疎水性ポリマー
ポリマー−薬物結合体は、1つ以上の疎水性ポリマーセグメントを含み得る。疎水性ポリマーセグメントは、ホモポリマー又はコポリマーであってよい。
ポリマー−薬物結合体は、1つ以上の親水性ポリマーセグメントを含んでもよい。1つ以上の親水性ポリマーセグメントは、任意の親水性の、生体適合性の非毒性ポリマー又はコポリマーであってよい。好ましくは、ポリマー−薬物結合体は、2つ以上の親水性ポリマーセグメントを含む。いくつかの実施形態においては、ポリマー−薬物結合体は、2つから6つの間の、より好ましくは3つから5つの間の親水性ポリマーセグメントを含む。或る特定の実施形態においては、ポリマー−薬物結合体は、3つの親水性ポリマーセグメントを含む。

ポリマー粒子中に存在する、1つ以上の抗緑内障剤、特に眼内圧(IOP)を下げる抗緑内障剤、例えばエタクリン酸(ECA)又はその誘導体に加えて、該製剤は、1つ以上の追加の治療剤、診断剤、及び/又は予防剤を含有してよい。活性剤は、酵素若しくはタンパク質、ポリペプチド又は核酸等の低分子活性剤又は生体分子であってもよい。好適な低分子活性剤として、有機化合物、及び有機金属化合物が挙げられる。いくつかの例では、低分子活性剤は、約2000g/mol未満、より好ましくは約1500g/mol未満、最も好ましくは約1200g/mol未満の分子量を有する。低分子の活性剤は、親水性、疎水性、又は両親媒性の化合物であってもよい。
ポリマー−薬物結合体は、当該技術分野で既知の合成法を使用して製造することができる。ポリマー−薬物結合体の製造のための代表的な方法論を、以下で論ずる。所定のポリマー−薬物結合体の適切な合成経路は、数多くの要因、例えばポリマー−薬物結合体の構造、該結合体を構成するポリマーのアイデンティティ、活性剤のアイデンティティ、並びに化合物の全体としての構造を考慮して決定することができる。それというのも、合成経路は、官能基の適合性、保護基の戦略及び不安定な結合の存在に関連しているからである。
ECA−L−システインは、当該技術分野で既知の技法を使用して製造することができる。例えば、エタクリン酸を水に添加し、エタクリン酸が溶けるまでpHを5.0に調整し、その後にpHを7に調整する。L−システインを、水中に溶解させ、そしてpHを7.0に調整する。それらの2つの溶液を、優しく揺すりながら1時間にわたって一緒に混ぜて、その後に溶液を凍結乾燥させる。
第一の工程において、セバシン酸を、無水酢酸中で還流させることで、アシル化されたポリセバシン酸前駆体(PreSA)を形成する。次いで過剰のPreSAを、ポリエチレングリコールメチルエーテルと混合し、そして無水のホットメルト重合条件下で重合させる。その後に、得られたポリマー(PEG−PSA)を、ECA−L−システインと反応させることで、ポリマー−薬物結合体を形成することができる。
結合体から形成されるか、又はマトリクス中に分散若しくは封入された結合体を有する、1つ以上の抗緑内障剤、特に眼内圧(IOP)を下げるための抗緑内障剤、例えばエタクリン酸(ECA)又はその誘導体の制御送達のためのポリマー性インプラント(例えば、ロッド、ディスク、ウェファー等)、マイクロ粒子及びナノ粒子が提供される。いくつかの実施形態においては、該粒子又はインプラントは、ポリマーマトリクス中に分散又は封入された1つ以上の抗緑内障剤を含有する。好ましい実施形態においては、該粒子又はインプラントは、ポリマーに共有結合された1つ以上の抗緑内障剤を含むポリマー−薬物結合体から形成される。
マイクロ粒子及びナノ粒子は、1種以上のポリマー−薬物結合体から形成され得る。いくつかの場合に、粒子は、単独のポリマー−薬物結合体から形成される(すなわち、該粒子は、同じ活性剤、疎水性ポリマーセグメント、分岐点(存在する場合)、及び1つ以上の親水性ポリマーセグメントを含むポリマー−薬物結合体から形成される)。
マイクロ粒子及びナノ粒子は、当該技術分野で既知のポリマーマイクロ粒子又はナノ粒子の形成のために適した任意の方法を使用して形成することができる。粒子形成のために使用される方法は、ポリマー−薬物結合体又はポリマーマトリクス中に存在するポリマーの特性並びに望まれる粒度及び粒度分布を含む様々な要因に依存することとなる。
この方法では、ポリマー−薬物結合体(又はポリマーマトリクス及びECA又はその誘導体)を、塩化メチレン等の揮発性有機溶媒に溶解する。その後、ポリマー−薬物結合体を含有する有機溶液を、ポリ(ビニルアルコール)等の界面活性剤を含む水溶液中に懸濁する。得られるエマルジョンを大半の有機溶媒が蒸発して固体ナノ粒子を残すまで撹拌する。得られたナノ粒子を水で洗浄し、凍結乾燥器で一晩乾燥させる。種々のサイズ及び形態のナノ粒子をこの方法によって得ることができる。
この方法では、ポリマー−薬物結合体(又はポリマーマトリクス及びECA又はその誘導体)をまず溶融させ、次いで非混和性溶媒(例えばシリコンオイル)中に懸濁させ、撹拌し続けながら、ポリマー−薬物結合体の融点より5℃高い温度にまで加熱する。エマルジョンが安定化されたら、該エマルジョンを、ポリマー−薬物結合体粒子が固化するまで冷却する。得られたナノ粒子を、適切な溶媒、例えば石油エーテルを用いてデカンテーションすることによって洗浄することで、易流動性の粉末が得られる。この技法で製造された粒子の外表面は通常は滑らかで緻密である。ホットメルト粒子形成は、加水分解に対して不安定な、或る特定のポリ無水物のようなポリマー−薬物結合体を含有する粒子を製造するために使用することができる。好ましくは、この方法によりマイクロ粒子を製造するために使用されるポリマー−薬物結合体は、75000ダルトン未満の総分子量を有することとなる。
また、溶媒除去を使用して加水分解に不安定なポリマー−薬物結合体から粒子を作製することができる。この方法では、ポリマー−薬物結合体(又はポリマーマトリクス、及びECA又はその誘導体)を塩化メチレン等の揮発性有機溶媒中に分散させるか、又は溶解させる。その後、この混合物を有機性油(シリコンオイル等)中に撹拌することによって懸濁させ、エマルジョンを形成する。エマルジョンから形成される固体粒子は、その後上清から単離され得る。この技法により製造された球体の外部の形態は、ポリマー−薬物結合体のアイデンティティに大きく依存する。
この方法では、ポリマー−薬物結合体(又はポリマーマトリクス及びECA又はその誘導体)を、塩化メチレン等の有機溶媒に溶解する。圧縮ガスの流れによって制御される微細化ノズルにより溶液を噴出させ、得られたエアロゾルを加熱された気体のサイクロンに漂わせて、微小滴から溶媒を蒸発させて粒子を形成させる。この方法を使用して、0.1ミクロン〜10ミクロンの範囲の粒子を得ることができる。
粒子を転相法(phase inversion method)を使用して、ポリマー−薬物結合体から形成することができる。この方法では、ポリマー−薬物結合体(又はポリマーマトリクス及びECA又はその誘導体)を「良好な」溶媒に溶解し、好ましい条件下で、ポリマー−薬物結合体が自発的にマイクロ粒子又はナノ粒子を生じるように、該溶液を強い非溶媒(non solvent)へと注ぐ。上記方法を使用して、例えば、典型的には狭い粒度分布で、約100ナノメートル〜約10ミクロンを含む広範囲のサイズのナノ粒子をもたらすことができる。
コアセルベーションを使用する粒子形成技法は、当該技術分野で、例えば英国特許第929406号、英国特許第929401号、並びに米国特許第3,266,987号、同第4,794,000号、及び同第4,460,563号において知られている。コアセルベーションは、2つの非混和液相へのポリマー−薬物結合体(又はポリマーマトリクス、及びECA又はその誘導体)溶液の分離を含む。1つの相は、高濃度のポリマー−薬物結合体を含有する濃厚なコアセルベート相であるのに対し、第2相は低濃度のポリマー−薬物結合体を含む。濃厚なコアセルベート相内では、ポリマー−薬物結合体はナノスケール又はマイクロスケールの小滴を形成し、それらは硬化して粒子となる。コアセルベーションは、温度変化、非溶媒の添加若しくはマイクロ塩(micro-salt)の添加(単純コアセルベーション)により、又は別のポリマーの添加により誘導されてポリマー間錯体を形成することができる(複合コアセルベーション)。
制御放出マイクロスフェアの極低温焼結に関する方法は、Gombotz et al.に対する米国特許第5,019,400号に記載される。この方法では、ポリマー−薬物結合体(又はポリマーマトリクス及びECA又はその誘導体)は溶媒中に溶解される。その後、混合物を、ポリマー−薬物結合体小滴を凍結するポリマー−薬物結合体液の氷点を下回る温度で液状の非溶媒の入った容器に微粒化する。ポリマー−薬物結合体に対する小滴及び非溶媒が温められるにつれて、小滴中の溶媒は溶けて、非溶媒中に抽出されてマイクロスフェアを硬化する。
固体又はヒドロゲルであってよいポリマーマトリクス中に分散又は封入されている、1つ以上の抗緑内障剤、特に眼内圧(IOP)を下げる抗緑内障剤、例えばエタクリン酸(ECA)又はその誘導体を含有する粒子を形成することもできる。該マトリクスは、非生分解性又は生分解性のマトリクスから形成することができるが、生分解性のマトリクスが好ましい。該ポリマーは、in vivo安定性のために必要な時間、すなわち送達が望まれる場所への分配のために必要とされる時間及び送達のために望まれる時間に基づいて選択される。
インプラントは、1種以上のポリマー−薬物結合体から形成することができる。好ましい実施形態では、インプラントは眼内インプラントである。好適なインプラントとして、限定されないが、ロッド、ディスク、ウェファー等が挙げられる。
インプラントは、当該技術分野で知られている任意の好適な技法を使用して製造され得る。インプラントの作製に適した技法の例として、溶媒蒸発法、相分離法、界面法、成型法、射出成型法、押出法、共押出方法、カーバープレス法、打抜き法、熱圧縮、及びそれらの組合せが挙げられる。インプラントの製造に適した方法は、インプラント中に存在するポリマー/ポリマーセグメントの特性、インプラント中に存在する1つ以上の抗緑内障剤、特に眼内圧(IOP)を下げる抗緑内障剤、例えばエタクリン酸(ECA)又はその誘導体の特性、及びインプラントの所望の形状及びサイズを含む多くの因子を考慮して選択され得る。インプラントの作製に適した方法が、例えば、米国特許第4,997,652号、及び米国特許出願公開第2010/0124565号に記載されている。
医薬製剤は、1つ以上の薬学的に許容可能な賦形剤と組み合わせて1種以上のポリマー−薬物結合体を含む。代表的な賦形剤として、溶媒、希釈剤、pH調整剤、防腐剤、抗酸化剤、懸濁化剤、湿潤剤、粘度調整剤、等張化剤、安定剤及びそれらの組合せが挙げられる。好適な薬学的に許容可能な賦形剤は、好ましくは一般に安全であると認定され(GRAS)、望ましくない生物学的副作用又は不要な相互作用を生じることなく個体に投与され得る材料から選択される。
ポリマー−薬物結合体から形成された粒子は、好ましくは眼への注射用の溶液又は懸濁液として製剤化される。
A. 治療されるべき疾患及び障害
1つ以上の抗緑内障剤、特に眼内圧(IOP)を下げる抗緑内障剤、例えばエタクリン酸(ECA)又はその誘導体の送達のための制御放出性投与製剤は、眼内圧の上昇を伴う疾患又は障害を治療するために使用することができる。投与により、1つ以上の抗緑内障剤は、治療的利益をもたらすのに十分高いが、細胞毒性を回避するのに十分低い濃度で長期にわたって放出される。
製剤は、硝子体内注射(例えば、前部、中部、又は後部の硝子体注射)、結膜下注射、前房内注射、側頭部角膜輪部を介する前眼房への注射、角膜実質内注射、脈絡膜下腔への注射、角膜間注射、網膜下注射、及び眼内注射によって眼に局所的に投与され得る。好ましい実施形態では、上記医薬組成物は硝子体内注射によって投与される。
100mgのエタクリン酸(ECA)を、3mLの水に添加し、そしてECAが溶けるまでpHを5.0に調整した。溶けた後には、pHを7.0に調整した。39mgのL−システインを、3mlの再蒸留水中に溶解させ、そしてpHを7.0に調整した。それらの2つの溶液を、優しく揺すりながら1時間にわたって混ぜて、その後に溶液を凍結乾燥させた。
(ポリエチレングリコール)3−co−ポリ(セバシン酸)(PEG3−PSA)又は(ポリエチレングリコール)−co−ポリ(セバシン酸)(PEG−PSA)を、溶融縮合によって合成した。セバシン酸(SA)を、無水酢酸中で還流させることで、セバシン酸(SA)プレポリマー(アシル−SA)を形成した。ポリエチレングリコール(PEG3)は、CH3O−PEG−NH2(2.0g)、クエン酸(26g)、ジシクロヘキシルカルボジイミド(DCC、83mg)及び4−(ジメチルアミノ)ピリジン(DMAP、4.0mg)を混合し、それらを10mLの塩化メチレンに添加し、室温で一晩撹拌し、次いで沈殿させ、エーテルで洗浄し、真空下で乾燥させることによって製造された。アシル−SA(90%重量/容量)及びPEG3(10%重量/容量)(又はPEG)を、180℃で30分間にわたって重合させた。重合全体を通じて、強力な窒素スイープを15分毎に30秒間にわたって行った。反応が終わったら、ポリマーを完全に冷却させ、クロロホルム中に溶解させた。その溶液を、過剰の石油エーテル中に滴下することで沈殿させた。沈殿物を濾過によって回収し、真空下で一定の重量になるまで乾燥させた。
120mgのPEG−PSA又はPEG3−SAを、1.2mLのジクロロメタン中に溶解させ、そして30mgのECA−L−システインを、1.2mLのDCM、300μlのメタノール及び300μlのDMSO中に溶解させた。それらの2つの溶液を一緒にして混ぜ、1時間にわたって撹拌し、40mLの1%ポリビニルアルコール(PVA、250000Mw、88%加水分解、Sigma)溶液中に注ぎ入れ、1分間にわたって3500rpmで均質化(シルバーソンホモジナイザー、モデルL4RT、イングランド、バッキンガムシャー州チェサム)し、次いで3時間にわたって撹拌して、ジクロロメタンを蒸発させた。
材料及び方法
以前の研究では、IOPの上昇を伴うヒト患者におけるECAの前房内投与は、ECA治療3時間〜24時間後にIOP降下をもたらし、それは3日間持続し、治療1週間後にIOPは治療前のレベルにまで徐々に戻ることが実証された。正常マウスにおけるPEG−SA−ECA−L−システイン粒子のIOP降下効果を評価するために、ECA(遊離薬物1μg)又はPEG−SA−ECA−L−システイン粒子(薬物活性剤1μg)を、正常C57BL/6マウスへと、角膜輪部にある強膜上静脈を介して投与した。
37℃での加速された無限シンク条件下でのin vitro薬物放出動態によって、ECA−L−システイン結合体が、図1に示されるように14日間にわたって連続的に放出されることが実証された。

- Xが、ポリ無水物を含む、請求項8に記載のポリマー結合体。
- Xが、ポリセバシン酸無水物を含む、請求項9に記載のポリマー結合体。
- Xが、1,6−ビス(p−カルボキシフェノキシ)ヘキサン(CPH)又はポリCPH(PCPH)とポリセバシン酸無水物との組合せ物を含む、請求項9に記載のポリマー結合体。

Rは、それぞれの場合に個別に、アルキル基、シクロアルキル基、ヘテロシクロアルキル基、アルキルアリール基、アルケニル基、アルキニル基、アリール基又はヘテロアリール基であり、それらの基は、任意に、アルキル、シクロプロピル、シクロブチルエーテル、アミン、ハロゲン、ヒドロキシル、エーテル、ニトリル、CF3、エステル、アミド、尿素、カルバメート、チオエーテル、カルボン酸及びアリールから個別に選択される1個から5個の間の置換基で置換されており、かつ、

PEGは、ポリエチレングリコール鎖を表す)によって定義される、請求項1〜13のいずれか一項に記載のポリマー結合体。 - 請求項1〜19のいずれか一項に記載の結合体を含むマイクロ粒子及び/又はナノ粒子の集団。
- 薬学的に許容可能なキャリア、マトリクス、ヒドロゲル又はインプラント中に、請求項1〜19のいずれか一項に記載のポリマー結合体又は請求項20に記載の粒子を含む製剤。
- 眼の疾患又は障害の治療方法であって、それを必要とする患者の眼に、請求項1〜19のいずれか一項に記載の結合体、請求項20に記載の粒子又は眼への投与のための薬学的に許容可能なキャリア、マトリクス、ヒドロゲル又はインプラント中の前記結合体を投与することを含む、方法。
