緩和する、その発症を遅延させる、もしくは上記の症状を予防するために患者に投与される治療剤または予防剤、あるいは眼の画像化または他には眼評価に有用な診断剤を指す。
非線状マルチブロックコポリマー薬物コンジュゲートを使用して、眼への送達にとって有用なナノ粒子、マイクロ粒子、およびインプラント(例えば、ロッド、ディスク、ウエハーなど)を形成することができる。ポリマー薬物コンジュゲートは、多価分岐点を介して共有結合的に接続している1つまたは複数の疎水性ポリマーセグメントおよび1つまたは複数の親水性ポリマーセグメントを含有して、少なくとも3つのポリマーセグメントを含有している非線状マルチブロックコポリマーを形成する。ポリマー薬物コンジュゲートは、1または複数の疎水性ポリマーセグメントに共有結合的に付着している、1種または複数の治療剤、予防剤、または診断剤をさらに含有している。ポリマー薬物コンジュゲートの使用により、より一層制御された薬物搭載量、および薬物放出プロファイルを有する粒子を形成することができる。さらに、コンジュゲートの溶解度は、可溶な薬物濃度、したがって毒性を最小限にするよう制御することができる。

本明細書に記載されている非線状マルチブロックコポリマー薬物コンジュゲートは、1つまたは複数の親水性ポリマーセグメントも含有することができる。好ましくは、ポリマー薬物コンジュゲートは、2つ以上の親水性ポリマーセグメントを含有している。一部の実施形態では、ポリマー薬物コンジュゲートは、2〜6つの間、より好ましくは3〜5つの間の親水性ポリマーセグメントを含有している。ある種の実施形態では、ポリマー薬物コンジュゲートは、3つの親水性ポリマーセグメントを含有している。

非線状マルチブロックコポリマー薬物コンジュゲートは、当該分野で公知の合成法を使用して調製することができる。ポリマー薬物コンジュゲートの調製に関する代表的な方法論は、以下で議論されている。所与のポリマー薬物コンジュゲートの合成に関する適切な経路は、官能基の適合性、保護基戦略、および不安定な結合の存在と関係するので、その経路は、全体として、ポリマー薬物コンジュゲートの構造、コンジュゲートを構築しているポリマーのアイデンティティ、活性剤のアイデンティティ、および化合物の構造などの、いくつかの要因を鑑みて決定することができる。
非線状マルチブロックコポリマー薬物コンジュゲートは、当該分野で公知の様々な技法を使用して、マイクロ粒子、ナノ粒子、およびインプラントを形成することができる。粒子またはインプラント形成のための適切な方法は、粒子/インプラントを形成するために使用される1種または複数のポリマー薬物コンジュゲートの物理的および化学的性質(すなわち、安定性および溶解度)、ならびに所望の粒子/インプラントのサイズおよび形態を考慮して、選択することができる。
マイクロ粒子およびナノ粒子は、1種または複数のポリマー薬物コンジュゲートから形成することができる。一部の場合、粒子は、単一ポリマー薬物コンジュゲートから形成される(すなわち、粒子は、同じ活性剤、疎水性ポリマーセグメント(複数可)、分岐点(存在する場合)、および親水性ポリマーセグメント(複数可)を含有しているポリマー薬物コンジュゲートから形成される)。
ポリマー薬物コンジュゲートから形成されるマイクロ粒子およびナノ粒子は、当該分野で公知のポリマーマイクロ粒子またはナノ粒子の形成に適した任意の方法を使用しても調製することができる。粒子形成のために使用される方法は、ポリマー薬物コンジュゲート中に存在しているポリマーの特性、および所望の粒子サイズ、およびサイズ分布を含めた、様々な要因に依存することになる。一部の薬剤が、ある種の溶媒の存在下で、ある種の温度範囲、および/またはある種のpH範囲において不安定であるので、粒子−薬物コンジュゲート中に存在している活性剤のタイプもまた、要因の1つとなり得る。
この方法では、ポリマー薬物コンジュゲートは、塩化メチレンなどの揮発性有機溶媒に溶解する。次に、ポリ(ビニルアルコール)などの界面活性剤を含有している水溶液中に、ポリマー薬物コンジュゲートを含有している有機溶液を懸濁させる。有機溶媒のほとんどが蒸発して、固体ナノ粒子が残るまで、この得られたエマルションを撹拌する。得られたナノ粒子を水により洗浄し、凍結乾燥装置中で一晩乾燥させる。様々なサイズおよび形態を有するナノ粒子は、この方法により得ることができる。
この方法では、ポリマー薬物コンジュゲートは最初に溶融され、次に、(シリコーンオイルのような)非混和性溶媒中に懸濁させて、連続撹拌しながらポリマー薬物コンジュゲートの融点より5℃高く加熱する。一旦エマルションが安定すると、ポリマー薬物コンジュゲート粒子が凝固するまで冷却する。得られたナノ粒子を石油エーテルなどの適切な溶媒を用いるデカンテーションにより洗浄すると、自由流動性粉末が得られる。この技法により調製された粒子の外部表面は、通常、滑らかで密である。ホットメルト粒子形成は、ある種のポリ無水物などの加水分解的に不安定なポリマー薬物コンジュゲートを含有している粒子を調製するために使用することができる。好ましくは、この方法によりマイクロ粒子を調製するために使用されるポリマー薬物コンジュゲートは、全分子量が75,000ダルトン未満を有することになる。
加水分解的に不安定なポリマー薬物コンジュゲートから粒子を調製するために、溶媒除去も使用することができる。この方法では、ポリマー薬物コンジュゲートは、塩化メチレンなどの揮発性有機溶媒に分散、または溶解する。次に、この混合物を有機オイル(シリコーンオイルなど)中で撹拌することにより懸濁させ、エマルションを形成する。固形粒子がエマルションから形成し、これを続いて上清から単離することができる。この技法により生じた球状の外的形態は、ポリマー薬物コンジュゲートのアイデンティティに大きく依存する。
この方法では、ポリマー薬物コンジュゲートは、塩化メチレンなどの有機溶媒に溶解する。圧縮ガスの流通により駆動されるマイクロ化ノズルによって、溶液をポンプ押出しして、得られたエアゾールを加熱した空気サイクロン中に懸濁して、マイクロ液滴から溶媒を蒸発させて粒子を形成させる。この方法を使用して、0.1〜10ミクロンの間の粒子を得ることができる。
転相法を使用して、ポリマー薬物コンジュゲートから粒子を形成することができる。この方法では、ポリマー薬物コンジュゲートを「良好な」溶媒に溶解し、溶液をポリマー薬物コンジュゲートにとって強い非溶媒中に注ぎ入れると、都合のよい条件下で、自発的にマイクロ粒子またはナノ粒子が生成する。この方法を使用して、例えば約100ナノメートル〜約10ミクロンを含む広範囲のサイズで、通常、狭い粒子サイズ分布を有するナノ粒子を生成することができる。
コアセルベーションを使用する粒子形成技術は、当該分野、例えばGB−B−929 406、GB−B−929 40 1、および米国特許第3,266,987号、同第4,794,000号、および同第4,460,563号において公知である。コアセルベーションは、2種の非混和液相中へのポリマー薬物コンジュゲート溶液の分離を必要とする。1つの相は密なコアセルベート相であり、この相は高濃度のポリマー薬物コンジュゲートを含有している一方、第2相は低濃度のポリマー薬物コンジュゲートを含有している。密なコアセルベート相内で、ポリマー薬物コンジュゲートが、ナノスケールまたはマイクロスケールの液滴を形成し、この液滴が粒子に固化する。コアセルベーションは、温度変化、非溶媒の添加またはマイクロ塩(単純コアセルベーション)の添加によって、または他のポリマーの添加によって誘発され、これによりインターポリマー複合体(複合コアセルベーション)を形成することができる。
放出制御マイクロスフィアの超低温キャスティングの方法が、Gombotzらの米国特許第5,019,400号に記載されている。この方法では、ポリマー薬物コンジュゲートは、溶媒に溶解する。次に、混合物を、液状非溶媒を含有している容器に、ポリマー薬物コンジュゲート溶液の凝固点よりも低い温度で噴霧すると、ポリマー薬物コンジュゲート液滴が凍結する。液滴、およびポリマー薬物コンジュゲート用の非溶媒が温められると、液滴中の溶媒が解凍し、非溶媒中に抽出されてマイクロスフィアが固化する。
インプラントは、1種または複数の非線状マルチブロックコポリマー薬物コンジュゲートから形成することができる。好ましい実施形態では、インプラントは眼内インプラントである。
インプラントは、ファイバー、シート、フィルム、マイクロスフィア、球状、ロッド、円形ディスク、またはプラークなどの任意の幾何学的形状とすることができる。インプラントのサイズは、インプラントの耐久性、インプラントの場所、インプラント挿入が提案される方法を考慮したサイズ制限、取り扱いの容易さなどの要因により決まる。
インプラントは、当該分野で公知の適切な任意の技法を使用して製造することができる。インプラントの調製に適した技法の例には、溶媒蒸発法、相分離法、界面法、成形法、射出成形法、押出成形法、共押出成形法、カーバープレス(carver press)法、型抜き法、熱圧縮、およびそれらの組合せが含まれる。インプラントの製造に適した方法は、インプラント中に存在しているポリマー/ポリマーセグメントの特性、インプラント中に存在している1種または複数の活性剤の特性、およびインプラントの所望の形およびサイズを含む、多くの要因を考慮して選択することができる。インプラントの調製に適した方法は、例えば、米国特許第4,997,652号および米国特許出願公開2010/0124565号において記載されている。
1種または複数の薬学的に許容される賦形剤と組み合わせた、1種または複数のポリマー薬物コンジュゲートから形成される粒子を含有している医薬製剤が提供される。代表的な賦形剤には、溶媒、希釈剤、pH調節剤、保存剤、抗酸化剤、懸濁化剤、湿潤剤、粘度調節剤、等張化剤、安定化剤、およびそれらの組合せが含まれる。適切な薬学的に許容される賦形剤は、一般に安全と認められる(generally recognized as safe)(GRAS)物質から好ましくは選択され、望ましくない生物学的副作用または不要な相互作用を引き起こすことなく、個体に投与することができる。
医薬組成物は、ポリマー薬物コンジュゲート中には存在していない、1種または複数のさらなる活性剤を含有することができる。一部の場合、1種または複数のさらなる活性剤は、1種または複数のポリマー薬物コンジュゲートから形成される粒子中に封入する、分散する、他には会合させることができる。ある種の実施形態では、1種または複数のさらなる活性剤は、薬学的に許容される担体に溶解するか、または懸濁することもできる。
ポリマー薬物コンジュゲートから形成された粒子は、好ましくは、眼に注射するための溶液または懸濁液として製剤化される。眼投与用医薬製剤は、好ましくは、1種または複数のポリマー薬物コンジュゲートから形成される粒子の無菌水溶液または懸濁液の形態にある。許容可能な溶媒には、例えば、水、リンガー溶液、リン酸緩衝生理食塩水(PBS)、および等張食塩溶液が含まれる。製剤はまた、1,3−ブタンジオールなどの無毒で、非経口的に許容可能な希釈剤または溶媒中の無菌溶液、懸濁液、またはエマルションとすることもできる。
ある種の実施形態では、1種または複数のポリマー薬物コンジュゲートから形成される粒子を含有している医薬組成物を使用して、1つもしくは複数の眼の疾患を処置または予防する。一部の実施形態では、1種または複数のポリマー薬物コンジュゲートから形成されるインプラントを使用して、1つもしくは複数の眼の疾患を処置または予防する。
本明細書で提供される1種または複数のポリマー薬物コンジュゲートから形成される粒子を含有している医薬組成物は、眼の1つもしくは複数の疾患または障害を処置あるいは予防するために、それを必要としている患者の眼に投与される。1種または複数のポリマー薬物コンジュゲートから形成されるインプラントはまた、眼の1つもしくは複数の疾患または障害を処置あるいは予防するために、それを必要としている患者の眼に投与することもできる。
1.投与形態
ポリマー薬物コンジュゲートは、硝子体内注射(例えば、硝子体の前面、中央部、または背面注射)、結膜下注射、前房内注射、側頭縁部を介する前眼房への注射、基質内注射、角膜内注射、網膜下注射、および眼内注射により、眼に局所投与することができる。好ましい実施形態では、医薬組成物は、硝子体内注射によって投与される。
好ましくは、ポリマー薬物コンジュゲートから形成される粒子およびインプラントは、長期間にわたり、有効量の1種または複数の治療剤を放出することになる。好ましい実施形態では、粒子およびインプラントは、少なくとも2週間の期間にわたり、より好ましくは少なくとも4週間の期間にわたり、より好ましくは少なくとも6週間の期間にわたり、最も好ましくは少なくとも8週間の期間にわたり、有効量の1種または複数の活性剤を放出する。一部の実施形態では、粒子およびインプラントは、3ヶ月以上の期間にわたり、有効量の1種または複数の活性剤を放出する。
加齢黄斑変性症の場合、患者における治療効果は、以下の1つまたは複数のものによって測定することができる。すなわち、ベースラインから所望の時間までの最高矯正視力(BCVA)の平均変化の評価、ベースラインと比較した、所望の時間で15文字(3行)未満の視力を失う患者の割合の評価、ベースラインと比較した、所望の時間で15文字(3行)またはそれ以上の視力を得る患者の割合の評価、所望の時間で20/2000またはそれより悪いスネレン等価視力を有する患者の割合の評価、米国国立眼病研究所の視覚機能の質問表の評価、ならびに蛍光眼底血管造影法を使用する所望時間におけるCNVサイズおよびCNV漏出量の評価である。
ポリマーの合成
(ポリエチレングリコール)3−co−ポリ(セバシン酸)(PEG3−PSA)を、溶融重縮合により調製した。手短に言えば、セバシン酸を無水酢酸中で還流し、セバシン酸プレポリマー(Acyl−SA)を形成させた。クエン酸−ポリエチレングリコール(PEG3)を、当該分野で公知の方法(Ben−Shabat, S.ら、Macromol. Biosci.6巻、1019〜1025頁(2006年))を使用して調製した。CH3O−PEG−NH22.0g、クエン酸26mg、ジシクロヘキシルカルボジイミド(DCC)83mg、および4−(ジメチルアミノ)ピリジン(DMAP)4.0mgを塩化メチレン10mLに加えた。この混合物を室温で一晩撹拌し、次に沈殿させ、エーテルにより洗浄して真空下で乾燥し、PEG3を単離した。次に、acyl−SA(90%w/v)およびPEG3(10%w/v)を180℃で30分間重合した。窒素ガスを15分毎に30秒間フラスコに掃引した。ポリマーを周囲温度まで冷却し、クロロホルムに溶解し、過剰の石油エーテル中に沈殿させた。沈殿物をろ過により集め、一定重量になるまで真空下で乾燥した。
DXR−PSA−PEG3ナノ粒子は、ジクロロメタン3mLおよびDMSO1mL中に、PEG3−PSAとDXRとを規定した比で溶解し、50℃で2時間反応させた後、1%ポリビニルアルコール(25kDa、Sigma社)を含有している水溶液100mL中にホモジナイズ(L4RT、Silverson Machines社、East Longmeadow、MA)することにより調製した。次に、撹拌しながら、室温でクロロホルムを2時間蒸発させることにより、粒子が固化した。粒子を遠心分離(4℃、20,000×gで、20分間)によって集め、2倍の蒸留水で3回洗浄した。粒子サイズは、ZetaSizer Nano ZS(Malvern Instruments社、Southborough、MA)を使用する動的光散乱法により決定した。サイズ測定は、25℃において、90°の散乱角で行った。
DXR−PSA−PEG3ナノ粒子を、2mg/mLで、リン酸緩衝生理食塩水(PBS、pH7.4)中に懸濁し、旋回台(140RPM)上、37℃でインキュベートした。選択した時間点において、上清を遠心分離(13,500×g、5分間)により集め、粒子を新しいPBS中で再懸濁した。DXR含有量は、480nmにおける吸光度により測定した。
上記で調製したDXR−PSA−PEG3ナノ粒子は、DXRを23.6%(重量基準)含有し、かつ平均粒子サイズ647nmを有した。インビトロ研究により、最大2週間、37℃においてPBS中に浸漬した条件下で、初期の急激な薬物放出段階がなく(すなわち、「バースト効果」がない)、DXRが、セバシン酸を含むコンジュゲートとして、ナノ粒子から定常的に放出されたことが示された。
材料および方法
病原体をもたないC57BL/6マウス(Charles River社、Wilmington、MA)を、Association for Research in Vision and Ophthalmology Statement for the Use of Animals in Ophthalmic and Vision Research、およびJohns Hopkins University Animal Care and Use Committeeの指針に準拠して処置した。
出生後日数(P)7およびP12において、75%酸素中に置かれたC57BL/6マウスを部屋の空気に戻し、PBS、またはダウノルビシン、ドキソルビシン、もしくはDXR−PSA−PEG3ナノ粒子を含有しているPBSの眼内注射を行った。P17では、網膜表面上の網膜NVの面積を測定した。手短に言えば、P17マウスに、ラット抗マウス血小板内皮細胞接着分子−1(PECAM−1)抗体(Pharmingen社、San Jose、CA)1μlを眼内注射し、12時間後、これらのマウスを安楽死させ、眼を室温で5時間、PBS緩衝ホルマリンに固定した。網膜を解剖して洗浄し、Alexa488によりコンジュゲートしたヤギ−抗ラットポリクローナル抗体(Invitroge社、Carlsbad、CA)を1:500希釈で用いて、室温で45分間インキュベートし、フラットマウントした。処置群に関してマスクされている観察者が、画像解析により網膜あたりのNVの面積を測定した。
光受容体においてVEGFを発現するヘミ接合ロドプシン/VEGFトランスジェニックマウスに、P14において、PBS、またはDXR−PSA−PEG3ナノ粒子10μgを含有しているPBS1μlを眼内注射した。P21、P28、P35、P42、またはP49に、マウスに麻酔をかけ、フルオレセイン標識デキストラン(平均分子量2×106、Sigma−Aldrich社)により灌流し、網膜フラットマウントを400倍の拡大倍率で蛍光顕微鏡検査法(Axioskop2 plus、Zeiss社、Thornwood、NY)により試験すると狭い被写界深度がもたらされ、その結果、網膜の外側縁部に沿った血管新生に焦点を合わせると、網膜血管の残りが焦点外になり、血管新生の描写および定量化が簡単になる。画像は、三色電荷結合素子ビデオカメラ(Cool SNAPTM−Pro、Media Cybernetics社、Silver Spring、MD)およびフレームグラバーによりデジタル化した。染色された血管新生を蛍光により認識するよう、画像解析ソフトウェア(Image−Pro Plus 5.0、Media Cybernetics社、Silver Spring、MD)を設定し、網膜あたりの血管新生の合計面積を計算した。画像解析を行う研究者は、処置群に関してマスクされている。
成体C57BL/6マウスに、PBS、あるいは0.1、1.0もしくは10μgのダウノルビシンまたはドキソルビシン、あるいは1.0もしくは10μgのDXR−PSA−PEG3ナノ粒子を含有しているPBS1μlを眼内注射した。注射後、Espion ERG Diagnosys機器を使用し、暗順応および明順応ERGを1日目、7日目、および14日目に記録した。暗順応記録に関しては、マウスに暗闇で一晩順応させ、明順応記録に関しては、マウスを強度30cd/m2でバックグラウンド白色光に10分間順応させた。マウスを、塩酸ケタミン(100mg/kg体重)およびキシラジン(5mg/kg体重)を腹腔内に注射して麻酔をかけた。0.5%トロピカミドおよび0.5%フェニレフリン塩酸塩(参天製薬株式会社、大阪、日本)を含有しているMidrin Pにより瞳孔を拡張した。マウスを39℃に加熱したパッド上に置き、隅角プリズム溶液(Alcon Labs社、Fort Worth、TX)の適用後、白金耳電極を各角膜に配置した。参照電極は、両眼間の前頭皮の皮下に配置し、接地電極を尾部に挿入した。マウスの頭を、両眼に等しい照射が確実となる、全体野ボウル型照射器の標準化位置に保持した。両眼の記録は、平衡電気的インピーダンスにより同時に行った。暗順応ERGは、−3.00〜1.40log cd−s/m2の範囲の白色光を強度レベル11で記録した。各フラッシュ強度について、6回の測定値を平均した。明順応ERGは、30cd/m2のバックグラウンドで、0.60〜1.40log cd−s/m2の範囲の白色光を強度レベル3で記録した。各フラッシュ強度について、5回の測定値を平均した。
ONL厚みを測定した。成体C57BL/6マウスに、PBS、あるいは0.1、1.0もしくは10μgのダウノルビシンまたはドキソルビシン、あるいは1.0もしくは10μgのDXR−PSA−PEG3ナノ粒子を含有しているPBS1μlを眼内注射した。マウスを安楽死させ、角膜輪部の12:00の位置に印を付け、眼を取り除き、最適な切断温度の化合物に埋め込んだ。視神経に沿って12:00または9:00の経線に対して平行に10マイクロメートルの凍結切片を切り出し、4%パラホルムアルデヒドに固定した。切片をヘマトキシリンおよびエオシンにより染色し、Axioskop microscope(Zeiss社、Thornwood、NY)により試験し、画像を、三電荷結合素子(CCD)カラービデオカメラ(IK−TU40A、東芝、東京、日本)およびフレームグラバーを用いてデジタル化した。Image−Pro Plusソフトウェア(Media Cybernetics社、Silver Spring、MD)を使用してONLの輪郭を描いた。処置群に関してマスクされている観察者により、ONLの厚みを6箇所の位置、すなわち上極と視神経の間の距離の25%(S1)、50%(S2)、および75%(S3)、ならびに下極と視神経の間の距離の25%(I1)、50%(I2)、および75%(I3)で測定した。
データは、平均値±SEMとして表した。統計解析は、スチューデントt検定を使用して行い、P<0.05を有意であると見なした。
アントラサイクリンは脈絡膜および網膜のNVを抑制する。
血管新生AMD患者における薬物効果(Saishin, Y.ら、J. Cell Physiol.、195巻、241〜248頁(2003年))を予測する脈絡膜NVのマウスモデル(Tobe,T.らAm.J.Pathol.153巻、1641〜1646頁(1998年))において、DNR10μgの眼内注射により、脈絡膜NVが抑制される一方、1または0.1μgの注射は、有意な効果を有さなかった(図1A)。同様に、DXR10μgの眼内注射により脈絡膜NVが抑制され、1または0.1μgの注射は、有意な効果を有さなかった(図1B)。
DNRおよびDXRは、代謝拮抗剤およびHIF−1阻害剤であるので、本発明者らは、ERGによって評価される網膜機能に対するそれらの効果を試験した。DNRまたはDXR1μgの眼内注射の14日後、暗順応および明順応のb波の平均振幅が有意に低下したが、0.1μgのDNRまたはDXRではそうではなかった。これらのデータにより、DNRおよびDXRは、眼のNVを強く抑制する一方、遊離薬物のボーラス注射は網膜毒性を引き起こし得ることが示される。
ジゴキシン0.01〜0.25μgを眼内注射することにより、HIF−1の転写活性および眼のNVが抑制されることが以前に実証されている(Yoshida, T.ら、FASEB J.24巻、1759〜1767頁(2010年))。網膜機能に対するDXRおよびDNRの有害作用が、HIF−1活性の抑制と関連するかを探索するため、網膜機能に対するジゴキシン0.25および0.05μgの眼内注射の影響を測定した。ジゴキシン0.25μgを眼内注射した1週間後、暗順応のa波の平均振幅、暗順応のb波の平均振幅、および明順応のb波の平均振幅は有意に低下した。網膜における6つの測定位置の内の3つにおいて、外核層の厚みも低下しており、光受容体細胞の死亡を示している。これらの結果は、ジゴキシン0.25μgを眼内注射した後、かなりの毒性があることと一致している。ジゴキシン0.05μgの眼内注射はそれほど有毒ではなかったが、暗順応および暗順応のb波の平均振幅の著しい低下を依然として引き起こした。したがって、アントラサイクリンおよびジゴキシンの両方について、遊離薬物の眼への注射は、網膜毒性のリスクをもたらす。
DXRナノ粒子の眼内注射の効果を、レーザー誘発性脈絡膜NVを有するマウスにおいてまず試験した。ブルッフ膜のレーザー誘発性破裂の後、C57BL/6マウスに、10、1.0、または0.1μgのDXR−PSA−PEG3ナノ粒子を眼内注射した。DXR−PSA−PEG3ナノ粒子1μgを投与された動物の眼底写真により、注射1日後に網膜後部を覆うナノ粒子の大きなオレンジ色の塊が、時間をかけてゆっくりと減少し、かつ14日目でも依然として容易に目視可能であることが示された。粒子は5週間もの期間、目視可能のままであった。
ロドプシンプロモーターが光受容体においてVEGF発現を駆動するrho/VEGFトランスジェニックマウスは、生後(P)7日目に開始したVEGFの発現を持続し、治療剤の活性期間を試験するための優れたモデルを提供する(Okamoto, N.ら、Am. J. Pathol.151巻、281〜291頁(1997年))。
DXR−PSA−PEG310μgの眼内注射後の14日目では、PBSを注射した眼と比較すると、暗順応または明順応のb波の振幅に有意な差異は存在しなかった。外核層の厚みも差異は存在しておらず、DXRナノ粒子が光受容体の細胞死を引き起こさなかったことを示している。

DXR−PSA−PEG3ナノ粒子を調製するため、PEG3−PSA80mgをジクロロメタン(DCM)6mLに溶解し、ドキソルビシン塩酸塩(DXR)(NetQem LLC、Durham、NC)20mgをジメチルスルホキシド(DMSO)2mLに溶解した。ポリマーおよび薬物の溶液を混合し、50℃で30分間維持した。L4RTホモジナイザー(Silverson Machines社、East Longmeadow、MA)を使用し、10,000rpmで3分間、得られた混合物を1%ポリビニルアルコール(PVA)溶液(25kDa、Polyscience社、Niles、IL)50mL中でホモジナイズした。粒子懸濁液を室温で2時間撹拌し、ジクロロメタンを除去した。粒子を遠心分離(4℃、20,000×g、20分間)によって集め、凍結乾燥前に、超純水で3回洗浄した。
粒子サイズを、Coulter MultisizerIV(Beckman−Coulter Inc.社、Fullerton、CA)を用いて決定した。マイクロ粒子の各回分について、100,000個を超える粒子をあるサイズにして、平均粒子径を決定した。粒子形態は、冷陰極電界放出型SEM(JEOL JSM−6700F、Peabody、MA)を使用する走査型電子顕微鏡(SEM)により特徴づけた。薬物搭載量は、DCMおよびDMSO中に粒子の乾燥粉末を溶解することにより決定し、UV分光光度計を使用して吸光度を490nmで測定した。
色素が沈着したダッチベルテッドウサギをこれらの研究に使用した(n=10)。動物は、Association for Research in Vision and Ophthalmology Statement of Use of Animals in Ophthalmic and Research、およびJohns Hopkins University Animal Care and Use Committeeの指針に準拠して処置した。眼内注射および水性体液の回収のために、ケタミン(25mg/kg)およびキシラジン(2.5mg/kg)の筋肉内注射により、動物に麻酔をかけた。鎮静すると、瞳孔を2.5%塩酸フェニレフリンおよび10%トロピカミドにより拡張した。眼球表面の麻酔は、0.5%塩酸プロパラカインの局所点眼法を使用して行った。
HPLCによる薬物含有量の定量前に、水性体液試料100μLまたは硝子体試料をメタノール200μLと混合し、4℃で3時間インキュベートした。遠心分離(15,000×g、10分)および0.2μmPTFEフィルターによるろ過後、ろ液150μLをc18逆相カラム(5μm、4.6×250mm、Grace、Deerfield IL)を装備したWaters HPLCシステムに注入した。放出薬物コンジュゲートを、水およびアセトニトリル(60%:40%、v/v)を含有するイソクラティック移動相により1mL/分で溶出し、蛍光検出器(励起波長:500nm、発光波長:551nm)を使用して検出した。推定検出限界は、10ng/mLまたは20nMであった。異なる濃度の一連のDXR水溶液を較正標準として使用した。データはEmpower3クロマトグラフィーデータソフトウェアを使用して分析した(Waters Corporation社、Milford MA)。
DXR−PSA−PEG3マイクロ粒子およびナノ粒子
DXR−SA−PEG3コンジュゲートからなるマイクロ粒子およびナノ粒子を合成し、記載した通りに特徴付けを行った。粒子は、凍結乾燥前、およびビヒクル(PBS)中に再構成した後に、あるサイズにした。マイクロ粒子は、凍結乾燥前に、平均サイズ27.2+1.0μmを示し、ナノ粒子は0.98+0.02μmを示した(表1、図6)。マイクロ粒子の平均薬物搭載量は13%であり、ナノ粒子の平均薬物搭載量はは20%であった(表1)。
ウサギは、右眼にDXR−PSA−PEG3マイクロ粒子またはナノ粒子、および左眼にビヒクル単独(PBS)の硝子体内注射(0.1mL)を受けた。指示した時間に、水性体液を集め(約0.1mL)、HPLCに基づく定量アッセイを使用して、放出された薬物コンジュゲートの存在について分析した。115日目(マイクロ粒子群)または105日目(ナノ粒子群)に、動物を安楽死させ、水性体液および硝子体を集めた。
3種のドキソルビシン(DXR)薬物搭載レベル10%、30%および50%を有する、直径0.5mm、長さ0.5cm、および質量1mgのロッド型DXR−PSA−PEG3コンジュゲートを首尾よく作製した。DXRのインビトロでの放出は、3種のロッドタイプすべてについて、少なくとも25日間、持続放出であることを実証した。
PEG3−PSAポリマーの調製
(ポリエチレングリコール)3−co−ポリ(セバシン酸)(PEG3−PSA)を溶融縮合により合成した。手短に言えば、セバシン酸を無水酢酸中で還流し、セバシン酸プレポリマー(Acyl−SA)を形成させた。ポリエチレングリコール(PEG3)は、塩化メチレン10mLに加えた、CH3O−PEG−NH2(2.0g)、クエン酸(26g)、ジシクロヘキシルカルボジイミド(DCC;83mg)、および4−(ジメチルアミノ)ピリジン(DMAP、4.0mg)を混合することにより調製し、室温で一晩撹拌し、次に沈殿させて、エーテルで洗浄して真空下で乾燥した。次に、acyl−SA(90%w/v)およびPEG3(10%w/v)を180℃で30分間重合した。窒素ガスを15分毎に30秒間フラスコに掃引した。ポリマーを周囲温度まで冷却し、クロロホルムに溶解し、過剰の石油エーテル中に沈殿させた。沈殿物をろ過により集め、一定重量になるまで真空下で乾燥すると、PEG3−PSAポリマーが生成した。
DXR−PSA−PEG3ロッドを調製するため、3種の異なる濃度のDXRを使用して、10%、30%および50%(w/w)の薬物搭載レベルを有するロッドを作製した。10%、30%および50%の薬物搭載ロットについては、PEG3−PSAおよびドキソルビシン塩酸塩(DXR)(NetQem LLC、Durham、NC)を9:1、7:3、および1:1(w/w)の比でCHCl3に加えた。PEG3−PSAおよびDXRを50℃で1時間インキュベートし、その後、CHCl3を真空により除去した。反応生成物をすりつぶして微粉末にし、次に、成形型として使用した直径0.5mmのガラス管中に圧縮した。反応生成物をすりつぶして微粉末にし、次に、成形型として使用した直径0.5mmのガラス管中に圧縮した。ロッドを成形型から押し出し、長さ0.5cmに裁断した。各ロッドは、約1mg(0.9〜1.2mg)の重量であった。
1つのロッド(約1mg)を、リン酸緩衝生理食塩水(PBS、pH7.4)1mlに加え、回転台(140RPM)上で、37℃でインキュベートした。選択した時点において、上清を集め、新しいPBSを加えた。DXR−コンジュゲート濃度を、480nmにおける吸光度により測定した。
3種の異なる薬物搭載レベル10%、30%および50%を有するロッド型DXR−PSA−PEG3コンジュゲートを作製した。DXR−PSA−PEG3コンジュゲートから、直径0.5mm、長さ0.5cm、および質量1mgを有するロッドを形成した。
完全生分解性DXR−PSA−PCPH−PEG3ポリマー薬物コンジュゲートからなるマイクロ粒子を合成し、これは、DXR−PSA−PEG3マイクロ粒子と比べて、よりゆっくりとした薬物放出速度、および一層持続した薬物放出期間を示した。ポリマーへのPCPHの付加により、ポリマー薬物コンジュゲートの疎水性が高まり、おそらくDXR溶解度の低下により、薬物放出期間が延びる結果となった。
材料および方法
1,6−ビス(p−カルボキシフェノキシ)ヘキサン(CPH)の合成
1,6−ビス(p−カルボキシフェノキシ)ヘキサン(CPH)(10.0g)を、窒素下、無水酢酸200mL中で30分間還流し、続いてろ過により未反応二酸、および蒸発により溶媒を除去した。残渣をジメチルホルムアミドおよびエチルエーテルから再結晶し、乾燥エチルエーテルにより洗浄して、真空下で一定重量になるまで乾燥した。
(ポリエチレングリコール)3−co−ポリ(セバシン酸)co−ポリ(CPH)(PEG3−SA−PCPH)を溶融縮合により合成した。手短に言えば、セバシン酸を無水酢酸中で還流し、セバシン酸プレポリマー(Acyl−SA)を形成させた。ポリエチレングリコール(PEG3)を、塩化メチレン10mLに加えた、CH3O−PEG−NH2(2.0g)、クエン酸(26g)、ジシクロヘキシルカルボジイミド(DCC;83mg)、および4−(ジメチルアミノ)ピリジン(DMAP、4.0mg)を混合することにより調製し、室温で一晩撹拌し、次に沈殿させて、エーテルで洗浄して真空下で乾燥した。次に、PEG3(10%、w/v)、acyl−SA(60%w/v)およびpreCHP(30%w/v)を180℃で30分間重合した。窒素ガスを15分毎に30秒間フラスコに掃引した。ポリマーを周囲温度まで冷却し、クロロホルムに溶解し、過剰の石油エーテル中に沈殿させた。沈殿物をろ過により集め、一定重量になるまで真空下で乾燥すると、PEG3−PSA−PCPHプレポリマーが生成した。
DXR−PSA−PCPH−PEG3マイクロ粒子を調製するため、PEG3−PSA−PCPH200mgをジクロロメタン(DCM)3mLに溶解し、DMSO1.5mLに溶解したドキソルビシン塩酸塩(DXR)(NetQem LLC、Durham、NC)40mgと混合した。50℃で30分間インキュベートした後、混合物をPVA100mL中、3,000rpmで1分間ホモジナイズした。2時間撹拌した後、粒子を遠心分離(9,000×g、25分間)により集め、凍結乾燥前に3回洗浄した。
粒子サイズは、Coulter Multisizer IV(Beckman−Coulter Inc.社、Fullerton、CA)を用いて決定した。マイクロ粒子の各回分について、100,000個を超える粒子をあるサイズにして、平均粒子径を決定した。
DXR−PSA−PCPH−PEG3マイクロ粒子(2mg)をリン酸緩衝生理食塩水(PBS、pH7.4)中で懸濁し、回転台(140RPM)上で、37℃でインキュベートした。選択した時間点において、上澄み液を遠心分離(13,500×g、5分間)により集め、粒子を新しいPBS中で再懸濁した。DXR−コンジュゲートは、480nmにおける吸光度により測定した。
DXR−PSA−PCPH−PEG3マイクロ粒子を合成し、これは、13.9%の薬物搭載レベルを有する24.3±8.7μmの平均サイズを示した。
Claims (16)

により記載されるポリマーコンジュゲート。 - Aが、抗緑内障剤、抗血管新生剤、抗増殖剤、抗感染症剤、抗炎症剤、成長因子、抗アレルギー剤からなる群から選択される、請求項1に記載のコンジュゲート。
- Zが、ポリ(アルキレングリコール)、多糖類、ポリ(ビニルアルコール)、ポリピロリドン、ポリオキシエチレンブロックコポリマー、およびそれらのコポリマーからなる群から選択される、請求項1または2に記載のコンジュゲート。
- Zが、出現毎に、ポリエチレングリコールを含む、請求項3に記載のコンジュゲート。
- Xが、ポリエステル、ポリカプロラクトン、ポリ無水物、およびそれらのコポリマーからなる群から選択される、請求項1に記載のコンジュゲート。
- Xが、ポリ無水物を含む、請求項1に記載のコンジュゲート。
- Xが、ポリセバシン酸無水物を含む、請求項6に記載のコンジュゲート。
- Xが、1,6ビス(p−カルボキシフェノキシ)ヘキサン(CPH)、またはCPHとポリセバシン酸無水物との組合せを含む、請求項6に記載のコンジュゲート。
- Yが、クエン酸由来である、請求項1に記載のコンジュゲート。
- 請求項1〜9のいずれか一項に記載のコンジュゲートを含む、マイクロ粒子および/またはナノ粒子集団。
- 請求項1〜9のいずれか一項に記載のコンジュゲート、または請求項10に記載の粒子、および薬学的に許容される賦形剤を含む、製剤。
