図1に示すように、この実施形態に係る処置システム10は、生体組織を処置するのに用いられる処置具12と、電源ユニット(コントローラ)14と、フットスイッチやハンドスイッチ等のスイッチ部16とを有する。処置具12と電源ユニット14とは、ケーブル18を介して接続されている。このケーブル18は、スイッチ部16から電源ユニット14に対して信号を送受信したり、電源ユニット14によって制御された電力を処置具12の後述する超音波振動子82に供給したりする。なお、本実施形態では、ケーブル18は処置具12の基端から延出され、ケーブル18の遠位端の図示しないコネクタは電源ユニット14に対して着脱可能である。
この実施形態に係るアクチュエータ28として、図示しないが回転モータの回転軸にボールネジを組み合わせて直動ロッド62aを直動させるようにしても良いことはもちろんである。つまり、アクチュエータ28は、超音波振動子ユニット24及びプローブ26を把持部22に対して一体的にプローブ26の長手軸Lに沿って移動させることができればよい。その際、アクチュエータ28は、超音波振動子ユニット24及びプローブ26を一体的に超音波振動の振幅よりも大きなストロークで移動させる。
設定部134では、超音波振動子82の制御パラメータ、及び、アクチュエータ28の制御パラメータを適宜に設定し、記憶させることができる。以下にユーザが設定部134で入力して記憶させる設定情報の一例について説明する。
図5に示すように、例えば骨などの生体組織の表面Fを削る際、内視鏡の一種である関節鏡160で削りたい部分の生体組織を観察しながら行う。ここでは、説明の簡略化のため、関節鏡160を動かさずに関節鏡160でプローブ26の処置部94を観察し続けることができる場合について説明する。すなわち、関節鏡160に対してプローブ26の処置部94が移動により近接及び離隔する場合について説明する。
ここでは、その代わりに、スイッチ部16が把持部22の外装ケース42に対して長手軸(中心軸)Lの軸周りに回転可能に配置されている。スイッチ部16は、外装ケース42に対して長手軸Lの軸周りの周方向Cに回転可能なリング体216を有する。このため、第1から第3スイッチ142,144,146はリング体216に配置されている。
Claims (15)
- 生体組織を処置するのに用いられる処置具であって、
ユーザに把持される把持部と、
電力の供給にしたがって超音波振動を発生する超音波振動子ユニットと、
長手軸を規定し、前記超音波振動子ユニットで発生した超音波振動を伝達可能で、その先端部に伝達された超音波振動の作用で生体組織を処置する処置部を有するプローブと、
前記プローブの外周面を覆う内シースと、
前記内シースの内周面と前記プローブの外周面との間に設けられ、前記内シースの内周面と前記プローブの外周面とに密着することで液体の浸入を防止する第1封止部材と、
前記内シースの外周面を覆う外シースと、
前記内シースの外周面と前記外シースの内周面との間に設けられ、前記内シースの外周面と前記外シースの内周面とに密着することで液体の浸入を防止する第2封止部材と、
前記超音波振動子ユニットと前記プローブと前記内シースとを前記外シースに対して一体的に前記長手軸に沿って移動させるアクチュエータと
を具備する処置具。 - 前記アクチュエータは、前記プローブの前記処置部に伝達される超音波振動の振幅よりも大きなストロークで前記超音波振動子ユニットと前記プローブと前記内シースとを前記外シースに対して一体的に前記長手軸に沿って移動させる、請求項1に記載の処置具。
- 前記内シースの先端は、前記プローブにおける超音波振動の最も先端側の節位置より先端側に位置し、
前記第1封止部材は、前記プローブにおける超音波振動の最も先端側の前記節位置に位置する、請求項1に記載の処置具。 - 前記第2封止部材は、前記内シースの外周面に密着した状態を維持しつつ前記内シースに対して位置ズレ可能で、前記外シースに対して位置ズレが防止されている、請求項1に記載の処置具。
- 前記第2封止部材は、前記把持部に配設され、前記内シースの外周面に密着した状態を維持しつつ前記内シースに対して位置ズレ可能で、前記把持部に対して位置ズレが防止されている、請求項1に記載の処置具。
- 前記第1封止部材は、前記超音波振動子ユニットで発生した超音波振動を前記プローブに伝達したときに、振動の節となる位置に配置されている、請求項1に記載の処置具。
- 前記第2封止部材は、前記第1封止部材よりも前記長手軸に沿って基端側の位置にある、請求項1に記載の処置具。
- 前記把持部に対して、前記超音波振動子ユニット、前記プローブ、前記内シース、及び前記外シースを前記プローブの長手軸の軸周りに回転させるノブを具備する、請求項1に記載の処置具。
- 前記把持部と前記超音波振動子ユニットとの間に配設され、前記超音波振動子ユニットを超音波振動の節位置で支持し、前記アクチュエータによって移動させることにより、前記超音波振動子ユニット及び前記プローブを前記プローブの長手軸に沿って、前記プローブの処置部に伝達される超音波振動の振幅よりも大きなストロークで移動させるカバーをさらに具備する、請求項1に記載の処置具。
- 前記外シースは、その先端に、前記アクチュエータによって前記超音波振動子ユニット及び前記プローブを前記長手軸に沿って移動させたときに、その先端に対する突出量を変更可能な開口を有する、請求項9に記載の処置具。
- 前記外シースは、その先端部の側面に、前記プローブの先端部の前記処置部を露出させる開口を有する請求項9に記載の処置具。
- 請求項1に記載の処置具と、
前記アクチュエータを制御する制御部と、
前記制御部に対して信号を出力するスイッチ部と
を具備する、処置システム。 - 前記スイッチ部は、
前記超音波振動子ユニットで超音波振動を発振させたときには前記アクチュエータを動作させない第1スイッチと、
前記超音波振動子ユニットで超音波振動を発振させながら前記アクチュエータを動作させる第2スイッチと
を有する、請求項12に記載の処置システム。 - 前記スイッチ部は、前記超音波振動子ユニットで超音波振動を発振させながら前記アクチュエータを動作させることが可能で、前記第2スイッチを作動させたときに比べて前記アクチュエータの動作量を変更する第3スイッチをさらに有する、請求項13に記載の処置システム。
- 前記スイッチ部は、前記把持部の外周に、前記長手軸の軸周りに前記把持部に対して回転可能に配置されている、請求項12に記載の処置システム。
