Description
本出願を行うに当たって実施された研究は、国立衛生研究所からの支援を利用している。米国政府は、本出願において一定の権利を有する。
本出願は、2008年9月16日に出願された米国特許出願第12/211,633号に基づき優先権を主張するものである。該出願は、引用により本明細書に組み込まれる。
次に、図1を参照すると、本開示の原理によるデバイスを組み込んだ例示的なシステム100が示されている。より具体的には、図1は、嚥下困難または言語障害を治療するためのデバイスを示す。例えば、バンド101は、通常は、嚥下困難治療のために、頸部の周囲に巻き付けることができる。このバンド101は、バイブレータ102を備えてもよく、このバイブレータ102は、感覚刺激を与えるために、喉頭を覆うように位置決めされ得る。いくつかの実施形態においては、バイブレータ102の指定接触部120が、喉頭を覆う患者の喉部の外側と接触状態になるように位置決めされる。さらに、バンド101は、患者の喉頭を覆って接触部120を調整自在に位置決めするための調節機構125を備えることが可能である。患者105により保持されるスプーン、フォーク、またはナイフなどの器具104上の、バイブレータ102に伝達可能な態様で接続されたボタン、スイッチ、または他の同等のアクチュエータなどのアクチュエータ103を作動させると、バイブレータ102は、作動し、喉部および喉頭に振動エネルギーを伝達する。アクチュエータ103は、不使用時には覆うことが可能である。一実施形態においては、アクチュエータ103は、スプーンの柄またはスプーンの柄形状設置部の頂部上で可逆的に摺動される小カバー内のボタンであってもよい。代替としては、アクチュエータ103は、器具から独立したものであることが可能である。したがって、一実施形態においては、アクチュエータ103は、刺激デバイスに物理的に接続されても、またはされなくてもよい、遠隔スイッチである。
本発明のシステムおよびデバイスは、発作、脳出血、外傷性脳損傷、嚥下困難、脳の術後、パーキンソン病、多発性硬化症、先天性異常、ALS、脳性麻痺、中枢神経系損傷、核上性麻痺、および、舌骨、咽頭、喉頭、口咽頭部位、または舌骨喉頭器官系の随意運動制御に影響を及ぼす任意の他の神経疾患、神経障害、神経損傷、神経欠陥、または神経変性疾患を含む(しかしそれらに限定されない)多数の症状および障害の治療のために使用することが可能である。予期される神経欠陥には、求心経路と遠心経路との間の、脊髄の、または脳幹内の相互作用、および脳半球の一次運動皮質内のより高次の相互作用を伴う、反射作用が含まれる。本開示のシステムおよび方法は、運動機能を随意制御する能力を喪失したまたは部分的に喪失した患者に対して適用されるが、さらには脳性麻痺などの随意運動制御の取得の妨げとなっている先天性障害を有して生まれた患者に対しても適用される。さらに、本開示のシステムおよび方法は、吃音症および喉頭ジストニアなどの様々な言語運動制御障害の治療にも適用可能である。
さらに、本開示は、本開示の少なくとも1つの刺激デバイスと、このデバイスのためのコンテナと、このデバイスを使用するための取扱説明書とを備える、キットに関する。一実施形態においては、このキットは、喉頭などの罹患した身体部分と接触状態で配置されるようになされた本発明のデバイスと、このデバイスのためのコンテナと、患者により作動されるスイッチと、このデバイスを使用するための取扱説明書とを備える。望ましくは、取扱説明書は、本明細書において開示される1つまたは複数の方法ステップに対応する少なくとも1つの指示を含む。一実施形態においては、バッテリなどの電源が、刺激デバイス内に収容される。一実施形態においては、使用の際に刺激装置を覆う使い捨てカバーが含まれる。一実施形態においては、刺激デバイスは、バルーンまたはチューブなどのチャンバ内の圧力を上昇させる少なくとも1つのポンプを備える。さらに、デバイスは、デバイスにより加えられる圧力をモニタリングするための、圧力センサ、伸張センサ、体積センサ、電力センサ、または他のセンサを備えてもよい。一実施形態においては、デバイスは、所望の圧力量または運動量、および/または喉頭を覆う領域の皮膚の感覚を上昇させるための皮膚に対する低レベルの電気刺激を設定するためのスイッチをさらに備える。さらに、刺激の周波数および/または振幅を設定するためのスイッチが存在してもよい。
本開示は、以下の実施例を参照することによってより良好に理解されよう。この実施例は、本発明およびその基礎をなす原理をさらに説明することを意図されるが、本発明の範囲を限定するようには意図されない。本開示の真の趣旨および範囲から逸脱することなく、上述の実施形態に対して様々な修正および変更を行うことができる。
この実施例は、重度の慢性咽頭嚥下困難を有する患者において喉部区域に低レベルの感覚刺激を与えることにより、安全な嚥下能力が向上するが、喉部の筋肉を活性化させる高レベルの電気刺激は、これらの患者の嚥下を向上させないことを実証する。
参加者の選択基準には、慢性的な安定した咽頭嚥下困難があること、6ヶ月以上にわたり誤嚥の危険性にさらされていること、ミニメンタルステート検査(Folstein、Folstein、およびMcHugh、1975年)において21点以上のスコアを有すること、厳しい食事制限が課されているおよび/または経腸栄養補給により栄養を受けていること、および研究の時点において医学的に安定していることが含まれた。研究のために含まれるものとして、全ての参加者は、研究のスクリーニング部分の際にビデオ蛍光透視検査に関して液体についての誤嚥の危険性を実演しなければならなかった。
参加者は、インフォームドコンセントを受け、参加前に同意内容を理解したことを証明するために10個の質問に正確に回答しなければならなかった。VitalStim(登録商標)電極(Chattanooga Group社、米国テネシー州ヒクソン、#59000)およびVitalStim(登録商標)Dual Channel Unitを、この研究のために使用した。2セットの電極を使用し、上部セットを、舌骨の上方の顎舌骨筋の領域を覆うオトガイ下領域に水平に配置した(図16)。下部セットを、胸鎖乳突筋の内側の甲状舌骨筋の領域を覆う、正中線の両側の甲状軟骨を覆う皮膚の上に配置した。この電極アレイは、初めの2人の著者(WijtingおよびFreed、2003年)の認定トレーニングの際に効果的であるとして推奨されたものであった。直径19mmの玉軸受を、測定の較正のために頸部の側面にテープ留めした。
各試験のビデオを、2次元運動測定システムであるPeak Motus 8(ViconPeak社、米国コロラド州センテニアル、80112)を使用してオフラインで取り込んだ。このシステムは、約60フィールド/s(約30フレーム/s)の、および608×456ピクセルのフレームサイズのビデオキャプチャボードを備えていた。玉軸受の半径(9.5mm)を、水平方向および垂直方向における全ての測定較正に使用した。研究者が、カーソルを使用して、各ビデオフレーム上において第2椎骨および第4椎骨の最前下隅部上の点を特定し、これらの2点間に直線を引いて、y軸を規定した。第2または第4椎骨のいずれかが視認できない場合には、第1椎骨および第3椎骨の最下前下隅部を、同様の様式で利用した。下方椎骨の前下隅部にてy軸に垂直な線を、x軸とした。舌骨の移動方向に対して前点および上点が正となり、後点および下点が負となる状態で、全ての点に関するx座標およびy座標を、起点である第2椎骨の前下隅部に対してmm単位で決定した。4つの点、すなわち2つの点在する椎骨それぞれの前下点、舌骨の前下点、および声門下気柱中の最後上点を、(喉頭の位置を追跡するために)各フレームに対してマークした。
4人の経験を積んだ認定医療言語聴覚士が、評点システムを決定するために、ランダムに選択された被験者のスクリーニングビデオテープを初めに調査した。Rosenbek侵入-誤嚥スケール(Rosenbek Penetration-Aspiration Scale)(Rosenbek、Robbins、Roecker、Coyle、およびWood、1996年)(Pen-Asp)を用いて複数の嚥下を評価すると、誤嚥の危険性により経腸栄養補給に依っている参加者の多くが、このスケールにおいてはスコア1である正常範囲内のスコアを得ることが認められた。これは、梨状陥凹内に極度の残留貯留が生じ、ボーラスが全く食道に進入しないが、侵入または誤嚥が発生しない場合に生じた。これらの参加者は、試験後には全ての残留物質を口中に吐き戻し、液体を全く嚥下しなかったが、物質が気道に進入しなかったため正常としてスコアを得た。Pen-Aspスケールにおけるスコア1は、天井(正常)であり、改良測定が可能とならないため、このスケールでは、これらの患者における嚥下の悪化を測定することしかできなかった。したがって、天井効果を生じない別のスケールを開発した。
位置測定の信頼性を判定するために、2人の検査員が、各フレーム上においてx軸およびy軸に関する舌骨の位置、およびy軸に関する喉頭の位置を測定し、次いで、10人の被験者のうち4人に関して刺激条件および刺激無し条件の両条件の下でのそれぞれの平均値を計算した。General Linear Model Systat 11(Systat Software, Inc.社、米国カリフォルニア州リッチモンド)からの出力を使用して、被験者因子内および被験者因子間に関する平均平方差を計算した。被験者間の平均平方差を求め、被験者内の平均平方差を減算し、次いでその結果を被験者間の平均平方差と被験者内の平均平方差との和で除算することにより、級内相関係数(ICC)を計算した(Fleiss、1999年)。
(1.参加者)
11人の参加者全員が、慢性の長期の嚥下困難を有していた(表1)。参加者の障害は、6人がCVA後(6ヶ月超後)のもの、2人が良性腫瘍のための開頭術後(2年および4年後)のもの、または2人が外傷性脳損傷後(2年および3年後)のものであった。1人の患者のみが、慢性進行性神経疾患、20年超のパーキンソン病、および2年を上回る期間の嚥下困難を有していた。
オン刺激条件およびオフ刺激条件におけるy軸に関する舌骨の運動に関するICCは、それぞれ0.99および0.94であり、x軸に関する舌骨の運動に関するICCは、0.94および0.87であった。刺激「オン」位置および刺激「オフ」位置におけるy軸に関する喉頭のICCは、それぞれ0.58および0.66であり、それらの測定における信頼性がはるかに低いことを示唆した。喉頭の運動は、極めて小さく、刺激「オン」条件における0.4mmから刺激「オフ」条件における0.18mmまでの平均位置の範囲であり、測定の変動性により、この測定における相違が引き起こされた。
第1の仮説を検討すると、「オフ」刺激条件と「オン」刺激条件の間の平均位置を比較する片側方向性t検定によって、y軸に関する舌骨位置の大幅な下降が実証された(f=-2.523、o7=9、p=0.016)(図16を参照)。図17においては、刺激装置が「オン」に切り替えられ、次いで「オフ」に切り替えられ、次いで再度「オン」に切り替えられた場合の、各患者における舌骨運動の個別のトレーシングが示され、刺激装置が「オフ」に切り替えられた場合には舌骨が挙上することを示している。安静時刺激により舌骨が後方に移動するという第2の仮説を検討すると、被験者内の「オフ」刺激条件および「オン」刺激条件における平均位置を比較する方向性t検定は、有意ではなかった(P=-0.102、αf/=9、p=0.460)。同様に、方向性t検定により、刺激の際にy軸に関して喉頭位置の下降がないことが判明した(£=0.696、d/=9、p=0.748)。
第1のセットの21件の反復評点の後に、ICCは、Pen-Aspスケールにおいては0.965であり、NIH-SSSにおいては0.764であった。NIH-SSSの信頼性に関する不安により、我々は、不一致が生じた各項目について、より詳細な判定ルールを実施した。新しい判定ルールを使用した第2のセットの18件の信頼性測定により、NIH-SSSに関して0.925のICCが得られ、判定ルールが作成および実施された後にこのスケールを使用した場合の十分な信頼性が実証された。
時間的な制約により、10人の参加者の中の8人のみが、この感覚条件を完了した。感覚レベルの刺激により嚥下が改善されるという第4の仮説について検討するために、1サンプル方向性t検定を計算して、参加者内の刺激無しの嚥下と刺激有りの嚥下との間の評点の平均変化を比較した。その結果は、ボンフェローニ補正されたp値0.05/2=0.025を使用すると、Pen-Aspスケールにおいては有意ではなかった(£=0.336、cf/=7、p=0.373)が、NIH-SSSにおいては有意であった(.=.2.355、df=7、p=.O25)。これは、図18に示される。8人の参加者の中の6人が、嚥下の際の感覚刺激によりNIH-SSSにおいて低下を示し、8人の参加者の中の5人が、Pen-Aspスケールにおいて低下を示した。
刺激の際に誤嚥の危険性および嚥下の安全性が悪化するという第5の仮説を検討するために、1サンプル方向性t検定を計算して、参加者内の刺激無しの嚥下と刺激有りの嚥下との間における評点の平均変化を調査した。その結果は、ボンフェローニ補正されたp値0.05/2=0.025では、Pen-Aspスケール(/=0.363、d/=9、p=0.637)においても、NIH-SSS(/=-0.881、d/=9、p=0.201)においても有意なものではなかった。NIH-SSSスケールにおいては、10人の参加者の中の5人において運動レベルの刺激により危険性が上昇し(図19)、Pen-Aspにおいては、同数の参加者において運動レベルの刺激により危険性が上昇または低下した(図20)。
Pen-Aspスケールにおける参加者の初期重度と感覚刺激による嚥下の変化との間のピアソン相関係数は、有意なものではなかった(/=0.142、p=0.737)。同様に、患者の初期重度およびNIH-SSSにおける感覚刺激による嚥下の変化(/=0.701、p=0.053)も、統計的有意性を得る目的でボンフェローニ補正されたa値0.025を使用した場合に、有意なものではなかった。統計的有意性を得る目的でボンフェローニ補正されたa値0.025を使用した場合に、Pen-Aspスケールにおける参加者の初期重度と運動刺激による嚥下の変化との両者間のピアソン相関係数は、有意なものではなく(/=-0.501、p=0.140)、NIH-SSSにおける参加者の初期重度と運動刺激による嚥下の変化との間の相関もまた、有意なものではなかった(/=-0.190、p=0.599)。
統計的有意性を得る目的でボンフェローニ補正されたo値0.025を用いて、ピアソン相関係数を、安静時刺激の際に舌骨が頸部において引き下げられる度合いと、Pen-AspおよびNIH-SSSにおける嚥下の変化との間において計算した。NIH-SSSにおける改善の度合いと、運動レベルの安静時刺激の際に舌骨が押し下げられる度合いとの間においては、有意な関係は見い出されなかった(r=-0.388、n=9、P=0.302)。運動刺激の際のPen-Aspスケールにおける改善は、運動レベルの安静時刺激の際に舌骨が押し下げられる度合いに有意に逆比例した(r=-0.828、n=9、p=0.006)。この関係により、安静時に最大の舌骨の押下げを被る人は、嚥下中に運動レベルの刺激を与える際にはPen-Aspスケールにおける低下が最大となることが実証された。
本研究の1つの目的は、頸部における舌骨および喉頭の位置に対する、表面電気刺激の生理学的な影響を判定することであった。我々は、オトガイ下電極対と喉頭電極対の両方が、参加者の最大許容レベルにて刺激を与えているときには、おそらく胸骨舌骨筋の刺激により、舌骨は下方に引かれると予測していた。これらのデータにより、この仮説は支持され、参加者の中の2人を除く全員において、安静時刺激の際に5mmから10mmもの舌骨の押下げがあった(図6Aおよび図6B)。また、我々は、舌骨が後方に引かれ得ることも予測していたが、限定的な前後方向の運動が舌骨において生じた。3人の参加者において、ある場合には5mmもの舌骨の前方運動があったが、他の参加者においては、後方への最小限の運動があった。2人の参加者においては、喉頭において最小限の上昇運動(2mm〜3mm)が生じたが、他の参加者は誰も、認め得るような喉頭の運動を経験せず(図6D)、この2mm〜3mmの変化は、おそらく測定変動によるものであった。これらの知見をまとめると、表面電気刺激の唯一の認め得る運動的影響は、頸部において舌骨を下降させて、嚥下に必要な方向とは逆方向への運動を生じさせることであった。

Claims (20)
- 少なくとも1つの刺激を患者の頸部の外表面に印加するように構成された刺激装置と、
前記患者による随意的なアクティブ化により前記刺激装置が前記少なくとも1つの刺激を印加するように構成された手動モードと、前記刺激装置がある間隔で前記少なくとも1つの刺激を印加するように構成された自動モードとを選択的に作動させるように構成されたスイッチと、
前記手動モードにおける少なくとも1つの刺激の印加回数、または前記自動モードにおける少なくとも1つの刺激の印加回数のいずれかを判定するように構成された第1カウンタと
を備えるデバイス。 - 前記デバイスは、前記手動モードにおける前記少なくとも1つの刺激の印加回数、または前記自動モードにおける少なくとも1つの刺激の印加回数のいずれかを判定するように構成された第2カウンタをさらに備え、前記第2カウンタの構成は、第1カウンタの構成と相反する、請求項1に記載のデバイス。
- 前記少なくとも1つの刺激は、振動刺激、圧力刺激、聴覚刺激、温度刺激、視覚刺激、嗅覚刺激、味覚刺激、またはそれらの組み合わせを有する、請求項1または2に記載のデバイス。
- 前記少なくとも1つの刺激は、振動刺激を有する、請求項1〜3のいずれか一項に記載のデバイス。

- 前記間隔は、約3分から約30分の間である、請求項1〜10のいずれか一項に記載のデバイス。
- 前記少なくとも1つの刺激は、患者の嚥下反射を誘発するように構成される、請求項1〜11のいずれか一項に記載のデバイス。
- 前記少なくとも1つの刺激は、患者の発話を生じさせるように構成される、請求項1〜12のいずれか一項に記載のデバイス。
- 生理学センサをさらに備える、請求項1〜13のいずれか一項に記載のデバイス。
- 前記生理学センサは、運動センサ、温度センサ、皮膚色センサ、ヘマトクリット値センサ、酸化センサ、および血圧センサからなる群より選択される、請求項14に記載のデバイス。
