本明細書では、患者における癌を治療するための方法及び組成物を提供する。癌の治療の幾つかの実施態様では、該方法は、患者に抗体若しくはその抗原結合フラグメントを投与することを含み、この場合、該抗体若しくはその抗原結合フラグメントはPSMAの細胞外ドメインに結合して、PSMAの酵素活性を阻害し、そして患者による葉酸摂取を制限することができる。幾つかの実施態様では、患者による葉酸含有栄養補助食品の摂取を禁止する(制止する)。他の実施態様では、患者による葉酸摂取は1日につき400μg以下に、又は患者における葉酸の血清レベルが10nmol/L以下になるように、又は患者における赤血球(RBC)葉酸レベルが300nmol/L以下になるように制限される。幾つかの実施態様では、患者による葉酸摂取は1日につき300、200、100、50、5μg以下に制限される。本明細書に記載する葉酸レベルは、化学発光イムノアッセイに基づくものであり、他の方法で測定する場合には、幾らか異なる可能性がある。当業者は、他の方法を用いて測定した葉酸レベルを本明細書に記載したレベルと比較することができるであろう。

図3において実証されるように、前立腺癌(PC)細胞の増殖速度は、利用可能な葉酸(folate)に直接比例して上昇し、このことは、PC細胞が葉酸(folate)をより多量に取り込むならば、PC細胞はより迅速に増殖することができることを示す。葉酸レベルが生理的レベルを超えて上昇するならば、増殖速度はほぼ2倍に上昇しうる。PSMAを発現する細胞系(例えば、LNCaP)では、PSMAによって消化されてモノグルタメート化葉酸(monoglutamated folate)になる、「天然」若しくは食物葉酸(例えば、ポリグルタ
メート化葉酸(polyglutamated folate)の形態で葉酸が提供される場合にも、同じ増殖上昇が見出される。前者(モノグルタメート化葉酸)は細胞膜を横切ることができるが、ポリグルタメート化葉酸はできない。
標準培地中で増殖したLNCaP細胞を、指示濃度の指示抗体と共にインキュベートする。細胞をPSMA酵素活性に関して検査した。簡単に説明すると、PSMAを放射性標識した基質(例えば、NAAG)と共にインキュベートした。インキュベーション後に、反応混合物を、標識グルタメート生成物を保持するが、該基質を保持しないカラム上をランさせた。γカウンター中で標識グルタメート量を測定した。標識グルタメートの測定された量は、酵素活性の量に比例する。PSMAは精製された、組換え体又は細胞関連型(cell-associated)であることができる。第1インキュベーション工程に阻害剤を含めるこ
とによって、そして適当な陽性対照と陰性対照を用いることによって、阻害に関して分析して、阻害のレベルを算出することができる。図4によって実証されるように、抗体J415は、5,000ng/ml(5μg/ml)の濃度において、PSMA/FolH1酵素活性を最大に阻害する。最大阻害は約70%である。J591もPSMA/FolH1酵素活性を阻害するが、J415ほどではない。酵素部位から離れた、PSMAの細胞内エピトープと結合する7E11抗体は、PSMA/FolH1阻害活性を有さない。

Claims (11)
- ホルモン療法に適したホルモンと、PSMAの細胞外ドメインに結合することができる抗体若しくはその抗原結合フラグメントを含み、該抗体若しくはその抗原結合フラグメントが細胞傷害剤若しくはアイソトープ剤にコンジュゲートされる、患者の前立腺癌の治療用薬剤であって、この場合、該抗体若しくはその抗原結合フラグメントの初回量は、ホルモン療法を開始した後2週間目以降に投与されることになる、薬剤。
- ホルモン療法に適したホルモンと、PSMAの細胞外ドメインに結合することができる抗体若しくはその抗原結合フラグメントを含み、該抗体若しくはその抗原結合フラグメントが細胞傷害剤若しくはアイソトープ剤にコンジュゲートされる、患者の前立腺癌の治療用薬剤であって、この場合、該抗体若しくはその抗原結合フラグメントの初回量は、ホルモン療法を開始した後3週間目〜4週間目に投与されることになる、薬剤。
- ホルモン療法に適したホルモンと、PSMAの細胞外ドメインに結合することができる抗体若しくはその抗原結合フラグメントを含み、該抗体若しくはその抗原結合フラグメントが細胞傷害剤若しくはアイソトープ剤にコンジュゲートされる、患者の前立腺癌の治療用薬剤であって、この場合、該抗体若しくはその抗原結合フラグメントの初回量は、該患者の血清テストステロンレベルが50ng/mL以下に達した後2週間目〜3週間目に該患者に投与されることになる、薬剤。
- 患者が高い前立腺特異的抗原(PSA)レベルを有し、直径0.9cmより大きい軟組織疾患を有さない、請求項1、2又は3のいずれかに記載の薬剤。
- 患者のPSMA発現細胞上のPSMAの表面レベルを測定することをさらに含み、該患者のPSMA発現細胞上のPSMAの細胞表面レベルが5倍以上に上昇した後1〜5日間目に、該抗体若しくはその抗原結合フラグメントの初回量が投与される、請求項1、2又は3のいずれかに記載の薬剤。
- 患者のPSMA発現細胞上のPSMAの表面レベルを測定することをさらに含み、PSMA発現細胞上のPSMAの細胞表面レベルが9倍以上に上昇した後1〜5日間目に、該抗体若しくはその抗原結合フラグメントの初回量が投与される、請求項1、2又は3のいずれかに記載の薬剤。
- 該抗体若しくはその抗原結合フラグメントが、抗体依存性細胞障害作用を引き出すことができる、請求項1、2又は3のいずれかに記載の薬剤。
- 該抗体若しくはその抗原結合フラグメントが、PSMAの細胞外ドメインに結合して、該PSMAの酵素活性を阻害することができる、請求項1、2又は3のいずれかに記載の薬剤。
- 患者における前立腺癌の治療用であり、ホルモン療法を開始した後2週間目以降に抗体若しくはその抗原結合フラグメントの初回量を投与するためのキットであって、PSMAの細胞外ドメインに結合することできる抗体若しくはその抗原結合フラグメントであって、細胞傷害剤若しくはアイソトープ剤にコンジュゲートする抗体若しくはその抗原結合フラグメントと;ホルモン療法を開始した後2週間目以降に該抗体若しくはその抗原結合フラグメントの初回量を投与するための使用説明書を含むキット。
- 患者における前立腺癌の治療用であり、ホルモン療法を開始した後2週間目以降に抗体若しくはその抗原結合フラグメントの初回量を投与するためのキットであって、PSMAの細胞外ドメインに結合することできる抗体若しくはその抗原結合フラグメントであって、細胞傷害剤若しくはアイソトープ剤にコンジュゲートする抗体若しくはその抗原結合フラグメントと;ホルモン療法を開始した後3週間目〜4週間目に該抗体若しくはその抗原結合フラグメントの初回量を投与するための使用説明書を含むキット。
